Description

  • Gebiet der Erfindung
  • Diese Erfindung betrifft eine chirurgische Vorrichtung zum Zurückhalten von geflochtenen Nahtmaterialien. Genauer betrifft diese Erfindung eine einstellbare Einwegnahtmaterialrückhaltevorrichtung, die zur Anwendung bei arthroskopischen Eingriffen geeignet ist.
  • Hintergrund der Erfindung
  • Chirurgen müssen oft Sehnen an Knochen und Weichgewebe an Knochen befestigen oder Risse in Sehnen im Rahmen eines arthroskopischen Eingriffes reparieren. Sie bevorzugen dazu, ein Nahtmaterial, wie beispielsweise ein geflochtenes Nahtmaterial oder Filament zu verwenden, da es eine geringe Größe und eine angemessene Festigkeit aufweist. Die Chirurgen befestigen das Nahtmaterial vor Ort, indem sie Knoten mit mehreren Würfen knüpfen. Es ist jedoch oft in Folge eines fehlenden Zuganges und beschränkter Sichtverhältnisse schwierig, Knoten arthroskopisch zu knüpfen. Viele dieser Eingriffe hängen vom extrakorporalen Knoten ab, um diese Schwierigkeiten zu umgehen. Bedauerlicherweise ist das extrakorporale Knoten zeitintensiv und schwierig. Selbst mit dem Aufkommen von Knotenmitnehmern und Nahtmaterialgreifern ist es für die Chirurgen immer noch schwierig, arthroskopische Knoten schnell zu knüpfen. Es wäre somit ein wesentlicher Beitrag zur Technik, eine Vorrichtung bereitzustellen, die chirurgische Knoten bei arthroskopischen chirurgischen Eingriffen ersetzten würde. Vorrichtungen zum Festlegen von Filamenten sind bekannt. Siehe zum Beispiel US 5,391,173 oder US 5,258,015 . Die erstere dient dem Festlegen von Filamentenpaaren. Die letztere dient dem Festlegen von einzelnen Strängen. Die Offenbarung des ersteren US-Patentes bildet die Grundlage für den Oberbegriff von Anspruch 1.
  • Zusammenfassung der Erfindung
  • Wir haben eine einstückige, einstellbare, Einwegnahtmaterialrückhaltevorrichtung, wie in Anspruch 1 definiert, gefunden, die zur Anwendung in der arthroskopischen Chirurgie geeignet ist, um ein Ende eines geflochtenen Nahtmaterials zu sichern.
  • BESCHREIBUNG DER ZEICHNUNGEN
  • 1 ist eine perspektivische Darstellung der Nahtmaterialrückhaltevorrichtung, die in ein geflochtenes Nahtmaterial eingreift.
  • 2 ist eine Vorderansicht der Nahtmaterialrückhaltevorrichtung.
  • DETAILLIERTE BESCHREIBUNG DER ERFINDUNG
  • Unter Bezugnahme auf 1 ist die einstellbare Einwegnahtmaterialrückhaltevorrichtung ein Stumpf mit einem Ende 4 und einer Basis 6. Durchgang 8 verbinden das Ende 4 mit der Basis, der im wesentlichen konzentrisch um die Mittellinie 10 angeordnet ist, die sich von der Mitte der Basis 6 zur Mitte des Endes 4 erstreckt. Das Ende 4 weist wenigstens zwei Schlitze 12 und 14 auf, die sich im allgemeinen von dem Ende zu der Basis erstrecken. Bevorzugterweise wird das Ende 4 vier Schlitze, 12, 14, 16 und 18, aufweisen, die die Finger 20, 22, 24 bzw. 26 definieren, wie in 2 gezeigt. Die Finger 20, 22, 24 und 26 weisen Punkte 20a, 22a, 24a und 26a auf, die ausgebildet sind, um in das geflochtene Nahtmaterial 28 einzugreifen, so dass das Nahtmaterial nicht von dem Ende 4 zur Basis 6 vorgeschoben werden kann, aber von der Basis 6 zum Ende 4 infolge der Flexibilität der Finger 20, 22, 24 und 26 vorgeschoben werden kann. Die Nahtmaterialrückhaltevorrichtung sollte einen Durchgang 8 mit einem ausreichenden Durchmesser aufweisen, um zu erlauben, dass ein geflochtenes Nahtmaterial leicht durch den Durchgang 8 hindurchgeht, der aber klein genug ist, um zu erlauben, dass die flexiblen Finger 20, 22, 24 und 26 in den Zopf 20 eingreift, wenn ein Nahtmaterial im Durchgang zur Basis 6 bewegt wird.
  • Die Rückhaltevorrichtung der Erfindung kann aus einem jeden biokompatiblen Material unter Verwendung üblicher Herstellungsverfahren hergestellt werden. Die Rückhaltevorrichtung kann aus verschiedenen biokompatiblen Metallen, z. B. Titan, rostfreien Stahl und Tantal, und Polymermaterialien zusammengesetzt sein. Bioabsorbierbare Polymermaterialien sind bevorzugt, die umfassen, aber nicht beschränkt sind auf aliphatische Polyester, die ausgewählt sind aus der Gruppe umfassend Homopolymer von epsilon- Caprolacton, Glycolid, Lactid (einschließlich d, l, dl und meso) und para-Dioxanon und Copolymere von wenigstens zweien davon. Bevorzugte nicht-absorbierbare Polymere umfassen, sind aber nicht darauf beschränkt, Materialien, die aus der Gruppe ausgewählt sind, die Nylons, Polyester und Polypropylen umfasst. Man hat von all diesen Materialien gezeigt, dass sie biologisch akzeptabel sind, wenn sie als Nahtmaterial oder als andere implantierbare medizinische Vorrichtungen verwendet werden.
  • Der bevorzugte Weg zum Herstellen der Rückhaltevorrichtung aus Polymermaterialien besteht darin, eine Polymerschmelze in eine geeignet konstruierte Form zu injizieren unter Prozessbedingungen, die üblicherweise beim Spritzgießen derartiger Polymersysteme verwendet werden. Nach dem Abkühlen der Polymerschmelze kann die geformte Rückhaltevorrichtung leicht aus der Form entnommen, untersucht, verpackt und sterilisiert werden.
  • Die Rückhaltevorrichtung kann durch eine jegliche der gut bekannten Sterilisationstechniken sterilisiert werden. Die ausgewählte Technik wird zu einem großen Umfang von dem Material abhängen, das zum Herstellen der Rückhaltevorrichtung verwendet wird. Geeignete Sterilisationstechniken umfassen Hitze- oder Dampfsterilisation, Bestrahlungssterilisation (wie beispielsweise Kobaltbestrahlung oder Elektronenstrahlbestrahlung), Ethylenoxid und andere in der Technik gut bekannte Sterilisationstechniken.

Claims (6)

  1. Eine sterile einstückige, einstellbare Einwegnahtmaterialrückhaltevorrichtung, die zur Anwendung bei arthroskopischer operativer Behandlung geeignet ist, um ein geflochtenes Nahtmaterial zu sichern, wobei die Vorrichtung einen Stumpf mit einem Ende (4) und einer Basis (6) umfasst, wobei ein Durchgang (8) sich von dem Ende (4) zu der Basis (6) erstreckt, das Ende (4) eine Vielzahl von Schlitzen (12, 14, 16, 18) aufweist, die sich durch das Ende (4) zu der Basis (6) erstrecken und wenigstens einen Finger (20, 22, 24, 26) zum Eingreifen in ein Nahtmaterial (28) definieren, und der Finger (20, 22, 24, 26) einen Punkt (20a, 22a, 24a, 26a) zum Eingreifen in das Nahtmaterial (28) aufweist; dadurch gekennzeichnet, dass der wenigstens eine Finger flexibel ist und die Basis einen geschlossenen Ring um den Durchgang aufweist.
  2. Sterile Rückhaltevorrichtung nach Anspruch 1, wobei die Rückhaltevorrichtung vier flexible Finger (20, 22, 24, 26) aufweist.
  3. Sterile Rückhaltevorrichtung nach Anspruch 1 oder 2, wobei die Nahtmaterialrückhaltevorrichtung aus einem biokompatiblen Material hergestellt ist, das ausgewählt ist aus der Gruppe, die Titan, Tantal und Polymermaterialien umfasst.
  4. Sterile Rückhaltevorrichtung nach Anspruch 1 oder 2, wobei die Nahtmaterialrückhaltevorrichtung aus einem bioabsorbierbaren Polymermaterial hergestellt ist, das ausgewählt ist aus der Gruppe von alyphalen Polyestern, Homopolymeren aus e-Caprolacton, Homopolymeren aus Glycolid, Homopolymeren aus Lactid, Homopolymeren aus p-Dioxanon, Copolymeren aus e-Caprolacton, Copolymeren aus Glycolid, Copolymeren aus Lactid und Copolymeren aus p-Dioxanon.
  5. Kombination eines geflochtenen Nahtmaterials (28) und einer sterilen Rückhaltevorrichtung nach einem der vorangehenden Ansprüche, wobei sich das sterile geflochtene Nahtmaterial durch den Durchgang (8) der Rückhaltevorrichtung erstreckt.
  6. Kombination nach Anspruch 5, wobei das Nahtmaterial (28) einen Knoten darin aufweist, um ein Auffasern des Zopfes zu verhindern.
