" Kniegelenkprothese "
Er irt bekannt, bei krankhaften Veränderungen des Kniegelenks, die einn Versteifung zur Folge haben, Kniegelenkprothesen zu implantieren. Die Verbesserung einer solchen Prothese liegt der Neuerung als Aufgabe zugrunde. Sie soll insbesondere alle anatomischen Funktionen unabhängig vom Bandapparat erfüllen. Ferner soll sie von geringem Materialaufwand und einfach zu implantieren sein.
Die Neuerung zur Lösung dieser Aufgabe ist durch einen in den Gelenkkopf des Oberschenkelknochens (Femur) eingesetzten Schiitton gekennzeichnet, der gleitend mit einer in de·' Gelenkkopf des Unterschonkelknochens eingelassenen Platte verbunden ist, wobei die Form des Schlittens im wesentlichen der Krümmung des Gelenkkopfes des Oberschenkelknochens angepaßt ist. Von Vorteil ist es hierbei, den Schlitten aus zwei durch einen Steg miteinander verbundene Kufen zu bilden und mit der Gleitplatte über einen Gleitstift zu verbinden, der in einem an der Gleitplatte befestigten Gl eitzylinder geführt ist. Zur Verbindung weist der Haltesteg zwei Halteösen auf, in welche der Gleitstift mit an seinem oberen Ende angeordneten Drehzapfen greift. Auch die Gleitplatte kann aus zwei über einen Steg miteinander verbundenen Einzelteilen bestehen. Der Verbindungssteg der Gleitplatte trägt dann den Gleitzylinder .

Di Lär.gsnut kann sich nach unter, zu erweitern, so '3eß die Drehbewegung dez Gleitstiftes im Gleitzylinder in Abhängigkeit von der Beinhaltung den nntürlirhen Verhältnissen angepaßt werden kann. Eine nach unten verlaufende Erweiterung der Gleitstiftnut ermöglicht in angewinkelter Haltung des Unterschenkels eine größere Drehbeweglichkeit als bei gestrecktem Unterschenkel. Statt einer Längsnut mit einem darin eingreifenden Stift oder einer Schraube, besteht die Möglichkeit, die Drehbeweglichkeit durch eine Abflachung am Umfang des Gleitstiftes in Verbindung mit einer Abflachung des Innenumfangs des Gleitzylinders zu begrenzen. Auch hierbei sind Unterschiede bei der Drehbewegung in Abhängigkeit von der Beinhaltung möglich, wenn die Abflachungen nach unten zu breiter werden, d.h., wenn die Abflachung am Gleitstift in Richtung auf sein unteres Ende eine Neigung in Richtung auf die Längsachse des Stiftes aufweist. Die Verwendung einer Schraube ermöglicht gleichzeitig eine Begrenzung der axialen Bewegung des Gleitstiftes.
Um einen guten Sitz des Schlittens am Gelenkkopf zu gewährleisten, sind an den Schlittenkufen Verankerungsstifte befestigt, die ggf. zusätzlich mit Haftrillen ausgestattet sind» Auch die dem Gelenkkopf zugewandte Seite des Schlittens kann zusätzlich mit Haftrillen zur Erhöhung der Haftfestigkeit ausgerüstet sein. Um einen Gegendruck beim Einschieben des Gleitstiftes in den Gleitzylinder zu vermeiden, kann eine den Stift vollständig durchsetzende Längs-* bohrung oder wenigstens eine Längsnut vorgesehen werden»

Die der Form der Gelenkköpfe (Condylus-Medialis und Condylus-Lateraliε) angepaßton Schlittenkufen 1 sind auf der Oberfläche einer geteilten Gleitplatte abgestützt. Die beiden Schlittenkufen 1 sind über einen Verbindung?ε ten 11 miteinander verbunden. Der Steo hat eine rrittli re Öffnung, Mo vor. zwei Seiton durch Halteösen 6 für einen ~y lindr 1 r ?hen Gleitstift. 5 eingefaßt ist. An vijT Innenseite der Gleitkufen 1 sind Ankerstifte 7 von keilförmiger Ausgestaltung angeordnet, die zur Verankerung in den Oberschenkelknochen greifen. Zur besseren Haftung sind Haftrillen 7a vorgesehen. Die innere Oberfläche des Schlittens kann zusätzlich mit Haftrililen la versehen sein. Dadurch wird ein höchstmöglichst verschiebefreier Sitz des Schlittens am Oberschenkelknochen gewährleistet. Der Gleitzylinder 5 ist an dem Verbindungssteg der beiden Gleitplattenteile ? befestigt. Zur Begrenzung der Verdrehung des Stiftes um seine Längsachse ist er mit einer Rotationsbegrenzungsnut 9 ausgeführt, j η die eine durch die Zylinderwand hindurchgreifende Begronzungsschraube reicht. Zur Gewährleistung einer naturgemäßen Drehmöglichkeit des Unterschenkels ist die Rotationsbegrenzungsnut 9 nach unten zu sich erweiternd ausgebildet, so daß der Unterschenkel in gestrecktem Zustand keine Drehbewegung um seine Längsachse ausführen kann, während dies bei einer angewinkelten Haltung durch Eingreifen der Schraube 3 in den unteren Teil der Nut 9 möglich ist. Alternativ hierzu besteht die Möglichkeit, wie in Fig. 5 gezeigt, sowohl den Innenumfang des Gleitzy]inders 5 mit einer Abflachung 12 als auch den Gleitstift 3 mit einer Abflachung zu versehen, die sich in Richtung auf das untere Ende hin verbreitern kann. Kit 3a ist die-' den Gleitstift völlig durchsetzende Entlüftungsbohrung bezeichnet.

Die Verschiebung des Drehpunkts senkrecht nach oben erfolgt zwangsläufig durch die unterschiedlichen Krümmungen des Schlittens, die stetig ineinander übergehen. DIo unterschiedlichen Krümmungen ergeben sich aus der For-π dpr, betreffenden Gelenkkopfes.
Zur Verdeutlichung r.ind in unterbrochener Strahlführung die Konturen der beteiligten Schenkelknochen und die Eeinurnri sr.e dargestellt. Hierbei bedeuten 20 den Oberschenkelknochen, 21 den Unterschenkelknochen, 22 den mittleren Gelenkkopf des Ober:rhenkelknochens, 23 den reitliehen Gelenkkopf des °bcrschenkelknochens 20 und 24 das am Unterschenkelknoch^n ansetzende Wadenbein.
Die neuerungsgemäß gestaltete Kniegelenkprothese ist universell einsetzbar, insbesondere bei arthrotischen Veränderungen des Gelenks unter Beteiligung des Handapparats und erfüllt alle anatomischen Funktionen, unabhängig von diesem. Sie benötigt wenig Material und ist einfach zu implantieren.

1. Kniegelenkprothese, dadurch gekennzeichnet, daß auf einer am Gelenkkopf des Unterschenkelknochens befestigbaren Platte (2) ein Schlitten (1) mit gekrümmten Kufen gleitbar angeordnet ist.
2. Kniegelenkprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Schlitten (1) aus zwei durch einen Steg (11) miteinander verbundenen Kufen besteht und mit der Gleitplatte
(2) über einen Gleitstift (3) verbunden ist, der in einem an der Gleitplatte befestigten Gleitzylinder (5) geführt ist.
3. Kniegelenkprothese nach Anspruch 2, dadurch gekennzeichnet, daß der Verbindungssteg (11) Halteösen (6) aufweist, in welche der Gleitstift (3) mit Drehzapfen (4) greift.
4. Kniegelenkprothese nach einem oder mehreren der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß die Gleitplatte
(2) aus zwei über einen Steg (10) miteinander verbundenen Einzelteilen besteht.
5. Kniegelenkprothese nach Anspruch 4, dadurch gekennzeichnet, daß der Gleitzylinder (5) am Verbindungssteg (10) der Gleitplatte (2) befestigt ist.
6. Kniegelenkprothese nach einem oder mehreren der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß der Gleitstift
(3) und/oder der Gleitzylinder (5) mit Mitteln zur Begrenzung einer Drehung des Stiftes um seine Längsachse versehen sind.
7. Kniegelenkprothese nach Anspruch 6, dadurch gekennzeichnet, daß der Gleitstift (3) mit einer Längsnut (9) versehen ist, in welche ein im Gleitzylinder (5) gehaltenes Führungselement greift.

11. Kniegelenkprothese nach Anspruch 10, dadurch gekennzeichnet, daß die Breite der Abflachungeri nach unten zunehinend ausgebildet ist.
12. Kniegelenkprothese nach einem oder mehreren der Ansprüche 1 bis 11, dadurch gekennzeichnet, daß der· Schlitten an seiner der Platte (2) abgewandten Seite mit wenigstens einem Verankerungsstift (7) ausgerüstet ist.
13· Kniogi1 lenkprothese nach Anspruch 12, dadurch gekennzeichnet, daß der Ankerstift (7) mit Haftrillen (7a) ausgestattet ist.
