Abstract
本发明属于生物医药技术领域,具体涉及一种用于生发的脱细胞基质‑细胞复合颗粒及制备方法。制备方法包括:利用真皮组织样品制备真皮脱细胞基质微载体,培养毛乳头细胞成团,再接种表皮细胞继续培养,制成模拟毛囊结构的脱细胞基质微载体‑细胞复合颗粒。本发明通过体外构建类毛囊前体结构,注射到患者皮下的方式生发,创伤小,恢复快,减少疼痛时间,降低感染的可能;可明显增加头发密度,长出的头发可保持数年;还可以反复应用,保持头发密度恒定,彻底解决脱发患者供区毛发不足的问题。

技术领域
本发明属于生物医药技术领域,具体涉及一种用于生发的脱细胞基质-细胞复合颗粒及制备方法。
背景技术
脱发性疾病是皮肤科中的常见病、多发病,仅我国脱发人口就已经超过2.5亿。调查显示,中国脱发人群的发病年龄为20到40岁之间,30岁左右时病情开展最快。因生活压力,环境污染和饮食结构变化等原因,与上一代人相比,目前的平均脱发年龄提前了近20年,且患病率随年龄增长而明显增加。脱发可根据成因细分为雄激素性脱发,神经性脱发,营养性脱发,化学性脱发,感染性脱发,物理性脱发等。其中90%以上属于雄激素脱发,因此男性脱发人数远远高于女性,男性雄激素脱发的患病率为20.2%,女性患病率为5.1%。主要表现为毛发进行性减少和毛囊的微小化。目前认为雄激素脱发是一种复杂的多基因遗传病。与雄激素受体和双氢睾酮等因素直接相关。另外不健康的生活方式,如高热量饮食、吸烟饮酒以及焦虑、抑郁等不良情绪状态也是雄激素脱发的危险因素,有三分之一左右的雄激素脱发发病与这些因素有关。
目前针对雄激素脱发主要采用的治疗方式有药物治疗、低能量激光,植发等。药物治疗作用原理包括拮抗雄激素、抑制5α-还原酶,增加头皮血供等,常用药非那雄胺,米诺地尔,螺内酯等均存在副作用。其中非那雄胺可特异性地抑制Ⅱ型5α-还原酶,阻碍双氢睾酮生成,降低其对毛发生长的负作用。连续服用非那雄胺,65%的患者头发3-6个月开始显著增多,但停药后一年左右会发生逆转。部分男性患者服用该药后可出现性欲减退、阳痿等症状。米诺地尔则是一种局部外用的血管舒张药,作用机理可能是通过介导钾离子通道开放,增加头皮血流量,使毛囊营养供应充足,从而促进生发。副作用包括皮炎和多毛症,一旦停用就反复,并可能出现短时间内大量掉发的停药反应。女性脱发患者使用螺内酯,拮抗雄激素,效果接近非那雄胺,但肝肾功能不全及血钾偏高者可出现严重不良反应。螺内酯联合米诺地尔治疗半年后,有效率接近60%。其他国产药物多为中成药,主要成分为天然植物来源的拮抗雄激素成分,如5α-还原酶抑制剂和雌激素样活性物质。都是从中医角度来调节机体激素和代谢水平,效果并不理想。药物治疗的不足之处在于需终生服用,患者需要承受一系列不良反应。
低能量激光疗法始于上世纪60年代,又被称为红光疗法、冷激光和软激光,通过光生物调节作用,进行机体损伤修复,现广泛用于健发、伤口愈合、抗菌、抗炎、免疫相关治疗过程。临床数据显示,650~900nm波长和5mw功率的低能量激光可穿透表真皮层,被富含黑色素的毛囊根部吸收,从而起到治疗效果。治疗过程中需要佩戴仪器,如生发头盔,每天佩戴1次,每次20~30min,连续使用3~6月。该疗法能够起到一定的控油,延缓脱发,增加毛发直径的作用,主要作为防脱生发辅助方法应用。
植发是提取脱发患者后枕部的健康毛囊,分离成单个毛囊单位,通过精细的外科技术,把毛囊单位移植到前额,头顶等毛发稀少的部位,让其在新的部位存活、自然生长,提升局部毛发密度,从而达到美观效果的手术。该手术技术已经成熟,效果立竿见影,植发后维持时间可长达数年以上。当前愈发年轻的脱发群体及颜值经济的发展,带动了植发市场井喷式的增长。该方法的不足在于,其本质是‘拆东墙补西墙′,毛囊和毛发总数并未增加,所以没有起到治疗作用,只是对外表进行了修饰。且脱发面积大、供区毛发不足的患者无法通过植发掩盖脱发问题。随着原生发继续脱落,植发后的患者数年后依旧会面临供区不足的问题。
毛囊是皮肤中生长毛发的微小附属器官,功能复杂。人体大约有五百万个毛囊,其中十万左右在头部。毛囊呈周期循环性生长,依次为生长期(Anagen)、退化期(Catagen)和静止期(Talogen)。毛囊下端膨大成毛球部,容纳毛乳头。毛乳头是真皮源性细胞,在毛囊形态发生、生长周期调控及维持毛发生长发育中起主导作用。此外,毛乳头细胞分泌许多生长因子,如干细胞因子(Stem cell factor,SCF)、血管内皮生长因子(Vascular endothelialgrowth factor,VEGF)、成纤维细胞生长因子(Fibroblast growth factor,FGF)、肝细胞生长因子(Hepatocyte growth factor,HGF)、胰岛素样生长因子-1(Insulin-like growthfactor,IGF-1)等,是毛囊中的一个区域性因子库。毛乳头细胞表达SCF、VEGF等因子的能力越强,其诱导毛囊再生的能力就越强。因此,其可以作为制备生发产品的一个新途径。
毛乳头细胞在体外培养有凝集性生长的特性,形态上有多个突起,表达α平滑肌肌动蛋白,合成较多的氨基葡聚糖。研究表明,体外培养的毛乳头细胞仍可诱导毛囊形成,但仅具有凝集性生长形态特征的毛乳头细胞才具有诱导毛囊形成的功能。因此,体外培养的高代毛乳头细胞因失去凝集性生长的特性,不能诱导毛囊再生。另外毛乳头位置特殊,获得困难等缺点,使通过接种毛乳头长出新发的构想难以实现。
发明内容
为了解决上述技术问题,本发明提供了一种用于生发的脱细胞基质-细胞复合颗粒及制备方法。
本发明的目的是提供一种用于生发的脱细胞基质-细胞复合颗粒的制备方法,包括以下步骤:
真皮脱细胞基质微载体的制备:
将真皮组织先用0.5~2mol/L氯化钠溶液浸泡,然后依次用0.1~0.5g/100mL十二烷基磺酸钠溶液和0.15~0.35g/100mL胰蛋白酶处理,脱去细胞,清洗后得到真皮脱细胞基质;冷冻干燥,粉碎,灭菌后得到真皮脱细胞基质微载体;
毛乳头细胞的培养:
取头皮毛囊清洗干净,取毛球部位用0.1~1g/100mL的I型胶原酶溶液在37±1℃消化毛球至毛乳头完全脱出;终止消化,清洗,最后用完全培养基重悬毛乳头,贴壁培养,细胞爬出后传代;
表皮细胞的培养:
将包皮洗净,放入0.6~2.4U/ml分散酶II溶液中分离真、表皮,撕下表皮放入0.15~0.35g/100mL胰酶和0.01~0.03mol/L EDTA的混合消化液中消化;中止消化后,过滤去除没有完全消化的细胞膜片,滤液中的表皮细胞经离心清洗,重悬,最后传代培养表皮细胞;
脱细胞基质微载体-细胞复合颗粒的构建:
将真皮脱细胞基质微载体置于含双抗的完全培养基中预培养36~48h;接种毛乳头细胞,同时加入含双抗的完全培养基,混合培养24~36h后,动态培养7~15d;然后再接种表皮细胞,动态培养5~8d,形成脱细胞基质微载体-细胞复合颗粒。
优选的,上述用于生发的脱细胞基质-细胞复合颗粒的制备方法,所述真皮脱细胞基质由离体的人包皮组织制备,或者由离体的人其他部位真皮组织制备,或者为猪真皮脱细胞基质。
优选的,上述用于生发的脱细胞基质-细胞复合颗粒的制备方法,将离体的包皮组织样品用0.5~2mol/L NaCl溶液浸泡,37℃恒温摇床振荡20~36h;撕去表皮留下真皮组织;将真皮组织样品放入0.1~0.5g/100mL十二烷基磺酸钠溶液内,室温振荡2~3h;0.15~0.35g/100mL胰蛋白酶溶液消化20~30min,脱去细胞,清洗后得到真皮脱细胞基质。
优选的,上述用于生发的脱细胞基质-细胞复合颗粒的制备方法,所述真皮脱细胞基质微载体的粒径为60~120μm。
优选的,上述用于生发的脱细胞基质-细胞复合颗粒的制备方法,毛乳头细胞和表皮细胞的接种密度均为3×105~1×107个/mL,真皮脱细胞基质微载体浓度10~120mg/mL,细胞溶剂和培养液均为含双抗的完全培养基。
优选的,上述用于生发的脱细胞基质-细胞复合颗粒的制备方法,动态培养的条件为:置于3D FloTrixTM mini Spin生物反应器中且置于37±1℃、5%CO2培养箱中动态培养。
本发明还提供了一种由上述方法制备的用于生发的脱细胞基质-细胞复合颗粒。
优选的,上述用于生发的脱细胞基质-细胞复合颗粒,将其溶于1%利多卡因、0.5%普鲁卡因、生理盐水、林格氏液和/或右旋糖酐40葡萄糖注射液中,配制成150~500mg/mL的注射液。上述溶解脱细胞基质-细胞复合颗粒的注射液直接购买市售相应规格的产品。
与现有技术相比,本发明提供的用于生发的脱细胞基质-细胞复合颗粒的制备方法,具有以下有益效果:
本发明采用真皮脱细胞基质制备微载体,为一种多孔性的三维网状颗粒。将体外培养后的毛乳头细胞接种于微载体中,进行动态培养。使毛乳头细胞与微载体充分结合,进入微载体内部,呈现凝集性生长的特性,保持其良好的毛囊诱导能力。然后在外部覆盖一层表皮细胞,模拟毛囊发育过程中表皮-真皮相互作用形成的毛囊前体结构,通过注射器在脱发部位进行皮下注射,诱导新发生成。
本发明通过体外构建脱细胞基质微载体-细胞复合颗粒,模拟毛囊前体结构,移植到患者皮下,促进其进一步发育成为完整毛囊,长出头发,可明显增加头发密度,长出的头发可保持数年。该脱细胞基质微载体-细胞复合颗粒可在体外大量构建,反复注射,保持头发密度恒定,彻底解决脱发患者供区毛发不足的问题。本发明的产品通过皮下注射的方法生发,头皮创伤小,不破坏原有毛囊,显著增加头发密度,不仅可以治疗脱发患者,也可以用于头发相对稀少或者有密发需求的正常人群。不需要长期服用药物,相应的药物副作用也降低。

具体实施方式
为了使本领域技术人员更好地理解本发明的技术方案能予以实施,下面结合具体实施例和附图对本发明作进一步说明。
下述实施例中,如未特殊说明,所用试剂、实验方法均为常规方法;所用培养基均为市售或者本领域常用的培养基配方,以能传代培养细胞为准。
下述实施例中,所用PBS溶液浓度0.01M、pH7.4。所用“双抗”是青链霉素混合液,可以直接添加到细胞培养液或者PBS溶液内,青霉素-链霉素混合溶液100×中,青霉素的含量为10000U/ml,链霉素的含量为10mg/ml;其作用是抑制细菌生长;避免细胞污染。
细胞外基质是由细胞合成后分泌至胞外、分布在细胞表面或之间的大分子,主要成分包括胶原蛋白、糖蛋白、生长因子等蛋白成分,透明质酸、硫酸软骨素等多糖分子及部分脂质。这些活性物质形成复杂的网状结构,在细胞增殖、分化、迁移和血管生成方面有着重要作用。脱细胞基质材料是通过物理、化学和生物的方法进行脱细胞处理、除去组织中的抗原成分后获得的材料。因为除去了天然材料中的细胞成分,有效降低了天然材料的免疫原性,同时能够保有基质材料完整的三维超微结构,以及多种生物活性成分。脱细胞基质可在体内形成组织修复支架,充当细胞外基质,提供有利于细胞生长分化的微环境,主动诱导和促进周围细胞的迁入、黏附、增殖和分化,促进血管生成。迁入的细胞进一步对材料进行改造、降解和塑形,实现组织的形成和结构重塑。
因具有凝集性生长特征的毛乳头细胞都有诱导毛囊形成的功能特性,甚至第26代凝聚成球状的毛乳头细胞团都能诱导毛囊形成。想要通过种植毛乳头细胞诱导毛囊形成,关键在于保持毛乳头细胞凝集性生长的特性。因此本发明通过体外构建脱细胞基质微载体-细胞复合颗粒,模拟毛囊前体结构,开发了一种生发的脱细胞基质-细胞复合颗粒的制备方法,包括以下实施例。
实施例1
一种用于生发的脱细胞基质-细胞复合颗粒的制备方法,包括以下步骤:
1.真皮脱细胞基质微载体的制备
将离体的包皮组织用生理盐水冲洗干净,1mol/L NaCl溶液浸泡,37℃恒温摇床200r/min振荡24h;撕去表皮留下真皮组织,真皮组织放入0.5g/100mL十二烷基磺酸钠(sodium dodecyl sulfonate,SDS)溶液内,室温振荡2h,0.25g/100mL胰蛋白酶溶液消化20min脱去细胞,纯化水进行震荡清洗,充分洗去残留试剂,得到真皮脱细胞基质。然后冷冻干燥6h,用低温粉碎机将脱细胞基质粉碎成粒径为60~120μm的微粒。钴60辐射灭菌,辐射剂量为25kGy,得到真皮脱细胞基质微载体,外观参见图1,4℃条件下低温保存。取少量样本在扫描电子显微镜下观察。结果参见图2。电镜下观察,本发明所制备的真皮脱细胞基质微载体为一种多孔性的三维网状结构。
优选的,本步骤取健康青少年包皮环切术所得新鲜包皮组织制备的真皮脱细胞基质微载体效果更佳。这些包皮组织常作为医疗废弃物丢弃,本发明利用了医疗废弃物,实现了资源的再利用。优选的,所述真皮脱细胞基质也可以由离体的人其他部位真皮组织制备,或者为猪真皮脱细胞基质。
2.毛乳头细胞的培养
取头皮毛囊,用含1×双抗的PBS溶液清洗干净,转移至超净台中操作。用显微剪剪下毛球后移至新的培养皿中。收集所有切下的毛球,用PBS溶液洗三次。用毛球组织2倍体积的0.2g/100mLI型胶原酶溶液,在37℃细胞培养箱中消化毛球。1.5h后在显微镜下可以看到部分毛乳头从真皮鞘中分离脱出,这时可用移液枪吹打并继续消化至毛乳头完全脱出。终止消化,1000转、10min离心弃上清,用含双抗的PBS溶液重悬,离心弃上清,重复两次,最后用3mL完全培养基重悬毛乳头,移入25cm2细胞培养瓶中培养,3天后毛乳头完全贴壁,每隔两天换液。培养一周后毛乳头细胞迁出较多,长满培养瓶的70%左右时传代培养。图3为人头皮毛囊在显微镜下的状态,图4为贴壁培养5d的毛乳头中长出的毛乳头细胞。
优选的,本步骤取毛发移植术后废弃的人头皮毛囊,也可来源于患者自身毛囊组织。
3.表皮细胞的培养
将包皮先用体积分数75%的酒精漂洗10s,晾干,再依次分别用含有4×、2×双抗的PBS溶液冲洗3遍,然后用眼科剪和眼科镊仔细地去除脂肪和系膜,之后放入分散酶II(dispaseⅡ)溶液中,分散酶II工作浓度为0.6-2.4U/ml,确保组织块全部浸没于Dispase溶液中,4℃作用18~24h分离真表皮。作用完后用镊子小心撕下表皮,用PBS溶液冲洗后放入终浓度0.25g/100mL胰酶和终浓度0.01mol/L EDTA的混合消化液中,37℃消化30min。然后加入含有体积分数10%胎牛血清的培养液中止消化,吸管反复吹打分散细胞,再用100目的筛网过滤去除没有完全消化的细胞膜片,接着2000r min离心5min,洗涤两次,重悬,计数,按2×106个/75cm2浓度接种,用K-SFM无血清培养基进行培养,接种3d后长满,显微镜下观察拍照,培养的表皮细胞参见图5。
优选的,本步骤将患者包皮环切术所得自身新鲜包皮组织保存在含双抗的PBS缓冲液中带回实验室,4h内进行处理。
4.脱细胞基质微载体-细胞复合颗粒的构建
将真皮脱细胞基质微载体置于含1×双抗的完全培养基中,37℃、5%CO2培养箱中预培养48h。将体外培养后的P5代毛乳头细胞8×107个接种于4g微载体中,同时加入40mL含1×双抗的完全培养基,混合培养24h后,转移到125mL生物反应器瓶中,将生物反应器瓶放在3D FloTrixTM mini Spin生物反应器底座上,置于37℃、5%CO2培养箱中动态培养7~15d,得到毛乳头复合微载体;毛乳头细胞接种于真皮脱细胞基质微载体,动态培养5d内,每天取少量微载体,统计微载体上贴壁细胞的数量,计算平均值。图6为脱细胞基质微载体上贴壁细胞数目的统计图,可见随着培养时间增长,微载体上的贴壁细胞数目逐渐增多。毛乳头细胞附着于微载体后大量增殖,深入微载体内部,并分泌细胞外基质。
将体外培养后的P5代表皮细胞8×107个接种于4g毛乳头复合微载体中,同时加入40mL含双抗的完全培养基,混合培养24h后,转移到125mL生物反应器瓶中,将生物反应器瓶放在3D FloTrixTM mini Spin生物反应器底座上,置于37℃、5%CO2培养箱中动态培养5~8d。形成脱细胞基质微载体-细胞复合颗粒。
5.生发效果检验
将体外培养3周左右的脱细胞基质微载体-细胞复合颗粒取出,PBS溶液清洗3次,用生理盐水配置成300mg/ml的悬液,灌装至1m L注射器中。注射到裸鼠皮下,每个接种部位注射0.1ml。间隔一段时间后切下移植部位,进行组织学检查,可见毛囊样结构形成。裸鼠皮下接种脱细胞基质微载体-细胞复合颗粒示意图见图7。
注射后6周,取下裸鼠背部接种脱细胞基质微载体-细胞复合颗粒的皮肤,做病理切片,图8A为横向切面,图8B为纵向切面,均可见类毛囊结构出现。
需要说明的是,本发明中涉及数值范围时,应理解为每个数值范围的两个端点以及两个端点之间任何一个数值均可选用,由于采用的步骤方法与实施例相同,为了防止赘述,本发明描述了优选的实施例。尽管已描述了本发明的优选实施例,但本领域内的技术人员一旦得知了基本创造性概念,则可对这些实施例做出另外的变更和修改。所以,所附权利要求意欲解释为包括优选实施例以及落入本发明范围的所有变更和修改。
显然,本领域的技术人员可以对本发明进行各种改动和变型而不脱离本发明的精神和范围。这样,倘若本发明的这些修改和变型属于本发明权利要求及其等同技术的范围之内,则本发明也意图包含这些改动和变型在内。

1.一种用于生发的脱细胞基质-细胞复合颗粒的制备方法,其特征在于,包括以下步骤:
真皮脱细胞基质微载体的制备:
将真皮组织先用0.5 ~ 2 mol/L氯化钠溶液浸泡,然后依次用0.1 ~ 0.5 g/100mL十二烷基磺酸钠溶液和0.15 ~ 0.35 g/100mL胰蛋白酶处理,脱去细胞,清洗后得到真皮脱细胞基质;冷冻干燥,粉碎,灭菌后得到真皮脱细胞基质微载体;
毛乳头细胞的培养:
取头皮毛囊清洗干净,取毛球部位用0.1 ~ 1g/100mL的I型胶原酶溶液在37±1 ℃消化毛球至毛乳头完全脱出;终止消化,清洗,最后用完全培养基重悬毛乳头,贴壁培养,细胞爬出后传代;毛乳头细胞传代培养至P10代后备用;
表皮细胞的培养:
将包皮洗净,放入0.6 ~ 2.4 U/ml分散酶II溶液中分离真、表皮,撕下表皮,放入0.15~ 0.35 g/100mL胰酶和0.01 ~ 0.03 mol/L EDTA 的混合消化液中消化;中止消化后,过滤去除没有完全消化的细胞膜片,滤液中的表皮细胞经离心清洗,重悬,最后传代培养表皮细胞;用 K-SFM 无血清培养基传代培养表皮细胞,收集P4 ~ P10代的表皮细胞备用;
脱细胞基质微载体-细胞复合颗粒的构建:
将真皮脱细胞基质微载体置于含双抗的完全培养基中预培养36 ~ 48 h;接种毛乳头细胞,同时加入含双抗的完全培养基,混合培养24 ~ 36 h 后,动态培养7 ~ 15d;然后再接种表皮细胞,动态培养5 ~ 8d,形成脱细胞基质微载体-细胞复合颗粒;
将脱细胞基质微载体-细胞复合颗粒溶于1%利多卡因、0.5%普鲁卡因、生理盐水、林格氏液和/或右旋糖酐40葡萄糖注射液中,配制成150 ~ 500 mg/mL的注射液。
2.根据权利要求1所述的用于生发的脱细胞基质-细胞复合颗粒的制备方法,其特征在于,所述真皮脱细胞基质由离体的人包皮组织制备,或者由离体的人其他部位真皮组织制备,或者为猪真皮脱细胞基质。
3.根据权利要求2所述的用于生发的脱细胞基质-细胞复合颗粒的制备方法,其特征在于,将离体的包皮组织样品用0.5 ~ 2 mol/L NaCl溶液浸泡,37 ℃恒温摇床振荡20 ~ 36h;撕去表皮留下真皮组织;将真皮组织样品放入0.1 ~ 0.5 g/100mL十二烷基磺酸钠溶液内,室温振荡2 ~ 3 h;0.15 ~ 0.35 g/100mL胰蛋白酶溶液消化20~30min,脱去细胞,清洗后得到真皮脱细胞基质。
4.根据权利要求1所述的用于生发的脱细胞基质-细胞复合颗粒的制备方法,其特征在于,所述真皮脱细胞基质微载体的粒径为60 ~ 120 μm。
5.根据权利要求1所述的用于生发的脱细胞基质-细胞复合颗粒的制备方法,其特征在于,毛乳头细胞和表皮细胞的接种密度均为3×10 5 ~1×10 7 个/mL,真皮脱细胞基质微载体浓度10 ~ 120 mg/mL。
6.根据权利要求5所述的用于生发的脱细胞基质-细胞复合颗粒的制备方法,其特征在于,动态培养的条件为:置于3D FloTrixTM mini Spin 生物反应器中且置于 37±1 ℃、5%CO2培养箱中动态培养。
7.根据权利要求1 ~ 6任一项所述方法制成的用于生发的脱细胞基质-细胞复合颗粒。
