Description
- Die Erfindung betrifft die Diagnose und die Behandlung des Schlafapnoesyndroms und seine Behandlung durch Elektrostimulation.
- Das Schlafapnoesyndrom (SAS), genauer das obstruktive (und nicht zentrale) Schlafapnoesyndrom (SAOS) ist ein Leiden, dessen Ursprung in der Regel eine Verstopfung der Atemwege ist und das eine gewisse Anzahl von Störungen nach sich ziehen kann, wie z.B. mühselige und unzureichende Atmung, Störungen des Herzrhythmus, Bluthochdruck.
- Verschiedene Behandlungen wurden von der Chirurgie vorgeschlagen, auf medikamentösem Wege oder auch durch Aufrechterhalten eines positiven Drucks in den Atemwegen mittels einer Gesichtsmaske, die während des Schlafs getragen wird.
- Die US-A-5 485 851 (Medtronic) schlägt auch vor, die SAS durch neuromuskuläre elektrische Stimulation zu behandeln. Dieses Dokument beschreibt einen implantierten Impulsgenerator, der durch einen Sensor gesteuert wird, der es ermöglicht, dem Atemrhythmus zu folgen und so das Auftreten einer Apnoe zu entdecken. Wenn eine Apnoe entdeckt wird, liefert der Generator eine Salve von elektrischen Impulsen an eine implantierte Elektrode zur Stimulation der Muskeln, welche die Luftwege des Patienten kontrollieren. Die Vorrichtung analysiert den metabolischen und funktionellen Zustand des Patienten, um die Stimulation nur während der Aktivitätsphasen anzuwenden, in denen ein SAS tatsächlich auftreten kann.
- Das Dokument US-A-5 485 851 stellt den nächsten Stand der Technik dar und offenbart eine aktive implantierbare medizinische Vorrichtung gemäß dem Oberbegriff des Anspruchs 1.
- Eines der Ziele der Erfindung ist es, eine SAS-Therapie durch andersartige Mittel vorzuschlagen, nicht mehr durch neurologische Stimulation der Muskeln, welche die Luftwege des Patienten kontrollieren, sondern, auf für die Erfindung charakteristische Weise, durch eine besondere Stimulation des Myokards im Fall eines entdeckten SAS.
- Die Erfindung lässt sich so besonders gut für die Behandlung des SAS bei Patienten anwenden, die bereits Träger eines Herzschrittmachers sind (weil es nicht notwendig sein wird, eine spezielle Vorrichtung zur Behandlung des SAS zu implantieren). Man kann aber ebenfalls vorsehen, einen Herzschrittmacher gemäß der Erfindung zu implantieren, mit dem einzigen Ziel, das SAS zu behandeln, wenn notwendig; die Technik der Implantierung von Schrittmachern ist in der Tat eine sehr gut beherrschte und verbreitete Technik, und das Einsetzen einer Vorrichtung gemäß der Erfindung benötigt aus der Sicht des Chirurgen keinerlei Änderung des modus operandi, den er gewohnt ist.
- Genauer ist die Vorrichtung der Erfindung eine aktive implantierbare medizinische Vorrichtung, umfassend: Mittel zur Herzstimulation, Mittel zur Messung der Atemaktivität des Patienten; und Mittel zur Bestimmung eines Aktivitätszustands, wobei dieser Zustand abhängig von vorbestimmten Kriterien einen Wert annehmen kann, der für den Schlafzustand des Patienten repräsentativ ist, Analysemittel, um da Auftreten einer Apnoe in Abhängigkeit vom Signal zu bestimmen, das durch die Messmittel geliefert wird, und Mittel zur Steuerung der Stimulationsmittel, die geeignet sind, die selektive Anwendung einer elektrischen Herzstimulation auf den Patienten im Fall der Entdeckung einer Apnoe zu steuern, wobei die Steuermittel nur während der Schlafphasen aktivierbar sind und im gegenteiligen Fall gehemmt sind, um die Behandlung des Schlafapnoesyndroms beim Patienten durch Elektrostimulation zu ermöglichen.
- Gemäß verschiedenen vorteilhaften Merkmalen:
- – sind die Mittel zur Herzstimulation Mittel, die dazu geeignet sind, im Fall des Auftretens einer Apnoe die Schlagfrequenz des Myokards im Verhältnis zum natürlichen Sinusrhythmus des Patienten zu beschleunigen;
- – entdecken die Analysemittel das Auftreten aufeinander folgender Apnoen im Laufe einer Schlafphase und bestimmen das Auftreten eines Schlafapnoesyndroms, wenn die Anzahl der entdeckten Apnoen während eines gegebenen Zeitraums eine vorbestimmte Schwelle überschreitet;
- – analysieren die Mittel zur Bestimmung eines Aktivitätszustands das Signal, das durch die Mittel zur Messung der Atemaktivität des Patienten geliefert wird und/oder durch zusätzliche Messmittel, die von den Mitteln zur Messung der Atemaktivität des Patienten verschieden sind;
- – umfassen die Mittel zur Messung der Atemaktivität des Patienten einen Atemminutenvolumensensor und/oder einen Sensor zur Messung der Sättigung des Bluts mit Sauerstoff.
- Weitere Merkmale der Erfindung werden bei der Lektüre der nachfolgenden detaillierten Beschreibung eines Ausführungsbeispiels offensichtlich werden, mit Bezug auf die einzige Figur, die ein Signal zeigt, das repräsentativ für den Atemrhythmus des Patienten ist, jeweils in Abwesenheit von Störungen und im Moment des Auftretens einer Apnoe.

1 stellt die Entwicklung des Atemrhythmus eines Patienten während des Schlafs dar, dargestellt hier durch die Entwicklung im Laufe der Zeit des Atemminutenvolumensignals (VE-Signal, auch MV-Signal genannt), die ein überwiegend physiologischer Parameter ist, der durch eine Impedanzmessung im Thorax erhalten wird. Aber obwohl das Atemminutenvolumensignal in der Regel am einfachsten für das Verfolgen des Atemrhythmus des Patienten zu verwenden ist, können andere Signale verwendet werden, die von anderen Arten von Sensoren stammen, als Variante oder als Ergänzung des Atemminutenvolumensensors, z.B. ein Sensor zur Messung der Sättigung des Bluts mit Sauerstoff. - Die Messung des Atemminutenvolumens ist für sich wohl bekannt; sie wird zwischen zwei Elektroden durchgeführt, die im Brustraum angeordnet sind, oder zwischen einer Elektrode (z.B. eine Stimulationselektrode, wenn die implantierte Vorrichtung ein Herzschrittmacher ist) und einem Gehäuse der implantierten medizinischen Vorrichtung. Die Impedanz wird durch Abgabe eines konstanten Stroms von einigen Hundert Mikroamper gemessen, mit einer Frequenz von einigen Hertz, typischerweise 8 Hz. Diese Technik wird z.B. durch Bonnet, JL und Koll. beschrieben, Measurement of Minute-Ventilation with Different DDDR Pacemaker Electrode Configurations, PACE, Bd. 21, 98, Teil 1, und sie wird in den Apparaten Chorus RM 7034 von ELA Medical eingesetzt.
- Man kann ausgehend von diesem Signal eine Atemperiode T bestimmen (
1 ), die als die Zeit definiert ist, die zwei Impedanzspitzen trennt. Die Spitzen entsprechen starken Impedanzen, die beim Einatmen erhalten werden (Lungen mit Luft gefüllt) und der Abfall der Impedanz entspricht einer Phase des Ausatmens. -
2 stellt das Atemminutenvolumensignal dar, das bei Patienten entdeckt wird, die unter Schlafapnoe leiden. Diese Patienten haben normale Ausatmungsphasen, weil der Lungendruck ausreicht, um die Verstopfung zu überwinden; im Gegenzug ist die Einatmung anormal, weil sich die Lungen nicht mit Luft füllen können. Wie es in dieser2 dargestellt ist, kann man also eine bedeutende Verlängerung der Atemperiode T nach einem Ausatmen feststellen. - Der erste Schritt besteht darin, eine Schlafapnoe zu diagnostizieren.
- Eine Apnoe wird klassischerweise als eine Atempause definiert, die von einer Dauer von mehr als zehn Sekunden ist; ein Phänomen, welches einfach durch das Verfolgen des Atemminutenvolumensignals zu entdecken ist.
- Darüber hinaus muss diese Pause während einer Schlafphase des Patienten auftreten, weil eine Apnoe im Wachzustand in keinem Fall ihren Ursprung in einem SAS haben kann.
- Um letzteres Kriterium zu respektieren, schlägt die Erfindung vor, eine Unterscheidung zwischen dem Schlaf und Wachen des Patienten vorzusehen, um eine Therapie nur während der Schlafphasen anzuwenden und jede Therapie zu verhindern, wenn die Apnoe während einer Wachphase auftritt, weil sie in diesem Fall normalerweise nicht pathologisch ist.
- Die Schlafphase wird natürlich auf automatische Weise diagnostiziert, sei es ausgehend vom Signal, das durch den Sensor für das Verfolgen des Atemrhythmus des Patienten geliefert wird, sei es durch einen anderen Sensor, z.B. einen Aktivitätssensor, der einen überwiegend physischen Parameter misst, wie z.B. die Beschleunigung, die von einem Sensor gemessen wird, welcher intern im Gehäuse ist.
- EP-A-0 719 568 (ELA Medical) beschreibt insbesondere ein Verfahren zur Bestimmung eines „Aktivitätskriteriums des Sensors", welches es ermöglicht, eine Unterscheidung zwischen den Ruhephasen (nachts oder tagsüber) des Trägers des Geräts und anderen Aktivitätsphasen zu treffen, insbesondere auf der Basis eines Atemminutenvolumensensors.
- EP-A-0 750 920 (ELA Medical) und EP-A-0 770 407 (ELA Medical) beschreiben ihrerseits medizinische Vorrichtungen, welche die kombinierten Informationen eines physiologischen Sensors und eines physischen Sensors verwenden, insbe sondere eines Atemminutenvolumensensors und eines Beschleunigungssensors, um einen Zustand der Aktivität oder der Ruhe des Patienten zu bestimmen.
- Wenn man so eine Apnoe diagnostiziert hat und bestätigt hat, dass diese tatsächlich eine Schlafapnoe ist, fährt man mit der Berechnung des Apnoeindex fort.
- Um dies zu tun, definiert man, wenn dieser Index eine vorbestimmte Schwelle überschreitet, z.B. mehr als zehn Apnoen pro Stunde (diese Anzahl kann eventuell in Abhängigkeit vom Patienten programmierbar sein) die Anwesenheit eines SAS.
- Sobald ein SAS diagnostiziert ist, wird dann eine elektrische Stimulation auf den Patienten angewendet, um die verhängnisvollen Wirkungen des SAS auszugleichen.
- Diese Stimulation ist gemäß der Erfindung eine Herzstimulation, z.B. eine Beschleunigung der Schlaffrequenz des Myokards, um die Wirkungen des SAS auszugleichen. Diese Herzstimulation wird angewendet, sobald ein SAS diagnostiziert ist, unter Erhöhung der Stimulationsfrequenz von einigen Schlägen pro Minute (typischerweise +10 cpm) im Verhältnis zum natürlichen Sinusrhythmus des Patienten.
Claims (7)
- Aktive implantierbare medizinische Vorrichtung, aufweisend: – Stimulationsmittel, – Mittel zur Messung der Atemaktivität des Patienten, und – Mittel zur Bestimmung eines Aktivitätszustands, wobei dieser Zustand abhängig von vorbestimmten Kriterien einen Wert einnehmen kann, der für den Schlafzustand des Patienten repräsentativ ist, – Analysemittel, um das Auftreten einer Apnoe in Abhängigkeit des durch die Messmittel gelieferten Signals zu bestimmen, und – Mittel zur Steuerung der Stimulationsmittel, wobei diese Steuermittel nur während der Schlafphasen aktivierbar sind und im gegenteiligen Fall gehemmt sind, um die Behandlung durch Elektrostimulation des Schlafapnoesyndroms beim Patienten zu ermöglichen, wobei die Vorrichtung dadurch gekennzeichnet ist, dass die Stimulationsmittel Herzstimulationsmittel sind und die Steuermittel angepasst sind, die selektive Anwendung eines elektrischen Herzstimulus auf den Patienten im Fall des Entdeckens einer Apnoe zu steuern.
- Vorrichtung gemäß Anspruch 1, in der die Mittel zur Herzstimulation Mittel sind, die dazu geeignet sind, im Fall des Auftretens einer Apnoe die Herzschlagfrequenz im Vergleich zum natürlichen Sinusrhythmus des Patienten zu beschleunigen.
- Vorrichtung gemäß Anspruch 1, in der die Analysemittel das Auftreten aufeinander folgender Apnoen im Laufe einer Schlafphase entdecken und das Auftreten eines Schlafapnoesyndroms feststellen, wenn die Anzahl der entdeckten Apnoen während einer gegebenen Zeitdauer einen vorbestimmten Schwellenwert überschreitet.
- Vorrichtung gemäß Anspruch 1, in der die Mittel zur Bestimmung eines Aktivitätszustands das von den Mitteln zur Messung der Atemaktivität des Patienten gelieferte Signal analysieren.
- Vorrichtung gemäß Anspruch 1, in der die Mittel zur Bestimmung eines Aktivitätszustands das Signal analysieren, das von zusätzlichen Messmitteln geliefert wird, die von den Mitteln zur Messung der Atemaktivität des Patienten verschieden sind.
- Vorrichtung gemäß Anspruch 1, in der die Mittel zur Messung der Atemaktivität des Patienten einen Atemminutenvolumensensor umfassen.
- Vorrichtung gemäß Anspruch 1, in der die Mittel zur Messung der Atemaktivität des Patienten einen Sensor zur Messung der Sauerstoffsättigung des Blutes umfassen.
