Abstract

Description
本发明涉及在病人的胃或食道内形成人造口(stoma opening)的装置。该装置包括一可植入的细长节流元件,该元件适合围绕着病人的胃或食道形成一基本上闭合的环,以使该环限定一个节流口(restrictionopening)。
胃带装置形式的这种装置,其中该带环绕病人的一部分胃以限制病人摄入食物,已经在外科上对病态肥胖病人使用,在带的上方形成一个小的胃袋并在胃中形成一减小的人造口。虽然使用这种带环绕胃可以在外科手术中获得最佳的人造口,但是在一些现有的胃带装置中都设有使人造口的尺寸具有较小后处理调整的调整装置。在所有这种现有技术的装置中,例如在美国专利4592339,欧洲专利0611561和国际专利申请WO9427504中所描述的装置中,调整装置主要包括一在带中可膨胀的空腔和与可膨胀空腔中的流体连接的注射口。注射口被皮下植入,通过注射针穿透病人皮肤并进入注射口允许从空腔中加入或取出流体。实际上,带用硅树脂制作,是一种被核准的植入材料,流体是一种与其它常规材料混合的等渗压盐溶液。
如此,病人使用上述现有技术装置的具有可调整人造口类型的胃带的唯一方法是由能够使用注射针从带中取回或增加流体的医生或护士来操作的。然而,现有装置的一个问题是当病人躺在床上睡觉时,可能发生人造口在胃壁附近完全闭合,从而引起病人呕吐或恶心。在这种情况下病人身边通常没有医生。
开始提出的这种装置也用于治疗由于食道裂孔疝而引起的烧心或返流疾病,也就是,胃底下部的胃的一部分立刻向上滑动穿过食道裂缝。结果使胃酸和食物回涌到食道中。在70年代末,根据美国专利3875928中的一种称为Angelchik的一种已有技术假体,用于手术治疗烧心和返流疾病。然而该Angelchik假体有一个主要缺点就是不能在术后调整节流口的大小。还有一个缺点就是由于假体的形状差,它不能令人满意地保护食道和周围区域不受损伤。因而,采用Angelchik假体的操作不再有实用意义。
一种操作技术,半-基底倍增(semi-fundoduplicatio),目前用于治疗烧心和返流疾病。最常用的一种操作尼森-基底倍增(Nissen semi-fundoduplicatio),其中一人抓住胃的底部,绕食道旋转3/4周,在胃部和食道之间进行缝合。尽管该操作进行的相当好,但它仍然存在三个缺点。第一,根据半-基底倍增(semi-fundoduplicatio)进行治疗的大部分病人失去打嗝的能力。第二,许多这类病人吞咽困难,即手术后难以吞咽。第三,不能在手术后以任何方式调整食道或胃的人造口。一天后,这些病人的症状是他们所患疾病的变化而造成的。例如由于胃酸漏进了食道,在夜间许多病人躺下时很难受。
本发明的主要目的是提供一种在病人的胃或食道形成人造口的新型装置,其中该新型装置适合治疗肥胖病人和有烧心和返流疾病的病人。
本发明的另一个目的是提供一种形成人造口的新型方便装置,为了调整人造口可以省去病人看医生或护士的需要从而使病人总是满意的。
据此,本发明提供一种在病人的胃或食道形成人造口的装置,该装置包括一可植入的细长节流元件,该元件适合围绕病人的胃或食道形成一基本上闭合的环,以使得该环限定一个节流口,一可植入的调整装置,用于调整节流元件的环以改变节流口的大小,和一控制装置,用于控制调整装置调整节流元件改变节流口大小。因此当细长节流元件植入病人体内时将类似调整病人的人造口。
根据本发明的优选实施例,该装置进一步包括一可植入的传感器,用于检测至少一个与病人有关的物理参数,特别是病人胃或食道内的压力,或者病人相对于水平方向的方位,其中控制装置响应传感器检测的物理参数的变化来控制调整装置。
控制装置可包括一可植入的内部控制单元,用于响应传感器的信号来直接控制调整装置。可替换地,控制装置可以包括一病人体外的外部控制单元,用于响应传感器的信号来直接或间接控制调整装置。外部控制单元可以存储由传感器检测的物理参数的信息并根据存储的信息手动操作来控制调整装置。
便利地,该装置进一步包括至少一个可植入的发送器,用于发送由传感器检测的物理参数的信息。
根据本发明的具体实施例,传感器包括压力传感器,用于检测病人胃或食道的压力作为物理参数。压力传感器可以是任何公知的或常用的压力传感器,例如示出在美国专利5540731,4846181,4738267,4571749,4407296或3939823中的传感器;或NPC-102医疗血管成形术传感器(MedicalAngioplasty Sensor)。
优选地,控制装置响应传感器检测的胃或食道的压力变化来控制调整装置改变节流口的大小。
在治疗肥胖病人的情况下,根据本发明的一个实施例,控制装置响应压力传感器检测压力在正常的压力范围内,等于和或大于预定值以控制调整装置减小节流口,并响应于压力传感器检测压力低于预定值以增大节流口。预定值最好是正常出现在胃中或者,可替换地,在病人开始进食之后不久食道内的压力。于是到达胃的食物增加了胃中的压力。
在进餐时间之间,例如在晚上,当胃是空的并且所传感的压力正好低于预定的高压力时,调整装置能够保持节流口尽可能的大,即基本上完全张开。结果人造口将相对地大,这可以使人造口完全闭合的风险最小化。所以当肥胖病人吃饭时,进入胃的食物增加了里面的压力,当压力传感器检测预定值时减小节流口。从而相对较少量的进食后肥胖病人的饥饿就得到满足,这将使肥胖病人减轻体重。
在治疗有烧心或返流疾病的病人时,根据本发明的另一个实施例,控制装置响应压力传感器检测压力在正常的压力范围内,等于和或大于预定值以控制调整装置增大节流口,并响应压力传感器检测的压力低于预定值而减小或关闭节流口。预定值最好是正常出现在食道中或者,可替换地,在病人开始进食之后不久胃中的压力。于是到达食道(或胃)的食物增加了食道(胃)中的压力。
在进餐时间之间,例如在晚上,当胃是空的并且所传感的压力正好低于预定的压力,控制装置控制调整装置调整节流装置以关闭节流口,所以,节流装置可以作为人造括约肌来工作。
除了上述与肥胖病人和有烧心和返流疾病的病人的治疗中检测胃或食道的正常压力有关的两个实施例外或可替换的实施例中,控制装置可以响应压力传感器检测压力等于或超过使病人致伤的过度高压力值以控制调整装置来增大节流口。例如,如果一个大块食物在人造口卡住,则在胃或食道中会出现一个使病人致伤的压力,通过增大节流口大块食物能够通过相应的增大了的节流口。
便利地,压力传感器通过检测胃或食道对节流元件施加的压力可以间接检测胃的压力。
根据本发明的另一具体实施例,传感器包括一位置传感器,用于检测病人相对于水平方向的方位来作为物理参数。位置传感器可以是示出在美国专利4942668和5900909中不引起身体排斥的传感器。
在治疗肥胖病人的情况下,控制装置响应位置传感器检测病人基本上处于水平方向也就是病人平躺着以控制调整装置增大节流口。
在治疗有烧心或返流疾病的病人的情况下,控制装置响应位置传感器检测病人基本上处于水平方向也就是病人平躺着以控制调整装置减小或关闭节流口。
可替换地,除了上述的压力传感器外,该装置进一步包括这种位置传感器。
控制装置响应一天内的时间以控制调整装置,并为此它适合包括一定时机构,用于控制调整装置调整节流元件在一天的不同时间段内将节流口保持在不同的尺寸。在提供上述任何类型传感器(压力或位置传感器)的情况下,只要由传感器检测的物理参数没有超过定时机构,则定时机构用来控制调整装置。

在附图1和2中的标号10处示意性地示出了根据本发明的装置用于在肥胖病人的胃(或食道)处形成人造口。装置10包括一细长的节流元件12,例如胃带,用外科手术可环绕着人体的胃14或者—如附图1示出的实施例—环绕着胃14上面的小袋和胃14的下面大部分之间用于形成人造口的胃和食道15植入人体内。细长的节流元件12基本上形成为一闭合环,该环限定一个节流口,例如在附图2中示意性地图解出的位置16处(附图2示出了与附图1中的图解相比有因次的开口16,它被设置成环绕着胃14而不是环绕着食道15)。
在治疗具有烧心和返流疾病的病人情况下,节流元件12没有形成示出在图1中的胃的上面小袋,而是环绕食道或环绕靠近贲门处的胃的上部分。
节流元件10包括一可以是任何合适类型的调整装置。示出在图2中实施例的调整装置包括一与带12成一体并限定内部空腔19的可膨胀元件18。一远离带12的可植入泵20通过一液体管11与带12连接。泵20固定在储液器上,储液器同样也是被植入,盛有液压液,示出在图2的位置21处。液压液是任何适合的不可压缩的流体,如果该流体从储液器21或空腔19中漏出将不会引起严重的疾病或对人体有伤害,例如盐水。通过泵送液压液从储液器21通过导管11进入空腔19,泵20引起元件18膨胀由此减小开口16的大小,而通过泵送液压液从空腔19回到储液器21中,泵20引起元件18收缩,从而使开口16扩大。
泵20由控制单元22控制。而控制单元22可以安装在人体13的外面,在优选实施例中,控制单元22可以安装在人体13的内部,最好安装在细长节流元件12上,并邻近空腔19。在控制单元22和泵20之间提供电互连(未示出)。给控制单元22和泵20供电的电池可以设置在控制单元22的右侧。可替换地,给控制单元22和泵20供电的电源可以安装在身体13的外部。来自外部电源的能量可以无线传送给植入的能量损耗部件。
示意性地示出在图2中的压力传感器23,可植入病人的体内13中用于检测胃14的压力。例如,示出在图2中的实施例,压力传感器23安装在节流元件12上并通过检测胃对可膨胀元件18施加的压力来间接检测胃14的压力。然而,传感器23可以安装在远离空腔19直接靠着胃的细长节流元件12的内侧,或可安装在任何其它合适的位置,只要当病人摄入食物时,压力传感器23能检测由胃引起的压力或相关值。
传感器23可以是任何合适的能实现上面所提到的功能的公知传感器或常规传感器。可植入传感器的非限定性的例子包括那些描述在美国专利5540731,4846181,4738167,4571749,4407296或3939823中的传感器,和NPC-102医疗血管成形术传感器。
可替换地调整装置可以机械调整节流元件12,例如可以设置马达来调整元件12。
控制单元22可以是示意性地示出在图2中的与无线遥控装置24有效通信的类型,其中元件22,24之间的锯齿线表示该两个元件之间的无线通信,并且实心断续线25表示遥控装置24在身体13的外面。遥控装置24可以不用机械地穿透病人而从身体13的外面设置控制单元22的控制参数。由装置24可设置的控制参数之一是传感器23检测的预定压力值并可传送到(或者以电连接或者以无线的方式)控制单元22以使泵20工作并且使液压液从空腔19中移走或泵送进空腔19中。来自遥控装置24的无线能量传送信号可以是电磁信号或声音或其它类型的波,磁性转移或数字脉冲。
在本发明的一个实施例中,控制单元22包括一安装在节流元件12上的定时机构,并且只要压力传感器23检测的压力没有超过预定值(表示胃14中的食物),则定时机构用于控制调整装置来调整节流元件12保持节流口在一天的不同时间段处于不同尺寸。例如在半夜当期望肥胖病人在他/她的胃14中有很少或没有食物时,控制单元22将自动操作泵20来泵送液压液从空腔19进入储液器21中以便使节流口16基本上完全张开,而在一天的其它时间,泵20能够被控制来改变节流口16的尺寸从最大值到最小值。
遥控装置24用来设置定时机构22。
例如,无线遥控装置24可以包括一信号发送器,和一可以被植入身体13内(例如作为控制单元22的一部分)的信号接收器,和一激发器单元,该激发器单元也可以植入到身体13内(例如作为控制单元22的一部分)用于把当信号发射给信号接收器时来自信号的无线能量转化为不同于波能量的能量,例如电能,用于给调整装置(例如通过操作泵20)和传感器供电。可替换地,一电池可植入人体13内(例如作为控制单元22的一部分),用于给调整装置(例如泵)和传感器供电,或者是一蓄电池(例如一电容器)可植入病人体内,用于给调整装置和传感器23供电
本发明也提供了一种使具有装置10的胖人最小化或消除反胃的方法,作为结果人造口16在进餐时间之间基本上关闭。该方法包括:(a)在胖人体内13植入(例如常规外科手术过程)与节流口16操作有关的调整装置和压力传感器23。(b)检测胃14中的压力;和(c)如果响应于(b),测定压力低于预定值(表示胃14中有很少或没有食物),然后控制调整装置使节流口1 6基本上完全张开,以便使反胃现象最小化或基本上消除。当晚上睡觉时胃中的压力在一个通常出现的压力值时,方法(b)和(c)可以基本上实现完全张开节流口16。
该方法进一步包括在步骤(a)实施的同时在人体13内植入控制单元22以便使控制单元22可以安装在节流元件12上或其它与植入有关的位置,并且以非侵入方式在病人体外操作控制单元22来控制调整装置。
此外,该发明提供一种治疗人体病态肥胖的方法,该方法包括:(a)用外科手术方法在人体内植入(最好是使用腹腔镜的手术)一细长节流元件12(参见附图2),该元件基本上形成一环绕着人体的胃14或食道15的闭环,限定一节流口16。(b)用外科手术方法在人体内植入一用于调整节流口16的调整装置,和一用于检测人体胃14压力的压力传感器23。(c)响应压力传感器23检测人体胃内的压力大于预定值时,则控制调整装置减小节流口16的尺寸。和(d)响应压力传感器23检测人体胃内压力小于预定值,则控制调整装置基本上完全张开节流口16。该方法也包括(e)控制调整装置响应一天的时间(例如用上面所描述的定时机构,例如作为控制单元22的一部分)来改变节流口16,除非超过由传感器23检测的胃中压力从而引起人造口小于完全张开时的尺寸,或者“闭合”(也就是有一个基本上阻止食物颗粒进一步进入胃中的最小尺寸)。
附图3示出了一个类似于附图2的实施例,同等的部件仅通过在同一参考标号前面加上“1”来表示。在附图3的实施例中,然而,两个小室26,27通过隔壁28以密封的方式来互相分离,并且泵20泵送流体从一个小室26到另一个小室27来改变节流口116的尺寸。传感器123可以是常规的位置传感器,检测病人相对于水平方向的方位。泵120和传感器123都固定到小室26,27里面的隔壁28上。常规锁定元件30以成形环的方式用来容纳细长节流元件112并通过线31与传感器123和泵120连接。在其它的实施例中,控制单元122可以包括一内部锁定机构,并可以通过无线遥控装置从人体外部被控制(象附图2中的遥控装置24)。如果需要则也可以包括一压力传感器23。
还可使用可替换的或另外的传感器,它们可以检测其它与植入有该传感器的人的睡眠或休息有关的物理参数。
虽然本发明在这里已经描述和示出了治疗肥胖病人的优选实施例,但是对本领域普通技术人员来说是可以理解的,对有烧心和返流疾病的病人来说也能使用所描述的装置10来治疗,并且在本发明的范围内可以作出各种变形,其范围与所附的权利要求的最宽解释一致以包含所有等同的装置和方法。
Claims (47)
1、一种用于在病人的胃或食道形成人造口的装置,该装置包括
一可植入的细长节流元件,该元件形成为一环绕着病人的胃或食道的基本上闭合的环,以使该环限定一个节流口,
一可植入的调整装置,用于调整成环状的节流元件的环以改变节流口的大小,和
一控制装置,用于控制调整装置调整节流元件来改变节流口的大小。
2、根据权利要求1的装置,进一步包括一可植入的传感器,用于检测至少一个与病人有关的物理参数,其中控制装置响应传感器检测到的物理参数的变化来控制调整装置。
3、根据权利要求2的装置,其中控制装置包括一可植入的内部控制单元,用于响应来自传感器的信号以直接控制调整装置。
4、根据权利要求2的装置,其中控制装置包括一病人体外的外部控制单元,用于响应来自传感器的信号以控制调整装置。
5、根据权利要求4的装置,其中外部控制单元响应来自传感器的信号以直接控制调整装置。
6、根据权利要求4的装置,其中外部控制单元存储由传感器检测的物理参数的信息并根据存储的信息手动操作以控制调整装置。
7、根据权利要求2-6中任何一项权利要求的装置,进一步包括至少一可植入的发送器,用于发送由传感器检测的物理参数的信息。
8、根据权利要求2-7中任何一项权利要求的装置,其中传感器包括一用于检测病人的胃或食道的压力作为物理参数的压力传感器。
9、根据权利要求8的装置,其中压力传感器通过检测胃对节流元件施加的压力来间接检测胃的压力。
10、根据权利要求8或9的装置,其中控制装置响应传感器检测胃或食道的压力的变化来控制调整装置以改变节流口的大小。
11、根据权利要求10的装置,其中该装置限制肥胖病人摄入食物,控制装置响应压力传感器检测压力等于或大于预定值以控制调整装置来减小节流口。
12、根据权利要求10或11的装置,其中该装置限制肥胖病人摄入食物,控制装置响应压力传感器检测压力低于预定值以控制调整装置来增大节流口。
13、根据权利要求10-12中任何一项权利要求的装置,其中该装置限制肥胖病人摄入食物,控制装置响应压力传感器检测压力等于或大于一过高的值以控制调整装置来增大节流口。
14、根据权利要求10的装置,其中该装置用于治疗烧心或返流疾病,控制装置响应压力传感器检测压力等于或大于预定值以控制调整装置来增大节流口。
15、根据权利要求10或14的装置,其中该装置用于治疗烧心或返流疾病,控制装置响应压力传感器检测的压力低于预定值以控制调整装置来减小或闭合节流口。
16、根据权利要求10,14和15的任何一项权利要求的装置,其中该装置治疗烧心或返流疾病,控制装置响应压力传感器检测压力等于或大于一过度高的值以控制调整装置来增大或完全张开节流口,以避免在胃或食道中出现致伤的高压力。
17、根据权利要求2-7中任何一项权利要求的装置,其中传感器包括一位置传感器,用于检测病人相对于水平方向的方位作为物理参数。
18、根据权利要求8-16中任何一项权利要求的装置,进一步包括一位置传感器,用于检测病人相对于水平方向的方位。
19、根据权利要求17或18的装置,其中该装置限定肥胖病人摄入食物,控制装置响应位置传感器检测病人基本上处于水平方位以控制调整装置来增大节流口。
20、根据权利要求17或18的装置,其中该装置治疗烧心或返流疾病,控制装置响应位置传感器检测病人基本上处于水平方位以控制调整装置来关闭节流口。
21、根据权利要求1的装置,其中控制装置响应一天的时间以控制调整装置。
22、根据权利要求21的装置,其中控制装置包括一定时机构,该定时机构用于控制调整装置调整节流元件保持节流口在一天的不同时间段内处于不同的尺寸。
23、根据权利要求2-7中任何一项权利要求的装置,其中控制装置包括一定时机构,只要由传感器检测的物理参数没有超过定时机构,该定时机构用于控制调整装置以调整节流元件保持节流口在一天的不同时间段内处于不同的尺寸。
24、根据权利要求1-23中任何一项权利要求的装置,其中控制装置包括一可植入的内部控制单元,并进一步包括一无线遥控装置,当内部控制单元植入病人体内时,在不用机械穿透病人的情况下,无线遥控装置设置来自病人体外的内部控制单元的控制参数。
25根据权利要求2或24的装置,其中至少一个可由无线遥控设置的控制参数是物理参数。
26、根据权利要求24或25的装置,其中内部控制单元包括一定时机构,该定时机构用于控制调整装置调整节流元件保持节流口在一天的不同时间段内处于不同的尺寸。
27、根据权利要求26的装置,其中无线遥控装置适于设置定时机构。
28、根据权利要求1的装置,其中控制装置包括一无线遥控装置。
29、根据权利要求28的装置,其中无线遥控装置包括一信号发送器,一可植入的信号接收器,和一可植入的激发器单元,用于把当信号从信号发送器到信号接收器时来自信号的无线能量转化为不同于供给调整装置的无线能量的能量。
30、根据权利要求2或29的装置,其中来自无线转换的能量用于供给植入的传感器。
31、根据权利要求1的装置,其中调整装置包括一节流元件中的可膨胀空腔,节流口的尺寸能随空腔的膨胀而减小并随空腔的压缩而增加,和一储存有液压液的储液器,调整装置将来自储液器的液压液供给空腔以使空腔膨胀,并将来自空腔的液压液供给储液器以压缩空腔,从而改变节流口的大小。
32、根据权利要求31的装置,其中储液器作为节流元件的附件。
33、根据权利要求31的装置,其中储液器是固定到节流元件上的。
34、根据权利要求31的装置,其中储液器与节流元件是成一体的。
35、根据权利要求31-34中任何一项权利要求的装置其中调整装置包括一泵,用于在空腔和储液器之间泵送流体。
36、根据权利要求35的装置,其中泵作为节流元件的附件。
37、根据权利要求35的装置,其中泵是固定到节流元件上的。
38、根据权利要求35的装置,其中泵与节流元件形成一整体。
39、根据权利要求35的装置,其中储液器,泵,传感器和节流元件形成一个整体。
40、根据权利要求35的装置,其中泵适于在病人皮下远离节流元件的位置植入。
41、根据权利要求1-30中任何一项权利要求的装置,其中调整元件适于机械调整节流元件。
42、根据权利要求1-41中任何一项权利要求的装置,进一步包括一用于给调整装置供电的可植入的电池。
43、根据权利要求1-41中任何一项权利要求的装置,进一步包括一用于给调整装置供电的可植入的蓄电池。
44、根据权利要求43的装置,其中蓄电池包括一电容器。
45、根据权利要求2-20,25和30中任何一项权利要求的装置,进一步包括一用于给传感器供电的可植入的电池。
46、根据权利要求2-20,25和30中任何一项权利要求的装置,进一步包括一用于给传感器供电的可植入的蓄电池。
47、根据权利要求46的装置,其中蓄电池包括一电容器。
