Description
Die Erfindung betrifft eine Gefäßstütze, die in ein
Blutgefäß eingeführt werden kann, um mit einem Ballon
katheter aufgeweitet zu werden und eine Stenose zu be
seitigen.
Aus DE 295 21 206 U1 ist eine Gefäßstütze bekannt, die
aus mehreren Ringen besteht. Jeder Ring bildet ein im
wesentlichen sinusförmiges Schleifenmuster. Die Ringe
sind durch längslaufende Verbindungselemente miteinan
der verbunden, welche ebenfalls Schleifen bilden. Auf
diese Weise entstehen zwei sich kreuz ende Mäandermu
ster, die eine einheitliche Zellenstruktur bilden, wo
bei beim Ausdehnen der Gefäßstütze das Wachsen der
längslaufenden Mäandermuster das Schrumpfen der umfäng
lichen Mäandermuster kompensiert. Damit wird vermieden,
daß die rohrförmige Gefäßstütze beim Aufweiten einer
Längskontraktion unterliegt und sich partiell gegenüber
der Gefäßwand verschiebt. Damit ein solcher Längenaus
gleich möglich ist, ist es erforderlich, daß die Ver
bindungselemente an den einander zugewandten Enden be
nachbarter Ringe ansetzen. Dadurch wird aber die Beweg
lichkeit der Ringe relativ zueinander eingeschränkt,
insbesondere in bezug auf ein Abknicken der Ringe. Auch
ein Verdrehen benachbarter Ringe relativ zueinander
wird behindert.
Der Erfindung liegt die Aufgabe zugrunde, eine Gefäß
stütze zu schaffen, bei der die Beweglichkeit der aus
Schleifenmustern bestehenden benachbarten Ringe relativ
zueinander verbessert ist und die eine gute Abstützung
der Gefäßwand ermöglicht.
Die Lösung dieser Aufgabe erfolgt erfindungsgemäß mit
den im Patentanspruch 1 angegebenen Merkmalen.
Bei der erfindungsgemäßen Gefäßstütze bestehen die Rin
ge in üblicher Weise aus periodischen Schleifenmustern.
Die Besonderheit besteht in der Art der Verbindung be
nachbarter Ringe. Dadurch, daß die Endpunkte eines Ver
bindungselements um jeweils mindestens die Weite einer
halben Periode des Schleifenmusters voneinander ent
fernt sind, wird erreicht, daß das Verbindungselement
sich jeweils über eine Strecke in Umfangsrichtung er
streckt und somit gewissermaßen eine "lange Leine" bil
den kann. Dabei ist das Verbindungselement zweckmäßi
gerweise schleifenförmig gebogen, um die Beweglichkeit
der benachbarten Ringe relativ zueinander noch zu erhö
hen.
Gemäß einer vorteilhaften Ausgestaltung der Erfindung
setzen die Verbindungselemente nicht an den einander
zugewandten Enden der Ringe an, sondern an den Mittel
abschnitten. Dadurch können sich die Ringe sowohl in
Längsrichtung als auch in Umfangsrichtung frei ver
schieben. Die Verbindungselemente dienen ausschließlich
dazu, die erforderliche Bewegungsfreiheit zwischen den
Ringen zu gewährleisten, sie müssen jedoch nicht zu
sätzlich einen Längenausgleich der Ringe herbeiführen.
Dies liegt daran, daß die Verbindungselemente an den
Ringen jeweils in der Ringmitte angreifen. Bei einer
radialen Aufweitung der Gefäßstütze bleibt die Position
der Ringmitte erhalten, so daß auch die Abstände der
Ringmitten benachbarter Ringe gleich bleiben.
Vorzugsweise sind die Endpunkte der Verbindungselemente
im Ursprungszustand in Umfangsrichtung um annähernd
eine ganze Periode des Schleifenmusters voneinander
entfernt. Hierdurch wird die Beweglichkeit noch weiter
erhöht und die Verbindungselemente greifen jeweils von
außen, d. h. an den einander abgewandten Flanken, an den
Schleifenmustern benachbarter Ringe an und erhalten
dadurch zusätzliche Länge.
Die Verbindungselemente bilden "Federelemente", die die
longitudinale Flexibilität verbessern. Eine Längsver
kürzung der Gefäßprothese bei Aufdehnung wird verhin
dert. Außerdem wird erreicht, daß keine größeren Flä
chen ungestützt bleiben, weil die Verbindungselemente
den Raum zwischen benachbarten Ringen ohne größere
Freiflächen weitgehend abstützen. Dadurch wird der Aus
bildung einer Restenose entgegengewirkt, die sich vor
zugsweise in ungestützten Flächen ausbildet.
Ein weiteres Merkmal der erfindungsgemäßen Gefäßstütze,
das selbständige Bedeutung hat, besteht darin, daß an
den äußeren Ringen der Gefäßstütze die äußeren Biegun
gen der Schleifenmuster verbreiterte Köpfe bilden. Die
se Köpfe haben die Form eines Omega, das eine größere
Materialmenge erfordert als die Schleifen und wegen der
zusätzlichen Biegungen eine größere Steifigkeit hat.
Dadurch wird vermieden, daß die Enden der Gefäßstütze
bei der Aufdehnung stärker gedehnt werden als die mitt
leren Ringe, und daß die Enden in die Gefäßwand einge
drückt werden und dort einen Gefäßreiz ausüben. Ferner
wird durch die Omega-Gestaltung der Enden verhindert,
daß sich diese Enden schon vor der Aufdehnung im ge
crimpten Zustand auf dem Trägerkatheter öffnen, was zu
einem Stentverlust während der Einführung führen würde.
Im folgenden wird unter Bezugnahme auf die Zeichnungen
ein Ausführungsbeispiel der Erfindung näher erläutert.
Es zeigen:
Fig. 1 einen Teil einer Abwicklung der erfindungsgemä
ßen Gefäßstütze und
Fig. 2 eine vergrößerte Detaildarstellung aus Fig. 1.
Die Gefäßstütze besteht aus einer rohrförmigen Struktur
und ist dadurch entstanden, daß aus einem Rohr durch
Ätztechnik, Funkenerosion, Laserschneidtechnik oder
eine andere Form der Materialentnahme Rohrmaterial ent
fernt wurde, so daß die dargestellten Streifen übrig
bleiben. Die rohrförmige Struktur der Gefäßstütze hat
im Ursprungszustand einen Außendurchmesser von etwa 1,5 mm
und eine Länge von etwa 16 mm. Im aufgeweiteten
Stützzustand beträgt der Außendurchmesser 3 bis 5 mm.
Sämtliche Wandteile sind Bestandteil der Rohrwand, aus
der die Gefäßstütze hergestellt wurde, und liegen auf
einem gemeinsamen Zylinder. Die Gefäßstütze besteht
somit aus einem plastisch deformierbaren einstückigen
Teil, das keine Schweiß- oder sonstige Nahtstellen auf
weist.
Die Gefäßstütze weist zahlreiche Ringe 10 auf, die in
axialem Abstand voneinander angeordnet sind. Jeder Ring
10 besteht aus einem streifenförmigen Schleifenmuster,
das hier sinusförmigen Verlauf hat. In den Zeichnungen
ist die Gefäßstütze im Ursprungszustand dargestellt.
Hierbei ist die Weite einer Periode P des Schleifenmu
sters eines Ringes kleiner als die Amplitude A des
Schleifenmusters. Bei einem Ausführungsbeispiel beträgt
die Periode P 0,84 mm und die Amplitude A 1,3 mm. Der
Abstand D zwischen zwei benachbarten Ringen 10 beträgt
0,8 mm.
Benachbarte Ringe 10 sind jeweils durch Verbindungsele
mente 11 untereinander verbunden, die als Federelemente
wirken. Jedes Verbindungselement 11 ist ein Streifen,
dessen Breite etwa zwei Drittel der Streifenbreite der
Ringe 10 beträgt.
Jedes Verbindungselement 11 ist mit einem ersten Ende
11a mit dem einen Ring und mit dem entgegengesetzten
Ende 11b mit dem benachbarten Ring verbunden. Die Enden
11a, 11b haben in Umfangsrichtung einen Abstand, der
einer Periode P entspricht. Die Enden 11a, 11b liegen
jeweils in dem Mittelabschnitt eines Ringes 10 bzw.
10a, genaugenommen auf der Mittellinie 12 dieses Rin
ges. Die Enden eines Verbindungselements 11 nähern sich
jeweils von außen den Streifen des zugehörigen Ringes 10 bzw. 10a
und stoßen senkrecht auf diesen Streifen
zu. Von dem Ende 11a bzw. 11b erstreckt sich ein gera
der Abschnitt 13 in axialer Richtung und in dem Zwi
schenraum zwischen den beiden Ringen sind die beiden
axialen Streifen 13 durch zwei gegensinnige Krümmungen
14, 15 untereinander verbunden. Die gegensinnigen Krüm
mungen 14, 15 erstrecken sich jeweils über mehr als
180°, vorzugsweise über mehr als 270°, und bilden eine
S-förmige Struktur, die die erforderliche Materialre
serve zur Verfügung stellt.
Zwei Verbindungselemente 11, die zwischen denselben
Ringen angeordnet sind, haben gegenseitige Abstände von
zwei Perioden, also Abstände von 2 P.
Bei dem in Fig. 1 dargestellten Ausführungsbeispiel
sind die Schleifenmuster in allen Ringen 10 unterein
ander phasengleich, d. h. ihre Scheitelpunkte liegen
alle auf derselben längslaufenden Linie. Hiervon abwei
chend besteht die Möglichkeit, daß die Schleifenmuster
benachbarter Ringe gegeneinander phasenverschoben sind.
In den beiden endseitigen Ringen 10a bilden die äußeren
Biegungen der Schleifenmuster verbreiterte Köpfe 16.
Dies bedeutet, daß die Biegungen omegaförmig gestaltet
sind. Dadurch wird erreicht, daß die Gefäßstütze an den
Enden eine größere Steifigkeit und Formstabilität hat
als dies ohne derartige Verstärkungen der Fall wäre.
Dadurch wird vermieden, daß die Enden der Gefäßstütze
sich beim Aufweiten des Ballons, der die Gefäßstütze
umgibt, trompetenförmig aufweiten und in die Gefäßwand
eindringen.
Claims (7)
1. Gefäßstütze aus einer rohrförmigen Struktur, mit
mehreren aus periodischen Schleifenmustern beste
henden Ringen (10, 10a) und Verbindungselementen
(11), die jeweils zwei benachbarte Ringe (10, 10a)
verbinden, wobei die rohrförmige Struktur von ei
nem Ursprungszustand aus in einen Stützzustand
radial aufweitbar ist,
dadurch gekennzeichnet,
daß die Verbindungselemente (11) sich zwischen
zwei Endpunkten (11a, 11b) erstrecken, die im Ur
sprungszustand in Umfangsrichtung um mindestens
die Weite einer halben Periode (P) des Schleifen
musters voneinander entfernt sind.
2. Gefäßstütze nach Anspruch 1, dadurch gekennzeich
net, daß die Verbindungselemente (11) von den Mit
telebenen benachbarter Ringe (10, 10a) abgehen.
3. Gefäßstütze nach Anspruch 1 oder 2, dadurch ge
kennzeichnet, daß die Endpunkte (11a, 11b) der Ver
bindungselemente (11) im Ursprungszustand in Um
fangsrichtung um annähernd eine ganze Periode (P)
des Schleifenmusters voneinander entfernt sind.
4. Gefäßstütze nach einem der Ansprüche 1-3, dadurch
gekennzeichnet, daß die Verbindungselemente (11)
zwei zueinander gegensinnige Krümmungen (14, 15)
aufweisen.
5. Gefäßstütze nach Anspruch 4, dadurch gekennzeich
net, daß die Krümmungen (14, 15) nur in dem Bereich
zwischen zwei Ringen (10, 10a) vorhanden sind.
6. Gefäßstütze nach einem der Ansprüche 1-5, dadurch
gekennzeichnet, daß die Verbindungselemente (11),
die dieselben Ringe (10, 10a) verbinden, einen ge
genseitigen Abstand voneinander haben, der die
Länge von mindestens zwei Perioden (P) des Schlei
fenmusters aufweist.
7. Gefäßstütze, insbesondere nach einem der Ansprüche
1-6, dadurch gekennzeichnet, daß an den äußeren
Ringen (10a) die äußeren Biegungen der Schleifen
muster verbreiterte Köpfe (16) bilden.
