The clinical effect observation of the traditional Chinese medicine enema formula prepared by the invention is as follows:
1. general data
(1) Inclusion criteria were: the Western medicine diagnosis refers to the consensus opinion on the diagnosis and treatment standard of inflammatory bowel disease in China (2012) formulated by the Cooperation group of the digestive disease society of the Chinese medical society, and routine examination of liver function, kidney function, blood, urine and feces has no obvious abnormality in the last 1 year. (2) Exclusion criteria: patients with severe ulcerative colitis, intestinal occupancy and other serious complications; ② pregnant or lactating women; severe liver and kidney insufficiency, malignant tumor and incapability of tolerating general anesthesia as evaluated by an anesthesiologist; patients with cardiovascular and cerebrovascular diseases; patients with other immune and organ failure diseases are combined; sixthly, the patients with psychosis are combined; seventhly, other serious infectious diseases such as pulmonary tuberculosis and HIV positive patients are combined; and ninthly, people allergic to various medicines participate in other clinical testers within 3 months.
(3) Case selection: the selection meets the inclusion standard and belongs to the diagnosis standard of Western medicine chronic ulcerative colitis, the disease stage is active stage, the disease degree is mild and moderate, 66 patients with spleen deficiency and dampness-heat syndrome are identified in Chinese medicine, and 33 patients are randomly divided into an observation group and a control group. In the observation group 33, 1 male was dropped, and the remaining 32 male were 18 female; age (37.68 + -10.52). In the control group 33, 1 female was dropped, and the remaining 32 cases were 19 male and 12 female; age (38.26 ± 9.75) years.
2. Method of treatment
(1) Control group: conventional treatment in western medicine: mesalazine enteric coated tablet (0.25 g per tablet, manufactured by sunflower pharmaceutical group Jia Mus Luling pharmaceutical Co., Ltd., national Standard H19980148), 1g per time, 4 times daily, is orally administered.
(2) Observation group: on the basis of the treatment of a control group, a traditional Chinese medicine retention enema formula is added, 200mL of the enema is used every time, 2 times are taken every day, 1 time is changed every 4 weeks after the treatment, the enema is retained, and the rest is carried out for 1 day after 14 days of continuous enema. The sausage filling method comprises the following steps: the temperature of the enema liquid is preferably (39 +/-0.5) DEG C, the left lateral decubitus position is firstly taken, the buttocks are fully exposed, a treatment towel is paved, the buttocks are lifted by about 10cm, the front end of an anal tube is lubricated, the anal tube is inserted into the rectum by 15cm, and the administration speed of the enema is adjusted according to the tolerance condition of a patient, and is usually 80 drops/min. And (3) posture adjustment: if the rectum, sigmoid colon and left half colon are ulcerated, the left lateral decubitus is taken, and the retention time of the enema is prolonged as much as possible; if the ulcer surface is extensive or affects the whole colon, the left lateral decubitus, the horizontal decubitus and the right lateral decubitus are sequentially taken for 20min, and then the patient is rested in a comfortable position.
(3) The course of treatment is as follows: the treatment period is 1 treatment course for 8 weeks in both groups.
3. Standard of therapeutic effect
(1) Comprehensive therapeutic effect evaluation standard
Clinical cure: the symptom sign disappears, and the abdominal pain, diarrhea, purulent bloody stool completely disappears; the absolute score for the endoscopic sub-score was 0 or 1 for the Mayo score.
The effect is shown: the symptoms are obviously relieved, and the abdominal pain, diarrhea, purulent bloody stool are obviously relieved; the total Mayo score is less than or equal to 2 points and no single subentry score is more than 1 point.
Improvement: the symptoms are relieved, and the bellyache, the diarrhea and the purulent bloody stool relatively disappear; the Mayo score decreased endoscopic sub-score by at least 1 point relative to baseline.
And (4) invalidation: if the above criteria are not met, the disease condition is not improved or worsened.
(2) Evaluation criteria of syndrome therapeutic effect
Refer to the clinical research principles of New Chinese medicine published by the publishing agency of Chinese medical science and technology in 5 months in 2002
Clinical cure: the symptoms and physical signs disappear or basically disappear, and the syndrome score is reduced by more than or equal to 95 percent.
The effect is shown: the symptoms and physical signs are obviously improved, and the syndrome integral is reduced by more than or equal to 70 percent.
The method has the following advantages: the symptoms and physical signs are obviously improved, and the syndrome integral is reduced by more than or equal to 30 percent.
And (4) invalidation: the symptoms and physical signs are not obviously improved or even aggravated, and the syndrome integral is reduced by less than 30 percent.
The calculation formula (nimodipine method) is: [ (before treatment integration-after treatment integration) ÷ before treatment integration ] × 100%
(3) The evaluation of the curative effect of the enteroscopy inflammation refers to the consensus opinion of traditional Chinese medicine diagnosis and treatment experts of ulcerative colitis (2017), and an improved Mayo activity index is adopted.
The clinical effect is as follows: the total Mayo score is reduced by more than or equal to 30 percent or more than or equal to 3 points from the baseline level, and meanwhile, the hematochezia sub-score is reduced by more than or equal to 1 point or the absolute score of the hematochezia sub-score is 0 or 1.
Clinical remission: the total Mayo score is less than or equal to 2 points and no single subentry score is more than 1 point.
Endoscope response: the Mayo score decreased endoscopic sub-score by at least 1 point relative to baseline.
And (3) mucosa healing: the absolute score for the endoscopic sub-score was 0 or 1 for the Mayo score.
4. Statistical method
Data were processed using SPSS19.0 statistical analysis software. The measured data are expressed as (x + -s), and the comparison between groups adopts t test (or corrects t test); the count data are expressed as percentages and the comparisons between groups are by x 2 Checking; the grade data is tested by rank sum H, and the difference with P less than 0.05 has significance statistics.
5. Results
(1) The two groups of the composition have a combined therapeutic effect, which is shown in Table 1

In 32 cases of an observation group, 11 cases are clinically cured, 14 cases are obviously effective, 5 cases are effective, 2 cases are ineffective, and the total effective rate is 93.75%; in 32 cases of the control group, 8 cases are clinically cured, 12 cases are obviously effective, 7 cases are effective, 5 cases are ineffective, and the total effective rate is 84.38%. Through statistical treatment, the curative effect of the treatment group is obviously better than that of the control group (P is less than 0.05).
(2) The two syndromes are compared in terms of efficacy, as shown in Table 2

In 32 observation groups, 12 cases are clinically cured, 14 cases are obviously effective, 5 cases are effective, 1 case is ineffective, and the total effective rate is 96.88%; in 32 cases of the control group, 9 cases are clinically cured, 12 cases are obviously effective, 7 cases are effective, 4 cases are ineffective, and the total effective rate is 87.50%. Through statistical treatment, the curative effect of the treatment group is obviously better than that of the control group (P is less than 0.05).
