Abstract
Die Erfindung betrifft die Verwendung von APJ-Rezeptor-Agonisten zur Herstellung eines Arzneimittels zur Behandlung und/oder Prophylaxe von koronaren Herzkrankheiten, insbesondere stabile und instabile Angina pectoris, akuter Myokardinfarkt, Myokardinfarktprophylaxe, plötzlicher Herztod, Herzinsuffiziens, sowie Bluthochdruck und den Folgen der Atherosklerose.
Description
REGULATION DES APJ-REZEPTORS ZUR BEHANDLUNG ODER PROPHYLAXE VON HERZKARNKHEITEN
Die Erfindung betrifft die Verwendung von APJ-Rezeptor-Agonisten zur Herstellung eines Arzneimittels zur Behandlung und/oder Prophylaxe von koronaren Herzkrank- 5 heiten, insbesondere stabiler und instabiler Angina pectoris, akutem Myokardinfarkt,
Myokardinfarktprophylaxe, plötzlichem Herztod, Herzinsuffizienz, sowie Bluthochdruck und den Folgen der Atherosklerose.
Das Herz benötigt als unablässig arbeitender Hohlmuskel zur Deckung seines Ener- 10 giebedarfs eine besonders intensive Versorgung mit Sauerstoff. Versorgungsstörungen betreffen daher in erster Linie den Sauerstofftransport, der bei verminderter Anpassungsfähigkeit der Durchblutung unzureichend sein kann. Eine Steigerung des Sauerstoffverbrauchs kann nur durch eine Zunahme der Herzdurchblutung abgedeckt werden. 15
Bei koronaren Herzkrankheiten wie stabile und instabile Angina pectoris, Herzinsuffizienz, Myokardinfarkt, plötzlichem Herztod, sowie den Folgen der Atherosklerose ist eine ausreichende Durchblutung von Teilen des Herzgewebes nicht mehr gewährleistet und es kommt zu Gewebsischämien, die zu Nekrose und Apoptose in den 20 betroffenen Arealen fuhren. Dadurch kommt es zu einer myokardialen Dysfunktion, die sich bis zur Herzinsuffizienz hin entwickeln kann.
Therapieverfahren und Wirkstoffe, die die koronare Durchblutung und somit das Sauerstoffangebot verbessern, aber auch solche, die den Sauerstoffverbrauch senken, 25 sind zur Behandlung von Symptomen der oben genannten Erkrankungen geeignet.
Dazu gehören die Dilatation größerer Koronargefäße, die Senkung der extravasalen Komponente des Koronarwiederstandes, die Senkung der intramyokardialen Wandspannung und die Dilatation der arteriolären Widerstandsgefäße in der systemischen 30 Zirkulation.
Substanzen und Verfahren, die zu einer Erhöhung des Koronarflusses im Herzen und/oder zu einer Blutdrucksenkung führen, können therapeutisch genutzt werden (Forth, Henschler, Rummel; Allgemeine und spezielle Pharmakologie und Toxikologie; Urban & Fischer Verlag (2001), München).
Die oben beschriebenen Wirkungen können über die Stimulation von G-Protein-ge- koppelte Rezeptoren gesteuert werden. G-Protein-gekoppelte Rezeptoren sind 7- transmembran-Domänen Proteine, deren Aktivierung zur Aktivierung verschiedener G-Proteine führen. Diese aktivierten G-Proteine wiederum können verschiedene weitere Signalsysteme aktivieren oder inaktivieren und somit zur Induktion der oben beschriebenen Mechanismen führen. Bei den G-Proteinen unterscheidet man zwischen Gas (Adenylylcyclase aktivierend), Gαi (Adenylylcyclase inhibierend) und Gαq (Zunahme IP3).
Die DNA-Sequenz, die für den humanen APJ-Rezeptor kodiert, wird im Sequenzprotokoll in der SEQ ID NO: 1 gezeigt. Die Aminosäuresequenz des humanen APJ- Rezeptor wird in der SEQ ID NO:2 des Sequenzprotokolls gezeigt. Der APJ-Rezeptor ist ein Gαi gekoppelter Rezeptor (O'Dowd et al., Gene, 136 (1993) 355-360), der unter anderem durch seinen natürlichen Liganden Apelin (Tatemoto et al., Biochem. Biophys. Res. Commun., 251 (1998) 471-476) aktiviert wird. Der Rezeptor zeigt eine
Homologie von 30% zum ATl-(Angiotensin) Rezeptor, wird aber nicht durch Angio- tensin aktiviert.
Der APJ-Rezeptor der Ratte (beschrieben in WO0068250) und der Maus (beschrie- ben in WO0068244) wird ubiquitär in vielen Regionen des Gehirns und in anderen
Geweben exprimiert (Hosoya et al., The Journal of Biological Chemistry, 275 (2000) 21061-21067; O'Carroll et al., Biochimica et Biophysica Acta, 1492 (2000) 72-80). Im Menschen ist die Expression des Rezeptors in Milz, Thymus, Prostata, Testis und im Verdauungstrakt beschrieben (Edinger et al., Journal of Virology, 72 (1998) 7934-7940).
Überraschenderweise wurde nun in der quantitativen Analyse der APJ-Rezeptor- mRNA-Expression im Menschen gefunden, dass die Expression des Rezeptors auf wenige Gewebe beschränkt ist und eine deutliche Expression des Rezeptors im humanen Herzen vorhanden ist (Abbildung 1). Es wurde außerdem gefunden, dass der APJ-Rezeptor sowohl in gesundem als auch in DCM-Herzen (DCM=dilative
Kardiomyopathie) vorkommt (Abbildung 2). Da die Expression des humanen APJ- Rezeptors im erkrankten Herzen eine Bedingung für den Einsatz von Wirkstoffen zur Stimulation des Rezeptors in Patienten mit koronaren Herzkrankheiten ist, schafft dieses Ergebnis die Voraussetzung für einen neuen Therapieansatz. Eine Abnahme der Rezeptor-Expression, wie sie z.B. für den ßl-Adrenorezeptor bei Herzinsuffizienz bekannt ist (Chakraborti et al., Cellular Signaling, 12 (2000) 499-513), würde bedeuten, dass eine therapeutische Stimulation ausgeschlossen ist.
Bekannt ist, dass die systemische Gabe des natürliche Liganden Apelin in der Ratte zu einer Blutdrucksenkung ohne Änderung der Herzfrequenz fuhrt (Lee eta al., Journal of Neurochemistry, 74 (2000) 34-41; Tatemoto et al., Regulatory Peptides 99 (2001) 87-92). Die direkte Applikation des Liganden ins Gehirn der Ratte bewirkt eine Inhibition der Wasseraufnahme bei dehydrierten Ratten ohne einen Effekt auf den Blutdruck zu haben (Reaux et al., Journal of Neurochemistry, 77 (2001) 1085- 1096). Demnach scheint die Blutdrucksenkung durch eine direkte Stimulation des
Rezeptors in Gefäßen ausgelöst zu werden.
Überraschenderweise bewirkt die systemische Gabe von Apelin, sowohl als Bolus, als auch als permanente Infusion, im Hund eine konzentrationsabhängige, sehr deut- liehe Koronarflusserhöhung ohne Änderung der Herzfrequenz (Abbbildung 3, 4).
Diese Koronarflusserhöhung geht weit über das reflektorische Maß der ebenfalls beobachteten Blutdrucksenkung hinaus.
Aufgrund dieses neuen Ergebnis kamen wir zu dem Schluss, dass Substanzen, die den APJ-Rezeptors stimulieren, wegen der daraus resultierenden Erhöhung des
Koronarflusses, zur Behandlung und/oder Prophylaxe von stabiler und instabiler
Angina pectoris, akutem Myokardinfarkt, Myokardinfarktprophylaxe, Herzinsuffizienz, plötzlichem Herztod, sowie Bluthochdruck und den Folgen der Atherosklerose beim Menschen eingesetzt werden können.
Die vorliegende Erfindung betrifft daher die Verwendung von APJ-Rezeptor-Agonisten zur Herstellung eines Arzneimittels zur Behandlung und/oder der Prophylaxe der oben genannten Krankheiten.
Agonisten im Sinne der Erfindung sind alle Substanzen, die eine Stimulation der biologischen Aktivität des Rezeptors bewirken. Besonders bevorzugte Agonisten sind Nukleinsäuren inklusive „locked nucleic acids", „peptide nucleic acids" und „Spiegelmere", Proteine inklusive Antikörper und niedermolekulare Substanzen, ganz besonders bevorzugte Agonisten sind niedermolekulare Substanzen.
Die Stimulation kann z.B. im unten beschriebenen APJ-Stimulationstest gemessen werden. Dabei liegt eine Stimulation des APJ-Rezeptors vor, wenn in dem Test eine mindestens 10 %ige Abnahme eines, durch Stimulation der Adenylatzyklase, erhöhten cAMP-Spiegels gemessen wird, wobei die Stimulation unter anderem durch Forskolin oder über adrenerge Rezeptoren erfolgen kann.
APJ-Rezeptor-Agonisten können an rekombinanten Zelllinien, die den humanen APJ-Rezeptor enthalten getestet werden.
Dabei werden solche APJ- Agonisten bevorzugt, die im unten angegebenen APJ-Sti- mulationstest mit einem EC5Q von 1 μM, bevorzugt weniger als 0,1 μM aktivieren.
Vorzugsweise können die erfindungsgemäßen APJ-Rezeptor-Agonisten die Blut/Hirn Schranke nicht passieren und wirken systemisch und nicht zentral.
Kurzbeschreibung der Abbildungen:

APJ-Stimulationstest
Die Wirkung des Apelins und anderer möglicher APJ- Agonisten wird an einer CHO Zelllinie getestet, die den kompletten offenen Leserahmen des humanen Rezeptorgens als rekombinantes Protein stabil exprimiert. Bei Aktivierung dieser Zelllinie mit Forskolin erhöht sich der interne cAMP-Spiegel. Dieser Anstieg kann durch gleich- zeitige oder vorherige Gabe von Apelin über die Stimulation des rekombinant exprimierten APJ-Rezeptors und die daraus resultierende Inhibition der Adenylylcyclase verhindert werden.
1500 CHO Zellen/Loch werden auf 384-Loch Platten (Greiner) in DMEM Medium (Gibco) mit 10 % FCS (fötales Kälberserum, Gibco) ausgesät und 2 Tage bei 37°C und 5 % CO2 kultiviert. Die Zellen werden mit Apelin- Verdünnungen (Verdünnungsreihen typischerweise von 10"13-10"7 M) inkubiert und anschließend mit 10"5 M Forskolin stimuliert. Der cAMP-Spiegel wird mit Hilfe des „cAMP Screen Kits" [Tropix (PE Biosystems)] gemäß der Anleitung bestimmt und die Wirkung von Apelin als prozentuale Inhibition der maximalen cAMP-Erhöhung nach Stimulation
mit Forskolin dargestellt. Der ECso-Wert der Wirkung von Apelin ist der Wert bei dem 50 % des Forskolinsignals inhibiert werden.
Für Apelin wird ein EC50-Wert von 10"n M bestimmt.
Untersuchungen der APJ-Rezeptor-Expression beim Menschen
Die relative Expression des APJ-Rezeptors in humanen Geweben wird durch die Quantifizierung der mRNA mittels der Echtzeit-Polymerasekettenreaktion ermittelt (sog. TaqMan-PCR, Heid et al., Genome Res 6 (10), 986-994). Gegenüber der klassischen PCR bietet die Echtzeit-PCR den Vorteil einer genaueren Quantifizierung durch Einführung eines zusätzlichen, fluoreszenzmarkierten Oligonucleotides. Diese sogenannte Sonde enthält am 5 '-Ende den Fluoreszenzfarbstoff FAM (6-Carboy- Fluorescein) und am 3 '-Ende den Fluoreszenzquencher TAMRA (6-Carboxy-tetra- methylrhodamin). Während der Polymerasekettenreaktion wird in der TaqMan-PCR durch die 5'-Exonukleaseaktivtät der Taq-Polymerase der Fluoreszenzfarbstoff FAM von der Sonde abgespalten und dadurch das vorher gequenchte Fluoreszenzsignal erhalten. Als sog. Schwellenwert [treshold cyle (Ct-Wert)] wird die Zyklenzahl aufgezeichnet, bei der die Fluoreszenzintensität ca. 10 Standardabweichungen über der Hintergrund-Fluoreszenz liegt.
Als Ausgangsmaterial für die PCR wird käuflich erworbene zelluläre Gesamt-RNA verwendet (Fa. Clontech). Im Falle der humanen Herzen werden kleine Stücke (ca. 0,5 g) explantierter Herzen von Patienten mit dilatativer Kardiomyopathie (erhalten vom Deutschen Herzzentrum Berlin) und die Gesamt-RNA hieraus mittels RNaesy-
Säulen (Fa. Qiagen) gemäß der Anleitung isoliert. Je 1 μg Gesamt-RNA je Gewebe wird zur Entfernung von Kontaminationen genomischer DNA mit 1 Einheit DNase I (Fa. Gibco) für 15 min bei Raumtemperatur umgesetzt. Die Inaktivierung der Dnase I erfolgt durch Zugabe von 1 μl EDTA (25 mM) und nachfolgendes Erhitzen auf 65°C (10 min).
Anschließend wird im selben Reaktionsansatz die cDNA-Synthese gemäß der Anleitung zum „SUPERSCRΓPT-II RT cDNA synthesis kit" (Fa. Gibco) durchgeführt und das Reaktionsvolumen mit destilliertem Wasser auf 200 μl aufgefüllt. Für die PCR wird zu je 5 μl der verdünnten cDNA-Lösung 7,5 μl Gemisch von Primer und Sonde sowie 12,5 μl TaqMan-Reaktionslösung [Universal Master Mix (Fa. Applied
Biosytems] gegeben. Die Endkonzentration der Primer ist jeweils 300 nM, die der Sonde 150 nM. Die Sequenz des „forward"- und „reverse"-Primers für den APJ- Rezeptor lautet: 5'-TCCCAGGAGTAAAAGCCAAGC-3' bzw. 5'- CCTTCAAGGGTCCTGTCAGC-3', die Sequenz der fluoreszierenden Sonde 5'- 6FAM-AGAGGTTGTTTTTGCCAAGAATCACAGAATGTTA-TAMRA-3'. Die
PCR erfolgt auf einem ABI-Prism-SDS-7700-Gerät (Fa. Applied Biosystems) gemäß der Anleitung des Herstellers. Dabei werden 40 Zyklen durchgeführt. Der Ct-Wert (s. o.) der für den APJ-Rezeptor für jedes Gewebe erhalten wird, entspricht dem Zyklus, in dem die Fluoreszenzintensität der freigesetzten Sonde das lOfache des Hinter- grundsignals erreicht. Je niedriger der Ct-Wert, umso früher beginnt also die Vervielfältigung, d. h. je mehr mRNA ist in der ursprünglichen Probe enthalten. Zum Ausgleich eventueller Schwankungen bei der cDNA-Synthese wird in allen untersuchten Geweben auch die Expression eines sog. „Haushaltsgenes" analysiert. Dieses sollte in allen Geweben ungefähr gleich stark exprimiert werden. Für die Normierung der APJ-Rezeptor-Expression wird hierfür ß-Actin verwendet. Die Sequenz des „forward"- bzw. „reverse" Primers für ß-Actin ist 5'-TCCACCTTCCAGCAGATGTG- 3' , und 5'-CTAGAAGCATTTGCGGTGGAC-3' die Sequenz der Sonde 5'-6FAM- ATCAGCAAGCAGGCAGTATGACGAGTCCG-TAMRA-3'. Die Auswertung der Daten erfolgt durch die sog. dCt-Methode entsprechend der Anleitung zum ABI Prism SDS 7700 (Fa. Applied Biosystems). Für die graphische Darstellung der Gewebeverteilung des APJ-Rezeptors wird das Gewebe mit der niedrigsten relativen Expression willkürlich gleich 1 gesetzt (Abbildung 1) und alle anderen Gewebe hierauf normiert. Ausgenommen aus der Darstellung sind Gewebe mit einem Ct-Wert >35. Für die Analyse der Expression des APJ-Rezeptors in den Herzen wird der Mit- telwert der relativen Expression aus den gesunden Kontrollherzen = 1 gesetzt und mit dem Mittelwert aus den DCM-Herzen verglichen (Abbildung 2).
Die Expressionsdaten des APJ-Rezeptors im Menschen zeigen, dass der Rezeptor im Herzen einen Ct-Wert von 31,64 aufweist, was einer mittleren Expression des Rezeptors im Herzen entspricht. Der Vergleich der mRNA Menge in gesundem und DCM-Gewebe zeigt, dass es keine signifikante Abnahme der Rezeptorexpression in DCM-Herzen gibt, somit eine Stimulation des Rezeptors durch Agonisten auch bei Patienten mit koronaren Herzkrankheiten therapeutisch genutzt werden kann.
In vivo Untersuchungen der Stimulation des APJ-Rezeptors durch Apelin
Erwachsene FBI- (Foxhound-Beagle-Irish-Setter) Hunde (20-30kg) werden initial mit Thiopental-Na (Trapanal, Byk-Gulden) 20-30 mg/kg i.v. narkotisiert. Zur Muskelrelaxation wird Alcuroniumchlorid (Alloferin, Röche) initial 0,1 mg/kg i.v. gegeben. Die Tiere werden inhibiert und mit einem Gemisch aus O2/N2O 1/3 mit einer Engström- Atempumpe mit 15-18 Atemzügen pro min und einem Volumen von
18-24 ml/kg beatmet. Die genaue Einstellung der Beatmung wird nach dem arteriellen CO2-Partialdruck durchgeführt, wobei ein mittlerer pCO2 von 35 - 45 mmHg eingehalten wird. Die Erhaltungsanästhesie wird mit Isofluran (Baxter) 1,5 -3 % gemacht. Die Körpertemperatur wird bei 38°C ± 0,1°C gehalten. Der arterielle Blutdruck wird über einen Katheder in der Femoralarterie gemessen. Es wird eine Thorakotomie auf der linken Seite am fünften Intercostalraum durchgeführt. Die Lunge wird zurückgelegt, fixiert und das Pericard eingeschnitten. Ein proximaler Abschnitt der LAD (linke Koronararterie) distal zur ersten diagonalen Verzweigung wird freipräpariert und ein kalibrierter, elektromagnetischer Flussmess- köpf (Gould Statham, Modell SP7515) um das Gefäß gelegt und mit einem Fluss- messgerät (Statham, Modell SP-2202) verbunden. Ein mechanischer Okkluder wird distal zum Flussmesskopf so angebracht, dass keine Verzweigungen zwischen Fluss- messkopf und Okkluder liegen.
Blutentnahmen und Substanzgaben werden durch einen Katheder in der Femoralvene durchgeführt. Ein peripheres EKG wird mit subcutan verankerten Nadeln abgeleitet.
Ein Mikrotip-Druckmanometer (Millarmodel PC-350) wird durch den linken Vorhof geschoben um den linksventrikulären Druck zu messen. Die Messung der Herzfrequenz wird über die R-Zacke des EKGs gesteuert. Die hämodynamischen Parameter und der Koronarfluss werden während des gesamten Versuchs über einen Vielfach- Schreiber aufgezeichnet.
Nach einer Stabilisierungszeit von einer Stunde wird das Experiment begonnen. Es werden entweder 10 μg/kg Apelin in physiologischer Kochsalzlösung als i.v. Bolus injiziert oder verschiedene Apelin Konzentrationen (1, 3, 5, 10 μg/kg/min) über 10 min infundiert. Herzfrequenz, Blutdruck und Koronarfluss werden registriert und ausgewertet.
Sowohl die Gabe als Bolus, als auch die Infusion von Apelin bewirkt eine signifikante Steigerung des Koronarflusses am Hund, trotz einer Senkung des Blutdrucks (Abbildung 3). Die Infusion von Apelin zeigt eine dosisabhängige Zunahme des
Koronarflusses, wobei das Maximum bei 3 μg/kg/min erreicht ist (Abbildung 4).
AP J-Rezeptor-Agonist Formulierungen
Die APJ-Rezeptor-Agonisten können in bekannter Weise in die üblichen Formulierungen überführt werden, wie Tabletten, Dragees, Pillen, Granulate, Aerosole, Sirupe, Emulsionen, Suspensionen und Lösungen, unter Verwendung inerter, nicht toxischer, pharmazeutisch geeigneter Trägerstoffe oder Lösungsmittel. Hierbei soll die therapeutisch wirksame Verbindung jeweils in einer Konzentration von 0,5 bis 90 Gew.-% der Gesamtmischung vorhanden sein, d.h. in Mengen, die ausreichend sind, um den angegebenen Dosierungsspielraum zu erreichen.
Die Formulierungen werden beispielsweise hergestellt durch Strecken der Wirkstoffe mit Lösungsmitteln und/oder Trägerstoffen, gegebenenfalls unter Verwendung von Emulgiermitteln und/oder Dispergiermitteln, wobei z.B. im Fall der Benutzung von
Wasser als Verdünnungsmittel gegebenenfalls organische Lösungsmittel als Hilfslösungsmittel verwendet werden können.
Die Applikation erfolgt in üblicher Weise, vorzugsweise oral, transdermal, intrave- nös oder parenteral, insbesondere oral oder intravenös. Sie kann aber auch durch Inhalation über Mund oder Nase, beispielsweise mit Hilfe eines Sprays erfolgen, oder topisch über die Haut.
Im Allgemeinen hat es sich als vorteilhaft erwiesen, Mengen von etwas 0,001 bis 10 mg/kg, bei oraler Anwendung vorzugsweise etwa 0,005 bis 3 mg/kg Körpergewicht zur Erzielen wirksamer Ergebnisse zu verabreichen.
Trotzdem kann es gegebenenfalls erforderlich sein, von den genannten Mengen abzuweichen, und zwar in Abhängigkeit vom Körpergewicht bzw. der Art des Applikati- onsweges, vom individuellen Verhalten gegenüber dem Medikament, der Art von dessen Formulierung und dem Zeitpunkt bzw. Intervall, zu welchen die Verabreichung erfolgt. So kann es in einigen Fällen ausreichend sein, mit weniger als der vorgenannten Mindestmenge auszukommen, während in anderen Fällen die genannte obere Grenze überschritten werden muss. Im Falle der Applikation größerer Mengen kann es empfehlenswert sein, diese in mehreren Einzelgaben über den Tag zu verteilen.
Claims
1. Verwendung von APJ-Rezeptor-Agonisten zur Herstellung eines Arzneimittels zur Behandlung und/oder Prophylaxe von koronaren Herzkrankheiten, Bluthochdruck und den Folgen der Atherosklerose.
2. Verwendung von Anspruch 1, wobei die koronaren Herzkrankheiten, stabile und instabile Angina pectoris, akuter Myokardinfarkt, Myokardinfarktpro- phylaxe, plötzlicher Herztod und Herzinsuffiziens sind.
3. Verwendung nach Anspruch 1 oder 2, wobei der APJ-Rezeptor-Agonist einen ECso-Wert von weniger als 1 μM hat.
4. Verwendung nach Anspruch 1 oder 2, wobei der humane APJ-Rezeptor-Ago- nist einen ECso-Wert von weniger als 100 nM hat.
