Abstract
Description

DBS刺激に関連するERNAの変化
本発明者らは、加えられた刺激に対して誘発された神経応答が共振活動を示すだけでなく、場合によっては、誘発された活動が複数の共振を含むと判定した。図7a、図7b、及び図7cは、それぞれ1.5mA、2.25mA、及び3.375mAでの連続DBSに応答するERNAを示している。1.5mAで、共振ERNAは単一のピークとして始まり、それが2つのピークにわずかに分岐し始めることが見て取れる。2.25mAで、優位なものが2つのピークのうち遅い方に切り替わる。しかし、1.5mAで優位だったより早期のピークは、より低い振幅で継続する。3.375mAでは、2つのピークが存在し、遅い方のピークが優位である。これらの複数の共振のピークは、異なる神経回路の活動に対応すると考えられる。これらの共振応答(または他の特徴、例えば時間的またはスペクトルの特性)間での相対的な振幅は、治療の状態の指標となり得る。
図20から図23は、患者の症状及び関連するERNAの変化に対するDBSの肯定的な効果のさらなる証拠を示している。これらの図に示されているデータは、電極アレイが植込まれたパーキンソン病の患者から収集されたものである。電極アレイは長期的に植込まれ、ERNAと運動状態の測定は植込みの数ヶ月後に行われた。このときのERNAと運動状態を測定することにより、電極アレイとその神経環境は安定していると推測できる。したがって、ERNAと運動状態との間で確認された関係は、重大な術中の処置の間または電極挿入から数日以内に実施された研究の間に測定された関係よりも、患者の長期にわたる状態をより良好に表している。そのような短期間の研究からの発見は、治療的DBS自体の適用ではなく、おそらく外科的植込み手順に関連するパーキンソン病の運動障害の一時的な緩和を特徴とする「スタン効果」によって混乱する可能性があることが見出された。
長期的に電極を植込まれた患者におけるDBSの効果の確認に加えて、本発明者らは、DBSに応答するERNAが存在し、全身麻酔下の患者において測定可能であると判定した。図27から図33は、覚醒している患者と全身麻酔下で記録されたERNAの比較を示している。これらの図のそれぞれについて、植込まれた電極は、振幅3.38mA、周波数130Hz、位相60μsの単極対称二相パルスで刺激され、1秒ごとに10の連続パルスのバーストへと時間的にパターン化された。電極は上記のように植込まれた。ERNAは、以下の表1に示すように、覚醒し、全身麻酔下で、様々な誘導用薬剤や維持用薬剤を使用して、21名の患者の左右のSTNで測定された。
全身麻酔は、吸入、静脈内、またはそれらの組み合わせのいずれかによって、当技術分野で知られている任意の方法で維持することができる。適切な吸入麻酔薬の例には、イソフルラン、セボフルラン、デスフルラン、メトキシフルラン、ハロタン、亜酸化窒素、及びキセノンが含まれる。適切と思われる他の吸入麻酔薬には、クロロエタン(塩化エチル)、クロロホルム、クリオフルラン、シクロプロパン、ジエチルエーテル、エンフルラン、エチレン、フルロキセン、メトキシプロパン、トリクロロエチレン、及びビニルエーテルが含まれる。吸入剤は、オピオイド(例えばフェンタニルまたはレミフェンタニルなどのフェンタニル誘導体)及び/または鎮静剤(プロポフォールまたはベンゾジアゼピンなど、例えばミダゾラム)及び/またはバルビツール酸塩(チオペンタールなど)などの静脈麻酔薬によって補充され得る。
DBSは共振神経活動を誘発する
DBSパルスから生じる神経活動を調査して、バイオマーカーとして実行可能に使用できる誘発応答があったかどうかを判定した。誘発された活動を維持するために、本発明者らは、非常に長い第2のフェーズを伴う従来の非対称パルスではなく、広い記録帯域幅と対称二相パルスを刺激に使用し、刺激のアーチファクトの一時的な持続期間を最小限に抑えるようにした。
ERNAがSTNに対する電極の位置によって変化することを確立するために、刺激されていない3つの電極から記録しながら、4つのDBS電極のそれぞれに10秒間のバースト刺激を連続して適用した。DBS植込み手術は、STN内に4つの電極のうち2つを配置することを目的としており、1つの電極は通常DBSが最大の利益をもたらす背側STNに、もう1つは腹側STNに配置する。電極の位置のこの変動は、STNの異なる領域からのERNA応答と核の外側の領域との比較を容易にした。8人のSTN−PD患者(n=16半球)では、ERNAの振幅と形態の両方が刺激電極と記録電極の位置に応じて変化することを見出した。図14は、1つの半球の各バーストの最後のパルスからの模範的なERNAを示している。最大の振幅と、標的STN位置の2つの中央電極で発生する最も明白な減衰振動形態の応答がある。また、コントロールとして、PSA内の遠位電極と視床の腹側中間核の近位電極を配置することを目的とした植込み軌道を使用して、2人の本態性振戦患者(n=4半球)に刺激を適用した。約2msを超えて誘発活動を誘発しないことが先行して示されている振戦のさらなる標的である。
ERNAが治療上有効なDBSによって変調されたかどうかを調査するために、10人のSTN−PD患者(n=19半球)に60〜90秒のブロックで電流の振幅(範囲0.67〜3.38mA)を徐々に増加させる、スキップ状態のパルス刺激を適用した。一般に、ERNAの2番目以降のピークは、潜時が漸近的に増加し、時間の経過と共にさらに離れるように広がり、刺激の振幅が増加するにつれて、共振活動の周波数の減少と一致することが一貫して観察された(図5、図6、図21aから図21e)ことが分かった。多くの場合、最初のピークの潜時も増加したが、この変化はすべての記録で一貫してはいなかった。ピークの振幅も一般に変化することが観察され、多くの場合、各刺激ブロックの開始時に大きくなり、その後徐々に減少する。
刺激の臨床的有効性は、刺激の直前及び2.25mAで60秒後に、パーキンソン病統一スケール(UPDRS;項目22及び23)に従って四肢の動作緩慢及び固縮を評価することによって確認された。両方の臨床徴候は2.25mAで有意に改善し、DBSが治療するものであることを示している(ウィルコクソン符号順位、動作緩慢:Z=−3.62、p<0.001、固縮:Z=−3.70、p<0.001)。
ERNA変調はDBSに続いて失われる
1. 対象の脳内の刺激に応答する神経活動を監視する方法であって、

3. 前記1つ以上の波形特性は、前記検出された共振応答における2番目以降のサイクルの少なくとも一部に基づいて判定される、条項1または2に記載の方法。
4. 前記1つ以上の波形特性が、以下の
a)前記共振応答の周波数;
b)前記共振応答の時間的包絡線;
c)前記共振応答の振幅;
d)前記共振応答の微細構造;
e)前記共振応答の減衰率;
f)前記刺激の前記開始と前記共振応答の時間的特徴の前記開始との間の遅延;
g)前記共振応答の2つのピーク間の持続期間のうちの1つ以上を含む、条項1から3のいずれか一項に記載の方法。
5. 前記刺激が複数のパルスを含む、先行条項のいずれか一項に記載の方法。
6. 前記1つ以上の波形特性を判定することは、前記検出された共振応答の2つ以上のサイクルにわたって第1の特性を比較することを含む、先行条項のいずれか一項に記載の方法。
7. 前記1つ以上の波形特性を判定するステップが、前記2つ以上のサイクルにわたる前記第1の特性の変化を判定することを含む、条項6に記載の方法。
8. 前記1つ以上の波形特性を判定するステップが、前記2つ以上のサイクルにわたる前記第1の特性の変化率を判定することを含む、条項6または7に記載の方法。
9. 前記共振応答が複数の共振成分を含む、先行条項のいずれか一項に記載の方法。
10. 前記複数の共振成分のうちの1つ以上が、前記標的神経構造とは異なる神経構造に由来する、先行条項のいずれか一項に記載の方法。
11. 前記1つ以上の判定された波形特性に基づいて、前記少なくとも1つの電極のうちの1つ以上の前記位置を調整することをさらに含む、条項8に記載の方法。
12. 前記共振応答の前記1つ以上の判定された波形特性に基づいて、前記刺激を適合させることをさらに含む、先行条項のいずれかに記載の方法。
13. 前記適合させることが、周波数、振幅、パルス幅、正味の電荷、電極の構成、または前記刺激の形態のうちの1つ以上を調整することを含む、条項12に記載の方法。
14. テンプレート共振応答と前記検出された共振応答を相関させること、及び
前記相関に基づいて前記刺激を適合させることをさらに含む、条項12または13に記載の方法。

前記相関に基づいて前記刺激を適合させることをさらに含む、条項12から14のいずれか一項に記載の方法。
16. 前記刺激が非治療的または治療的である、先行条項のいずれか一項に記載の方法。
17. 前記刺激が、第1の期間によって分離された複数のバーストを含むパターン化された信号を含み、各バーストが、第2の期間によって分離された複数のパルスを含み、前記第1の期間が前記第2の期間よりも長く、前記検出することは前記第1の期間のうちの1つ以上の間に実行される、先行条項のいずれか一項に記載の方法。
18. 前記バーストの少なくとも1つの中の前記複数のパルスが異なる振幅を有する、条項17に記載の方法。
19. 前記バーストの前記少なくとも1つの連続パルスの振幅においてランプを生成するよう、異なる前記振幅が選択される、条項18に記載の方法。

24. 前記1つ以上の第1の波形特性及び前記1つ以上の第2の波形特性に基づいて、前記患者に提供される治療の前記効果を判定することをさらに含む、条項23に記載の方法。
25. 前記少なくとも1つの電極が、前記脳の異なる神経構造内に配置された2つ以上の電極を含む、条項23から24のいずれか一項に記載の方法。
26. 前記少なくとも1つの電極が、前記脳の異なる半球の内部に配置された2つ以上の電極を含む、条項25に記載の方法。
27. 前記少なくとも1つの電極の1つ以上が、前記検出された共振応答に基づいて前記標的神経ネットワークに位置しているかどうかを判定することをさらに含む、先行条項のいずれか一項に記載の方法。
28. 前記検出された共振応答に基づいて前記第1の電極及び前記第2の電極のうちの1つ以上を移動させることをさらに含む、条項27に記載の方法。
29. 前記刺激を適用するステップ、共振応答を検出するステップ、及び前記検出された共振応答の1つ以上の波形特性を判定するステップを繰り返すことをさらに含む、先行条項のいずれかに記載の方法。
30. 2つ以上の検出された共振応答間で共通の波形特性を比較することをさらに含む、条項29に記載の方法。
31. 2つ以上の検出された共振応答間で共通の特性の変化の程度を比較することをさらに含む、条項29または30に記載の方法。
32. 2つ以上の検出された共振応答間で共通の特性の変化率を判定することをさらに含む、条項29から31のいずれか一項に記載の方法。
33. 前記刺激を適用するステップ、共振応答を検出するステップ、及び前記検出された共振応答の1つ以上の波形特性を判定するステップが、前記少なくとも1つの電極のうちの1つ以上が前記標的神経構造に配置されていると判定されるまで繰り返される、条項30から32のいずれか一項に記載の方法。
34. 前記1つ以上の波形特性に基づいて、前記標的神経構造の治療的刺激のために使用する前記少なくとも1つの電極の1つ以上を選択すること、及び
前記少なくとも1つの電極の前記選択された1つ以上を介して前記標的神経構造に治療的刺激を適用することをさらに含む、先行条項のいずれか一項に記載の方法。
35. 前記少なくとも1つの電極を事前定義された軌道に沿って前記脳に挿入することをさらに含み、
前記刺激を適用するステップ、共振応答を検出するステップ、及び前記検出された共振応答の1つ以上の波形特性を判定するステップが、前記事前定義された軌道及び前記標的神経構造に関する共振応答のプロファイルを生成するため前記少なくとも1つの電極が挿入されるまで繰り返される、条項29に記載の方法。
36. 共振応答の前記プロファイルを使用して、前記標的神経構造に対する前記1つ以上の電極の位置を判定する、条項35に記載の方法。
37. 前記少なくとも1つの電極が複数の電極を含み、前記刺激を適用するステップ、共振応答を検出するステップ、及び前記検出された共振応答の1つ以上の波形特性を判定するステップが、共振応答のプロファイルを生成するための前記少なくとも1つの電極の異なる組み合わせを使用することを繰り返す、条項29に記載の方法。
38. 神経応答の前記プロファイルに基づいて前記少なくとも1つの電極の1つ以上を選択すること、及び
前記少なくとも1つの電極のうちの前記選択された1つ以上に治療的刺激を適用することをさらに含む、条項36または37に記載の方法。
39. 前記少なくとも1つの電極のうちの前記選択された1つ以上が複数の電極を含む、条項38に記載の方法。

41. 前記神経標的構造が皮質−大脳基底核−視床皮質回路の一部である、先行条項のいずれか一項に記載の方法。
42. 前記神経標的構造が視床下核、淡蒼球内節、黒質網様部、脚橋被蓋核である、先行条項のいずれか一項に記載の方法。
43. 刺激に応答する全身麻酔下の対象の脳の神経活動を監視する方法であって、
a. 前記脳の標的神経構造に前記刺激を適用すること、及び
b. 前記標的神経構造またはその近くに植込まれた電極で前記刺激によって誘発された神経応答を検出することを含み、
前記刺激は、第1の期間によって分離された複数のバーストを含むパターン化された信号を含み、各バーストは第2の期間によって分離された複数のパルスを含み、前記第1の期間が前記第2の期間よりも長く、前記検出することは前記第1の期間のうちの1つ以上の間に実行される、前記方法。
44. 刺激に応答する対象の脳の神経活動を監視する方法であって、
a. 前記患者に全身麻酔を誘起すること、
b. 前記脳の標的神経構造に前記刺激を適用すること、及び
c. 前記標的神経構造またはその近くに植込まれた電極で前記刺激によって誘発された神経応答を検出することを含み、前記刺激は、第1の期間によって分離された複数のバーストを含むパターン化された信号を含み、各バーストは第2の期間によって分離された複数のパルスを含み、前記第1の期間が前記第2の期間よりも長く、前記検出することは前記第1の期間のうちの1つ以上の間に実行される、前記方法。
45. 前記第1の期間が前記第2の期間以上の長さである、条項43または44に記載の方法。
46. 前記刺激が二相性である、条項43から45のいずれか一項に記載の方法。
47. バースト内の前記複数のパルスが異なる振幅を有する、条項43から46のいずれか一項に記載の方法。
48. ランプを生成するために前記異なる振幅が選択される、条項47に記載の方法。
49. 前記複数のバーストのそれぞれにおける最終パルスは、実質的に同一である、条項43から48のいずれか一項に記載の方法。
50. 前記刺激が非治療的または治療的であるようにパターン化されている、条項43から49のいずれか一項に記載の方法。
51. プロポフォール、チオペンタールナトリウム、エトミデート、メトヘキシタール、ケタミン、及びセボフルランから選択される薬剤を使用して、前記対象の全身麻酔が誘起される、先行条項のいずれか一項に記載の方法。
52. 前記対象の全身麻酔が、ソフルラン、セボフルラン、デスフルラン、メトキシフルラン、ハロタン、亜酸化窒素、及びキセノンから選択される薬剤を使用して維持される、先行条項のいずれか一項に記載の方法。
53. 前記対象の全身麻酔が、フェンタニル誘導体、バルビツール酸塩、またはベンゾジアゼピンを使用して維持される、先行条項のいずれか一項に記載の方法。
54. 前記対象の全身麻酔がプロポフォールを使用して誘起され、セボフルランとレミフェンタニルを併用して維持される、先行条項のいずれか一項に記載の方法。
Claims (24)
- 対象の脳内の刺激に応答する神経活動を監視する方法であって、
a. 前記対象に全身麻酔を誘起すること、
b. 前記脳の標的神経構造に植込まれた少なくとも1つの電極の1つ以上に前記刺激を適用すること、
c. 前記脳の前記標的神経構造またはその近くの前記少なくとも1つの電極の1つ以上での前記刺激によって誘発された前記標的神経構造からの共振応答を検出すること、及び
d. 前記検出された共振応答の1つ以上の波形特性を判定することを含む、前記方法。 - 全身麻酔下で対象の脳内の刺激に応答する神経活動を監視する方法であって、
a. 前記脳の標的神経構造に植込まれた少なくとも1つの電極の1つ以上に前記刺激を適用すること、
b. 前記脳の前記標的神経構造またはその近くの前記少なくとも1つの電極の1つ以上での前記刺激によって誘発された前記標的神経構造からの共振応答を検出すること、及び
c. 前記検出された共振応答の1つ以上の波形特性を判定することを含む、前記方法。 - 前記1つ以上の波形特性は、前記検出された共振応答における2番目以降のサイクルの少なくとも一部に基づいて判定される、請求項1または2に記載の方法。
- 前記1つ以上の波形特性が、以下の
a)前記共振応答の周波数;
b)前記共振応答の時間的包絡線;
c)前記共振応答の振幅;
d)前記共振応答の微細構造;
e)前記共振応答の減衰率;
f)前記刺激の開始と前記共振応答の時間的特徴の開始との間の遅延;
g)前記共振応答の2つのピーク間で経過した時間のうちの1つ以上を含む、請求項1から3のいずれか一項に記載の方法。 - 前記共振応答の前記1つ以上の判定された波形特性に基づいて、前記刺激を適合させることをさらに含む、先行請求項のいずれかに記載の方法。
- 前記刺激は、前記共振応答における隣接するピーク間の経過した前記時間に基づいて適合される、請求項5に記載の方法。
- 前記共振応答における2つの隣接するピーク間で経過した前記時間が、患者の運動症状の緩和に関連する第1の所定の期間よりも短い場合に、前記刺激が適合される、請求項6に記載の方法。
- 前記刺激が、第1の期間によって分離された複数のバーストを含むパターン化された信号を含み、各バーストが、第2の期間によって分離された複数のパルスを含み、前記第1の期間が前記第2の期間よりも長く、前記検出することは前記第1の期間のうちの1つ以上の間に実行される、先行請求項のいずれか一項に記載の方法。
- 前記脳の標的神経構造に第2の刺激を適用すること、
前記標的神経構造またはその近くに植込まれた前記少なくとも1つの電極の1つ以上での前記第2の刺激によって誘発された前記標的神経構造からの第2の共振応答を検出すること、
前記検出された第2の共振応答の1つ以上の第2の波形特性を判定することをさらに含む、先行請求項のいずれか一項に記載の方法。 - 前記少なくとも1つの電極の1つ以上が、前記検出された共振応答に基づいて前記標的神経ネットワークに位置しているかどうかを判定することをさらに含む、先行請求項のいずれか一項に記載の方法。
- 前記刺激を適用するステップ、共振応答を検出するステップ、及び前記検出された共振応答の1つ以上の波形特性を判定するステップを繰り返すことをさらに含む、先行請求項のいずれかに記載の方法。
- 前記1つ以上の波形特性に基づいて、前記標的神経構造の治療的刺激のために使用する前記少なくとも1つの電極の1つ以上を選択すること、及び
前記少なくとも1つの電極の前記選択された1つ以上を介して前記標的神経構造に治療的刺激を適用することをさらに含む、先行請求項のいずれか一項に記載の方法。 - 前記少なくとも1つの電極が複数の電極を含み、前記刺激を適用するステップ、共振応答を検出するステップ、及び前記検出された共振応答の1つ以上の波形特性を判定するステップが、共振応答のプロファイルを生成するための前記少なくとも1つの電極の異なる組み合わせを使用することを繰り返す、請求項12に記載の方法。
- 前記刺激を適用するために使用される前記少なくとも1つの電極のうちの前記1つ以上が少なくとも2つの電極を含み、及び/または前記共振応答を検出するために使用される前記少なくとも1つの電極のうちの前記1つ以上は少なくとも2つの電極を含む、先行請求項のいずれか一項に記載の方法。
