Abstract

技术领域
本发明在更广的方面涉及用于掌骨-指骨或者指节间的关节假肢,其这样用于关节造形术中,在不同的关节部件和骨结构的重构(reconstruct)中,用于在重构各肌肉、腱、韧带和其它柔软组织之后再次移动关节。这种假肢在发生损伤事件或者诸如风湿性关节炎的关节变性疾病之后植入。
尤其是,本发明涉及重构掌骨和指骨或者两个指骨之间的关节的的关节假肢,即,基本包括将连接到近侧骨的第一部分或者腿骨,和将连接到远端骨的第二部分或者腿骨,这两个腿骨分别连接到关节接头的半部分,所述两个半部分彼此铰接。
背景技术
手和足关节造形术领域中公知几种关节假肢。
最初,腿骨具有钉或者螺钉形状的端部以插入对应的远和近侧骨中。此后,为了保证更长时间的寿命和一定的可靠性,腿骨-骨组合通过所谓的“固定件”或者插入件的压力连接装置来实现,所述插入件初始插入到掌骨或指骨管以在其中骨接合。
但是,考虑到骨管中只存在有限的面积以插入这些固定件,它们基本上具有简单的锥形形状,并且它们不适合进行可能的修复替换,而关节接头磨损需要这种修复替换。在实际中,为了替换所述关节,通常需要拉出现有的固定件并将其它通常尺寸更大的固定件插入,但这并不是总是可能的。
发明内容
解决本发明的问题是提供一种用于掌骨-指骨或者指节间的关节假肢,能够克服上述参照现有技术中所提及的缺点。
所述问题通过根据本发明的如上所述用于掌骨-指骨或者指骨之间的关节假肢而解决,并且其特征在于至少所述第一腿骨和第二腿骨包括膨胀插入件和可逆地膨胀所述插入件的膨胀装置,所述膨胀装置可移除地连接到所述关节接头的一半部分。
根据本发明的关节假肢的其它特征和优点在掌骨-指骨或者指节间用时在结合附图的下述实施例的详细说明中变得显而易见,所述实施例只是出于说明而不是限制性的示例。

具体实施方式
现在参照附图,根据本发明的用于掌骨-指骨或者指节间的关节假肢在全文中用10指示。
假肢10包括第一延长部分或者腿骨12,以例如连接到近侧骨,此后也作为近侧部分或者腿骨12来指示,以及第二延长部分或者腿骨14,以例如连接到远侧骨,此后其也用远侧部分或者腿骨14来指示。为了指示假肢10的尺寸,可以指出这些腿骨12、14的典型长度是18mm。
在图示中可选地将第一部分12组装到远侧骨上,而将第二部分14组装到近侧骨上,本发明的假肢10与与此后描述的类型总是相同的。
近侧腿骨12和远侧腿骨14通过关节接头16彼此连接,所述关节接头装备两个彼此铰接在一起的半部分13和15。具体精确而言,例如,此关节接头16通过将近侧腿骨12的凸起端连接到远侧腿骨14的凹入端15。关节接头16也包括销18,所述销18例如具有3mm直径,并两个端部13,15彼此铰接。至少一个衬套20(在示例中,衬套20为两个,它们位于凸起端部13的两个相对侧面上)设置在所述两个端部13,15的相对壁之间:此衬套20由低摩擦和较高的耐磨损材料制造,诸如Optima或者超高分子量聚乙烯UHMWPE或者铝。销18具有基准头部68,具有用于将假肢10安置在其中的凹陷70,并稳定适于插入所述销18的工具。与基准头部68相对的端部设有凹平面,所述凹平面适于起压钉器(punch)邻接以拔出销18。
在凸起端部13的相对端部上的近侧部分12可以插入到膨胀插入件22中,所述插入件22是中空的并位于近侧骨的掌骨或者指骨管中。相似地,凹入端15的相对端上的远侧部分14可以插入另外的膨胀插入件,所述插入件与插入件22的类型相同,其是中空并位于远侧骨的掌骨或者指骨管中。
根据本发明的假肢10可以可选地使用单个膨胀插入件22,其中近侧12或者远侧14部分被插入,各相对部分传统上是固定的。
插入件22优选地由钛制造,并且其在骨接触区域用羟磷灰石涂敷,这样其可以与骨形成一体。
具体而言,根据本发明的一方面,此插入件22,在轴线X-X基本具有圆柱形腔24,所述插入件22具有稍微的平截锥外部型面26。锥形的平截头在靠近变宽的基部在两个轴向相对区域28,29变平。与锥形的平截头的较小基部相对应,插入件22被闭合,而更宽的基部打开并用圆锥分叉30以基本圆柱形套环32打开并延展。套环32具有通口34,所述通口34例如是四边形的,优选是矩形的,尺寸大于插入件22的腔24并与所述腔24相连通。
插入件22的外部轮廓26具有轴向槽36。此外,至少在截头锥轮廓26的一定的高度上,尤其使在朝向较小基部的侧面,实现通切口38。切口38根据基本平行于圆柱形腔24的轴X-X并与腔24的圆柱体的基周长相切的切口平面实现。有利地,这些切口38可以是沿着周长等距四个。
如前所述,近侧部分12具有凸起端部13:此端部通过基本平行六面体形状的主体40形成,对应近侧部分12的轴Y-Y而安置:主体40具有孔42,所述孔42的轴线K-K指向垂直于轴Y-Y从其以轴心差d(例如1mm)而偏移:轴线K-K对应接头瞬时旋转中心的平均值安置。
近侧部分12以基本圆柱形差拍套环44延展超过凸起端13。优选的四边形横截面平行六面体形状主体46设置在所述套环44的底部,适于插入插入件22的通口34中。
例如,超出所述平行六面体形状主体46,套筒48与第一腿骨12一体形成:其内部至少在套筒48的自由端的相对侧面上被攻丝,锥形内终部50朝向衬套48的自由端打埋头孔。
通切口52平行于轴线Y-Y,所述切口例如是四个并围绕衬套48等距离分布,切口从自由端至少设置在衬套48的一部分上。
衬套48的内孔显示正交到达主体40的孔42:此外,与其中衬套48的内孔到达孔42的区域相对的直径的侧面上的主体40提供了用于将孔42连接到外部的通口42a。
在示例中,主体40的孔42的各侧上,提供衬套20,所述衬套20是带孔板:其在邻接主体40的侧面上是扁平的,并在相对的侧面上基本具有圆柱表面54,此圆柱的轴线垂直于板孔的轴线,在实际上与孔42的轴线K-K重合。板孔对应近侧部分12的孔42安置。具体而言,在板的扁平侧上,尺寸可以用于插入孔42中的基本圆柱形套环56围绕此孔凸起:孔是内截头锥,并朝向基本圆柱形表面54打埋头孔。此外,如图所示,材料释放装置(material release)优选地设置在基本圆柱形套环56上,用于执行诸如沿着轴线U-U以相对板孔的轴线K-K的轴心差u轴向偏心并通过孔的截头锥更小基部的平面,这显示在图10e中:可以注意到,一旦衬套20组装到孔42中,轴线U-U也平行于近侧部分12的轴线Y-Y,以及邻接与差拍套环(beat collar)44的圆柱的基部的衬套20的侧表面21。
如前所提及,远部14具有凹入端部15:此端部具有一对翼41,所述翼41相对远侧部分14的轴线Z-Z对称安置:各翼41具有孔43,根据垂直于轴线Z-Z的轴线H-H取向并从其以轴心差h轴向偏心:两个翼41的两个孔43彼此相对并且尺寸适于销18,轴心差h通常对应孔42的轴心差d。具体而言,两个孔43的直径不是等同的:孔43允许销18以间隙插入(to be insertedwith backslash),而销18在另外的孔43中过盈配合。
远侧部分14以基本圆柱形差拍套环45超出凹入端15延展。优选矩形横截面平行六面体形状主体47设置在所述套环45的底部,适于插入所述插入件22的通口34中。
例如超出所述平行六面体形状主体47,套筒49与第二腿骨14一体形成:其内部至少在套筒49的自由端的相对侧面上攻丝,锥形内终部51朝向套筒49的自由端打埋头孔。
通切口53平行于轴线Z-Z,所述切口例如是四个并围绕套筒49等距离分布,切口从自由端至少设置在套筒49的一部分上。
套筒49的内孔显示显示到达包括在两个翼41之间的部分14的区域。
用作夹具的型面(profile)58然后在一个侧面上相对近侧部分12的远侧部分14的过度的旋转设置在两个翼41之间。优选地,在远侧部分14和近侧部分12之间保证90°接头。
根据本发明的另外一方面,在各衬套48,49中,为了将近侧12和远侧14部分分别固定在对应的膨胀插入件22中,杆形件60被插入。其具有外部带螺纹端64并在相对端具有反锥锥形腿骨62,即锥形是朝向自由端打埋头孔。杆形件60在端部64的头部也具有螺丝起子切口66或者所述杆形件60的另外的旋转操作装置,所述杆形件60的形状为诸如嵌入六边形形状。端部64的螺纹的尺寸对应衬套48和49的内螺纹:这将在后面更好地观察到,在此假肢10的组装的过程中,可以从主体40的孔42以及从包括在两翼41之间的部分14的区域分别进入此切口66,用于通过螺丝驱动锥形销60而旋转操作,所述销这样就在各衬套48,49中沿着轴线Y-Y和Z-Z移动(在嵌入六角形的情况下,显然使用螺丝扳手)。必须指出基本圆柱形状套环56的材料释放装置用于允许在两个衬套20被安置的主体40上的孔42进入切口66。
总之,各杆形件60分别与套筒48和49形成膨胀装置59,所述套筒48或者49通过所述杆形件48或者49膨胀并当套筒48或者49被插入到插入件22的腔24中时导致插入件22膨胀,所述膨胀是可逆的类型:这些膨胀装置59也被可移除地连接到关节接头16的半部13或者15。具体而言,在示例中,套筒48和49属于近侧12和远侧14部分,其中形成关节接头16的凸起1和凹入15端部分别被形成。
根据本发明的用于掌骨-指骨或者指节间的关节假肢操作将在下面进行说明。
首先,两个膨胀插入件22分别位于近侧和远侧骨的掌骨或者指骨管中。
然后膨胀部件60分别从带螺纹端部64分别插入到各套筒48和49中,以分别将带螺纹端部64的第一螺纹与套筒48和49的内螺纹进行螺纹连接。
分别预先组装到近侧12和远侧14部分的各膨胀部件60的截头锥端62插入各插入件22的腔24中。
通过方便的螺丝起子,各切口66的操作现在分别从主体40的孔42自通口42a启动并从包括在两翼41之间的部分14的区域开始,进行套筒48和49的内螺纹上的带螺纹端64的螺纹连接。这样膨胀部件60分别沿着轴线Y-Y和Z-Z轴向移动,这样锥形腿骨62分别在套筒48和49的内锥终部上操作。通过切口52和53,套筒48和49弹性可变形并且它们压缩腔24的壁。然后操作各套筒48和49的内锥终部50和51的杆形件60的锥形腿骨62如何使得套筒48和49在径向方向中如同反向夹头或者膨胀夹头操作地朝向插入件22弹性变形就是非常明显的。
此套筒48和49相对插入件22的腔24的压缩,增加直到差拍套环44和45分别邻接在插入件22的各套环32上,导致插入件22的外截锥型面26膨胀。所述膨胀通过切口38的存在而增加。但是,这些切口38的特殊布置防止了较大的通端口形成在插入件22的外部和插入件的内部之间,防止骨组织增加插入件22的内部:实际上,打开切口38的外部时,切口38的内部自身闭合。
实际上,在假肢10的移植之后紧随的步骤中,插入件22由于套筒48和49的膨胀的缘故已经具有较好的稳定性:这样允许更快地愈合。最后,插入件22和骨之间产生骨接合:此稳定的紧固对应周向槽36而产生,这由于通过外部型面26的膨胀的结果而变形:总之,在这些槽中发生骨组织生长,从而以更稳定的方式固定所述插入件22。
现在很明显通过在翼41之间插入主体40而实现关节接头16:孔42的轴线K-K对应孔43的轴线H-H,这样销18被安置以稳定的方式将凸起端13连接到凹入端15。
需要记住,在连接步骤中,设置了两个衬套20,其在孔42中安置有圆柱套环56,侧表面21邻接在近侧部分12的差拍套环44的圆柱基部上。
实际上,必须注意通过基本圆柱形表面54,轴线Z-Z相对近侧部分12的轴线Y-Y旋转的远侧部分14能够相对轴线Z-Z的自然旋转平面围绕轴线Y-Y承受小的侧向旋转:例如承受在一侧上5度的侧向旋转,在另外一侧上是5度。
通过上述结构,近侧部分12上的远侧部分14的扭曲也得以有利地防止。实际上,必须首先考虑插入件22在两个相对的区域28和29上变平,这与骨髓解剖学相似;此外,插入到插入件22的套环32的对应的通口24中的平行六面体46和47用作各近侧12和远侧14部分的扭转防止措施。
在诸如由于磨损所导致的损坏有必要进行假肢10的拆装的情况下,分别将两个插入件保留在近侧和远侧骨中来移除和替换受损坏的部件的情况下是有利的。实际上,一旦销18被移除,膨胀部件60逐渐失去螺纹连接:套筒48和49使得腔24的壁上的压缩变松直到近侧12和远侧14部分从各插入件22中分离。
就插入件22而言,必须注意其在前部闭合只是为了防止骨向内生长,这样使得套筒48和49不可移动。
此外,位于套环32之前的插入件22的锥形分叉30防止形成不利的突然骨质吸收。
通过根据本发明的用于掌骨-指骨或者指节间的关节假肢实现的主要的优点是其使得假肢移植时间非常快。
另外的一个优点是在后续对诸如磨损部件的功能性替换时假肢的拆装很容易,而不会使得骨与插入件的整合变松。
普通技术人员可以对上述的用于掌骨-指骨或者指节间的关节假肢进行一些变化,其范围由所附的权利要求限定。
Claims (38)
1.一种用于掌骨-指骨或者指节间的关节假肢(10),包括将连接到近侧骨的第一腿骨(12)以及将连接到远侧骨的第二腿骨(14),所述两个腿骨(12,14)分别连接到关节接头的半部分(13,15),所述两个半部分(13,15)彼此铰接,其特征在于,至少所述第一腿骨(12)和所述第二腿骨(14)之一包括膨胀插入件(22)和可逆地膨胀所述插入件(22)的膨胀装置(59),所述膨胀装置(59)可移除地连接到所述关节接头(16)的半部分。

36.根据权利要求35所述的假肢(10),其特征在于,所述杆形件(60)具有反锥锥形腿骨(62),即朝向自由端打埋头孔,所述杆形件(60)在相对端(64)螺纹连接到锥形腿骨(62)并在头部以所述杆形件(60)的旋转操作装置(66)结束。
37.根据权利要求36所述的假肢(10),其特征在于,所述旋转操作装置是螺丝起子切口(66)或者嵌入六角形。
38.根据权利要求20,31和36所述的假肢(10),其特征在于,所述端部(64)的螺纹的尺寸对应套筒(48,49)的内螺纹,所述旋转操作装置(66)适于分别从主体(40)的通口(42a)和从包括在两翼(41)之间的第二腿骨(14)的区域进入。
