Description
Herzklappenprothese zum Ersatz der Aortenklappe
Die Erfindung betrifft eine Herzklappenprothese zum Er¬ satz der Aortenklappe oder Pulmonalklappe nach dem Oberbegriff des Anspruch 1, wie sie aus der DE 83 27 414 Ul vorbekannt ist.
Zum Stand der Technik wird weiter auf die DE 35 41 478 AI, die DE 23 55 959 B2 und DE 36 08 435 C2 hingewie¬ sen.
Bei der eingangs genannten Herzklappenprotheεe sind die Segel des biologischen Herzklappenpräparats an einen wellenförmig ausgebildeten Kunststoffträger angesetzt. Der Ort, an dem die Kommisεuren der Klappensegel folgt daher nicht der Weitung bzw. der Verengung der Aorta radial .
Der Erfindung liegt damit die Aufgabe zugrunde, eine Herzklappenprothese der eingangs genannten Art zu schaffen, bei der der Ort, an dem die Kommissuren der Klappensegel verankert sind, sich bei der Weitung bzw. Verengung der Aorta den jeweiligen Druckverhältnissen entsprechend radial bewegen kann.
Erfindungsgemäß wird diese Aufgabe durch die im kenn¬ zeichnenden Teil des Anspruchs 1 angegebenen Merkmale gelöst. Die Unteransprüche geben vorteilhafte Ausge¬ staltungen der Erfindung an.
Die Erfindung wird im folgenden anhand einer Zeichnung erläutert. Dabei zeigt:
Fig. 1 eine perspektivische Darstellung einer solchen Herzklappenprothese, und
Fig. 2 eine MittelSchnittdarstellung durch eine solche Herzklappenprothese, wobei links einer der die Kommissurenelemente bildenden Filz¬ streifen geschnitten erkennbar ist.
Die Herzklappenprothese besteht aus einem Stützring 10 mit über den Umfang im wesentlichen gleichbleibender Höhe. Außen um den Stützring herum ist ein Nahtring 16 angeordnet.
Auf dem Stützring 10 stehen im wesentlichen achsenpar¬ allel ausgerichtete Filzstreifen 12 auf, die mit dem Stützring 10 nicht starr, sondern frei aufstehend ver¬ bunden sind.
In den Stützring 10 ist ein biologisches Herzklappen¬ präparat - vorzugsweise vom Schwein gewonnen - an deren Aortenwandung 14 ' unterhalb und oberhalb der Veranke¬ rung X an den Stützring angenäht, während die Kommissu- ren 14*' des biologischen Herzklappenpräparats an den nachgiebigen Filzstreifen 12 angenäht sind.
Bei der Implantation der hier vorgeschlagenen Herzklap¬ penprothese wird der fest mit dem Stützring 10 verbun¬ dene Nahtring 16 an die Aortenwandung des Patienten herznah angenäht. Die freien, von dem Stützring 10 weg¬ weisenden Enden der Filzstreifen 12 werden an die Aor¬ tenwandung des Patienten angeheftet.
Aufgrund der Nachgiebigkeit der die Kommissurelemente bildenden Filzstreifen 12 verhält sich die Herzklappen¬ prothese nach der Erfindung etwa wie die natürliche menschliche Herzklappe: Der Ort, an dem die Kommissuren der Klappensegel verankert sind, bewegt sich bei der Weitung bzw. Verengung der Aorta radial den jeweiligen Druckverhältniεsen entsprechend, was zur zuverlässigen Funktion der Klappensegel beiträgt. Der erreichbare Strömungεquerεchnitt iεt größer, da bei einer Weitung der Aorta aufgrund deε Auεεtoßdrucks sich die Veranke¬ rungspunkte der Kommissuren entsprechend nach außen be¬ wegen. Es wird so - wie bei der natürlichen Klappe - ein divergenter Fluß bewirkt, da, anders als bei dem Stand der Technik - keine starren Kommissurelemente vorhanden sind. Die sich bei dem Stand der Technik hin¬ ter den Kommissurelernenten bildenden Toträume werden vermieden.
Claims
1. Herzklappenprothese zum Ersatz der Aortenklappe oder Pulmonalklappe mit einem aus Kunststoff oder Me¬ tall bestehenden Stützring (10), drei Kommissurelemen- ten (12) und drei durch ein biologisches Herzklappen¬ präparat gebildeten Segeln (14), deren im Stützring (10) angeordnete Aortenwandung (14') mit dem Stützring (10) und deren Kommissurenwand (14'') an den Rändern der Kommissurelemente (12) angenäht sind,
dadurch gekennzeichnet, daß
die Kommisεurelemente (12) durch auf dem Stützring (10) achsparallel ausgerichtete und frei aufstehende Strei¬ fen aus Filz gebildet sind, die an die Aortenwand deε Patienten anheftbar sind.
2. Herzklappenprothese nach Anspruch 1, dadurch ge¬ kennzeichnet, daß die Aortenwandung (14') der Segel (14) unterhalb und oberhalb der Verankerung (X) der Se¬ gel (14) an der Aortenwandung (14') des am Stützring (10) angenäht sind.
