技术领域
本发明涉及手术成像系统,并且特别地涉及用于自动分段解剖结构的系统。
背景技术
二维(2D)图像或三维(3D)图像体积的自动分段是指自动描绘相邻结构之间的边界并且可选地正确识别每个结构的过程。例如,先前未查看的脊柱x射线图像的成功自动分段表示图像中的每个椎骨或椎体被自动地突出显示或勾勒出轮廓,以及每个椎骨被自动地或半自动地正确识别(“L2”、“L3”等)。
存在使用深度学习或神经网络来训练计算机模型以通过比较新图像与一组已知图像来识别图像平面或体积内的结构的方法。由于扫描图像体积比扫描2D图像平面在计算上更复杂,因此自动分段3D图像体积的过程,例如计算机断层扫描(CT)可能比自动分段简单的2D x射线的过程更慢。此外,3D图像分段的准确度可能取决于通过用户输入对软件起点的做种。
相比之下,用于识别2D图像平面中的结构(诸如脊柱区域的2D x射线图像中的椎骨水平)的分段软件更快且对做种的依赖程度更低,但往往不如对应的3D分段软件那么可靠,这是因为在2D图像中存在较少的信息。
因此,需要一种将2D分段的速度与3D分段的准确度相结合的系统和方法。
发明内容
根据本发明的一个方面,公开了一种从锥形束CT图像识别和分段解剖结构的方法。一种图像处理系统从锥形束CT装置接收至少一个x射线图像,其是用锥形束CT成像装置从患者的360度扫描获取的一组x射线图像的一部分。所述x射线图像包含要分段的至少一种解剖结构。然后分析接收到的x射线以便基于存储的解剖结构模型识别和分段包含在x射线图像中的至少一个解剖结构。一旦完成360度旋转,将创建来自x射线图像集的3D图像体积。然后将从x射线图像导出的识别和分段信息添加到创建的3D图像体积中。
有利地,从2D x射线图像而不是从3D图像体积进行分段和识别。由于2D x射线图像在成像系统旋转时可用,因此在旋转完成之前就开始进行处理。此外,2D图像的图像处理可能比3D体积快得多。因此,本发明的方法可以节省大量时间,同时非常准确。

现转而参看附图,图1和图2示出了根据示范性实施例的手术机器人系统100。手术机器人系统100可包含例如手术机器人102、一个或多个机器人臂104、基座106、显示器110、包含例如导管114的末端执行器112,以及一个或多个跟踪标记118。手术机器人系统100可包含患者跟踪装置116,其也包含一个或多个跟踪标记118,所述患者跟踪装置适于直接固定到患者210(例如固定到患者210的骨头)。手术机器人系统100还可利用例如定位在相机支架202上的相机200。相机支架202可具有任何合适的配置以将相机200移动、定向并支撑在期望的位置。相机200可包含能够识别例如在从相机200的角度可见的既定测量体积中的有源和无源跟踪标记118的任何合适的一个或多个相机,例如一个或多个红外相机(例如双焦点或立体摄影测量相机)。相机200可以扫描既定的测量体积并且检测来自标记118的光以便识别和确定标记118在三维中的位置。举例来说,有源标记118可包含由电信号(例如红外发光二极管(LED))激活的红外发射标记,且无源标记118可包含反射红外光的回射标记(例如它们将进入的IR辐射反射进入入射光的方向),所述IR光例如由相机200上的照明器或其它合适的装置发射。
图1和2示出了手术机器人系统100在手术室环境中的放置的潜在配置。举例来说,机器人102可以位于患者210附近或旁边。虽然描绘在患者210的头部附近,但是应当理解,机器人102可以位于患者210附近的任何合适的位置,这取决于患者210正在进行手术的区域。相机200可以与机器人系统100分开并且定位在患者210的足部。所述位置允许相机200对手术视野208具有直接的视线。再次,可以设想相机200可以位于具有到手术视野208的视线的任何合适的位置。在所示的配置中,外科医生120可以定位在机器人102的对面,但仍然能够操纵末端执行器112和显示器110。手术助手126可以再次与外科医生120对置地定位,并且可以触及末端执行器112和显示器110两者。如果需要,外科医生120与助手126的位置可以颠倒。麻醉师122和护士或手术技术员124的传统区域保持不受机器人102和相机200的位置阻碍。
关于机器人102的其它组件,显示器110可以被附接到手术机器人102,且在其它示例性实施例中,显示器110可以与手术机器人102分开,不管是在具有手术机器人102的手术室内,还是在远处的位置。末端执行器112可以耦合到机器人臂104并且由至少一个马达控制。在示范性实施例中,末端执行器112可包括导管114,导管能够接收和定向用于对患者210进行外科手术的手术器械608(本文进一步描述)。如本文所使用,术语“末端执行器”与术语“末端施行器”和“施行器元件”可互换使用。虽然通常用导管114示出,但应理解,末端执行器112可以用适用于外科手术的任何合适的仪器代替。在一些实施例中,末端执行器112可包括用于以期望的方式实现手术器械608的移动的任何已知结构。
手术机器人102能够控制末端执行器112的平移和定向。机器人102能够例如沿着x轴、y轴和z轴移动末端执行器112。末端执行器112可以配置成用于围绕x轴、y轴和z轴中的一个或多个以及Z框轴线选择性旋转(使得与末端执行器112相关联的一个或多个欧拉角(例如翻转、俯仰和/或横偏)可以被选择性地控制)。在一些示例性实施例中,与利用例如仅包括旋转轴的六自由度机器人臂的传统机器人相比,对末端执行器112的平移和定向的选择性控制可以允许医疗程序的执行具有显著提高的准确度。举例来说,手术机器人系统100可以用于在患者210上操作,并且机器人臂104可以被定位在患者210的身体上方,末端执行器112选择性地朝向患者210的身体相对于z轴成角度。
在一些示例性实施例中,手术器械608的位置可以动态更新,使得手术机器人102可以在手术期间始终知道手术器械608的位置。因此,在一些示范性实施例中,手术机器人102可以将手术器械608快速移动到期望位置,而无需医师的任何进一步帮助(除非医师需要)。在一些进一步的实施例中,手术机器人102可以配置成在手术器械608偏离所选择的预先规划的轨迹的情况下校正手术器械608的路径。在一些示范性实施例中,手术机器人102可配置成允许停止、修改和/或手动控制末端执行器112和/或手术器械608的移动。因此,在示例性实施例中,在使用中,医师或其它用户可以操作系统100,并且可以选择停止、修改或手动控制末端执行器112和/或手术器械608的自主移动。包含手术机器人102对手术器械608的控制和移动的手术机器人系统100的进一步细节可以查阅第13/924,505号共同待决美国专利申请,其全部内容以引用的方式并入本文中。
机器人手术系统100可包括一个或多个跟踪标记118,其配置成三维地跟踪机器人臂104、末端执行器112、患者210和/或手术器械608的移动。在示例性实施例中,多个跟踪标记118可以被安装(或以其它方式被固定)到机器人102的外表面上,例如(且不限于)机器人102的基座106上、机器人臂104上,或末端执行器112上。在示例性实施例中,多个跟踪标记118中的至少一个跟踪标记118可以被安装或以其它方式被固定到末端执行器112。一个或多个跟踪标记118可以进一步被安装(或以其它方式被固定)到患者210。在示例性实施例中,多个跟踪标记118可以定位在患者210上与手术视野208间隔开,以减小被外科医生、手术工具或机器人102的其它部分遮挡的可能性。此外,一个或多个跟踪标记118可以进一步被安装(或以其它方式被固定)到手术工具608(例如螺丝刀、扩张器、植入物插入器等)。因此,跟踪标记118使得被标记的对象(例如末端执行器112、患者210和手术工具608)中的每一个都能够被机器人102跟踪。在示例性实施例中,系统100可以使用从每个被标记对象收集的跟踪信息计算例如末端执行器112、手术器械608(例如定位在末端执行器112的管114中)的定向和位置、以及患者的相对位置210。
在示例性实施例中,标记118中的一个或多个可以是光学标记。在一些实施例中,通过用于检查或检验末端执行器112的位置,末端执行器112上的一个或多个跟踪标记118的定位可使位置测量的准确性最大化。包含手术机器人102和手术器械608的控制、移动和跟踪的手术机器人系统100的进一步细节可以查阅第13/924,505号共同待决美国专利申请,其全部内容以引用的方式并入本文中。
示例性实施例包含联接到手术器械608的一个或多个标记118。在示范性实施例中,例如耦合到患者210和手术器械608的这些标记118,以及耦合到机器人102的末端执行器112的标记118可包括传统的红外发光二极管(LED)或能够使用商业上可获得的红外光学跟踪系统(例如)跟踪的二极管。是加拿大安大略省滑铁卢北方数字公司(Northern Digital Inc.)的注册商标。在其它实施例中,标记118可包括能够使用商业上可获得的光学跟踪系统(例如Polaris Spectra)跟踪的常规反射球。PolarisSpectra也是北方数字公司的注册商标。在示范性实施例中,耦合到末端执行器112的标记118是有源标记,其包括可以接通和关断的红外发光二极管,并且耦合到患者210和手术器械608的标记118包括无源反射球。
在示范性实施例中,由标记118发射和/或反射的光可以由相机200检测并且可以用于监视被标记对象的位置和移动。在替代实施例中,标记118可包括射频和/或电磁反射器或收发器,并且相机200可包含射频和/或电磁收发器,或由射频和/或电磁收发器代替。
类似于手术机器人系统100,图3示出了与本公开的示范性实施例一致的处于对接配置的手术机器人系统300和相机支架302。手术机器人系统300可包括机器人301,机器人包含显示器304、上臂306、下臂308、末端执行器310、竖直柱312、脚轮314、机柜316、平板抽屉318、连接器面板320、控制面板322和信息环324。相机支架302可包括相机326。这些组件参照图5更详细地描述。图3示出了处于对接配置的手术机器人系统300,其中相机支架302例如在不使用时与机器人301嵌套。所属领域的技术人员将认识到,相机326和机器人301可以在手术程序期间彼此分离并且定位在任何适当的位置处,例如如图1和2所示。
图4示出了与本公开的示范性实施例一致的基座400。基座400可以是手术机器人系统300的一部分并且包括机柜316。机柜316可以容纳手术机器人系统300的某些组件,包含但不限于电池402、配电模块404、平台接口板模块406、计算机408、手柄412和平板抽屉414。这些组件之间的连接和关系参照图5更详细地描述。

经由可以被提供给配电模块404的电源548,将输入电力供应给系统300。配电模块404接收输入电力并且配置成产生不同的电源电压,所述电源电压被提供给系统300的其它模块、组件和子系统。配电模块404可以配置成向平台接口模块406提供不同的电压供应,所述不同电压供应可以被提供给如计算机408、显示器304、扬声器536、驱动器508等其它组件,以例如为马达512、514、516、518和末端执行器310、马达510、环324、相机转换器542以及用于系统300的其它组件供电,所述其它组件例如用于冷却机柜316内的电气组件的风扇。
配电模块404还可以将电力提供给其它组件,例如可以位于平板抽屉318内的平板充电站534。平板充电站534可以与平板546进行无线或有线通信以用于为平板546充电。平板546可以由与本公开一致并在本文描述的外科医生来使用。
配电模块404还可以连接到电池402,所述电池在配电模块404没有从输入电力548接收电力的情况下用作临时电源。在其它时间,如果必要,配电模块404可以用以对电池402进行充电。
平台子系统502的其它组件还可包含连接器面板320、控制面板322和环324。连接器面板320可以用于将不同的装置和组件连接到系统300和/或相关联的组件和模块。连接器面板320可以包含接收来自不同组件的线路或连接的一个或多个端口。举例来说,连接器面板320可具有可将系统300接地到其它设备的接地端子端口、将脚踏板544连接到系统300的端口以及连接到跟踪子系统532的端口,所述跟踪子系统可包括位置传感器540、相机转换器542以及与照相机支架302相关联的相机326。[连接器面板320中的端口还可连接到成像装置,用于接收所扫描的图像并基于附加到所述成像装置的光学/导航标记控制C形臂的位置和定向]连接器面板320还可包含用于允许USB、以太网、HDMI与例如计算机408的其它组件通信的其它端口。
控制面板322可提供控制系统300的操作和/或提供关于系统300的信息的各种按钮或指示器。举例来说,控制面板322可包含按钮,其用于使系统300通电或断电、升高或下降竖直柱312、以及升高或下降稳定器520-526,所述稳定器可被设计成接合脚轮314以锁定系统300以免于物理移动。其它按钮可以在紧急情况下停止系统300,这可以移除所有马达功率并施加机械制动器来阻止发生所有运动。控制面板322还可以具有指示器,其用于向用户通知某些系统状况,例如线路电力指示器或电池402的充电状态。
环324可以是视觉指示器,以向系统300的用户通知系统300正在操作的不同模式以及对用户的某些警告。
计算机子系统504包含计算机408、显示器304和扬声器536。计算机504包含操作系统和用于操作系统300的软件。计算机504可以接收和处理来自其它组件(例如跟踪子系统532、平台子系统502和/或运动控制子系统506)的信息以向用户显示信息。此外,计算机子系统504还可包含扬声器536以向用户提供音频。
跟踪子系统532可包含位置传感器504和转换器542。跟踪子系统532可以对应于包含相机326的相机支架302,如参照图3所描述。位置传感器504可以是相机326。跟踪子系统可以跟踪位于系统300的不同组件和/或在手术程序期间由用户使用的器械上的某些标记的位置。此跟踪可以与本公开一致的方式进行,包含使用分别跟踪例如LED或反射标记等有源或无源元件的位置的红外技术。具有这些类型的标记的结构的定位、定向和位置可以被提供给计算机408,可以在显示器304上向用户展示。举例来说,具有这些类型的标记并且以这种方式(其可以被称为导航空间)跟踪的手术器械608可以向用户展示相关的患者的解剖结构的三维图像。
运动控制子系统506可以配置成物理地移动竖直柱312、上臂306、下臂308或旋转末端执行器310。物理移动可以通过使用一个或多个马达510-518来进行。举例来说,马达510可以配置成竖直地升高或降低竖直柱312。如图3所示,马达512可以配置成围绕与竖直柱312接合的点横向移动上臂308。如图3所示,马达514可以配置成围绕与上臂308接合的点横向移动下臂308。马达516和518可以配置成以可控制翻转并且可控制倾斜的方式来移动末端执行器310,由此提供可以移动末端执行器310的多个角度。这些移动可以通过控制器538来实现,所述控制器可以通过测压元件来控制这些移动,所述测压元件设置在末端执行器310上并且由接合这些测压元件的用户激活以按期望的方式移动系统300。
此外,系统300可以通过用户在显示器304(其可以是触摸屏输入装置)上指示手术器械或组件在显示器304上的患者解剖结构的三维图像上的位置,来提供竖直柱312、上臂306和下臂308的自动移动。用户可以通过踩踏脚踏板544或一些其它输入装置来起始此自动移动。
图6示出了与示范性实施例一致的手术机器人系统600。手术机器人系统600可包括末端执行器602、机器人臂604、导管606、器械608和机器人基座610。器械工具608可以附接到包含一个或多个跟踪标记(例如标记118)的跟踪阵列612,并且具有相关联的轨迹614。轨迹614可以代表器械工具608配置成一旦其穿过或固定在导管606中而行进的移动路径,例如器械工具608插入到患者体内的路径。在示范性操作中,机器人基座610可以配置成与机器人臂604和末端执行器602进行电子通信,使得手术机器人系统600可以辅助用户(例如外科医生)在患者210上操作。手术机器人系统600可以与之前描述的手术机器人系统100和300一致。
跟踪阵列612可以安装在器械608上以监视器械工具608的定位和定向。跟踪阵列612可以附接到器械608并且可包括跟踪标记804。如图8中最佳所见,跟踪标记804可以是例如发光二极管和/或其它类型的反射标记(例如如本文其它地方描述的标记118)。跟踪装置可以是与手术机器人系统相关联的一个或多个视线装置。作为实例,跟踪装置可以是与手术机器人系统100、300相关联的一个或多个相机200、326,并且还可跟踪跟踪阵列612以获得器械608相对于机器人臂604、机器人基座610、末端执行器602和/或患者210的限定域或相对定向。跟踪装置可以与结合相机支架302和跟踪子系统532描述的那些结构一致。
图7A、7B和7C分别示出了与示范性实施例一致的末端执行器602的俯视图、前视图和侧视图。末端执行器602可包括一个或多个跟踪标记702。跟踪标记702可以是发光二极管或其它类型的有源和无源标记,例如之前已经描述的跟踪标记118。在示范性实施例中,跟踪标记702是由电信号(例如红外发光二极管(LED))激活的有源红外发射标记。因此,跟踪标记702可以被激活,使得红外标记702对于相机200、326可见,或者可以被解除激活,使得红外标记702对于相机200、326不可见。因此,当标记702为有源时,末端执行器602可以由系统100、300、600来控制,并且当标记702被解除激活时,末端执行器602可以被锁定在适当位置并且不能被系统100、300、600移动。
标记702可以使得标记702可被一个或多个相机200、326或与手术机器人系统100、300、600相关联的其它跟踪装置可见的方式设置在末端执行器602上或所述末端执行器内。相机200、326或其它跟踪装置可在通过跟随跟踪标记702的移动而移动到不同位置和视角时跟踪末端执行器602。标记702和/或末端执行器602的位置可显示在与手术机器人系统100、300、600相关联的显示器110、304上,例如图2所示的显示器110和/或图3所示的显示器304。此显示器110、304可以允许用户确保末端执行器602关于机器人臂604、机器人基座610、患者210和/或用户处于期望位置。
举例来说,如图7A所示,可以将标记702放置在末端执行器602的表面周围,使得远离手术视野208并且面向机器人102、301和相机200、326放置的跟踪装置能够通过末端执行器602相对于跟踪装置100、300、600的一系列共同定向来检视标记702中的至少3个。举例来说,以这种方式分布标记702允许当末端执行器602在手术视野208中平移和旋转时通过跟踪装置监视末端执行器602。
另外,在示范性实施例中,末端执行器602可以配备有红外(infared,IR)接收器,所述红外(IR)接收器可以检测何时外部相机200、326准备好读取标记702。在此检测后,末端执行器602然后可以照亮标记702。由IR接收器检测到外部相机200、326准备好读取标记702可以表示需要将标记702(其可以是发光二极管)的工作循环同步到外部相机200、326。这也可以允许机器人系统作为整体实现较低的功耗,由此标记702将仅在适当的时间被照亮而不是被连续照亮。此外,在示范性实施例中,可以关闭标记702以防止干扰其它导航工具(例如不同类型的手术器械608)。
图8描绘了一种类型的手术器械608,其包含跟踪阵列612和跟踪标记804。跟踪标记804可以是本文所描述的任何类型,包含但不限于发光二极管或反射球。由与手术机器人系统100、300、600相关联的跟踪装置来监视标记804,并且所述标记可以是视线相机200、326中的一个或多个。相机200、326可以基于跟踪阵列612和标记804的位置和定向来跟踪器械608的定位。外科医生120等用户可以以某种方式定向器械608,使得由跟踪装置或相机200、326充分辨识跟踪阵列612和标记804以例如在示范性手术机器人系统的显示器110上显示器械608和标记804。
外科医生120可以将器械608放置到末端执行器602的导管606中并调整器械608,这种方式在图8中是明显的。末端执行器112、310、602的中空管或导管114、606的尺寸和配置被设定为接收手术器械608的至少一部分。导管114、606配置成通过机器人臂104定向,使得手术器械608的插入和轨迹能够到达患者210体内或体上的期望解剖目标。手术器械608可包含大致圆筒形器械的至少一部分。虽然螺丝刀作为手术工具608被举例说明,但应该理解,任何合适的手术工具608都可以由末端执行器602定位。作为实例,手术器械608可包含一个或多个导丝、套管、牵开器、钻机、扩眼器、螺丝刀、插入工具、移除工具等。虽然中空管114、606总体上显示为具有圆筒形配置,但所属领域的技术人员将会理解的是,导管114、606可以具有期望用于容纳手术器械608并接近手术部位的任何合适的形状、尺寸和配置。
图9A至图9C示出了与示范性实施例一致的末端执行器602和机器人臂604的一部分。末端执行器602可进一步包括主体1202和夹具1204。夹具1204可包括手柄1206、球1208、弹簧1210和唇缘1212。机器人臂604可进一步包括凹陷1214、安装板1216、唇缘1218和磁体1220。
末端执行器602可以通过一个或多个耦合件机械地介接和/或接合手术机器人系统和机器人臂604。举例来说,末端执行器602可以通过定位耦合件和/或增强耦合件与机器人臂604接合。通过这些耦合件,末端执行器602可以与柔性无菌屏障外的机器人臂604紧固。在示范性实施例中,定位耦合件可以是磁力运动学支架,并且增强耦合件可以是五杆跨中心夹紧联动件。
关于定位耦合件,机器人臂604可包括安装板1216(其可以是非磁性材料)、一个或多个凹陷1214、唇缘1218和磁体1220。磁体1220安装在每个凹陷1214下方。夹具1204的部分可包括磁性材料并且被一个或多个磁体1220吸引。通过夹具1204和机器人臂604的磁性吸引,球1208变成安置到相应的凹陷1214中。举例来说,如图9B所示的球1208将安置在凹陷1214中,如图9A所示。此安置可以被认为是磁力辅助的运动学耦合。无论末端执行器602的定向如何,磁体1220都可以配置成足够坚固以支撑末端执行器602的整个重量。定位耦合件可以是唯一地约束六个自由度的任何类型的运动学安装件。
关于增强耦合件,夹具1204的部分可以配置为固定的接地联动件,并且这种夹具1204可以用作五杆联动件。当唇缘1212和唇缘1218以固定末端执行器602和机器人臂604的方式接合夹具1204时,闭合夹具手柄1206可将末端执行器602固定到机器人臂604。当夹具手柄1206闭合时,当夹具1204处于锁定位置时弹簧1210可被拉伸或受到应力。锁定位置可以是提供经过中心的联动件的位置。由于闭合位置经过中心,因此在没有施加到夹具手柄1206以释放夹具1204的力的情况下联动件不会打开。因此,在锁定位置中,末端执行器602可牢固地固定到机器人臂604。
弹簧1210可以是张紧的弯曲梁。弹簧1210可由表现出高刚度和高屈服应变的材料组成,例如原始PEEK(聚醚醚酮)。末端执行器602和机器人臂604之间的连接可以在末端执行器602和机器人臂604之间提供无菌屏障而不妨碍两个耦合件的紧固。
增强耦合件可以是具有多个弹簧构件的联动件。增强耦合件可以与凸轮或基于摩擦的机构锁定。增强耦合件还可以是足够强大的电磁体,其将支持将末端执行器102紧固到机器人臂604。增强联接件可以是多件式套环,其完全与末端执行器602和/或机器人臂604分开,在末端执行器602与机器人臂604之间的接合部上方滑动,并且用螺旋机构、跨中心联动件或凸轮机构紧固。
参考图10和11,在手术程序之前或期间,可以进行某些配准程序以跟踪导航空间和成像空间中的患者210的目标解剖结构。为了进行这种配准,可以使用配准系统1400,如图10所示。
为了跟踪患者210的位置,患者跟踪装置116可包含将被固定到患者210的刚性解剖结构的患者固定器械1402,并且动态参考基座(dynamic reference base,DRB)1404可以被牢固地附接到患者固定器械1402。举例来说,患者固定器械1402可被插入到动态参考基座1404的开口1406中。动态参考基座1404可包含对跟踪装置(例如跟踪子系统532)可见的标记1408。这些标记1408可以是光学标记或反射球,例如跟踪标记118,如前所述。
患者固定器械1402附接到患者210的刚性解剖结构并且可以在整个手术程序中保持附接。在示范性实施例中,患者固定器械1402被附接到患者210的刚性区域,例如远离经受手术程序的目标解剖结构定位的骨骼。为了跟踪目标解剖结构,通过使用临时放置在目标解剖结构上或附近的配准固定装置以便将动态参考基座1404与目标解剖结构的位置配准,使动态参考基座1404与目标解剖结构相关联。
通过使用枢转臂1412,将配准固定装置1410附接到患者固定器械1402。通过将患者固定器械1402插入穿过配准固定装置1410的开口1414,将枢转臂1412附接到患者固定器械1402。通过例如将旋钮1416插入穿过枢转臂1412的开口1418,将枢转臂1412附接到配准固定装置1410。
使用枢转臂1412,配准固定装置1410可以放置在目标解剖结构上方,并且其位置可以使用配准固定装置1410上的跟踪标记1420和/或基准点1422在成像空间和导航空间中确定。配准固定装置1410可以包含在导航空间中可见的标记1420的集合(例如标记1420可以被跟踪子系统532检测到)。如本文先前所描述,跟踪标记1420可以是在红外光下可见的光学标记。配准固定装置1410还可以包含在成像空间(例如三维CT图像)中可见的基准点1422的集合,例如轴承球。如关于图11更详细描述,使用配准固定装置1410,目标解剖结构可以与动态参考基座1404相关联,从而允许将导航空间中的对象的描绘叠加在解剖结构的图像上。位于远离目标解剖结构的位置处的动态参考基座1404可以成为参考点,从而允许从手术区域移除配准固定装置1410和/或枢转臂1412。
图11提供了与本公开一致的用于配准的示范性方法1500。方法1500在步骤1502处开始,其中可以将目标解剖结构的图形表示(或图像)导入到系统100、300、600,例如计算机408。图形表示可以是患者210的目标解剖结构的三维CT或者荧光镜扫描,其包含配准固定装置1410和基准点1420的可检测成像图案。
在步骤1504处,检测基准点1420的成像图案并将其配准在成像空间中并存储在计算机408中。可选地,此时在步骤1506处,配准固定装置1410的图形表示可以叠加在目标解剖结构的图像上。
在步骤1508处,通过辨识标记1420来检测并配准配准固定装置1410的导航图案。标记1420可以是经由位置传感器540由跟踪子系统532通过红外光在导航空间中辨识的光学标记。因此,目标解剖结构的定位、定向以及其它信息被配准在导航空间中。因此,可以通过使用基准点1422在成像空间中辨识配准固定装置1410,并且可以通过使用标记1420在导航空间中辨识所述配准固定装置。在步骤1510处,将成像空间中的配准固定装置1410的配准转移到导航空间。举例来说,通过使用基准点1422的成像图案相较于标记1420的导航图案的位置的相对位置来完成此转移。
在步骤1512处,配准固定装置1410的导航空间(已经与成像空间一起配准)的配准被进一步转移到附接到患者固定器械1402的动态配准阵列1404的导航空间。因此,配准固定装置1410可以被移除,并且因为导航空间与成像空间相关联,所以可以使用动态参考基座1404来跟踪导航空间和成像空间两者中的目标解剖结构。
在步骤1514和1516处,导航空间可以叠加在成像空间上,并且对象具有在导航空间中可见的标记(例如具有光学标记804的手术器械608)。可以通过手术器械608在目标解剖结构的图像上的图形表示来跟踪对象。
图12A-12B示出了可与机器人系统100、300、600结合使用以获取患者210的手术前、手术中、手术后和/或实时图像数据的成像装置1304。使用成像系统1304,可以针对任何适当的程序使任何适当的主题成像。成像系统1304可以是任何成像装置,例如成像装置1306和/或C形臂1308装置。可能需要从多个不同位置拍摄患者210的x射线,而不需要x射线系统中可能需要的患者210的频繁手动重新定位。如图12A所示,成像系统1304可以是C形臂1308的形式,其包含终止于“C”形的相对远端1312的细长C形构件。C形构件1130可以进一步包括x射线源1314和图像接收器1316。臂的C形臂1308内的空间可以为医生提供空间来在基本上不受来自x射线支撑结构1318的干扰的情况下照料患者。如图12B所示,成像系统可包含成像装置1306,所述成像装置具有附接到支撑结构成像装置支撑结构1328的机架外壳1324,例如具有轮1332的轮式移动手推车1330,其可包围图像捕获部分(未示出)。图像捕获部分可包含x射线源和/或发射部分以及x射线接收和/或图像接收部分,其可安置成彼此相隔大约一百八十度,并且相对于图像捕获部分的轨道安装在转子上(未示出)。图像捕获部分可操作以在图像获取期间旋转三百六十度。图像捕获部分可围绕中心点和/或轴旋转,从而允许从多个方向或多个平面获取患者210的图像数据。虽然本文举例说明了某些成像系统1304,但应该理解,所属领域的普通技术人员可以选择任何合适的成像系统。
现转而参看图13A-13C,手术机器人系统100、300、600依靠末端执行器112、602、手术器械608和/或患者210(例如患者跟踪装置116)相对于期望的手术区域的准确定位。在图13A-13C所示的实施例中,跟踪标记118、804刚性地附接到器械608和/或末端执行器112的一部分。
图13A描绘了手术机器人系统100的一部分,其具有机器人102,机器人包含基座106、机器人臂104和末端执行器112。未示出的其它元件,例如显示器、相机等也可以如本文所描述存在。图13B描绘了末端执行器112的特写视图,其具有导管114和刚性固定到末端执行器112的多个跟踪标记118。在此实施例中,多个跟踪标记118附接到导管112。图13C描绘了具有刚性附连到器械608的多个跟踪标记804的器械608(在这种情况下为探针608A)。如本文其它地方所描述,器械608可包含任何合适的手术器械,例如但不限于导丝、套管、牵开器、钻机、扩眼器、螺丝刀、插入工具、移除工具等。
当要以3D跟踪器械608、末端执行器112或其它对象时,跟踪标记118、804的阵列可以刚性地附接到工具608或末端执行器112的一部分。优选地,跟踪标记118、804被附接,使得标记118、804不妨碍(例如不妨碍手术操作、可视性等)。标记118、804可以附连到器械608、末端执行器112或例如要用阵列612跟踪的其它对象。通常,三个或四个标记118、804与阵列612一起使用。阵列612可包含线性区段、交叉件,并且可以是不对称的,使得标记118、804相对于彼此处于不同的相对位置和定位。举例来说,如图13C所示,示出了具有4标记跟踪阵列612的探针608A,并且图13B描绘了具有不同4标记跟踪阵列612的末端执行器112。
在图13C中,跟踪阵列612用作探针608A的手柄620。因此,四个标记804附接到探针608A的手柄620,不妨碍轴杆622和尖端624。这四个标记804的立体摄影测量跟踪允许器械608作为刚性主体被跟踪并且允许跟踪系统100、300、600在探针608A在跟踪相机200、326前方四处移动时精确地确定尖端624的位置和轴杆622的定向。
为了能够自动跟踪一个或多个工具608、末端执行器112或其它要以3D跟踪的对象(例如多个刚性主体),每个工具608、末端执行器112或类似物上的标记118、804以已知的标记间隔不对称地布置。不对称对准的原因是,使得哪一标记118、804对应于刚性主体上的特定位置以及标记118、804是从前方还是后方检视(即,镜像)都是明确的。举例来说,如果标记118、804被布置在工具608或末端执行器112上的正方形中,那么系统100、300、600将不清楚哪个标记118、804对应于正方形的哪个角。举例来说,对于探针608A,不清楚哪个标记804最接近轴杆622。因此,不知道轴杆622从阵列612延伸的方向。因此,每个阵列612以及因此每个工具608、末端执行器112或其它要跟踪的对象应该具有唯一的标记图案,以使其能够与正被跟踪的其它工具608或其它对象区分。不对称且唯一的标记图案允许系统100、300、600检测各个标记118、804,然后对照所存储的模板检查标记间隔以确定它们代表哪个工具608、末端执行器112或其它对象。然后可以将检测到的标记118、804自动分类并以正确的顺序指派给每个所跟踪对象。如果没有该信息,则除非用户手动指定哪个检测到的标记118、804对应于每个刚性主体上的哪个位置,否则不能执行刚性主体计算以提取关键的几何信息,例如工具尖端624和轴杆622的对准。这些概念对于3D光学跟踪方法的技术人员来说通常是已知的。
现转而参看图14A-14D,示出了具有可移动跟踪标记918A-918D的末端执行器912的替代型式。在图14A中,具有可移动跟踪标记918A-918D的阵列示出于第一配置中,且在图14B中,可移动跟踪标记918A-918D示出于相对于第一配置成角的第二配置中。图14C示出例如如由图14A的第一配置中的相机200、326所见的跟踪标记918A-918D的模板;且图14D示出例如如由图14B的第二配置中的相机200、326所见的跟踪标记918A-918D的模板。
在此实施例中,涵盖了4标记阵列跟踪,其中标记918A-918D并非全部相对于刚性主体位于固定位置中,且替代地,可例如在测试期间调整阵列标记918A-918D中的一个或多个,以给出关于正被跟踪的刚性主体的更新后信息,而不妨碍对所跟踪标记918A-918D的自动检测和分类的过程。
当跟踪任何工具,例如连接到机器人系统100、300、600的末端执行器912的导管914时,跟踪阵列的主要目的是在相机坐标系中更新末端执行器912的位置。当使用例如如图13B中示出的刚性系统时,反射标记118的阵列612从导管114刚性地延伸。因为跟踪标记118刚性地连接,所以对相机坐标系中的标记位置的了解还在相机坐标系中提供导管114的中心线、尖端和尾部的准确位置。通常,关于末端执行器112距此阵列612的位置的信息和关于目标轨迹距另一被跟踪源的位置的信息用以计算必须针对机器人102的每个轴线输入的将使导管114移动成与轨迹对准并沿着轨迹向量将尖端移动到特定位置的所需移动。
有时,期望的轨迹在不方便或不可达的位置中,但如果可转动导管114,那么可到达期望的轨迹。举例来说,如果导管114可超出间距(手腕上下角度)轴线的限度朝上转动,那么远离机器人102的基座106指向的极陡峭轨迹可能是可到达的,但如果导管114平行于将其连接到手腕的末端的板而附接,那么所述极陡峭轨迹可能并非可到达的。为了到达此轨迹,机器人102的基座106可移动,或具有不同导管附接的不同末端执行器112可与工作的末端执行器交换。这两个解决方案可能是费时且繁琐的。
如图14A和14B中最佳所见,如果阵列908配置成使得标记918A-918D中的一个或多个不在固定位置中,且替代地,可调整、转动、枢转或移动标记918A-918D中的一个或多个,那么机器人102可提供关于正被跟踪的对象的更新后信息,而不妨碍检测和跟踪过程。举例来说,标记918A-918D中的一个可固定在适当位置,且其它标记918A-918D可以是可移动的;标记918A-918D中的两个可固定在适当位置且其它标记918A-918D可以是可移动的;标记918A-918D中的三个可固定在适当位置且其它标记918A-918D可以是可移动的;或所有标记918A-918D可以是可移动的。
在图14A和14B中示出的实施例中,标记918A、918B刚性地直接连接到末端执行器912的基座906,且标记918C、918D刚性地连接到管914。类似于阵列612,可提供阵列908以将标记918A-918D附接到末端执行器912、器械608或要跟踪的其它对象。但是,在此状况下,阵列908由多个单独组件组成。举例来说,标记918A、918B可通过第一阵列908A连接到基座906,且标记918C、918D可通过第二阵列908B连接到导管914。标记918A可附连到第一阵列908A的第一端,且标记918B可分离一段直线距离并附连到第一阵列908A的第二端。虽然第一阵列908是大体上线性的,但是第二阵列908B具有弯曲或V形配置,相应根端连接到导管914,且以V形状从其发散到远端,其中标记918C在一个远端处且标记918D在另一远端处。虽然在本文中举例说明了具体配置,但是应了解,涵盖了包含不同数目个和不同类型的阵列908A、908B以及不同布置、数目和类型的标记918A-918D的其它非对称设计。
导管914可例如跨越铰链920或到基座906的其它连接器相对于基座906可移动、可转动或可枢转。因此,标记918C、918D是可移动的,使得当导管914枢转、转动或移动时,标记918C、918D也枢转、转动或移动。如图14A中最佳所见,导管914具有纵向轴线916,所述纵向轴线在大体上正交或竖直的定向上对准,使得标记918A-918D具有第一配置。现转而参看图14B,导管914枢转、转动或移动,使得纵向轴线916现相对于竖直定向成角,使得标记918A-918D具有不同于第一配置的第二配置。
相比于对于图14A-14D描述的实施例,如果在导管914与臂104(例如手腕附接)之间存在转动,其中所有四个标记918A-918D保持刚性地附接到导管914,且此转动由用户调整,那么机器人系统100、300、600将不能够自动地检测到导管914定向已改变。机器人系统100、300、600将跟踪标记阵列908的位置,并将假设导管914在先前定向上附接到手腕(机器人臂104)而计算错误的机器人轴线移动。通过保持一个或多个标记918A-918D(例如两个标记918C、918D)刚性地在管914上并保持一个或多个标记918A-918D(例如两个标记918A、918B)跨越转动,对新位置的自动检测变得可能,且基于在机器人臂104的末端上检测到新工具或末端执行器112、912而计算正确的机器人移动。
标记918A-918D中的一个或多个配置成根据任何合适的方式而移动、枢转、转动等等。举例来说,标记918A-918D可由例如夹具、弹簧、杠杆、滑动件、肘节等等铰链920或用于执行以下操作的任何其它合适的机构移动:个别地或组合地移动标记918A-918D、个别地或组合地移动阵列908A、908B、相对于另一部分移动末端执行器912的任何部分、或相对于另一部分移动工具608的任何部分。
如图14A和14B中示出,通过仅松开夹具或铰链920、相对于其它部分908A、908B移动阵列908A、908B的部分并再收紧铰链920使得导管914定向于不同位置中,阵列908和导管914可变得可重新配置。举例来说,两个标记918C、918D可与管914刚性地互连,且两个标记918A、918B可跨越到附接到机器人臂104的末端执行器912的基座906的铰链920刚性地互连。铰链920可呈例如翼形螺母等夹具的形式,可松开并再收紧夹具以允许用户在第一配置(图14A)与第二配置(图14B)之间快速切换。
相机200、326检测例如图14C和14D中标识的模板中的一个中的标记918A-918D。如果阵列908在第一配置(图14A)中且跟踪相机200、326检测到标记918A-918D,那么所跟踪标记与如图14C中示出的阵列模板1匹配。如果阵列908是第二配置(图14B)且跟踪相机200、326检测到相同标记918A-918D,那么所跟踪标记与如图14D中示出的阵列模板2匹配。阵列模板1和阵列模板2由系统100、300、600辨识为两个相异工具,其各自具有导管914、标记918A-918D与机器人附接之间的其自有的唯一限定的空间关系。用户可因此在第一配置与第二配置之间调整末端执行器912的位置,而不向系统100、300、600通知改变,且系统100、300、600将适当地调整机器人102的移动以保持在轨迹上。
在此实施例中,存在两个组装位置,其中标记阵列与允许系统100、300、600将组合件辨识为两个不同工具或两个不同末端执行器的唯一模板匹配。在这两个位置(即,分别在图14C和14D中示出的阵列模板1和阵列模板2)之间或外部转动的任何位置中,标记918A-918D将不与任何模板匹配,且虽然相机200、326检测到个别标记918A-918D,但系统100、300、600将不会检测到任何阵列存在,其结果与从相机200、326的视野临时阻挡标记918A-918D的情况相同。应了解,对于其它配置可存在其它阵列模板例如识别不同器械608或其它末端执行器112、912等等。
在所描述的实施例中,在图14A和14B示出了两个离散组装位置。但是,应了解,在转动接点、线性接点、转动接点与线性接点的组合、木栓板或其它组合件上可存在多个离散位置,其中可通过调整阵列的一个或多个标记918A-918D相对于其它标记的位置来产生唯一标记模板,其中每个离散位置与特定模板匹配,并通过不同已知属性限定唯一工具608或末端执行器112、912。另外,虽然对于末端执行器912举例说明,但是应了解,可移动且固定的标记918A-918D可与任何合适的器械608或要跟踪的其它对象一起使用。
当使用外部3D跟踪系统100、300、600来跟踪附接到机器人的末端执行器112的三个或更多个标记的完全刚性主体阵列(例如如图13A和13B中所描绘)时,有可能直接跟踪或计算相机200、326的坐标系中的机器人102的每个区段的3D位置。接点相对于跟踪器的几何定向通过设计已知,且接点的线性位置或角位置从机器人102的每个马达的编码器已知,从而完全限定从末端执行器112到基座116的所有移动部分的3D位置。类似地,如果跟踪器安装于机器人102的基座106上(未示出),那么同样地有可能基于从每个马达的编码器已知的接点几何形状和接点位置而跟踪或计算从基座106到末端执行器112的机器人102的每个区段的3D位置。
在一些情形中,可能需要从刚性地附接到末端执行器112的少于三个标记118跟踪机器人102的所有片段的位置。具体地说,如果工具608被引入到导管114中,那么可能需要通过正被跟踪的仅一个额外标记118跟踪机器人902的完全刚性主体运动。
现转而参看图15A-15E,示出了仅具有单个跟踪标记1018的末端执行器1012的替代型式。末端执行器1012可类似于本文中所描述的另一末端执行器,并可包含沿着纵向轴线1016延伸的导管1014。类似于本文中所描述的其它跟踪标记,单个跟踪标记1018可刚性地附连到导管1014。此单个标记1018可起到添加缺失的自由度以允许完全刚性主体跟踪的作用,和/或可起到充当监视标记以确保关于机器人和相机定位的假设有效的作用。
单个跟踪标记1018可作为到末端执行器1012的刚性延伸部附接到机器人末端执行器1012,刚性延伸部在任何方便的方向上突出且并不阻挡外科医生的视野。跟踪标记1018可附连到导管1014或末端执行器1012上的任何其它合适的位置。当附连到导管1014时,跟踪标记1018可定位在导管1014的第一端与第二端之间的位置处。举例来说,在图15A中,单个跟踪标记1018示出为安装于窄轴1017的末端上的反射球面,所述窄轴从导管1014前向延伸,并在导管1014的中点上方和导管1014的入口下方纵向定位。此位置允许标记1018由相机200、326大体上可见,而且将不阻挡外科医生120的视觉或与手术附近的其它工具或对象碰撞。另外,在此位置中具有标记1018的导管1014被设计成使引入到导管1014中的任何工具608上的标记阵列在导管1014上的单个标记1018可见的同时可见。
如图15B中示出,当紧密配合的工具或器械608放置在导管1014内时,器械608变得在6个自由度中的4个中以机械方式受约束。也就是说,除了围绕导管1014的纵向轴线1016之外,器械608无法在任何方向上旋转,且除了沿着导管1014的纵向轴线1016之外,器械608无法在任何方向上平移。换句话说,器械608可仅沿着导管1014的中心线平移并围绕所述中心线旋转。如果另外两个参数,例如(1)围绕导管1014的纵向轴线1016的旋转角度;和(2)沿着导管1014的位置,是已知的,那么相机坐标系中的末端执行器1012的位置被完全限定。
现参考图15C,系统100、300、600应该能够知晓工具608何时实际上定位于导管1014内部,而非替代地定位于导管1014外部,和仅位于相机200、326的视野内某处。工具608具有纵向轴线或中心线616和具有多个所跟踪标记804的阵列612。刚性主体计算可用以基于阵列612在工具608上的所跟踪位置而确定工具608的中心线616定位于相机坐标系中的何处。
从单个标记1018到导管1014的中心线或纵向轴线1016的固定法线(竖直)距离DF是固定的并在几何学上已知,且可跟踪单个标记1018的位置。因此,当从工具中心线616到单个标记1018的检测到的距离DD与从导管中心线1016到单个标记1018的已知固定距离DF匹配时,可确定工具608在导管1014内(工具608的中心线616、1016与导管1014重合)或恰好在此距离DD与固定距离DF匹配的可能位置的地点中的某一点处。举例来说,在图15C中,从工具中心线616到单个标记1018的检测到的法线距离DD与由两个位置中的透明工具608表示的两个数据框架(所跟踪标记坐标)中从导管中心线1016到单个标记1018的固定距离DF匹配,且因此可需要额外的考量以确定工具608何时定位在导管1014中。
现转而参看图15D,已编程逻辑可用以寻找跟踪数据的框架,其中从工具中心线616到单个标记1018的检测到的距离DD保持固定在正确长度,而不管工具608在空间上相对于单个球面1018按大于某一最小距离移动,以满足工具608在导管1014内移动的条件。举例来说,可通过第一位置中的工具608检测到第一框架F1,且可通过第二位置(即,相对于第一位置线性移动)中的工具608检测到第二框架F2。工具阵列612上的标记804可从第一框架F1向第二框架F2移动多于既定量(例如大于总共5mm)。即使在此移动的情况下,从工具中心线向量C'到单个标记1018的检测到的距离DD在第一框架F1与第二框架F2两者中大体上相同。
逻辑上,外科医生120或用户可将工具608放置于导管1014内,并使工具608略微地旋转或使其向下滑动到导管1014中,且系统100、300、600将能够从跟踪五个标记(工具608上的四个标记804加导管1014上的单个标记1018)来检测到所述工具在导管1014内。知道了工具608在导管1014内后,可计算在空间上限定机器人末端执行器1012的位置和定向的所有6个自由度。在无单个标记1018的情况下,即使确定地已知工具608在导管1014内,也不知道导管1014沿着工具的中心线向量C'定位于何处,以及导管1014如何相对于中心线向量C'旋转。
侧重于图15E,在跟踪到单个标记1018的存在以及工具608上有四个标记804的情况下,有可能通过单个标记1018和通过中心线向量C'建构导管1014和工具608的中心线向量C'和法线向量。此法线向量具有定向,所述定向相对于在手腕(在此实例中,平行于该片段定向)远端的机器人的前臂在已知定向上并在特定固定位置处与中心线向量C'相交。为方便起见,可建构三个相互正交的向量k'、j'、i',如图15E中示出,从而限定导管1014的刚性主体位置和定向。三个相互正交的向量中的一个k'由中心线向量C'构成,第二向量j'由穿过单个标记1018的法线向量构成,且第三向量i'是第一向量k'与第二向量j'的向量叉积。当所有接点处于零处时,相对于这些向量k'、j'、i'的机器人接点位置已知且固定,且因此刚性主体计算可用以在机器人处于起始位置处时确定机器人的任何区段相对于这些向量k'、j'、i'的位置。在机器人移动期间,如果从跟踪系统检测到工具标记804的位置(此时工具608在导管1014中)和单个标记1018的位置,且每个接点的角度/线性位置从编码器已知,那么可确定机器人的任何区段的位置和定向。
在一些实施例中,固定工具608相对于导管1014的定向可能是有用的。举例来说,末端执行器导管1014可围绕其轴线1016在特定位置中定向以允许机械加工或植入物定位。虽然附接到插入到导管1014中的工具608的任何物件的定向从工具608上的所跟踪标记804已知,但是在导管1014上没有额外跟踪标记1018(或在其它实施例中没有多个跟踪标记)的情况下,相机坐标系中的导管1014自身的旋转定向未知。此标记1018基于标记1018相对于中心线向量C'的定向而提供从-180°到+180°的基本上“时钟位置”。因此,单个标记1018可提供额外的自由度以允许完全刚性主体跟踪,和/或可充当监视标记以确保关于机器人和相机定位的假设有效。
图16是用于将机器人102的末端执行器1012(或本文中所描述的任何其它末端执行器)导航并移动到期望目标轨迹的方法1100的框图。单个标记1018在机器人末端执行器1012或导管1014上的另一用途是在没有附接到机器人102的完全跟踪阵列的情况下作为方法1100的部分实现机器人102的自动化安全移动。当跟踪相机200、326不相对于机器人102移动(即,它们在固定位置中),跟踪系统的坐标系与机器人的坐标系共配准,且机器人102经校准使得可仅基于每个机器人轴线的经编码位置而在机器人笛卡尔坐标系中准确地确定导管1014的位置和定向时,此方法1100起作用。
对于此方法1100,跟踪器的坐标系和机器人的坐标系必须共配准,这意味着需要从跟踪系统的笛卡尔坐标系到机器人的笛卡尔坐标系的坐标变换。为方便起见,此坐标变换可以是机器人技术的领域中众所周知的平移和旋转的4×4矩阵。此变换将被称为Tcr以指“变换-相机到机器人”一旦此变换已知,那么对于每个被跟踪标记以向量形式作为x、y、z坐标接收到的跟踪数据的任何新框架可乘以4×4矩阵,且所得x、y、z坐标将处于机器人的坐标系中。为了获得Tcr,在机器人上的完全跟踪阵列在机器人的坐标系中已知的位置处刚性地附接到机器人时跟踪所述完全跟踪阵列,接着使用已知刚性主体方法来计算坐标的变换。应显而易见,当还读取额外标记1018时,插入到机器人102的导管1014中的任何工具608可与刚性附接的阵列提供相同的刚性主体信息。也就是说,工具608仅需要插入到导管1014内的任何位置并在导管1014内处于任何旋转,而不是插入到固定位置和定向。因此,有可能通过以下操作来确定Tcr:将具有跟踪阵列612的任何工具608插入到导管1014中并读取工具的阵列612加导管1014的单个标记1018,同时从每个轴线上的编码器确定机器人的坐标系中的导管1014的当前位置。
在图16的方法1100中提供了用于将机器人102导航并移动到目标轨迹的逻辑。在进入循环1102之前,假设先前已存储了变换Tcr。因此,在进入循环1102之前,在步骤1104中,在紧固机器人基座106之后,当机器人静态时,存储导管中插入的工具的大于或等于一帧跟踪数据;且在步骤1106中,从此静态数据和先前校准数据计算从相机坐标到机器人坐标的机器人导管位置的变换Tcr。Tcr应保持有效,只要相机200、326不相对于机器人102移动即可。如果相机200、326相对于机器人102移动,且需要重新获得Tcr,则可使系统100、300、600提示用户将工具608插入到导管1014中并接着自动执行必需的计算。
在方法1100的流程图中,每一帧所收集数据由患者210身上的DRB 1404的所跟踪位置、末端执行器1014上的单个标记1018的所跟踪位置和每个机器人轴线的位置的快照组成。从机器人的轴线的位置,计算出末端执行器1012上的单个标记1018的位置。此计算出的位置与如从跟踪系统记录的标记1018的实际位置相比较。如果值一致,那么可保证机器人102在已知位置中。变换Tcr应用于DRB 1404的所跟踪位置,以使得可根据机器人的坐标系提供机器人102的目标。可接着命令机器人102移动以到达目标。
在步骤1104、1106之后,循环1102包含从跟踪系统接收DRB 1404的刚性主体信息的步骤1108;将目标尖端和轨迹从图像坐标变换到跟踪系统坐标的步骤1110;以及将目标尖端和轨迹从相机坐标变换到机器人坐标(应用Tcr)的步骤1112。循环1102进一步包含从跟踪系统接收机器人的单个杂散标记位置的步骤1114;以及将单个杂散标记从跟踪系统坐标变换到机器人坐标(应用所存储Tcr)的步骤1116。循环1102还包含根据正向运动学确定机器人坐标系中单个机器人标记1018的当前位置的步骤1118。来自步骤1116和1118的信息用以确定来自变换后的所跟踪位置的杂散标记坐标是否与小于既定容限的计算出的坐标一致的步骤1120。如果是,那么继续进行到步骤1122,计算并将机器人移动应用于目标x、y、z和轨迹。如果否,那么继续进行到步骤1124:在继续进行之前停止并要求全阵列插入到导管1014中;步骤1126:在插入阵列之后重新计算Tcr;以及继续进行以重复步骤1108、1114和1118。
相比于省略对单个标记1018的连续监视以检验位置的方法,此方法1100具有优点。在不具有单个标记1018的情况下,将仍有可能使用Tcr来确定末端执行器1012的位置并将末端执行器1012发送到目标位置,但将不可能检验机器人102实际上在预期位置中。举例来说,如果相机200、326已凸起且Tcr不再有效,那么机器人102将移动到错误的位置。出于此原因,单个标记1018提供安全价值。
对于机器人102的既定固定位置,理论上有可能将跟踪相机200、326移动到单个所跟踪标记1018保持未移动的新位置,这是因为单个所跟踪标记是单点而非阵列。在此状况下,系统100、300、600将不会检测到任何错误,这是因为在单个标记1018的计算出的和所跟踪位置方面将存在一致。但是,一旦机器人的轴线致使导管1012移动到新位置,那么计算出的和所跟踪位置将不一致且安全检查将起作用。
举例来说,术语“监视标记”可参考相对于DRB 1404在固定位置中的单个标记使用。在此实例中,如果DRB 1404凸起或以其它方式移开,那么监视标记的相对位置改变且可警告外科医生120导航可能存在问题。类似地,在本文中所描述的实施例中,通过机器人的导管1014上的单个标记1018,系统100、300、600可持续检查相机200、326是否已相对于机器人102移动。如果跟踪系统的坐标系与机器人的坐标系的配准丢失(例如因为相机200、326凸起或发生故障或因为机器人发生故障),那么系统100、300、600可警告用户且可作出校正。因此,此单个标记1018还可被认为是用于机器人102的监视标记。
应显而易见的是,在全阵列永久地安装于机器人102上(例如多个跟踪标记702安装于图7A-7C中示出的末端执行器602上)的情况下,不需要作为机器人监视标记的单个标记1018的此类功能性,这是因为不需要相机200、326相对于机器人102在固定位置中,且基于机器人102的所跟踪位置而在每个框架处更新Tcr。使用单个标记1018而非全阵列的原因是,全阵列更庞大且具干扰性,由此阻挡外科医生的视野和对大于单个标记1018的手术视野208的访问,且到全阵列的视线比到单个标记1018的视线更容易受到阻挡。
现转而参看图17A-17B和18A-18B,描绘了例如植入物固持器608B、608C等器械608,所述器械包含固定跟踪标记804和可移动跟踪标记806两者。植入物固持器608B、608C可具有手柄620和从手柄620延伸的外轴622。轴杆622可大体上垂直于手柄620定位,如所示出,或可在任何其它合适的定向上定位。内轴626可在一端处通过旋钮628延伸穿过外轴622。使用所属领域的技术人员已知的典型连接机构,在另一端处,植入物10、12在植入物固持器608B、608C的尖端624处连接到轴杆622。举例来说,旋钮628可旋转以使植入物10、12扩张或铰接所述植入物。以引用的方式并入本文中的第8,709,086号和第8,491,659号美国专利描述可膨胀融合装置和安装方法。
当跟踪例如植入物固持器608B、608C等工具608时,跟踪阵列612可包含固定标记804与一个或多个可移动标记806的组合,所述可移动标记构成阵列612或以其它方式附接到植入物固持器608B、608C。导航阵列612可包含至少一个或多个(例如至少两个)固定位置标记804,所述固定位置标记相对于植入物固持器器械608B、608C定位在已知位置。这些固定标记804将不能够相对于器械几何形状在任何定向上移动,并将适用于限定器械608在空间上的位置。另外,存在可附接到阵列612或器械自身的至少一个标记806,其能够相对于固定标记804在预定边界内移动(例如滑动、旋转等等)。系统100、300、600(例如软件)使可移动标记806的位置与植入物10的特定位置、定向或其它属性(例如图17A-17B中示出的可膨胀椎间体间隔件的高度,或图18A-18B中示出的铰接式椎间体间隔件的角度)相关。因此,系统和/或用户可基于可移动标记806的位置而确定植入物10、12的高度或角度。
在图17A至图17B中示出的实施例中,四个固定标记804用以限定植入物固持器608B,且第五可移动标记806能够在预定路径内滑动以提供关于植入物高度(例如收缩位置或膨胀位置)的反馈。图17A示出其初始高度处的可膨胀间隔件10,且图17B示出膨胀状态下的间隔件10,其中可移动标记806平移到了不同位置。在此状况下,当植入物10膨胀时,可移动标记806移动成更接近固定标记804,但预期此移动可逆转或以其它方式不同。标记806的线性平移的量将对应于植入物10的高度。虽然仅示出了两个位置,但是将有可能将此作为连续功能,可通过此连续功能使任何既定膨胀高度与可移动标记806的特定位置相关。
现转而参看图18A到18B,四个固定标记804用以限定植入物固持器608C,且第五可移动标记806配置成在预定路径内滑动以提供关于植入物铰接角度的反馈。图18A示出处于其初始线性状态下的铰接式间隔件12,且图18B示出按某一偏移角处于铰接式状态下的间隔件12,其中可移动标记806平移到不同位置。标记806的线性平移的量将对应于植入物12的铰接角度。虽然仅示出了两个位置,但是将有可能将此作为连续功能,可通过此连续功能使任何既定铰接高度与可移动标记806的特定位置相关。
在这些实施例中,可移动标记806持续滑动以基于位置而提供关于植入物10、12的属性的反馈。还预期,可存在可移动标记806必须处于的缜密位置,所述缜密位置将还能够提供关于植入物属性的其它信息。在此状况下,所有标记804、806的每个缜密配置与特定定向上或特定高度下的植入物固持器608B、608C和植入物10、12的特定几何形状相关。另外,可针对任何其它类型的已导航植入物的其它可变属性而使用可移动标记806的任何运动。
虽然相对于可移动标记806的线性移动描绘并描述,但是可移动标记806不应限于仅滑动,这是因为可存在标记806的旋转或其它移动可适用于提供关于植入物10、12的信息的应用。这一组固定标记804与可移动标记806之间的任何相对位置改变可以是植入物10、12或其它装置的相关信息。另外,虽然以可扩张和铰接式植入物10、12为例进行说明,但是器械608可与其它医疗装置和材料一起工作,例如间隔器、笼、板、紧固件、钉子、螺钉、杆、销钉、线材结构、缝合线、锚定夹、钉、支架、骨移植物、生物制剂等。
现将参考图19-27说明本发明的一个方面,此方面与确定成像装置的成像臂的3维位置以用于拍摄椎体的最佳图像有关。
大多数常规系统不具有导航能力且依赖于用户来定位C形臂。少数系统可能具有一些允许用户返回到先前所存储的位置的导航功能。换句话说,现有系统可具有让用户知道成像系统过去可能在哪里的能力。相反,如结合图19-27描述的本发明提出了让用户知道成像系统将来需要在哪里以拍摄最佳图像。
图23是根据本发明的一个方面的x射线成像装置2300的实例,其能够针对其C形臂2316的3D位置和定向进行自动定位。成像装置2300包含检测器面板组件2314,所述检测器面板组件包含用于从x射线源2312接收x射线透射的传感器阵列(未示出)。在转让给本发明的申请人的第1,044,8910号美国专利中更全面地描述了成像装置2300,所述专利以引用的方式并入本文中。成像装置2300能够通过物理I/O缆线经由连接器面板320或经由包含WiFi、蓝牙等众所周知的无线传输方法以无线方式与手术机器人系统300通信。
不同于不需要校准环的成像系统2300,手动操作的例如1308的C形臂通常会具有如图12A所示的安装到检测器面板组件1316的校准环2200。校准环2200包含两个间隔开的环2210、2212,每个环具有平面表面。每个平面表面包含以选定的图案彼此间隔开的多个不透射线标记2206、2208。两组多个沿圆周间隔开的光学标记2202、2204也安装到环2210、2212。不透射线标记2206、2208用于执行成像装置1308的初始配准(即,C形臂相对于患者的位置和定向从成像空间到相机坐标系统的映射),使得跟踪子系统532可以在手术程序期间跟踪C形臂1308的位置和定向。对于自动导航的成像系统2300,校准环是不必要的,并且可以利用定位在系统的每个移动部分中的光学标记2310或编码器来完成系统的跟踪和导航。编码器可用于在任何使用时间标记C形臂2316的相对位置和定向。
一旦已经执行初始配准,跟踪子系统532的相机326就可以在手术程序期间通过光学标记2202、2204且任选地通过C形臂2316上的标记2310持续跟踪C形臂2316的位置和定向。
图21示出针对每一脊椎节段确定成像装置的3维(3D)位置和定向以用于拍摄最佳AP和横向图像使得仅需要一组图像的方法的流程图。图21中的处理步骤可由计算机408中的图像控制模块409、成像装置2300自身的处理器、其它远程定位的处理器或其组合执行。在一个实施例中,图像控制模块409包含存储在存储器410中的计算机可执行代码。
在步骤2100中,用户(通常是手术室中的X射线技术员)将成像装置2300定位在患者检查台(未示出)周围,使得躺在检查台上的患者位于C形臂2316内。一旦成像装置2300被定位,用户就可拍摄一对x射线图像(一个AP图像和一个横向图像),而不考虑C形臂2316定位的准确度或最佳性,只要包含所关注的脊椎节段即可。图26中示出了典型的AP图像2602和横向图像2604。一旦这两个图像被拍摄下来,它们就会被计算机408接收并存储。
除了所述图像之外,计算机408还接收并存储用于这两个图像2602、2604中的每一个的C形臂的3D位置和定向(例如,C形臂的成像面板/增强器或x射线源或这两者的3D位置和定向)。
在步骤2102中,将所关注的椎体分段以用于稍后分析。分段是一种用来识别例如椎体的身体部位上的某些点或特征的过程。所述分段可以是手动、半自动或全自动的。在图26中示出了分段后的椎体的图示。用于识别椎体相关点的半自动或全自动分段方法在本领域中是众所周知的。举例来说,被称作“ITK-SNAP”(可在www.itksnap.net获得)的开源软件程序可允许用户交互式地将每个椎体分段出来。
步骤2102还可识别脊椎节段,作为分段过程的部分。此识别过程可以是完全手动的,这需要用户识别每个节段。替代地,识别过程也可以是半自动或全自动的。在半自动情况下,用户可识别至少一个节段,而其余节段基于图像处理自动地应用。举例来说,一旦用户将一个椎体识别为L4(如图26中所示),则计算机408例如基于已分段椎体的前凸角度自动地识别所有其它节段。作为双重检查,计算机408可要求用户通过半自动或全自动过程确认自动识别的节段是正确的。
在步骤2104中,计算机408要求用户识别所关注的脊椎节段。随后,用户使用图形用户界面例如通过触碰触摸屏显示装置304上所显示的节段(例如,从L1到L4的四个节段)来识别它们。
在步骤2106中,计算机408从数据库检索包含所关注的椎体的脊柱的3D模型2702。3D模型可基于并非特定于任何患者的统计模型,因为大多数脊柱一般都遵循标准模式,或者它可以基于来自3D扫描的相关特定患者。替代地,可以通过患者特定的数据来增强标准3D模型,例如从图像2602、2604导出的前凸和后凸角度。随后按比例调整检索到的椎体2702,使得所述椎体的大小与AP和横向图像中的那些椎体的大小相同。按比例调整可基于从步骤2102获得的分段信息。
在步骤2106中,对于所关注的每个椎体,计算机408对检索到的选定椎体的3D模型与AP图像2602和横向图像2604中对应的已分段椎体执行对准。例如,可使用的一种方法是“荧光-CT合并(fluoro-CT merge)”。可以在2015年5月28日出版的《应用仿生学和生物力学(Applied Bionics and Biomechanics)》第2015卷第9页的文章编号为478062的标题为《用于脊柱外科手术的图像辅助导航系统(Image-Assisted Navigation System for SpinalSurgery)》(从http://dx.doi.org/10.1155/2015/478062下载)的文章中找到荧光-CT合并的一个算法,此文章以引用的方式并入本文中。实质上,通过计算机408调整3D脊椎模型的位置和定向(包含X、Y、Z、横偏、翻转和俯仰),直到实现最佳对准为止。
图27A和27B以图形方式示出对准方法。图27B示出了来自x射线源2708的横向测试图像2710。横向测试图像2710和x射线源2706分别对应于在拍摄测试图像时成像装置2300的检测器面板组件2314的检测器面板2710和x射线源2312。AP测试图像2710和x射线源2706分别对应于在拍摄测试图像时成像装置2300的检测器面板2710和x射线源2312。可以看出,模型椎体2702一直进行按比例调整和操控到所述椎体与AP和横向图像中对应的椎体最紧密地匹配为止。
对在步骤2104中所识别的每个所关注椎体重复步骤2106。
随后,在步骤2110中,基于在步骤2108中确定的椎体的最佳3D位置和定向,计算机408确定最佳C形臂2316定向和位置(例如,检测器面板2314或x射线源2312或这两者的3D位置和定向),以便使所述椎体以完美的AP和横向角度居中。随后,将确定的用于椎体的最佳C形臂2316定向和位置存储在存储器410中。
步骤2110的最佳C形臂2316定向和位置确定可以在图27A中部分地看到。图27A示出已与AP测试图像2602和横向测试图像2604中的对应椎体对准的模型椎体2702。从显示测试AP图像2602的左屏画面2720可以很容易地看出,C形臂2316的最佳位置将包含将其顺时针旋转约15度并将其向下移动约半个脊椎节段以使椎体居中。根据显示横向测试图像2604的右屏画面2722,C形臂2316的最佳位置将包含将其逆时针旋转约10度并将其向左移动约半个脊椎节段以使椎体在图像中居中。
随后,将椎体的最佳C形臂2316定向和位置存储在存储器中。在一个实施例中,存储用于拍摄这两个图像中的一个的定向和位置信息。那么拍摄另一图像只是将C形臂2316旋转90度的问题。在替代实施例中,存储用于拍摄AP和横向图像两者的定向和位置信息。如果存在尚未处理的任何额外节段,则可重复步骤2106-2110。
在步骤2112中,计算机408在显示装置304中显示供用户选择的用于最佳成像的可用脊椎节段,其中一个实例在图24A中示出。对于每个节段,显示器304显示两个用户输入按钮2402和2404。这些按钮用于将成像系统2300定位到理想或最佳成像位置。取决于所使用的图像设备,定位可以是手动或自动的。按钮2402用于拍摄AP图像,而框2404用于拍摄横向图像。用户可以通过例如鼠标、触摸屏或键盘的输入装置选择要拍摄的图像。在一个实施例中,用户可以通过显示装置304的触敏屏幕触摸输入按钮来进行选择。
在判断2114中,计算机408确定成像装置2300是否具有自动定位能力。自动定位能力允许计算机408发送位置和定向命令,以便以在步骤2110中所确定的最佳3D位置和定向移动和旋转成像装置2300的C形臂2316。
如果确定成像装置2300具有此类能力,则控制传递到步骤2116。在步骤2116中,计算机408将C形臂2316的最佳3D位置和定向发送到成像装置。
在一个实施例中,计算机408将绝对位置和定向数据发送到成像装置2300。在成像装置2300知道其在手术室内的确切位置的情况下,这是可能的。在另一实施例中,计算机408发送一步一步地渐进性地移动并定位C形臂2316的移动指令。计算机408根据成像装置2300对患者的初始配准知道C形臂2316的相对位置。根据台架上的配准数据和光学标记2310,计算机408可以跟踪C形臂2316相对于患者的相对位置和定向。根据跟踪数据并在跟踪标记的同时,计算机可以向成像装置2300发出一系列渐进性定位命令,直到到达最佳C形臂2316位置和定向。
如果在步骤2114中确定成像装置不具有此类自动定位能力(例如,图12A的成像系统1304),则控制传递到步骤2118。在步骤2118中,计算机408在显示装置304上以图形方式显示相对于最佳位置的C形臂1308位置的指示,且使用户移动并定向所述C形臂1308。当用户移动C形臂1308时,持续更新显示装置304上的图形显示以向所述用户显示所述C形臂1308接近其最佳位置的程度。C形臂1308的位置可由校准环2200上的光学标记2202、2204或位于所述C形臂上的一些其它可跟踪标记来跟踪。图25示出图形显示的一个实例。
左侧图像显示C形臂1308的x-y-z坐标。虚线圆表示C形臂1308的最佳位置。虚线圆的中心表示最佳X-Y位置,其中虚线圆的大小表示最佳Z位置。实线圆表示C形臂1308的实际位置。当用户移动C形臂1308时,实线圆移动并改变其大小以指示其相对于最佳位置的实际3D(X-Y-Z)位置。

一旦两个坐标上的实线圆与相应虚线圆对准,成像装置1304就已准备好拍摄适当图像。举例来说,如果用户已经选择了L1-AP 2402,则成像装置2300拍摄AP图像。这可以通过致动成像装置上的适当按钮来完成,或者可以发送来自计算机408的指令来完成。
替代地,一旦脊椎节段被选定,计算机408就可以向成像装置2300发送基于已经通过步骤2102-2110确定的所存储最佳位置和定向来拍摄最佳AP和横向图像两者的指令。
在一个实施例中,对于由成像装置2300拍摄并存储的每个图像,计算机408还将所述图像以及C形臂2316的位置和定向信息存储在存储器410中。这可以通过光学标记2310和2202-2204来实现,也可以通过成像装置的控制C形臂2316的每个轴和3D位置的内部定位元件来实现,所述内部定位元件例如发动机中的编码器。
在拍摄图像时,新获取的图像中可能会出现额外的脊椎节段。举例来说,当拍摄最佳L1图像时,那些图像可能包含例如L4的新节段。在本发明的一个方面中,计算机408将新获取的图像(AP图像及横向图像两者)及其位置和定向存储在存储器410中,且随后如果在步骤2104中将新脊椎节段识别为所关注节段,就对这个新节段重复步骤2102到2112。
在本发明的另一方面中,计算机408可优化已经获得的最佳3D位置和定向数据。在同一实例中,计算机408可基于新获取的AP和横向图像对L2和L3重复步骤2102到2112。由于图像是基于对L1而言最佳的3D位置和定向数据拍摄的,因此所述图像可能还包含对L2和L3而言对准最佳性更高的节段。因此,L2和L3的优化3D和定向位置可能会比以前更加准确。
在步骤2112中,除了用于旧节段(例如,L1-L3)的输入按钮2402、2404之外,计算机408还在显示装置304上显示新节段(例如,L4的AP和横向)的输入按钮2410、2412的图形表示。
可以理解,上述方法大大减少了在手术室中定位x射线成像装置的设置时间,因为每个脊椎节段仅需要两次荧光拍摄(一组AP和横向图像),而不是需要10次或更多。此有利特征产生许多益处,包含大大减少手术时间、大大减少患者以及医务人员受到的辐射照射,以及由于手术程序所需时间减少而降低手术成本。可能更重要的是,因为本发明允许拍摄更佳的图像,所以其允许医生更准确地放置植入物,这使得在许多手术中,患者能够得到更好的手术结果。
在本发明的另一方面,公开了从锥形束CT图像而不是从重建的3D体积数据识别和分段解剖结构的系统和方法。
锥形束CT重建是用于创建3D图像体积的已知方法,其中将从不同已知视角拍摄的2D x射线镜头或图像组合在一起以形成3D体积。通常,x射线在机器人旋转平台(例如,参见图23中的成像装置2300)上拍摄,以小角度增量拍摄各个镜头。例如,成像装置2300的成像机构可以使x射线发射器2312和收集器2314围绕患者旋转360°,每增加1度就拍摄一张x射线图像。已知拍摄每个x射线的定向,锥形束重建软件可以将来自360个单独的2D x射线投影的信息组合成一个3D体积。从一组镜头重建体积的过程是计算密集型的,并且即使使用现代计算机处理器,其通常也需要至少20秒才能完成。重建软件还可能需要在开始图像处理之前存在所有的镜头。换句话说,处理可能需要等到360°旋转完成。
根据本发明的一个方面,设想了一种方法,其对来自采集的一系列镜头的2D样本使用软件以用于分段目的。到成像装置2300的旋转完成为止,组合来自多个2D视角的分段统计将提供3D体积的快速且可靠的自动分段。
图28示出了在使用作为图像控制模块409的一部分的2D自动分段软件时的工作流程的流程图。如关于图5和图21所讨论的,图像控制模块409存储在用于手术机器人系统的计算机子系统504的存储器410中。除了如参考图21所讨论的针对每个脊椎节段确定图像装置的3D位置和定向之外,图像控制软件包括执行图28的分段步骤的附加软件,以及用于锥形束CT的3D体积重建软件。
在步骤2800中,图像控制模块409开始锥形束CT成像装置2300的3D旋转,其包括x射线发射器、收集器和其他电子元器件的初始化。在步骤2802中,图像控制模块409控制成像装置2300的c形臂2316移动到合适的角位置并且拍摄x射线图像。在第一图像的情况下,成像装置2300可以将c形臂2316移动到相对于装置2300的基部的零度偏移处(例如,垂直于地板)以拍摄该第一图像。所拍摄的图像然后被传输到并且存储在存储器410中。在后续成像中,模块409控制c形臂2316移动到预定角度位置。例如,c形臂2316可以移动一度。
由于c形臂2316的定向可以由跟踪子系统532通过跟踪标记2310进行跟踪,因此针对所拍摄的每个图像的定向数据被记录和传输至存储器410以供存储。
在步骤2804中,模块409确定是否应该分析在步骤2802中刚拍摄的图像以进行分段。例如,模块409确定图像是否是N的倍数,其中N是整数。在一个实施例中,N至少为5。如果N=5,则表示每第5个图像(与前一个图像偏移5度)将被分析。
虽然仅选择选定的一组2D x射线图像进行处理以节省时间,但模块409可能对来自c形臂2316的每个2D图像执行分段步骤2806。
如果决定为是,则控制转到步骤2806。在步骤2806中,模块409基于先前存储的模型执行自动分段方法。该模型可以用深度学习或神经网络来增强或训练,以训练计算机模型通过比较图像与一组已知图像来识别图像平面内的结构。在脊柱的情况下,分段可以包括确定每个椎体的边界、其中心(椎体中心的x、y、z位置)、每个主体的3D角定向以及每个主体的脊椎节段。
当自动分段完成时,该方法还可以生成一组置信因子,其中可以包括用于识别脊椎节段、图像中的每个椎体的中心和每个主体的3D角定向的置信水平。
关于针对脊椎节段的置信因子,置信度可以反映软件对于已经正确确定的每个椎体的节段的确定性。
关于椎体中心的X、Y、Z坐标,置信度可以反映中心查找方法对于坐标是在某个公差(例如,1mm)内正确找到的值的确定性。
关于描述3D定向的单位向量,置信度可以反映定向查找方法对于在某个公差(例如,1度)内正确找到定向的确定性。
如果步骤2804的决定为否,则控制转到步骤2808。在步骤2808中,模块409用从步骤2806获得的分段和识别信息更新x射线图像的显示。随着扫描的进行,显示装置304可以显示信息,诸如进度条,并且还可以在屏幕上显示上一个2D镜头,以及调整和显示2D镜头上和在体积将出现在的屏幕区域中的最新标签。
在步骤2810中,确定3D图像旋转是否完成。在一个实施例中,当以一度间隔拍摄了所有360个图像时,旋转完成。如果旋转尚未完成,控制则返回到步骤2802,其中c形臂2316增加预定角度间隔并且拍摄下一个x射线图像。
如果步骤2810中的决定为是,控制则转到步骤2811。在步骤2811中,通过分析从步骤2806获得的所有数据来细化步骤2806中每个图像的分段和识别。
在一个实施例中,使用了加权方案。对于脊椎节段查找方法而言,数据列表可能显示识别顶部节段的平均确定性为85%L1、75%L2、75%L3、75%L4、70%L5,其中那些值中的每一个都被视为36个图像的简单平均值。在这种情况下,该方法将命名顶部节段L1,这是因为这是最好的猜测。可能存在没有明显答案的情况。例如,可能存在这样一种情况,考虑到所有图像,平均确定性分数显示顶部节段被命名为L1并且还显示出下方的下一个节段被命名为L1。在这种情况下,当考虑顶部椎骨与下方的下一个相比时,该方法可以查看周围的单独图像并且查看在任一个镜头中的L1确定性是否比所有其他节段分开更大的边缘。例如,考虑前五个2D x射线图像,假设顶部节段具有如下的确定性分数:
1)85%L1、75%L2、74%L3、76%L4、70%L5
2)85%L1、76%L2、75%L3、75%L4、69%L5
3)86%L1、75%L2、76%L3、75%L4、70%L5
4)85%L1、74%L2、75%L3、75%L4、70%L5
5)84%L1、75%L2、75%L3、74%L4、71%L5
但进一步假设下方的下一个节段具有如下的确定性分数:
1)95%L1、45%L2、45%L3、45%L4、45%L5
2)83%L1、80%L2、85%L3、82%L4、76%L5
3)80%L1、88%L2、83%L3、84%L4、76%L5
4)84%L1、74%L2、84%L3、82%L4、76%L5
5)83%L1、88%L2、78%L3、82%L4、77%L5
在两个数据集中,5个值的平均值将相同。然而,在第二个数据集中,L1在第一个镜头中与竞争名称有很大的分离(95%L1对45%L2或其他=50%的确定性差异),而在第一种情况下,确定性的分离很小(节段名称是L1的与其他名称的确定性差异为9%到11%)。因此,该方法可以决定下方的下一个节段比第一节段更可能是L1。由于已知节段是按顺序编号的,其中L1在L2之上,L2在L3之上,等等,该方法可以首先考虑所有节段,并且找到最可能的整体节段,然后根据其几何意义下降而命名其他的。
然而,对于中心查找方法,椎骨的节段被命名是什么可能并不重要,并且可以不同地使用加权。随着旋转进行,只关注顶部椎骨(无论命名为什么),算法可能会找到中心(XYZ),其具有用于前7个镜头的下列确定性:95%、92%、75%、25%、25%、75%、95%。
该方法可以排除那两个值,而不是在用于仅有25%确定性(甚至考虑加权)的两个镜头的中心的X、Y、Z坐标中取平均值,这是因为其低于选定的阈值(例如,70%)。保留的其他5个图像可以使用加权平均,以便考虑它们的确定性。这5个剩余值的加权平均值将使用常用方法计算:(重量*值)之和/重量之和。
在步骤2811之后,控制然后转到步骤2812。在步骤2812中,模块409通过组合在不同于先前步骤的已知角定向下的所有拍摄的2D x射线图像(例如,1度增量的360个图像)来使用众所周知的锥形束重建方法创建3D图像体积2900(参见图29A)。
在3D重建之后,传统方法将涉及使用刚刚创建的3D体积进行分段和识别的附加步骤,这在计算上非常密集。
然而,根据本发明的原理,分段信息已经在之前的步骤中从2D图像导出。因此,分段和识别信息被送入步骤2812并且叠加在3D图像体积上,如图29B所示,以节省大量时间。由于锥形束旋转通常发生在手术室中,因此节省手术时间对于患者安全和手术成本而言都非常重要。
在步骤2814中,具有分段信息的更新的3D图像体积2910被显示在显示装置304上以供医师操纵。分段信息可以包括描绘每个椎体(例如,参见图26)、中心位置和每个主体的3D定向(2902-2908)的边界点。识别信息可以包括每个椎体(L1-L4)的节段以及在扫描体积中可见的其他骨骼或解剖结构。
模块409的图形用户界面部分允许医师以六个自由度移动3D图像体积2900以帮助医师规划植入物。
本领域普通技术人员将理解,虽然已关于椎体描述了本发明,但是本发明的原理也可以应用于体内的任何其他组织结构,诸如膝关节、踝关节、手指等。

Claims (9)
1.一种从锥形束CT图像识别和分段解剖结构的方法,所述方法包括:
从锥形束CT装置接收至少一个x射线图像,其是从患者的360度扫描获取的多个x射线图像的一部分,所述至少一个x射线图像包含至少一个解剖结构;
基于存储的解剖结构模型识别和分段包含在所述x射线图像中的所述至少一个解剖结构;以及
从源于所述360度扫描的多个x射线图像创建3D图像体积;
将从所述至少一个x射线图像导出的所述识别和分段信息添加到所述创建的3D图像体积。
2.根据权利要求1所述的方法,其中所述识别和分段的步骤包括识别和分段包含在所述x射线图像中的至少一个椎体。
3.根据权利要求1所述的方法,其中:
所述接收步骤包括以规则间隔的角定向接收一组x射线图像;以及
针对每个接收的x射线图像,
确定针对所述每个x射线图像的匹配置信水平;
基于所述置信水平确定最佳识别和分段信息。
4.根据权利要求3所述的方法,其中所述确定最佳识别和分段信息的步骤包括通过基于所述置信水平对所述x射线图像进行加权来确定所述解剖结构的中心。
5.根据权利要求4所述的方法,其中所述确定最佳识别和分段信息的步骤包括排除具有比预定置信水平低的置信水平的所述x射线图像。
6.根据权利要求1所述的方法,其中所述接收步骤包括接收来自所述360度扫描的每个第N个x射线图像,其中N等于5或更大。
7.根据权利要求6所述的方法,其中所述接收每个第N个x射线图像的步骤包括接收用于所述每个第N个x射线图像的角定向信息。
8.根据权利要求1所述的方法,其中所述分段步骤包括确定所述至少一个解剖结构的中心。
9.根据权利要求8所述的方法,其中所述确定置信水平的步骤包括确定所述至少一个解剖结构的所述中心的置信水平。
