胰腺癌进展通常以明显的促结缔织增生反应(包括成纤维细胞、免疫细胞和致密的细胞外基质)为特征。转化生长因子-β(TGF-β)途径是已被鉴定为该疾病的发病机理的主要贡献因子的信号传导系统之一(Truty和Urrutia,2007)。由于高度纤维化的肿瘤微环境,常规化疗和放疗在胰腺肿瘤中仅具有中度的抗肿瘤活性。类似地,在其他癌症类型中高度有效的免疫疗法(诸如α-PD-1疗法)已被证明在胰腺癌中无效。因此,迫切需要新的胰腺癌治疗方法。
现在越来越多的证据支持溶血磷脂酸(LPA)在调控胰腺癌发生、进展和转移中的生理作用(Chen等人,2021)。LPA是一种生物活性磷脂,其与至少六种受体LPAR1-6接合,这些受体各自与参与各种细胞活性诸如细胞迁移、增殖和分化的不同G蛋白偶联。LPA存在于各种生物流体中,并且其在血浆中的水平在其在凝血中的作用方面得到了很好的表征。
LPA由细胞外lysoPLD自分泌运动因子(autotaxin,ATX)(也称为外核苷酸焦磷酸酶/磷酸二酯酶2(ENPP2))从溶血磷脂酰胆碱(LPC)生成。已报道,在包括胰腺癌在内的多种癌症中ATX表达增加。已报道,胰腺癌患者的胰腺组织中ATX和LPA均过表达,并因此ATX-LPA轴可代表胰腺癌中的潜在靶标。
在胰腺癌治疗中,对靶向疗法的需求日益增加。靶向疗法针对存在于肿瘤中的特异性受体或酶,并且不像传统治疗方法如化疗那样损害健康组织。
发明内容
本发明涉及用于在胰腺癌的治疗中使用的化合物。所述化合物是ATX抑制剂。本发明人认识到ATX抑制剂在用于治疗胰腺癌的靶向疗法中可能有用。
在第一方面,本发明提供了一种式I的化合物:
或其药学上可接受的盐,其用于在治疗患者的胰腺癌中使用。所述式I的化合物在本文中可称为“化合物1”。
术语胰腺癌包括任何外分泌或神经内分泌胰腺癌类型。在一些情况下,其中所述胰腺癌是胰腺导管腺癌(PDAC)。PDAC是最普遍的胰腺肿瘤疾病,占所有胰腺恶性肿瘤的90%以上。
合适地,化合物1以包含化合物1或其药学上可接受的盐和药学上可接受的稀释剂、载剂或赋形剂的药物组合物施用。优选地,但不是必须地,所述药物组合物适合口服施用。
在一些情况下,所述治疗是组合疗法并且包含一种或多种另外的化疗剂和/或TGF-β途径抑制剂。本发明人已经观察到,化合物1与批准的化疗剂一起施用既改善了肿瘤生长抑制又降低了在单独的化疗剂中观察到的副作用。
例如,所述方法可包括施用治疗有效量的另外的化疗剂,任选地两种另外的化疗剂。合适的化疗剂包括吉西他滨和白蛋白结合型紫杉醇。因此,本发明的方法可包括施用吉西他滨和/或白蛋白结合型紫杉醇。
本发明人已经观察到,当施用TGF-β途径抑制剂时,改善了治疗结果,特别是在患者生存方面。合适的TGF-β途径抑制剂可包括加鲁色替(galunisertib)、伐托色替(vactosertib)、LY3200882和AVID200。

现在将讨论本发明的各个方面和实施方案。其他方面和实施方案对于本领域技术人员来说将显而易见。本文中提及的所有文件均通过引用并入本文。
化合物1
WO2016124939描述了各种ATX抑制剂化合物及其在治疗其中涉及ATX活性的增殖性病症中的用途,包括化合物1。
化合物1是WO2016124939中的实施例40,该文献全文通过引用并入本文。WO2016124939描述了超过200个实施例。化合物1的结构根据化学式I。
其IUPAC名称为N-[(S)-1-(4-氯-苯基)-乙基]-3-[3-(4-三氟甲氧基-苄基)-3H-咪唑并[4,5-b]吡啶-2-基]-丙酰胺。其合成和表征分别描述于WO2016124939第77页和第82页,所述信息通过引用明确并入本文。
化合物1可作为游离碱或作为药学上可接受的盐提供和施用。在一些情况下,化合物1作为游离碱提供和施用。
制剂
合适地,化合物1以被配制用于口服施用的药物组合物的形式提供。所述药物组合物可以胶囊的形式提供或者可以片剂的形式提供。在一些情况下,其以片剂的形式提供。在其他情况下,其以胶囊的形式提供,例如作为硬壳或软壳胶囊(例如,羟甲基纤维素(HPMC)胶囊)内的粉末状或颗粒状组合物或液体组合物。换句话说,口服剂型是优选的。
所述制剂合适地包含一种或多种药学上可接受的填充剂、崩解剂、助流剂和/或润滑剂。
使用化合物1治疗胰腺癌
化合物1是ATX抑制剂。ATX是治疗胰腺癌的一个有吸引力的靶标,因为它在细胞外起作用并在多个水平上刺激癌症生长、存活和转移。
本领域公认LPA-ATX途径在胰腺癌中经常被激活。因此,本发明的方法可涉及以ATX-LPA途径的上调为特征的胰腺癌的治疗。因此,本发明的方法可涉及通过调节患者的ATX-LPA途径来治疗胰腺癌。
所述胰腺癌可以是任何外分泌或神经内分泌胰腺癌类型。因此,本发明的方法涉及胰腺癌的治疗,诸如但不限于胰腺导管腺癌(PDAC)和胰腺神经内分泌肿瘤(PanNET或PNET)。在一些情况下,所述胰腺癌是胰腺导管腺癌。在一些情况下,所述胰腺癌是胰腺神经内分泌肿瘤。
本发明的方法
如下文更详细地描述,本发明人惊奇地发现,化合物1在胰腺癌的临床前模型中显示出强大的抗肿瘤活性,并且耐受性良好。因此,可以提供在胰腺癌中具有抗肿瘤活性和有利的安全特性的ATX抑制剂。
另外,本发明人惊奇地发现,化合物1增加了标准护理化疗诸如吉西他滨的抗肿瘤活性。因此,可以提供可以增加胰腺癌中的化疗功效的ATX抑制剂。
因此,在一些情况下,本发明的方法涉及组合疗法,所述组合疗法包括用化合物1或其药学上可接受的盐和另外的化疗剂(例如吉西他滨或白蛋白结合型紫杉醇/>)治疗患者。
应当理解,化合物1和另外的化疗剂将合适地(尽管不是必须地)在不同的时间和/或以不同的方案给予,并且可被配制用于通过不同的途径施用。例如,化合物1或其药学上可接受的盐可作为口服剂量(例如,每日口服剂量)给予,而另外的化疗剂可作为输注物给予。例如,吉西他滨和白蛋白结合型紫杉醇可在28天周期中在第1、8和15天给予。
在一些情况下,所述组合疗法包括用化合物1或其药学上可接受的盐和吉西他滨和白蛋白结合型紫杉醇治疗患者。
此外,本发明人已发现,化合物1、加鲁色替(TGF-β途径抑制剂)和化疗的三联组合在临床前胰腺癌模型中表现出对结果的进一步改善。在一些情况下,所述方法包括施用TGF-β途径抑制剂。合适的TGF-β途径抑制剂可包括加鲁色替(galunisertib)、伐托色替(vactosertib)、LY3200882和AVID200。
因此,在一些情况下,本发明的方法涉及组合疗法,所述组合疗法包括用化合物1或其药学上可接受的盐、TGF-β途径抑制剂诸如加鲁色替(LY2157299一水合物,EliLilley)和另外的化疗剂(例如吉西他滨或白蛋白结合型紫杉醇)治疗患者。
应当理解,化合物1和TGF-β途径抑制剂另外的化疗剂将合适地(尽管不是必须地)在不同的时间和/或以不同的方案给予,并且可被配制用于通过不同的途径施用。
因此,本发明人惊奇地发现,可以用化合物1在有或没有化疗以及有或没有TGF-β途径抑制剂的情况下治疗胰腺癌,并且这种治疗的耐受性良好。这为胰腺癌的治疗提供了新的单一疗法和组合疗法选择。
化合物1的剂量可每天一次(QD)、优选地每天两次(BID)提供,优选地但不是必须地口服施用。可使用其他施用方法。合适的日剂量可为5mg至2g,例如10mg至1g。在一些情况下,当化合物1以组合疗法施用时,化合物1的施用在施用其他药剂的暂停期间(例如,在28天化疗周期的第21-28天期间)继续。

实施例1
胰腺癌中的ATX(ENPP2)表达
使用TCGA(癌组织)和GTEX(正常组织)数据将人胰腺癌样品中的ENPP2(编码自分泌运动因子的基因)表达与正常胰腺组织进行比较。发现肿瘤样品中的ENPP2表达是正常组织中的1.85倍(参见图1和表1)。

实施例2
化合物1在原位mPA6115-luc小鼠模型中的活性
在胰腺癌的原位mPA6115-luc小鼠模型中评价化合物1的治疗功效。该模型通过将来源于KPC小鼠中自发形成的供体肿瘤的mPA6115-luc细胞植入雌性野生型C57BL/6受体小鼠的胰腺中来构建。该模型中的病理学和肿瘤微环境与人类胰腺癌非常相似,并且以有限的免疫细胞浸润为特征。
在每只小鼠的胰腺被膜下区域接种50uL含Matrigel的PBS(1:1)中的mPA6115-luc肿瘤细胞(1x106个)。将肿瘤细胞接种的日期表示为第0天。
在肿瘤细胞接种后4天基于总通量(p/s,最小通量>1E6)开始随机分组。将每组10只小鼠分配为通过BID口服强饲媒介物(1%甲基纤维素)或化合物1(在1%甲基纤维素中为10mg/kg)进行治疗。每周两次通过生物发光成像对肿瘤生长和转移进行成像。
如图2中可以看出,基于总生物发光,用化合物1治疗mPA6115-luc荷瘤小鼠显示出肿瘤生长显著减少。
实施例3
化合物1在异种移植PANC-1小鼠模型中的活性
在胰腺癌的皮下PANC-1小鼠模型中评价化合物1的治疗功效。
在BALB/c裸小鼠的右前胁区皮下接种0.1ml PBS中的PANC-1肿瘤细胞(5x106个)。将肿瘤细胞接种的日期表示为第0天。当平均肿瘤大小>100mm3时,开始随机分组。每个研究组纳入10只小鼠。每周两次测量肿瘤生长和体重。
如图3中所见,用化合物1治疗PANC-1荷瘤小鼠显示出肿瘤生长减少。
实施例4
化合物1加吉西他滨在原位PANC-1小鼠模型中的活性
在胰腺癌的原位PANC-1小鼠模型中评价有或没有吉西他滨的情况下化合物1的治疗功效。该模型通过将PANC-1肿瘤细胞植入BALB/C裸鼠的胰腺细胞中来构建。
在每只小鼠的胰腺被膜下区域接种50uL含Matrigel的PBS(1:1)中的PANC-1肿瘤细胞(3x106个)。将肿瘤细胞接种的日期表示为第0天。在肿瘤细胞接种后10天基于体重开始随机分组。将每组10只小鼠分配为通过口服强饲媒介物(1%甲基纤维素,BID)、吉西他滨(25mg/kg,Q4D)、化合物1(10mg/kg,BID)或化合物1(10mg/kg,BID)和吉西他滨(25mg/kg,Q4D)进行治疗。在第42天终止后,通过重量测量肿瘤大小。
如图4所描绘,单独用吉西他滨治疗携带PANC-1荷瘤小鼠显示出41%的肿瘤生长抑制,但治疗的耐受性差,并且5只小鼠死亡或被处死以达到人道终点。单独用化合物1治疗产生5%的适度肿瘤生长抑制,然而化合物1和吉西他滨的组合不仅产生47%的改善肿瘤生长抑制,还将吉西他滨治疗的死亡率降低到仅两只小鼠。还参见表2。

实施例5
化合物1加吉西他滨和/或加鲁色替在原位RC416小鼠模型中的活性
在胰腺癌的原位RC416小鼠模型中评价有或没有吉西他滨和/或加鲁色替的情况下化合物1的治疗功效。该模型通过将来源于KPC小鼠中自发形成的供体肿瘤的RC416细胞植入雌性野生型C57BL/6受体小鼠的胰腺中来构建。该模型中的病理学和肿瘤微环境与人类胰腺癌非常相似,并且以高循环ATX和TGF-β为特征。
在每只小鼠的胰腺被膜下区域接种RC416肿瘤细胞。将肿瘤细胞接种的当天表示为第0天。在肿瘤细胞接种后7天基于体重开始随机分组。将每组5只小鼠分配为用媒介物(1%甲基纤维素,BID p.o.)、化合物1(10mg/kg,BID p.o.)、加鲁色替(50mg/kg,BIDp.o.)、吉西他滨(75mg/kg,QW i.p.)或化合物1加吉西他滨、化合物1加加鲁色替和吉西他滨以及加鲁色替加吉西他滨的组合进行治疗。最多治疗小鼠28天并且通过存活测量抗肿瘤活性。
图5中描绘的结果表明,单独用吉西他滨治疗产生的存活益处有限。然而,令人惊讶的是,吉西他滨加化合物1或加鲁色替的组合使得存活几乎加倍。甚至更令人惊讶的是,所有三种治疗的组合进一步增强了总存活,2只小鼠存活超过65天。
