De vanskeligheder, man støder på ved udvikling af fremgangsmåder og præparater, der kontinuerligt frigiver farmaceutiske præparater på ensartet måde over et længere tidsrum, er kendte.
5 På området kontrol med frigivelse af lægemidler er der blevet offentliggjort nylige udviklinger, som beskriver fremgangsmåder til kontrol af frigivelsen af lægemidler ved mikroindkapsling og indeslutning inden i en biosønderdelelig grundmasse.
10 Den foreliggende opfindelse tilvejebringer et hidtil ukendt præparat til parenteral indgivelse af en praktisk taget ensartet og kontinuerlig mængde af et peptid over et længere tidsrum, hvilket præparat er ejendommeligt ved, at det indeholder et komprimeret og delvist overtrukket fedtsyls resalt af et peptid, som udviser en vis grad af opløselighed i vandigt fysiologisk miljø, at overtrækket er biosønder-deleligt eller ikke-biosønderdeleligt, at fedtsyresaltet er udvalgt fra en fedtsyre med 10-20 carbonatomer, og at pep-tidet er 20 His-D-Trp-Ala-Trp-D-Phe-Lys-NI^.
Overraskende nok har det for de delvist overtrukne implanteringer af komprimerede C^Q-C2Q-fedtsyresalte af peptidet 25 His-D-Trp-Ala-Trp-D-Phe-Lys-NH2 almindeligvis vist sig, at opløsningshastigheden falder ved forøgelse af carbonindholdet i den fedtsyre, der anvendes til fremstilling af saltet, medens dette ikke er tilfældet, hvis saltene er uovertrukne.
30 Denne forskel er illustreret på tegningen, hvor figur 1 er en kurve over opløsningen af uovertrukne, komprimerede C^Q-CjQ-fedtsyre-peptidsalte , figur 2 er en kurve over opløsningen af komprimerede og delvist overtrukne C^Q-C20“fedtsyre-peptidsalte, og 35 figur 3 er en kurve, der viser virkningen af for skellige overtræk på opløsningsprofilen af tristearatsaltet af peptidet.

En fordel ved præparatet ifølge den foreliggende 5 opfindelse er, at saltet kan afprøves, så snart opløsningskarakteristika af det komprimerede og delvis overtrukne fedtsyresalt af peptidet er blevet bestemt, hvilket tilvejebringer nøjagtig kontrol af doserne, idet det rene, komprimerede salt foreligger som et implantat. Præparatet ifølge den foreliggende opfindelse kan indeholde op til ca. 50 vægt% af et fortyndingsmiddel eller blanding af fortyndingsmidler og op til ca. 5 vægt% 15 af et smøremiddel eller en blanding af smøremidler. Egnede fortyndingsmidler til den foreliggende opfindelse er ethyl-cellulose og ricinusvoks. Et egnet smøremiddel til den foreliggende opfindelse er magnesiumstearat.
Udtrykket "vandigt fysiologisk miljø" betyder 20 legemet af et varmblodet dyr samt et miljø in vitro, som kan efterlignes ved hjælp af vandige væsker, såsom phosphat-pufferopløsninger ved en temperatur på 35-40°C.
Den foreliggende opfindelse angår endvidere en fremgangsmåde til fremstilling af det her omhandlede præparat i 25 form af et implantat, hvilken fremgangsmåde er ejendommelig ved, at man komprimerer et fedtsyresalt af peptidet, som udviser en vis grad af opløselighed i vandigt fysiologisk miljø, i form af et implantat, overtrækker implantatet med et overtræksmateriale og fjerner en del af overtræksmateria-30 let fra det overtrukne implantat til blotlæggelse af peptidet, hvorhos overtrækket er biosønderdeleligt eller ikke-biosønderdeleligt, fedtsyren er udvalgt fra en Cio-c20-fe<*t_ syre, og peptidet er

His-D-Trp-Ala-Trp-D-Phe-Lys-NH2 i et organisk opløsningsmiddel, såsom methanol, med en tilstrækkelig mængde af den ønskede fedtsyre til tilvejebringelse af fuldstændig saltdannelse. Derefter kan saltet iso-5 leres ved fjernelse af opløsningsmidlet, vask og tørring.
Det isolerede, rene salt sammenpresses ved komprimering eller ekstrusion til et implantat, fortrinsvis et cyclindrisk implantat. Det komprimerede materiale overtrækkes med et biosønderdeleligt eller ikke-biosønderdeleligt overtræk ved 10 konventionel teknik. Forud for implantation i et dyr fjernes et specifikt område af overtrækket til blotlæggelse af det komprimerede salt. Overtrækket kan f.eks. skæres af i den ene ende eller begge ender af det cylindriske implantat.
Det har ved en in vivo undersøgelse med stude vist 15 sig, at de ønskelige opløsningskarakteristika af de her omhandlede præparater bibeholdes, og at frigivelsesprofilerne for frigivelse in vivo og opløsning in vitro er sammenlignelige, hvilket viser effektiviteten af de her omhandlede præparater til parenteral indgift af et fedtsyresalt af pep-20 tidet. I et kontrolleret forsøg frigiver et implantat af tristearatsaltet af peptidet gennemsnitligt 1,8% af den mængde peptid, der er til stede i implantatet, in vivo pr. dag sammenlignet med 1,6% pr. dag in vitro.

Fremstilling af tristearatsaltet af His-D-Trp-Ala-Trp-D--Phe-Lys-NH^ 1,025 g (3,6 mmol) stearinsyre sættes til en op-5 løsning af 1,0 g (0,9 mmol) af peptidet
His-D-Trp-Ala-Trp-D-Phe-Lys-NH2 i 15 ml methanol og den resulterende opløsning opvarmes 10 i et varmtvandsbad i 3 timer. Afdampning af methanolet under formindsket tryk frembringer et hvidt, pulveragtigt produkt, som vaskes med 2440 ml ether, filtreres og tørres til opnåelse af 0,509 g (74,7%) af det ønskede tristearat-salt med følgende elementæranalyse for C99Hig5Ni2°151 15 Beregnet: C: 67,43%, H: 9,43%, N: 9,53%.

Fremstilling af delvist overtrukne komprimerede implantater af txi-C^q-C^Q-fedtsyresalte af et peptid
Implantater fremstilles ved afvejning af en 5 tilstrækkelig mængde af det ønskede C1Q-C20-trifedtsyre-salt af peptidet
His-D-Trp-Ala-Trp-D-Phe-Lys-NH2 10 til tilvejebringelse af ca. 200 mg af peptidet som tri-acetatækvivalentet. . Derefter sammenpresses saltet på 2 forskærerpresse ved 70-350 kg/cm i en cylindrisk matrice med en diameter på 4,76 mm. Mindre implantater (der tilvejebringer peptidtriacetatækvivalenter på ca. 53 mg) 15 fremstilles ved sammenpresning af en hensigtsmæssig mængde CiQ-^Q-trifedtsyresalt på en roterende tabletpresse under anvendelse af en punsel med en diameter på 3,18 mm og en matrice til opnåelse af cylindriske implantater.
De implantater, der fremstilles som ovenfor, 20 overtrækkes med både biosønderdelelige og ikke-biosønder-delelige overtræk ved hjælp af nedennævnte metoder A og B.
Metode A
Ikke-biosønderdelelig siliconepolymer 25 10 dele ren siliconeelastomer blandes med 1 del hærdningsmiddel på et urglas med en spatel. Denne blanding afluftes i en dessikator i 30 minutter. Man griber implantaterne i enderne ved hjælp af pincetter, de rulles ind i siliconepolymeren, anbringes på enden på aluminium-30 folie og hærdes ved 40°C i 5 timer. Den ene ende eller begge ender fjernes med et barberblad, hvilket efterlader "søjlen" af cylinderen overtrukket.
Alternativt kan implantaterne overtrækkes ved neddypning med 20-40% af et medicinsk klæbemiddel, 35 solgt under handelsnavnet "SILASTIC " ® af Dow Corning, 7 som er blevet dispergeret i hexan, og tørres og hærdes ved 40-50°C natten over før fjernelse af overtrækket fra den ene ende eller begge basisender.

Hvert implantat gribes i begge ender med en pincet, dyppes i polymeropløsningen, og derefter af-10 dampes chloroforraet ved stuetemperatur. Hvert implantat overtrækkes 2 gange. Efter tørring af overtrækkene natten over ved stuetemperatur fjernes polymerenderne med et barberblad, hvilket efterlader den lange cylindriske "søjle" overtrukket.

Eksempel 21 Opløsningsforsøg
Opløsningsforsøg udføres ved 39°C under anvendelse af kolbeomrystningsmetoden. En phosphatpufferet fysiologisk 5 saltopløsning (PBS) indstillet på pH-værdi 7,1 er opløs-ningsmsdiet (3,45 g Nal^PO^I^O, 3,55 g , 9,50 g NaCl opløst i destilleret vand til 1000 ml).
Implantatet anbringes i et engangspolypropylenglas med flad bund (Sarstedt nr. 58.537 med hætte nr.

67.790), og 30 ml PBS tilsættes. Glassene anbringes ved 39 C
i et "Fisher ® Shaking Water Bath" (Model 129 - omrystningsindstilling på 2 3/4). PBS udskiftes dagligt, og prøver bestemmes ved måling af optisk tæthed ved 210 nm til opnåelse af opløsningsprofilerne.

Resultaterne af disse forsøg en opsummeret i tegningens figurer 1, 2 og 3. Figur 1 viser opløsningen af uovertrukne, komprimerede implantater af ciq-C20--fedtsyresalte af peptidet 20
His-D-Trp-Ala-Trp-D-Phe-Lys-NE^ i vandigt fysiologisk miljø ved 39°C i 10-48 dage. Opløsningskinetik og -hastighed ændres ved overtrækning af det cylindriske implantat og blotlæggelse af den ene 25 ende eller begge ender. Figur 2 viser, at for mange af implantaterne er fuldstændig opløsning ikke opnået efter 100 dage. Anvendelse af forskellige C10-C20 -fedtsyresalte sammen med forskellige overtræk på de komprimerede implantater tilvejebringe en måde til regu-30 lering af frigivelseshastigheden af peptidet over et forlænget tidsrum..Figur 3 viser variationerne i frigivelses-profilerne opnået med tristearatsaltet af peptidet
His-D-Trp-Ala-Trp-D-Phe-Lys-NH0 35 Δ i DK 167140 B1 12 opnået med nogle forskellige biosønderdelelige og ikke-bio-sønderdelelige overtræksmaterialer. I fig. 3 er A en ikke-biosønderdelelig siliconepolymer, B er biosønderdelelig (L-lactid/glycolid, 57/43), c er biosønderdelelig (L-lactid/gly-5 colid, 71,6/28,4), D er biosønderdelelig (poly-L-lactid), og E er biosønderdelelig (tetramethylencarbonat/glycolid, 50/50).
Eksempel 22 10 Qpløsninqskarakteristika for delvis overtrukne. komprimerede implantater af tristearatsaltet af His-D-Trp-AlarTrp-D-Phe-Lvs-NHo. når det indgives til en stud fin vivo^ I. Fremstilling af H3-mærket peptidtristearatsalt 15 - 17,2 mg (0,0158 mmol) H3-mærket (0,24 mci/mg) pep-tidtriacetat og 413 mg (0,378 mmol) udmærket peptidtriacetat opløses i 45 ml destilleret vand i et centrifugeglas ("Falcon® nr. 2070" - plast). Ammoniumhydroxid (30% NH3) 20 tilsættes dråbevist, indtil blandingen er basisk (pH-værdi 10). Hvide partikler udfældes omgående. Efter 10 minutter anbringes glasset i et bæger med isvand og får lov til at stå i 20 minutter. Glassene centrifugeres i 15 minutter (ca. 2600 omdrejninger/min, "Damon-IEC Clinical Centrifuge 25 Model CL" med rotor model nr. 801). Vandet dekanteres over i et andet centrifugeglas, og der tilsættes 3 dråber ammoniumhydroxid. Efter 5 minutter anbringes glasset i et bæger med isvand og får lov at stå i 20 minutter. De 3 glas centrigueres som ovenfor. Vandlaget dekanteres over i en ^ 30 liters plastflaske til frysning, oplagring og recirkulering. Hvert glas med fast centrifugemateriale opløses i 15 ml me-* thanol og anbringes i en 200 ml udvindingskolbe. 1,32 g (4,72 mmol) stearinsyre tilsættes, og kolben anbringes i et 600 ml bæger med varmt vand. Denne methanolopløsning omrøres 35 indtil opløsning og derefter i yderligere 3 timer. Produktet koncentreres i vakuum til opnåelse af et hvidt faststof. Produktet vaskes 2 gange med ether, hver gang med 40 ml, DK 167140 B1 13 filtreres og tørres i en dessikator over "Drierite" natten over til opnåelse af 662,3 mg (4,49 mCi) .
II. Fabrikation af implantat 5 En forskærerpresse (Model M, 25 ton presse) anvendes til fabrikation af implantatet. De H3-mærkede salte til opløsningsimplantater skæres med koldt tristearat (1:3 varmt til koldt) og blandes grundigt med en morter og støder før sammenpresning, medens dyreimplantaterne fremstilles direkte 10 ud fra H3-salte. To tristearatsalt-implantater (ét til opløsning, ét til dyreimplantation) sammenpresses ved 350 kg/cm2 i en matrice med en diameter på 4,76 mm til opnåelse af cylindre.
15 III. Implantatovertræknincr
Et biosønderdeleligt polymerovertræk påføres ved opløsning af poly-(glycolsyre/mælkesyre)-copolymer (1 g, L-lactid/glycolid-forhold på 71,6/28,4, logaritmisk viskositetstal 0,51) i 3 ml chloroform. Hver af implantaterne gribes i begge 20 ender med pincetter og dyppes i polymeropløsningen, derefter afdampe s chloro formet ved stuetemperatur i et stinkskab.
Hvert implantat overtrækkes 2 gange og tørres natten over ved stuetemperatur, polymerenderne fjernes med et barberblad, hvilket efterlader den lange cylindriske 25 "søjle" overtrukket.
IV. Indgift af peptidimplantat og opførsel in vivo
Under lokal bedøvelse foretages et lille snit på øreflippen på en stud, der vejer 340 kg. En lomme
• O
30 laves subkutant, og det H -mærkede tristearatimplantat anbringes med pincet. Snittet sys sammen for at forhindre at implantatet kommer ud. Studen holdes i et kvæg--metabolisme-bur. Den får op til ca. 4,50 kg kvægvækst-ration nr. 632 pr. dag. Hø og vand'gives ad libitum.
35 Den samlede mængde urin og fæces opsamles dagligt, volumen og vægt måles, og repræsentative prøver sendes til DK 167140 B1 14 ' bestemmelse af radioaktivitet. Blodprøver tages med mellem rum til undersøgelse af peptidblodniveauer.
Beregninger af peptidfrigivelse in vivo er baseret på den daglige, samlede mængde af radioaktivitet, der fore-5 kommer i urin og fæces.
V. Sammenlignelig opløsning in vitro
Opløsningsforsøg udføres ved 39°C under anvendelse af kolbeomrystningsmetoden. En phosphatpufferet fysio-10 logisk saltopløsning (PBS) indstillet på pH-værdi 7,1 er opløsningsmediet (3,45 g Nal^PO^'I^O, 3,55 g NajHPO^ og 9,50 g NaCl opløst i destilleret vand til 1000 ml).
Implantatet anbringes i et engangspolypropylenglas med flad bund (Sarstedt nr. 58.537 med hætte nr.
15 65.790), og 30 ml PBS tilsættes. Glassene anbringes ved 39°C
i et "Fisher ® Shaking Water Bath" (Model 129 - omrystningsindstilling på 2 3/4).
PBS udskiftes dagligt, og prøver undersøges for peptidfrigivelse ved optælling under anvendelse af en 20 scintillationstæller. Under forsøget skal nogle af prøverne fortyndes 1 til 10 med PBS før optælling.
VI. Resultater oa diskussion
Tabel VI opsummerer data in vitro og in vivo fra 25 mærket tristearat hos stude. I frigivelsesprofilen in vitro frigiver implantatet 1,6-2,5 mg/dag efter den voldsomme indledning, indtil pGLA-(copolymer-L-lactid/glycolid)-overtrækket begynder at miste integritet efter 3 uger. Frigivelsessystemet in vivo, beregnet ud fra den daglige udskillelse 30 af radiomærket materiale, ligner in vitro-profilen. Implantatet in vivo frigiver gennemsnitligt 3,2 mg/dag i løbet af de 43 dage, forsøget varer. Peptidet findes i et stigende niveau i studens blod med tiden.

Claims (4)
1. Præparat til parenteral indgivelse af en praktisk taget ensartet og kontinuerlig mængde af et peptid over et længere tidsrum, kendetegnet ved, at det indehol- 5 der et komprimeret og delvist overtrukket fedtsyresalt af peptidet, som udviser en vis grad af opløselighed i vandigt fysiologisk miljø, at overtrækket er biosønderdeleligt eller ikke-biosønderdeleligt, at fedtsyresaltet er udvalgt fra en fedtsyre med 10-20 carbonatomer, og at peptidet er 10 His-D-Trp-Ala-Trp-D-Phe-Lys-NH2.
2. Præparat ifølge krav 1, kendetegnet ved, at saltet er tristearat.
3. Fremgangsmåde til fremstilling af et implantat til parenteral indgivelse af en praktisk taget ensartet og kon- 15 tinuerlig mængde af et peptid over et længere tidsrum, kendetegnet ved, at man komprimerer et fedtsyresalt af peptidet, som udviser en vis grad af opløselighed i vandigt fysiologisk miljø, i form af et implantat, overtrækker implantatet med et overtræksmateriale og fjerner 20 en del af overtræksmaterialet fra det overtrukne implantat til blotlæggelse af peptidet, hvorhos overtrækket er biosønderdeleligt eller ikke-biosønderdeleligt, fedtsyren er udvalgt fra en C^o-c2 o“fedtsYre/ °9 peptidet er His-D-Trp-Ala-Trp-D-Phe-Lys-NH2· 25
