Abstract

技术领域

背景技术
癌症是仅次于心血管疾病的第二大死因,每年死亡人数超过600万,预计未来几十年新发病例数将大幅增加,到2040年全球新发病例将比2020年增加近五成。癌症的发生和发展是一个长期的过程,诱发癌症发生的因素有多种,一般为遗传因素、环境因素、饮食和生活方式。近年来,研究发现,在许多血液肿瘤和恶性实体瘤中存在染色质修饰物和重塑物的缺陷,这证实了表观遗传改变在癌症病因学中的作用。表观遗传改变保留了完整的DNA结构,但可以在不影响DNA结构的情况下,通过改变染色质的结构,转录因子的可及性,介导癌基因的转录上调来驱动恶性肿瘤,它们可以出现在癌症发展过程中的任何时间。越来越多的研究证明表观遗传学与肿瘤病人的诊断与治疗有关。靶向表观遗传修饰的表观遗传疗法成为了肿瘤治疗的新兴方法。
辛二酰苯胺异羟肟酸(SAHA)是美国食品药品监督管理局(FDA)批准使用的一种泛组蛋白去乙酰化酶抑制剂,可以阻断I类和II类组蛋白去乙酰化酶,分子量很小(<300),分子式为C14H20N2O3,SAHA可以通过抑制HDAC的活性来降低乙酰化组蛋白和非组蛋白的累积、抑制细胞的增殖和肿瘤生长,被广泛应用于临床实体瘤(如乳腺癌)的治疗中,但近期的临床床试验显示SAHA单独使用时疗效不佳,且易产生明显的副作用和耐药性。以SAHA为基础的联合用药方案或将成为未来实体瘤治疗的主要方向。
LLY-507是一种高效的蛋白质赖氨酸甲基转移酶SMYD2的选择性抑制剂,分子式为C36H42N6O,LLY-507可以有效地抑制SMYD2对组蛋白H3和H4的甲基化。LLY-507对SMYD2的选择性比对其他24种蛋白或DNA甲基转移酶高100倍(包括相关的家族成员SMYD3,SUVH420H1,SUV420H2)。而SMYD2在多种肿瘤细胞中被发现有异常高表达,是肿瘤恶性特性的关键分子标志物。目前有研究证明LLY-507可以以剂量依赖方式抑制胃癌、卵巢癌、乳腺癌等多种肿瘤细胞的增殖、迁移和侵袭,促进细胞的凋亡。
为了解决SAHA单独使用疗效不佳和耐药性的问题,探索靶向表观遗传修饰的联合用药方法,对表观遗传修饰异常的肿瘤患者的治疗有非常重要的意义,具有很好的临床应用前景。
发明内容
本发明所要解决的技术问题为:提供一种靶向表观遗传修饰的联合用药,提高SAHA的抗肿瘤效果,降低副作用。

本发明提供了一种组蛋白去乙酰化酶抑制剂与组蛋白甲基转移酶抑制剂联合用药物组合物,本发明发现,将组蛋白去乙酰化酶抑制剂辛二酰苯胺异羟肟酸(SAHA)和组蛋白甲基转移酶抑制剂LLY-507联合使用能显著抑制肿瘤细胞的增殖、迁移、侵袭,并促进肿瘤细胞凋亡,其作用效果显著优于二者单独使用,经金式公式评价二者联合使用的Q值,结果显示二者联用具有协同增效作用。本发明还发现,这种协同增效作用主要是通过RASAL1-Ras-Raf-1轴抑制Ras信号通路来实现的。
本发明的辛二酰苯胺异羟肟酸(SAHA)与LLY-507联用,不仅提供了更多用药安全性,而且降低了辛二酰苯胺异羟肟酸(SAHA)的临床使用剂量,可减少大剂量使用辛二酰苯胺异羟肟酸(SAHA)所产生的毒副作用,提高临床治疗安全指数,降低耐药性。因此,本发明为表观遗传修饰异常的肿瘤临床治疗提供了一种全新的、高效的联合用药方案,具有良好的临床应用前景。

具体实施方式
下述实施例中的实验方法,如无特殊说明,均为常规方法。下述实施例中所用的试验材料,如无特殊说明,均为从商业渠道购买得到的。
实施例1:辛二酰苯胺异羟肟酸(SAHA)和组蛋白甲基转移酶抑制剂LLY-507最佳联用浓度的确定
取对数生长期AGS和MGC-803胃癌细胞制成单细胞悬液,在显微镜下进行细胞计数,按照5000个/孔的浓度接种于96孔板中,次日细胞贴壁后加入不同药物处理。处理方式为:SAHA(1.0μM和2.0μM)与LLY-507(2.5μM、5.0μM、1.0μM或8.0μM)分别联合作用胃癌细胞48h,在时间终点加入CCK-8溶液并置于细胞培养箱内继续孵育1小时,用酶标仪测定不同处理组在波长450nm处的吸光值,计算细胞活力和药物抑制率。根据金式公式来评价药物体外联用是否有协同作用。
Q=E(a+b)/(Ea+Eb-Ea·Eb),其中E(a+b)为两药联用的抑制率,Ea为A药单用的抑制率,Eb为B药单用的抑制率。若Q值在0.85~1.15之间表示两药相加,Q>1.15表示两药产生协同,Q<0.85表示两药产生拮抗。
实验结果如图1和表1,当1.0μM SAHA与5.0μM LLY-507联合作用时,AGS和MGC-803细胞的Q值分别为1.23和2.01,表明在此浓度下,SAHA和LLY-507有协同抑制胃癌细胞增殖的作用。且该研究发现,两药联用降低了辛二酰苯胺异羟肟酸(SAHA)的使用剂量(SAHA对AGS和MGC-803的IC50分别为1.55μM和1.74μM),可减少大剂量使用SAHA所产生的毒副作用,提高临床治疗安全指数,降低耐药性。

实施例2:辛二酰苯胺异羟肟酸(SAHA)和组蛋白甲基转移酶抑制剂LLY-507联合用药对胃癌细胞增殖的影响
取对数生长期的AGS和MGC-803胃癌细胞,吹打成单细胞悬液,按照1000个/孔的浓度接种于6孔板中,待细胞贴壁24h后,细胞分别用0.1%DMSO、SAHA(1.0μmol/L)、LLY-507(5.0μmol/L)、SAHA(1.0μmol/L)+LLY-507(5.0μmol/L)处理48h,更换新鲜的完全培养基继续培养12天,期间每3天更换一次培养基。待出现肉眼可见的克隆且显微镜下可见单个细胞集落大于50个细胞(大小在0.1~1.0mm之间)时,终止培养。弃培养液,使用PBS润洗2次,加入4%多聚甲醛固定20min。PBS清洗2次后,加入结晶紫染液,室温染色30min。加入PBS反复冲洗后,晾干。拍照计数并计算克隆形成率。
实验结果如图2所示,AGS细胞经SAHA和LLY-507联合处理48h后,其相对克隆形成率约为(34.31±9.96)%,显著低于SAHA或LLY-507单独处理组(其克隆形成率分别为(84.53±7.39)%和(93.96±9.52)%)。同样地,在MGC-803细胞中也获得了相似的实验结果,即:SAHA和LLY-507联用对MGC-803细胞克隆形成能力的抑制效果最佳。表明SAHA和LLY-507联用可显著提高SAHA对胃癌细胞增殖的抑制作用,起到协同增效的作用。
实施例3:辛二酰苯胺异羟肟酸(SAHA)和组蛋白甲基转移酶抑制剂LLY-507联合用药对胃癌细胞迁移的影响
取对数生长期的AGS和MGC-803胃癌细胞,吹打成单细胞悬液,按照1.5×105个/孔的浓度接种于6孔板中,待细胞贴壁24h后,分别用0.1%DMSO、SAHA(1.0μmol/L)、LLY-507(5.0μmol/L)、SAHA(1.0μmol/L)+LLY-507(5.0μmol/L)处理48h。弃掉培养液,用无菌200μL枪头垂直板底划痕。加入PBS清洗漂浮细胞,重复2-3次,弃PBS后更换无血清培养基。利用倒置显微镜在不同时间段拍照(0h、48h),记录药物处理组和对照组细胞伤口愈合情况。
实验结果如图3所示,与对照组相比,SAHA或LLY-507单独处理可显著降低胃癌细胞AGS和MGC-803的伤口愈合能力,即迁移能力,而SAHA和LLY-507联用可显著提高SAHA或LLY-507单用对胃癌细胞迁移的抑制作用,起到协同增效的作用。
实施例4:辛二酰苯胺异羟肟酸(SAHA)和组蛋白甲基转移酶抑制剂LLY-507联用对胃癌细胞侵袭的影响
取对数生长期的胃癌细胞AGS和MGC-803,接种于6孔板中(1.5×105个/孔),待细胞贴壁24h后,分别加入含0.1%DMSO、SAHA(1.0μmol/L)、LLY-507(5.0μmol/L)、SAHA(1.0μmol/L)+LLY-507(5.0μmol/L)的新鲜培养基,于37℃培养箱中孵育48h,用Transwell技术评估各处理组细胞侵袭的差异。具体方法为:a)取稀释好的Matrigel胶50μL缓慢沿小室壁铺于上室,置于37℃细胞培养箱,待胶完全凝固。b)在24孔板中加入500μL含20%FBS的完全培养基,将小室放入24孔板中。收集上述不同药物处理的细胞,用无血清培养基制成单细胞悬液(密度为2.5×105个/mL),取200μL细胞悬液接种于上室,于37℃、5%CO2培养箱中培养24h。c)取出小室,用PBS淋洗小室2-3次,加入500μL预冷的4%多聚甲醛于下室,滴加一滴于上室,固定20min。d)弃掉固定液,PBS淋洗小室2-3次,用棉签轻轻擦净上室接种侧细胞,晾干小室,加入1×结晶紫染液染色30min。e)弃染色液,PBS淋洗小室2-3次,晾干小室。倒置显微镜拍照,并用Image J计数进入下室的细胞量并计算细胞相对侵袭率。
实验结果如图4所示:AGS和MGC-803细胞经不同药物处理48h后,SAHA+LLY-507处理组中透过基质胶穿孔至下室的细胞数量均显著低于SAHA或LLY-507单独处理组;其中在AGS细胞SAHA或LLY-507单独处理组的侵袭率为对照组的(58.57±2.65)%和(54.13±3.14)%;MGC-803细胞SAHA或LLY-507单独处理组的侵袭率为对照组的(59.89±2.90)%和(53.48±2.11)%;而两种细胞SAHA+LLY-507处理组的侵袭率低至对照组的(34.17±1.65)%和(26.74±2.84)%。表明:SAHA和LLY-507联用能更有效地抑制胃癌细胞的侵袭。
实施例5:辛二酰苯胺异羟肟酸(SAHA)和组蛋白甲基转移酶抑制剂LLY-507联用对胃癌细胞凋亡的影响
取对数生长期的胃癌细胞AGS和MGC-803,分别接种于6孔板中(2×105个/孔),待细胞贴壁24h后,分别加入含0.1%DMSO、SAHA(1.0μmol/L)、LLY-507(5.0μmol/L)、SAHA(1.0μmol/L)+LLY-507(5.0μmol/L)的新鲜培养基,于37℃培养箱中孵育48h。收集各组全部细胞(包括培养上清中的细胞),用250μL 1×Binding Buffer重悬,并加入2.5μL Annexin V-FITC和5μL PI。轻柔涡旋混匀后,室温避光孵育5min。使用BD流式细胞分析仪检测不同处理组细胞的凋亡率。
结果如图5、6、7和8所示,AGS细胞经SAHA、LLY-507和SAHA+LLY-507处理48h后,其凋亡率分别为(19.09±1.12)%、(8.40±0.67)%和(38.03±1.27)%,显然,SAHA和LLY-507联合处理组的凋亡率显著高于SAHA或LLY-507单独处理组。同时,SAHA和LLY-507联用也可有效诱导胃癌细胞MGC-803的凋亡,表明SAHA和LLY-507联用确实能更加有效地诱导胃癌细胞的凋亡,起到协同增效的作用。
实施例6:辛二酰苯胺异羟肟酸(SAHA)和组蛋白甲基转移酶抑制剂LLY-507联合在体内抑制MGC-803移植瘤生长
构建MGC-803细胞的裸鼠移植瘤模型,并随机分为4组,即对照组(给与DMSO)、SAHA给药组(25mg/kg.day)、LLY-507给药组(150mg/kg.day)、SAHA(25mg/kg.day)与LLY-507(150mg/kg.day)联合给药组。给药方式为腹腔注射,连续给药20天。每天监测裸鼠状态和肿瘤的体积,在实验终点用颈椎脱臼法处死小鼠,剥离肿瘤组织并称重。
实验结果如图9所示,与对照组相比,在对荷瘤小鼠体重无明显影响的情况下,SAHA、LLY-507单独使用均能有效抑制裸鼠体内肿瘤的生长(体积和重量),SAHA和LLY-507联用进一步减少了肿瘤的体积和重量,发挥协同抑制肿瘤生长的效果,瘤体平均重量仅为对照组的38%。
实施例7:LLY-507的药代动力学特征
使用在线工具ADMETlab(http://admet.scbdd.com/)分析LLY-507的吸收、分布、代谢和排泄等药代动力学特征,结果显示LLY-507具有高生物利用性、低药物相互作用潜力和相对低的副作用,表明LLY-507具有作为口服活性抑制剂的潜力(表2)。

Claims (10)
1.联合用药物在制备抗肿瘤药物中的用途,所述联合用药物为辛二酰苯胺异羟肟酸或其药学上可接受的盐与LLY-507或其药学上可接受的盐。
2.根据权利要求1所述的用途,其特征在于,肿瘤为表观遗传修饰异常的肿瘤。
3.根据权利要求1所述的用途,其特征在于,所述肿瘤为胃癌。
4.根据权利要求1所述的用途,其特征在于,所述抗肿瘤是指预防、缓解或治疗肿瘤。
5.根据权利要求1所述的用途,其特征在于,所述辛二酰苯胺异羟肟酸或其药学上可接受的盐与LLY-507或其药学上可接受的盐的摩尔比为1∶5或2∶2.5或2∶5。
7.一种药物组合物,其特征在于,含有有效量的辛二酰苯胺异羟肟酸或其药学上可接受的盐和有效量的LLY-507或其药学上可接受的盐。
8.根据权利要求7所述的药物组合物,其特征在于,所述辛二酰苯胺异羟肟酸或其药学上可接受的盐与LLY-507或其药学上可接受的盐的摩尔比为1∶5或2∶2.5或2∶5。
9.根据权利要求7所述的药物组合物,其特征在于,所述药物组合物中还包括药学上可接受的载体或辅料。
