Abstract

In order to enable the position of a patient in a bed to be changed easily, without excessive physical exertion, to avoid decubitus ulcers (bedsores), the invention calls for shafts (4a and 4b) which can be lifted upwards to be disposed one along each side of the bed. Rotation of the shafts (4a and 4b) winds a sheet (7) spread over the mattress (3) round them or unwinds the sheet. The patient is moved into the required position by coordinated rotation of the shafts (4a and 4b) and the shaft-lifting levers (5a, 6a; 5b, 6b) or by relocating another special device.

Description

The invention relates to an auxiliary device for a bed, in particular a patient bed, according to the preamble of patent claim 1.

Such an auxiliary device is known from GB-A-2 102 675. This has an adjustable frame on the bed, which carries optionally drivable rollers at mattress height, the rollers being electrically driven via a chain drive which runs around the underside of the bed, a cloth extending between the rollers across the bed to which the patient lies and which can be moved in either direction. This known construction is relatively complex and can not be easily attached to any conventional bed.

The beds, consisting essentially of a bed frame, a resilient underlay (mattress) and an adjustable headboard, have already been adapted to the needs of patients and nursing staff in a variety of ways. The lying surfaces are adjustable in height, head and foot sections are variable, and the bed frame is mobile.

However, there are special problems in patients who themselves can hardly or not move at all. Lying on the same part of the body for a long time creates slightly sore spots on the surface of the body, which are very hazardous to health, difficult to heal and also painful. This leads to the dreaded pressure points. Such a clinical picture is difficult and costly to cure and can lead to death in the advanced stage.

The nursing staff is therefore required to regularly move such immobile patients to a different position so that the parts of the body that are exposed to stress are constantly changed. This is physically very difficult work with possible long-term damage to the spine and the associated loss of the caregiver. In addition, someone is not always available to change the location, especially at night.

In order to relieve the nursing staff, various mechanical auxiliary devices for changing the position of people with reduced mobility have already become known, apart from GB-A-1 102 675. For example, according to SU-PS 1 776 400 on fixed or height-adjustable supports belts are provided which reach under the patient and lift them from the bed support by adjusting the height of the supports or can turn them into a side position by hanging them on one of the supports. The disadvantage here is that the whole bed is a special construction and the straps are not usable as long-term support.

Außerdem ist für die Anbringung eine angepaßte Bettkonstruktion nötig.DE-OS 35 90 309 provides a bed which is arranged between two shafts, two opposite sides of the sheet being wound around one shaft each. In order to turn a person lying on the sheet, the sheet is wound from one shaft to the other. The laterally arranged shafts and hinges are a hindrance for the operating personnel. By folding the hinges to which the shafts are attached, the shafts are to be lowered so that a person can get out of bed better. This is a cumbersome construction that is unlikely to be practically accepted due to the difficult handling. In addition, an adapted bed construction is necessary for the attachment.

Another auxiliary device is described in EP-A-0 374 784. In this known device shafts are arranged on both sides of the bed, which can be moved up and down with spindle drives. An endless loop is wrapped around the waves, which lifts the patient when the spindle columns are raised. The patient can be moved to another position by rotating the shafts in one direction or the other. The upper run and the lower run of the sheet are pressed together when the patient is lifted, so that great friction must be overcome when the shafts are turned.

The invention has for its object to provide a structurally not very complex auxiliary device with a receiving device for the bed sheet to be wound up, which does not impede the nursing staff, is easy to handle, can bring about any desired changes in the patient's position and can be easily attached to any conventional bed .

To achieve this object, the features specified in claim 1 are proposed.

Appropriate developments of the invention are characterized in the subclaims.

The holding devices that can be attached to the head and foot ends for receiving the waves at different heights can on the one hand bring the patient into any desired height and inclination position and can also rotate about his body axis in another desired position. In the lower starting position, the waves and levers are in no way disturbing for the nursing staff.

There is a mattress 3 between the head end 1 and the foot end 2 of a bed. A shaft 4a is arranged parallel to the right edge of the bed and is rotatably mounted in levers 5a and 5b both at the head end 1 and at the foot end 2. The levers 5a and 5b can be pivoted upwards towards the center of the bed, as can be seen in the positions indicated. The shaft 4a is raised over the mattress 3 to the middle of the bed. A corresponding counter-rotating device is also attached to the left side of the bed. In Fig. 1, only the levers 6a and 6b are indicated in different positions.

Der besseren Übersicht wegen ist nur die Betthälfte zum Kopfende 1 hin dargestellt.2, 3, 4 and 5 show the operation of the auxiliary device. For the sake of clarity, only the half of the bed towards the head end 1 is shown.

Above the mattress 3 there is a sheet-like support 7 (sheet) which is attached to the two side shafts 4a and 4b in such a way that it is wound up or unwound when the shafts 4a and 4b are rotated. Depending on the direction of shaft rotation, the cloth-like pad 7 moves in this way to the right or left edge of the bed. The possibility of rotation of the shafts 4a and 4b is indicated by the arrow arcs P and P1. In addition, the shafts 4a and 4b can be raised by means of the levers 5a and 6a (and the lever at the foot end, not shown) according to the arrows P2.

Fig. 3 shows a position with the levers 5a and 6a slightly raised and the lateral shafts 4a and 4b raised up to that point. This is the safety position because the patient cannot roll out of bed. The raised shafts 4a and 4b together with the support 7 form a barrier.

Fig. 4 shows the patient after changing the position from the back to the side. The levers 5a and 6a are still further pivoted upwards, and the pad 7 has lifted off the mattress 3. Depending on the direction of rotation of the shafts 4a and 4b, the patient can be rolled onto his left or right side. His back areas are relieved of pressure from the couch, the stressed areas can be supplied with blood again, which prevents the development of sores (pressure ulcers). The device can be operated both by hand and by motor. With manual drive, the shafts 4a and 4b are rotated using modified hand cranks. All of this happens without significant physical effort for the nursing staff. It is also possible to carry out a regular automatic change of the patient's position by equipping it with a program control. The times and the extent of the lever movements can be adapted to all requirements.

Fig. 5 zeigt den Patienten in Seitenlage, wobei die Lagestabilisierung durch weiteres Hochfahren der Wellen je nach Lage links oder rechts erreicht wird.5 shows the patient in a lateral position, the position stabilization being achieved by further raising the waves to the left or right, depending on the position.

In addition to the one described above, the special device can also consist of special guides and of electric drive, hydraulic or pneumatic cylinders.

The constructive structure of the individual components of the auxiliary device is of crucial importance for practical testing.

9a montiert sind, sind alle unfallträchtigen beweglichen Teile an den kritischen Stellen schützend abgedeckt.6a and 6b show the structure of the assembly unit to be attached to the foot end 2 or head end 1 of a bed with the levers 5a and 6a, which are each latched and pivotably mounted in a guide chamber 8a or 9a. The levers 5a and 6a can be snapped into a locking plate 10a and 11a at fixed swivel heights. The two guide chambers 8a and 9a are connected to one another by means of an exchangeable intermediate plate 12. For example, if the auxiliary device is mounted from a narrower bed to a wider bed then the intermediate bracket that is decisive for the narrower bed only has to be replaced by a wider intermediate bracket. Because the levers 5a and 6a are each mounted in a guide chamber 8a and 9a, all moving parts that are prone to accidents are protected at the critical points.

Fig. 7 zeigt die drehbar gelagerten Wellen 4a bzw. Fig. 7 shows the rotatably mounted shafts 4a and 4b with the levers 5a and 5b on a larger scale. In order to be able to compensate for different lever movements and also transverse displacements of the head part and foot part against each other, the shaft consists of telescopically displaceable sections. The support 7 is received by a tubular middle part 13, into which one of end parts 14 rotatably mounted on the levers is inserted, while a driven counterpart 15 is coupled in a rotationally secure manner to the middle part 13 by means of a pipe clip 16 extended through the middle part 13 and the driven counterpart 15 is. When the counterpart 15 is rotated by means of a crank attached to a crank pin 17, the central part 13 is rotated about the longitudinal axis in order to wind or unwind the support 7. The middle part 13 rotates freely around a telescopic part 18 of the other end part 14 which is inserted into the middle part 13. If the auxiliary device is driven by a motor M mounted on the end part 14, then only the tubular linch pin 16 has to be converted to the end part 14.

Gegenstück 15 und Mittelteil 13.Now the middle part is set in rotation via the end part 14, whereby it rotates freely around the telescopic part 18 ′ of the counterpart 15. End part 14 and counterpart 15 are provided with a universal joint 20 or 20 'between telescopic part 18 or 18' and a lever bearing 19 or 19 '. If the shaft is lifted on one side, excessive kink loading is prevented, and problem-free transverse displacement of the headboard and footboard of the bed is made possible. Telescopic guide and universal joint ensure the balance of the Differences in length between the bearing points and bridging the changing angles between end piece 14 or counterpart 15 and middle part 13.

Fig. 8 zeigt eine solche Nutzungsmöglichkeit.Transporting a patient from a bed to a stretcher or treatment table, e.g. an X-ray table is particularly difficult for the caregiver. The auxiliary device according to the invention brings significant relief here. The levers of a shaft can be pulled apart for extension so that a "ramp" protrudes above the edge of the bed, which can be brought to and at the level of the transfer table. Now the patient can be easily pushed out of bed onto the X-ray table or the like. 8 shows such a possibility of use.

The most varied functions in manual operation of the auxiliary device are to be listed again in a table in the most important functions for better understanding.

A. Aufziehen der Auflage bei leerem Bett.A. Spread the pad when the bed is empty.

  • 1. Auflage über dem Bett entfalten.Unfold the 1st edition over the bed.
Place with the long side across the bed and with the fleece band down.
  • 3. Auflagemarkierung in Längsachse und Bettmitte positionieren.3. Position the support mark in the longitudinal axis and center of the bed.
Swivel the tubular shafts (right and left) into an upper position and snap them into place. Press the velcro strips of the support straight and firmly onto the Velcro strips of the tubular shafts. If the head is to be supported, start at the head side, otherwise as required.
Turn both tube shafts one after the other by means of the winches, with a slightly firm support, until about 5-6 turns are wound up. Here the right tube shaft is turned clockwise and the left one in Turned counterclockwise. The two markings remain in the middle of the bed. It is rotated until the pad is nice and smooth and tight.
Loosen the support with the two winches on both sides until it touches the mattress.
Swing the two tube shafts to the lowest position, smooth the sheet (support), slightly tighten both tube shafts, and the bed is ready.
B. Auflage aufziehen mit Patienten im BettB. Mount the pad with patients in bed

Unfold the 1st pad and have it ready to pass it under the patient.
Lift the patient up with the pad and guide the pad underneath and align it or push it together next to the patient and roll it over the pad as with bed sheets.
Lower the patient, swivel the shafts (right and left) into an upper position and snap them into place. Press the velcro strips of the support straight and firmly onto the Velcro strips of the tubular shafts.
Depending on the size of the patient, latch the side tubular bars (left and right) into the highest or second-highest catch. If the head is to be supported, start with the support bracket closer to the head side, otherwise attach as required.
Use the winches to turn both tube shafts until the support is slightly tensioned. The right tubular shaft is turned clockwise and the left one counterclockwise. The two markings remain in the middle of the bed.
Swing down the two tubular shafts, tighten slightly, and the bed is ready.
C. Patienten drehenC. Turn patient 1. Drehen entgegen und im Uhrzeigersinn1. Turn counterclockwise and clockwise

Depending on the size of the patient, latch the side tube shafts (left and right) into the highest or second-highest catch. Depending on requirements, the pad should be loosened slightly before swinging up.

Slightly lift the patient by turning the two cranks in opposite directions.
Turning the two cranks clockwise turns the patient counterclockwise, i.e. in the opposite direction.
If you change the direction of rotation of the two cranks, the patient turns clockwise.
This is possible from the back to the prone position and any desired intermediate position.
It is no longer possible to turn beyond the prone position, otherwise the support will overturn and unwind from the shaft.

2. Ab Bauchlage wieder zurück2. Back from the prone position

The patient must be turned back in the opposite direction of rotation. This then goes back to the prone position.
The patient can therefore not be rotated continuously in one direction of rotation, but must be rotated back and forth or left and right in the cycle.

D. SonderfunktionenD. Special functions 1. Stabilisieren der Seitenlagen1. Stabilize the side layers

The tubular shafts located in the back of the patient are locked in one of the three locking positions so that the patient is optimally supported.
Pad is easily relaxed.
If necessary, additional cushions are used.

2. Unterstützung der Bauchlage2. Support the prone position

The patient is raised by turning the two crank handles in opposite directions.
A pillow is placed under the chest and chin so that the patient can breathe freely.
The patient is drained.
Unconscious patients are left lying with their breathing openings exposed in a slightly tense condition.

3. Sicherheit gegen Herausfallen3. Security against falling out

The two tubular shafts are locked in the first position.

4. Sichtschutz gegen unerwünschte Blicke4. Privacy protection against unwanted looks

Both pipe shafts are snapped into one of the two upper positions when the support is relaxed.

In hospital and nursing home operations, it is advantageous for reasons of time not to perform the functions manually, but via a motor drive. When the motor is switched on using a foot control, the caregiver has the option of securing any supply hoses or the like with his free hands when moving the patient.
