本发明公开了一种控释型狂犬疫苗可溶性微针,包括不含药的针尖、含药针体和背衬,含药针体包括载有狂犬疫苗的双层微球和基质材料,双层微球系内外两层均由高分子聚合物构成的载体,外层材料与内层材料的重量比为2:1。本发明所制得的控释型狂犬疫苗可溶性微针,针体机械强度高,足以刺入皮肤,突释量较低,可以持久的进行控释释放,降低了狂犬疫苗注射次数,并且与传统注射剂相比,减少注射痛苦,提高患者依从性。

技术领域
本发明涉及狂犬疫苗的给药技术领域,具体涉及一种具有控释作用的狂犬疫苗可溶性微针递送系统。
背景技术
狂犬病是由狂犬病毒属病毒引起的一种人畜共患急性传染病,发病后通常导致急性脑炎或脑膜炎,病死率几乎100%。目前人类狂犬病主要分布在亚洲、非洲、拉丁美洲和加勒比地区,99%发生在发展中国家。亚洲狂犬病病例居世界第一,每年约有3万人死亡。及时接种疫苗是预防狂犬病发生和传染的有效途径。
狂犬疫苗需要注射多次才能达到抵抗病毒所需的抗体水平。尽管美国免疫咨询实践委员会将接种程序由五针法改进为四针法,但还是具有患者依从性差等缺点。因此减少疫苗的注射次数成为疫苗领域的研究趋势之一。
2018年WHO公布了狂犬疫苗的WHO立场文件,在人用狂犬疫苗的接种方式上首先推荐皮内注射。皮肤是机体重要的免疫器官,与肌肉注射和皮下注射相比,皮内接种使用较少的抗原量就可以诱导强烈的免疫应答,为狂犬疫苗接种提供了一种节约成本和剂量的方案,多个临床研究也证实了采用皮内注射接种狂犬疫苗在暴露后预防中的有效性,因此,WHO认为皮内接种在简化疫苗接种程序、降低治疗费用和提高方案可行性等方面更具优势(Organization W H.Rabies vaccines:WHO position paper,April 2018–Recommendations[J].Vaccine,2018,36:5500-5503.)。
微针给药是近年来发展的一种新型皮内给药系统,已在病毒疫苗、DNA疫苗及细菌疫苗接种等领域开展研究,发现其在免疫效果、疫苗稳定性及节省剂量等方面有较大优势,具有广阔的市场前景。可溶性微针具有更高的生物相容性,不产生物危害废物,优势更明显。单一材料制备的可溶性微针常存在机械性能较差等缺点,选择合适的复合材料制备微针能够达到较好的力学性能([1]占浩慧,黄颖聪,马凤森.单一或复合高分子材料用于载药可溶性聚合物微针的制备[J].材料导报,2019,33(10):1738-1744.)。
文献(Rabies vaccination in dogs using a dissolving microneedle patch)提出一种将狂犬疫苗载入可溶性微针进行给药的方法。但也存在微针中的狂犬疫苗含量低等缺点,需要通过进一步优化疫苗设计或其他方法来产生更好的免疫效果。
微球是一种新型的给药技术,可以通过调节、控制药物的释放度实现长效控释、缓释、靶向作用,而且同时能保护药物的效能不受破坏(尤其是多肽和蛋白质类药物),降低药物的不良气味和口味,明显减少给药次数,降低毒性,提高产品的疗效,明显提高了病患用药的依从性、方便性,在目前的临床应用上已经看出具有明显的优势,非常具有潜力(王玉静,孙秀芝,徐丽,洪燕龙.微球制剂的研究进展及前景分析[J].生物化工,2017,3(02):80-83.)。
文献(Immunostimulatory effect of tetanus toxoid loaded chitosannanoparticles following microneedles assisted immunization)提出一种破伤风疫苗负载的壳聚糖纳米颗粒与中空微针相结合的给药方式,也证明了微针与疫苗微球结合给药的可能性。微针和微球的结合产生了持久的体液和细胞免疫,又使微球包裹的抗原获得持续释放的特性。虽然微球与微针结合具有以上优点,但微球制剂的药物突释现象一直是难以解决的问题。突释效应造成在短时间内释放出大量的药物,使体内的血药水平陡然升高,产生不良反应。最终削弱微球制剂的缓释作用。因此,突释现象已成为微球控释系统研究者面临的一个急待解决的问题。
为解决微球制剂的突释现象,引入了带有被无药壳层包围的药物包封颗粒核的双层微球(Xu Q,Chin S E,Wang C H,et al.Mechanism of drug release from double-walled PDLLA(PLGA)microspheres[J].Biomaterials,2013,34(15):3902-3911.)。简而言之,双层微球就是在传统单层微球的周围包裹一层无药壳层,通过内层微球和外层壳层相互作用来达到控制突释的效果。
对于内层微球来说影响其释放速率的原因有很多,包括药物的性质,聚合物的性质等。疫苗作为一种特殊的蛋白质药物其物种类繁多,性质也存在很大差异。文献(Theinfluence of protein solubilisation,conformation and size on the burstrelease from poly(lactide-co-glycolide)microspheres)中提到蛋白质的水溶性影响PLGA微球的初始释放速率。文献(乳酸/羟基乙酸共聚物的分子量及其单体组成比例对利培酮微球性质的影响)中提到对于同种药物不同组分比的乳酸-羟基乙酸共聚物(PLGA)微球对其初始释放速率的影响不同。因此要根据药物的性质选择合适聚合物材料。
对于外层壳层来说,其聚合物的种类和内外层聚合物质量比影响药物的释放速率(Berkland C,Cox A,Kim K,et al.Three-month,zero-order piroxicam release frommonodispersed double-walled microspheres of controlled shell thickness[J].journal of biomedical materials research part a,2004,70A(4):576-584.)。当双层微球内外层聚合物质量比很小时,并不能很好解决突释,当内外层质量比过大又会阻碍药物释放影响治疗效果。
因此,双层微球选择何种材料以及内外层选择何种比例才能解决突释现象,并有良好的控释效果具有不确定性。
综上所述,如何将双层微球与可溶性微针结合开发出一种能减少狂犬疫苗注射次数并且具有控释作用的狂犬疫苗可溶性微针贴片,是本领域技术人员急需解决的技术难题。
发明内容
本发明要解决的技术问题在于,通过选择适宜的种类及含量的双层微球材料负载狂犬疫苗,解决了上述现有技术存在的不足之处。
为此,本发明采取了以下技术方案:
一种控释型狂犬疫苗可溶性微针,包括不含药的针尖、含药针体和背衬,含药针体包括载有狂犬疫苗的双层微球和基质材料,双层微球系内外两层均由高分子聚合物构成的载体,外层材料与内层材料的重量比为2:1。
优选的,双层微球中,外层材料选自不同聚合度的Glu-PLGA(端羟基聚(乳酸-羟基乙酸)共聚物)、PDLLA(聚D,L-丙交酯)、PLA(聚乳酸)中的一种,内层材料选自不同聚合度的PLGA(聚乳酸-羟基乙酸共聚物)、POE(聚原酸酯)中的一种。
更优选的,双层微球中,外层材料为Glu-PLGA,内层材料为PLGA(50:50)。
优选的,基质材料选自至少一种高分子聚合物,基质材料与载有狂犬疫苗的双层微球的含量比为(5:3)~(2:1)。
更优选的,基质材料与载有狂犬疫苗的双层微球的含量比为5:3。
更优选的,基质材料为PVP与PVA的组合,PVP与PVA的含量比为(5:1)~(1:1)。
更优选的,基质材料为含量比为5:1的PVP与PVA的组合。
本发明还提供了一种控释型狂犬疫苗可溶性微针的优选处方,其含药针体中各组分的重量百分比如下:载有狂犬疫苗的双层微球占33.3%~37.5%,PVA占10.4%~11.1%、PVP占52.1%~55.6%。
本发明还提供了一种控释型狂犬疫苗可溶性微针的最佳处方,其含药针体中各组分的重量百分比如下:载有狂犬疫苗的双层微球占37.5%、PVA占10.4%、PVP占52.1%。
与现有技术相比,本发明提供的狂犬疫苗可溶性微针,有益效果如下:
①本发明通过选取合适的材料,制备成不含药物的针尖和含有药物微球的针体,防止粒径较大的双层微球影响微球的机械强度,保证了微针的机械性能,使微针能够刺入皮肤。
②本发明利用带有被无药壳层包围的狂犬疫苗包封颗粒核的双层微球,与单层微球相比,突释量得到更好的控制,保证后续给药的剂量。
③本发明所制得的控释型狂犬疫苗可溶性微针,微针和双层微球的结合既产生了持久的体液和细胞免疫,双层微球包裹的抗原又具有持续释放的特性,有效地增加了药物释放时间让机体持续产生更多的抗体,从而可以降低给药频次,增加患者的依从性。
综上所述,本发明所制得的控释型狂犬疫苗可溶性微针,针体机械强度高,足以刺入皮肤,突释量较低,可以持久的进行控释释放,降低了狂犬疫苗注射次数,并且与传统注射剂相比,减少注射痛苦,提高患者依从性。

具体实施方式
以下是本发明的具体实施例,对本发明的技术方案做进一步的描述,但是本发明的保护范围并不限于这些实施例。凡是不背离本发明构思的改变或等同替代均包括在本发明的保护范围之内。
实施例1控释型狂犬疫苗可溶性微针的制备
(1)狂犬疫苗双层微球的制备
本发明采用W1/O1/O2/W2法合成负载狂犬疫苗的双层微球。制备过程如下:使用均质机将适量狂犬疫苗原液与含有内层微球材料的二氯甲烷溶液乳化,所得的乳液继续与含有外层微球材料的二氯甲烷溶液乳化;将最终得到的乳液注入0.5%(w/v)PVA水溶液中,使用机械搅拌器在800r/min下室温搅拌8h使有机溶剂蒸发。洗涤、离心,循环三次使杂质去除;冷冻干燥24h,得到载有狂犬疫苗的微球冻干粉。
(2)狂犬疫苗可溶性微针的制备
采用光刻和蚀刻工艺制备微针阳模,将聚硅氧烷(PDMS)与固化剂按质量比10:1混合后,浇注到放有微针阳模的长方形容器中;将容器放置在真空干燥箱,抽真空除去混合液中的气泡;再放入烘箱,干燥后去除,获得聚硅氧烷阴模。
利用上述微针阴模分三段入模制备微针,首先将一定量的微针基质溶液注入阴模针孔中,抽真空、干燥形成第一段不含药物的针尖;接着将一定量的狂犬疫苗双层微球与基质溶液混合物均匀注入针孔中,抽真空、干燥形成第二段含药针体;最后再注入基质溶液形成背衬,离心除去气泡,将阴模置于低温实验箱中,干燥,脱模,即得成品。
实施例2双层微球内外材料的种类与用量对突释量的影响
双层微球的内外材料种类与用量的选择,对于药物的初始释放速率存在很大影响,本实施例中假定双层微球的总量为一定值,仅改变双层微球内外材料的种类与用量,考察其对双层微球突释量的影响。
本实施例中,双层微球内外材料选自:端羟基聚(乳酸-羟基乙酸)共聚物(Glu-PLGA)、聚乳酸-羟基乙酸共聚物(PLGA(80:20))、聚D,L-丙交酯(PDLLA)、聚乳酸-羟基乙酸共聚物(PLGA(65:35))、聚乳酸-羟基乙酸共聚物(PLGA(50:50))中的任意组合。外层与内层的含量比选自:1:1或2:1。
本实施例中,双层微球的制备方法参照实施例1,考察如表1所示不同的双层微球处方对双层微球中药物初始释放的影响,每次实验重复三次,实验结果见表1。
突释量的检查方法:精密称取定量狂犬疫苗双层微球,以pH=7.4的PBS缓冲液作为释放介质,以叠氮化钠(NaN3)水溶液(0.01%w/v)作为抑菌剂加入到试管中。将试管放置在恒温振荡器中,并在37℃下以100rpm的水平搅拌。在24小时内每隔特定时间,将试管离心,并取出2ml上清液,并加入等量新鲜PBS缓冲液保持恒定的pH吸收条件。用BCA试剂盒测定24小时内蛋白质的释放量,用于表征双层微球的突释量。
本发明规定,当突释量<15%时,视为符合要求。

由表1可知,仅实验组2、实验组10的突释量<15%,符合要求。
进一步,通过体外释放实验观察实验组2、实验组10所制得微球在7天内的药物累计释放量,每次实验重复三次,实验结果见表2。

由表2可知,
1)实验组10所述的微球处方,虽然突释量符合要求,但由于外层材料与内层材料的重量比过大(2:1),会影响药物的后续释放。
2)与实验组10相比,实验组2的外层材料与内层材料的重量比也是2:1,但药物累计释放量相对提升很多,故确定实验组2的微球处方(即Glu-PLGA:PLGA(50:50)=2:1)为最佳处方,可作为本发明的负载狂犬疫苗的载体。
进一步,对按最佳处方制得的双层微球进行相关指标测定,如通过紫外分光光度法测定其包封率和载药量,粒度分析仪来确定微球粒径。测量结果如下:双层微球的包封率为72.34±2.3%,载药量为3.2±0.1%,粒径为6.9±0.9μm。
实施例3基质材料的种类对狂犬疫苗可溶性微针性能的影响
基质材料的选择直接影响微针的制备及皮肤刺入、药物释放等性能,因此选择适宜的基质材料至关重要。
本实施例中假定基质材料在针体中的重量百分比为一定值,仅改变基质材料的种类,考察如表3所示不同种类的基质材料对微针机械强度的影响。
本实施例中,基质材料选自:蔗糖、壳聚糖、聚乙烯醇(PVA)、聚乙烯吡咯烷酮(PVP)、透明质酸(HA)中的一种或其两两组合。
本实施例中,狂犬疫苗可溶性微针的制备方法参照实施例1,每次实验重复六次,结果见表3。
微针机械强度检查方法:以猪耳皮肤为模型,将皮肤角质层面朝上固定在一处平台上,使用双面胶带将微针贴片固定在平台上方的可移动圆柱形探针上,探头以0.4mm S-1的速度降到皮肤上,插入30s后去除微针贴片,用锥虫蓝将皮肤表面染色,然后使用数码相机拍照。计算确定染色通道的数量,计算微针的穿孔成功率。穿孔成功率的计算公式如下:
穿孔成功率=(染色孔洞数/微针的针数量)*100%。
本发明规定,当穿孔成功率达到80%及以上,视为符合要求。

由表3可知,不同的基质材料对于微针机械强度的影响不同,仅处方5的穿孔成功率大于80%,符合要求。故,本发明选择PVA和PVP的组合作为微针的基质材料。
实施例4基质材料中各组分的重量比对狂犬疫苗可溶性微针性能的影响
在实施例3的基础上,假设微针针体中基质材料的重量百分比为一定值(65%),载有狂犬疫苗的双层微球的重量百分比含量为(35%),仅改变基质材料中各组分的重量比,考察不同比例的基质材料对微针机械强度影响的差异。
本实施例中,狂犬疫苗可溶性微针的制备方法参照实施例1,微针机械强度的测试参照实施例3,每次实验重复六次,结果见表4。

由表5可知,PVP:PVA除1:5组外,其余各组的穿孔成功率均大于80%,即当PVP与PVA的含量比为(5:1)~(1:1)时,均符合要求;并且当PVP:PVA=5:1时微针的机械强度最高。考虑到疫苗加入一般会降低微针的机械强度,选择最优处方(5:1)用于开展进一步的筛选试验。
实施例5药载比对狂犬疫苗可溶性微针性能的影响
在实施例4的基础上,假定微针针体中基质材料的各组分及其重量比为一定值(PVP:PVA=5:1),仅改变药载比(即基质材料与疫苗微球的比例),考察不同药载比对双层微球突释量和微针机械强度的影响。
本实施例中,狂犬疫苗可溶性微针的制备方法参照实施例1,突释量和微针机械强度的测试方法对应参照实施例2和实施例3,每次实验重复三次,结果见表5。

由表5可知,基质材料与疫苗微球的含量比为(5:3)~(3:5)时,突释量均小于15%,符合要求;但基质材料与疫苗微球的含量比为(5:3)~(2:1)时,穿孔成功率才大于80%,符合要求。
故,综合考虑穿孔成功率和突释量,选择基质材料与疫苗微球的含量比为(5:3)~(2:1),且当基质材料与疫苗微球的含量比为5:3时微针的载药量最高,因此确定其为最佳含量比。
综上所述,本发明提供的一种狂犬疫苗可溶性微针,优选处方为:针体中各组分的重量百分比如下:载有狂犬疫苗的双层微球占33.3%~37.5%,PVA占10.4%~11.1%、PVP占52.1%~55.6%。
最佳处方为:针体中各组分的重量百分比如下:载有狂犬疫苗的双层微球占37.5%、PVA占10.4%、PVP占52.1%。
实施例7不同免疫方式所产生中和抗体水平的比较
在实施例6的基础上,用最佳处方按实施例1的方法制备本发明的控释型狂犬疫苗可溶性微针。比较肌肉注射、皮内注射和微针给药所产生中和抗体的水平,结果见表6。
肌肉注射:按照第0天(接种前),第3天,第7天使用一次0.5ml狂犬疫苗剂量对6只狂犬病模型地鼠进行肌肉注射,并分别在第0天(接种前),第3天(接种前),第7天(接种前),第28天收集血液。用快速荧光聚焦抑制试验(RFFIT)测量狂犬疫苗抗原的抗体效价。
皮内注射:按照第0天(接种前),第3天,第7天使用两次0.1ml狂犬疫苗的皮内剂量对6只狂犬病模型地鼠进行注射,并分别在第0天(接种前),第3天(接种前),第7天(接种前),第28天收集血液。用快速荧光聚焦抑制试验(RFFIT)测量狂犬疫苗抗原的抗体效价。
微针给药:按照第0天(接种前),第7天使用两次与皮内注射含有相同剂量狂犬疫苗的双层微球可溶性微针贴片对6只狂犬病模型地鼠进行给药,并分别在第0天(接种前),第3天,第7天(接种前),第28天收集血液。用快速荧光聚焦抑制试验(RFFIT)测量狂犬疫苗抗原的抗体效价。
表6不同免疫方式所产生中和抗体水平(n=6)
注:与肌肉注射组相比,*P<0.05,**P<0.01;与皮内注射组相比,ΔP<0.05,ΔΔP<0.01。
由表6可知,皮内注射给药能够达到与传统肌肉注射相当的免疫效果,并且所用的疫苗量只有肌肉注射的2/5。而使用本发明制得的狂犬疫苗可溶性微针,所用疫苗量比皮内注射组更低,免疫效果也比皮内注射更优。
综上所述,通过微针给药可以减少狂犬疫苗的注射次数,节省疫苗量,并能够达到免疫所需的抗体水平。
实施例8狂犬疫苗可溶性微针的释放曲线
在实施例6的基础上,用最佳处方按实施例1的方法制备本发明的控释型狂犬疫苗可溶性微针,通过释放实验观察释放曲线。
释放试验:精密称取定量的狂犬疫苗可溶性微针,以pH=7.4的PBS缓冲液作为释放介质,加入适量的PBS缓冲液,37℃恒温震荡。在预先设定的时间内(0、3、7、14天),抽取2ml上清液,并补充等量新鲜PBS缓冲液,用BCA试剂盒测定蛋白释放情况,计算释放蛋白的质量百分比,画出累积释放曲线。
由图1可知,本发明制得的狂犬疫苗可溶性微针,突释量为11.4±0.5%。
此外,用传统的复乳法制备了负载狂犬疫苗的单层PLGA微球,经实验检测其突释量为45.7±1.3%。
可见,本发明的狂犬疫苗可溶性微针(双层微球),与单层微球相比,能更好的控制突释量,实现长效控释的目的。
Claims (10)
1.一种控释型狂犬疫苗可溶性微针,包括不含药的针尖、含药针体和背衬,其特征在于,所述的含药针体包括载有狂犬疫苗的双层微球和基质材料,所述的双层微球系内外两层均由高分子聚合物构成的载体,其中外层材料选自Glu-PLGA,内层材料选自PLGA(50:50),外层材料与内层材料的重量比为2:1。
2.根据权利要求1所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的基质材料与载有狂犬疫苗的双层微球的含量比为(5:3)~(2:1)。
3.根据权利要求2所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的基质材料与载有狂犬疫苗的双层微球的含量比为5:3。
4.根据权利要求2所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的基质材料与载有狂犬疫苗的双层微球的含量比为2:1。
5.根据权利要求1~4中任一项所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的基质材料选自至少一种高分子聚合物。
6.根据权利要求5所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的基质材料为PVP与PVA的组合,PVP与PVA的含量比为(5:1)~(1:1)。
7.根据权利要求6所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的基质材料为含量比为1:1的PVP与PVA的组合。
8.根据权利要求6所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的基质材料为含量比为5:1的PVP与PVA的组合。
9.根据权利要求8所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的含药针体中各组分的重量百分比如下:载有狂犬疫苗的双层微球占33.3%~37.5%,PVA占10.4%~11.1%、PVP占52.1%~55.6%。
10.根据权利要求9所述的控释型狂犬疫苗可溶性微针,其特征在于,所述的含药针体中各组分的重量百分比如下:载有狂犬疫苗的双层微球占37.5%、PVA占10.4%、PVP占52.1%。
