技术领域
本发明专利涉及医药发明领域,具体为一种辅助治疗肝癌的中药组合物及其制备方法。
背景技术
恶性肿瘤就是人们所说的癌症,它是100多种相关疾病的统称。当身体内细胞发生突变后,它会不断地分裂,不受身体控制,最后形成癌症。多数癌症是根据他们起始的器官或细胞类型来命名的。癌症是常见多发病,严重危害着人民的生命和身体健康,由于癌症发病的部位不同,可有各种不同的癌症,如肝癌、胃癌、骨癌、皮肤癌、肠癌等等。
肝癌是指来源于肝细胞和肝胆管细胞的恶性肿瘤,分为原发性和继发性两大类,是我国常见的恶性肿瘤之一,我们国家是世界上肝癌高发的国家之一。早期肝癌常症状无特异性,有时候病人会觉得右上腹胀痛、隐痛不适,肝癌在中晚期的时候会出现比较明显的症状,表现为右上腹的胀痛、钝痛,甚至一部分病人由于肝癌的自发破裂,引起剧烈腹痛。中晚期肝癌的症状则较多,常见的临床表现有肝区疼痛、腹胀、纳差、乏力、消瘦,进行性肝大或上腹部包块等;部分患者有低热、黄疸、腹泻、上消化道出血等。肝癌一旦发现,需要积极治疗,以挽救生命。临床上一般采取西医的手术、放化疗与中药结合疗法,但晚期患者因癌细胞扩散而治愈率较低,因此要做到肝癌的早期发现、早期诊断、早期治疗。中医认为肝癌的形成与情志具有非常重要的关系。第一、在肝癌的发生中,很多的肝癌患者都有情志障碍,或者有情感打击,或者是感情出现挫折等现象的发生。这些情况可以造成肝脏的肝气郁结、气血运行不畅,从而出现急剧的发生。第二、是中医讲的邪毒,邪毒集聚在肝脏,这些病毒可以造成肝硬化,出现肝癌等。第三、是血虚,在肝脏的发病里占有很重要的因素,肝是主血的,血脉亏虚、血运不足,造成肝脏的失养,也会造成肿瘤的发生。肝癌在中医临床中多属于“肝积”、“痞气”、“膨胀”等范畴。祖国医学认为原发性肝癌是由于感受湿热毒邪迁延留滞、饮食内伤、七情郁结等所致肝脾失和,气血痰毒瘀结脉络,日久渐聚积成块停于胁腹而成。中医对原发性肝癌的辨证治疗,多以健脾益气为主,配合化癖软坚、清热解毒,补气补血,体现肝癌以虚为本、本虚标实的病机。首当固本,再依据临床症状予以理气,祛瘀及清热解毒之法。调节五脏六腑功能恢复机体阴阳气血平衡状态提高患者自身抗肿瘤免疫能力改善患者全身身体状况最大程度的延长患者生存期。目前利用现代中医理论以及古人用药搭配理念研发所得的中药制剂,在治疗肝癌方面取得了良好疗效,同时增强了机体活力与抵抗力,并最大限度的减少了药物的副作用,提高了患者生活质量。
综上所述,肝癌是个世界性的难题,因此如何治疗肝癌,减少病人的痛苦、改善患者的生存质量是目前急需解决的问题,现有医疗技术仍需要加大对肝癌治疗的进一步研究。
发明内容
本发明的目的在于提供一种辅助治疗肝癌的中药组合物。该组合物具有组方严谨,配伍科学,疗效显著,能最大限度地发挥药物疗效,与顺铂配合用药,具有协作增效的效果。
本发明的另外一个发明目的是提供以上所述组合物的的制备方法,该制备方便简单,患者依从性好,毒副作用小。
为了实现以上发明目的,本发明的具体技术方案为:
一种辅助治疗肝癌的中药组合物,由以下重量份的原料制成:清半夏5-15份、陈皮5-15份、茵陈5-15份、厚朴10-20份、荆芥10-20份、防风10-20份、地肤子10-20份、生薏米10-20份、白鲜皮5-15份、莪术5-15份、红景天7-17份、炒山药5-15份、灵芝5-15份、火麻仁5-15份、木香4-8份、黄芩5-15份、酒大黄1-5份、枳实5-15份、细辛5-15份、炒栀子5-15份、淡竹叶4-8份、内金5-15份、炒山楂5-15份、炒麦芽5-15份、炒神曲5-15份。
作为本发明中一种较好的实施方式,一种辅助治疗肝癌的中药组合物,由以下重量份的原料制成:清半夏10份、陈皮10份、茵陈10份、厚朴15份、荆芥15份、防风15份、地肤子15份、生薏米15份、白鲜皮10份、莪术10份、红景天12份、炒山药10份、灵芝10份、火麻仁10份、木香6份、黄芩10份、酒大黄3份、枳实10g、细辛10份、炒栀子10份、淡竹叶6份、内金10份、炒山楂10份、炒麦芽10份、炒神曲10份。
作为本发明中一种较好的实施方式,以上所述的一种辅助治疗肝癌的中药组合物的制备方法,包含以下步骤:
1)按比例称取各原料,然后将清半夏、陈皮、茵陈、厚朴、荆芥、防风、地肤子、生薏米、白鲜皮、莪术、红景天、炒山药、灵芝、火麻仁、木香、黄芩、酒大黄、枳实、细辛、炒栀子、淡竹叶、内金、炒山楂、炒麦芽、炒神曲,分别粉碎过60目筛,加8-10倍量乙醇回流提取二次,每次2-4小时,合并提取液,滤过,减压回收乙醇并浓缩至稠膏,真空干燥得醇提物;
2)取步骤1)醇提后药渣,加10倍量水煎煮二次,每次2小时,合并煎液,滤液减压浓缩至相对密度为1.13-1.20,真空干燥即得水提物;
3)将步骤1)得到的醇提物和步骤2)得到的水提物混合均匀,共同粉碎,即得。
作为本发明中一种较好的实施方式,将步骤3)制备得到的药物粉末过120目筛,加入药学上可接受的载体,制成成常规制剂。

本方法中各药物说明:
清半夏:南星科植物半夏(Pinellia ternata(Thunb.)Breit.)的块茎。生半夏用白矾加工炮制后入药,清半夏毒性及辛燥之性降低。性味辛,温,有毒。入脾、胃经。燥湿化痰,消痞散结。同时清半夏还可以跟薏米这类药物使用,对于脾虚、湿阻所造成的痰湿也具有比较好的效果。
陈皮:芸香科植物橘 Citrus reticulata Blanco 及其栽培变种的干燥成熟果皮。气香,味辛、苦。归肺、脾经。理气健脾,燥湿化痰。苦能泻能燥,辛能散,温能和。其治百病,总是取其理气燥湿之功,同补药则补,同泻药则泻,同升药则升,同降药则降。配伍厚朴、半夏去湿而能健脾行气。
厚朴:木兰科木兰属植物厚朴Magnolia officinalis Rehd.et Wils.或凹叶厚朴Magnolia offinalis Rehd.et Wils.var.biloba Rehd.et Wils.的干燥干皮、根皮及枝皮。具有燥湿消痰,下气除满的功效。味苦、辛,性温;归脾、胃、肺、肝经。主治湿阻中焦、脘腹胀满,食积气滞、腹胀便秘,肺气不降、痰饮喘咳,湿浊不分、霍乱泄泻,七情郁结、痰气互阻之梅核气等。厚朴酚及其甲醇提取物对体内二期致癌试验引起的小鼠皮肤肿瘤有明显的抑制作用。
茵陈:菊科植物茵陈蒿Artemisia capillaris Thunb.的幼苗。性苦、辛,微寒。归脾、胃、肝、胆经。功能清热利湿;退黄。主治:黄疸、小便不利、湿疮瘙痒、传染性黄疸型肝炎等。药理学研究有利胆,保护肝功能,解热,抗炎,降血脂,降压,扩冠等作用。
荆芥:唇形科植物荆芥Schizonepeta tenuifolia Briq.的干燥地上部分。味辛,性微温。归肺、肝经。解表散风,透疹,消疮。
防风:伞形科多年生草本植物防风Saposhnikovia divaricata (Turcz.)Schischk.的根。味辛、甘,性微温。归肺、脾、肝、膀胱经。气香升散,具有祛风解表,除湿止痛,疏肝解痉。防风多糖体内应用能够明显抑制实体肿瘤的生长,提高肿瘤免疫小鼠腹腔的吞噬活性,并能提高肿瘤免疫小鼠腹腔肿瘤细胞混合接种时的肿瘤活性。地肤子:藜科植物地肤Kochia scoparia(L.)Schrad.的果实。性寒,味辛、苦。功能清热利湿,祛风止痒。
生薏米:禾本科植物薏米的种仁。其性凉,味甘、淡,入脾、肺、肾经,具有利水、健脾、除痹、清热排脓的功效。长于利水渗湿,清热排脓,除痹止痛,常用于小便不利,水肿,脚气,肺痈,肠痈,风湿痹痛,筋脉挛急及湿温病在气分。临床实验证实,薏米具有一定的抑菌、抗病毒功效。生物实验还证明薏米对抗癌也有比较显著的疗效。当癌症患者在放疗、化疗时出现白细胞下降、食欲不振、腹水、浮肿时,可以用薏米佐餐。
白鲜皮:芸香科白鲜属植物白鲜Dictamnus dasycarpus Turcz.的干燥根皮。味苦,性寒;归脾、胃、膀胱经。具有清热燥湿,祛风解毒的功效。主治湿热疮毒,湿疹,疥癣,湿热黄疸,风湿热痹。《本草纲目》:“白鲜皮,气寒善行,味苦性燥,足太阴、阳明经去湿热药也。兼入手太阴、阳明,为诸黄风痹要药。
莪术:姜科姜黄属植物莪术Curcuma aeruginosa Roxb.[C.zedoaria nonRosc.]、广西莪术Curcuma kwang siensis S.G.Lee et C.F.Liang和温郁金Curcumawenyujin Y.H.Chen et C.Ling的根茎。味辛、苦,性温;归肝、脾经具有行气破血,消积止痛之功效。用于西医诊为肝脾肿大、肝硬化、肝癌、胃癌、食道癌、乳腺癌、宫颈癌、卵巢囊肿、子宫肌瘤慢性胃炎胃癌术后消化不良等属气滞血瘀证者,软组织损伤,外伤肿痛属瘀血阻滞证者。莪术挥发油对多种癌细胞既有直接破坏作用,又能通过免疫系统使特异性免疫增强而获得明显的免疫保护效应,从而具有抗癌作用。同时莪术还有抗炎、抗血小板聚集、抑制血栓形成的作用。
红景天:景天科属植物红景天Rholiola rosea Limn.或大花红景天R.euryphylla(Frod.)S.H.Fu.的根茎。味甘,性寒;归脾、肺经。具有健脾益气,清肺止咳,活血化瘀的功效。主治脾气虚证,肺阴虚、肺热咳嗽。红景天素对S180肉瘤细胞有抑制作用。
炒山药:薯蓣科薯蓣属植物薯蓣Dioscorea opposita Thunb.的根茎。味甘,性平;归脾、肺、肾经。具有益气养阴,补脾肺肾,固精止带的功效。主治脾虚食少,倦怠乏力,便溏泄泻,肺虚喘咳,肾虚遗精,带下尿频,内热消渴等。山药具有增强细胞免疫和体液免疫的作用。
灵芝:多孔菌科植物赤芝或紫芝的全株。性为甘,平。归心、肝、脾、肺、肾五经。主治虚劳、咳嗽、气喘、失眠、消化不良,恶性肿瘤等。经过科研机构数十年的现代药理学研究证实,灵芝对于增强人体免疫力,调节血糖,控制血压,辅助肿瘤放化疗,保肝护肝,促进睡眠等方面均具有显著疗效。
火麻仁:桑科植物大麻(Cannabis sativa L.)的干燥成熟种子,别名又叫大麻仁、火麻、线麻子。味甘,性平,归脾、胃、大肠经,功效润肠通便、润燥杀虫。麻仁甘平滑利,柔中有刚,能入脾滋其阴津,化其燥气。
木香:菊科云木香属植物木香Aucklandia lappa Decne.的根。味辛、苦,性温,归脾、胃、肝、肺经。具有行气止痛,调中导滞的功效。主治胸胁胀满,脘腹胀痛,呕吐泄泻,痢疾后重。气芳香而辛散温通,擅长调中宣滞,行气止痛,尤对脘腹气滞胀痛之证,为常用之品。
黄芩:唇形科黄芩属植物黄芩Scutellaria baicalensis Ceorgi的干燥根。味苦,性寒;归肺、胆、脾。具有清热燥湿,泻火解毒,止血,安胎的功效。主治湿温,暑湿,胸闷呕恶,湿热痞满,黄疸泻痢,肺热咳嗽,高热烦渴,血热吐衄,胎动不安。
酒大黄:蓼科植物掌叶大黄,唐古特大黄或药用大黄的干燥根及根茎。酒大黄是属于大黄药材炮制品中的一种。味苦,性寒。归胃、脾、大肠、肝、心经。酒制善清上部火热,制大黄泻下力缓,泻火解毒,用于火毒疮疡,正虚邪实证。
枳实:芸香科柑橘属植物酸橙Citrus aurantium L.及其栽培变种或甜橙Citrussinensis Osbeck的干燥幼果。苦、辛、酸,性温;归脾、胃、肝经。具有破气消积,化痰除痞的功效。主治胃肠积滞,湿热泻痢,胸痹,结胸,气滞胸胁疼痛,产后腹痛。
细辛:马兜铃科细辛属植物细辛Asarum sieboldii Miq.的根,气味辛、温,无毒。香散温通,气胜味烈,具有较好的蠲痹通阳、散寒止痛、宣通鼻窍、温肺化饮的作用,可以应用于肺系疾病、祛脏腑经络之寒,治疗脏寒痹证及各类痛症等。
炒栀子:栀子是茜草科植物栀子的果实。炒栀子性味苦寒,入心肝肺胃三焦经,功效是泻火除烦、清利湿热、凉血止血、消肿止痛。栀子苦能降泄,寒能清热,轻浮上达,既入气分,又行血分,既长于泻心肺之实热而除胸膈烦热,又能泻三焦实火,清肝胆之湿热而利小便。
淡竹叶:禾本科淡竹叶属植物淡竹叶Lophatherum gracile Brongn.的干燥茎叶。味甘、淡,性寒;归心、胃、肝经。具有清热泻火,除烦,利尿的功效。主治热病烦渴,口疮尿赤,热淋涩痛。
内金:也叫鸡内金,是鸡的砂囊内壁,鸡的消化器官,用于研磨食物,因入药治疗效果极佳,故而以“金”命名。鸡内金具有健运脾胃、促进消化、固涩精液、止遗止泄、通畅淋证、化坚消结石的功效。可用于治疗食物积滞不消化、腹胀、食欲不振、急慢性胃肠炎导致的呕吐腹泻、小儿营养障碍、小儿脾胃受损、遗精、遗尿、尿路感染、泌尿系统结石、胆道系统结石等。
炒山楂:为蔷薇科山楂属植物山里红Crataegus pinnatiida Bge.var.majorN.E.Br.或山楂Crataegus pinnatifida Bge.的干燥成熟果实。味酸、甘,性微温;归脾、胃、肝经。具有消食化积,行气散瘀的功效。主治饮食积滞,泻痢腹痛,疝气痛,瘀阻胸腹痛,痛经。炒山楂的功效是可以健脾胃、消食积、化淤滞。还可以疏通肝经,对于长时间生气所导致的气滞、焦虑、抑郁,有一定的调节作用。
炒麦芽:别名大麦糵、麦糵、大麦毛、大麦芽。为禾本科大麦属植物大麦Hordeumvulgare L.的成熟果实经发芽干燥而成。味甘,性平;归脾、胃、肝经。具有消食健胃,回乳消胀的功效。主治米面薯芋食滞,断乳,乳房胀痛。炒麦芽味甘、咸,性平偏温,以消食和中力胜,有调和脾胃之效,用治于饮食停滞,脘腹胀满,不饥恶食,乳积不消,脾胃虚弱,饮食减退等。
炒神曲:别名六神曲、六曲,是用面粉、赤豆、杏仁、青蒿、苍耳、辣蓼混合后发酵而制成的曲剂。味甘、温,归脾、胃经,具有健胃和中的作用,配伍对应的中药可以用于治疗肠鸣腹泻、食少纳呆等病症。可以配伍相应的中药治疗食积停滞,脘腹胀痛等病症。
中医治疗肝癌的基本原则辩证施治,认为虚证是肿瘤发生发展的重要原因和病机。因正气不足、气血虚弱、导致脏腑功能失调,因而出现气滞、血瘀、湿聚、痰结等一系列病理变化,并最终形成肿瘤,因此,需对症调配。
半夏燥湿化痰,陈皮消痞散结,理气健脾燥湿,与厚朴配合去湿而能健脾行气。茵陈、地肤子善清肝搜邪,且清而不克。荆芥、防风皆味辛而微温,同人肝经,理气疏肝,祛风解表,为风药中之润剂。莪术长于破血行气,消积止痛。薏米长于利水渗湿,清热排脓,除痹止痛。白鲜皮气寒善行,味苦性燥,清热燥湿,祛风解毒。诸药相伍疏肝健脾、化瘀消积、泻火解毒,是为君药。红景天甘寒,健脾益气,山药甘平,补脾益胃。两药配伍,可增强健脾益气,益胃养阴。共奏健脾益气,活血化瘀,运湿和中的功效。灵芝微苦、平,扶正固本,具有增强人体免疫功能,消除疲劳,保肝护肝,抑制癌细胞生长。火麻仁甘平滑利,柔中有刚,能入脾滋其阴津,化其燥气。四药合用,补益脾肾,滋阴养血,培元固本,是为臣药。木香辛温芳香,有健胃消食,行气消胀,行气止痛之功,黄芩苦寒,气薄味厚,清热燥湿,泻火解毒,二药伍用,一温散,一寒折,调升降,理寒热,调气行滞。枳实苦辛酸温,有破气消积,化痰除痞之功;厚朴苦辛温,可燥湿消痰,下气除满。二药配伍,增强行气散结,消痰除满作用。炒栀子苦能降泄,寒能清热,轻浮上达,既入气分,又行血分,泻火除烦、清利湿热、凉血止血、消肿止痛。细辛香散温通,气胜味烈,具有较好的蠲痹通阳、散寒止痛。淡竹叶性味甘淡,调和清热解毒之力,双解表里之热,清热除烦,兼以生津,诸药合用,清热利胆、养阴柔肝,凉血解毒共为佐药。内金理气消胀,配炒三仙(炒神曲、炒麦芽、炒山楂),畅气消滞,助消化而除胀满,运中焦而健脾胃,是为使药。全方合用,用药宜各方照顾,于理气之中益气,健脾之中加以运脾,养血之中酌加活血,滋肝之中兼予达郁,使邪去而不伤正,补而邪不滞。共奏清热利湿、疏肝健脾,软坚消痞,缓急止痛,调和气血之功,故用之临床,效果颇佳。
与现有技术相比,本发明的积极效果体现在:
(一)该中药组合物利用各药材之间君臣佐使的严格配比关系,具有扶正培本、益气补肾、利湿解毒和活血散结之功效。药理实验显示,本发明所述中药组合物可有效抑制肿瘤增长及促进肿瘤细胞调亡,充分发挥了对肝癌前病变的防治作用,对肝癌细胞具有明显的抑制效果,可有效用于肝癌的治疗作用。临床实践充分证明,辨证施治可以缓解症状、提高生存质量、延长生存期、降低放化疗的毒副作用等,可有效降低复发率,提高患者的生存质量,能明显改善患者的临床症状,配合西医的介入治疗以及放化疗技术具有显著的减毒增效作用,有临床推广应用价值。
(二)药物的制备方法简单易操作,更够保留药物组分的有效成分,无明显毒副作用,且生产成本低,适合广大肝癌患者服用,有较好的社会和经济价值。
(三)该药物还可有效用于肝癌的治疗,在试验过程中我们发现本发明提取物与其他化疗药物杀伤肝癌细胞的协同作用。将本发明提取物与临床一线抗癌药顺铂联合应用,采用MTT法观察其对人肝癌细胞SMMC-7721的抗肿瘤活性,实验结果显示联合应用对人肝癌细胞SMMC-7721可产生显著的协同杀伤作用。两药联用尤其是 IC50分别为69.59mg/L和2.73mg/L时对人肝癌细胞SMMC-7721增殖抑制作用最佳。表明本发明提取物与顺铂联合用药确实可起到提高药效,降低用药剂量的作用,还有助于克服单用化疗药所引起的毒副作用大、容易发生耐药等缺点。

一种辅助治疗肝癌的中药组合物,由以下重量份的原料制成:清半夏5-15份、陈皮5-15份、茵陈5-15份、厚朴10-20份、荆芥10-20份、防风10-20份、地肤子10-20份、生薏米10-20份、白鲜皮5-15份、莪术5-15份、红景天7-17份、炒山药5-15份、灵芝5-15份、火麻仁5-15份、木香4-8份、黄芩5-15份、酒大黄1-5份、枳实5-15份、细辛5-15份、炒栀子5-15份、淡竹叶4-8份、内金5-15份、炒山楂5-15份、炒麦芽5-15份、炒神曲5-15份。

作为优选,以上所述的一种辅助治疗肝癌的中药组合物的制备方法,包含以下步骤:
1)按比例称取各原料,然后将清半夏、陈皮、茵陈、厚朴、荆芥、防风、地肤子、生薏米、白鲜皮、莪术、红景天、炒山药、灵芝、火麻仁、木香、黄芩、酒大黄、枳实、细辛、炒栀子、淡竹叶、内金、炒山楂、炒麦芽、炒神曲,分别粉碎过60目筛,加8-10倍量乙醇回流提取二次,每次2-4小时,合并提取液,滤过,减压回收乙醇并浓缩至稠膏,真空干燥得醇提物;
2)取步骤1)醇提后药渣,加10倍量水煎煮二次,每次2小时,合并煎液,滤液减压浓缩至相对密度为1.13-1.20,真空干燥即得水提物;
3)将步骤1)得到的醇提物和步骤2)得到的水提物混合均匀,共同粉碎,即得。

将上述重要组合物制备成口服制剂,且该口服制剂为片剂的制备方法步骤如下:
按照前述的重量称取以上中药组份,将其分别粉碎成粗粉过60目筛,分别加9倍质量的乙醇进行回流提取二次,每次3小时,合并提取液,滤过,减压回收乙醇并浓缩,得醇提物;取醇提后药渣,分别加10倍量水煎煮二次,每次2小时,合并煎液,滤液减压浓缩至相对密度为1.15,得水提物;取所得醇提物和所得水提物混合均匀,经喷雾干燥成细粉,加入药学上可接受的适量辅料,压片包衣即可。规格:每片标示重量为0.4g。服用量:成人:6片/次,3次/天。

一种辅助治疗肝癌的中药组合物,包括以下含量的组分: 清半夏120g、陈皮120g、茵陈120g、厚朴130g、荆芥130g、防风130g、地肤子130g、生薏米130g、白鲜皮120g、莪术120g、红景天120g、炒山药120g、灵芝120g、火麻仁100g、木香70g、黄芩120g、酒大黄20g、枳实120g、细辛100g、炒栀子100g、淡竹叶60g、内金120g、炒山楂120g、炒麦芽100g、炒神曲120g。
将上述重要组合物制备成口服制剂,且该口服制剂为胶囊剂的制备方法步骤如下:

将上述重要组合物制备成口服制剂,且该口服制剂为颗粒剂的制备方法步骤如下:

北京医科大学/中国协和医科大学联合出版社出版的图书参照我国卫生部医政司编《新编中国常见恶性肿瘤诊治规范》的原发性肝癌诊断标准。
1.2中医辨证标准
参照国家中医药管理局 /国家卫生健康委员会《中医病证分类与代码》。
2、病例纳入标准
本发明对原发性肝癌介入术后、射频术后临床患者中医证候及生活质量的改善情况进行了研究和分析。
3、观察对象
2015年1月--2016年12月甘肃省人民医院医院肿瘤科门诊及住院患者,共计160例,其中男性89例,女性71例;年龄在20--70岁之间。平均年龄为(52士8.5)岁。预计生存在6个月以上;对照组给予单纯介入治疗,治疗组给予介入联合本发明中药组合物治疗。
4、试验方法
观察指标包括:患者临床中医证候疗效,生活质量,生存期,安全性观察。
患者服用本发明实施例1制得的片剂,每次6片,每日3次,温开水冲服。2个月为1个观察疗程,用药1个疗程。
观察期间160例患者中治疗组75例获得随访,5例失访,对照组 74例获得随访,6例失访,总失访率6.9%(11/160例)。
5、试验结果:
5.1肝癌患者除了疼痛,其临床症状主要以两胁疼痛、腹水、腰膝酸软、纳呆、腹胀、头目眩晕、五心烦热、情绪抑郁、盗汗、耳鸣、乏力为主,临床观察过程中,经给于本发明中药组合物配合治疗后,多项症状的频数分布较治疗前明显下降,具有统计学意义(P<0.05)。

5.2 中医证候的临床疗效:75例患者经肝癌治疗后,显效18例,有效 46例,总有效率为85.3%。患者生活质量治疗后较治疗前有明显改善,差异具有统计学意义。
5.3生存率:治疗组0.5年、1年、1.5年、2年生存率优于对照组(P<0.05)。详见表3。

5.4安全性评价:治疗过程中两组均有少数患者出现发热、恶心呕吐、 肝区疼痛等不良反应,但均轻微,考虑与介入或射频治疗有关,对症处理后可缓解,未见其他严重不良反应。
肝癌患者行手术根治或介入治疗的目标不单单是为了缩小瘤块和延长患者生存时间,提高和维护患者的生存质量同样重要。实验和临床观察结果显示:本发明药物对肝癌患者体力状况、主要临床症状的改善优于化疗药单独使用。本发明中药组合物辅助配合用于原发性肝癌手术后治疗,结果显示,能够有效地改善肝癌患者的临床症状,能明显减少癌症复发率;用于介入术后辅助治疗,具有很强的增效作用;能明显延长中晚期肝癌患者的生存期、提高其生活质量。但由于发病的具体原因难于确定和患者的个体差异,现难以确定对哪些具体患者有特效,需要广大医务工作者在这方面做进一步的实验研究。
病例1:
杜某,男,66岁,北京人,2014年在北京协和医院检查发现肝内多个占位 ,诊为肝癌合并肝硬化,患者行介入治疗后复查提示肿瘤有复发情况。服用按照实施例2中的中药组合物配方制备的汤剂2个月,医院复查为肝内占位缩小,继续服用该药剂,1年后复查肝内占位明显缩小。
病例2:
蔺某,男,51岁,甘肃嘉峪关市人,2018年6月,患者在兰州大学附属医院诊断为肝右叶肝癌并肝内多发转移、门脉癌栓,AFP 579ng/ml ,无法行手术治疗。采用化疗及本发明药物配合治疗。服药1个月后,患者原有腹胀痛、纳呆、头目眩晕症状减轻,精神好转,继续使用本方,服药2个月后复查CT提示肝内肿块较前缩小、减少,AFP下降至326ng/ml ,仍继续使用本方,2个月后复查AFP下降至正常,复查CT提示肝内只有2个小肿块,未见明显强化,患者自觉生活、活动如常﹐后仍间断调整服用本发明组合物组方,定期复查未见有新变化,身体状况好,至今已带瘤生存3年。
实验2 本发明所述的中药组合物增强顺铂对人肝癌细胞的增殖抑制实验
1药品及试剂顺铂氯化钠注射液:(江西东亚药业有限责任公司); DMEM高糖培养粉(美国 Gibco公司);胎牛血清(杭州四季青公司);胰蛋白酶(美国Invitrogen);EDTA(合肥生工生物技术研究所);四甲基偶氮唑盐(北京华美公司分装);青–链霉素(自行配制DMSO,美国Sigma公司);其它试剂均为国产AR级试剂。
人肝癌细胞株SMMC-7721 ,购自兰州大学药学院微生物实验室。
2、实验方法
2.1 细胞培养及细胞悬液的制备人肝癌SMMC-7721细胞培养于含10%胎牛血清的高糖DMEM培养液中,置37℃、5%CO2的培养箱, 待细胞长满瓶底后,用等体积质量浓度为2g/L的胰酶和质量浓度为0.25g/L的EDTA消化几分钟,镜下观察胞质回缩、细胞间隙增大后,立即停止消化,离心去上清,更换培养液,分瓶培养,取对数生长期细胞用于实验。
2.2本发明提取物溶液与顺铂溶液的配制
本发明提取物溶液与顺铂溶液的配制:取按照实施例1制备得到的药物粉末(即取所得醇提物和所得水提物混合均匀,经喷雾干燥成的细粉)与DMSO按1mg: 1mL的比例配制成药物原液,其浓度为1mg/mL;1mL药物原液(1mg/mL)+9mL10%胎牛血清的高糖DMEM培养液,分别调节培养液剂量稀释药物(本发明组合物药物的终浓度为14.2mg/L、28.5mg/L、57mg/L、115mg/L、228mg/L、450mg/L,顺铂的终浓度为0.65mg/L、1.3mg/L、2.5mg/L、5.2mg/L、10.5mg/L、20mg/L,两药合用的是1:1方案)。
2.3 肿瘤细胞体外增殖抑制实验
采用MTT法,设实验组、细胞对照组和空白对照组,取对数生长期细胞(1~5)×107/L接种于96孔板上,每组设3个复孔,每孔接种100ul,置 37℃、5%CO2的培养箱中24h后,弃上清液,实验组每孔分别加入200ul培养液稀释的上述药物。细胞对照组和空白对照组只加入等量的培养液,继续培养48h,在终止培养前4h实验组及对照组每孔加入20ul四甲基偶氮唑盐(5g/L),再培养4h后,小心吸弃上清,并加入150ul青–链霉素溶解四甲基偶氮沉淀,用振荡器混匀后,在酶标仪上用550nm测定吸光值(A)。按下式计算药物对肿瘤细胞生长的抑制率:肿瘤细胞生长抑制率(IR)=(正常对照组A值-实验组A值)/正常对照组A值×100%。

本发明中药组合物提取物与顺铂单独应用对SMMC-7721细胞均有增殖抑制作用,且呈现明显的剂量依赖效应关系,IC50分别为69.59mg/L和2.73mg/L。以浓度的对数与抑制率进行且直线相关分析:本发明中药组合物: P <0.01;顺铂:p<0.01,具有统计学意义。
2.4 本发明所述的中药组合物与顺铂联合应用对SMMC-7721细胞的增殖抑制作用评价。
采用两药相互作用指数评价两药相互作用性质,其计算公式=AB/(A×B)。根据活细胞数(吸光值)进行计算,AB 为两药联合组与对照组吸光值的比值;A或B是各药物单独使用组与对照组吸光值的比值。当相互作用指数<1时两药作用性质为协同;当相互作用指数<0.7时,协同作用非常显著;相互作用指数=1,则两药作用性质为相加;相互作用指数 >1,则两药作用性质为拮抗。

根据MTT结果显示:不同浓度的本发明组合物提取物与顺铂联用时显示不同程度的协同作用。抑制率均较单用其中一种药物时提高,单独用药组和联合用药组之间差异具有显著性。采用两药相互作用指数分析,当本发明中药组合物28.5mg/L、57mg/L、115mg/L、与顺铂系列浓度联合用药具有协同或显著协同作用(相互作用指数<1)。提示在合适的剂量下本发明提取物与顺铂联合应用可起到降低用药剂量,提高疗效的作用。
以上所述实例仅是本专利的优选实施方式,但本专利的保护范围并不局限于此。应当指出,对于本技术领域的普通技术人员来说,在不脱离本专利原理的前提下,根据本专利的技术方案及其专利构思,还可以做出若干改进和润饰,这些改进和润饰也应视为本专利的保护范围之内。
Claims (9)
1.一种辅助治疗肝癌的中药组合物,其特征在于由以下重量份的原料制成:清半夏5-15份、陈皮5-15份、茵陈5-15份、厚朴10-20份、荆芥10-20份、防风10-20份、地肤子10-20份、生薏米10-20份、白鲜皮5-15份、莪术5-15份、红景天7-17份、炒山药5-15份、灵芝5-15份、火麻仁5-15份、木香4-8份、黄芩5-15份、酒大黄1-5份、枳实5-15份、细辛5-15份、炒栀子5-15份、淡竹叶4-8份、内金5-15份、炒山楂5-15份、炒麦芽5-15份、炒神曲5-15份。
2.如权利要求1所述的一种辅助治疗肝癌的中药组合物,其特征在于由以下重量份的原料制成:清半夏10份、陈皮10份、茵陈10份、厚朴15份、荆芥15份、防风15份、地肤子15份、生薏米15份、白鲜皮10份、莪术10份、红景天12份、炒山药10份、灵芝10份、火麻仁10份、木香6份、黄芩10份、酒大黄3份、枳实10份、细辛10份、炒栀子10份、淡竹叶6份、内金10份、炒山楂10份、炒麦芽10份、炒神曲10份。
3.如权利要求1或2所述的一种辅助治疗肝癌的中药组合物的制备方法,其特征在于,包含以下步骤:
1)按比例称取各原料,然后将清半夏、陈皮、茵陈、厚朴、荆芥、防风、地肤子、生薏米、白鲜皮、莪术、红景天、炒山药、灵芝、火麻仁、木香、黄芩、酒大黄、枳实、细辛、炒栀子、淡竹叶、内金、炒山楂、炒麦芽、炒神曲,分别粉碎过60目筛,加8-10倍量乙醇回流提取二次,每次2-4小时,合并提取液,滤过,减压回收乙醇并浓缩至稠膏,真空干燥得醇提物;
2)取步骤1)醇提后药渣,加10倍量水煎煮二次,每次2小时,合并煎液,滤液减压浓缩至相对密度为1.13-1.20,真空干燥即得水提物;
3)将步骤1)得到的醇提物和步骤2)得到的水提物混合均匀,共同粉碎即得。
4.如权利要求3所述辅助治疗肝癌的中药组合物的制备方法,其特征在于:将步骤3)中制备得到的药物过120目筛,加入药学上可接受的载体,制成常规制剂。
5.如权利要求4所述辅助治疗肝癌的中药组合物的制备方法,其特征在于:所述药学上可接受的载体为填充剂、粘合剂、湿润剂、崩解剂、润滑剂中的任意一种或几种的混合物。
6.一种辅助治疗肝癌的药物制剂,其特征在于:由权利要求1或2所述的辅助治疗肝癌的中药组合物制成。
7.如权利要求6所述的辅助治疗肝癌的药物制剂,其特征在于:所述药物制剂的剂型为胶囊、片剂、口服液、颗粒或滴丸。
8.如权利要求7所述的辅助治疗肝癌的药物制剂,其特征在于:所述药物制剂剂型中的胶囊为软胶囊,片剂为分散片、咀嚼片或口崩片。
9.根据权利要求3或4所述方法制备得到的辅助治疗肝癌的中药组合物的应用,其特征在于:该组合物在制备辅助治疗肝癌的药物中的应用。
