Abstract

Ein Hüftgelenkpfannen-System enthält ein halbkugelartiges Hüftpfannenimplantat (2) und mindestens einen Abstandshalter (20), 20', 20''). Vom Rand (8) des Hüftpfannenimplantats (2) geht mindestens eine Nut aus, die in der Wandung (10) des Hüftpfannenimplantats (2) transversal zu dem Rand (8) verläuft und von der Außenseite (4) des Hüftpfannenimplantats (2) zugänglich ist. Der Abstandshalter (20, 20', 20'') weist einen in die Nut passenden Vorsprung (22) auf. Seine Außenseite ist im Bereich des Vorsprungs (22) an die Außenkontur des Hüftpfannenimplantats (2) im Bereich der Nut angepasst.

Description

Die Erfindung betrifft ein Hüftgelenkpfannen-System, das insbesondere zur Rekonstruktion des Hüftpfannenbereichs nach Entfernung einer gelockerten künstlichen Hüftpfanne bei mehr oder minder ausgeprägten Knochendefekten angewendet werden kann.
Es kommt relativ häufig vor, dass sich künstliche Hüftpfannen im Laufe der Zeit lockern. Dann ist ein erneuter chirurgischer Eingriff erforderlich, bei dem in der Regel ein neues Hüftpfan­ nenimplantat eingesetzt wird. Wenn bei dem Patienten im Bereich des Implantats Knochendefekte vorliegen, gibt es verschiedene Möglichkeiten, für einen Halt des neuen Hüftpfannenimplantats zu sorgen. So kann ein größeres Hüftpfannenimplantat als das entfernte eingesetzt werden, und zwar mit oder ohne Knochenze­ ment. Ferner lassen sich die Knochendefekte mit Zement oder Fremdknochen auffüllen. Es werden auch Hüftpfannenimplantate verwendet, die einen fest angebrachten Abstandshalter aus Metall zum Ausfüllen eines Knochendefektes aufweisen. Eine weitere Möglichkeit ist die Verwendung von Abstützschalen, die sich breitflächig im Darm- und Sitzbein abstützen.
All diese herkömmlichen Methoden haben jedoch Nachteile. Bei einer Auffüllung des Defektes mit Zement oder Fremdknochen lässt sich in vielen Fällen nur eine zeitlich begrenzte Behebung der Probleme erreichen. Ferner führt die Verwendung großer Fremd­ knochentransplantate zu einer hohen Versagerquote. Rekonstruk­ tionsprothesen mit einem Abstandshalter aus Metall entsprechen häufig nicht dem individuellen Defekt. Für die Implantation von Abstützschalen ist ein großer operativer Zugangsweg mit ausge­ dehnter Muskelablösung erforderlich.
Es ist Aufgabe der Erfindung, eine patientenfreundliche und ökonomische Möglichkeit zur Rekonstruktion einer künstlichen Hüftpfanne zu schaffen.
Diese Aufgabe wird gelöst durch ein Hüftgelenkpfannen-System mit den Merkmalen des Anspruchs 1. Vorteilhafte Ausgestaltungen der Erfindung ergeben sich aus den Unteransprüchen.
Das erfindungsgemäße Hüftgelenkpfannen-System weist ein halbku­ gelartiges Hüftpfannenimplantat auf, von dessen Rand mindestens eine Nut ausgeht, die in der Wandung des Hüftpfannenimplantats transversal zu dem Rand verläuft und von der Außenseite des Hüftpfannenimplantats zugänglich ist. Ferner ist mindestens ein Abstandshalter vorgesehen, der einen in die Nut passenden Vorsprung aufweist und dessen Außenseite im Bereich des Vor­ sprungs an die Außenkontur des Hüftpfannenimplantats im Bereich der Nut angepasst ist. Bei einer bevorzugten Ausführungsform sind die Nut und der Vorsprung für eine formschlüssige Verbindung eingerichtet, wobei die Nut vorzugsweise als Schwalbenschwanznut gestaltet ist (und der Vorsprung eine darauf abgestimmte Form aufweist).
Der Vorsprung des Abstandshalters lässt sich vom Rand des Hüftpfannenimplantats her in die Nut einschieben. Wenn der Abstandshalter mit dem Hüftpfannenimplantat verbunden ist, bildet er eine Art größerer Vorsprung zum Ausfüllen eines Knochendefekts. Verbleibende Hohlräume zwischen dem Implantat und dem Knochen des Patienten werden von Gewebe durchwachsen oder können gegebenenfalls auf herkömmliche Weise, z. B. mit Knochenzement, ausgefüllt werden; wegen der hohen Qualität des Abstandshalters treten jedoch viel weniger Folgeprobleme auf als bei herkömm­ lichen Methoden.
In einer bevorzugten Ausführungsform der Erfindung sind mehrere Abstandshalter gleicher und/oder unterschiedlicher Form und/oder Größe vorgesehen. In diesem Fall ist das Hüftgelenkpfannen-System ein modulares System, das eine individuelle Anpassung der Prothese während der Operation an die gegebene Situation ermöglicht. Es lässt sich durch Auswahl der auf die vorhandenen Knochendefekte abgestimmten Abstandshalter eine optimale Verankerung und somit eine dauerhafte Rekonstruktion erzielen. Die mit dem Hüftpfannenimplantat verbundenen Abstandshalter können sich an Stellen guter Knochenqualität abstützen. Wenn die verbleibenden Hohlräume in Folge der Auswahl geeigneter Abstands­ halter klein genug sind, ist sogar die Verwendung von Knochenze­ ment oder ähnlichem entbehrlich. Diese Knochendefekte werden von Gewebe durchwachsen (biologische Rekonstruktion).
Vorzugsweise weist das Hüftgelenkpfannen-System eine Sicherungs­ einrichtung auf, um einen mit dem Hüftpfannenimplantat ver­ bundenen Abstandshalter gegen Verschieben zu sichern. So kann die Sicherungseinrichtung eine am Rand des Hüftpfannenimplantats aufschraubbare Platte enthalten, die mit einem Vorsprung einen mit dem Hüftpfannenimplantat verbundenen Abstandshalter überdeckt und vorzugsweise mit dem Abstandshalter verschraubbar ist. Die Größe und Form des Vorsprungs der Platte ist vorzugsweise auf die Endseite des betrachteten Abstandshalters abgestimmt. Zur Anpassung an verschiedene Konfigurationen von mit dem Hüftpfan­ nenimplantat verbundenen Abstandshaltern enthält das System vorzugsweise mehrere Platten, die mit unterschiedlichen Anord­ nungen von Vorsprüngen versehen sind. Es ist vorteilhaft, wenn sich diese Platten über den gesamten Rand des Hüftpfannenimplantats erstrecken, wobei die jeweils vorhandenen Vorsprünge die Abstandshalter abdecken.
Damit mehrere Abstandshalter mit dem Hüftpfannenimplantat verbunden werden können, weist das Hüftpfannenimplantat in bevorzugter Ausführungsform mehrere Nuten auf. Diese Nuten können über die gesamte Außenseite des Hüftpfannenimplantats verteilt sein. In der Regel reicht es jedoch aus, wenn das Hüftpfannen­ implantat in dem vom Sitz- und Schambein des Patienten abgewandt zu positionierenden Bereich mehrere Nuten aufweist, da diese Zone am anfälligsten für Knochendefekte ist.
Das Hüftpfannenimplantat kann mindestens eine durch seine Wandung führende Öffnung oder Bohrung zur Aufnahme eines im Knochen des Patienten zu verankernden Befestigungselements aufweisen, und zwar vorzugsweise eine Bohrung im Zentrum des Hüftpfannenimplan­ tats und mehrere darum verteilte Bohrungen. Letztere sind vorzugsweise an Stellen, die nach der Implantation auf das Sitzbein und auf das Schambein des Patienten ausgerichtet sind.
Mit Hilfe der Bohrungen und durch die Bohrungen geführter Befestigungselemente (z. B. Knochenschrauben) lässt sich eine stabile Verankerung des Hüftpfannenimplantats in der Zone der Kraftübertragung, in welcher in praktisch allen Fällen noch eine suffiziente Knochenqualität vorhanden ist, erzielen. Dies gilt insbesondere für die zentrale Bohrung, aber auch zum Sitzbein und zum Schambein führende Fixierungsschrauben lassen sich in der Regel gut verankern.
Als Material für das Hüftpfannenimplantat und die Abstandshalter kommen z. B. Titan oder Titanlegierungen in Frage. Die Innenseite des Hüftpfannenimplantats kann eine Auskleidung aus einem Polyolefin aufweisen, vorzugsweise aus einem UHMW-Polyethylen (d. h. einem Polyethylenmaterial mit einem besonders hohen Molekulargewicht).
Das erfindungsgemäße Hüftgelenkpfannen-System erlaubt also eine Anpassung an die individuelle Situation des Patienten und ermöglicht kürzere Operationszeiten. Der operative Zugangsweg ist kleiner als bei der Verwendung herkömmlicher Abstützschalen, was eine raschere postoperative Rehabilitation des Patienten zur Folge hat. Insgesamt ergibt sich eine optimale biomechanische Rekonstruktion, verbunden mit einer längeren Haltbarkeit des Implantats.
Im Folgenden wird die Erfindung anhand eines Ausführungsbeispiels näher erläutert. Die Zeichnungen zeigen in

In Fig. 1 ist ein Hüftpfannenimplantat 2 des Hüftgelenkpfannen- Systems im Längsschnitt dargestellt. Die Schnittebene verläuft durch die in Fig. 3 eingezeichnete Achse I-I. Das Hüftpfannenimplantat 2 ist halbkugelartig gestaltet und hat eine Außenseite 4, eine Innenseite 6, einen Rand 8 und eine Wandung 10.
Von dem Rand 8 ausgehend ist in die Wandung 10 eine Nut 12 gefräst, siehe auch Fig. 2. Die Nut 12 ist im Ausführungsbei­ spiel als Schwalbenschwanznut gestaltet, d. h. sie hat einen trapezförmigen Querschnitt. Im Ausführungsbeispiel verläuft die Achse der Nut 12 in der Papierebene der Fig. 1. In Azimutrich­ tung versetzt sind im Ausführungsbeispiel zwei weitere gleich­ artige Nuten vorgesehen. Eine größere Zahl von Nuten, die über die Peripherie des Randes 8 verteilt angeordnet sind, ist jedoch ebenfalls denkbar. Die Nuten dienen zum Befestigen von Abstands­ haltern, wie weiter unten näher erläutert.
In seinem Zentrum weist das Hüftpfannenimplantat 2 eine Öffnung oder Bohrung 14 auf, siehe Fig. 1 und Fig. 3. Im Ausführungs­ beispiel enthält das Hüftpfannenimplantat 2 zwei weitere Bohrungen 16 und 17, siehe Fig. 3. Die Bohrung 14 und die Bohrungen 16 und 17 sind zum Aufnehmen von Befestigungselementen wie z. B. Knochenschrauben bestimmt, mit deren Hilfe das Hüftpfan­ nenimplantat 2 im Knochen eines Patienten verankert wird. Dabei wird das Hüftpfannenimplantat 2 vorzugsweise so eingesetzt, dass die Bohrungen 16 und 17 zum Sitzbein und zum Schambein des Patienten weisen, da die Befestigungselemente in diesen Knochen in der Regel gut halten.
Bei einer Variante dieser Ausführungsform ist eine weitere Bohrung im Bereich des unteren Endes der Nut 12 vorgesehen, also ungefähr auf halbem Wege zwischen dem Rand 8 und der zentralen Bohrung 14.
Im Ausführungsbeispiel ist die Innenseite 6 des Hüftpfannenimp­ lantats 2 mit einer Auskleidung aus UHMW-Polyethylen (Polyethylen mit besonders hohem Molekulargewicht) versehen. Dies ist eine übliche Maßnahme bei Hüftgelenkimplantaten, die die Reibung zwischen dem Hüftpfannenimplantat und dem Gelenkkopf vermindert.
In den Rand 8 des Hüftpfannenimplantats 2 sind vier Gewindeboh­ rungen 18 eingebracht, siehe Fig. 3.
Eine weitere Komponente des Hüftgelenkpfannen-Systems sind Abstandshalter 20, 20' und 20". Vorzugsweise stehen dem Operateur Abstandshalter in unterschiedlicher Form und Größe zur Auswahl, so dass er im Verlauf der Operation ein oder mehrere Abstands­ halter auswählen und mit dem Hüftpfannenimplantat 2 verbinden kann, wie im Folgenden anhand des Abstandshalters 20 erläutert. Die Abstandshalter 20, 20' und 20" dienen zum Ausfüllen von Knochendefekten.
Der Abstandshalter 20 (siehe auch Fig. 4) hat einen Vorsprung 22, der im Ausführungsbeispiel schwalbenschwanzartig gestaltet ist und in die Nut 12 oder eine der anderen Nuten an dem Hüftpfannenimplantat 2 passt. Die Außenseite 24 des Abstands­ halters 20 ist im Bereich des Vorsprungs 22 an die Außenkontur des Hüftpfannenimplantats 2 im Bereich der Nut 12 (bzw. der anderen Nuten) angepasst. Um den Abstandshalter 20 mit dem Hüftpfannenimplantat 2 zu verbinden, wird er vom Rand 8 her mit dem Vorsprung 22 in die Nut 12 eingeschoben. Die Fig. 1 veranschaulicht die endgültige Lage des Abstandshalters 20 an dem Hüftpfannenimplantat 2. Wenn die Tiefe der Nut 12 und die Länge des Vorsprungs 22 aufeinander abgestimmt sind, ist die Stirnseite 26 des Abstandshalters 20 in einer Ebene mit dem Rand 8 ausge­ richtet. Die durch den in die Nut 12 eingreifenden Vorsprung 22 hergestellte Verbindung ist formschlüssig und daher sehr stabil. An der Stirnseite 26 ist der Abstandshalter 20 mit einer Gewindebohrung 28 versehen.
In Fig. 5 ist der Abstandshalter 20' in Draufsicht auf seine Stirnseite 26' dargestellt. Der Abstandshalter 20' hat die gleichen Bestandteile wie der Abstandshalter 20, also einen Vorsprung 22' und eine Außenseite 24', die in ihrer Form dem Vorsprung 22 und der Außenseite 24 des Abstandshalters 20 entsprechen, aber im Übrigen eine andere Grundform, siehe auch Fig. 3. Die Fig. 3 zeigt ferner, dass der Abstandshalter 20" spiegelbildlich zu dem Abstandshalter 20' konstruiert ist.
Außer den in den Figuren dargestellten Abstandshaltern 20, 20' 20" sind viele andere Formen und Größen von Abstandshaltern denkbar. Vorzugsweise hat der Operateur während der Operation eine größere Zahl von Abstandshaltern zur Auswahl, so dass er die geeigneten Abstandshalter individuell und in Anpassung an die Operation mit dem Hüftpfannenimplantat 2 verbinden kann.
Die Fig. 6 zeigt eine Platte 30 in Draufsicht. Die Platte 30 hat drei Vorsprünge 32, 32' und 32" sowie insgesamt sieben durch­ gängige Bohrungen 34. Sie dient als Sicherungseinrichtung, um zu verhindern, dass sich die mit dem Hüftpfannenimplantat 2 verbundenen Abstandshalter 20, 20', 20" verschieben. In ihrer Form ist die Platte 30 auf die Form des Randes 8 und die Form der Stirnseiten der Abstandshalter 20, 20', 20" abgestimmt. Durch die Bohrungen 34 können Schrauben in die Gewindebohrungen 18 sowie 28, 28' und die entsprechende Gewindebohrung an dem Abstands­ halter 20" eingeschraubt werden.
Die Form der Vorsprünge 32, 32' und 32" der in der Fig. 6 gezeigten Platte 30 stimmt mit den Formen der Stirnseiten 26, 26' und der des Abstandshalters 20" überein. Vorzugsweise enthält das Hüftgelenkpfannen-System weitere Platten zur Auswahl, bei denen die Vorsprünge einer anderen Auswahl oder Konfiguration von Abstandshaltern entsprechen.
Ein vorteilhaftes Material für das Hüftpfannenimplantat 2 ist Titan oder eine Titanlegierung. Die Abstandshalter bestehen vorzugsweise aus demselben Material. Auch die Platte 30 kann aus diesem Material gefertigt sein.

Claims (11)

1. Hüftgelenkpfannen-System,
  • - mit einem halbkugelartigen Hüftpfannenimplantat (2), von dessen Rand (8) mindestens eine Nut (12) ausgeht, die in der Wandung (10) des Hüftpfannenimplantats (2) transversal zu dem Rand (8) verläuft und von der Außenseite (4) des Hüftpfannenimplantats (2) zugänglich ist, und
  • - mit mindestens einem Abstandshalter (20), der einen in die Nut (12) passenden Vorsprung (22) aufweist und dessen Außenseite (24) im Bereich des Vorsprungs (22) an die Außenkontur des Hüftpfannenimplantats (2) im Bereich der Nut (12) angepasst ist.
2. Hüftgelenkpfannen-System nach Anspruch 1, dadurch gekenn­ zeichnet, dass die Nut (12) und der Vorsprung (22) für eine formschlüssige Verbindung eingerichtet sind.
3. Hüftgelenkpfannen-System nach Anspruch 2, dadurch gekenn­ zeichnet, dass die Nut (12) als Schwalbenschwanznut gestal­ tet ist.
4. Hüftgelenkpfannen-System nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass mehrere Abstandshalter (20, 20', 20") gleicher und/oder unterschiedlicher Form und/oder Größe vorgesehen sind.
5. Hüftgelenkpfannen-System nach einem der Ansprüche 1 bis 4, gekennzeichnet durch eine Sicherungseinrichtung (30), die dazu eingerichtet ist, einen mit dem Hüftpfannenimplantat (2) verbundenen Abstandshalter (20, 20', 20") gegen Ver­ schieben zu sichern.
6. Hüftgelenkpfannen-System nach Anspruch 5, dadurch gekenn­ zeichnet, dass die Sicherungseinrichtung eine am Rand (8) des Hüftpfannenimplantats (2) aufschraubbare Platte (30) aufweist, die mit einem Vorsprung (32) einen mit dem Hüftpfannenimplantat (2) verbundenen Abstandshalter (20) überdeckt und vorzugsweise mit dem Abstandshalter (20) ver­ schraubbar ist.
7. Hüftgelenkpfannen-System nach Anspruch 6, dadurch gekenn­ zeichnet, dass mehrere Platten (30) mit unterschiedlichen Anordnungen von Vorsprüngen (32, 32', 32") vorgesehen sind, die auf unterschiedliche Konfigurationen von mit dem Hüftpfannenimplantat (2) verbundenen Abstandshaltern (20, 20', 20") abgestimmt sind.
8. Hüftgelenkpfannen-System nach einem der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass das Hüftpfannenimplantat (2) in dem vom Sitz- und Schambein des Patienten abgewandt zu positionierenden Bereich mehrere Nuten (12) aufweist.
9. Hüftgelenkpfannen-System nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass das Hüftpfannenimplantat (2) mindestens eine durch seine Wandung (10) führende Öffnung (14, 16, 17) zur Aufnahme eines im Knochen des Patienten zu verankernden Befestigungselements aufweist, wobei vorzugs­ weise eine Bohrung (14) im Zentrum des Hüftpfannenimplantats (2) und nach der Implantation auf das Sitzbein und auf das Schambein des Patienten ausgerichtete Bohrungen (16, 17) vorgesehen sind.
10. Hüftgelenkpfannen-System nach einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, dass das Hüftpfannenimplantat (2) und/ oder der bzw. die Abstandshalter (20, 20', 20") Titan oder eine Titanlegierung aufweisen.
11. Hüftgelenkpfannen-System nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, dass die Innenseite des Hüftpfannen­ implantats (2) eine Auskleidung aus einem Polyolefin, vorzugsweise aus UHMW-Polyethylen, aufweist.
