技术领域
本发明属于生物技术和医学领域,具体地说,本发明涉及FABP5作为新型生物标志物用于诊断动脉粥样硬化。
背景技术
动脉粥样硬化(atherosclerosis,AS)是一种以胆固醇等脂质代谢障碍为病变基础的慢性复杂疾病。《中国心血管病报告2017》指出,我国心血管病死亡占城乡居民总死亡原因首位,而AS正是引发各类心血管疾病的主要原因。AS的特点表现为受累动脉病变从内膜开始,一般先有脂质和复合糖类积聚、出血及血栓形成,进而纤维组织增生及钙质沉着,并有动脉中层的逐渐蜕变和钙化,最终导致动脉壁增厚变硬、血管腔狭窄。AS的病因及发病机制多样,主要包括内皮损伤、脂质代谢异常、血流动力学损伤、遗传、理化损伤等。尽管AS发病机制复杂,但大量研究表明血脂代谢异常,尤其是高胆固醇血症是促进AS发病的重要危险因素。血浆中过多的胆固醇沉积于动脉内皮下,促进大量泡沫细胞聚集及动脉粥样斑块病变形成,导致动脉管腔狭窄或斑块破裂后血栓栓塞动脉管腔以致心肌缺血坏死。因此,寻找通过调控胆固醇代谢从而防治AS发生发展的新靶点具有重要意义。
氧化型低密度脂蛋白(oxidized low density lipoprotein,Ox-LDL)是低密度脂蛋白(low density lipoprotein,LDL)的氧化产物,大量流行病学及实验研究发现AS的发生发展与LDL的氧化修饰有重要关联,动脉壁中的Ox-LDL是造成动脉壁持续性损伤最重要的因素之一。当LDL受到氧化修饰为Ox-LDL后,无法与低密度脂蛋白受体(low densitylipoprotein receptor,LDLR)结合,进入正常代谢途径,而是被细胞表面清道夫受体识别。清道夫受体不受细胞内脂质浓度的负反馈调节,因而大量Ox-LDL进入细胞内,致使胆固醇外流受阻,细胞内大量胆固醇堆积,泡沫细胞形成。在Ox-LDL形成过程中产生的许多中间代谢产物如过氧化脂质、丙二醛、4-羟壬烯醛等具有细胞毒作用,可诱导巨噬细胞成为巨噬细胞源性泡沫细胞。同时,Ox-LDL也可诱导平滑肌细胞表面A1类清道夫受体(Class A1scavenger receptor,SR-AI)的表达,导致平滑肌细胞摄取Ox-LDL,继而产生平滑肌源性泡沫细胞。泡沫细胞不断堆积形成脂质条纹乃至脂质斑块,最终促进AS进程。因此,Ox-LDL通过诱导细胞内胆固醇代谢障碍、泡沫细胞形成进而影响AS发生发展,是AS最为重要的致病因素之一。
因此,本领域技术人员致力于深层次的研究动脉粥样硬化发病机制,以期获得更有效地诊断或治疗动脉粥样硬化的临床应用方案。
发明概述
本发明的目的是提供一种新的动脉粥样硬化生物标志物FABP5蛋白以及其片段、类似物和衍生物,在诊断和/或治疗动脉粥样硬化中的用途。
本发明第一方面提供了一种FABP5蛋白、其编码基因或其检测试剂的用途,用于制备诊断试剂或试剂盒,所述检测试剂或试剂盒用于诊断或辅助诊断动脉粥样硬化。
在另一优选例中,所述检测试剂选自下组:抗体、引物、探针、测序文库、核酸芯片(如DNA芯片)、蛋白质芯片、或其组合。
在另一优选例中,所述的FABP5蛋白包括FABP5全长蛋白或FABP5蛋白片段。
在另一优选例中,所述的FABP5蛋白、其编码基因来源于哺乳动物,更佳地来源于灵长动物和人。
在另一优选例中,所述诊断为组织样品检测或血清检测。
在另一优选例中,所述的FABP5蛋白偶联有或带有可检测标记。
在另一优选例中,所述可检测标记选自下组:生色团、化学发光基团、荧光团、同位素或酶。
在另一优选例中,所述FABP5蛋白还包括FABP5蛋白的衍生物。
在另一优选例中,所述FABP5蛋白的衍生物包括经修饰的FABP5蛋白、氨基酸序列与天然FABP5蛋白同源且具有天然FABP5蛋白活性的蛋白分子、FABP5蛋白的二聚体或多聚体、含有FABP5蛋白氨基酸序列的融合蛋白。
在另一优选例中,所述“氨基酸序列与天然FABP5蛋白同源且具有天然FABP5蛋白活性的蛋白分子”是指其氨基酸序列与FABP5蛋白相比具有≥85%的同源性,较佳地≥90%的同源性,更佳地≥95%的同源性,最佳地≥98%同源性;并且具有天然FABP5蛋白活性的蛋白分子。
本发明第二方面提供了一种FABP5或FABP5拮抗剂的用途,用于制备药物或组合物,所述药物或组合物用于选自下组的一种或多种用途:
(1)抑制ABCG1蛋白水平的降低;
(2)提高ABCG1蛋白水平;和
(3)预防或治疗体内ABCG1蛋白水平降低所引起的疾病。
在另一优选例中,所述FABP5拮抗剂选自下组:
(a)降低或抑制FABP5的活性的物质;
(b)降低FABP5的表达或稳定性的物质。
另一优选例中,所述FABP5拮抗剂选自:抗体、多肽、sh-RNA、dsRNA、miRNA、siRNA、反义寡核苷酸、化合物或其组合。
在另一优选例中,所述FABP5拮抗剂选自:抗FABP5抗体,针对FABP5的反义寡核苷酸、siRNA或dsRNA,FABP5的化学抑制剂(化合物)。
在另一优选例中,所述“ABCG1蛋白水平降低所引起的疾病”选自:动脉粥样硬化、高胆固醇血症、高甘油三酯血症、心肌梗死、缺血性脑卒中和冠状动脉粥样硬化性心脏病。
本发明第三方面提供了一种检测动脉粥样硬化的方法,包括:
a)准备受试者测试样品;和
b)检测测试样品中FABP5蛋白、或其编码基因的水平,并将检测结果与参比值进行比较,其中FABP5蛋白、或其编码基因的水平显著高于参比值,表明受试者患有动脉粥样硬化,或患动脉粥样硬化的几率高于正常人群。
在另一优选例中,所述的“显著高于”指测试样品的FABP5蛋白、或其编码基因的水平E1与参比值E0之比≥2,较佳地≥4,更佳地≥6。
在另一优选例中,所述测试样品为血清样品。
在另一优选例中,所述参比值为正常血清中FABP5蛋白、或其编码基因的水平。
在另一优选例中,所述检测步骤(b)包括通过RT-qPCR方法进行检测。
在另一优选例中,所述的检测步骤(b)包括使用抗FABP5蛋白的抗体进行检测。
在另一优选例中,检测步骤(b)通过免疫组化或酶联免疫反应法(ELISA法)实现。
在另一优选例中,所述抗FABP5蛋白的抗体是单克隆抗体或多克隆抗体(如抗血清)。
在另一优选例中,所述方法为非治疗性和非诊断性的。
本发明第四方面提供了一种诊断试剂盒,所述的试剂盒包括:
(a)FABP5蛋白、或其编码基因;和/或
(b)抗FABP5蛋白抗体,或者,特异性扩增FABP5蛋白编码基因的引物或引物对;
以及标签或说明书;
其中,所述的组分(a)、(b)分别位于一个或多个不同的容器或位于同一容器中。
在另一优选例中,所述组分(a)可作为对照品或参照品。
在另一优选例中,所述标签或说明书注明所述试剂盒用于:检测或诊断动脉粥样硬化。
本发明第五方面提供了一种筛选或确定预防和/或治疗动脉粥样硬化的潜在治疗剂的方法,包括步骤:
(a)在测试组中,在培养体系中,在测试化合物的存在下,培养表达FABP5蛋白的细胞一段时间T1,检测测试组所述培养体系中的FABP5蛋白、或其编码基因的水平L1;
并且在不存在所述测试化合物且其他条件相同的对照组中,检测对照组所述培养体系中的FABP5蛋白、或其编码基因的水平L2;和
(b)将上一步骤所检测的L1、L2进行比较,从而确定所述测试化合物是否是预防和/或治疗贫血或其相关疾病的潜在治疗剂;
其中,如果L1显著低于L2,则表示所述测试化合物是预防和/或治疗动脉粥样硬化的潜在治疗剂。
在另一优选例中,所述“显著低于”指L2/L1≥2,较佳地,≥3,更佳地,≥4。
在另一优选例中,所述的方法是非诊断和非治疗性的。
在另一优选例中,所述表达FABP5蛋白的细胞为平滑肌细胞。

发明详述
本发明经过广泛而深入的研究,发现并分离出了新的动脉粥样硬化诊断标志物,可用于动脉粥样硬化诊断或辅助性诊断或和/或治疗。具体地,本发明人发现Ox-LDL通过上调HASMC中FABP5的表达从而抑制细胞内ABCG1的表达,进而导致细胞内胆固醇流出障碍,促进AS的发生发展。在此基础上完成了本发明。
在本发明中,术语“FABP5”“FABP5蛋白”、“FABP5多肽”或“动脉粥样硬化标志物FABP5”可互换使用,都指具有人FABP5蛋白氨基酸序列的蛋白或多肽。这些蛋白或多肽可以是分离的。
脂肪酸结合蛋白-5(fatty acid binding protein 5,FABP5)又名E-FABP、PA-FABP,属于脂肪酸结合蛋白家族(FABPs)一亚类,分子量为15kD,由FABP5基因编码。FABP5基因定位于8q21.13,共含4个外显子,全长3911bp。FABP5最初被发现于角质形成细胞中,后在脂肪、脑、肝脏、肾脏、乳腺等组织中被相继检测出。FABP5作为脂质运载体对硬脂酸和亚油酸具有高度亲和力并能特异性结合脂肪酸,通过增加脂肪酸的水溶性或通过与磷脂双分子层的的相互作用及水溶介导过程加强脂肪酸转运至膜受体,从而加快脂肪酸的摄取,影响脂质代谢过程。此外,FABP5可以结合和转运各种配体,参与肿瘤生长相关的信号转导,FABP5还在神经损伤修复中起着重要作用,是创面愈合的重要分子,其缺乏将抑制角质形成细胞的迁移,影响创面愈合。尽管FABP5被认为与皮肤病、呼吸系统疾病及肿瘤的发生发展有关,但在AS方面的研究相对较少,在本发明之前FABP5在AS中的作用及具体生理、病理机制仍不清楚。
在本发明一个优选地实施方式中,FABP5蛋白的氨基酸序列如NP_001435.1所示:

如本文所用,“分离的”是指物质从其原始环境中分离出来(如果是天然的物质,原始环境即是天然环境)。如活体细胞内的天然状态下的多核苷酸和多肽是没有分离纯化的,但同样的多核苷酸或多肽如从天然状态中同存在的其他物质中分开,则为分离纯化的。
如本文所用,“分离的FABP5蛋白或多肽”是指FABP5多肽基本上不含天然与其相关的其它蛋白、脂类、糖类或其它物质。本领域的技术人员能用标准的蛋白质纯化技术纯化FABP5蛋白。基本上纯的多肽在非还原聚丙烯酰胺凝胶上能产生单一的主带。
本发明的多肽可以是重组多肽、天然多肽、合成多肽,优选重组多肽。本发明的多肽可以是天然纯化的产物,或是化学合成的产物,或使用重组技术从原核或真核宿主(例如,细菌、酵母、高等植物、昆虫和哺乳动物细胞)中产生。根据重组生产方案所用的宿主,本发明的多肽可以是糖基化的,或可以是非糖基化的。本发明的多肽还可包括或不包括起始的甲硫氨酸残基。
本发明还包括人FABP5蛋白的片段、衍生物和类似物。如本文所用,术语“片段”、“衍生物”和“类似物”是指基本上保持本发明的天然人FABP5蛋白相同的生物学功能或活性的多肽。本发明的多肽片段、衍生物或类似物可以是(i)有一个或多个保守或非保守性氨基酸残基(优选保守性氨基酸残基)被取代的多肽,而这样的取代的氨基酸残基可以是也可以不是由遗传密码编码的,或(ii)在一个或多个氨基酸残基中具有取代基团的多肽,或(iii)成熟多肽与另一个化合物(比如延长多肽半衰期的化合物,例如聚乙二醇)融合所形成的多肽,或(iv)附加的氨基酸序列融合到此多肽序列而形成的多肽(如前导序列或分泌序列或用来纯化此多肽的序列或蛋白原序列,或与抗原IgG片段的形成的融合蛋白)。根据本文的教导,这些片段、衍生物和类似物属于本领域熟练技术人员公知的范围。
在本发明中,术语“FABP5多肽”指具有人FABP5蛋白活性的SEQ ID NO.1序列的全长多肽或成熟多肽。该术语还包括具有与人FABP5蛋白相同功能的、SEQ ID NO.1序列的变异形式。这些变异形式包括(但并不限于):若干个(通常为1-50个,较佳地1-30个,更佳地1-20个,最佳地1-10个)氨基酸的缺失、插入和/或取代,以及在C末端和/或N末端添加一个或数个(通常为20个以内,较佳地为10个以内,更佳地为5个以内)氨基酸。例如,在本领域中,用性能相近或相似的氨基酸进行取代时,通常不会改变蛋白质的功能。又比如,在C末端和/或N末端添加一个或数个氨基酸通常也不会改变蛋白质的功能。该术语还包括人FABP5蛋白的活性片段和活性衍生物。
该多肽的变异形式包括:同源序列、保守性变异体、等位变异体、天然突变体、诱导突变体、在高或低的严紧度条件下能与人FABP5 DNA杂交的DNA所编码的蛋白、以及利用抗人FABP5多肽的抗血清获得的多肽或蛋白。本发明还提供了其他多肽,如包含人FABP5多肽或其片段的融合蛋白。除了几乎全长的多肽外,本发明还包括了人FABP5多肽的可溶性片段。通常,该片段具有人FABP5多肽序列的至少约10个连续氨基酸,通常至少约30个连续氨基酸,较佳地至少约50个连续氨基酸,更佳地至少约80个连续氨基酸,最佳地至少约100个连续氨基酸。
发明还提供人FABP5蛋白或多肽的类似物。这些类似物与天然人FABP5多肽的差别可以是氨基酸序列上的差异,也可以是不影响序列的修饰形式上的差异,或者兼而有之。这些多肽包括天然或诱导的遗传变异体。诱导变异体可以通过各种技术得到,如通过辐射或暴露于诱变剂而产生随机诱变,还可通过定点诱变法或其他已知分子生物学的技术。类似物还包括具有不同于天然L-氨基酸的残基(如D-氨基酸)的类似物,以及具有非天然存在的或合成的氨基酸类似物。应理解,本发明的多肽并不限于上述例举的代表性的多肽。
修饰(通常不改变一级结构)形式包括:体内或体外的多肽的化学衍生形式如乙酰化或羧基化。修饰还包括糖基化,如那些在多肽的合成和加工中或进一步加工步骤中进行糖基化修饰而产生的多肽。这种修饰可以通过将多肽暴露于进行糖基化的酶(如哺乳动物的糖基化酶或去糖基化酶)而完成。修饰形式还包括具有磷酸化氨基酸残基(如磷酸酪氨酸,磷酸丝氨酸,磷酸苏氨酸)的序列。还包括被修饰从而提高了其抗蛋白水解性能或优化了溶解性能的多肽。
在本发明中,“人FABP5蛋白保守性变异多肽”指与SEQ ID NO:1的氨基酸序列相比,有至多10个,较佳地至多8个,更佳地至多5个,最佳地至多3个氨基酸被性质相似或相近的氨基酸所替换而形成多肽。这些保守性变异多肽最好根据表I进行氨基酸替换而产生。

本发明的多核苷酸可以是DNA形式或RNA形式。DNA形式包括cDNA、基因组DNA或人工合成的DNA。DNA可以是单链的或是双链的。DNA可以是编码链或非编码链。
编码FABP5的成熟多肽的多核苷酸包括:只编码成熟多肽的编码序列;成熟多肽的编码序列和各种附加编码序列;成熟多肽的编码序列(和任选的附加编码序列)以及非编码序列。术语“编码多肽的多核苷酸”可以是包括编码此多肽的多核苷酸,也可以是还包括附加编码和/或非编码序列的多核苷酸。
本发明还涉及上述多核苷酸的变异体,其编码与本发明有相同的氨基酸序列的多肽或多肽的片段、类似物和衍生物。此多核苷酸的变异体可以是天然发生的等位变异体或非天然发生的变异体。这些核苷酸变异体包括取代变异体、缺失变异体和插入变异体。如本领域所知的,等位变异体是一个多核苷酸的替换形式,它可能是一个或多个核苷酸的取代、缺失或插入,但不会从实质上改变其编码多肽的功能。
本发明还涉及与上述的序列杂交且两个序列之间具有至少50%,较佳地至少70%,更佳地至少80%,最佳地至少90%或95%相同性的多核苷酸。本发明特别涉及在严格条件下与本发明所述多核苷酸可杂交的多核苷酸。在本发明中,“严格条件”是指:(1)在较低离子强度和较高温度下的杂交和洗脱,如0.2×SSC,0.1%SDS,60℃;或(2)杂交时加有变性剂,如50%(v/v)甲酰胺,0.1%小牛血清/0.1%Ficoll,42℃等;或(3)仅在两条序列之间的相同性至少在90%以上,更好是95%以上时才发生杂交。并且,可杂交的多核苷酸编码的多肽与SEQ ID NO:2或4所示的成熟多肽有相同的生物学功能和活性。
本发明还涉及与上述的序列杂交的核酸片段。如本文所用,“核酸片段”的长度至少含15个核苷酸,较好是至少30个核苷酸,更好是至少50个核苷酸,最好是至少100个核苷酸以上。核酸片段可用于核酸的扩增技术(如PCR)以确定和/或分离编码FABP5蛋白的多核苷酸。
本发明的人FABP5核苷酸全长序列或其片段通常可以用PCR扩增法、重组法或人工合成的方法获得。对于PCR扩增法,可根据本发明所公开的有关核苷酸序列,尤其是开放阅读框序列来设计引物,并用市售的cDNA库或按本领域技术人员已知的常规方法所制备的cDNA库作为模板,扩增而得有关序列。当序列较长时,常常需要进行两次或多次PCR扩增,然后再将各次扩增出的片段按正确次序拼接在一起。
一旦获得了有关的序列,就可以用重组法来大批量地获得有关序列。这通常是将其克隆入载体,再转入细胞,然后通过常规方法从增殖后的宿主细胞中分离得到有关序列。
此外,还可用人工合成的方法来合成有关序列,尤其是片段长度较短时。通常,通过先合成多个小片段,然后再进行连接可获得序列很长的片段。
目前,已经可以完全通过化学合成来得到编码本发明蛋白(或其片段,衍生物)的DNA序列。然后可将该DNA序列引入本领域中已知的各种现有的DNA分子(或如载体)和细胞中。此外,还可通过化学合成将突变引入本发明蛋白序列中。
应用PCR技术扩增DNA/RNA的方法被优选用于获得本发明的基因。用于PCR的引物可根据本文所公开的本发明的序列信息适当地选择,并可用常规方法合成。可用常规方法如通过凝胶电泳分离和纯化扩增的DNA/RNA片段。
本发明也涉及包含本发明的多核苷酸的载体,以及用本发明的载体或FABP5蛋白编码序列经基因工程产生的宿主细胞,以及经重组技术产生本发明所述多肽的方法。
通过常规的重组DNA技术,可利用本发明的多核苷酸序列可用来表达或生产重组的FABP5多肽。一般来说有以下步骤:
(1).用本发明的编码人FABP5多肽的多核苷酸(或变异体),或用含有该多核苷酸的重组表达载体转化或转导合适的宿主细胞;
(2).在合适的培养基中培养的宿主细胞;
(3).从培养基或细胞中分离、纯化蛋白质。
本发明中,人FABP5多核苷酸序列可插入到重组表达载体中。总之,只要能在宿主体内复制和稳定,任何质粒和载体都可以用。表达载体的一个重要特征是通常含有复制起点、启动子、标记基因和翻译控制元件。
本领域的技术人员熟知的方法能用于构建含人FABP5编码DNA序列和合适的转录/翻译控制信号的表达载体。这些方法包括体外重组DNA技术、DNA合成技术、体内重组技术等。所述的DNA序列可有效连接到表达载体中的适当启动子上,以指导mRNA合成。表达载体还包括翻译起始用的核糖体结合位点和转录终止子。
此外,表达载体优选地包含一个或多个选择性标记基因,以提供用于选择转化的宿主细胞的表型性状,如真核细胞培养用的二氢叶酸还原酶、新霉素抗性以及绿色荧光蛋白(GFP),或用于大肠杆菌的四环素或氨苄青霉素抗性。
包含上述的适当DNA序列以及适当启动子或者控制序列的载体,可以用于转化适当的宿主细胞,以使其能够表达蛋白质。
宿主细胞可以是原核细胞,如细菌细胞;或是低等真核细胞,如酵母细胞;或是高等真核细胞,如哺乳动物细胞。代表性例子有:大肠杆菌,链霉菌属的细菌细胞;真菌细胞如酵母;植物细胞;果蝇S2或Sf9的昆虫细胞;CHO、COS、或293细胞的动物细胞等。
用重组DNA转化宿主细胞可用本领域技术人员熟知的常规技术进行。当宿主为原核生物如大肠杆菌时,能吸收DNA的感受态细胞可在指数生长期后收获,用CaCl2法处理,所用的步骤在本领域众所周知。另一种方法是使用MgCl2。如果需要,转化也可用电穿孔的方法进行。当宿主是真核生物,可选用如下的DNA转染方法:磷酸钙共沉淀法,常规机械方法如显微注射、电穿孔、脂质体包装等。
获得的转化子可以用常规方法培养,表达本发明的基因所编码的多肽。根据所用的宿主细胞,培养中所用的培养基可选自各种常规培养基。在适于宿主细胞生长的条件下进行培养。当宿主细胞生长到适当的细胞密度后,用合适的方法(如温度转换或化学诱导)诱导选择的启动子,将细胞再培养一段时间。
在上面的方法中的重组多肽可在细胞内、或在细胞膜上表达、或分泌到细胞外。如果需要,可利用其物理的、化学的和其它特性通过各种分离方法分离和纯化重组的蛋白。这些方法是本领域技术人员所熟知的。这些方法的例子包括但并不限于:常规的复性处理、用蛋白沉淀剂处理(盐析方法)、离心、渗透破菌、超处理、超离心、分子筛层析(凝胶过滤)、吸附层析、离子交换层析、高效液相层析(HPLC)和其它各种液相层析技术及这些方法的结合。
重组的人FABP5蛋白或多肽有多方面的用途。这些用途包括(但不限于):用于筛选对抗FABP5蛋白功能的抗体、多肽或其它物质。
另一方面,本发明还包括对人FABP5 DNA或是其片段编码的多肽具有特异性的多克隆抗体和单克隆抗体,尤其是单克隆抗体。这里,“特异性”是指抗体能结合于人FABP5基因产物或片段。较佳地,指那些能与人FABP5基因产物或片段结合但不识别和结合于其它非相关抗原分子的抗体。本发明的抗体可以通过本领域内技术人员已知的各种技术进行制备。
本发明不仅包括完整的单克隆或多克隆抗体,而且还包括具有免疫活性的抗体片段,如Fab'或(Fab)2片段;抗体重链;抗体轻链;遗传工程改造的单链Fv分子;或嵌合抗体。
抗人FABP5蛋白的抗体可用于免疫组织化学技术中,检测活检标本中的人FABP5蛋白。
利用本发明蛋白,通过各种常规筛选方法,可筛选出与FABP5蛋白发生相互作用的物质,如受体、抑制剂、激动剂或拮抗剂等。
本发明蛋白的抗体、抑制剂、激动剂、拮抗剂或受体等,当在治疗上进行施用(给药)时,可提供不同的效果。通常,可将这些物质配制于无毒的、惰性的和药学上可接受的水性载体介质中,其中pH通常约为5-8,较佳地pH约为6-8,尽管pH值可随被配制物质的性质以及待治疗的病症而有所变化。配制好的药物组合物可以通过常规途径进行给药,其中包括(但并不限于):肌内、腹膜内、静脉内、皮下、皮内、或局部给药。
FABP5拮抗剂(如抗体和反义序列)可直接用于疾病的诊断或治疗,例如,用于AS的诊断和治疗。此外,还可联用其他治疗剂。
本发明还提供了一种药物组合物,它含有安全有效量的FABP5蛋白的拮抗剂以及药学上可接受的载体或赋形剂。这类载体包括(但并不限于):盐水、缓冲液、葡萄糖、水、甘油、乙醇、及其组合。药物制剂应与给药方式相匹配。本发明的药物组合物可以被制成针剂形式,例如用生理盐水或含有葡萄糖和其他辅剂的水溶液通过常规方法进行制备。诸如片剂和胶囊之类的药物组合物,可通过常规方法进行制备。药物组合物如针剂、溶液、片剂和胶囊宜在无菌条件下制造。活性成分的给药量是治疗有效量,例如每天约1微克-10毫克/千克体重。
本发明还涉及定量和定位检测人FABP5蛋白水平的诊断试验方法。这些试验是本领域所熟知的,且包括FISH测定和放射免疫测定。试验中所检测的人FABP5蛋白水平,可以用于诊断AS。
一种检测样品中是否存在FABP5蛋白的方法是利用FABP5蛋白的特异性抗体进行检测,它包括:将样品与FABP5蛋白特异性抗体接触;观察是否形成抗体复合物,形成了抗体复合物就表示样品中存在FABP5蛋白。
FABP5蛋白的多核苷酸可用于FABP5蛋白相关疾病的诊断和治疗。编码FABP5蛋白的多核苷酸的一部分或全部可作为探针固定在微阵列或DNA芯片上,用于分析组织中基因的差异表达分析和基因诊断。用FABP5蛋白特异的引物进行RNA-聚合酶链反应(RT-PCR)体外扩增也可检测FABP5蛋白的转录产物。
本发明还提供了一种检测AS的试剂盒,它含有特异性扩增FABP5的引物对和/或FABP5特异性抗体。此外,还可含有特异性探针和/或PCR缓冲液等。
下面结合具体实施例,进一步详陈本发明。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围。下列实施例中未注明详细条件的实验方法,通常按照常规条件如美国Sambrook.J等著《分子克隆实验室指南》(黄培堂等译,北京:科学出版社,2002年)中所述的条件,或按照制造厂商所建议的条件。除非另外说明,否则百分比和份数按重量计算。以下实施例中所用的实验材料和试剂如无特别说明均可从市售渠道获得。
实施例
(1)细胞培养
复苏人主动脉平滑肌细胞(HASMC)(购自ATCC细胞库,PCS-100-012)于细胞培养皿中,加入4ml完全培养基(DMEM培养基+10%胎牛血清),置于细胞培养箱(37℃,5%CO2)孵育培养。待细胞汇合度达80~90%后取出细胞,加入500μl胰酶消化细胞,于显微镜下观察细胞开始皱缩时,加入200μl胎牛血清终止消化,反复轻轻吹打细胞,使其从皿底脱落,转至15ml离心管,1300r/min离心5min,弃去上清,加入适量完全培养基重悬细胞,等量分装至2-4个新的细胞培养皿中,用完全培养基补足4ml/皿,继续于细胞培养箱中孵育培养。重复以上步骤,保持细胞正常传代,以备后续实验使用。
(2)细胞内总蛋白提取
将待处理的细胞从细胞培养箱中取出,弃去培养液,用PBS洗2遍,弃去PBS,加入80μl蛋白裂解液,使裂解液覆盖皿底,静置5min,待细胞被充分裂解后,用细胞刮刮下细胞,用加样枪吸取全部细胞悬液并转移至1.5ml EP管中,4℃、13000r/min离心3min,收集上清并转移至标记好的新1.5ml EP管中,定量后于-20℃保存待用。
(3)细胞内总RNA提取
将待处理的细胞从细胞培养箱中取出,弃去培养液,用PBS洗2遍,弃去PBS,加入500μl RNAiso Plus裂解细胞,使其能够完全覆盖整个皿底,5-10min后用加样枪吸取裂解液反复吹打细胞,使其均从皿底脱落,冰上静置5min,将细胞裂解液全部收集并转移至1.5ml
EP管中并做好标记。加入三氯甲烷(RNAiso Plus体积的1/5),盖紧EP管盖,快速剧烈上下颠倒振荡15s,使有机相与水相充分接触,室温静置2-3min,待到溶液乳化充分后,室温静置5min,4℃、12000g-14000g离心15min,取出EP管,可见分成三层:上层为无色水相,富含细胞RNA;中间层为白色层,其中含有大量蛋白质;底层为红色有机相。用加样枪慢慢轻柔地吸上层上清,转移至已做好标记的新1.5ml EP管中。加入与吸取的上清等体积的异丙醇,轻柔上下颠倒混匀后,室温静置10min,4℃、12000g离心10min,弃上清,加入75%乙醇洗涤RNA,轻弹起EP管底沉淀,4℃、12000g离心5min,弃上清,重复洗涤一次,室温下干燥RNA沉淀10-15min,待白色沉淀变成透明无色物质之后,加入20μl DEPC水,定量后-80℃保存待用。
(4)实时荧光定量PCR(qPCR)
①使用Takara Real Time RNA逆转录试剂盒将提取的总RNA反转录为cDNA,体系如下表所示:
逆转录反应条件为:37℃,15min;85℃,5s;4℃维持,将经反转录得到的mRNA反应液稀释5倍,以用于后续PCR反应。
②使用Takara实时荧光定量PCR试剂盒进行PCR反应,体系如下表所示:
③体系配制完成后,使用Cobas Z480实时定量PCR仪进行扩增反应,条件为:90℃,10min;[95℃,10s;60℃,60s;循环40次];95℃,1min;60℃,30s;40℃,30s。实时荧光定量PCR反应用GAPDH为内参,并采用2-ΔΔCt计算目的基因mRNA的相对表达量。
(5)蛋白质免疫印迹试验(Western Blot)
①制备分离胶及浓缩胶,体系如下表所示:

②将制备好的凝胶安装至垂直电泳槽中,在第一孔泳道中加入5μl marker指示剂,其余各孔泳道分别加入80μg蛋白样品(体积视各样品浓度而定),接通电源,80V电泳20min,待蛋白样品处于积层胶与分离胶分界线,然后将电压调至120V,电泳1h,蛋白在电压作用下逐渐往下转移,并依次分离至不同水平位置。根据marker和蛋白分子大小,待所需蛋白达到可完全分离位置时,停止电泳。
③取出电泳槽中的凝胶,根据marker切下目的蛋白对应的条带,打开转膜夹,在转膜夹黑色面依次放上滤纸、凝胶条带、PVDF膜,滤纸,用钝性板水平在PVDF膜上来回轻轻按压以赶走里面的气泡,然后盖上转膜板白色板,转膜板的黑面对准转膜槽的黑面,转膜板的白面对着槽的红面。将转膜槽装满转膜液,使所有目的蛋白所在凝胶均浸泡在转膜液中,再将转膜槽置于冰水混合物中,打开电源,100V、70min,进行转膜。
④取出转膜后的条带,置于5%脱脂牛奶中,于摇床室温封闭2h。
⑤取出条带,用TBST清洗3次,每次5min。
⑥配制与目的蛋白的对应抗体,将条带放入抗体中溶液中,于4℃摇床慢速孵育过夜。
⑦取出条带,用TBST清洗3次,每次5min。
⑧根据一抗种属配制对应的二抗(稀释比例为1:5000-1:10000),将条带移入二抗溶液中,室温摇床上缓慢孵育1h。
⑨取出条带,用TBST清洗3次,每次5min。
⑩在条带上滴加超敏发光液,于曝光仪中进行曝光并分析结果。
(6)免疫荧光
取消毒后的细胞培养板及玻片,将玻片放入细胞培养板中,加入1ml细胞悬液,24h后
观察细胞状态,若贴于玻片生长说明爬片成功,取出玻片,PBS浸洗3次,每次5min,用4%多聚甲醛固定爬片15min,PBS浸洗3次,每次5min,用0.5%Triton X-100室温通透20min,PBS浸洗3次,每次5min,吸水纸吸干PBS,在玻片上滴加正常胎牛血清,室温封闭1h,吸水纸吸尽封闭液,直接在每张玻片上滴加能覆盖满玻片的一抗并放入湿盒,4℃孵育过夜。次日取出玻片,PBST浸洗3次,每次5min,吸水纸吸干爬片上多余液体后滴加稀释好的荧光二抗,湿盒中37℃避光孵育1h,PBST浸洗3次,每次5min,滴加DAPI避光孵育5min以进行染核,PBST浸洗3次,每次5min,吸水纸吸干爬片上的液体,用含抗荧光淬灭剂的封片液封片,然后在荧光显微镜下观察采集图像。
(7)小干扰RNA瞬时转染
取出待处理的细胞,计数后铺板,密度为5×105/中皿,24h后观察细胞贴壁状态,当细胞贴壁铺满皿底70%-80%,且细胞状态良好时,进行转染:弃去铺板细胞中的培养基,PBS清洗后加入1750μl Opti-mem,取1.5ml EP管,按处理组及对照组分别加入250μl Opti-mem、10μl Lipo3000及10μl siRNA-NC或siRNA-FABP5,混匀后静置10min,然后将混合液分别加入带有1750μl Opti-mem的细胞中,使总体系约为2ml,随后将其放入细胞培养箱继续培养。孵育6-8h后用取出细胞,弃去其中液体,PBS清洗后加入4ml DMEM完全培养基,于细胞培养箱继续培养24h后提取细胞总RNA或48h后提取细胞总蛋白,以进行后续实验。
(8)临床血液样本收集分别收集正常人及临床AS患者的血液样本,纳入标准如下:
①正常人组纳入标准:
常规体检各指标均正常且以往无慢性病史者。
②AS组纳入标准:
A.冠状动脉粥样硬化纳入标准:
根据2010年中华人民共和国卫计委颁布的《冠状动脉粥样硬化性心脏病诊断标准》,符合者即可纳入。
B.主动脉粥样硬化:
影像学X线或CT显示主动脉高密度影,钙化者可纳入。
C.颈动脉粥样硬化纳入标准
颈动脉粥样硬化斑块的诊断参照第8版《外科学》的诊断标准。纳入人群:a.均符合颈动脉粥样硬化斑块的诊断标准;b.均签署知情同意书。排除人群:a.接受过颈动脉内膜剥脱术的患者;b.有MRI扫描禁忌证的患者;c.曾做过颈部放射治疗的患者;d.有房颤等心脏病病史。
D.脑动脉粥样硬化纳入标准:
符合1988年全国第3届神经精神科学术会议修订的早期脑动脉硬化症诊断标准:a.年龄在45岁以上;b.有初发高级神经活动不稳定的症状或脑弥漫性损害症状;c.眼底动脉硬化Ⅱ级以上;d.主动脉增宽;e.颞动脉或桡动脉等周围血管硬化症或有冠心病;f.神经系统阳性体征,如深反射不对称,掌颏反射阳性或吸吮反射阳性;g.血清胆固醇增高;h.排除其他脑血管疾病。上述8项中5项或5项以上者即为纳入实验组。
离心收集各样本血清后经ELISA检测血清中FABP5及常规血脂指标,包括:总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(apoA1)、载脂蛋白B(apoB)的表达水平,比较FABP5及其他血脂指标在AS组及正常人组间的差异性,通过将FABP5与其他血脂指标两两或多重组合的方式分别观察不同指标组合在AS及正常人组间的差异性是否更为显著。
在上述分析结果的基础上结合各样本的临床病例对样本进行分类比较:
①AS不合并其他慢性疾病与AS合并其他慢性疾病比较;
②AS血管不同狭窄程度比较(以冠状动脉狭窄为例,一级:正常无狭窄,二级:轻度狭窄、狭窄小于30%,三级:中度狭窄、狭窄介于30%至50%,四级:重度狭窄、狭窄介于分之50%至90%,五级:次全闭塞、狭窄成度大于90%,六级:完全闭塞、无血流通过);
③AS累及血管支数比较(观察三支,即右冠状动脉、左冠状动脉前降支、左冠状动脉回旋支发生病变的数量,轻度:累及1支,中度:累及2支,重度:累及3支)。
(9)酶联免疫吸附实验(ELISA)
使用Elabscience公司的人FABP5ELISA检测试剂盒对收集到的上述血清样本中FABP5的含量进行检测:
①配制标准品:将标准品原液(20ng/ml)倍比稀释至10ng/ml、5ng/ml、2.5ng/ml、1.25ng/ml、0.63ng/ml、0.31ng/ml、0ng/ml,待用。
②分别设置空白孔、标准孔、待测样品孔、空白孔加样品稀释液100μl,余孔分别加标准品或待测样品100μl,注意不要有气泡,加样时将样品加于酶标板底部,尽量不触及孔壁,轻轻晃动混匀,覆膜后37℃孵育90min。
③弃去孔内液体,甩干,每孔加入生物素化抗体工作液100μl,酶标板覆膜,37℃孵育1h。
④弃去孔内液体,甩干,洗板3次,每次浸泡1-2min,甩干并在吸水纸上将孔内液体拍干。
⑤每孔加酶结合物工作液100μl,酶标板覆膜后37℃孵育30min。
⑥弃去孔内液体,甩干,洗板3次,方法同③。
⑦每孔加底物溶液(TMB)90μl,酶标板覆膜,37℃避光孵育15min。
⑧每孔加终止液50μl,终止反应。
⑨立即用酶标仪在450nm处测量各孔吸光度(OD值)并记录。
结果
(1)FABP5的亚细胞定位
经免疫荧光检测FABP5在平滑肌细胞中的亚细胞定位,发现FABP5表达于细胞质,于细胞膜及细胞核均鲜有表达(图1)。
(2)FABP5在人动脉内膜斑块组织中的表达显著上调。
分别取3例人颈动脉内膜斑块组织及3例正常颈动脉内膜组织进行基因芯片分析,寻找斑块组织与正常组织中差异表达的基因(信使RNA,即mRNA),经检测共发现4230个在斑块组织中上调的mRNA及4128个下调的mRNA,其中FABP5的上调倍数达210.79,P<0.01。(图2、表1)

(3)Ox-LDL能够诱导平滑肌细胞中FABP5的表达上调。
首先,分别使用0μg/ml,25μg/ml,50μg/ml,100μg/ml的ox-LDL处理平滑肌细胞,48h后经Western Blot及qPCR检测细胞中FABP5的蛋白及mRNA表达水平,结果表明FABP5的表达随ox-LDL刺激浓度的升高而上升。(图3-A,图4)
其次,使用100μg/ml的ox-LDL分别处理平滑肌细胞0h,12h,24h,48h,经WesternBlot及qPCR检测细胞中FABP5的蛋白及mRNA表达水平,结果表明FABP5的表达随ox-LDL刺激时间的升高而上升。(图3-B,图4)
(4)FABP5能够下调平滑肌细胞中ABCG1的表达。
使用FABP5的小干扰RNA(siRNA)处理平滑肌细胞,从而使细胞中FABP5的表达降低(图5),经qPCR检测,当FABP5表达下降后,某些与胆固醇代谢相关基因的表达均有升高,其中ABCG1的上调最为显著(图6)。经Western Blot及qPCR进一步验证,发现干扰平滑肌细胞中FABP5的表达后,ABCG1的蛋白及mRNA水平均显著上升(图7)。说明FABP5能够下调平滑肌细胞中ABCG1的表达。
(5)Ox-LDL通过上调平滑肌细胞中FABP5的表达,从而下调ABCG1的表达。
首先,采用ox-LDL及FABP5的siRNA处理平滑肌细胞,分组如下:
①空白对照组(-/-)
②50μg/ml ox-LDL 48h处理组(+/-)
③FABP5基因敲除载体处理组(-/+)
④50μg/ml ox-LDL 48h+FABP5基因敲除载体处理组(+/+)
待稳定表达后,采用Western Blot检测细胞中ABCG1的蛋白表达水平,结果发现,与空白对照组相比,ox-LDL 48h处理组中ABCG1的表达显著下调,FABP5基因敲除载体处理组中ABCG1的表达显著上调,而50μg/ml ox-LDL 48h+FABP5基因敲除载体处理组中ABCG1的表达较ox-LDL单独处理组有所上升,说明Ox-LDL通过上调平滑肌细胞中FABP5的表达,从而下调ABCG1的表达(图8)。
其次,采用ox-LDL及FABP5的过表达质粒处理平滑肌细胞(FABP5的过表达质粒效果见图9),从而对经siRNA处理后的结果进行验证,分组如下:
①空白对照组(-/-)
②50μg/ml ox-LDL 48h处理组(-/+)
③FABP5基因过表达载体处理组(+/-)
④50μg/ml ox-LDL 48h+FABP5基因过表达载体处理组(+/+)
待稳定表达后,采用Western Blot检测细胞中ABCG1的蛋白表达水平,结果发现,与空白对照组相比,ox-LDL 48h处理组中ABCG1的表达显著下调,FABP5基因过表达载体处理组中ABCG1的表达显著下调,而50μg/ml ox-LDL 48h+FABP5基因过表达载体处理组中ABCG1的表达较ox-LDL单独处理组下调更为显著,说明Ox-LDL通过上调平滑肌细胞中FABP5的表达,从而下调ABCG1的表达,验证了siRNA的实验结果(图10)。
(6)FABP5的临床验证。
通过收集临床血清样本,获得动脉粥样硬化患者组(AS组)32例,正常人组(NC组)41例,糖尿病人组(DM组)32例,动脉粥样硬化合并糖尿病人组(AS+DM组)32例,经ELISA检测上述四组血清FABP5的表达,结果发现相比NC组,FABP5在AS组中的表达显著上升,差异具有统计学意义,而DM组中FABP5的表达与NC组无明显统计学差异,但在AS+DM组中,FABP5的表达同样显著升高,且与NC组相比差异具有统计学意义(图11),说明FABP5在动脉粥样硬化患者中高表达,且不受其他慢性疾病干扰,即FABP5对动脉粥样硬化具有切实的诊断价值,能够作为诊断检测指标进行AS的临床诊断。
讨论
胆固醇代谢紊乱一直以来是促进动脉粥样硬化(AS)发病的重要危险因素,ox-LDL能够通过诱导细胞内胆固醇代谢障碍、泡沫细胞形成进而影响AS发生发展。因此,探讨由ox-LDL导致的胆固醇代谢失衡及有关作用机制对AS的防治具有重要意义。
脂肪酸结合蛋白-5(fatty acid binding protein 5,FABP5)属于脂肪酸结合蛋白家族(FABPs)一亚类,分子量为15kD,由FABP5基因编码。我们经研究发现AS斑块组织中FABP5的表达显著高于正常组织,Ox-LDL能够明显促进HASMC中FABP5的表达,FABP5则能明显抑制HASMC中ABCG1的表达,回复实验结果表明Ox-LDL通过上调HASMC中FABP5的表达进而下调ABCG1的表达,由于ABCG1已被证实能促进细胞内胆固醇流出从而维持胆固醇代谢平衡。由此,本发明认为FABP5-ABCG1途径参与Ox-LDL对细胞内胆固醇代谢的调控从而影响AS进展。此外,经临床验证证实FABP5在AS病人中的表达显著高于正常健康人(P<0.05),而在糖尿病(DM)患者中的表达与正常健康人无明显差异(DM常与AS并存,故设立DM组用于鉴别诊断),但在AS合并DM患者中的表达亦显著高于正常健康人(P<0.05)。由此表明FABP5对特异性诊断AS具有一定临床应用价值。
综上,本研究发现Ox-LDL通过上调HASMC中FABP5的表达从而抑制细胞内ABCG1的表达,进而导致细胞内胆固醇流出障碍,促进AS的发生发展。FABP5对特异性诊断AS具有一定临床应用价值。本研究成果为阐明由ox-LDL导致的胆固醇代谢障碍、AS发生及有关作用机制奠定了理论基础,同时也为防治AS提供了新靶点。

Claims (10)
1.一种FABP5蛋白、其编码基因或其检测试剂的用途,用于制备诊断试剂或试剂盒,所述诊断试剂或试剂盒用于诊断或辅助诊断动脉粥样硬化。
2.如权利要求1所述的用途,其特征在于,所述检测试剂选自下组:抗体、引物、探针、测序文库、核酸芯片(如DNA芯片)、蛋白质芯片、或其组合。
3.一种FABP5或FABP5拮抗剂的用途,用于制备药物或组合物,所述药物或组合物用于选自下组的一种或多种用途:
(1)抑制ABCG1蛋白水平的降低;
(2)提高ABCG1蛋白水平;和
(3)预防或治疗体内ABCG1蛋白水平降低所引起的疾病。
4.如权利要求3所述的用途,其特征在于,所述FABP5拮抗剂选自下组:
(a)降低或抑制FABP5的活性的物质;
(b)降低FABP5的表达或稳定性的物质。
5.如权利要求3所述的用途,其特征在于,所述FABP5拮抗剂选自:抗体、多肽、sh-RNA、dsRNA、miRNA、siRNA、反义寡核苷酸、化合物或其组合。
6.如权利要求3所述的用途,其特征在于,所述FABP5拮抗剂选自:抗FABP5抗体,针对FABP5的反义寡核苷酸、siRNA或dsRNA,FABP5的化学抑制剂。
7.如权利要求3所述的用途,其特征在于,所述“ABCG1蛋白水平降低所引起的疾病”为动脉粥样硬化。
8.一种检测动脉粥样硬化的方法,其特征在于,包括步骤:
a)准备受试者测试样品;和
b)检测测试样品中FABP5蛋白、或其编码基因的水平,并将检测结果与参比值进行比较,其中FABP5蛋白、或其编码基因的水平显著高于参比值,表明受试者患有动脉粥样硬化,或患动脉粥样硬化的几率高于正常人群。
9.一种诊断试剂盒,其特征在于,所述的试剂盒包括:
(a)FABP5蛋白、或其编码基因;和/或
(b)抗FABP5蛋白抗体,或者,特异性扩增FABP5蛋白编码基因的引物或引物对;
以及标签或说明书;
其中,所述的组分(a)、(b)分别位于一个或多个不同的容器中。
10.一种筛选预防和/或治疗动脉粥样硬化的潜在治疗剂的方法,其特征在于,包括步骤:
(a)在测试组中,在培养体系中,在测试化合物的存在下,培养表达FABP5蛋白的细胞一段时间T1,检测测试组所述培养体系中的FABP5蛋白、或其编码基因的水平L1;
并且在不存在所述测试化合物且其他条件相同的对照组中,检测对照组所述培养体系中的FABP5蛋白、或其编码基因的水平L2;和
(b)将上一步骤所检测的L1、L2进行比较,从而确定所述测试化合物是否是预防和/或治疗贫血或其相关疾病的潜在治疗剂;
其中,如果L1显著低于L2,则表示所述测试化合物是预防和/或治疗动脉粥样硬化的潜在治疗剂。
