技术领域

发明背景
在多种情况下,希望对神经施加电刺激以产生复合动作电位(CAP)。神经调节系统将电脉冲施加到组织上以便产生治疗作用。这样的系统通常包括植入的电脉冲发生器、以及可以通过经皮感应传输可再充电的电源,比如电池。电极阵列连接到脉冲发生器,并定位于靶神经通路附近。电极施加到神经通路的电脉冲引起神经元去极化并产生传播的动作电位。
几乎所有的神经调节应用都应用于包含多于一种类型纤维的神经(在本文中称为“混合神经”)。通常情况是,当受刺激时,混合神经的很大比例的纤维不会产生受试者或外部观察者(比如外科医生或临床医生)可直接、立即感知到的作用。例如,受试者通常不能感觉到自主神经系统的纤维的刺激。
面对所有类型的神经调节系统的控制问题是达到治疗作用所需的足够水平的神经募集,但能量消耗最小。所采用的刺激范例的功耗直接影响电池需求,进而影响装置的物理大小和寿命。对于可再充电系统,功耗增大会导致更频繁的充电,并且鉴于电池仅允许有限数量的充电循环,最终这会缩短装置的植入寿命。
混合神经的神经调节的一个示例是骶神经刺激(SNS),其中刺激频率通常较低(<20Hz),并且电荷可能非常高(例如,每个刺激可能包含最高达7mA或更高的电流,脉冲宽度为210us)。已显示SNS对大便失禁(FI)、尿潴留(UR)、急迫性尿失禁(UUI,也称为膀胱过度活动症(OAB))、顽固性便秘和慢性盆腔痛以及其他类似适应症具有治疗作用。

发明内容
根据第一方面,本发明提供了一种包括多种神经纤维类型的混合神经的神经刺激方法,所述方法包括:
将植入式电极阵列定位在包括多种神经纤维类型的混合神经附近,所述植入式电极阵列包括多个电极;
从所述植入式电极阵列中的至少一个标称刺激电极传递电刺激,所述电刺激是根据一组刺激参数被传递;
从所述植入式电极阵列中的至少一个标称记录电极获得由所述电刺激诱发的电生理反应的记录;
通过评估所述电生理反应的记录的一个或多个选定特征并根据所观察到的选定特征识别所述电刺激募集的一种或多种纤维类型的募集水平,从而分析所述记录;以及
以实现所述混合神经的一种或多种纤维类型相对于其他纤维类型的选择性募集的方式完善所述刺激参数。
根据第二方面,本发明提供了一种非暂时性计算机可读介质,用于混合神经的神经刺激,所述混合神经包括多种神经纤维类型,所述非暂时性计算机可读介质包括指令,当由一个或多个处理器执行时,所述指令将引起执行以下操作:

植入式电极阵列,所述植入式电极阵列被配置为被植入到包括多种神经纤维类型的混合神经附近,所述植入式电极阵列包括多个电极;以及

本发明的一些实施例较过去的方法可能是附加地或可替代地有利的,过去的方法比如通过术中观察肌肉激活来靶向特定肌肉或肌肉群(比如肛门括约肌)的激活,用于评估刺激电极与混合神经的接近度。通过单独寻求肌肉激活或其替代物作为神经刺激的目标,只要观察肌肉激活,这些过去的方法无视哪些纤维类型被刺激募集。与这样的过去方法相比之下,本发明的基于观察到的电生理反应测量值来靶向一种或多种特定纤维类型的方法避免了受刺激的神经反应从刺激部位向肌肉传播的混淆效应,因此传播通常会经过一系列神经分支、突触、终点,并进一步涉及运动神经元对肌肉纤维的激活,所有这些都会导致肌肉观察结果至少部分无视在刺激部位发生的神经募集的性质。此外,在一些实施例中,本发明基于观察到的电生理反应测量值靶向特定纤维类型的方法进一步打开了以下可能性:在与肌肉激活无关并且因此完全不能被肌肉观察检测到的纤维募集的基础上传递治疗。例如,患者或外部临床观察都可能无法感知自主神经系统纤维的神经调节。因此,本发明的这样的实施例认识到,为了优化混合神经的神经调节,需要使用募集的纤维的客观量度。
在本发明的一些实施例中,被完善以实现一种或多种纤维类型的选择性募集的刺激参数可以包括以下中的任一个或多个:术中电极放置;术中电极阵列类型选择,包括引线、桨状或袖套阵列选择;刺激频率;刺激幅度;刺激波形;刺激脉冲宽度;刺激电极的选择,包括通过电流引导在电极之间插入刺激部位;刺激形状(双相、三相等);刺激极性(单极、双极、三极等)、刺激电极尺寸、刺激电极形状等。
在本发明的一些实施例中,所述刺激参数以实现一种或多种希望纤维类型的选择性募集同时进一步实现至少一种非选定纤维类型的选择性非募集或减少募集的方式被完善。
所述记录的、根据其识别电刺激募集的至少第一纤维类型的募集水平的一个或多个选定特征可以包括以下中的任一个或多个:一个或多个ECAP拐点;一个或多个ECAP峰位置;一个或多个ECAP峰幅度;ECAP传播速度;在记录部位观察到的电生理反应的传播或非传播;ECAP持续时间;不应期;强度-持续时间曲线特征,包括时值或基强度;生长曲线特征,包括阈值和斜率;随着刺激电流的增大,ECAP峰的数量;迟发反应的存在、幅度和/或潜伏期、以及刺激频率变化情况下的电生理反应的幅度和形状。
例如,在选定特征是传导速度的实施例中,可以根据有髓鞘纤维的直径与传导速度之间的线性关系的先验知识确定募集的纤维类型。
在选定特征是传导速度的实施例中,可以通过确定从刺激部位到距刺激部位已知距离的单个测量电极的传播时间来测量传导速度。更优选地,可以通过观察第一测量电极和第二测量电极处的神经反应并确定第一测量电极与第二测量电极之间的传播时间来测量传导速度。确定两个或多个测量电极的传导速度允许检查ECAP波形的特定要素,比如ECAP的峰到达时间或零交叉到达时间,从而提高传导速度确定的准确性,继而提高对募集的纤维类型的识别的准确性。
在本发明的一些实施例中,在对不同纤维分布下的神经反应的形态有先验知识的情况下,可以使用模型来解决以下反向问题:从一个或多个获得的对通过电刺激刺混合神经激诱发的电生理反应的记录中检索纤维分布。例如,本申请人的关于纤维分布建模的国际专利公开号WO 2016161484(PCT/AU 2016/050263)的教导可以应用于此目的。
可以通过评估从两个或更多个间隔开的测量电极获得的记录来确定选定特征,所述记录是单个电生理响应事件。获得相同电生理反应的空间上不同的记录例如允许确定:相对于记录电极附近,记录的选定特征是混合神经上的传播神经反应还是电极的远场中的比如肌电活动等非传播反应,由此有助于确定纤维类型。虽然应注意,贴近激活的肌肉获得的记录将观察到传播性CMAP,但本发明的当前实施例通常将利用从受影响的肌肉向远侧植入的电极阵列,使得在这样的实施例中将观察到肌电活动,作为电生理反应的记录的非传播分量。
在选定特征是记录的非传播特征的实施例中,可以认为这种非传播分量在记录中的存在,比如刺激后4-10毫秒出现的分量,是由于通过例如刺激运动神经元进行的刺激产生的肌电活动引起的。因此,可以推断出肌肉激活是由Aα传出纤维的激活引起的,这些传出纤维具有高传导速度,无法在记录中直接观察到,因为在刺激完成之前或在刺激伪影充分解决以允许直接观察Aα纤维反应之前,Aα反应可能已在记录电极处结束。然而,可以参考记录中在刺激(至少对于SNS)后4-10毫秒内出现的非传播分量来观察选择性Aα募集。因此,可以将这种非传播分量的幅度视为对募集的Aα纤维数量的度量,从而允许Aα纤维的选择性募集。
本发明的进一步实施例可以提供一种或多种募集的纤维类型的激活模式的区分。例如,不受理论的限制,应注意,Aα纤维的激活可能是电刺激进行的直接激活的结果,也可能是通过反射弧间接激活的结果,比如当刺激Ia本体感受纤维时引出的H反射。鉴于Ia纤维与Aα纤维具有基本相同的传导速度,因此激活模式可能很难确定,因此无法通过这种措施加以区分。然而,进一步应注意的是,随着刺激频率的增加,由H反射引起的Aα纤维激活将在相对低的频率(比如约30Hz的刺激速率)下下降,而直接Aα纤维激活直到达到较高的刺激频率才下降。因此,本发明的一些实施例可以通过应用变化后的刺激速率、识别高于其时激活下降的阈值频率、并且根据阈值频率确定激活模式来识别一种或多种纤维类型的激活模式。
在一些实施例中,选定特征可以包括在刺激之后不到1毫秒的记录中出现和/或具有在80-120m/s范围内的传导速度的传播反应,被视为指示Aα纤维的激活。
在一些实施例中,选定特征可以包括在刺激之后不到6毫秒在记录中出现和/或具有在3-15m/s范围内的传导速度的传播反应,被视为指示B纤维的激活。
在一些实施例中,选定特征可以包括在刺激之后不到6毫秒在记录中出现和/或具有在0.5-2m/s范围内的传导速度和/或具有超过10毫秒持续时间的传播反应,被视为指示C纤维的激活。
在一些实施例中,选定特征可以包括在刺激之后不到3毫秒在记录中出现和/或具有在30-80m/s范围内的传导速度的传播反应,被视为指示Aβ纤维的激活。
在一些实施例中,基于由任何合适的纤维分类系统定义的这种纤维类型的已知特征,选定特征可以对应于任何关注的纤维类型。
在一些实施例中,为了确定由电刺激募集的一种或多种纤维类型中的每一种纤维类型的募集水平,可以评估电生理反应的记录的多于一个选定特征。例如,可以通过参考记录中的迟发反应的幅度以及还参考迟发反应是否在多个记录电极之间不传播来确定运动纤维的募集水平。
在一些实施例中,例如,为了更有效地治疗单一病症和/或同时治疗两种或更多种并存或共病病症的目的,可以靶向两种或更多种纤维类型,例如其中每种纤维类型关于相应病症具有治疗性。例如,可以靶向交感神经纤维类型和副交感神经纤维类型之一,以便在第一时间以激起选定器官或身体系统的活动,而可以靶向交感神经纤维类型和副交感神经纤维类型中的另一个,以便在第二时间抑制选定器官或身体系统的活动。
可以在电生理响应的记录中监测多个选定特征,或者在一些实施例中,监测所有上述选定特征。在一些实施例中,可以应用机器学习分类器以便按照存在的纤维类型对观察到的电生理反应进行分类。
在本发明的一些实施例中,电极阵列包括单根植入式引线。在本发明的一些实施例中,电极阵列包括连接到单个植入式脉冲发生器(IPG)的多根电极引线。
在本发明的一些实施例中,所述至少一个标称记录电极和所述至少一个标称刺激电极被定位成与所述混合神经的单个分支相邻,所述单个分支是所述混合神经的一段,在所述段中,在所述标称记录电极与所述标称刺激电极之间的神经之间没有神经分支或神经合并发生。

在一些实施例中,混合神经可以包括骶神经。在这样的实施例中,优先募集的第一纤维类型可以包括副交感神经纤维或其他纤维类型。可以向选定纤维类型提供骶神经刺激,以提供用于粪便失禁(FI),尿潴留(UR)、急迫性尿失禁(UUI,又称为膀胱过度活动症(OAB))、顽固性便秘和慢性盆腔痛中的一种或多种的治疗。

在本发明的一些实施例中,完善刺激参数包括通过临床试错等对刺激参数进行临床拟合的过程。在一些实施例中,完善刺激参数可以包括术中电极重新定位。
在其他实施例中,完善刺激参数可以包括由植入的装置的处理器管理的自动反馈过程。
一些实施例可以通过从第一电极施加超最大刺激来募集神经的所有纤维来在空间上靶向所述选定纤维类型,并且通过使用用于在选定圆周位置处记录的选定电极段来观察在所述选定圆周位置处的所募集的反应,分析所记录的反应以确定与那个位置相邻的一种或多种纤维类型,以及随后在希望募集如此识别的所述一种或多种纤维类型时从所述选定电极段施加刺激。其他实施例可以通过在选定圆周位置使用选定电极段施加刚好高于刺激阈值的刺激来在空间上靶向所述选定纤维类型,以募集在那个段附近的纤维,观察在第二电极处的募集的反应,以及分析所述记录以确定来自所述选定电极段的刺激正在募集的纤维类型。这样的实施例可以以这种方式调查多个电极段以确定与所有这样的电极段相邻的纤维类型。其他实施例可以通过使用除了以圆周方式以外在空间上不同的电极来应用等效空间靶向,比如以网格图案或允许不同电极募集不同纤维子组的任何其他形式的空间电极变化。

应当了解,在记录中电生理反应的发生时间通常在本文中通过参考刺激之后的时间量来指代。然而,此时间量取决于正观察的纤维的传导速度以及相应的记录电极与刺激部位的距离两者。应当理解,本文中提出的时间段可以特别地适用于具有与刺激电极间隔约6mm、12mm和18mm的记录电极的单根植入引线。然而,替代的电极阵列的几何形状和配置可以提供距刺激部位其他距离的电极,并且基于传导速度的简单计算允许确定每种给定纤维类型的反应的替代预期到达时间,并且这种替代方案在本发明的范围内。类似地,当由肌肉的远场激活引起的迟发反应是关注的选定特征时,迟发反应的发生时间将取决于从刺激部位到肌肉的距离,并且这种迟发反应的发生时间的变化因此还可以仅根据刺激部位和相关的解剖学来简单地确定,并且这种迟发反应的发生时间的变化也在本发明的范围内。
在本发明的第三方面的装置的一些实施例中,控制单元进一步被配置为以实现混合神经的一种或多种纤维类型相对于其他纤维类型的选择性募集的方式完善刺激参数。在本发明的第三方面的装置的一些实施例中,该装置是植入式装置,而在其他实施例中,该装置可以是用于试验或术中使用的外部装置。

具体实施方式
图1示意性地展示了植入的骶神经刺激器100。刺激器100包括被植入在患者下腹部区域或臀后上部区域的合适位置处的电子模块110、以及被植入骶骨内并且通过适合的引线连接到模块110上的电极组件150。植入的神经装置100的操作的许多方面可由外部控制装置192重新配置。而且,植入的神经装置100起到数据收集的作用,其中所收集的数据被传送到外部装置192。
图2是植入的神经刺激器100的框图。模块110包含电池112和遥测模块114。在本发明的实施例中,遥测模块114可以使用任何合适类型的经皮通信190,比如红外(IR)传输、电磁传输、电容传输和电感传输,以在外部装置192与电子模块110之间传输功率和/或数据。
模块控制器116具有存储患者设置120、控制程序122等等的相关联存储器118。控制器116控制脉冲发生器124根据患者设置120和控制程序122产生电流脉冲形式的刺激。电极选择模块126将所产生的脉冲切换到电极阵列150中的适当电极,以将电流脉冲传递到选定电极周围的组织。也可以设置其他电极阵列,并且可以由电极选择模块126类似地寻址,例如,如在下面进一步讨论的图5和图6的情况。因此,可以选择阵列150中的一个或多个电极在给定时间用作标称刺激电极,同时可以选择阵列150中的一个或多个电极在给定的时间用作标称感测电极,即使电极可能在物理上是相同的,并且在其他时间可能起到不同的作用。测量电路128被配置为捕获在电极选择模块126在电极阵列中选择的感测电极处感测到的神经反应的测量值。这样的测量值通常将包括阵列150上的两个感测电极之间的差分测量值。然而,可以附加地或替代地从阵列150中的单个感测电极获得测量值,该单个感测电极在电学上被称为模块110的壳体上的参考电极,或者例如被称为控制器116的系统接地。感测电极在本文中也被称为记录电极。
图3是展示了植入刺激器100与神经180(在这种情况下为骶神经)的相互作用的示意图,然而,替代实施例可以定位于包括周围神经、内脏神经、脊神经或脑结构的任何希望神经组织附近。电极选择模块126选择电极阵列150中的刺激电极2来将电流脉冲传递到包括神经180的周围组织,并且还选择阵列150中的返回电极4用于刺激电流恢复以保持零净电荷转移。
向神经180传递适当的刺激会诱发神经反应,神经反应包括如图所示沿神经180传播的复合动作电位,用于治疗目的,在骶神经刺激器的情况下可能会刺激逼尿肌的希望的肌肉纤维的运动功能。为此,刺激电极用于传递<20Hz的刺激。
装置100进一步被配置用于感测在沿神经180传播的复合动作电位(CAP)的存在和强度,无论这样的CAP是由来自电极2和4的刺激诱发、还是以其他方式诱发。为此,阵列150中的任何电极可以由电极选择模块126选择以用作测量电极6和测量参考电极8。测量电极6和8感测的信号被传到测量电路128,该测量电路例如可以根据本申请人的国际专利申请公开号WO 2012155183的教导操作,该专利申请的内容通过援引并入本文。
图4展示了包括来自具有相似特性的募集纤维的动作电位贡献的电诱发复合动作电位(ECAP)的典型形式。图4中所示的复合动作电位的形状和持续时间是可预测的,因为它是响应于刺激通过轴突整体产生动作电位产生的离子电流的结果。在大量纤维中产生的动作电位加起来形成复合动作电位(CAP)。CAP是来自大量单纤维动作电位的反应之和。记录的CAP是大量不同纤维去极化的结果。动作电位在每根纤维上的传播速度很大程度上由那根纤维的直径决定。从一组相似纤维的放电产生的CAP被测量为正峰值电位P1,然后是负峰N1,接着是第二正峰P2。这是由在动作电位沿着各个纤维传播时经过记录电极的激活区域引起的。从Aβ纤维观察到的电诱发CAP信号通常具有在微伏特范围内的最大幅度以及2-3ms的持续时间。
CAP曲线采用典型形式,并且以任何合适的参数为特征,其中一些参数在图4中指示。取决于记录的极性,正常记录的曲线可以采取与图4所示的相反的形式,即具有两个负峰N1和N2、以及一个正峰P1。
在几乎所有的神经调节应用中,都希望单一类别的纤维反应,但是刺激可能将动作电位募集在其他类别的纤维上,从而引起不利的副作用。而且,记录诱发的神经反应的困难已经产生了使用替代指标的常规解决方案,比如对肌肉收缩的观察,而没有任何关于实际纤维类型募集的知识。
根据本实施例,由给定刺激诱发的CAP的特征可以是例如图4、图8a或图8b的曲线上的拐点的参数。峰的位置和幅度可以单独使用或组合使用,以在峰与CNS疾病的状态和严重程度之间产生关联。其他电生理数据可以用于补充ECAP数据。例如,掩蔽探针研究可以用于确定不应期和相对不应期。不应期的测量允许估计被刺激的纤维的频率反应。特别地,较短的不应期与较高的传导速度相关,因此允许确定募集了哪种(哪些)纤维类型以产生观察到的ECAP,从而可以为设置刺激频率参数提供指导。所有这些神经生理特性都可以用来识别受刺激的神经,并可以用来指导纤维类型靶向和刺激参数选择。
外周的几乎所有主要神经都具有混合性质,这意味着神经包含在一起运行单独各种类型和功能的纤维。周围神经在各个阶段捆在一起,形成脊神经(比如S3神经,其是SNS的主要靶)。在连接脊髓之前,脊神经分裂成前根和后根。简而言之,前根主要包含各种传出纤维,而后根则主要包含各种传入纤维。因此,混合神经可以同时包含传入和传出轴突。混合神经的另一个示例是迷走神经(VN),其包含运动纤维、交感神经纤维、副交感神经纤维和感觉纤维。
混合神经是束的异质集合,并且已经显示束将起相似功能并具有共同的生理特性的神经纤维捆绑在一起。可以从形成每个脊神经后根和前根的小根观察到这种功能分离。图5是人类S2神经的前小根中的纤维的功能性分离的截面示意图。根由2个小根组成。出现了三种不同的神经分布模式:躯体类型(S),主要是大而粗的有髓鞘纤维,无副交感神经纤维;植物类型(V),富含副交感神经纤维;和混合类型(M)。注意植物类型和躯体类型的束的地形聚集。主要是副交感神经纤维的束集中在任一根的右小根上。相反,在左小根中发现纯躯体束。似乎神经纤维不仅仅遵循随机分布,而是某种功能组织。
例如通过仅从神经的一侧施加一定幅度的刺激来刺激混合神经的任何给定子区段,该幅度仅会募集在刺激电极附近的束中的纤维,因此将激活起到不同功能的特定部分纤维。本发明认识到,靶向混合神经的适当纤维对于许多神经调节应用而言非常重要。
然而,从纯解剖学角度来看,这样的靶向实质上是不可能的,因为混合神经中每个束的路线都沿着神经变化。束可以交叉、合并和分裂。来自正中神经的这的一个示例在图6中给出,该图是正中神经束的三维(3D)重建。图6a是在任意位置的正中神经的3D模型,示出了不同功能束的一些变化模式。绿色代表运动神经束,黄色代表感觉神经束,而紫色代表混合(感觉和运动)神经束。图6b是图6a的局部放大图,而图6c呈现了一个横截面图像。
图6证明,仅基于解剖学很难解决纤维类型靶向,因为所希望的纤维类型在神经的不同神经部位处位置明显不同。进一步地,神经内的束的含量和布置存在患者间差异。这些变化发生在比典型的神经刺激电极间距小得多的尺度上,因为典型的植入电极阵列利用长3mm、彼此间隔4mm、即以7mm的间距定位的电极。然而,在沿图6所示的神经的7mm的空间中,任何给定的神经束都可以在束中占据任何或所有位置,从而无法根据周围的解剖学取向选择性地靶向那个束。仅利用较小的电极将无法解决这些不确定性。
然而,本发明认识到可以基于神经纤维的生理特性、比如髓鞘形成状态和直径来对神经纤维进行分类。这些特性产生电生理特性的差异,从而允许通过尤其所产生的动作电位的传导速度、其不应期和强度-持续时间曲线的量度来对受刺激的神经纤维进行分类。例如,有髓鞘纤维的直径与传导速度之间存在线性关系。存在多种神经纤维类型分类系统,然而,不管在整个本文件中使用的命名法或分类,应当理解的是,电生理特性差异的量度允许根据本发明区分纤维类型,不考虑命名法。
图7示出了针对不同电流自记录袖套电极从猪的子宫颈迷走神经记录的神经反应,该记录袖套电极位于距围绕同一神经的刺激袖套电极约25mm远处。
图8a和图8b示出了从2名接受SNS治疗的人类患者的S3骶神经获得的电生理反应。尽管对于每个人类患者使用相同的手术技术放置电极,但是在图8a与图8b之间观察到的反应明显不同。图8a和图8b的骶骨反应是通过标准圆柱形电极的刺激而获得的,因此优先激活了与该电极最近的神经侧的纤维。应注意,在图8a和图8b中,记录的三个通道表示为CHn,是从沿着引线的连续电极获得的,这些连续电极与同一引线上的刺激电极的距离递增(分别距刺激6mm、12mm和18mm),使得作为动作电位沿神经的神经反应传播在较晚的时间在更多的远端记录电极的通道上发生,而未传播的信号在各个通道上在时间上未隔开,从而允许本发明将神经反应与其他电生理活动区分开。还应注意,在图8a中,电极4用于刺激,使得CH3最靠近刺激部位,而CH1离刺激部位最远,与图8b相比之下,在该图中使用电极1进行刺激,使得CH2最靠近刺激部位,而CH4离刺激部位最远。尽管刺激稍小,但注意到图8b中的反应幅度仍是图8a中的反应几十倍大。这种变化在实践中是常见的,并且很可能归因于每个相应电极阵列相对于神经的位置差异。图8b中的大幅度反应展示了通过应用本发明以便将刺激尽可能减少到达到希望治疗作用的最低水平,可以实现的功率节省量,这还可以具有进一步好处:避免过高的募集水平,过高的募集水平可能会造成疼痛、伤害或可能导致不利的副作用。
相比之下,图7的迷走神经反应是通过神经周围的袖套电极的刺激获得的,使得所有纤维类型都是根据其阈值募集的,而与它们在神经周长周围的位置无关。这种基于神经束位置的募集差异解释了从迷走神经和骶神经获得的反应差异,如一方面将图7与图8a进行比较而另一方面与图8b进行了比较时看到的。
特别地,在图7中,在小电流下,仅会募集较大的纤维类型,并且观察到的ECAP展现出很少的峰。随着电流的增大,除了预期的峰幅度增加之外,还会募集较小的纤维,并且所得的ECAP开始显示附加峰。
另一方面,在图8a中,观察1-2毫秒左右的时间范围的AβECAP,并观察肌电反应。因为传导速度和每个相应记录电极距刺激部位的距离,可以将1-2毫秒左右时间范围内的ECAP具体识别为AβECAP。进一步地,在大约4-10毫秒左右的时间范围内观察到的信号分量可以具体与Aα反应(直接激活或通过反射弧,例如通过Ia纤维激活)关联。虽然Aα反应的快速传导速度意味着此神经反应本身在记录的非常早期部分(例如在<1毫秒的时期内)就被掩盖了,但是因为存在在4-10毫秒左右的时间范围内观察到的非传播信号分量,应注意到Aα反应的存在,这是由肌肉激活产生的电场,并因此必须由于Aα激活而出现,因为这是Aα纤维的作用。在图8b中,观察到B纤维(最可能是神经节前副交感神经传出)主导了反应,并且同样,这个观察到的记录分量可以被具体被识别为B纤维反应,这是因为观察到的该反应的传导速度超过了正在使用的三个间隔的记录电极。尽管因此可能是B纤维,但是反应也可能源自Aδ纤维,并且同样应注意的是,所使用的命名法不被认为是限制本发明实现的纤维类型区分。
交感神经系统和副交感神经系统在调节内脏功能中起互补作用。根据经验,副交感神经纤维负责“休息或消化”反应,而交感神经纤维负责“战斗或逃跑”反应。例如在排尿情况下,副交感神经刺激膀胱并使尿道放松,而交感神经纤维抑制膀胱体并刺激膀胱底和尿道。在膀胱神经支配的情况下,神经节前交感神经纤维在头端腰段的前根离开脊髓,神经节前副交感神经纤维通过骶神经的前根离开脊髓。因此,在一些实施例中,本发明认识到,与靶向远程肌肉反应作为治疗的替代的现有方法相比之下,选择性以这些纤维类型为靶可以用于失禁治疗。因此,本发明的这样的实施例可以提供用于测量来自骶神经的诱发复合动作电位(ECAP)以通过更好地靶向适当的神经并通过基于ECAP观察结果在慢性植入物中施加闭环刺激来改善SNS的技术。
更一般地,本发明的其他实施例可以将类似的靶向方法应用于身体的任何混合神经的神经调节,以便为与任何这样的神经相关的功能障碍提供治疗。图9展示了许多这样的交感神经(红色)和副交感神经(蓝色)传出通路,这些传出通路形成可以在其上应用本发明的方法的混合神经的一部分。中断的红线指示颅神经和脊神经的神经节后分支。
应注意,心脏、喉、食道、胃、肝、胰、肠和肾的副交感神经支配都起源于第10颅神经,也称为迷走神经。肠、膀胱和生殖器的副交感神经支配起源于脊髓的骶段。应注意,大肠的上部部分和下部部分是从不同部分受神经支配的,上部部分是迷走神经,下部部分是骶神经。
骶神经是包含完整范围的纤维类型和功能的混合神经。其包含C纤维以及从B(副交感神经)到Aα运动神经元的有髓鞘纤维,并携带传入和传出神经信号。骶神经不是同质的,并由小根构成,进一步细分为束,保留了一定程度的纤维类型功能分离。
神经纤维按其功能及其物理特性(主要是髓鞘形成状态和纤维直径)分类。重要的是应注意,物理特性和功能的这种分离不是绝对的,并且具有各种功能的神经信号是通过特性相似的纤维携带的(功能多于纤维的种类)。根据经验,有髓鞘纤维的直径(单位μm)与传导速度(单位m/s)的相关系数约为6。由于此特性,可以使用从SNS电极测量的ECAP来确定传导速度,进而确定刺激范例正在募集哪些纤维。这可以在2个水平上改善治疗:适当神经的病因特异性靶向以及闭环反馈控制,用于维持稳定的治疗并提高有效性。
由于骶神经是不均匀的混合神经,引线相对于神经纤维的位置在确定哪些纤维类型被SNS激活方面起着重要作用。不受理论的束缚,由SNS电极做出的电生理反应的记录可以区分激活了肌肉传出、感觉传入、躯体传入和传出、副交感神经纤维还是C纤维。ECAP特性和迟发反应特性都可以用来确定刺激激活哪些纤维。
例如,在图8a中,未传播的迟发反应的存在指示Aα传出的激活(直接或间接),而Aβ反应的存在指示感觉纤维被激活。在图8b中,B纤维反应的存在可以指示副交感神经纤维的激活。由于骶神经的分段性质(分离成束,具有相似功能的纤维束聚集在这些束中),从SNS观察到的反应并不均匀(参见图8)。进一步地,骶丛的患者间可变性很可能引起每个骶神经中存在的纤维类型的变化。
与使用SNS观察到的反应类型无关,患者身体姿势的变化以及引起电极相对于神经运动的内部机制都反映在神经反应幅度的变化中。图10a展示了Aβ反应的幅度的变化(图8a中的示例在1-2毫秒左右),在图10a中表示为FAST(快速)。图10b示出了肌电反应的幅度变化(图8a中的示例大约4-10毫秒),在图10b中表示为LR。此数据是从接受S3骶神经根的SNS试验的患者获得的。患者在图10的实验开始时坐着,在约300秒时站立,然后在约360秒时坐回去。神经反应幅度的增大(从坐着到站着大约增加了一倍)被认为是患者感觉异常的增加。在肌电反应中观察到类似的幅度增大。
图11a展示了与图10相同的人类SNS患者的神经Aβ反应的生长曲线的测量值。图11b展示了肌电反应生长曲线的测量值。本文中的生长曲线是指响应于刺激强度增大的电生理反应幅度的量度。当电生理反应的记录包括多个分量时,比如同时包括ECAP和肌电反应,则可以获得每种这样的分量的单独生长曲线。图11a的Aβ反应和图11b的肌电反应两者都显示阈值行为,因为对于在低于特定阈值的电流水平下施加刺激,没有出现反应。通过检查,图11a中的阈值出现在1.5mA左右,而通过检查,图11b中的阈值出现在2mA左右。高于相应的阈值时,每条生长曲线都展现出线性区段,其中线性增大刺激电流引起神经Aβ反应和肌电反应两者都近似线性增加。当达到最大募集时,两条曲线都预期在较高的刺激幅度处达到平稳状态,然而,这些较高的刺激值未在此实验中应用,因为这会使患者感到疼痛。因此,图11展示了肌电反应和Aβ反应各具有线性部分,该线性部分可以由在图11所示的相应线性范围内操作的反馈环路单独或共同利用。
图11c展示了来自经S3骶神经刺激治疗尿失禁的人类患者的记录。在大约3-10毫秒的时间段期间,在0.7mA刺激和1.3mA刺激下,所有记录通道CH1、CH2和CH4上都可以看到清晰的迟发反应。在这些记录中看不到快速响应(ECAP),因为刺激脉冲宽度遮蔽了ECAP。图11d示出了在每个相应记录电极处所测量的响应于刺激幅度增大的迟发反应幅度的生长曲线。如图11d中看到的,这是平稳的肌电反应的示例。这样的记录使得能够确定刺激的有效范围,因为低于阈值时没有治疗益处发生,并且在生长曲线中没有额外的纤维被募集超过该平稳状态。图11e是被认为是B纤维反应的慢反应的平稳生长曲线的图。曲线上指示了患者开始感觉到刺激的生长曲线部分。值得注意地,此患者阈值处于此B纤维反应的平稳期,这意味着在图11e中观察到的B纤维慢反应与感觉无关,并且募集了第一纤维类型而没有募集其他纤维。
一些实施例可以进一步提供分阶段的纤维靶向,由此在第一操作阶段中,希望的纤维类型的募集水平被监测并维持在图11a、图11b或图11e的曲线的希望范围内,并且其中当失去希望纤维类型的募集或观察时(在姿势变化或引线迁移时可能常见),采用第二操作阶段,其中次级纤维类型的募集替代地被监测并维持在希望范围内,以便即使在选择的纤维类型的募集尤其难以追踪时,也确保总体上持续的神经募集。对失去对希望纤维类型的募集或观察的替代反应可以是触发任何其他合适的事件,比如警告信号或自动重编程程序。附加地,可以响应于失去对所需纤维类型的募集或观察而进行线性测试,由此改变刺激幅度以探索募集是否做出线性反应;对这种刺激幅度变化没有线性募集反应可能指示治疗功效的丧失,这可以用于触发重新编程或警告信号等。
如关于图8b所指出的,还可以识别自主神经系统的B纤维的募集和选择性靶募集。可以类似地靶向肌电反应。图12展示了在人类SNS患者中观察到的肌电反应的幅度随时间的变化(上迹线)、以及在涵盖开环模式和闭环模式的一段时间内的相关控制变量(下部迹线)。因此,图12展示了基于肌电反应的成功闭环控制。
具备了这些见解,本发明的实施例可以因此提供混合神经中的病因特定纤维靶向。例如,在SNS的情况下,代替依靠过去利用肌肉反应的方法而不了解募集的纤维类型,可以使用本发明来执行靶向和编程。混合神经中的病因特定纤维靶向识别出在刺激混合神经时的大多数情况下,只有纤维的子集与通过刺激治疗的病症有关。刺激混合神经的所有纤维将不可避免地导致不利的副作用或低效的刺激。
从最广义的意义上讲,这些特定的实施例通过使用电生理学测量来选择性地靶向混合神经中的纤维类型的子集,从而优化了对混合神经的神经调节治疗。这将包括优化刺激参数,使得获得希望纤维类型的反应,同时将不利的纤维类型的反应最小化。在理想情况下,仅希望纤维将被装置激活,但是在许多实施例中,简单地实现希望纤维类型的优先靶向和/或优先最小化其他纤维类型的募集仍然可以产生本发明的益处。
在一个实施例中,本发明提出了一种装置,该装置可以刺激和记录从放置在混合神经附近的电极获得的电生理信号。然后可以分析神经反应以指示受刺激纤维的性质。在示例中,电生理反应可以指示特定神经纤维的存在或不存在。基于希望的治疗结果,此信息用于优化治疗。例如,可以调节用于施用治疗的各种参数,比如但不限于刺激波形和引线位置,以募集希望纤维类型。在一些情况下,可以同时治疗几种原因,每种原因都需要刺激不同纤维子集(比如均位于骶神经中的用于失禁的B纤维和用于盆腔疼痛的感觉纤维)。在这种情况下,该方法和装置将使用电生理学测量来连续地优化所传递的刺激方案,以便引起希望纤维的反应,同时最小化来自不想要的纤维的响应。
在图14所示的一个实施例中,提出了一种方法,该方法由2个或更多个电极植入到靶混合神经附近,靶混合神经的仅纤维的子集是希望刺激靶,并且提出了一种能够记录对刺激的电生理反应的植入式控制单元装置。通过记录的反应评估受激纤维群,并对电极放置、电极选择以及刺激参数(包括但不限于脉冲宽度、刺激频率、刺激波形、刺激幅度)进行优化,使得优选靶向希望纤维类型。附加地,所记录的反应可以用于优化刺激参数,以使功耗最小。只要观察到希望反应,就应该使用为了实现这一点需要最少功率的参数。在一项SNS试验中,观察到B纤维在刺激幅度下被完全募集,该刺激幅度仅是参考过去肌肉激活阈值方法所定义幅度的四分之一左右。此观察指示由B纤维募集提供的治疗益处可以以无视纤维类型而操作的现有SNS技术的当前状态的一小部分功率来实现。
为了评估刺激范例激活了哪些纤维类型,使用神经反应的特征(诱发复合动作电位或ECAP)。不受理论的限制,可以用于评估受激纤维类型的ECAP特征包括传导速度(包括潜伏期)、强度持续时间特征和不应期。附加地,基于存在和潜伏期,肌电反应(迟发反应(LR))可以用作Aα传出激活(直接或间接)的替代量度。
例如,在骶神经刺激的情况下,靶向副交感神经纤维(轻度有髓鞘的B型纤维),将电极放置在S3骶神经附近,如图14所示。第一电极用于刺激,并且从相同阵列中的靠近靶神经的1个或多个电极获得神经记录。受刺激纤维的传导速度从神经记录中获得,并放置电极,使得B纤维反应最大化,而所有其他反应被最小化。进一步地,改变刺激参数,使得B纤维反应最大化,而所有其他反应被最小化。
在其他实施例中,比如图13和图15的实施例,提供了一种装置,该装置使用具有不同特征的多个电极来优化引线定位和刺激参数选择,以刺激混合神经。这使用了多于1根放置在靶神经附近的引线。然后为多于1根引线进行刺激参数的选择。在许多病症情况下,比如膀胱过度活动症(OAB)以及盆腔疼痛,将进行电极选择,使得一根或几根引线上的1个或几个电极最大化B纤维激活(用于缓解OAB)和Aβ纤维激活(用于缓解疼痛)。再一次应注意的是,本发明不受机制理论的限制,因为可以就任何实现治疗好处的合适纤维方面不论理论化作用机制如何,都可以进行纤维类型靶向。
在其他实施例中,可以依次刺激神经丛的神经,以使用诱发的电生理反应的记录确定神经丛的哪个分支包含希望的纤维。然后在神经丛的最佳候选分支上优化治疗。
另外,不必在同一根引线上记录给定刺激电极的ECAP。可以优化记录部位(最大信号幅度),并且可以选择任何靠近靶神经的电极。这是有效的,因为混合神经中的束在神经内不平行延伸(如图13至15所示,只是为了说明简单起见,但如图6所示交叉、合并和分裂)。
此装置的另一个实施例将包括植入袖套电极来代替硬膜外引线,如图16所示。袖套电极各自由几个不同的触点组成(与一些袖套电极具有沿整个袖套延伸的一个连续电极相反),并且可以旋转2个或更多个袖套电极并沿着混合神经上下移动以找到优化离散刺激和单独束的记录的位置。在一个实施例中,图16的布置可以用于选定纤维类型的空间靶向。这包括袖套电极1610围绕神经的基本上整个圆周施加超最大刺激,以募集神经的所有纤维。电极1610可以是围绕神经连续延伸的非分段电极,或者可以是分段电极,其中刺激由电极1610的所有段同时施加以确保所有纤维被募集。然后可以通过使用选定段(例如,分段袖套电极1620的段1622(为清楚起见未示出其他段))在选定圆周位置处观察到由刺激引起的募集反应。然后可以特别分析在段1622处观察到的反应,以确定与那个段1622相邻的主导纤维类型。然后,可以使用此知识来识别哪种(哪些)纤维类型与每个分段袖套电极的所有段相邻。继而,可以使用此知识来优选使用给定的段,比如段1622,以传递刺激,该刺激将优选募集已知与那个段相邻的纤维类型。其他实施例可以通过代替地使用段1622来施加仅刚好高于刺激阈值的刺激,并且因此仅募集在段1622附近的纤维来获得相同的知识。然后可以在袖套电极1610处观察到反应,并对这些反应进行分析以确定由段1622募集的纤维类型,从而确定与1622相邻的纤维的类型。可以在多个段上进行调查,由此确定与所有这样的电极段相邻的纤维类型,然后可以通过使用比如查找表等这样的知识来应用空间纤维类型靶向。
附加地,反应可以用于优化激励参数,以最小化功耗。只要观察到希望反应,就应该使用为了实现这一点需要最少功率的参数。
更一般地,可以使用不同的引线形状,桨状引线、经皮引线、袖套电极,同时实现根据本发明的纤维类型靶向。
在一些实施例中,该装置在外部连接到电极,在其他实施例中,该装置是可完全植入的。在一些实施例中,电极可以在单根引线上,或在放置在刺激靶附近的多根引线上,每根引线可以包含多个电极。在其他实施例中,一些电极可以位于靶神经的远侧,以用作用于测量的参考电极或用于刺激的返回电极。在装置是可完全植入系统的情况下,植入物的本体可以用作一个电极。
在一些实施例中,引线可以是硬膜外的,类似于骶神经刺激或脊髓刺激中使用的引线。在其他实施例中,引线可以是桨状引线或袖套电极。在一些实施例中,电极可以进一步是分段的。在一个示例中,袖套电极在一些情况下可以是围绕神经的环形电极,并且在一些情况下可以是分段环,其中袖套电极的电学上不同的部分在神经的圆周周围占据不同的位置,从而允许通过从选定段进行刺激来选择性地靶向神经周围的不同圆周位置,或者通过从选定段进行记录来选择性地观察神经周围的不同圆周位置。在一个示例中,硬膜外引线可以具有圆形电极触点,在其他示例中,触点可以是分段的。
在一些实施例中,刺激和记录在相同的电极上进行,在其他实施例中,刺激和记录在不同的电极上进行,无论是在相同的电极引线或电极阵列上,还是在不同的电极引线或阵列上。在一些实施例中,植入在靶附近的一些电极跨越的面积足够小以避免神经内的纤维束合并或分支。例如,在一些实施例中,植入到骶神经附近的电极可以跨越不到5厘米、优选地不到2厘米的距离。
在一些实施例中,外部装置可以用于在引线插入期间分析神经反应。外部装置可以用于将刺激脉冲传递到患者的靶神经。外部装置可以包括控制单元、显示模块和刺激模块。刺激单元被配置为将刺激脉冲传递至希望的神经并分析所产生的神经反应。此后,控制单元适于分析神经反应并基于一个或多个参数来区分所募集的神经。控制单元被配置为以一种或多种模式传输信息以显示神经募集。在第一模式下,以ECAP波形和肌电波形的集合的形式显示神经募集。在第二种模式下,神经募集显示为用相应纤维类型加标签的波形。在第三模式下,神经募集显示为具有定时分区的波形,以指示被募集的纤维类型。在第四种模式下,显示神经募集以及纤维的特征,比如传导速度等。
在一些实施例中,外部装置适于操作一个或多个电极。外部装置适于通过桨状引线、圆柱形引线和袖套电极传递脉冲。进一步地,外部装置被配置为以上述电极的任何组合进行操作。
植入式装置:在一些实施例中,植入式装置被配置为执行与外部装置一样的所有功能,不同之处在于其被植入患者体内。
在一些实施例中,由刺激引起的电生理反应可以被实时显示。在一些实施例中,可以进一步处理这些反应以增加信噪比。例如,平均可以用于减少在每个单独反应上发现的随机噪声,而拟合和去除算法可以用于减少信号中的伪影分量。这些信号可用于识别某个纤维类型或某些纤维类型的激活。然后可以通过试错、蛮力探索或刺激参数空间的任何合适的搜索技术来更改刺激参数,以便如所显示的反应所指示的那样优化希望的纤维子集的纤维激活。
在一些实施例中,电生理反应曲线图的轴线可以被加标签以有助于反应的解释。例如,基于刺激和记录电极的配置,反应曲线图的轴线可以被加标签以指示应该记录给定纤维类型的反应(如果存在)的时间。
在一些实施例中,可以对神经反应进行标记和/或加标签以帮助识别刺激所募集的纤维类型。例如,反应的峰可以用其时间和幅度参数加标签。
在一些实施例中,具有变化的刺激参数的反应形态的变化可以用于识别其中峰定义不佳的ECAP中的纤维类型贡献。在一个示例中,电流缓慢上升,并观察或自动处理ECAP形状。最初,最大的纤维将首先对外部刺激做出反应,ECAP形态在较高幅度下的变化可能指示第二较慢的纤维类型的募集。在另一个示例中,刺激的频率可以增加到适当的水平,并且随着时间的过去观察到反应。小纤维将首先疲劳,大纤维将最后疲劳。形态随时间的变化可以用于确定ECAP的哪个部分与哪种纤维类型相关。进一步地,在刺激频率增加时,肌电反应的幅度减小可能是由于肌肉疲劳或H反射降低所致。这些特性和其他特性可以用于确定被刺激的纤维类型。
在一些实施例中,可以对所记录的神经反应进行处理以显示对电生理反应的记录的不同要素的传导速度的估计值。
在一些实施例中,可以对电生理反应的记录进行处理,使得显示对正在募集的每种类型纤维的数量的估计值。在一些情况下,估计值可以给出纤维类型的比例或正被激活的纤维数量的绝对估计值。
在一些实施例中,可以通过在反馈布置中自动调整刺激参数以接近恒定神经募集的方式进行刺激,以便接近靶电生理反应。在一个示例中,优选地靶向混合神经中的B纤维。在选择刺激电极和优化B纤维募集的参数后,可以自动调节电流(或其他参数)以保持恒定的反应幅度。在另一个示例中,可以将迟发反应用作运动纤维激活的替代量度,然后可以自动调节刺激以维持恒定的迟发反应幅度。
电生理反应的幅度或其一部分可以通过评估峰幅度、峰峰幅度、曲线下面积、使用滤波器的卷积或任何其他方法来完成。
在一些实施例中,植入式装置可以被配置为在靶纤维类型上建立闭环刺激。例如,可以在混合神经中的B纤维的神经反应上建立反馈环路。本文披露的方法使植入式装置能够在某些纤维类型的神经反应上建立反馈环路。由于植入式装置被配置为基于神经反应来确定纤维类型,因此植入式装置可以基于靶向纤维类型的反应来维持闭环刺激。植入式装置可以在包括但不限于Aα、Aβ、Aδ、B和C纤维的至少一种或多种纤维类型上维持闭环刺激。在一些情况下,可以在刺激引起的肌电反应上维持闭环刺激。如果无法观察到希望反应,则可以将另一个反应用作建立和维持闭环刺激的替代量度。
在一些实施例中,可以自动监测神经募集的大变化,大变化可以指示例如引线迁移。在一个示例中,该装置可以通过在较小程度上改变电流幅度并监测反应曲线来测试会导致功效丧失的电极位置变化。如果在刺激幅度变化的情况下未观察到变化,则将警告信号发送给使用者,或者停止刺激或发生其他合适的动作。
在一些进一步的实施例中,植入式装置可以被配置为检查由刺激脉冲募集的纤维类型。此特征可以用于确保在大部分刺激持续时间内募集希望的纤维类型。在一些情况下,当植入式装置在超过阈值的持续时间内未能检测到纤维类型的反应时,植入式装置可以向患者发出警报:不再募集希望的纤维类型。在这种情况下,医生可能会考虑可能的原因并采取补救动作。在其他实施例中,在这种事件中可以触发替代性动作,比如刺激范例的自动重新配置。这样的功能可以替代地由比如临床医生的编程器装置等外部装置来执行。
在一些实施例中,连续监测可以用于同时持接近恒定的神经募集和警告引线放置或神经特性的显著变化。进一步的实施例可以提供在施加下一个校正性刺激脉冲之前测试反馈环路的完整性。这可以通过确保如果电流增大则反应增加来完成。这在失去信号时(在改变姿势或在应用充电器或其他干扰源时信号可能发生)可以防止环路发散。
在一些实施例中,该方法和装置可以应用于迷走神经刺激、后根刺激、骶神经刺激或前根刺激。在一些实施例中,该方法和装置可以在多于一个位置同时或串联使用。例如,该装置可以用于骶神经和支配膀胱和肠的下胸或上腰前根中。不受理论的限制,提出了这样的装置可以在对于膀胱和肠控制特定的位置刺激交感神经系统和副交感神经系统。刺激脊髓和/或迷走神经和/或骶神经的前根可以产生类似的实施例,其中靶不同于膀胱和肠,比如心脏、喉、气管、支气管、食道、胃、肝、胰腺、小肠、脾、大肠、肾或性器官中的任何一个或多个。例如,肝的交感神经和副交感神经刺激可以通过刺激胸段T5-T12中的交感神经纤维和起源于迷走神经的副交感神经纤维来实现。迷走神经刺激可以通过本发明的一些实施例来改善,可以选择性地避免刺激在过去的解决方案中引起喉咙、吞咽肌肉、声带等的疲劳、在过去的解决方案中不必要地被募集的纤维类型。应注意,还有脊髓的部分病变可以通过靶向特定纤维来最好地治愈。
在其他实施例中,编程和显示模块不必是植入的装置的一部分。在这样的实施例中,植入的刺激器本身仅执行由控制模块设置的程序,并且仅在外部编程器上而不是在植入的装置上实时或稍后才观察到反应。
因此,前述内容揭示了患者之间纤维类型募集的巨大差异、以及对募集的纤维数量的、与姿势相关的显着影响。进一步地,在募集的纤维类型与试验结果之间有明显的区别。这些首次人体结果显示电生理反应的记录可以用于识别通过SNS治疗募集的纤维类型,并且纤维类型可能与治疗有效性相关。这些结果为改善的靶向以及可能大大改善治疗的闭环SNS铺平了道路。
值得注意地,本发明的纤维靶向允许一些受试者在刺激水平下获得有效的治疗,这些刺激水平比常规用于设置刺激水平的肌肉反应阈值或感觉反应阈值小几个数量级。这将使电池寿命增加很多倍。
持续进行纤维类型靶向以控制反馈过程的实施例的进一步优点是随着姿势变化、咳嗽、打喷嚏等对相对于神经的引线运动发生反应。与将刺激固定在单个高水平上的常规方法相比之下,基于电生理测量值的反馈的使用允许对刺激进行不断修正,以确保通过持续适当地完善刺激参数来确保继续募集希望纤维类型。即使在简单的实施例中也可以获得类似的好处,这些实施例简单地检测到失去希望纤维类型的募集并向使用者发出警报,例如寻求临床帮助。可替代地,在一些实施例中,在所确定的刺激范例不能募集希望纤维类型的情况下,可以对刺激范例进行自动更改。
应当理解,在实施例中命名的纤维类型是基于当前的理解,并且在本发明的应用中不应被认为是限制性的。
可以通过安装在印刷电路板上的分立部件和/或集成电路的组合和/或专用集成电路(ASIC)来实现所描述的电子功能。
本领域的技术人员将认识到,在不脱离广义描述的本发明的精神或范围的情况下,可以如具体实施例中所示对本发明进行许多变化和/或更改。因此,目前实施例在所有方面都被认为是说明性的而非限制性或约束性的。
Claims (38)
1.一种包括多种神经纤维类型的混合神经的神经刺激方法,所述方法包括:
将植入式电极阵列定位在包括多种神经纤维类型的混合神经附近,所述植入式电极阵列包括多个电极;
从所述植入式电极阵列中的至少一个标称刺激电极传递电刺激,所述电刺激是根据一组刺激参数被传递;
从所述植入式电极阵列中的至少一个标称记录电极获得由所述电刺激诱发的电生理反应的记录;
通过评估所述电生理反应的记录的一个或多个选定特征并根据所观察到的选定特征识别所述电刺激募集的一种或多种纤维类型的募集水平,从而分析所述记录;以及
以实现所述混合神经的一种或多种纤维类型相对于其他纤维类型的选择性募集的方式完善所述刺激参数。
2.如权利要求1所述的方法,其中,所述刺激参数以实现至少一种希望纤维类型的选择性募集同时进一步实现至少一种非选定纤维类型的选择性非募集或减少募集的方式被完善。
3.如权利要求1或权利要求2所述的方法,其中,被完善以实现所述希望纤维类型的选择性募集的所述刺激参数包括以下中的一个或多个:术中电极放置;术中电极阵列类型选择,包括引线、桨状或袖套阵列选择;刺激频率;刺激幅度;刺激波形;刺激脉冲宽度;刺激电极选择、刺激阶段;刺激极性、刺激电极大小和刺激电极形状。
4.如权利要求1至3中任一项所述的方法,其中,所述记录的、根据其识别所述电刺激对所述一种或多种纤维类型的募集水平的一个或多个选定特征包括以下中的一个或多个:一个或多个电生理反应拐点;一个或多个电生理反应峰位置;一个或多个电生理反应峰幅度;电生理反应传播速度;所述电生理反应的传播或非传播;电生理反应持续时间;不应期;强度-持续时间曲线特征,包括时值或基强度;生长曲线特征,包括阈值和斜率;随着刺激电流增大,电生理反应峰的数量;迟发反应的存在、幅度和/或潜伏期、对变化的刺激频率的电生理反应的反应特性、所述电生理反应的零交叉、所述电生理反应的波瓣宽度。
5.如权利要求1至4中任一项所述的方法,其中,所述选定特征是传导速度,并且根据有髓鞘纤维的直径与所述传导速度之间的关系确定募集的所述纤维类型。
6.如权利要求1至5中任一项所述的方法,其中,所述选定特征是传导速度,并且通过确定从所述刺激部位到距所述刺激部位已知距离的单个测量电极的传播时间来测量所述传导速度。
7.如权利要求1至6中任一项所述的方法,其中,所述选定特征是传导速度,并且其中,通过观察在第一测量电极和第二测量电极处的神经反应并确定所述第一测量电极与所述第二测量电极之间的传播时间来测量所述传导速度。
8.如权利要求1至7中任一项所述的方法,其中,通过评估从两个或更多个间隔开的测量电极获得的所述电生理反应的记录、观察单个电生理反应来确定所述选定特征。
9.如权利要求1至8中任一项所述的方法,包括基于与肌肉激活无关的纤维的募集传递治疗。
10.如权利要求1至9中任一项所述的方法,其中,所述选定特征包括所述记录的非传播特征,所述非传播特征是由于引起远场肌肉激活的运动纤维的激活而产生的。
11.如权利要求1至10中任一项所述的方法,其中,所述选定特征被采取用于指示Aα纤维或Ia纤维的激活,并且包括以下中的至少一个:所述刺激后不到1毫秒出现的响应、以及传导速度在80-120m/s范围内的反应。
12.如权利要求1至11中任一项所述的方法,其中,所述选定特征被采取用于指示B纤维的激活,并且包括以下中的至少一个:所述刺激后不到6毫秒出现的响应、以及传导速度在3-15m/s范围内的反应。
13.如权利要求1至12中任一项所述的方法,其中,所述选定特征被采取用于指示C纤维的激活,并且包括以下中的至少一个:所述刺激后不到6毫秒出现的反应;传导速度在0.5-2m/s范围内的反应;以及持续时间超过10毫秒的反应。
14.如权利要求1至13中任一项所述的方法,其中,所述选定特征被采取用于指示Aβ纤维的激活,并且包括以下中的至少一个:所述刺激后3毫秒内出现的反应;以及传导速度在30-80m/s范围内的反应。
15.如权利要求1至14中任一项所述的方法,其中,为了确定由所述电刺激募集的两种或更多种纤维类型中的每一种纤维类型的募集水平,评估所述电生理反应的记录的多于一个选定特征。
16.如权利要求15所述的方法,其中,靶向所述两种或更多种纤维类型,用于治疗单一病症。
17.如权利要求15所述的方法,其中,靶向所述两种或更多种纤维类型,用于治疗两种或更多种并存病症或共病病症。
18.如权利要求1至17中任一项所述的方法,其中,应用机器学习分类器以便按照存在的纤维类型对观察到的神经反应进行分类。
19.如权利要求1至18中任一项所述的方法,其中,所述至少一个标称记录电极和所述至少一个标称刺激电极被定位成与所述混合神经的单个分支相邻,所述单个分支是所述混合神经的一段,在所述段中,在所述标称记录电极与所述标称刺激电极之间的神经之间没有神经分支或神经合并发生。
20.如权利要求1至19中任一项所述的方法,其中,所述标称记录电极和所述标称刺激电极的位置分开小于60mm。
21.如权利要求20所述的方法,其中,所述标称记录电极和所述标称刺激电极的位置分开小于30mm。
22.如权利要求21所述的方法,其中,所述标称记录电极和所述标称刺激电极的位置分开小于20mm。
23.如权利要求1至22中任一项所述的方法,其中,所述混合神经包括至少两种纤维类型,包括Aα、Aβ、Aδ、Aγ、B和C纤维类型中的两种或更多种,或包括其他命名系统纤维类型。
24.如权利要求1至23中任一项所述的方法,其中,所述混合神经包括迷走神经。
25.如权利要求24所述的方法,其中,所述一种或多种纤维类型包括副交感神经纤维,以提供比如难治性癫痫或抑郁症的一种或多种与脑有关的病症的治疗,或在外周或内脏发挥治疗作用。
26.如权利要求25所述的方法,提供心脏、喉、气管、支气管、食道、胃、肝、胰腺、小肠、脾、大肠或肾的治疗。
27.如权利要求1至23中任一项所述的方法,其中,所述混合神经包括骶神经。
28.如权利要求27所述的方法,用于提供用于粪便失禁(FI),尿潴留(UR)、急迫性尿失禁(UUI)、顽固性便秘和慢性盆腔痛中的一种或多种的治疗。
29.如权利要求1至23中任一项所述的方法,其中,所述混合神经包括脊神经的根。
30.如权利要求29所述的方法,其中,所述第一纤维类型包括副交感神经纤维,用于为大肠、膀胱和生殖器中的任何一个或多个提供治疗。

33.如权利要求29所述的方法,其中,所述第一纤维类型包括交感纤维,用于通过刺激胸段T12和腰段L1-L3的一个或多个前根中的交感纤维为肾、膀胱、生殖器和/或下肠道提供治疗。
34.如权利要求1至33中任一项所述的方法,包括通过从第一电极施加超最大刺激来募集所述神经的所有纤维来在空间上靶向所述选定纤维类型,通过使用用于在选定圆周位置处记录的选定电极段来观察在所述选定圆周位置处的所募集的反应,分析所记录的反应以确定与那个位置相邻的一种或多种纤维类型,以及随后在希望募集如此识别的所述一种或多种纤维类型时从所述选定电极段施加刺激。
35.如权利要求1至33中任一项所述的方法,包括通过在选定圆周位置使用选定电极段施加刚好高于刺激阈值的刺激来在空间上靶向所述选定纤维类型,以募集在那个段附近的纤维,观察在第二电极处的募集的反应,以及分析所述记录以确定来自所述选定电极段的刺激正在募集的纤维类型。
36.一种非暂时性计算机可读介质,用于混合神经的神经刺激,所述混合神经包括多种神经纤维类型,所述非暂时性计算机可读介质包括指令,当由一个或多个处理器执行时,所述指令将引起执行以下操作:
从位于混合神经附近的植入式电极阵列的至少一个标称刺激电极传递电刺激,所述混合神经包括多种神经纤维类型,所述植入式电极阵列包括多个电极,所述电刺激是根据一组刺激参数传递的;
从所述植入式电极阵列中的至少一个标称记录电极获得由所述电刺激诱发的电生理反应的记录;
通过评估所述电生理反应的记录的一个或多个选定特征并根据观察到的所述选定特征识别由所述电刺激募集的纤维类型的募集水平来分析所述记录;以及
以实现所述混合神经的一种或多种纤维类型相对于其他纤维类型的选择性募集的方式完善所述刺激参数。
37.一种神经刺激装置,包括:
植入式电极阵列,所述植入式电极阵列被配置为被植入到包括多种神经纤维类型的混合神经附近,所述植入式电极阵列包括多个电极;以及
控制单元,所述控制单元被配置为从所述植入式电极阵列中的至少一个标称刺激电极传递电刺激,所述电刺激是根据一组刺激参数被传递;所述控制单元进一步被配置为从所述植入式电极阵列中的至少一个标称记录电极获得由所述电刺激诱发的电生理反应的记录;所述控制单元进一步被配置为通过评估所述记录的一个或多个选定特征来分析所述记录,以及根据观察到的所述选定特征中识别由所述电刺激募集的至少第一纤维类型的募集水平,并且所述控制单元进一步被配置为以实现所述混合神经的一种或多种纤维类型相对于其他纤维类型的选择性募集的方式完善所述刺激参数。
38.如权利要求37所述的装置,其中,所述控制单元是用于试验或术中使用的外部非植入式装置。
