技术领域
本发明属于医学检验技术领域,具体涉及一种代谢标志物的检测试剂在制备乳腺癌筛查和预后产品中的应用。
背景技术
最新的全球癌症统计报告(GLOBOCAN)报道,在女性人群中乳腺癌(BreastCancer,BC)占比已超过肺癌居所有癌症首位,2020年新发病例大约230万例,占全部新发癌症病例的11.7%,且女性乳腺癌已成为癌症死亡第五大原因,占全部癌症死亡病例的6.9%。目前,乳腺癌的筛查主要是通过乳腺X线和超声检查的方法。乳腺癌的诊断则需要结合患者临床表现、体格检查、影像学检查和组织病理学诊断等进行综合鉴别诊断。其中组织病理学诊断是乳腺癌确诊和治疗的重要依据,但是该技术方法需要对患者乳腺组织进行侵入性的活检。同时影像学上的筛查,也是基于组织形态学上的改变,常常使得检查位于癌症转移或是中晚期阶段,不能发挥癌症早期以及快速诊断的作用。因此,能够特异、快速、无创的筛查和诊断乳腺癌在疾病预防和临床治疗中显得尤为重要。
液体活检(Liquid Biopsy),不仅为疾病的诊断和评估提供无创伤和最小疾病检测的方法,而且还能实时监测疾病进展和提供治疗疗效等信息,是一种可能替代组织活检的检测方法。目前,血液样本的液体活检已被广泛用于检测实体瘤和免疫系统疾病,但是基于尿液样本液体活检的潜力尚未得到充分挖掘。
代谢组学(Metabolomics),是一种测量生物体内代谢过程所有小分子产物的方法,是对独特细胞生理过程中的代谢谱进行的研究。尿液代谢组学主要是通过核磁共振或质谱(Mass Spectrometry,MS)对尿液中小分子进行广泛鉴定与分析,并筛选出特异性较高的生物标志物。目前已有多项研究表明,尿液中差异性代谢产物具有作为肿瘤早期筛查与诊断标志物分子的巨大潜力。
有鉴于此,发掘更多特异性高的乳腺癌生物标志物是本发明要解决的技术问题。
发明内容
为了解决上述技术问题,本发明提供了一种代谢标志物的检测试剂在制备乳腺癌筛查和预后产品中的应用。
实现本发明的具体方案如下:
一种代谢标志物的检测试剂在制备乳腺癌筛查和预后产品中的应用,所述代谢标志物为3,4,5,6,7,8-六氢-2(1H)-萘酮,PubChem CID为572673;
所述检测试剂用于检测样品中3,4,5,6,7,8-六氢-2(1H)-萘酮的含量。
进一步的,所述样品为受试者的尿液。
更进一步的,通过气相色谱/质谱分析的方法在尿液中检测3,4,5,6,7,8-六氢-2(1H)-萘酮的含量。
更进一步的,所述检测试剂包括H2SO4和内标物,所述内标物为4-methylpentan-2-ol的标准品,PubChem CID为7910。

进一步的,所述试剂盒中,所述H2SO4和4-methylpentan-2-ol分别以水溶液的形式存在,H2SO4的浓度为5M,4-methylpentan-2-ol的浓度为0.8mg/mL。
本发明具有如下有益效果:
本发明研究首次发现,相对正常健康人对照,乳腺癌患者的尿液中3,4,5,6,7,8-六氢-2(1H)-萘酮的量发生显著升高,可作为尿液标志物筛查受试者是否患有乳腺癌,评价乳腺癌治疗效果。ROC分析发现该标志物的特异度79.4%、灵敏度73.2%、AUC值=0.822、Cut-off值=27984569。该结果为乳腺癌的早期筛查、动态监测和预后判断提供特异性的、迅速的的检测手段,具有广泛的临床应用价值。本发明试剂盒中的检测试剂可单次与受试者尿液混合,而同时高精度的定量检测多种代谢标志物,本发明的测试样品取样方便,对病人无任何痛苦,本发明的问世,将为乳腺癌的筛查和预后起到积极的作用。

具体实施方式
下面结合附图和具体实施例对本发明进行详细说明,但不应理解为本发明的限制。如未特殊说明,下述实施例中所用的技术手段为本领域技术人员所熟知的常规手段,下述实施例中所用的材料、试剂等,如无特殊说明,均可从商业途径得到。
实施例1:乳腺癌生物标志物的筛选
1.材料和方法
1.1.研究人口和设计
共入选75例女性研究对象,均来自湖南省郴州市第一人民医院,其中34例健康对照者和41例乳腺癌患者(luminal A型17例、luminal B型6例、HER2阳性型10例和三阴型8例),其中,Ⅰ期5例、Ⅱ期27例、Ⅲ期9例,健康对照组(Control)和乳腺癌组(BC)之间年龄、血压、吸烟和药物使用等临床信息方面均无统计学差异。具体为采样之前告知研究对象并签署知情同意书,采集晨起第1次尿液的中段尿10mL,为减少饮食对尿液代谢产物的影响,要求研究对象在采集前1天避免高盐高脂及刺激性饮食,且采集前晚20点以后禁食。
1.2.样品采集和处理
尿液样本采集后,5000×g离心30min,去除沉淀。取250μL尿液,加50μL的5M H2SO4,加3μL的0.8mg/mL内标物(4-methylpentan-2-ol,PubChem CID 7910),充分混匀。
将前处理样品放入萃取瓶中,密封后置于固相微萃取(SPME)装置上。萃取头为50/30μm DVB/CAR/PDMS纤维萃取头(Supelco,USA)。25℃平衡10min,萃取前将进样口于250℃老化3min至无杂峰。60℃萃取40min。萃取完成后,在进样口250℃解析1min,进行后续分析。
1.3非靶向代谢组学分析
方法:使用AB Sciex公司TripleTOFTM 5600+飞行质谱系统的电喷雾离子源及正负离子模式,并通过高能诱导裂解技术进行二次裂解,并通过动态排除法去除非必要数据信息。条件:正/负离子喷雾电压分别为3.4kV和2.5kV,温度为325℃,扫描模式为全扫描,扫描范围为35~475m/z。
通过ProteoWizard(Version 3)软件的XCMS程序对质谱数据进行峰识别、过滤和对齐操作后,获得质荷比、保留时间和峰面积等信息。将质荷比和保留时间在标准数据库中进行比对,获得具体代谢化合物名称。对峰面积进行归一化处理后,再通过PCA方法进行数据降维处理。通过OPLS-DA模型进行VIP值计算,通过Mann-Whitney U检验对健康对照组和乳腺癌组之间差异代谢化合物进行筛选。
结果:通过数据库匹配、数据标准化、计量学以及统计学分析等发现,与正常健康人群相比,乳腺癌患者尿液中165种代谢物发生显著性变化(p<0.05),按照VIP>1&FC>1.5&P<0.05的标准,筛选出12个差异代谢物(表1)。

1.3靶向气相色谱/质谱(GC/MS)分析
仪器采用赛默飞Q Exactive GC Orbitrap高分辨质谱系统(ThermoFisher,USA),色谱柱采用DB-5ms(60m×0.25mm×0.25μm)毛细管色谱柱(Agilent,USA)。色谱柱升温程序为初始温度70℃,保持1min,以12℃/min升至100℃,以2℃/min升至180℃,保持5min,再以12℃/min升至280℃,保持2min。其中进样口温度为250℃,载气为氦气(He),流速为1mL/min,分流比为1:5。
质谱仪为EI电离源,离子源温度为250℃,扫描模式为Full MS-SIM,扫描范围为35~475m/z,溶剂延迟0min。
1.4结果判读
质谱仪在完成自动峰识别、对齐和校正等过程,输出包含化合物名称、保留时间、精确m/z值和峰面积等参数的数据。采用SPSS(Version 23,IBM,USA)软件,将12种化合物相应峰面积以平均数±标准差(Mean±SD)进行表示,并使用t检验对Control和BC组尿液该化合物相对含量进行统计学分析,p<0.05代表具有显著性差异。再采用SPSS软件进行接受者操作特征(Receiver Operating Characteristic,ROC)曲线分析,计算出该化合物指示乳腺癌的特异度(Specificity)、灵敏度(Sensitivity)、AUC(Areas Under the Curve,AUC)值以及临界(Cut-off)值。
2.结果
如图1所示,与Control组相比,BC组尿液中代谢产物3,4,5,6,7,8-六氢-2(1H)-萘酮的相对量显著上调。如图2所示,34例健康对照者和41例乳腺癌患者的尿液质谱检测3,4,5,6,7,8-六氢-2(1H)-萘酮的峰面积结果分别为22620606±31995898和77543432±73071516。以3,4,5,6,7,8-六氢-2(1H)-萘酮筛查乳腺癌的特异度79.4%、灵敏度73.2%、AUC值=0.822、Cut-off值=27984569,ROC曲线如图2所示。这表明通过质谱检测尿液中3,4,5,6,7,8-六氢-2(1H)-萘酮可以进行乳腺癌筛查,特异度和灵敏度较高,具有乳腺癌广泛筛查的巨大潜力。
因此,待测尿液样本通过质谱检测出3,4,5,6,7,8-六氢-2(1H)-萘酮的峰面积≥27984569,则待测对象为乳腺癌患者;若测尿液样本通过质谱检测出3,4,5,6,7,8-六氢-2(1H)-萘酮的峰面积<27984569,则待测对象为非乳腺癌患者。以上乳腺癌筛查标准仅供参考。
实施例2:评价差异代谢产物对乳腺癌治疗效果的监测(预后)作用
在签署知情同意书后,收集33例乳腺癌患者治疗后1个月和22例乳腺患者治疗后3个月的尿液样本。两组患者之间年龄、血压、吸烟和药物使用等临床信息方面均无统计学差异。样本采集、处理、检测和分析过程同实施例1。33例乳腺癌患者治疗后1个月和22例乳腺患者治疗后3个月的尿液质谱检测3,4,5,6,7,8-六氢-2(1H)-萘酮的峰面积结果分别为36889413±49339311和31649317±39039063。如图3所示,与乳腺癌患者治疗前相比,乳腺癌患者治疗后1和3个月尿液中代谢产物3,4,5,6,7,8-六氢-2(1H)-萘酮的相对量显著下调,且与健康对照者相比无统计学差异。这表明质谱检测尿液中3,4,5,6,7,8-六氢-2(1H)-萘酮可以评价乳腺癌治疗效果,进而动态、实时的监测乳腺癌的治疗疗效,具有广泛的临床应用价值。

Claims (6)
1.一种代谢标志物的检测试剂在制备人类乳腺癌筛查和预后产品中的应用,其特征在于,所述代谢标志物为3,4,5,6,7,8-六氢-2(1H)-萘酮,PubChem CID为572673;
所述检测试剂用于检测样品中3,4,5,6,7,8-六氢-2(1H)-萘酮的含量;
所述样品为人类受试者的尿液。
2.根据权利要求1所述的应用,其特征在于,通过气相色谱-质谱法在尿液中检测3,4,5,6,7,8-六氢-2(1H)-萘酮的含量。
3.根据权利要求2所述的应用,其特征在于,所述检测试剂包括H2SO4和内标物,所述内标物为4-methylpentan-2-ol的标准品,PubChem CID为7910。
4.根据权利要求3所述的应用,其特征在于,所述H2SO4和4-methylpentan-2-ol分别以水溶液的形式存在,H2SO4的浓度为5M,4-methylpentan-2-ol的浓度为0.8mg/mL。
5.根据权利要求4所述的应用,其特征在于,所述人类乳腺癌筛查和预后产品为试剂盒。
6.根据权利要求5所述的应用,其特征在于,所述试剂盒包括所述检测试剂。
