根据另一个方面,公开了用于髋部整形外科手术的系统。该系统包括股骨头部件、细长柄部件以及外科器械。细长柄部件包括近侧主体、从近侧主体向上并向内侧延伸的颈部、从近侧主体向下延伸的渐缩柄、以及定位在颈部上并正交于颈部的纵向轴线延伸的锚定件。颈部被构造成接收股骨头部件。
外科器械包括被构造成接合股骨头部件的板、被构造成接合锚定件以将外科器械固定到细长柄部件的臂以及被构造成沿着轴线移动板以将力施加到股骨头部件的致动器。

具体实施方式
虽然本发明的概念易于具有各种修改形式和替代形式,但本发明的具体示例性实施方案已在附图中以举例的方式示出,并且将在本文中详细说明。然而应当理解,本文无意将本发明的概念限制为所公开的具体形式,而是相反,本发明的目的在于涵盖所附权利要求限定的本发明的实质和范围内的所有修改形式、等同形式和替代形式。
在整篇说明书中,当提及整形外科植入物或假体和本文所述的外科器械以及患者的自然解剖结构时,可使用表示解剖学参考的术语,例如前、后、内、外、上、下等等。这些术语在解剖学研究和整形外科领域都具有公知的含义。除非另外说明,否则在书面具体实施方式和权利要求中使用的这些解剖参考术语旨在与其熟知的含义一致。
参见图1,整形外科假体例证性地体现为髋部假体的股骨整形外科植入物10。股骨整形外科植入物10(在下文中为植入物10)包括头部件12和细长柄部件14,该细长柄部件14被构造成插入患者的外科手术准备的股骨18的髓腔16中。具体地,股骨柄部件14被植入到患者的股骨18的经外科手术准备(例如钻孔)的髓腔16中。
头部件12包括球形外表面20,该球形外表面20被构造成接合患者的自然髋臼(未示出)或植入患者骨盆骨中的假体髋臼窝。头部件12可用植入物级生物相容性金属构造,尽管也可使用其它材料。此类金属的示例包括钴(包括诸如钴铬合金的钴合金)、钛(包括诸如Ti6A14V合金的钛合金)和不锈钢。头部件12还包括远侧表面22,该远侧表面22具有限定于其中的开口24,并且内壁从开口24向内延伸以限定头部件12中的渐缩孔28。
头部件12被固定到形成在柄部件14的细长颈部34的端部32中的耳轴30。如图1所示,细长颈部34具有延伸穿过耳轴30的纵向轴线36。在例示性实施方案中,头部件12沿着纵向轴线36推进以推进柄部件14的渐缩耳轴30,该柄部件14的渐缩耳轴30可被推进到头部件12的渐缩孔28中以将耳轴30(以及因此柄部件14)和股骨头部件12彼此锥形锁定。当安装在柄部件14上时,股骨头部件12被定位为承载在患者的天然髋臼或假体承窝上,该假体承窝已被植入到患者的骨盆中以取代他或她的髋臼。通过这种方式,整形外科髋部假体10和天然或人工髋臼共同用作取代患者髋部的自然关节的系统。
如图1所示,细长颈部34从附接至细长柄部件14的近侧主体40的下端部38向内侧并向上延伸。如以下更详细描述的,细长柄部件14还包括定位在颈部34上的工具接合特征部或锚定件42。渐缩柄44远离近侧主体的相反端部向下延伸。渐缩柄44具有限定股骨柄部件14的最下表面的圆形远端46。
在本文所述的例示性实施方案中,柄部件14体现为“配合和填充”类型的股骨柄。因此,柄部件14体现为单体金属结构。柄部件14可用植入物级生物相容性金属构造,尽管也可使用其它材料。此类金属的示例包括钴(包括诸如钴铬合金的钴合金)、钛(包括诸如Ti6Al4V合金的钛合金)和不锈钢。此类金属金属股骨柄部件14还可涂覆有表面处理剂,例如羟基磷灰石,以增强生物相容性。此外,可对接合自然骨的股骨柄部件14的表面(诸如近侧主体40的外表面)进行纹理化以有利于将该部件固定到骨。此类表面也可为多孔涂覆的,以促进骨向内生长用于持久固定。
如图2A和图2B所示,细长柄部件14的锚定件42限定在前表面50和后表面54中。柄部件14的前表面50从颈部34的下端部38延伸到耳轴30的基部,并且开口52限定在前表面50中、与颈部34的下端部38相邻。后表面54从颈部34的下端部38延伸到耳轴30的基部。另一个开口56限定在后表面54中、与颈部34的下端部38相邻。在例示性实施方案中,锚定件42包括从开口52,56向内延伸的内壁60。如图2A和图2B所示,内壁60限定穿过细长颈部34的通路62。
内壁60包括在开口52,56之间延伸的内侧表面64和弯曲侧表面66。在例示性实施方案中,内侧表面64是大体上平面的并且与细长颈部34的纵向轴线36正交地延伸。在其它实施方案中,内侧表面64(以及因此锚定件42)可仅横向于轴线36延伸。另外,如图2A和图2B所示,内侧表面64在连接至侧表面66的上边缘68与下边缘70之间延伸。在例示性实施方案中,纵向轴线36在边缘68,70之间的表面64上的中点72处与内侧表面64相交。应当理解,在其它实施方案中,轴线36可从中点72偏离。
现在参见图3,示出用于将股骨头部件12固定到细长柄部件14的外科器械100。外科器械100包括主体102、被构造成接合股骨头部件12的致动器104以及被构造成接合细长柄部件14的锚定件42的一对细长臂106。器械100的主体102包括从远端110延伸到近端112的细长孔108。在例示性实施方案中,细长孔108在远端110处具有内螺纹。
器械100的致动器104具有延伸穿过细长孔108的轴114和附接至轴114的远端118的接合板116。轴114的远端118包括被构造成接合主体102的内螺纹孔108的多个外螺纹120。手柄122在与远端118相反的端部处固定到轴114。如以下更详细描述的,手柄122可操作以旋转轴114并沿着由轴114限定的轴线124移动接合板116。
如图3所示,接合板116包括固定到轴114的远端118的基部126。板116还包括被构造成接合股骨头部件12的远侧表面128。在例示性实施方案中,远侧表面128是凹形的以限定接收股骨头部件12的球形外表面20的凹陷部130。接合板116例示性地由聚合物材料(诸如硬塑料)形成。应当理解,在其它实施方案中,板116可部分地或完全由金属材料形成。在其它实施方案中,板116还可包括由弹性体材料形成的垫圈,该垫圈被构造成接合该头部件12。
如上所述,器械100还包括被构造成接合细长柄部件14的锚定件42的一对细长臂106。每个臂106包括从远端134延伸到近端136的细长主体132。接合销138在垂直于或正交于由致动器104限定的轴线124的方向上从每个臂106的远端134向外延伸。如图3所示,接合销138被成形为接收在细长柄部件14中限定的通路62中。
细长臂106被联接到器械100的主体102,使得壁106(以及因此接合销138)可相对于轴线124重新定位。在例示性实施方案中,主体102在细长孔108的每侧上具有限定于其中的一对通道140。每个通道140的尺寸被设定成接收臂主体132中的一个的对应近端136。应当理解,在其它实施方案中,器械可包括突出部、开口、通道或通路的其它组合以将臂附接至主体。
器械100还包括调整机构142,该调整机构142被构造成允许臂106相对于主体102(以及因此轴线124)选择性地移动。在例示性实施方案中,调整机构142包括一对螺旋夹具144,146,该对螺旋夹具144,146可由使用者操作来独立地将臂106中的每一个定位并固定在相对于轴线124的期望位置处。应当理解,在其它实施方案中,调整机构可包括被构造成允许臂选择性地移动的其它紧固件、螺钉、突出部等。
在外科手术过程中,细长柄部件14可插入到患者的股骨18的经外科手术准备的髓腔16中,如图4所示。为了附接股骨头部件12,外科医生可将所选择的头部件12与细长柄部件14的耳轴30对准。外科医生可沿着纵向轴线36推进头部件12,使得耳轴30被接收在头部件12中所限定的渐缩孔28中。
使用者(诸如外科医生或外科助手)可随后使用外科器械100来可控制地施加足够的准静态轴向力,以将头部件12接合到柄部件14。如本文所用的准静态轴向力是指逐渐施加的、峰值大小从低到高增加的轴向力。相反地,如本文所用的准静态轴向力是指或多或少、瞬时地(诸如像锤击)施加的高轴向力。
为了附接外科器械100,使用者可操作螺旋夹具144,146来使夹具144,146从细长臂106的近端136脱离接合,从而释放臂106以便相对于轴线124移动。使用者可随后将臂106从轴线124拉离,这使接合销138分开。可将接合板116推进成与股骨头部件12的球形外表面20接触,并且轴线124与柄部件14的细长颈部34的纵向轴线36对准。
如图6所示,接合销138可与限定在柄部件14中的通路62对准。使用者可随后将销138推进到通路62中。当销138定位在期望位置处时,使用者可操作夹具144,146以将细长臂106(以及因此销138)锁定就位。在器械100如图6所示定位的情况下,使用者可旋转手柄122以在由箭头150指示的方向上沿对准轴线36、124推进头部件12。为了将准静态轴向力施加在部件12,14上,器械100的接合销138接合柄部件14的锚定件42的内侧表面64,使得当手柄122旋转时,致动器104沿着轴线36,124将力施加到部件12,14。
在例示性实施方案中,器械100被构造成施加至少约4kN的峰值轴向力。应当理解,在其它实施方案中,器械可包括测力仪或其它传感器,以测量施加到部件12,14的力。在使用者已施加所需量的力来将头部件12固定到柄部件14之后,使用者可使器械100的致动器反向并将器械从外科区域移除。
虽然器械100是例示性的手动操作,但是应当理解,在其它实施方案中,器械100可包括用于施加轴向力的马达或其它驱动机构。被构造成施加准静态轴向力的其它外科器械在名称为“ASSEMBLY TOOL”的共同未决的美国专利申请序列号62/103,611中有所公开,该专利申请与本申请同时提交。此类外科器械可被构造用于与特定、兼容类型的锚定件一起使用,但可被适配用于与柄部件14的锚定件42一起使用。在其它实施方案中,柄部件可包括与图7-13所示的构型类似的其它锚定件构型,这将在下文更详细地描述。图7-13所示的实施方案的一些特征部基本上类似于上文参考图1-6的实施方案所讨论的那些。此类特征部在图7-13中标注有与在图1-6中所使用的参考数字相同的参考数字。
现在参见图7-8,示出了另一个细长柄部件(下文称为柄部件214)。类似于图1-6的实施方案,柄部件214包括在柄部件214的细长颈部234的端部32中形成的耳轴30。细长颈部234也具有延伸穿过耳轴30的纵向轴线36。如图7所示,细长颈部234从附接至细长柄部件214的近侧主体240的下端部38向内侧并向上延伸。类似于图1-6的实施方案,渐缩柄(未示出)远离近侧主体的相反端部向下延伸。
细长柄部件214还包括定位在颈部234上的工具接合特征部或锚定件242。细长柄部件214的锚定件242限定在前表面250和后表面254中。柄部件214的前表面250从颈部234的下端部38延伸到耳轴30的基部,并且开口252限定在前表面250中、与颈部234的下端部38相邻。后表面254从颈部234的下端部38延伸到耳轴30的基部。另一个开口256限定在后表面250中、与颈部34的下端部38相邻。在例示性实施方案中,锚定件242包括从开口252向内延伸的内壁258和从开口256向内延伸的另一个内壁260。
如图7-8所示,内壁258是从开口252延伸到基部表面264的多个内壁262中的一个。基部表面264和内壁262配合以限定位于前表面250中的闭合孔口266。类似地,内壁260是从开口256延伸到基部表面272的多个内壁270中的一个。基部表面272和内壁270配合以限定位于后表面254中的闭合孔口274。在例示的实施方案中,孔口266,274是方形的。在其它实施方案中,孔口可以是矩形的、圆形的或其它几何形状。
在例示性实施方案中,内壁258,260包括内侧表面278,该内侧表面278是大体上平面的并且定位在延伸穿过正交于纵向轴线36的细长颈部234的公共虚平面中。换句话讲,内侧表面278(类似于柄部件14的内侧表面64)相对于轴线36正交延伸。
柄部件214的锚定件242可以类似于以上关于图1-6所描述的方式由适当成形的外科器械接合。例如,外科器械100的接合销138可被成形为接收在柄部件214的闭合孔口266,274中,并由此接合锚定件242。在其它实施方案中,可使用其它外科器械。
现在参见图9,示出了细长柄部件(下文称为柄部件314)的另一个实施方案。类似于图1-8的实施方案,柄部件314包括在柄部件314的细长颈部334的端部32中形成的耳轴30。细长颈部334也具有延伸穿过耳轴30的纵向轴线36。如图9所示,细长颈部334从附接至细长柄部件314的近侧主体340的下端部38向内侧并向上延伸。渐缩柄(未示出)远离近侧主体的相反端部向下延伸。
细长柄部件314还包括定位在颈部334上的工具接合特征部或锚定件342。在例示性实施方案中,颈部334具有在其下端部38处限定在内侧表面346中的开口344。多个内壁348从开口344向内延伸到基部表面350。闭合凹坑352由表面350和内壁348限定。在例示的实施方案中,闭合凹坑352是长方形的。在其它实施方案中,孔口可以是方形的、圆形的或其它几何形状。
锚定件342包括内壁348中的一个的上表面354。上表面354正交于纵向轴线36延伸。在其它实施方案中,表面354(以及因此锚定件342)可仅横向于轴线36延伸。上表面354可以类似于以上关于图1-8所描述的方式由适当成形的外科器械接合。例如,外科器械100可仅用单个接合销138来构造,该接合销138被成形为接收在柄部件314的闭合凹坑352中,并且由此接合锚定件342的上表面354。在其它实施方案中,可使用其它外科器械。

应当理解,上述柄部件的锚定件可被构造用于在整形外科手术的其它部分中使用。例如,外科医生可在植入和提取过程中使用锚定件来握住柄,从而消除对用于执行那个步骤的单独特征部的需要。

细长柄部件,所述细长柄部件包括:(i)近侧主体,(ii)颈部,所述颈部从所述近侧主体向上并向内侧延伸,所述颈部具有纵向轴线并且被构造成接收所述股骨头部件,(iii)渐缩柄,所述渐缩柄从所述近侧主体向下延伸,(iv)第一开口,所述第一开口限定在所述颈部的前表面中,(v)第二开口,所述第二开口限定在所述颈部的后表面中,以及(vi)锚定件,所述锚定件定位在所述颈部上并正交于所述颈部的纵向轴线延伸,所述锚定件具有从所述第一开口和所述第二开口向内延伸的一对内壁,其中,所述内壁包括平面的内侧表面和连接至所述平面的内侧表面的弯曲表面,以及
外科器械,所述外科器械包括:(i)板,所述板被构造成接合所述股骨头部件,(ii)臂,所述臂被构造成接合所述锚定件以将所述外科器械固定到所述细长柄部件,以及(iii)致动器,所述致动器被构造成沿着轴线移动所述板以将力施加到所述股骨头部件以推进所述股骨头部件到所述颈部上。
