Description

Traditional Chinese medicine composition for treating infantile indigestion with phlegm-heat cough and preparation method thereof
Technical Field
The invention relates to a traditional Chinese medicine composition and a preparation method thereof, in particular to a traditional Chinese medicine composition for treating infantile indigestion with food retention and phlegm-heat cough and a preparation method thereof, and belongs to the field of traditional Chinese medicines.
Background
Cough is a common and frequently occurring disease of infantile lung system. Cough due to food accumulation and phlegm-heat is a more special syndrome type in children's cough, and belongs to the category of internal injury cough. Cough due to retention of food and phlegm-heat in children, or referred to as cough with retention of food and phlegm, cough with retention of food, cough with retention of food, and cough with retention of food refers to cough in children caused by retention of food in middle energizer, accumulation of food due to long-term retention of food, stagnation of food and transformation of heat, stagnated heat burning body fluids into phlegm, turbid phlegm evaporating to the lung and obstructing the airway, and affecting the function of dispersing, purifying and descending of the lung due to the fact that the viscera are tender, the shape and qi are not full, the spleen is often insufficient, the transport and transformation of the body fluid are not known to be proper, and the excessive eating is greasy and thick. The "pulse cause syndrome treatment" is recorded as: "the syndrome of cough caused by retention of food in the middle energizer, failure of transportation and transformation, formation of phlegm into fluid retention, and ascending of phlegm-fire is ". In recent years, as the dietary structure of people tends to be more eutrophic, and in addition, children like to eat rich and sweet tastes such as high sugar, high protein and high fat, the self-control ability is poor, and the diet is not known to be hungry and full, so that the cough caused by food retention and phlegm heat of children due to heat transformation caused by food retention and stagnation is frequently attacked, and the cough accounts for more than 70% of cough patients according to the incomplete statistics of related pediatric clinics. The disease is similar to the gastroesophageal reflux cough in modern medicine, the cough is aggravated after eating the cough and even eating sweet and crying, and although antibiotics or simple cough-relieving and phlegm-resolving medicines are used for treating the disease, the curative effect is poor and the disease is easy to repeat.
The traditional Chinese medicine holds that the cough caused by food retention and phlegm heat belongs to the diseases of spleen (including stomach) and lung channels. The spleen is the source of phlegm generation, and the lung is the device for storing phlegm. Food stagnation, transforming heat into fire, transforming fluid into phlegm, phlegm-heat accumulation, stagnation in lung and cough due to failure of dispersing and descending of lung. Therefore, the lung and spleen should be treated simultaneously in the treatment of the clinical symptoms, the drugs for promoting digestion and resolving food stagnation, and regulating the lung to resolve phlegm and relieve cough are applied simultaneously, so that the traditional Chinese medicine composition not only clears away the stagnated heat of the spleen and stomach and removes food stagnation, but also regulates the lung to resolve phlegm and relieve cough to treat the root cause of disease, achieves the purposes of treating the root cause of disease and treating both principal and secondary aspect of disease. At present, the traditional Chinese medicine has obvious and reliable curative effect on treating the cough caused by food stagnation and phlegm heat. Chinese patent CN1425447A discloses a Chinese medicinal composition consisting of 10 medicaments of hawthorn, areca seed, immature bitter orange, loquat leaf, snakegourd fruit, weeping forsythia, radish seed, pepperweed seed, cicada slough and platycodon root, which can clear heat, lower lung qi, eliminate stagnation and relieve cough, and is used for treating cough, exacerbation at night, wheezy phlegm in throat, abdominal distension and halitosis caused by food stagnation and phlegm-heat in the lung. The medicine names of the invention are children food retention removing and cough stopping oral liquid (approved document: national standard Z10970022) and children food retention removing and cough stopping granules (approved document: national standard Z20060159), and the trade names thereof

Are all Chinese patent medicine products of Lunan Qianpu pharmaceutical Co. The Wangbai and the like adopt a random, contrast and multi-center clinical research method to prove that the total effective rate of the curative effect of the oral liquid for removing food retention and relieving cough for treating the symptoms of phlegm-heat cough and food retention in children is 84.71 percent, and no adverse drug reactions (Wangbai, et al, modern medicine and clinic, No. 25, No. 5 of 2010).
Betel nut is used as a main medicine in the prescription of the preparation for removing food retention and relieving cough for children, is dry mature seed of Areca nut Areca catechu L, and is widely used for treating diseases such as abdominal pain due to parasitic infestation, food retention, qi stagnation, edema, beriberi and the like for a long time. In recent years, in southern areas such as Hunan, Hainan and Taiwan, wind is prevailing for chewing betel nuts, and the edible betel nuts are prepared by processing fresh or dried young fruits of Areca catechu L. However, studies on eating betel nuts show that long-term chewing of betel nuts can lead to addiction, possibly even carcinogenesis or other toxic and side effects. The international agency for research on cancer (IARC) in 2003 placed Areca nut (Areca nut) on the list of class 1 carcinogens. It should be noted that, the medicinal betel nut is decocted with a conventional dose for oral administration, and no experimental and clinical research reports are reported yet for causing cancer or toxic and side effects.
Cicada slough serving as an animal medicine in a formula of a preparation for removing food retention and relieving cough for children is a peel shell which falls off when nymphs of Cryptotympana pustulata Fabricius of cicada animals, and is widely used for treating wind-heat type cold, pharyngalgia hoarseness, measles imperviousness, rubella pruritus, convulsion and the like in clinic. For a long time, cicada slough medicinal materials are all from wild, and naturally fallen shells are picked up manually for medicinal use. Particularly, in recent years, with the continuous expansion of the activity range of human beings, the living space of the black locust is greatly compressed, the growth conditions of the black locust are damaged along with the use of a large amount of chemical fertilizers and pesticides, and the reproduction of the black locust is further damaged when the nymphs are eaten in a large amount, so that the medicinal material resources of the cicada slough and the annual yield are continuously reduced directly due to the reasons. The habitat space of the black locust in China is still not optimistic within a foreseeable time, so that the yield of the cicada slough medicinal material tends to shrink day by day and even runs out, and further the medicinal material market supply and the production of related traditional Chinese medicine products are seriously influenced.
Disclosure of Invention
In view of the problems of betel nut and cicada slough 2 medicines in the formulation of the children stagnation-removing and cough-relieving preparation, the potential toxicity of the betel nut should be scientifically examined according to the scientific and careful attitude so as to avoid the health risk; aiming at the current situation that the resource supply of the cicada slough is gradually reduced, reasonable and ordered utilization of wild medicinal material resources is realized from saving and intensive utilization of the wild medicinal material resources. Therefore, the invention aims to provide a novel traditional Chinese medicine composition for treating the infantile indigestion with food retention and phlegm heat cough and a preparation method thereof (CN1425447A) on the basis of the traditional Chinese medicine to analyze the etiology and pathogenesis of the infantile indigestion with food retention and phlegm heat cough, and aims to provide the traditional Chinese medicine composition for treating the infantile indigestion with food retention and phlegm heat cough and the preparation method thereof, so that the magnolia officinalis and the burdock carry out targeted effective substitution on the betelnut and the cicada slough 2 medicines in the original composition, and the characteristics of phlegm heat accumulation of the disease are combined, and 4 medicines of dried orange peel, pinellia ternate, scutellaria baicalensis and cortex mori radicis added, so that the functional indications and the exact clinical curative effect of the infantile indigestion with food retention and phlegm cough relieving composition are basically maintained, the effects of clearing heat and eliminating phlegm are enhanced, the product production dilemma caused by the shortage of medicinal material resources can be solved, and the possible potential toxicity risk can be avoided.
The invention aims to provide a traditional Chinese medicine composition for treating infantile indigestion with phlegm-heat cough, which is prepared from hawthorn, pinellia ternate, dried orange peel, platycodon grandiflorum, snakegourd fruit, immature bitter orange, mangnolia officinalis, scutellaria baicalensis, fructus forsythiae, radish seed, semen lepidii, burdock, cortex mori radicis and folium eriobotryae, has the effects of clearing heat, regulating lung, helping digestion, eliminating stagnation, reducing phlegm and relieving cough, and is used for treating infantile indigestion with phlegm-heat cough symptoms such as heavy cough and aggravation at night, wheezy phlegm in throat, excessive phlegm or little phlegm, difficult expectoration, feverish sensation in palms and soles, abdominal distension, anorexia, halitosis, dry stool, red tongue with thick greasy coating and the like.
The invention relates to a traditional Chinese medicine composition for treating infantile indigestion with phlegm-heat cough, which is prepared from the following traditional Chinese medicine components:

preferably, the hawthorn is fried hawthorn, the pinellia ternate is prepared pinellia ternate, the magnolia officinalis is ginger-processed magnolia officinalis, the radish seed is fried radish seed, the semen lepidii is fried semen lepidii, the burdock seed is fried burdock, and the loquat leaf is honey-processed loquat leaf.
Spleen earth is the mother of lung meridian, and children's cough due to retention of food and phlegm-heat belongs to the "mother disease and child", so it should be treated by both lung and spleen; food stagnation is the root of the disease, and phlegm cough is the symptom of the disease, so it is necessary to treat the disease at the root and treat both the symptoms and root causes. In the recipe, hawthorn has the functions of removing food retention and resolving stagnation, can eliminate all food retention and especially is good at eliminating greasy food retention of meat, and is called as sour and slightly sweet in taste and can supplement sour juice in stomach, so that food retention and accumulation can be eliminated to treat meat accumulation, which is particularly effective in the medical science records of Zhongzhong Zhong xi; lai Fu Zi can direct qi downward, promote digestion and resolve phlegm, which is good at relieving the accumulation of cereal and flour, so it is said in Ben Cao Xin Shu that it is effective in relieving dyspnea, cough and dyspnea, and relieving flour food. The two herbs are mutually reinforced to promote digestion and remove food stagnation, so that food stagnation can be eliminated, so that stagnation of heat and phlegm will not be followed, and heat generation and phlegm generation will not be caused.
The spleen is the source of phlegm generation, and the lung is the qi for storing phlegm. Pinellia ternate is a key medicine for strengthening spleen and stomach and eliminating phlegm, enters spleen and stomach and lung meridian to make spleen strengthen dampness and remove, so that phlegm is not originated, enters lung to disperse and dissolve lung, and is not remained, so that pinellia ternate is called as a main phlegm and is assisted by cold and heat clearing medicines in the invention of materia medica. Tangerine peel, pericarpium Citri Reticulatae regulates qi to invigorate the spleen, eliminates dampness and phlegm, and is also an essential herb for treating phlegm, "all patients with cough and phlegm have the best effect of treating lung with pungent and sweet flavor" ("Puji Fang-cough gate"), "Changsha Yao Ji" (Changsha Yao Ji) is good at lowering turbid pathogen and relieving vomiting, activating stagnancy and eliminating phlegm, harmonizing with the other things without breaking qi and damaging healthy qi, and also good herb for moving stagnation and regulating qi. Ban Xia can dispel phlegm by guiding qi downward with the help of Chen Pi; tangerine peel, pericarpium Citri Tangerinae and pinellia Tuber can help to remove phlegm and direct qi downward. Snakegourd fruit has the functions of clearing heat, resolving phlegm, and resolving stagnation, and is recorded in the book Ben Cao Yan Yi Bu Yi (supplement to the Yan Yi of materia Medica): sweet in flavor and moist in nature, sweet in flavor can tonify lung, moisten body and descend qi. For phlegm in the chest, it is the essential herb for cough because it is sweet and mild in action of descending, so it is indicated for lung fire to expel and lose the action of descending. The two herbs of Gua Lou and ban Xia are combined to strengthen the actions of resolving phlegm and dissipating nodulation by combining cold and warm herbs and applying dryness and moistening mutually. Platycodon grandiflorum is yellow millet which is a medicine for opening lung qi, eliminating phlegm and benefiting qi, and the essential herbs for relieving depression and promoting phlegm are also not for cough and dyspnea with the effect of regulating qi and eliminating phlegm (materia Medica in materia Medica), which is a yellow millet which is a medicine for opening lung qi. The pinellia ternate and the like are used together to mutually promote, disperse and descend in order, so that spleen qi is transported and phlegm is self-transformed, the effects of regulating qi and reducing phlegm are achieved, and the pinellia ternate and the like are used as ministerial drugs.
Food stagnation is apt to cause heat transformation, and heat-clearing herbs should be added on the basis of food stagnation. Baikal skullcap root, bitter in taste and cold in nature, excels in clearing lung fire and excessive heat in upper energizer, "Du Jiang Tan Huo cough, dyspnea and Chun Qin Mo Neng Qing" (materia Medica), and "Ben Cao invent" means that it "removes damp heat in stomach and eliminates grain, does not kill grain with pathogenic heat, can remove heat, then stomach and grain disappear ". Ting Li Zi is bitter and descending and pungent in property, cold in nature and clearing heat, specially purging phlegm fire and fluid retention in lung to relieve dyspnea and cough, so it is good for treating cough caused by lung obstruction, relieving dyspnea and removing phlegm retention in chest, and for descending qi and moving phlegm and clearing heat in lung, as recorded in KAI BAO BENCH. Mulberry bark, cortex Mori, with its cold nature, exclusively enters lung meridian to lower lung fire, purge lung qi and resolve phlegm cough, is mainly indicated for dyspnea with cough and heat phlegm and spittle blood, which are caused by stagnation of excess pathogen and obstructed lung orifice, so it seeps and disperses to promote lung qi and cure all syndromes. Loquat leaves, with bitter taste and cold nature, can clear and descend lung qi to relieve cough, so all the pathogens of wind, dampness, warm, heat, summer-heat and dryness in lung can be used to protect soft gold (meaning lung) to descend and cure the disease (Chongqing Tang Nianjiao). The four medicines, the scutellaria is good at clearing lung heat, the lepidium seed, the mulberry bark and the loquat leaf have the functions of clearing lung heat, eliminating phlegm, descending qi and relieving asthma, and the medicines are mutually used for greatly enhancing the effect of clearing lung heat and relieving cough and are also used as ministerial medicines together.
Food retention may cause abdominal distension due to improper diet and dysfunction of spleen in transportation and qi stagnation in the stomach and intestine. Zhi Shi is good at breaking qi and relieving stuffiness, and resolving food stagnation, so it is called in Ben Cao Hua Yi that it is named as "specially purging stomach and food … …, with fierce nature and rapid action of opening and resolving hard mass, and has the action of pushing wall and pouring wall", and it is also used for pungent, bitter and diarrhea-relieving because of spleen qi stagnation and failure of transportation and transformation if there is no desire for diet. It is the most important herb for qi in blood. Hou Po also purges the middle energizer, relieves stagnation and removes stasis, "Hou Po is used in ancient prescription for purging qi of accumulation and stagnation" (compendium of medicine), "all kinds of food stagnation, qi stagnation and fulminant swelling … …, Cheng is essential herb" (Ben Cao Jing Shu). The bitter orange and the magnolia bark are bitter and cold in nature and pungent and warm in nature, and are combined with each other, so that the cold and the heat can be mutually prepared, the cold and the heat can complement each other, the monarch and ministerial medicines can be better assisted to strengthen the effects of removing food retention and promoting qi circulation, and the traditional Chinese medicine is an assistant and guide medicine.
In addition, "the most children's diseases are impaired by food and the most children's diseases are mostly hot," as said in the book of junior science, the most children's febrile diseases belong to pure yang, six qi are involved, and both qi and blood are converted into heat; the food is not transformed into food, and it is steamed inside, also transforming into heat. Forsythia fruit has the actions of clearing heat and removing toxicity, dispelling wind and heat, so it can be used for both clearing stomach heat and regulating the central apparatus to recover the functions of spleen and stomach, and clear food retention and heat in the stomach to stop vomiting, because Forsythia fruit has a strong and cool nature and qi in the qi system. Great burdock achene has the effects of dispersing lung heat, ventilating and smoothing lung, eliminating phlegm, relieving sore throat, promoting eruption, detoxifying and diminishing swelling, and is recorded in the book of drug property endowment: arctium lappa fruit is used for dispelling wind and eliminating phlegm. It is said that "the herbs in Yao Hua Yi" can ascend and descend, relieve heat and toxicity, and have bitter flavor and clear fire, pungent flavor and dispel wind … …, so it is indicated for stagnant fire in lung meridian and wind-heat in lung meridian. The two medicines not only assist ministerial medicines in strengthening the heat-clearing function of lung and stomach, but also play the roles of detoxifying and relieving sore throat aiming at secondary and concurrent syndromes, and are also used as adjuvant and conductant medicines.
Based on the physiological and pathological characteristics of young yin and yang of children, incomplete qi, frequent insufficiency of spleen, easy deficiency and easy excess, the medicines are formulated according to the compatibility theory of monarch, minister, assistant and guide traditional Chinese medicine and following the treatment and tonifying concepts of simultaneous treatment of lung and spleen, treatment of both symptoms and root causes and taking the stage of peace as the stage, so that the effects of promoting digestion, resolving food stagnation and guiding stagnation are achieved, the effects of clearing lung, eliminating phlegm and relieving cough are achieved, the medicines are combined, the effects of clearing heat, regulating lung, promoting digestion, resolving food stagnation, eliminating phlegm and relieving cough are achieved, and the infant patients with the syndrome of food stagnation, phlegm and heat and cough can obtain good effects.

2 method
2.1 grouping and modeling 60 healthy SD rats, and dividing the SD rats into 6 groups by a random digital table remainder method, namely a normal control group, a model control group, a positive control group, a test high-dose group, a test medium-dose group and a test low-dose group, wherein each group comprises 10 rats and each half of male rats and female rats. In the experiment, except the normal control group, the other 5 groups are fed with the special feed, and are simultaneously fed with milk with the concentration of 50% for intragastric administration for 1 mL/time and 1 time/d, and each group freely takes food and drinks and continuously molds for 15 d. Food intake and body weight of each rat were recorded daily. After the model is made, the rat has decreased appetite, slow action, narrow eyes, listlessness and obvious stomach enlargement, and a large amount of food residues in the stomach can be found after the model is dissected, thereby proving the success of the model making.
2.2 after the model is successfully made, feeding each group with conventional feed, and carrying out dose conversion according to dose conversion coefficients of different animals in the appendix of the clinical research guiding principle of new traditional Chinese medicines, wherein a positive control group is administered with 15/kg/d (20 times of human dose) of the oral liquid for removing food retention and relieving cough for children, a test high dose group, a test medium dose group and a test low dose group are administered with 15g/kg/d (20 times of human dose), 7.5g/kg/d (10 times of human dose) and 3.75g/kg/d (5 times of human dose) of the tested medicines in sequence, a normal control group and a model control group are administered with normal saline with equal volume, and the administration is carried out for 1 time every day and the administration is carried out for 15d continuously.
2.3 detection index and method
(1) Determination of Motilin (MTL) in peripheral blood: after 14d administration, fasting is carried out for 16h without water prohibition, each group is drenched with 2 mL/body of nutritional semisolid paste after 15d administration for 40min, after 20min, 10% chloral hydrate (0.30mL/100g) is used for abdominal anesthesia, abdominal aorta is quickly sampled, the obtained mixture is injected into an EDTA tube to be fully shaken and immediately stored at 4 ℃, and the obtained mixture is centrifuged at 4000r/min for l0min within 3h and stored at-80 ℃ to be tested. The content of Motilin (MTL) in peripheral blood is detected by a radioimmunoassay.
(2) Determination of the rate of intestinal transit: after the abdominal aorta blood sampling is finished, the rat is killed by dislocation of cervical vertebra immediately, mesentery is separated rapidly, small intestine is taken out and laid on white paper, and the distance from pylorus to the full length of ileocecal part and the distance from pylorus to the front edge of carbon-powder semisolid paste are measured. The percentage of the distance from the pylorus to the front edge of the carbon powder semisolid paste to the whole length from the pylorus to the ileocecal part is taken as the small intestine propulsion rate:
the small intestine propulsion rate (%) (the distance of the carbon-powder semisolid paste propelled in the small intestine/the full length of the rat small intestine) × 100%.
