技术领域

背景技术
由于眼睛的明确解剖结构,基因疗法载体的眼部施用具有许多优点。特别地,眼睛易于接近使得能够进行快速和渐进的检查;相对封闭的结构和小尺寸的眼睛需要较低剂量的载体进行递送;血-视网膜屏障防止载体泄漏到全身循环中,维持相对免疫免除的环境;并且已经鉴定出主要或部分涉及特定眼部疾病的单个或多个基因。
基因疗法载体的眼部施用已显示出一些有希望的结果。目前,已有许多针对视力丧失相关疾病的临床基因疗法试验,这些试验主要针对遗传性视网膜疾病。例如,临床试验研究了莱伯先天性黑蒙(LCA)、莱伯遗传性视神经病变和视网膜色素变性。迄今为止,AAV载体,尤其是AAV2血清型,已成为眼部基因疗法中最常用的载体。参见Lee等人,Progress inretinal and eye research 68:31-53,2019。

目前,需要靶向视网膜中特定细胞类型的改进的基因治疗方法。而且,没有可以逆转巴藤病症状的疗法。因此,本领域仍然有治疗巴藤病的需求。
发明内容
本公开提供了包含优化的基因疗法载体和糖苷水解酶的组合物,该载体靶向特定细胞类型,例如视网膜中的特定细胞。这些优化的基因疗法载体可用于结合糖苷水解酶的施用,使用玻璃体内递送将转基因递送至特定的视网膜细胞。糖苷水解酶的施用增强了基因疗法在视网膜内的渗透。本公开提供治疗视力相关病症的方法,包括结合糖苷水解酶的施用,使用玻璃体内递送施用优化的基因疗法载体。靶向视网膜中特定细胞类型的基因治疗方法在治疗视力丧失相关疾病方面具有优势。
本公开提供包含基因疗法载体和糖苷水解酶的组合物。例如,糖苷水解酶是神经氨酸酶、乳糖酶、淀粉酶、几丁质酶、纤维素酶、蔗糖酶、麦芽糖酶、转化酶或溶菌酶。在一些实施方案中,基因疗法载体是AAV8、AA9或Anc80。

例如,视力相关病症是巴藤病、先天性白内障、先天性青光眼、视网膜变性、视神经萎缩、眼部畸形、斜视、眼球错位、青光眼、湿性年龄相关性黄斑变性、干性年龄相关性黄斑变性、视网膜色素变性、无脉络膜血症、莱伯先天性黑蒙、莱伯遗传性视神经病变、早发性视网膜营养不良、色盲、性连锁视网膜劈裂症、Usher综合征1B、新生血管性年龄相关性黄斑变性、Stargardt黄斑变性、糖尿病性黄斑变性或糖尿病性黄斑水肿。在一个具体实施方案中,视力相关病症是CLN巴藤病,例如CLN1病、CLN2病、CLN3病、CLN4病、CLN5病、CLN6病或CLN8病。
在任何公开的方法、用途或组合物中,转基因是编码感兴趣的多肽的多核苷酸序列或者是抑制、干扰或沉默感兴趣的基因例如siRNA或miRNA的表达的核酸。示例性转基因是编码RPE65、RPGR、ORF15、CNGA3、CMH、ND4、PDE6B、ChR2、MERTK、hRS1、hMYOJA、hABCA4、CD59、抗hVEGF抗体、内皮抑素-血管抑制素、sFLT01或sFLT-1的多核苷酸。另外的示例性转基因包括针对RTP801的siRNA、针对VEGFR-1的siRNA、针对VEGF的siRNA或针对ADRB2的siRNA。在一个实施方案中,转基因编码CLN多肽,例如CLN1、CLN2、CLN3、CLN4、CLN5、CLN6或CLN8。
在本文公开的任何公开方法、组合物或用途中,基因疗法载体和糖苷水解酶同时或依次施用。此外,在任何公开的方法、组合物或用途中,基因疗法载体和糖苷水解酶使用相同的递送模式施用或各自施用,或各自使用不同的递送模式施用。在任何公开的方法、用途或组合物中,基因疗法载体和糖苷水解酶作为单次施用进行施用,例如将基因疗法载体和糖苷水解酶混合。或者,基因疗法载体和糖苷水解酶分开施用。在一些实施方案中,在施用基因疗法载体前至少约30分钟施用糖苷水解酶。
在任何公开的方法、用途或组合物中,基因疗法载体是AAV1、AAV2、AAV3、AAV4、AAV5、AAV6、AAV7、AAV8、AAV9、AAVRH10、AAVRH74、AAV11、AAV12、AAV13、AAVTT或Anc80、AAV7m8和它们的衍生物。
在任何公开的方法、用途或组合物中,基因疗法载体包含CMV启动子——p546或CB启动子。
此外,在任何公开的方法、用途或组合物中,使用鞘内递送施用基因疗法载体和/或糖苷水解酶,并且该方法进一步包括在施用基因疗法载体后将受试者置于Trendelenburg卧位。

具体实施方式
针对眼睛治疗视力相关疾病的AAV基因疗法的优化需要特异性靶向不同细胞类型。本公开提供了比较不同基因疗法载体、启动子和施用途径的实验数据,以确定用于将转基因靶向递送至小鼠和非人类灵长类动物的视网膜中的特定细胞类型的最佳基因疗法载体。

在任何施用方法中,组合物可包含非离子、低渗透造影剂。例如,所述组合物可以包括非离子低渗透造影剂,其中所述非离子低渗透造影剂选自由以下组成的组:碘比醇、碘海醇、碘美普尔、碘帕醇、碘喷托、碘普胺、碘佛醇、碘昔兰和其组合。
本文提供的方法包括将包括本文提供的rAAV的组合物的有效剂量或有效多剂量施用于有需要的受试者(例如,包含但不限于人类患者的动物)的步骤。如果在视力相关病症的症状出现之前施用该剂量,则该施用是预防性的。如果在视力相关病症的症状出现之后施用该剂量,则施用是治疗性的。有效剂量是减轻(消除或减少)与视力相关病症相关的至少一种症状的剂量,其减缓或阻止所述病症的进展、减少所述病症的程度、导致所述病症的缓解(部分或全部)和/或延长生存期和/或视力。与治疗前的受试者相比或与未治疗的受试者相比,本文提供的方法导致视力丧失或视网膜变性的稳定、进展减缓,或视力或黄斑变性的改善。

糖苷水解酶
本公开提供了在载体中与糖苷水解酶例如神经氨酸酶、乳糖酶、淀粉酶、几丁质酶、纤维素酶、蔗糖酶、麦芽糖酶、转化酶或溶菌酶组合施用优化的基因疗法。外切葡糖苷酶从聚糖链中去除末端唾液酸。这些唾液酸存在于所有细胞类型和大多数分泌蛋白的聚糖链的最外端。已知糖苷水解酶可以清除细胞膜残留物,据认为这种清除允许更大的病毒受体靶向和细胞进入。例如,神经氨酸酶处理去除N-连接的半乳糖残基,从而增强使用N-连接的半乳糖残基作为进入细胞的受体的AAV的渗透,例如AAV9(参见Shen等人,J.Biol.Chem.286:13532-13540,2011)。本文举例说明了向视网膜细胞的递送,然而所公开的方法、组合物和用途可以靶向其中糖苷水解酶清除细胞膜上的受体的任何细胞类型。
还提供了包含优化的基因疗法载体的联合疗法。术语“联合疗法”和“联合治疗”是指将公开的优化基因疗法载体与提高对视网膜细胞的靶向性的试剂例如酶一起施用。在一些实施方案中,该方法包括将糖苷水解酶与基因疗法载体的施用联合地施用给受试者。例如,糖苷水解酶是神经氨酸酶、乳糖酶、淀粉酶、几丁质酶、纤维素酶、蔗糖酶、麦芽糖酶、转化酶或溶菌酶。可以通过本文所述的任何施用途径将所述试剂与载体同时或依次施用。
如本文所使用的组合包含同时治疗或顺序治疗。例如,基因疗法载体和糖苷水解酶使用相同的施用方式同时施用,或者基因疗法载体和糖苷水解酶分别使用不同的施用方式同时施用。在另外的实例中,基因疗法载体和糖苷水解酶使用相同的施用方式依次施用,或者基因疗法载体和糖苷水解酶分别使用不同的施用方式依次施用。本文所描述的方法与标准医学治疗的组合是特别考虑的。
在一些实施方案中,优化的基因疗法载体与糖苷水解酶例如神经氨酸酶同时施用。在其他实施方案中,优化的基因疗法载体在糖苷水解酶例如神经氨酸酶之前或之后立即施用。在其他实施方案中,优化的基因疗法载体在糖苷水解酶如神经氨酸酶的施用的约15分钟内、约20分钟内、约25分钟内、约30分钟内、约45分钟内、1小时内、2小时内、3小时内、4小时内、5小时内、6小时内、7小时内、8小时内、9小时内、12小时内、24小时内、36小时内或48小时内施用。在一些实施方案中,糖苷水解酶在施用优化的基因疗法载体之前或在施用优化的基因疗法载体之后施用。
在一些实施方案中,优化的基因疗法载体和糖苷水解酶例如神经氨酸酶在单次玻璃体内注射中施用。然而,本公开还提供了在单独的玻璃体内注射中施用优化的基因疗法载体和糖苷水解酶例如神经氨酸酶的组合。例如,在施用优化的基因疗法载体前约30分钟施用糖苷水解酶,例如神经氨酸酶。此外,优化的基因疗法载体和糖苷水解酶可以使用不同的施用方式施用。
实施例
虽然以下实施例描述了具体的实施方案,但应理解,本领域技术人员将想到变化和修改。因此,只有权利要求中出现的这些限制才能适用于本发明。
实施例1
scAAV9.GFP和Anc80.GFP的产生
人GFP cDNA克隆获自马里兰州Rockville的Origene。在杂交鸡β-肌动蛋白启动子(CB)、CMV增强子-启动子或P546启动子下,将GFP cDNA进一步亚克隆到自互补AAV9基因组或Anc80基因组中,并进行体外和体内测试。图1中提供了质粒构建体的示意图,其显示插入AAV2 ITR之间的GFP cDNA。质粒构建体还包含CB启动子、内含子例如猿猴病毒40(SV40)嵌合内含子和牛生长激素(BGH)多腺苷酸化信号(BGH PolyA)中的一种或多种。将图1中的构建体封装到AAV9基因组或Anc80基因组(统称为“AAV”)中。
实施例2
联合神经氨酸酶的scAAV9.CB.GFP玻璃体注射
将小鼠麻醉用于如上所述的玻璃体内注射。用30G针在异色边缘(lumbus)和巩膜之间做一个小切口。使用通过管子连接到Hamilton注射器或通过30G针头和Hamilton注射器的细玻璃吸管,将病毒或载体通过该切口递送到玻璃体空间中。在注射之前和之后,局部施加奥复星和vetropolycin,允许小鼠通过标准护理(恢复用加热笼,笼底食物,长吸管)恢复并监测直至稳定。
通过一次玻璃体内注射将scAAV9.CB.GFP施用于小鼠(1-5个月大),并在两个月的过程中在不同时间点监测表达。AAV和Anc80的施用剂量范围为9x109和3.2x1010 vg,其用PBS稀释或直接注射rAAV。此外,在单次玻璃体内注射中,rAAV与神经氨酸酶或不与神经氨酸酶同时施用。对于同时施用,在注射前立即将rAAV与神经氨酸酶混合,并作为单一溶液施加。如图2A所示,玻璃体内注射导致GFP在视网膜中的表达。此外,施用scAAV9.CB.GFP联合神经氨酸酶增强了转基因到视网膜外层的渗透。下面列出了视网膜染色标记物的关键词:

Otx2是所有双极细胞的核标志物(红色染色)并染色视网膜的内核层。如图2C所示,玻璃体内注射AAV9.CB.GFP,将转基因递送至双极细胞,以及神经氨酸酶,增强了转基因对视网膜内核层的渗透。
Pax6是无长突细胞的标志物(红色染色),并且染色视网膜的内核层。如图2D所示,玻璃体内注射AAV9.CB.GFP,将转基因递送至无长突细胞,以及神经氨酸酶,增强了转基因对视网膜内核层的渗透。
Sox2是穆勒神经胶质细胞的标志物(红色染色),并且染色视网膜内层的所有核。如图2E所示,玻璃体内注射AAV9.CB.GFP,将转基因递送至穆勒神经胶质细胞,以及神经氨酸酶,增强了转基因对视网膜内核层的渗透。
实施例3
为了进一步研究神经氨酸酶对在有或没有神经氨酸酶的情况下接受玻璃体内注射AAV9.CB.GFP的8周龄野生型小鼠的转导的影响,如图3所示。如实施例2中详细描述的,将~2x1010 vg的AAV9.CB.GFP注射到小鼠中。图4A和图4B中的视网膜组织针对转基因GFP和双极细胞特异性标记物Otx2进行了染色。在玻璃体内注射AAV载体之前或之后添加神经氨酸酶显著增加了双极细胞的病毒转导(见图4C)。
