技术领域

背景技术
慢性炎性疾病的治疗问题是现代医学最实际且具有社会意义的问题之一。目前,慢性炎性疾病的病原学治疗中的主要药物是皮质类固醇-泼尼松龙、地塞米松、氢化可的松、倍他米松等[Am J Respir Crit Care Med.2017Aug 15;196(4):414-424]。糖皮质类固醇(GCS)是用于治疗支气管哮喘、慢性肾小球肾炎、间质性肾炎、类风湿性关节炎的最有效药物;它们在较小程度上用于治疗慢性阻塞性支气管炎、自身免疫性胰腺炎和溃疡性坏死性结肠炎。它们的治疗作用是由于强大的抗炎作用引起,所述抗炎作用与对炎性细胞和由这些细胞产生的介质的抑制作用相关,并且包括抑制细胞因子(白介素)和促炎介质的产生以及它们与靶细胞的相互作用[Mediators Inflamm.1998;7(4):229-37]。
重要的是要注意,在长期治疗过程中,在很大一部分患者中对皮质类固醇药物的敏感性降低,即发生了对类固醇的抗性。在不存在明显治疗性作用的情况下表现出对类固醇治疗的低敏感性,并且这需要提高皮质类固醇剂量。然而,在类固醇抗性患者中,类固醇剂量的提高提供了仅短时间的抗炎和治疗性作用提高。除了在长期皮质类固醇治疗过程中出现抗性之外,在临床实践中该疾病还以类固醇抗性形式出现,这使对用于病原学治疗的药物的选择极大地复杂化并且是治疗这些患者的主要问题[CurrAllergy AsthmaRep.2002Mar;2(2):144-50]。

具体实施方式
化合物1(本发明的主题)和许多其他化学化合物的制备在WO2014/168522中进行了描述。本专利申请描述了具有抗病毒作用的戊二酰亚胺衍生物、其用于治疗鼻窦炎和另一些上呼吸道疾病的用途。
WO 2015/072893描述了化合物1用于治疗与嗜酸性粒细胞炎症(包括嗜酸性粒细胞哮喘)的发生相关的疾病的用途。然而,嗜酸性粒细胞炎症的发生是类固醇敏感型哮喘的主要特征,而在接受用高剂量的全身皮质类固醇进行治疗的治疗抗性患者中的支气管肺泡灌洗(BAL)显示出大量中性粒细胞,即,类固醇抗性患者主要患有中性粒细胞炎症[TuratoG.,Baraldo S.,Zuin R.The laws of attraction:chemokines,neutrophils andeosinophils in severe exacerbations of asthma.Thorax.2007;62(6):465-466]。
在化合物1(本发明的主题)活性的临床研究中,出乎意料地发现了化合物1有效提高对标准皮质类固醇治疗的响应者数目并且还抑制异常干扰素γ信号传导的治疗性用途。克服皮质类固醇抗性不能通过化合物1发挥抗病毒作用或抑制嗜酸性粒细胞炎症的能力来预测或说明。
因此,化合物1具有先前未知的与对异常干扰素γ信号传导作用相关的药理学活性并且提高了患者对皮质类固醇治疗的响应,这表明了化合物1用于治疗与异常干扰素γ信号传导相关的疾病(例如干燥综合征、皮肌炎、系统性红斑狼疮或系统性硬化病)、用于治疗患有咳嗽的患者以及用于在类固醇抗性患者中治疗病症(例如哮喘、类风湿性关节炎、系统性红斑狼疮和胃肠疾病)的潜在适用性。
术语和定义
术语“皮质类固醇”包含类固醇激素和/或其合成类似物的亚类。
术语“化合物1”是指化合物1-(2-(1H-咪唑-4-基)乙基)哌啶-2,6-二酮,其也由以下结构式表示:
术语“类固醇抗性”意指其中类固醇治疗无效的疾病状态,所述类固醇治疗通常在患有所述疾病的患者的治疗中是有效的。类固醇抗性患者包括但不限于根据响应常规参数对用全身或经口皮质类固醇进行的治疗无响应或响应不良或者响应不足的患者。

术语“治疗”和“疗法”涵盖在哺乳动物中,优选在人中对病理学病症的治疗,并且包括:a)降低、b)阻断(抑制)疾病的过程,c)减轻疾病严重程度(即诱导疾病消退),d)逆转该术语适用的疾病或病症或者所述疾病或病症的一个或更多个症状。
术语“预防”涵盖危险因素的消除以及在哺乳动物中,优选在人中对疾病的亚临床阶段的预防性治疗,旨在降低疾病的临床阶段发生的可能性。用于预防性治疗的患者的选择是基于已知与一般群体相比与疾病进展至临床阶段的风险提高相关的因素。预防性治疗包括a)一级预防和b)二级预防。一级预防定义为对患有尚未达到临床阶段的疾病的患者进行的预防性治疗。二级预防是对疾病的相同或相似临床病症的复发的预防。用于化合物的治疗性用途的方法
本发明的主题还包括向需要适当治疗的对象施用治疗有效量的根据本发明的化合物。治疗有效量意指在施用于或递送至患者时最有可能提供患者对治疗(预防)的期望响应的化合物的量。根据患者的年龄、体重和一般状况,疾病严重程度,药物施用的方法,与其他药物组合使用等,所需的精确量可因对象而异。
根据本发明的化合物或包含所述化合物的药物组合物可以以有效用于治疗或预防疾病的任意量(优选地,活性物质的日剂量多至0.2g/患者/天,最优选地日剂量为10至200mg/天,优选100mg)并通过有效用于治疗或预防疾病的任何施用途径(优选地通过经口施用途径)施用于患者。
将药物与特定的合适可药用载体以期望剂量混合之后,可将根据本发明的组合物经口、肠胃外、表面等施用于人或其他动物。
施用可一天、一周(或任意其他时间间隔)进行一次和数次二者或者不时进行。另外,化合物可每天施用于患者持续指定的天数(例如,2至10天),随后是不施用的时间(例如,1至30天)。
当根据本发明的化合物用作组合治疗的一部分时,在期望的治疗期内施用每个组合治疗组分的剂量。组合治疗的化合物可以以包含所有组分的剂型和以单独的组分剂型二者同时施用于患者身体。
化合物1在组合治疗中的用途
尽管根据本发明的化合物1可作为单独的活性药剂施用,但其也可与一种或更多种其他药剂组合使用;特别地,其他药剂可以是糖皮质类固醇、白三烯受体拮抗剂、支气管扩张剂、单克隆抗体等。当以组合施用时,治疗剂可在不同时间以不同的剂型同时或依次施用,或者治疗剂可以以单一剂型组合。
当涉及与其他药剂组合使用的本发明化合物时,短语“组合治疗”意指同时或依次施用所有药剂,以使得将以任何方式提供该药物组合的有益作用。共施用特别是意味着例如以具有固定比例活性物质的一种片剂、胶囊剂、注射剂或其他形式共递送,以及对于每种化合物分别以数个分开的剂型同时递送。
因此,本发明的化合物1可与预防和治疗相应疾病的技术人员已知的另外的治疗组合施用,包括使用抗菌药、细胞抑制药和细胞毒性药、用于抑制药物之一的症状或副作用的医用制剂。
如果剂型是固定剂量,则这样的组合在可接受的剂量范围内包含根据本发明的化合物。如果不能将这些药物组合,则本发明的化合物1也可与其他药剂依次施用于患者。本发明不限于特定的施用顺序;本发明的化合物可与另一种药物的施用同时或者在另一种药物的施用之前或之后的任何时间施用于患者。
实施例
根据本发明的化合物的制备
化合物1(本发明的主题)和许多其他化学化合物的制备在WO2014/168522中进行了描述。本专利申请描述了具有抗病毒作用的戊二酰亚胺衍生物、其用于治疗鼻窦炎和另一些上呼吸道疾病的用途。
根据本发明的化合物的生物学活性的表征
化合物1(本发明的主题)的生物学活性已在广泛的临床前试验中以及在支气管哮喘患者的多中心、双盲、随机化II期临床试验(治疗期为12周)中进行了研究。已显示化合物1的治疗性用途有效地提高了针对用吸入型皮质类固醇进行的标准治疗的响应者数目。对吸入型皮质类固醇抗性的克服不能通过化合物1发挥抗病毒作用或抑制嗜酸性粒细胞炎症的能力来预测或说明。
实施例1.化合物1在临床试验中的活性的研究
在有关评价不同剂量的化合物1相对于安慰剂在支气管哮喘患者的12周治疗中之有效性和安全性的多中心、双盲、随机化、平行组II期临床试验研究(PULM-XC8-02,NCT03450434)中,出乎意料地发现化合物1的治疗性用途有效地提高了针对用吸入型皮质类固醇进行的标准治疗的响应者数目。因此,化合物1潜在地可用于治疗与类固醇抗性的发生相关的疾病,特别是用于治疗类固醇抗性哮喘。
在临床试验中,将符合条件的患者1∶1∶1∶1随机化为四组之一:
-以2mg/天剂量的化合物1;
-以10mg/天剂量的化合物1;
-以100mg/天剂量的化合物1;以及
-安慰剂。
在研究治疗阶段期间,患者在用低剂量吸入型皮质类固醇进行的基线类固醇治疗的背景下接受化合物1或安慰剂持续12周。化合物1或安慰剂在早餐之前30分钟一天一次经口施用。
在100mg/天的化合物1剂量下获得了临床上显著作用。
对支气管哮喘患者的临床试验结果的探索性分析表明,化合物1有效地提高了针对标准治疗的响应者数目。例如,使用基线类固醇治疗和安慰剂导致29名患者中仅13名的FEV1提高了100ml或更多,而使用基线类固醇治疗和化合物1导致29名患者中20名的响应,因此使得对基线类固醇治疗具有响应的患者数目显著提高(图1)。因此,使用化合物1的作用之一是克服对皮质类固醇的抗性,这是纳入临床试验中的患者基线治疗的基础。
为了研究哮喘患者中IFN-γ基线水平的作用,另外分析了在临床研究中获得的结果;根据试验纳入时患者的血液中IFN-γ基线水平研究了患者对治疗的响应(通过Bio-Plex Pro Human Chemokine Panel Assay(Bio-Rad)确定)。
在基线类固醇治疗和安慰剂组中,患者对基线治疗的响应随IFN-γ基线水平的提高而降低(图2)。在IFN-γ水平>100pg/ml的患者中使用基线类固醇治疗不导致呼吸功能的积极变化,如通过FEV1变化确定的(观察到降低0.1L或更多,图2),即这些患者表现出对类固醇治疗的抗性(缺乏对治疗的正响应)。在基线类固醇治疗的背景下,以100mg/天的剂量施用的化合物1的使用在患者中(特别是在IFN-γ水平大于100pg/ml的患者中)提供了对治疗的显著更大且临床上显著的响应(图3)。该事实表明了克服类固醇抗性,其尤其可由异常IFN-γ信号传导引起。
此外,在临床试验中,还分析了化合物1对IFN-γ信号转导的作用。其表明在基线类固醇治疗的背景下施用的化合物1在IFN-γ基线水平>100pg/ml的患者中抑制了干扰素-γ依赖性细胞因子CXCL10(干扰素-γ诱导的蛋白质IP10)的浓度,而在基线类固醇治疗的背景下接受安慰剂的患者组的CXCL10水平略微提高(图4)。另外,与使用仅基线治疗和安慰剂相比,在基线治疗的背景下化合物1的治疗性用途还导致患者血浆中IFN-γ浓度的负动态,而与纳入时IFN-γ水平无关(图5)。因此,通过使用化合物1诱导的对类固醇抗性的抑制可潜在地与对IFN-γ的异常活性(信号传导)的作用相关。
在该研究中,使用雌性balb/c小鼠(6至8周龄)。将化合物I胃内施用3次:在直肠施用唑酮之后1小时、25小时和49小时。在施用唑酮之前以及施用唑酮之后24、48和72小时测量动物的体重。在施用唑酮之后72小时,根据以下评分量表在显微镜下评价肠壁损伤:
0=无损伤,
1=充血,无溃疡,
2=充血,肠壁增厚,无溃疡,
3=1处溃疡,肠壁未增厚,
4=2处或更多处溃疡或炎症部位,
5=2处或更多处严重的溃疡和炎症部位,或者沿结肠的长度延伸>1cm的1处溃疡/炎症部位,以及
6至10=损伤覆盖结肠长度的>2cm长,每另外包含1cm,评分就提高1。
所有数据均通过描述性统计:算术平均值(M)和算术平均值的标准误差(m)进行分析。使用Shapiro-Wilk检验来检查所获得实验数据分布的正态性。用单因素ANOVA(具有Dunnett事后分析)对正态分布进行分析以评估组间差异。用单因素ANOVA(具有Tukey事后分析)对非正态分布进行分析以比较数个组。差异在5%的置信水平下确定。研究结果在表1和2中示出。

研究结果表明,当胃内施用时,化合物I在动物中降低了结肠壁的损伤程度并防止体重减轻。因此,化合物I在溃疡性结肠炎鼠模型中具有治疗性作用。化合物1的作用强度不亚于泼尼松龙。
实施例3.化合物1在由吸入柠檬酸和IFN-γ诱导的咳嗽豚鼠模型中活性的研究。
根据方法[Am J Respir Crit Care Med.2018.V.198(7).P.868-879]研究化合物I在由吸入柠檬酸和IFN-γ诱导的咳嗽豚鼠模型中的活性。
在该研究中,使用Aguti系豚鼠。使所有实验动物吸入在生理盐水中制备的柠檬酸溶液(0.3M)8分钟。使病理对照组和接受治疗组在吸入柠檬酸盐之前7小时时吸入IFN-γ(10μg/kg)3分钟。在吸入IFN-γ之后,即在吸入柠檬酸溶液之前7小时,即刻胃内施用化合物1一次。通过对从吸入柠檬酸开始的8分钟内的咳嗽发作次数进行计数来评价镇咳活性。所有数据均通过描述性统计:算术平均值(M)和算术平均值的标准误差(m)进行分析。使用Shapiro-Wilk检验来检查所获得实验数据分布的正态性。用单因素ANOVA(具有Dunnett事后分析)对正态分布进行分析以评估组间差异。用单因素ANOVA(具有Tukey事后分析)对非正态分布进行分析以比较数个组。差异在5%的置信水平下确定。研究结果在表1和2中示出。
