Description

- Die aus Kunststoffen, wie Polymethacrylsäuremethylester und insbesondere aus Polyurethan, gefertigten Endoprothesen zeigen eine gute Gewebefreundlichkeit. Die Gelenkprothesen werden von dem tragenden Knochenmassiv auch auf die Dauer ohne Abstoßungszeichen fest gehaltert. Aber es hat sich gezeigt, daß bei entsprechend stark belasteten Gelenken, z. B. dem Hüftgelenk, der künstliche Gelenkkopf so erheblichen Abreibungserscheinungen unterliegt, daß nach wenigen Jahren wegen Verschleißes der künstliche Gelenkkopf entfernt werden und durch einen neuen ersetzt werden muß.
- Dadurch wird der große Vorteil der Kunststoffendoprothesen gegenüber den Metallkappen, daß sie gegenüber dem Knochenmassiv keine Fremdkörpererscheinungen bewirken, wieder durch einen schweren Nachteil aufgehoben.
- Erfindungsgemäß wird dieser Nachteil dadurch überwunden, daß man die Kunststoffprothese, gleichgültig, ob aus Kohlenwasserstoffabkömmlingen, Diisocyanaten oder Polyamiden aufgebaut hinsichtlich der dem Verschleiß durch Abreib.ung unterworfenen Gelenkfläche armiert mit einem kräftigen Überzug aus alloplastisch geeignetem Metall, z. B. Chrom-Molybdän-Wolfram, oder mineralischen Stoffen, z. B. reinem Quarz oder Porzellan, also einem harten, abriebfesten Material mit halballoplastischen Eigenschaften. Unter solchen Eigenschaften ist zu verstehen, daß zwar keine Verankerung im Knochengewebe eintritt, sondern eher eine allmähliche Abstoßung, daß aber im Gelenkspalt hinreichende Gewebefreundlichkeit vorliegt, so daß keine Reizerscheinungen eintreten. Eine solche aus zwei verschiedenen .Stoffen aufgebaute Endoprothese des Gelenkes, z. B. des Hüftgelenkes oder des Kniegelenkes, vereinigt in sich die beiden für diesen Zweck erwünschten Eigenschaften der alloplastischen Verträglichkeit mit dem tragenden Knochengewebe und dadurch solide Verankerungsmöglichkeit darin einerseits mit der mechanischen Abriebfestigkeit des halballoplastischen, aus metallischen oder mineralischen Stoffen aufgebauten Oberflächenteiles des Gelenkkopfes (oder Pfanne usw.) andererseits.
- In der Zeichnung ist dieser Erfindungsgegenstand in den Fig. I bis 5 dargestellt.
- Fig. I und 2 zeigen eine Femurkopfendoprothese in der Seitenansicht und im Schnitt. a ist die Oberflächenarmierung mittels einer Kappe aus einem korrosionsfreiem Metall, z. B. einer Chrom-Wolfram-Molybdän-Legierung. Im Zenith der Konkavität ist der nagelförmige Stützstab b bei c angeschweißt oder genietet, dessen freies Ende d schlank spitz ausläuft. Die Kappe überschreitet zweckmäßig eine Halbkugel um etwa 5 bis 15 und ist dann bei e nach innen eingekrümmt, wodurch sie sich in die Kunststoffmasse des Prothesenkopfes f eingräbt und so mit dieser mechanisch verbunden. ist. g ist der Prothesenschaft mit der Verankerungsspitze k. Der Kappenrand des Kunststoffkopfes f kann gegen Drehung sichernde Zapfen i tragen.
- Fig. 3 zeigt eine Tibiakopfprothese (Kniegelenk).
- Die Metallplatte a ist hier mittels des eingekrümmten Randes e mit der Kunststoffgrundlage f des Kopfes der Prothese mechanisch verbunden. g ist der (schräg gestellte) Prothesenschaft mit der Verankerungsspitze k.
- Fig. 4 zeigt eine Femurkondylenprothese. Hier wird die Fixierung der Metallarmierung an der eigentlichen Kunststoffprothese dadurch bewerkstelligt, daß die Metallplatte a mehr oder minder zahlreiche Löcher von etwa Trichterform besitzt, die. sich bei der Kunststoffanbringung füllen und verankernd wirken. Jedes Loch h enthält nach der Kunststoffanbringung einen kegelförmigen -Zapfen, der diese verankernde Wirkung hat und die Platte a mit der Kunststoffgrundlage f, die gegen den Knochen z. B. wabig aufgelockert ist, fest verbindet. k ist der hier schräg nach oben gestellte Verankerungszapfen.
- Fig. 5 zeigt eine schaftfreie Femurkopfendoprothese.ES ist hier gezeigt, daß man auch hier mittels eines zentral gestellten trichterförmigen Loches h eine mechanische Verbindung zwischen -Metallarmierung und- Kunststoffgrund erreichen kann.
Claims (2)
- PATENTANSPRÜCHE: I. Zweistoff-Endoprothese, gekennzeichnet durch die ganze oder teilweise Armierung der abriebbeanspruchten Gelenkoberfläche der Kunststoffprothese mittels massiver Gleitflächen, z. B. in Kappenform, die aus korrosionsfreiem Metall oder auch aus mineralischen Stoffen, wie Quarz oder Porzellan, aufgebaut sind.
- 2. Zweistoff-Endoprothese nach Anspruch I, dadurch gekennzeichnet, daß die mechanische Verbindung dieser Gleitflächen mit der Kunststoffgrundlage bewirkt wird mittels Randwulstes, umgekrümmten Randes oder aber Kunststoffverzapfung durch konische Löcher in den Gleitflächen, die in Einzahl oder Vielzahl angeordnet sein können, oder auch mittels aufgerauhter Rückflächen der Gleitflächen.
