Abstract
The invention discloses a traditional Chinese medicine composition for treating acute gouty arthritis, a preparation method and application thereof, wherein the active ingredients of the traditional Chinese medicine composition are mainly prepared from the following raw materials in parts by weight: 10-30 parts of gardenia, 10-20 parts of coptis chinensis, 10-20 parts of frankincense, 10-20 parts of rhizoma paridis, 10-20 parts of Indian hemp, 8-15 parts of radix trichosanthis, 5-15 parts of achyranthes root, 5-15 parts of dahurian angelica root, 5-15 parts of pubescent angelica root, 5-15 parts of raw pollen typhae, 3-10 parts of rhubarb, 3-10 parts of lightyellow sophora root and 3-10 parts of borneol. The traditional Chinese medicine composition disclosed by the invention has an obvious curative effect on acute gouty arthritis, and is free from toxic and side effects and adverse reactions.
Description
Technical Field
The invention belongs to the field of traditional Chinese medicines, and particularly relates to a traditional Chinese medicine composition for treating acute gouty arthritis, a preparation method and application thereof.
Background
Along with the change of life style of people, the prevalence of gout has a remarkable rising trend, and is more and more paid attention to the medical community.
Gout is a genetically related group of clinical syndromes that are long-term disorders of purine metabolism or uric acid excretion, leading to sustained increases in blood uric acid and deposition of urate crystals, causing tissue damage. The clinical characteristics of gout are hyperuricemia, repeated attacks of gouty acute arthritis, tophus deposition, tophus chronic arthritis and joint deformity caused by the hyperuricemia, and chronic interstitial nephritis and uric acid kidney stone formation caused by kidneys are often involved.
The traditional Chinese medicine for treating gout in the prior art mainly comprises oral administration and external application, and the external application medicine mostly achieves the aim of treating gout by clearing heat, detoxicating, activating blood and dissolving stasis. The medicine with definite curative effect and high safety is urgently needed to be applied to clinic.

The invention also provides application of the traditional Chinese medicine composition in preparing a medicine for treating acute gouty arthritis.
The invention also provides a traditional Chinese medicine preparation for treating acute gouty arthritis, which comprises the traditional Chinese medicine composition for treating acute gouty arthritis, and auxiliary materials or medicinal carriers.
The Chinese medicinal preparation is an external preparation, such as a cataplasm.
The invention also provides a preparation method of the traditional Chinese medicine preparation for treating acute gouty arthritis, which comprises the following steps:

Compared with the prior art, the invention has the beneficial effects that:
the components of the traditional Chinese medicine composition act synergistically, especially the gain effect of rhizoma paridis, rhizoma polygoni multiflori, radix trichosanthis and raw pollen typhae on basic compositions is remarkable in curative effect on acute gouty arthritis, and pain of patients is relieved or eliminated within 3 days and the shortest time is 24 hours, so that the traditional Chinese medicine composition is free from toxic and side effects and adverse reactions.
Meanwhile, according to the unexpected discovery of the extraction mode in the preparation process, the paris polyphylla, the eight-edge hemp, the radix trichosanthis and the raw pollen typhae are extracted separately from other raw materials, so that the drug effect can be further improved.

The present invention will be described more fully hereinafter with reference to the accompanying drawings, in which preferred embodiments are shown, to facilitate understanding of the invention, but the scope of the invention is not limited to the specific embodiments described.
Unless defined otherwise, all technical and scientific terms used hereinafter have the same meaning as commonly understood by one of ordinary skill in the art. The terminology used herein is for the purpose of describing particular embodiments only and is not intended to be limiting of the scope of the present invention.
Unless otherwise specifically indicated, the various raw materials, reagents, instruments, equipment and the like used in the present invention are commercially available or may be prepared by existing methods.
According to the principle of treatment based on syndrome differentiation, the original prescription of the invention is cape jasmine, coptis root, frankincense, achyranthes root, dahurian angelica root, pubescent angelica root, rhubarb, kuh-seng and borneol, which has a certain curative effect on acute gouty arthritis, but has low efficiency within 3 days and long treatment period. After continuous research and improvement, rhizoma paridis, rhizoma polygonati, radix trichosanthis and raw pollen typhae are added to finally become the formula of the invention.
The active ingredients of the traditional Chinese medicine composition for treating acute gouty arthritis in some specific embodiments of the invention are mainly prepared from the following raw materials in parts by weight: 10-30 parts of gardenia, 10-20 parts of coptis chinensis, 10-20 parts of frankincense, 10-20 parts of rhizoma paridis, 10-20 parts of Indian hemp, 8-15 parts of radix trichosanthis, 5-15 parts of achyranthes root, 5-15 parts of dahurian angelica root, 5-15 parts of pubescent angelica root, 5-15 parts of raw pollen typhae, 3-10 parts of rhubarb, 3-10 parts of lightyellow sophora root and 3-10 parts of borneol.
Preferably, the active ingredients are prepared from the following raw materials in parts by weight: 18-25 parts of gardenia, 12-18 parts of coptis chinensis, 12-18 parts of frankincense, 12-18 parts of rhizoma paridis, 12-18 parts of rhizoma polygoni multiflori, 10-15 parts of radix trichosanthis, 8-12 parts of radix achyranthis bidentatae, 8-12 parts of radix angelicae pubescentis, 8-12 parts of raw pollen typhae, 5-8 parts of rheum officinale, 5-8 parts of radix sophorae flavescentis and 5-8 parts of borneol.
In the present invention: gardenia jasminoides ellis, fructus Gardeniae, having effects of clearing heat and cooling blood. The frankincense is mainly used for regulating qi and activating blood, relieving pain, expelling toxin and treating qi and blood stagnation, carbuncle, sore and swelling. Paris polyphylla is bitter and slightly cold, and has the effects of clearing heat and detoxicating, and relieving swelling and pain. The Indian hemp is plant of Sambucus genus of Caprifoliaceae family, perennial shrub herb, and has effects of dispelling pathogenic wind, promoting blood circulation, removing blood stasis and relieving pain. Radix trichosanthis, has effects of clearing heat and purging pathogenic fire, and relieving swelling and expelling pus. Achyranthes root has the functions of removing blood stasis, dredging channels, tonifying liver and kidney, strengthening tendons and bones and guiding blood down. Radix Angelicae Dahuricae has effects of eliminating disease and dampness, expelling pus, promoting granulation, promoting blood circulation, and relieving pain. Radix angelicae pubescentis is slightly warm in nature and has the effects of dispelling wind and removing dampness, relieving arthralgia and stopping pain, and belongs to wind-damp-dispelling and cold-dispelling herbs. Raw pollen Typhae has cool nature and smooth nature, has effects of promoting blood circulation and removing blood stasis, and can be used for treating pain due to qi and blood stagnation. The main functions of the rheum officinale are as follows: purging heat toxin, removing accumulation and stagnation, and promoting blood stasis. Ku Shen has the actions of clearing heat, drying dampness and inducing diuresis because of its bitter and cold nature.
Wherein, cape jasmine, coptis root, indian hemp and the like are monarch drugs, clear heat and remove dampness, activate blood and reduce swelling; olibanum, rhizoma paridis and the like are ministerial drugs for clearing heat, removing blood stasis and relieving pain; radix Trichosanthis, radix Achyranthis bidentatae, radix Angelicae Dahuricae, radix Angelicae Pubescentis, pollen Typhae, radix et rhizoma Rhei, and radix Sophorae Flavescentis, etc. have effects of clearing heat, promoting diuresis, promoting blood circulation, removing blood stasis, and relieving pain; the borneol is used as a guiding drug, can be used as a transdermal drug and is also a good transdermal absorption promoter. The medicines are used together, and have the effects of clearing heat, promoting diuresis, removing blood stasis and relieving pain.
The preparation method of the traditional Chinese medicine composition comprises the steps of reflux-extracting other raw materials except borneol by adopting an ethanol water solution (preferably 60-90% ethanol water solution) for 1-2 hours, filtering, and concentrating filtrate into thick paste. Then making into external preparation such as paste or patch, such as cataplasma. The cataplasm is prepared by mixing Borneolum Syntheticum, cataplasm matrix and soft extract, spreading on backing material, and cutting.
The ethanol aqueous solution is adopted for extraction, so that the effective components which are soluble in water or ethanol are extracted effectively, and compared with pure water or ethanol extraction, the prepared composition has better curative effect.
The inventor accidentally found that if the first medicinal liquid is extracted from rhizoma paridis, radix trichosanthis and raw pollen typhae, the second medicinal liquid is extracted from the rest raw materials except borneol, and the first medicinal liquid and the second medicinal liquid are mixed and concentrated into thick paste, the separate extraction has better curative effect than the total mixed extraction, and the inventor deduces that the effect is influenced by the fact that a certain effective component in the rhizoma paridis, radix trichosanthis and raw pollen typhae has chemical action with a certain effective component in the rest raw materials under the reflux condition. The concentration is carried out under reduced pressure at a temperature of not higher than 45 ℃.
The cataplasm matrix of the invention can adopt the matrix material disclosed in CN1137730C, and the invention specifically adopts the matrix material in the embodiment 1. The matrix is not easy to age, and has the advantages of water retention, air permeability and comfortable adhesion.
Example 1
Raw materials: 20g of gardenia, 15g of coptis chinensis, 15g of frankincense, 15g of rhizoma paridis, 15g of Indian hemp, 12g of radix trichosanthis, 10g of achyranthes root, 10g of dahurian angelica root, 9g of pubescent angelica root, 9g of raw pollen typhae, 6g of rhubarb, 6g of lightyellow sophora root and 6g of borneol.
Pulverizing the above materials according to weight ratio, percolating rhizoma paridis, herba Ardisiae Japonicae, radix Trichosanthis and pollen Typhae with 90% ethanol for 1 hr, and filtering to obtain first medicinal liquid. Percolating fructus Gardeniae, coptidis rhizoma, olibanum, achyranthis radix, radix Angelicae Dahuricae, radix Angelicae Pubescentis, radix et rhizoma Rhei and radix Sophorae Flavescentis with 90% ethanol for 1 hr, and filtering to obtain second liquid medicine. Mixing the first liquid medicine and the second liquid medicine, concentrating under reduced pressure (45 ℃) to thick paste with the relative density of 1.2-1.3, adding borneol, uniformly mixing, adding the mixed thick paste into a cataplasm matrix, mixing and evenly stirring, coating on a backing material, and cutting to obtain the cataplasm.
Example 2
The starting materials were the same as in example 1.
Pulverizing the raw materials according to the weight ratio, percolating the raw materials except borneol for 90% ethanol for 1 hour, concentrating to obtain thick paste with the relative density of 1.2-1.3, adding borneol, uniformly mixing, adding the mixed thick paste into a cataplasm matrix, stirring and mixing uniformly, coating on a mounting material, and cutting to obtain the cataplasm.
Example 3
Raw materials: gardenia jasminoides fruit 22g, coptis root 18g, frankincense 18g, paris polyphylla 18g, hemp seed 18g, radix trichosanthis 15g, achyranthes root 12g, dahurian angelica root 12g, pubescent angelica root 12g, pollen typhae 12g, rhubarb horsetails 8g, kuh-seng 8g and boneol 8g.
The preparation method is the same as in example 1.
Example 4
Raw materials: 18g of gardenia, 12g of coptis chinensis, 12g of frankincense, 12g of rhizoma paridis, 12g of Indian hemp, 10g of radix trichosanthis, 9g of achyranthes root, 8g of dahurian angelica root, 9g of pubescent angelica root, 9g of raw pollen typhae, 6g of rhubarb, 6g of lightyellow sophora root and 6g of borneol.
The preparation method is the same as in example 1.
Comparative example 1
The difference from embodiment 1 is that a building is not added. The preparation method is the same as in example 1.
Comparative example 2
The difference from example 1 is that no octagon was added. The preparation method is the same as in example 1.
Comparative example 3
The difference from example 1 is that no radix Trichosanthis was added. The preparation method is the same as in example 1.
Comparative example 4
The difference from example 1 is that no raw pollen Typhae was added. The preparation method is the same as in example 1.
Preliminary studies on the initial formulation, the inventive formulation and the comparative formulation (comparative examples above) were carried out by pharmacological/toxicological experiments (rats, rabbits), proved to be effective (inhibition of plantar swelling in rats) and free of toxic side effects (no abnormalities in the course of one week), and finally clinical experiments were carried out voluntarily by volunteers (clinical study of the initial formulation was carried out first). A summary of some of the clinical trials is set out below.
Clinical study experiment
(1) Diagnostic criteria:

Patients with acute gouty arthritis were selected according to the above criteria, and were aged between 18 and 75 years.
(2) Efficacy evaluation criteria:

Invalidation: after the traditional Chinese medicine provided by the invention is used, the pain sensation of joints and acute red swelling are not relieved after 3 days.
(3) The using method comprises the following steps: the cataplasm of the above examples and comparative examples was applied to the joint pain part once a day for 8 to 12 hours each time.
(4) Results: 50 gouty arthritis patients in each example group voluntarily participated in the experiment, and 20 gouty arthritis patients in each comparative example group voluntarily participated in the experiment. The specific results are shown in Table 1 below:

The results show that the traditional Chinese medicine composition has obvious curative effect on acute gouty arthritis, and the pain of a patient is relieved or eliminated within 3 days and the shortest time is 24 hours, so that the traditional Chinese medicine composition has no toxic or side effect and adverse reaction. The comparative example shows that the addition of rhizoma paridis, rhizoma Aristolochiae, radix Trichosanthis and pollen Typhae plays an important role. Examples 1 and 2 show that the medicine effect can be further improved by separately extracting paris polyphylla, rhizoma polygoni multiflori, radix trichosanthis and raw pollen typhae from other raw materials.
The foregoing is merely a preferred embodiment of the present invention and is not intended to limit the present invention in any way. Therefore, any simple modification, equivalent variation and modification of the above embodiments according to the technical substance of the present invention shall fall within the scope of the technical solution of the present invention.

The traditional Chinese medicine composition for treating acute gouty arthritis is characterized by comprising the following active ingredients in parts by weight: 10-30 parts of cape jasmine, 10-20 parts of coptis chinensis, 10-20 parts of frankincense, 10-20 parts of rhizoma paridis, 10-20 parts of Indian hemp, 8-15 parts of radix trichosanthis, 5-15 parts of achyranthes root, 5-15 parts of dahurian angelica root, 5-15 parts of pubescent angelica root, 5-15 parts of raw pollen typhae, 3-10 parts of rhubarb, 3-10 parts of lightyellow sophora root and 3-10 parts of borneol.
