本发明涉及到以含有与去铁利硫素(desferrithiocin)相关的某写化合物的药用组合物治疗疟疾。该组合物特别适用于治疗恶性疟原虫(Plasmodium falciparum)的疟疾。
背景
疟疾是折磨人类最古老和最广泛传播的一种疾病。在人类寄生虫病中,它是最致命的。在发展中国家它是地方性的,并且每年感染超过500百万人,其中超过2.7百万人丧生。在20世纪中叶,在某种程度上由于用于这种疾病的新治疗方法,每年疟疾的发生率降低。然而,近些年,包括美国的成千上万例病例,疟疾的病例世界范围内显著提高。这种升高部分地是由于这种疾病耐药菌株的出现。
疟疾是由疟原虫属真核疟原虫引起的。100种疟原虫中,已知四种可引起人体疟疾。这四种中的三种,间日疟原虫(Plasmodiumvivax)、三日疟原虫(Plasmodium malariae)、孵型疟原虫(Plasmodiumovale)引起该疾病的相对良性型。第四种,恶性疟原虫(Plamodiumfalciparum),是恶性和最致命的,对世界范围内大多数疟疾引起的死亡负责。
恶性疟原虫经雌性按蚊属(Anopheles)蚊子叮咬进入人宿主。感染过的蚊子叮人,并注入小量含有抗凝剂和恶性疟原虫寄生虫的单倍体孢子体的唾液。该孢子体进入循环系统并在一小时左右到达肝脏。在肝脏内它们会进入肝实质细胞,该阶段在这种疾病的周期内称为红细胞外发育期。此时期5-7天期间,它们会进行多次无性分裂、裂体生殖、倍增30000至40000倍,并制造裂殖体。
作为裂殖体,它们会离开肝脏,重新进入血流,侵入红细胞,并且开始红细胞内发育期。一旦进入红细胞,恶性疟原虫作为一种单核原虫开始增大。另一个1-3天时期,该原虫无性分裂以制备含有6-24核的裂殖体。该裂殖体分裂并制备单核裂殖体。这导致红细胞溶解和释放裂殖体进入血流以感染其它的红细胞。
一些裂殖体分化成大配子体和小配子体,它们不能引起红细胞溶解。这些雄性和雌性性别形态可由蚊子摄取,在那里它们会受精成合子,合子在蚊子唾液腺内制备孢子体以便可以再感染其它的人宿主。
疟疾是无症状的,直至红细胞内期,从红细胞同时释放裂殖体和碎片进入循环系统,引起典型的疟疾征象和症状。这些包括阵发(痉挛和惊厥)、高烧、寒战(强直和寒战)、大量出汗、呕吐、贫血、头痛、肌肉疼痛、脾大和低血糖。恶性疟原虫疟疾内,由于裂殖体的释放发生在每隔大约48小时,症状隔日、每三天发作一次。在红细胞内期裂殖体释放之间,人宿主感觉正常并无症状。
恶性疟原虫疟疾最严重的后果是感染的红细胞聚集、凝块或沉积,包括粘着在血管壁上。取决于沉积的位置,由于限制流向重要器官的血液,发生威胁生命的结果。这些包括脑型疟脑病、肺水肿、急性肾衰、严重的血管内溶血和血红蛋白尿。由恶性疟原虫疟疾引起的巨大数量的死亡主要归因于这些结果。
疟疾传统的治疗方法基于或者控制蚊子的数量、疫苗,或者化学疗法。对于化学疗法,药物通常针对疾病的特定阶段。这些药物包括组织杀裂殖体剂,例如氯喹,用于根除肝脏内的红细胞外发育期;血液杀裂殖体剂,例如氯喹、叶酸抗体和8-氨基喹啉,优选乙胺嘧啶、伯氨喹、扑疟喹,用于破坏红细胞内期;杀配子体药,例如4-氨基喹啉,用于杀配子;以及用于杀孢子体的钉孢子体药。
近些年,疟疾的最有效治疗方法,尤其适用于恶性疟原虫疟疾的,是4-氨基喹啉、氯喹。选择该药治疗该疾病能有效抑制间日疟原虫型和恶性疟原虫型的红细胞内期。氯喹充当一种血液杀裂殖体剂并很少产生严重的副作用。它抑制原生动物细胞内核酸和蛋白质合成。它可以用于急性开始恶性的隔日恶性疟原虫性疟疾的治疗和预防。
化学疗法已经按预防性用于许多疟疾的治疗,这已经导致引起疟疾的疟原虫属耐药性菌株出现。疟原虫属耐氯喹性现在已经广泛流传,并成为一个严重问题。这导致选择性的化学治疗剂的发展。
出现的化合物包括叶酸抗体,包含砜和磺胺药物,例如氨苯砜、周效磺胺、磺胺嘧啶和磺胺甲氧吡嗪;primines,以及双胍。这些化合物与p-氨基苯甲酸(PABA)竞争,干扰四氢叶酸的合成,并充当血液杀裂殖体剂。然而,它们的有效剂量极其有毒,并且疟原虫很容易产生对这些药物的耐药性。
疟原虫属产生耐药性的能力与这些药物不希望的副作用结合,导致新的疗法不断发展。因而,目前可以得到许多适于疟疾治疗的化合物,或者正在发展。
通过参考Goodman and Gilman’s治疗的药理学基础(ThePharmacological Basis of Therapeutics,),第8版,McGraw-Hill,Inc.(1993),41章,978-998页,可以发现可用于抗疟疾治疗的抗原生动物化合物。
如上所述,耐一种或多种有效的疟疾治疗的恶性疟原虫疟疾菌株现在是无所不在的。发展直接攻击寄生虫的新化合物时,新的耐药性菌株产生。此外,有效药物的持续的未预计的副作用提出了问题。当必须给予多种药物以治疗超过一种疟原虫属并发感染时,这个问题特别实际,也变得非常普通。因而,不仅更多可以选择的适于疟疾,尤其适于恶性疟原虫疟疾的化学治疗法是期望的,而且整个根除疟原虫属寄生虫的作用新原理也是期望的。这些原理使得耐这些新药物的寄生虫的菌株的出现更加困难。
发明目的
本发明的一项目的是提供一种新族的抗疟药,它抗恶性疟原虫疟疾特别有效,并且在治疗方法上具有相对低的毒性。
本发明的另一项目的是提供一种采用该新族的抗疟药治疗疟疾的新方法。
本发明的另一项目的是提供一种治疗恶性疟原虫疟疾,尤其是治疗耐传统化学抗疟剂的恶性疟原虫疟疾的新方法。

详细地描述和优选的实施方案
本发明基于上述化学式的铁-螯合的化合物有效的发现,即,抑制恶性疟原虫性疟疾。这些化合物夺去了该寄生虫其代谢过程中必需的铁。本发明的化合物在对于人给药时,完全可以采用由于疟疾治疗需要短暂的给药间隔而使长期治疗所无法使用的剂量来给药。
应用证实了化合物适于根除疟原虫属寄生虫,它目标为调节细胞铁的代谢。铁,地壳中第四丰富的元素,在一切生命中也是普遍存在的。真核细胞的微生物需要铁以维持生命。例如,铁对于由细胞色素使用和作为带电蛋白质的酶的辅因子是关键的。
铁螯合剂,例如desferrithiocin(DFT)已经用于β-地中海贫血和再生障碍性贫血中输血诱发的铁负荷过多的治疗,在本发申请的发明之前,此化合物并不预计用于疟疾的治疗。DFT是优异的铁螯合剂。它口服可良好的吸收,并在螯合铁离子时是高效的。然而,超时使用时它有高肾毒性。由于在患有β-地中海贫血的病人中,螯合治疗是输血诱导的铁负荷过多的终生治疗,所以这是特别有疑问的。DFT的慢性毒性已经导致它彻底放弃慢性铁负荷过多的治疗,尽管它作为铁螯合剂是高效的。
已经发现,因为以本申请公开的化合物治疗疟疾仅需要短暂的给药间隔,病人可避免与历史上的DFT和相关化合物一起使用时的慢性毒性。事实上,新发现的用于疟疾治疗的铁载体毒性的大小,允许新发现的DFT类似物发展,该类似物甚至是更有效的铁螯合剂,然而在该疾病的治疗的短时期内仍然具有易控制的毒性。因而,对于传统的铁螯合治疗是不能接受的DFT类似物,成为可行的适于疟疾治疗的替代者,尤其是致命的恶性疟原虫疟疾。
desferrithiocin药效基因的X-射线晶体照相术研究表明,三个配位中心,即芳族羟基、二氢噻唑氮和羧基,对于该化合物的铁清除能力是重要的。对上述官能团的任何结构性修正会影响DFT与铁配位的能力。然而,如何发生了最小化DFT类似物的毒性的理解,仍然是不清楚的。本发明提供一个范围的化合物,它能平衡最佳铁螯合能力和最小毒性。
本发明中使用的化合物
“烷基”指具有指出的碳原子数目的完全饱和的烃基。例如,1至6烷基,包括,例如,甲基、乙基、n-丙基、i-丙基、n-丁基、t-丁基、n-戊基、戊基、n-己基,和类似物。

位于以上化学式中的游离羟基,可通过常规的保护基团进行保护。这些保护基团使羟基免于不期望的缩合反应、取代反应和类似反应。保护基团可容易地引入和清除,即,不需要不期望的次级反应发生,例如,通过溶剂分解或还原,以本身公知的方式。在“有机化学保护基团”,Plenum出版社,伦敦,纽约(1973)以及“现代有机化学(Methoden der organischen Chemie)”,Houben-Weyl,第4版,15/1卷,Georg Thieme Verlag,Stuttgart(1974)中,介绍了保护基团及其引入和分离的方法。

以上的羧基W的反应官能衍生物是,例如,酸酐、活性酯或活性酰胺、氰、化学式-C(ORa)3或者-C(=NH)-Ra,其中Ra为低级烷基。相应的衍生物在本领域已知。

在以上化学式的化合物中,上述羧基W的活化可以在原位进行。
以上化学式有关半胱氨酸的化合物的反应衍生物是这样一种化合物,其中为了反应,以上述化学式的化合物和羧基活化氨基和/或巯基,换言之,呈现亲核形式。例如,通过与亚磷酸盐反应活化氨基。
W代表羧基的以上化合物与半胱氨酸衍生物的反应,优选在适合的缩合剂或脱水条件下进行,例如,通过共沸蒸馏清除反应中的水。常规的缩合剂是,例如,碳二亚胺,例如,N,N′-二乙基-,N,N′-二丙基,N,N′-二环己基-或N-乙基-N′-(3-二甲基氨丙基)-碳二亚胺,适合的羰基化合物,例如,羰二咪唑,或1,2-噁唑鎓化合物,例如,2-乙基-5苯基-1,2-噁唑鎓-3′-磺酸酯或2-叔-丁基-5-甲基-异噁唑鎓高氯酸盐,适合的酰氨基化合物,例如,2-乙氧基-1-乙氧基羧基-1,2-二氢化氮杂萘,此外,二苯基磷酰基叠氮化物。缩合反应优选在无水反应介质中实施,优选溶剂或稀释剂,例如,二氯甲烷、苯或四氢呋喃以及,如果必需的话,冷却或加热时,例如,在室温或略微升高的温度,和/或惰性气体下。如果采用W表示羧基酸酐衍生物的化合物,该反应基本上在同样的条件下实施,在有碱性剂例如钠或钾盐或碳酸,或叔氨基化合物如三-C1-C4-烷基胺,例如,三乙基胺,或吡啶碱如吡啶或喹啉存在的情形下。
按照本发明,该过程优选的形式是以上W表示氰的化学式与以上半胱氨酸衍生物化学式反应的化合物。该反应室温下在惰性溶剂例如水溶剂中进行,或者优选地,在稍微升高的温度,例如,在大约50℃至80℃,并且优选在惰性气体中。羧酸(或其中的反应官能衍生物)与该化学式的化合物
或者与其可转化的化合物反应。按照本发明,优选的羧基官能衍生物是N-琥珀酰亚氨基酯。在惰性溶剂,例如质子惰性溶剂,例如,二甲基甲酰胺、二甲基亚砜或二噁烷或C1-C4烷醇,例如甲醇,室温下或冷却时,例如,大约0℃,进行该反应。
在所制备的保护了一个或多个官能(羟基)团的化合物中,后面的可被任意地阶段性或同时地、以本质上已知的方式、通过溶剂分解作用游离,尤其水解作用或酸解,或者某些情形下也通过谨慎的还原游离。甲硅烷基保护基可与氟化物,例如,四乙铵氟化物方便地分离。
本发明的化合物盐可以以本质上已知的方式制备。因而,可以形成具有酸基的化合物盐,例如,通过以金属化合物,例如碱土金属的适合的有机羧酸盐处理,例如,α-乙基己酸的钠盐,或者以无机酸的碱金属或碱土金属盐处理,例如,碳酸氢钠,或者以铵盐或适合的有机胺处理,优选化学数量的数量或仅仅小量过量的使用的成盐剂。以常规方式得到本发明化合物的酸加成盐。可制成本发明化合物的内盐(两性离子形式),例如,通过中和该化合物或盐,例如酸加成盐,至等电点,例如,弱碱性,或通过以液体离子交换器处理。
盐可以常规方式转化成游离的化合物:金属和铵盐可转化成游离的化合物,例如,通过以适合的酸,和酸加成盐处理,例如,通过以适合碱性剂处理。
原材料可商业上得到和/或已知,或者通过公知的方法制备。
消旋体可以本质上已知的方式分离,例如,在光学对映异构体转化成非对映异构体之后,例如,通过与旋光性酸或碱反应。
当化合物为羧酸时,羧酸的外消旋混合物通过首先以旋光性胺碱处理溶解,以制备非对映体盐。可用于此目的的旋光性胺碱的实例是(R)-(+)-∝-甲基苯基胺,(S)-(-)-∝-甲基苯甲基胺,(1R,2S)-(-)-麻黄素,奎宁,和奎尼丁。如此制备的非对映异构的盐具有不同的性能,例如溶解度,并且非对映异构体可因此通过选择性的重结晶由合适的溶剂中分离。通过分离出的非对映异构体盐的再酸化,接着可得到旋光性羧酸。
可以选择地,可以以旋光性乙醇处理羧酸的外消旋混合物,以制备非对映异构体酯的混合物。可用于此目的的旋光性乙醇的实例是(1R,2S,5R)-(-)-甲醇,(1R,2S,5R)-(+)-甲醇,(R)-(-)-2-辛醇,和(S)-(+)-2辛醇。因此而制备的非对映异构体酯的混合物然后可通过色谱法分离。接着通过常规技术,从分离出的非对映异构体酯中得到旋光性羧酸,例如以氢氧化钠或氢氧化锂在再酸化之后处理这些酯。
如果化合物为酯,通过采用以上描述的一种方法,首先溶解对应的羧酸的外消旋混合物,酯的外消旋体溶入对映异构体。借助于类似于用于制备外消旋酯的方法,通过对应的旋光羧酸的酯化作用可得到旋光性酯。
可以选择地,羧酸的外消旋混合物或酯的外消旋混合物,采用适合的手性固定相和适合的洗脱剂,通过高性能液体色谱法可分离成各自的对映体。

药物制剂
上述化合物可用于治疗疟疾,因而可用于药物组合物的制备,该组合物的与有机或无机、固体或液体、药学上可接受的载体一起的混合物中含有有效量的活性物质。因而,本发明的另一方面是这里描述的与药学上可接受的赋形剂一起的一种抗疟疾组合物。
按照本发明的药物组合物适于肠道给药,例如口服,适于对人体非肠道给药,例如皮下的或静脉内,该药物中含有与药学上可接受载体一起的药理学活性物质。活性物质的剂量取决于各种因素,例如年龄、体重、疟疾症状的严重程度,以及其它的医生应识别的因素。
该新药物制剂含有大约10%至大约95%的活性物质,优选大约20%至大约90%。按照本发明的药物组合物,例如,可以是单位剂型,例如糖锭剂、片剂、胶囊、栓剂或安瓿,并含有大约0.1g至大约2.0g活性组分,优选大约0.3g至大约1.0g。
本发明的药物组合物以本质上公知的方式制备,例如,通过常规的混合、制粒、成型、溶解或冻干步骤。如果期望,通过与一种或多种固体载体合用得到口服用的药物组合物,在加入适合的助剂之后如果期望或需要,所得的混合物制粒并加工该混合物或制粒,以制备片剂或糖芯。这样做期间,它们也可与能释放活性物质或允许它们以控制量分散的塑料载体混合。
适合的载体尤其是填料,例如糖,例如,乳糖、多糖、甘露醇或山梨醇、纤维素制剂和/或磷酸钙,例如磷酸三钙或磷酸氢钙,以及粘合剂,例如淀粉,例如,玉米、小麦、稻米或马铃薯淀粉,明胶,西黄蓍胶,甲基纤维素,羟丙基甲基纤维素,羧甲基纤维素钠和/或聚乙烯吡络烷酮,和/或,如果需要,崩解剂,例如上述的淀粉,以及羧甲基淀粉,交联的聚乙烯吡络烷酮,琼脂,海藻酸或其盐,例如海藻酸钠。助剂特别是流动性调节剂和润滑剂,例如,硅胶,滑石,硬脂酸或其盐,例如硬脂酸镁或钙,和/或聚乙烯丙二醇。为糖芯可提供包衣,如果期望,它们可抗胃酸,其中,那里使用浓缩的糖溶液,该溶液在适合的有机溶剂或溶剂混合物中任意含有阿拉伯胶、滑石、聚乙烯吡咯烷酮、聚乙烯丙二醇和/或二氧化钛、漆溶液或者,为了制造可抗胃酸的包衣,使用合适的纤维素制剂溶液,例如邻苯二甲酸醋酸纤维系或邻苯二甲酸羟丙基甲基纤维素。例如,为了识别或为了显示不同剂量的活性物质,可向片剂或糖锭剂加入着色物质或颜料。
其它的口服给药药物组合物是明胶制的干填胶囊以及明胶制软的、密封胶囊和增塑剂例如甘油和山梨醇。干填胶囊的混合物中含有颗粒型的活性组分,与填充剂一起,例如,玉米淀粉、粘合剂和/或助流剂,例如滑石或硬脂酸镁以及任意的稳定剂。软胶囊中,活性组分优选溶解或悬浮在适合的液体或蜡状物质中,例如脂肪油,液体石蜡或聚乙烯丙二醇,也可加入稳定剂。
其它的口服给药剂型是,例如,常规方法制备的糖浆剂,例如,它在悬浮形态下含有浓度大约为5%至20%的活性组合物,优选大约10%,或给药时可提供适合的单次剂量,例如5或10ml的类似浓度。例如,还适合的剂型是粉末状或振摇制备的液态浓缩液,例如,乳剂。这些浓缩液可压缩成单次剂量数量。
适于肠道给药的特别适合的剂型是水溶性形式活性组分的无菌水溶液,例如,水溶性盐,或无菌注射水悬浮液,该悬浮液含有增强粘度的物质,例如,钠盐,羧甲基纤维素,山梨醇和/或右旋糖酐,以及任意的稳定剂。此外,含有或不含有助剂的活性组分,也可以是冻干形式,并在肠道给药之前通过加入适合的溶剂形成溶液。
通常,本发明的注射用组合物可以是
1.注射预备用溶液,或
2.仅在使用之前与溶剂合用的干燥可溶性组合物,
3.或预备给药之前稀释的液体浓缩液。
制备注射用组合物中,应当对溶液张性调节集中严格的注意力以避免刺激。
载体通常没有治疗活性,并是无毒的,但活性组分以适于吸收的形式位于身体组织中。当化合物以水溶液出现时,吸收通常发生得最迅速和彻底。然而,水溶性液体载体或水不溶性液体的代替物的改变可影响吸收速率。优选地,适于皮下组合物的最大值的载体水符合USP注射用水标准的水。通常,混合用的合适质量的水通过蒸馏或者反渗透以符合USP标准。在Remington:The Science and Practice ofPharmacy”第19版,1526-1528页详细说明了适合的标准。制备皮下注射用组合物的过程中,可以采用水载体,水溶性载体,以及非水载体。官方认可确定的水载体,由于其在肠道普遍地有效利用。
水溶性载体也可用于本发明的肠道组合物制剂中。采用这些溶剂,主要是影响该化合物的溶解度。该组中最重要的溶剂是乙醇、聚乙二醇和丙二醇。
这些载体包括不挥发性油,例如,植物起源的允许正常代谢的油。对于USP皮下注射液可注射的组合物,USP规定限制不饱和度和游离脂肪酸含量。通常使用的油是玉米油,棉籽油,花生油,以及芝麻油。最近发展的某些中链的脂肪酸酯的中性油也是可用的,也叫做分离的椰子油。中链脂肪酸,即,大约8至10碳原子,包括化合物MIGLYOL,该化合物售做Dynamit Nobel。五种类型称之为MIGLYOL810,812,828,829,以及840的MIGLYOL是有益的。在Dynamit Nobel的贸易文献中更完整地描述了它们。某一其它的也可用做非水载体的酯是,例如,甘油三酯,丙二醇二酯,以及类似物。
额外物质也可以包含在本发明的注射用组合物中,以增强或保护该组合物的质量。因而,添加的物质可影响溶解度,有助于患者舒适,增强化学稳定性,或者保护制剂抗微生物生长。因而,该组合物可包括适合的溶解剂,使溶液等张的物质,充当抗氧剂的物质,以及充当防腐剂以阻止微生物生长的物质。这些物质含量适合于它们的功能,但不会反向影响该组合物治疗疟疾的作用。适合的抗微生物剂的实例包括硫柳汞,苄索氯铵,洁尔灭,苯酚,甲基p-羟基苯甲酸盐和丙基p-羟基苯甲酸盐。在“Remingtons”1529页可发现适合的缓冲液和抗氧化剂。
通常,本发明无菌、非肠道注射用组合物包含按重计大约0.1%至按重量计大约50%的化合物,剩下的为适当的赋形剂或赋形剂类。
制品
本发明的另一方面是包含抗疟疾组合物的制品,该组合物含有化学式(I)或化学式(IV)表示的化合物,以及药学上可接受的赋形剂,该制品进一步包括给予人体足够量的抗疟疾组合物以在时间内治疗疟疾的书面说明。这是本发明的重要的方面,由于在化合物被批准任何特定用途之前,它必须通过美国食品和药品管理局的上市批准。部分的该步骤包括提供一种伴随该最终被出售的药物组合物的标签。该标签包括组合物的名称和其它项,例如临床药理学、作用机理、抗药性、药动学、吸收率、生物利用度、禁忌症以及类似项,它也提供必需的剂量,给药和用途,如上所述。因而,药物与适合的标签说明结合对于一旦药物投入市场后其适当的用途是重要的。
疟疾的治疗
这里使用的术语“治疗”包括了哺乳动物任何通过铁螯合剂可治疗的疾病的治疗方法,尤其是人,它包括:
(i)预防易患疾病但并没有诊断具有该疾病的受体发生疾病;
(ii)抑制疾病,即抑制其发展;或
(iii)缓解疾病,即引起疾病康复。
疟疾是一种侵犯各种哺乳动物,尤其是人和非人的灵长类,以及某些鸟类的疾病。本发明的化合物和组合物可用于治疗哺乳动物和鸟类的疟疾。它主要用途在于治疗人类由名为恶性疟原虫的原生动物经起的恶性疟疾,尽管该化合物也可用于治疗更多的良性原生动物引起的疟疾,例如间日疟原虫,三日疟原虫,和卵形疟原虫。该化合物以能从代谢中夺取寄生虫的铁的水平和时间释放,从而引起寄生虫的死亡。
通常,本发明的化合物可以肠道给药,即口服,或非肠道(即腹膜内-IP;肌内-IM;皮下-SC;静脉-IV;等等),例如通过经IV单独注入或输入。皮下给药是指适于注射用的组合物的形式的药物注入仅在皮肤之下的网隙结缔组织。注射剂可以是溶液、悬浮液,或能提供控制释放的活性物的制剂。通常,皮下给药与适于皮下给药的赋形剂一起进行,这意味着赋形剂必须符合USP在适于注射用组合物上的标准。因而,该组合物需要无菌以避免任何归因于注射点有菌的并发症。治疗的特别模式取决于疾病的严重程度,病人的年龄,病人的体型大小,病人的相对健康状态,以及其它的治疗医师应考虑的因素。位于组合物中注射用并将被注射的活性组分的含量是治疗有效量,换言之,对显示疟疾征象和症状的动物给药时,足以导致如上所述的成功治疗的用量。治疗有效量变化取决于受体,痛苦的严重程度和给药的方式,并能通过本领域普通技术人员根据本说明书公开的内容按常规确定。例如,如果病人病情严重并且不能摄取任何食物,于是在直到72小时的时间内提供单独注入或IV输入是必要的。在某些情形下,持续治疗直到一个月,通常该治疗方法不超过两星期,优选小于一星期。通过征象和症状改善的迹象监护病人。
通常范围的适合的剂量为每天大约1至大约250mg/Kg受体体重,优选大约5至大约150mg/Kg。
给出随后的实施例以教导本领域的普通技术人员如何制备用于本发明的化合物。化合物的数值标准参考本申请表I阐述的化合物。化学式A、B、C等名称指伴随实施例的反应顺序或方案。DFT指desferrithiocin。

通式C的化合物2,35-7,9和14-17,通过环化缩合的o-羟基芳基腈A与半胱氨酸衍生物B(方案1)合成[Bergeron等人,J.Med.Chem.34:2072-2078(1991);Bergeron等人,J.Med.Chem.37:1411-1417(1994);Bergeron等人,J.Med.Chem.42:2432-2440(1999)]。
(S)-和(R)-去甲基DFTs 2通过在pH6磷酸盐缓冲剂和甲醇中,得自2-羟基吡啶-N-氧化物的2-氰-3-羟基吡啶分别与D-或L-半胱氨酸缩合合成。2-羟基吡啶与D-或L-半胱氨酸的反应分别产生(S)-和(R)-去氮杂去甲基DFTs 3。
(S)-去氮杂DFT(9)的产物,等同于4-甲基铜绿酸(aeruginoicacid)的天然产物[Ryoo等人,J.Antibiot.50:256-258(1997)],4′-羟基去氮杂DFT(28)通过2-氰苯酚或2,4-二羟基苄腈分别与(S)-α-甲基半胱氨酸在缓冲的含水CH3OH中进行。后面的氰化物通过在醋酸钠和醋酸中处理2,4-二羟基苯甲醛和硝基乙烷制备。6N HCl中DFT(1)的水解产生罕见的氨基酸(S)-α-甲基半胱氨酸。
三配位基螯合剂15-17,配位中心之间有间隔的(S)-3同系物,如下合成。通过在甲醇磷酸盐缓冲剂中加热D-半胱氨酸与2-羟基苯基乙腈制备(S)-4,5-二氢-2-(2-羟基苯基甲基)-4-噻唑羧酸(15)。(S)-4,5-二氢-2-(2-羟苯基)-4-噻唑醋酸(16)和(S)-4,5-二氢-2-(2-羟苯基)-4-噻唑丙酸(17)通过(S)-3-氨基-4-巯基丁酸或(S)-4-氨基-5巯基丙酸分别与2-氰苯酸、酚在甲醇磷酸盐缓冲剂中反应制备。β-或γ-氨基酸由部分保护的L-天门冬氨酸和L-谷氨酸顺序制备[Chauvel等人,J.Med.Chem.37:1339-1346(1994);Wilk等人,Neruopeptides 16:163-168(1990)]。
(R)-和(S)-4,5-二氢-2-(2-,4-羟苯基)-4-噻唑羧酸(5)通过2,4二羟基苄腈,在磷酸盐缓冲剂和甲醇中,分别与L-或D-半胱氨酸缩合制备。
合成(S)-4,5-二氢-2-(2-羟基-3-甲氧基苯基)-4-噻唑羧酸(6)的关键步骤是由D-半胱氨酸与2-羟基-3-甲氧基苄腈进行环化缩合制备。
(S)-4,5-二氢-2-(2-羟基-4-羧基苯基)-4-噻唑羧酸(7)通过类似的反应程序合成。4-甲酰基-3-羟基苯甲酸在醋酸钠和醋酸中采用硝基乙烷转化成4-氰-3-羟基苯甲酸;与D-半胱氨酸的环合完成了二羧酸螯合剂7的合成。
2-氰-3-4-羟基吡啶与DL-高半胱氨酸的缩合产生外消旋的2-(3-羟基-2-吡啶)-4H-5,6-二氢-1,3-噻嗪-4-羧酸(14),一种2位六元的同型物(方案1)[Bergeron等人,J.Med.Chem.37:1411-1417(1994)]。
实施例2:合成化合物18-20
DFT同型物J的稠环,尤其化合物18-21,通过半胱氨酸B(R3=H,B=S,p=0)的对映异构体与o-羟基萘基或-喹啉基腈I的环化合成(方案2)[Bergeron等人,J.Med.Chem.39:1575-1581(1996)]。

实施例3:合成化合物22-24
(S)-去甲基desferrithiocin(DMDFT,2)的羧酸基转化至N-甲基异羟戊酯或转化至5配位二异羟戊酯,分别产生化合物22和23[Bergeron等人,J.Med.Chem.37:1411-1417(1994)]。
化合物24,2位N-苄基异羟戊酯,通过在N,N-二异-丙基乙基胺(DIEA;3当量)和DMF中,以(苯并三唑-1-基氧代)三(二甲基氨基)膦六氟合磷酸盐(BOP试剂)激活2位盐酸N-苄基羟胺的N-酰化作用合成。尽管经葡聚糖LH-20色谱法分离旋光性的异羟戊酯是可行的,该化合物经重结晶差向异构。旋光值从[α]D=-16.4°减少至实质上的零。由于自发的差向异构化作用,没有进行评价旋光物质的生物学性质的尝试。一切研究中采用的是外消旋产物。
实施例4:合成化合物29,29a,33,35和38
化合物29通过如方案1描述的2,5-二羟基苄腈与D-半胱氨酸缩合合成。

该芳基,按顺序,通过在钠盐和醋酸中,加热2,5-二羟基苯甲醛与硝基乙烷制备。
通过类似于5′-羟基去氮杂去甲基DFT29的合成,(S)-4,5-二氢-2-(2,5-二羟苯基)-4-甲基-4-噻唑羧酸(5′-羟基去氮杂DFT,33)由(S)-α-甲基半胱氨酸与2,5-二羟基苄腈在缓冲的含水CH3OH中环化缩合生成。异构的二羟基去氮杂去甲基DFT 35由必要的芳基腈和D-半胱氨酸在甲醇磷酸盐缓冲剂(pH 6)中合成。二羟基去氮杂DFT 38通过处理(S)-α-甲基半胱氨酸与同组的三羟基苄腈制备。在醋酸钠和醋酸中加热2,4,5-三羟基苯甲醛和硝基乙烷,产生相应的化合物35和38的三羟基苄腈、芳香族前体。
(S)-4,5-二氢-2-(5-氟-2-羟苯基)-4-甲基-4-噻唑羧酸(5′-氟代去氮杂去甲基DFT,29a)是由D-半胱氨酸在偏酸性缓冲剂中环合5-氟代-2-羟基苄腈制备而得。该芳香衍生物可通过在二氯乙烯使用异硫氰酸甲酯、氯化铝和三氯化硼的p-氟代苯酚直接氰化,继之以在含水碱基中加热制备[Adachi等人,Syn.Commun.20:71-84(1990)]。
实施例5:合成化合物40-43
双-DFT化合物40和41的合成取决于1,3-二甲氧基苯(51)C-2质子的酸度,该化合物已经金属化(n-Buli/THF),在加入两个芳香环的情形下接着于2-位与1-溴丙烷[Brown等人,J.Med Chem.32:807-826(1989)]和1,4-二碘代丁烷[Tanaka等人,Chem.Lett.:1905-1908(1989)]烷基化。因而,51与1,9-二氯壬烷(52a)或1,11-二溴十一烷(52b)分别去质子注入和区域专一的烷基化,生成四甲氧基化合物53a,b(方案3)。以BBr3/CH2Cl2清除四甲基保护基团,收得四苯酚54a,b。苯酚每个环的Vilsmeier-Haack的邻甲酰化产生二醛55a,b,它在HOAc中与硝基乙烷/NaOAc直接转化成二腈56a,b[Karmarkar等人,Synthesis:510-512(1985)]。在这些条件下我们最近已经甲酰化2-甲基间苯二酚,制备2,4-二羟基-3-甲基苯甲醛,它生成(S)-4,5-二氢-2-(2,4-二羟基-3-甲基苯酚)-4-噻唑羧酸。为完成己配位螯合剂40和41的合成,双腈56a,b在pH6缓冲剂内经D-半胱氨酸(>2等份)反应。

实施例8:用于本发明的化合物
A.化学式(I)的化合物的制备,其中A为CH;R1、R2、R3、R4、R5、R6和R11为H;n为0;p为0;C(O)R为COOH。
2,4-二羟基苄腈按照Ber.Dtsch.Chem.Ges.1981,24,3651的Marcus法制备,如下:
2,4-二羟基苯甲醛(5.0g,36.2mmol)、醋酸钠(5.94g,72.4mmol)、硝基乙烷(5.44g,72.4mmol)和冰醋酸(10ml)的混合物回流6小时。冷却之后,向混合物中灌入冰(100g),并以乙酸乙酯(4×50ml)提取。结合的有机层以饱和NaHCO3洗涤,直至水层pH保持在8,干燥(Na2SO4),真空清除溶剂。闪光色谱法(SiO2,环己烷∶醋酸乙酯-1∶1)制得浅黄色固体的2,4-二羟基苄腈(2.87g,59%)。1H NMR(300MHz,DMSO-d6)δ6.33(d,1H,J=8.6Hz),6.43(s,1H),7.37(d,1H,J=8.6Hz),10.35(s,1H),10.78(s,1H)。IR(KBr)2200cm-1。
D-半胱氨酸盐酸一水合物(6.8g,38.7mmol)加至如上述制备的2,4-二羟基苄腈(3.5g,25.9mmol),在脱气的甲醇(105ml)和0.1M,pH5.95(70ml)磷酸盐缓冲剂中。仔细加入NaHCO3(3.25g,38.7mmol),在70℃于氩气下搅拌54小时。在减压下清除挥发性组分,以1N HCl酸化溶液至pH2。所得的棕色沉淀真空过滤,以水(40ml)和乙醇(20ml)洗涤该固体。在饱和NaHCO3(700ml)溶解粗产物,以乙酸乙酯(2×200ml)水溶液洗涤。水层经细玻璃过滤,并以1NHCl酸化至pH2。沉淀产物真空滤过。水至以乙酸乙酯提取(4×400ml),干燥混合物有机提取物(Na2SO4),真空清除溶剂。与沉淀产物混合的剩余的固体在40℃高真空干燥12小时,制得4,5-二氢-2-(2,4-二羟基苯基)-噻唑-4(S)-羧酸(4.08g,66%),mp266-268℃(误差)(

沿着本实施例A部分的工艺,但以相应的吡啶醛替代2,4-二羟基苯甲醛,相应的吡啶化合物:4,5-二氢-2-(3′,5′-二羟基吡啶-2′-基)-噻唑-4(S)-羧酸。
实施例9:用于本发明的化合物
A.化合物(S)-去甲基desferrithiocin,N-甲基异羟戊酯的制备
苯并三唑-1-基氧代三(二甲基氨基)膦己氟代磷酸盐(BOP)(442.3mg,1.0mmol)加至0℃、(S)-去甲基desferrithiocin溶液(参见实施例1)(224.2mg,1.0mmol)和盐酸N-甲基羟胺(83.52mg,1.0mmol)的甲酰胺溶液中。该混合物于0℃搅拌15分钟,室温下放置过夜。高真空去除溶剂,残留物以乙酸乙酯处理(EtOAc,30ml)。以10ml的饱和NaHCO3、饱和NaCl、10%枸椽酸和饱和NaCl洗涤有机相,通过旋转蒸发清除溶剂。葡聚糖凝胶LH-20柱纯化残留物,以3%EtOH/甲苯洗脱,产生120mg的黄色固体的(S)-去甲基desferrithiocin,N-甲基异羟戊酯(47%):α25-41.3°(c2.34);NMR(CDCl3/d6DMSO)δ3.27(S,3H)3.53(dd,2H,J=9.6),5.70(t,1H,J=9),7.30(D,2H,J=3),8.10(t,1H,J=3),8.10(t,1H,J=3)。分析计算值(C10H11N2O3S):C,47.42,H,4.38;N,16.59。实测值:C,47.66,H,4.41;N,16.45。
实施例10:动物模型
一种疗效筛选为正常的(未负荷铁)插入胆管的Sprague-Dawley大鼠。此模型中,在口服或肠道给予螯合剂后胆汁和尿的收集,允许迅速测定排泄的数量和途径。然而,铁负荷的Cebus apella猴用做较好的筛选。迄今,灵长类模型中该铁螯合剂作用具有数量上可预言的给药后铁清除数量和途径。
活性的主要测量是化合物的疗效,如啮齿动物和灵长类动物模型确定的,并以皮下注射DFO为对照。化合物疗效是有关螯合剂促进的、与理论量相关的铁排泄的相对评价。例如,己配位螯合剂DFO生成1∶1铁络合物,生成常数3×1030M-1[Anderegg等人,Helv.Chim.Acta 46:1409-1422(1963)];如果功效为100%,给予动物的1mmol的DFO会引起1mmol的铁络合物排泄。实际上,当给予人DFO时,仅仅5%计算的铁排泄[Kirking等人,Clin.Pharmacol.10:775-783(1991)]。在DFT类似物的情形下,计算效率基于2∶1络合物的生成以及生成常数假定类似于母体化合物的生成常数,4×1029M-1[Anderegg等人,J.Chem.Soc.,Chem.Commun.1194-1196(1990)]。

实施例11:DESFERRTHIOCIN类物对体内疟原虫的影响
对体内疟原虫的发展阶段和形态学进行研究;显示对细胞毒性的最小有效浓度。目标结构包括核膜、食物泡膜、线粒体蓄积的高电子密度的物质、未消化原料和膨胀的内质网。
为了确定抑制细胞生长和细胞毒性的化合物,该寄生虫暴露于至少两种不同浓度的药物。48小时后清除螯合剂。在两个时间点测定寄生虫血症(48和96h)。各种化合物对疟原虫的影响迥然不同。为了对照,DFO可作为阳性对照。在开始48-h持续暴露在10-5M期间,DFO抑制疟疾发展,该化合物清除后恢得正常的发展。然而,10-4M浓度下,DFO是细胞毒性的;与该化合物48-h培养期内疟原虫死亡。
表I中标记为8和22的化合物在此体外分析中是活性的。可类似试验其它的化合物。
