Abstract
Die Erfindung betrifft eine Gefäßprothese oder einen Gewebeflicken aus einem biokompatiblen Polymer mit einer mikroporösen, feinfibrillären Struktur sowie ein Verfahren zur Verbesserung des E-Moduls der genannten Gefäßprothesen oder Gewebeflicken. Erfindugsgemäß werden die Gefäßprothesen oder Gewebeflicken einem abschließenden Recken (Dehnen) mit einem Aufdehnungsgrad zwischen 30% und 250% und anschließender Rückstellung unterzogen.
Description
Gefäßprothese oder Gewebeflicken aus biokompatiblen Polymeren
Die Erfindung betrifft eine Gefäßprothese oder einen Gewebeflicken (Patch) mit einer mikroporösen, feinfibrillären Struktur, aus einem biokompatiblem Polyurethan, insbesondere aus Polyurethan, Polyamid, Polysulfon, Polyester, isotaktisches Polypropylen, Polynitril und/oder Polyvinylchlorid, deren Mischungen und/oder deren Copolymere sowie ein Verfahren zur Verbesserung des E-Moduls dieser Werkstücke.
Kleinlumige Gefäßprothesen mit Durchmessern unterhalb von 4 mm sind bisher auf dem Markt nicht verfügbar. Die Entwicklung solcher Gefäße stellt nach wie vor eine große Herausforderung dar. Alle bisherigen Versucher scheiterten vor allem daran, dass die hergestellten Gefäße, hervorgerufen durch Thrombenablagerungen und Hyperplasien, frühzeitig zu verschließen drohen.
Auch die bisher nach dem Stand der Technik bekannten Gewebeflicken (Patches), die bei Gewebedefekten eingesetzt werden, erwiesen sich nicht in dem gewünschten Maß als mit dem natürlichen Gewebe kompatibel.
Es ist daher Aufgabe der vorliegenden Erfindung, Gefäßprothesen, insbesondere kleinlumige Gefäßprothesen oder Gewebeflicken zu schaffen, die eine solch differenzierte natürliche Struktur besitzen, dass eine weitgehend physiologische, axiale und tangentiale Elastizität (Compliance) erreicht wird. Nach einer weiteren Aufgabenstellung sollen die Gefäßprothesen oder Patches Offenheitsraten aufweisen, die das Einwachsen und die Bildung einer dünnen und stabilen Neointima zulässt.
Die physiologische Compliance ist wesentlich für die Offenheitsrate des Gefäßes, was sich aus Salacinski et al.: „The mechanical behavior of vascular grafts", Journal of Biomaterials Applications ,Vol. 15-January 2001 , Seite 241 ff., sowie aus „Cardio-
vascular biomaterials", Garth W. Hastings, 1991 , Chapter 1 , Seite 1 bis 16, „Mecha- nical Properties of Arteries and Arterial Grafts", T.V. How, ergibt.
Überraschenderweise lässt sich die Elastizität bzw. das E-Modul von Gefäßprothesen oder Gewebeflicken aus biokompatiblen Polymeren verbessern, wenn diese einem abschließenden Recken (Dehnen) mit einem Aufdehnungsgrad von 30 % bis 250 %, vorzugsweise 60 % bis 125 %, und anschließender Rückstellung (Entlastung) unterzogen worden sind. Dieses Ergebnis ist überraschend, da es bei einer Dehnung von Werkstoffproben in Folienform, die weit über den linear elastischen Bereich hinausgeht, zu einer Verhärtung des Polymers, d.h. zu einer Erhöhung des Elastizitätsmoduls kommt. Ein solcher Effekt wird auch in „Porous polyurethane vascular protheses with variable compliances" von Shu Qin Liu and Makoto Kodama in Journal of Biomedical Materials Research, Vol. 26, 1489-1502 (1992) beschrieben. Besitzt das Polymer jedoch eine mikroporöse, feinfibrilläre Struktur, verringert sich der E-Moduls des Gefäßes oder Patches. Die mikroporöse, feinfibrilläre Polymer- Struktur wird erreicht, wenn das in der DE 28 06 030 C2 beschriebene Verfahren verwendet wird, wobei die Polymere aus einer Lösung mittels Düsen zu Mikrofasem versponnen werden. Derart hergestellte Fibrillen werden über mehrere hundert Schichten unter definierten Winkeln auf Formen gewickelt und an ihren Kreuzungspunkten schichtweise fest miteinander verschmolzen, so dass Gefäße oder Patches hergestellt werden, die eine mechanisch und biologisch stabile, mikroporöse Struktur besitzen. Die dem Blut zugewandte Innenseite des Gefäßes oder Patches soll möglichst glatt sein, wohingegen die Außenseite eine rauhe Gestalt aufweisen kann, da dies in vielen Fällen einer erwünschten Oberflächenbeschaffenheit der künstlichen Blutgefäßprothese entspricht, die ein sicheres, rasches Anwachsen von Bindegewebe an die Blutgefäßprothese nach ihrer Implantation gewährleistet und so die Lage dieser Prothese fixiert. Das einem abschließenden Recken unterzogene Gefäß oder Patch besitzt gegenüber entsprechenden Werkstücken, die diese Behandlung nicht erfahren haben, eine bisher nicht erreichbare ultraweiche und flexible Materialstruktur, die den betreffenden Gefäßen oder Patches eine naturähnliche Längs- und Querelastizität verleiht. Die für Gefäßprothesen außerordentlich hohe Compliance
leitet dabei die Pulswellen des Blutes physiologisch im Sinne einer Windkesselfunktion fort, was in Caninen Carotis- bzw. Femoralis-Interponaten an triphasischer Flussgeschwindigkeitsamplitude erkennbar ist. In solchen Prothesen wird somit eine laminare Strömung aufrechterhalten, so dass der bei bekannten Gefäßprothesen gefürchtete Kalibersprung vermieden wird, durch den an den Anastomosen blutschädigende Turbulenzen mit einhergehender Ablösung und Totwassergebieten und die Bildung von Hyperplasien entstehen. Die durch das abschließende Recken geschaffene flexible Materialstruktur verleiht den Gefäßen sowie den Patches eine besonders gute Formbeständigkeit für optimale Flusseigenschaften mit guter Knickstabilität bei anliegendem Innendruck.
Die verwendeten Polymere sollen zunächst in Lösungsmitteln, vorzugsweise organischen Lösungsmitteln löslich sein, so dass sie über spezielle geeignete Düsen zu feinen Fäden oder Fasern versponnen werden können.
Bevorzugte Werkstoffgruppen sind Polyurethane, Polyamide, Polysulfone, Polyester, isotaktisches Polypropylen, Polynitril und/oder Polyvinylchlorid. Es können auch Copolymere dieser Gruppen verwendet werden, ferner Varianten dieser Stoffgruppen, die als segmentierte Polymere ausgeführt sind, wie z.B. mit Hart- und Weichsegmenten ausgestattete Polyurethane.
Je nach Anwendungsfall ist es vorteilhaft, die Gefäße oder das Patch in Längs- und Querrichtung gleichermaßen zu dehnen oder ihm eine bevorzugte Richtung zu geben, z.B. das Gefäß oder das Patch in Längsrichtung weicher als in Querrichtung auszulegen. Durch eine unterschiedlich starke Aufdehnung eines Gefäßes oder eines Patches in beiden Richtungen kann damit das Verhalten dieses Werkstückes dem natürlichen Vorbild weitestgehend angepasst werden.
Je nach Werkstofftyp des verwendeten Polymers und je nach Abzugsgeschwindigkeit sowie sonstiger Reckparameter kommt es zu einer fast vollständigen Rückstellung des Gefäßes oder des Patches, oder zu einer geringfügigen bleibenden Dehnung
von 3 bis 5 %, die nach einer Weiterbildung der Erfindung derart berücksichtigt wird, dass die Porengröße der Gefäßprothese oder des Gewebeflickens vor dem Recken um ein zu erwartendes, nicht rückstellbares Dehnungsmaß kleiner ausgebildet wird. Insbesondere bei Gefäßen, bei denen es auf eine bestimmte Porengröße ankommt, die günstig für das Einwachsen von Zellen ist, wird bei der Herstellung des Patches oder der Gefäßprothese die Oberflächenporengröße bewusst kleiner gestaltet, so dass sie nach dem Reckvorgang auf die gewünschte Weite eingestellt wird.
Erfindungsgemäß können unterschiedliche Reck- bzw. Dehnverfahren verwendet werden. Neben einem ein- oder biaxialen Dehnen des Patches oder des Gefäßes, das im einfachsten Fall durch eine konventionelle Zugprüfmaschine herbeigeführt werden kann, können schlauchförmige Gefäßprothesen durch Aufbringen eines Innendruckes, der durch ein gasförmiges Medium wie Luft oder Stickstoff oder ein flüssiges Medium aufgebracht wird, gereckt werden.
Will man Leckagen, die durch die Porosität des Gefäßes naturbedingt vorhanden sind, vermeiden, so kann die Druckaufbringung auch durch einen dehnbaren, vorzugsweise elastischen Hilfskörper erreicht werden, der vor dem Recken in die Gefäßprothese eingeführt wird und der dann beaufschlagt wird. Das erfindungsgemäße Verfahren kann auch derart ausgestaltet sein, dass die Gefäßprothese oder der Gewebeflicken vor dem Recken mit einer in Wasser löslichen physiologischen Substanz, vorzugsweise Polyvinylalkohol (PVA), Polyvinylpyrolidon oder Gelatine (Kollagen) mit einer entsprechenden Viskosität getränkt wird. Diese Substanz zieht ganz oder teilweise in den Vlies der Gefäßprothese oder des Gewebeflickens ein, vorzugsweise auf der Außenseite des Vlies-Werkstoffes. Im Rahmen der vorliegenden Erfindung ist es jedoch auch möglich, das Gefäß innen oder außen oder auch in einer Zwischenschicht durch eine zusätzlich eingebaute flüssigkeitsundurchlässige Schicht abzudichten.
Insbesondere zum Recken von Gewebeflicken kann nach einer Weiterbildung der Erfindung ein mechanisch größenverstellbarer Hilfskörper verwendet werden, auf
den der Gewebeflicken aufgespannt wird oder der in die schlauchförmige Prothese eingeführt wird. Betreffender Hilfskörper kann ein elastisches Bauteil sein, das vor dem Recken zusammengefaltet in das Gefäß geschoben und das dann aufgespannt und anschließend auf die ursprüngliche Größe im Sinne einer Feder oder eines Expanders zurückgestellt wird. Alternativ können auch in einen schlauchförmigen Gefäßprothesenkörper mehrere dünne Stäbe eingeführt werden, die dann radial zur Gefäßaufweitung nach außen geführt werden. Schließlich lassen sich schlauchförmige Gefäßprothesen auch mittels eines einführbaren Aufweitungsdornes aufweiten.
An Polyurethan-Probekörpern durchgeführte Versuche haben ergeben, dass das E-Modul nach dem Recken um bis zu 50 % geringer war.
Verwendbare Polyurethane erhält man, indem man mindestens ein aliphatisches und/oder mindestens ein cycloaliphatisches Diisocyanat mit einem Makrodiol des Typs Polycarbonat, Polyester, Polyether, Polysiloxan oder Polysulfon mit einem durchschnittlichen Molekulargewicht von 500 bis 6000 umsetzt und das so gewonnene Prepolymer weiter umsetzt mit einem Kettenverlängerungsmittel, das ein niedermolekulares Diol oder ein Gemisch aus niedermolekularen Diolen oder eine Mischung aus dem niedermolekularen Diol mit einem Makrodiol des Typs Polyester, Polyether, Polysiloxan oder Polysulfon oder Polycarbonat mit einem durchschnittlichen Molekulargewicht von 500 bis 6000 darstellt, wobei das Verhältnis von NCO- Endgruppen des Prepolymers zu OH-Gruppen des Kettenverlängerungsmittels 1 ,01 : 1 bis 1 ,05 : 1 beträgt und dass man das erhaltene Polymer, gegebenenfalls nach Behandlung mit einem Reagenz zur Deaktivierung der noch vorhandenen NCO- Gruppen, einer Molekulargewichtsfraktionierung unterwirft, bei der der niedermolekulare Polyurethananteil mit einem Massenanteil von 10 bis 55 Gew.% also nicht verwendbarer Anteil abgetrennt und gegebenenfalls verworfen wird und der zurückbleibende hochmolekulare Anteil als biokompatibles Polyurethan mit verbesserten Eigenschaften gewonnen wird.
Als aliphatische Diisocyanate sind geeignet geradkettige oder verzweigte C2- bis C10- Alkyldiisocyanate, die durch Methyl, Ethyl, n-Propyl, i-Propyl oder Butyl substituiert sein können. Bevorzugt seien C4- bis C8-Alkylisocyanate, besonders bevorzugt C5- und C6-Alkylisocyanate genannt, die jeweils durch Methyl, Ethyl, n-Propyl, i-Propyl oder Butyl substituiert sein können. Ganz besonders bevorzugt sind Hexandiisocya- nate, die mit Methylresten substituiert sein können. Im einzelnen seien genannt 1 ,6- Diisocyanato-2,2,4,4-tetramethylhexan, 1 ,6-Diisocyanato-2,4,4-trimethylhexan und 1 ,6-Diisocyanato-2,2,4-trimethyl-hexan.
Als cycloaliphatische Diisocyanate sind solche mit Cyclobutyl-, Cyclopentyl-, Cyclo- hexyl-, Cycloheptyl-, Cyclooctyl-, Cyclononyl- oder Cyclodecylgruppen geeignet, wobei die cycloaliphatischen Reste über einen oder mehrere Methylenreste verknüpft sein können. Bevorzugt sind Cyclopentyl-, Cyclohexyl- sowie Dicyclohexylmethan- diisocyanate, besonders bevorzugt sind Cyclohexyl- und Dicyclohexylmethandiiso- cyanate. Im einzelnen seien genannt 4,4'-Dicyc!ohexylmethandiisocyanat, 1 ,4-Cyclo- hexyldiisocyanat, 1 ,3-Bis-(isocyanatomethyl)-cyclohexan, 1 ,4-Bis-(isocyanatomethyl)- cyclohexan und Isophorondiisocyanat. Ganz besonders bevorzugt werden in das erfindungsgemäße Verfahren 4,4'-Dicyclohexylmethandiisocyanat und 1 ,4-Cyclo- hexyldiisocyanat eingesetzt. Selbstverständlich sind auch Isomerengemische der genannten Diisocyanate geeignet.
Als Makrodiole sind geeignet Polyester, Polyether, Polysiloxane oder Polysulfone, die zwei OH-Endgruppen aufweisen mit einem mittleren Molekulargewicht von Mw = 500 bis 6000 (Mw = Gewichtsmittel), bevorzugt Polyester, Polyether, Polysiloxane oder Polysulfone mit zwei OH-Endgruppen mit einem mittleren Molekulargewicht von Mw = 500 bis 4000 (Mw = Gewichtsmittel), besonders bevorzugt Polyester, Polyether, Polysiloxane oder Polysulfone mit zwei OH-Endgruppen mit einem mittleren Molekulargewicht von Mw = 1000 bis 3000 (Mw = Gewichtsmittel), ganz besonders bevorzugt Polyester, Polyether, Polysiloxane oder Polysulfone mit zwei OH-Endgruppen mit einem mittleren Molekulargewicht von Mw = 1000 bis 2400 (Mw = Gewichtsmittel).
Als Polyester seien genannt solche mit d- bis Cι0-Alkylenbausteinen, bevorzugt solche mit C2- bis C6-Alkylenbausteinen, besonders bevorzugt C2- bis C4-Alkylen- bausteinen, wobei diese jeweils durch Methylgruppen substituiert sein können.
Als Polyether seien genannt solche mit Cr bis Cι0-Alkylenbausteinen, bevorzugt solche mit C2- bis C6-Alkylenbausteinen, besonders bevorzugt C2- bis C4-Alkylen- bausteinen, wobei diese jeweils durch Methylgruppen substituiert sein können.
Als Polysiloxane seien genannt solche mit C-i- bis C-io-Alkylenbausteinen, bevorzugt solche mit C2- bis C6-Alkylenbausteinen, besonders bevorzugt C2- bis C -Alkylen- bausteinen, wobei diese jeweils durch Methylgruppen substituiert sein können.
Als Polysulfone seien genannt solche mit C bis C-io-Alkylenbausteinen, bevorzugt solche mit C2- bis Cβ-Alkylenbausteinen, besonders bevorzugt C2- bis C - Alkylen- bausteinen, wobei diese jeweils durch Methylgruppen substituiert sein können.
Als niedermolekulare Diole sind geeignet C2- bis Cio-Alkyldiole, die gegebenenfalls durch niedere Alkylreste wie C-ι- bis C3-Reste substituiert sein können. Im einzelnen seien genannt Ethylenglykol, 1 ,3-Propandiol, 1 ,4-Butandiol, 1 ,4-Bis-(hydroxymethyl)- cyclohexan, 1 ,6-Hexandiol, 2,2,4-Trimethyl-1 ,6-hexandiol und 2,4,4-Trimethyl-1 ,6- hexandiol, bevorzugt 1 ,4-Butandiol, 1 ,6-Hexandiol, 2,2,4-Trimethyl-1 ,6-hexandiol und 2,4,4-Trimethyl-1 ,6-hexandiol, besonders bevorzugt 1 ,4-Butandiol und 2,2,4-Tri- methyl-1 ,6-hexandiol. Selbstverständlich kann auch ein Gemisch aus niedermolekularen Diolen eingesetzt werden. Im allgemeinen wird ein Gemisch aus zwei Diolen verwendet. Das Diol kann auch im Gemisch mit einem Makrodiol des Typs Polyester, Polyether, Polysiloxan oder Polysulfon wie im einzelnen oben benannt verwendet werden, wobei das Polyester, Polyether, Polysiloxan oder Polysulfon mit einem mittleren Molekulargewicht wie oben genannt eingesetzt wird.
Als Makrodiole sind geeignet Polycarbonate, die zwei OH-Endgruppen aufweisen mit einem mittleren Molekulargewicht von Mw = 500 bis 6000 (Mw = Gewichtsmittel),
bevorzugt Polycarbonate mit zwei OH-Endgruppen mit einem mittleren Molekulargewicht von Mw = 500 bis 4000 (Mw = Gewichtsmittel), besonders bevorzugt Polycarbonate mit zwei OH-Endgruppen mit einem mittleren Molekulargewicht von Mw = 1000 bis 3000 (Mw = Gewichtsmittel), ganz besonders bevorzugt Polycarbonate mit zwei OH-Endgruppen mit einem mittleren Molekulargewicht von Mw = 1000 bis 2400 (Mw = Gewichtsmittel). Als Polycarbonate seien genannt solche mit C1- bis C10- Alkylenbausteinen, bevorzugt solche mit C2- bis C6-Alkylenbausteinen, besonders bevorzugt C2- bis C4-Alkylenbausteinen, wobei diese jeweils durch Methylgruppen substituiert sein können. Im einzelnen seien genannt Polyethylencarbonat, Polypro- pylencarbonat, Polytetramethylencarbonat, Polypentamethylencarbonat und Poly- hexamethylencarbonat.
Als niedermolekulare Diole sind geeignet C2- bis C10-Alkyldiole, die gegebenenfalls durch niedere Alkylreste wie C1- bis C3-Reste substituiert sein können. Im einzelnen seien genannt Ethylenglykol, 1 ,3-Propandiol, 1 ,4-Butandiol, 1 ,4-Bis-(hydroxymethyl)- cyclohexan, 1 ,6-Hexandiol, 2,2,4-Trimethyl-1 ,6-hexandiol und 2,4,4-Trimethyl-1 ,6- hexandiol, bevorzugt 1 ,4-Butandiol, 1 ,6-Hexandiol, 2,2,4-Trimethyl-1 ,6-hexandiol und 2,4,4-Trimethyl-1 ,6-hexandiol, besonders bevorzugt 1 ,4-Butandiol und 2,2,4-Tri- methyl-1 ,6-hexandiol. Selbstverständlich kann auch ein Gemisch aus niedermolekularen Diolen eingesetzt werden. Im allgemeinen wird ein Gemisch aus zwei Diolen verwendet. Das Diol kann auch im Gemisch mit einem Makrodiol des Typs Polycarbonat wie im einzelnen oben benannt verwendet werden, wobei das Polycarbonat mit einem mittleren Molekulargewicht wie oben genannt eingesetzt wird.
Bei der Umsetzung des Prepolymers mit dem Kettenverlängerer kann in an sich bekannter Weise ein Katalysator verwendet werden. Als Katalysatoren können beispielsweise Dibutylzinndilaurat, Zinnoctoat oder Diazabicyclooctan eingesetzt werden.
Das Verhältnis von NCO-Endgruppen des Prepolymers zu OH-Gruppen des Ketten- verlängerers beträgt im allgemeinen 1 ,01 : 1 bis 1 ,05 : 1 , bevorzugt 1 ,02 : 1 bis 1 ,04 : 1 , besonders bevorzugt 1 ,025 : 1 bis 1 ,035 : 1.
Die Molekulargewichtsfraktionierung des erfindungsgemäßen Verfahrens wird in an sich bekannter Weise durchgeführt. Geeignete Verfahren sind Fällungsreaktionen, Festphasenextraktion, Flüssigphasenextraktion, Adsorptionschromatographie, Fällungschromatographie nach Baker-Williams, Verteilungsfraktionierung, Gelpermea- tionschromatographie (GPC) und Kontinuierliche Polymerfraktionierung (CPF). Besonders geeignet für die Molekulargewichtsfraktionierung sind Fällungsreaktionen, Gelpermeationschromatographie und Kontinuierliche Polymerfraktionierung. Bei der Molekulargewichtsfraktionierung wird im allgemeinen ein niedermolekularer Anteil mit einem Massenanteil von 10 bis 55 Gew.% abgetrennt, bevorzugt wird ein niedermolekularer Anteil mit einem Massenanteil von 20 bis 50 Gew.%, besonders bevorzugt einer mit einem Massenanteil von 30 bis 45 Gew.% abgetrennt und verworfen.
Das erfindungsgemäße Verfahren wird im allgemeinen wie folgt durchgeführt. In einer geeigneten Apparatur, z.B. in einem mit Rührer, Stickstoffzuführung und Kühler mit Ableitungsrohr ausgerüsteten Dreihalskolben, wird zur Bildung des Prepolymers das Diisocyanat mit dem Makrodiol vermischt und unter ständigem Rühren erhitzt. Die Temperatur beträgt im allgemeinen 50 bis 120°C, bevorzugt 60 bis 100°C besonders bevorzugt 70 bis 90°C. Die Reaktionszeit für die Prepolymerbildung beträgt mindestens 5 h, bevorzugt ist eine Reaktionszeit für die Prepolymerbildung von 10 bis 20 h, besonders bevorzugt 14 bis 19h.
Währenddessen wird in einem weiteren Gefäß das Kettenverlängerungsmittel, gegebenenfalls nach Zugabe eines Katalysators in an sich bekannter Weise, z.B. Dibutyl- zinndilaurat, Zinnoctoat oder Diazabicyclooctan, gemischt und sodann zum Prepoly- mer zugegeben, sobald die Prepolymerbildung beendet ist. Anschließend wird die Reaktionsmischung bei einer Temperatur von 50 bis 120°C, bevorzugt 60 bis 100°C, besonders bevorzugt 70 bis 90°C unter ständigem Rühren für mindestens 48h erhitzt. Das resultierende Polymer wird nach eventueller Deaktivierung der überschüssigen NCO-Gruppen mit einem geeigneten Deaktivierungsreagenz, z.B. sekundären Aminen, bevorzugt Dibutylamin, gereinigt und getrocknet. Es ist auch möglich die oben beschriebene Reaktion in Anwesenheit von einem oder mehreren
Lösungsmitteln durchzuführen. Geeignet sind die Lösungsmittel Dimethylacetamid, Dimethylformamid, Chloroform, Methylenchorid, Trichlorethylen, Tetrahydrofuran und Dioxan, bevorzugt sind Dimethylacetamid, Dimethylformamid und Chloroform, besonders bevorzugt sind Dimethylacetamid und Chloroform. Ganz besonders bevorzugt wir Dimethylacetamid als Lösungsmittel eingesetzt. Wird die Umsetzung in Lösung durchgeführt, kann das entstandene Polymer durch Ausfällen in einem geeigneten Fällungsmittel, z.B. i-Propanol oder Wasser, abgetrennt und getrocknet werden.
Das erhaltene Polymer wird im Anschluss einer Molekulargewichtsfraktionierung unterworfen. Die Molekulargewichtsfraktionierung sei anhand von Fällungsreaktionen erläutert. Dazu wird das Polymer zunächst in Lösung gebracht. Als Lösungsmittel für das Polymer sind geeignet Dimethylacetamid, Dimethylformamid, Chloroform, Methylenchorid, Trichlorethylen, Tetrahydrofuran und Dioxan, bevorzugt sind Dimethylacetamid, Dimethylformamid und Chloroform, besonders bevorzugt ist Dimethylacetamid. Zu einer solchen Polymerlösung wird in an sich bekannter Weise langsam ein Nichtlösungsmittel, z.B. i-Propanol und/oder Wasser, bevorzugt i-Propa- nol, zugegeben. Dadurch wird die Löslichkeit des Polymers langsam geringer. Dies führt dazu, dass Moleküle mit dem höchsten Polymerisationsgrad zuerst ausfallen und kürzere Ketten in Lösung verbleiben. Die Polymerlösung wird bei konstanter Temperatur, z.B. Raumtemperatur, gehalten und das Fällungsmittel unter Rühren zugesetzt. Sobald die Lösung trübe wird, erhöht man die Temperatur, bis sich das ausfallende Polymer löst. Anschließend wird die Lösung auf die ursprüngliche Temperatur abgekühlt, das so ausgefällte Polymer wird abgetrennt und getrocknet. Geeignete Kombinationen aus Lösungs- und Fäilungsmittel können neben weiteren dem Fachmann bekannten Verfahren z.B. durch Trübungstitrationen bestimmt werden.
Das verbesserte E-Modul einer durch Recken nachbehandelten Gefäßprothese ergibt sich aus der Zeichnung, in der in einem Spannungs-/Dehnungs-Diagramm, in dem die Dehnungsmaße auf der waagerechten X-Richtung in % und die Spannungsmaße in N/mm2 aufgetragen sind.
In diesem Diagramm zeigt die Messkurve 1 das Elastizitätsverhalten einer Gefäßprothese, die keinem abschließenden Recken unterzogen worden ist. Demgegenüber gestellt ist durch Messkurve 2 das Elastizitätsverhalten desselben Polyurethan- Werkstückes, der abschließend gereckt worden ist. Hieraus ersieht man deutlich, dass bei einer Dehnung um 20 % eine um 60 % verringerte Spannung erreicht werden konnte. Die jeweils niedrigeren Spannungswerte, die einem geringeren E-Modul entsprechen, sind bis zu einer Dehnung von ca. 100 % festzustellen. Durch die Verbesserung des E-Moduls kann eine weitestgehende Annäherung an die natürlichen Gefäße bzw. Gefäßwände erreicht werden.
Claims
1. Gefäßprothesen oder Gewebeflicken (Patch) mit einer mikroporösen, fein- fibrillären Struktur, aus einem biokompatiblen Polymer, insbesondere aus Polyurethan, Polyamid, Polysulfon, Polyester, isotaktischem Polypropylen, Polynitril und/oder Polyvinylchlorid, deren Mischungen und/oder der Copoly- mere, g e k e n n z e i c h n e t d u r c h eine Elastizität, die durch abschließendes Recken (Dehnen) mit einem Aufdehnungsgrad von 30 bis 250 %, vorzugsweise 60 % bis 125 %, und anschließender Rückstellung erzeugt worden ist.
2. Verfahren zur Verbesserung des E-Moduls von Gefäßprothesen oder Gewebeflicken aus biokompatiblen Polymeren, insbesondere aus Polyurethan, Polyamid, Polysulfon, Polyester, isotaktischem Polypropylen, Polynitril und/oder Polyvinylchlorid, deren Mischungen und/oder der Copolymere, mit einer mikroporösen, feinfibrillären Struktur, gekennzeichnet durch ein abschließendes Recken (Dehnen) mit einem Aufdehnungsgrad zwischen 30 % und 150 %, vorzugsweise zwischen 60 % und 125 %, und anschließendem Entspannen.
3. Verfahren nach Anspruch 2, dadurch gekennzeichnet, dass die Porengröße der Gefäßprothese oder des Gewebeflickens vor dem Recken um ein zu erwartendes, nicht rückstellbares Dehnungsmaß kleiner ausgebildet wird.
4. Verfahren nach Anspruch 2 oder 3, dadurch gekennzeichnet, dass das Recken ein- oder biaxial durchgeführt wird.
5. Verfahren nach einem der Ansprüche 2 bis 4, dadurch gekennzeichnet, dass die Gefäßprothese oder der Gewebeflicken vor dem Recken mit einer in Wasser löslichen physiologischen Substanz, vorzugsweise Polyvinylalkohol (PVA), Polyvinylpyrolidon oder Gelatine (Kollagen) getränkt wird, die in die Gefäßprothese oder den Gewebeflicken ganz oder teilweise einzieht, vorzugsweise auf der Außenseite.
6. Verfahren nach einem der Ansprüche 2 bis 5, dadurch gekennzeichnet, dass die Gefäßprothese schlauchförmige ist und der zum Recken erforderliche Druck durch ein gasförmiges Medium, vorzugsweise durch Luft oder durch N oder ein flüssiges Medium von innen aufgebracht wird.
7. Verfahren nach Anspruch 6, dadurch gekennzeichnet, dass zur Vermeidung von Leckagen ein dehnbarer, vorzugsweise elastischer Hilfskörper in die zu reckende Gefäßprothese eingeführt wird, der danach mit dem druckaufbringenden Medium beaufschlagt wird.
8. Verfahren nach einem der Ansprüche 2 bis 5, dadurch gekennzeichnet, dass das Recken mit einem mechanisch größenverstellbaren Hilfskörper, auf den der Gewebeflecken zuvor aufgespannt wird oder der in die schlauchförmige Prothese eingeführt wird, durchgeführt wird.
9. Verfahren nach einem der Ansprüche 2 bis 5, dadurch gekennzeichnet, dass zum Aufweiten einer schlauchförmigen Gefäßprothese ein Ziehdorn verwendet wird.

Verfahren nach einem der Ansprüche 2 bis 9, dadurch gekennzeichnet, dass man zur Herstellung der Gefäßprothese oder des Gewebeflickens mindestens ein aliphatisches und/oder mindestens ein cycloaliphatisches Diisocyanat mit einem Makrodiol des Typs Polycarbonat oder des Typs Polyester, Polyether, Polysiloxan oder Polysulfon mit einem durchschnittlichen Molekulargewicht von 500 bis 6000 darstellt, wobei dass das Verhältnis von NCO-Endgruppen des Prepolymers zu OH-Gruppen des Kettenverlängerungsmittels 1 ,01 : 1 bis 1 ,05 : 1 beträgt und dass man das erhaltene Polymer, gegebenenfalls nach Behandlung mit einem Reagenz zur Deaktivierung der noch vorhandenen NCO-Gruppen, einer Molekulargewichtsfraktionierung unterwirft, bei der der niedermolekulare Polyurethananteil mit einem Massenanteil von 10 bis 55 Gew.% abgetrennt und verworfen wird und der zurückbleibende hochmolekulare Anteil als biokompatibles Polyurethan mit verbesserten Eigenschaften gewonnen wird.
