Description

矫正全踝部植入件
本申请是申请日为2017年8月24日、申请号为201710735773.8、发明名称为“矫正全踝部植入件”的申请的分案申请。
背景技术
全踝部替换系统随时间劣化并且需要替换。在矫正踝部外科手术中,全踝部植入件被移除并且替换以新的全踝部植入件。当移除初始植入件时,留下大的骨空间。此骨空间大于在全踝部替换过程中移除的初始植入件的尺寸。
在利用当前技术的外科手术之前不能确定骨空间的尺寸。当前踝部矫正系统在矫正外科手术过程中还要求另外地切割和/或修正一个或多个踝骨。初始部件必须被移除并且替换以增强件,其不提供与初始部件同样的固定接合。
发明内容
在多个实施方式中,矫正植入件包括在第一平坦表面与第二平坦表面之间延伸的本体以及从本体的第一平坦表面延伸的头部。头部构造为将矫正植入件联接到多部件假体的至少一个附加部件。至少一个联接机构构造为将本体联接到第一骨。
在多个实施方式中,公开了一种外科手术方法。外科手术方法包括在患者中形成切口并且暴露植入在患者中的多部件假体。多部件假体的至少一个部件被拆卸并且矫正植入部件联接到多部件假体的第一部件。矫正植入部件包括:本体,其在第一平坦表面与第二平坦表面之间延伸;头部,其从本体的第一平坦表面延伸并且构造为将矫正植入件联接到多部件假体的至少一个附加部件;以及至少一个联接机构,其构造为将本体联接到骨。

矫正植入部件100a具有从上表面114延伸到下表面116的预定厚度。本体102可以具有任何适当的预定厚度,诸如例如范围从2mm到20mm、诸如例如4mm、8mm、12mm、16mm和/或2mm与20mm之间任意范围的厚度。在一些实施方式中,本体102在底面116附近具有第一预定宽度以及在顶面114附近具有第二预定宽度。第一预定宽度可以大于、小于、和/或等于第二预定宽度。在一些实施方式中,第二预定宽度小于第一预定宽度,使得本体102的厚度从底面116到顶面114逐渐减小。在一些实施方式中,预定宽度选择为与在全踝部矫正过程中移除的初始胫骨托的宽度匹配。例如,在一些实施方式中,胫骨托可以具有12mm–30mm的宽度。

如图4D中所示,在一些实施方式中,头部168从胫骨平台132的上表面154延伸。头部168的尺寸设计并且构造为与由胫骨托矫正植入件100d限定的头部开口134匹配。头部168可以限定与穿过胫骨托矫正植入件100d形成的紧固件孔140同心的紧固件孔(未示出)。在一些实施方式中,升高的对准引导部130与头部168提供了胫骨平台130相对于胫骨托矫正植入件100d的固定接合位置。在一些实施方式中,头部168具有与形成在胫骨托矫正植入件100a上的头部112相同的长度和周边。

在一些实施方式中,全踝部矫正包括距骨假体的矫正和/或替换。图10示出了距骨矫正板300a的一个实施方式。距骨矫正板300a包括具有在上表面304与下表面306之间延伸的预定厚度的本体302。上表面304与下表面306大体上限定平坦表面。侧壁308围绕本体302的周边延伸。在一些实施方式中,矫正板300a包括从本体302的上表面延伸的头部312。头部312尺寸设计并且构造为将距骨圆顶(或其它距骨假体)联接到矫正板300a。在一些实施方式中,头部312具有圆柱形形状并且在上表面304上方延伸预定高度。移除特征314可以延伸通过头部312的上部以允许在相同和/或随后的矫正手术过程中移除矫正板300a。在一些实施方式中,头部312限定从顶面至少部分地延伸到头部312和/或本体302中的紧固件孔316。
在一些实施方式中,一个或多个突出部320(或杆)从矫正板300a的底面306延伸。一个或多个突出部320的尺寸设计并且构造为将矫正板300a锚定到切除距骨。突出部320从底面306以预定角度延伸预定长度且具有圆形远端324。例如,在一些实施方式中,一个或多个杆320从底面306以5-90°之间的预定角度延伸,诸如例如5°、10°、15°、30°、45°、60°、75°、90°和/或任意其它适当角度。矫正板300a可以包括任意适当数量的杆,诸如例如,一个杆、两个杆、三个杆、四个杆和/或任意其它数量的杆。

图16A-图16C示出了具有通过其形成的多个紧固件孔350的距骨矫正板300g的一个实施方式。距骨矫正板300g与上面结合图13描述的距骨矫正板300d类似,并且类似的描述这里不再重复。距骨矫正板300g包括从上表面304延伸穿过下表面306的多个紧固件孔350。多个紧固件孔350可以包括锁定和/或非锁定紧固件孔350。例如,在一些实施方式中,一个或多个紧固件孔350构造为容纳从田纳西州孟菲斯的瑞特医疗可获得的Ortholoc 3Di锁定螺钉。紧固件孔350可以包括任意适当的锁定和/或非锁定接合。
在一些实施方式中,对准特征352从头部312的上表面延伸到本体302的下表面306。对准特征352的尺寸设计并且构造为将一个或多个其它矫正元件的互补对准特征容纳在其中。对准特征352可以构造为与形成在距骨假体和/或全踝部替换和/或矫正的其它元件上的任意适当的对准特征(诸如突出部、凹槽、嵌入件和/或任意其它适当对准特征)配合。
图17A-图17H示出了距骨矫正板300h-300o的各个实施方式。距骨矫正板300h-300o与结合图10描述的距骨矫正板300a类似,并且类似描述这里不再重复。如图17A-图17H中所示,距骨矫正板300h-300o可以包括第一部分360与第二部分362。第一部分360的尺寸设计并且构造为在矫正手术过程中联接到第一骨,比如切除距骨。第一部分360可以包括通过其的多个紧固件孔350。第一部分360可以具有尺寸设计并且构造为适配各种尺寸的距骨切除的各种尺寸、厚度、和/或其它参数。在一些实施方式中,第一部分360限定大体上矩形和/或椭圆形形状。
在一些实施方式中,距骨矫正板300h-300o包括第二部分362,此第二部分尺寸设计并且构造为当在矫正手术过程中距骨矫正板300h-300o联接到距骨时在比如例如舟骨的第二骨上方延伸。第二部分362沿着第一部分360的第一侧364延伸并且可以具有基本上圆形或喇叭形状。第二部分362的尺寸设计并且构造为桥接距骨-舟骨关节。在一些实施方式中,第二部分362包括尺寸设计并且构造为将第二部分362附接到距骨和/或舟骨的多个紧固件孔350。距骨板矫正板300h-300o的第一部分360和/或第二部分362可以包括任意数量的紧固件孔350,诸如例如一个、两个、三个、四个或更多紧固件孔350。紧固件孔350可以包括锁定、非锁定、多轴锁定、和/或压紧狭槽。在一些实施方式中,第二部分362包括至少一个压紧紧固件孔356,该压紧紧固件孔构造为将压紧螺钉容纳在其中,此压紧螺钉从舟骨通过进入到距骨中以提供距骨-舟骨关节的另外融合。
在一些实施方式中,全踝部矫正件包括距骨矫正增强件。图18A-图18B示出了距骨矫正增强件400a的一个实施方式。距骨矫正增强件400a包括在上表面404与下表面406之间延伸的本体402。侧壁408围绕本体402的周边延伸。侧壁408与上表面406可以限定圆形和/或弯曲连接。在一些实施方式中,侧壁408和/或下表面406限定开口410,该开口尺寸设计并且构造为容纳通过其的中间杆322。增强件400a的尺寸设计并且构造为在矫正手术过程中填充距骨中的一个或多个骨空间。距骨增强件400a可以具有用于填充骨空间的任意适当形状,诸如例如图18A-图18B中所示的椭圆形形状。
在一些实施方式中,距骨增强件400a的底面406限定一定尺寸的螺钉孔420。在一些实施方式中,螺钉426联接到多驱动件422(参见图18C)并且插入到螺钉孔420中。多驱动件422包括把手部分424。螺钉头428与多驱动件422的远端部分连接以允许外科医生将螺钉426驱动到螺钉孔420中。尽管示出了多驱动件422,应该理解的是可以使用其它驱动机构并且在本公开的范围内。
图19A-图19B示出了距骨增强件400b的另一个实施方式。距骨增强件400b与结合图18A-图18C描述的距骨增强件400a类似,并且类似的描述这里不再重复。距骨增强件400b具有圆形和/或部分圆形形状的本体402。通过本体402限定的一个或多个切口440从侧壁408至少部分地延伸到本体402中。在一些实施方式中,切口440的尺寸设计并且构造为将骨的一部分容纳在其中。在其它实施方式中,切口440的尺寸设计并且构造为当增强件400b联接到距骨板时容纳一个或多个栓钉320。
图20示出了具有与其联接的距骨增强件400c的距骨板300p的一个实施方式。距骨板300p包括从其延伸的多个栓钉320(参见图12)。距骨增强件400c的尺寸设计并且构造为利用螺钉426和/或任何其它适当的附接装置联接到距骨板300p。距骨增强件400c在矫正手术过程中填充距骨中的骨空间。在一些实施方式中,距骨增强件400c永久地联接到距骨板300p。在一些实施方式中,在矫正手术过程中,距骨增强件400c可以由外科医生选择性地联接到距骨板300p。距骨增强件400c包括具有形成在其中的多个切口440的圆顶状本体402。多个切口的尺寸形成并且构造为容纳通过其的一个或多个栓钉330。在一些实施方式中,距骨增强件400c包括形成在上表面404中(诸如例如位于圆顶状本体402的顶点处)的杆孔420。
在一些实施方式中,全踝部矫正件包括距骨圆顶初始假体和/或矫正距骨圆顶。图21A-图21D示出了尺寸设计且构造为在矫正手术过程中替换初始距骨圆顶的矫正距骨圆顶500的一个实施方式。距骨圆顶500包括在上表面504与下表面506之间延伸的本体502。上表面504限定具有凸形或鞍形的铰接表面。铰接表面的尺寸设计且构造为抵靠容纳在胫骨矫正植入部件中的胫骨平台132铰接。下表面506是平坦表面。在一些实施方式中,头部开口514从下表面506至少部分地延伸到本体502中。头部开口514的尺寸设计并且构造为将从一个或多个踝部假体部件(诸如距骨矫正板)延伸的头部容纳在其中。头部开口514可以居中地定位在下表面506上和/或可以与中心偏置。在一些实施方式中,矫正距骨圆顶500包括从下表面506至少部分地延伸到本体502中的多个间隙孔510。多个间隙孔510的尺寸设计并且构造为至少将紧固件的上部容纳在其中。例如,在一些实施方式中,多个间隙孔510构造为容纳插入到距骨板中的一个或多个紧固件的上部。
如上所述,矫正植入部件/增强件可以具有各种形状与几何构造。在一些实施方式中,矫正植入部件/增强件由等离子喷涂钛形成,但是包括但不限于由瑞特医疗技术公司可获得的

的其它材料、以及其它金属、陶瓷、塑料、以及骨生长材料。
可以在利用荧光检查进行手术前评估之后选定全踝部矫正系统2的部件的尺寸与形状以识别植入在骨中的多部件假体的位置,或者可以在检查植入件位置以后由外科医生或者其它医疗护理提供者在外科手术进行时执行适当的矫正植入部件/增强件4-10的选定。在一些实施方式中,矫正植入部件/增强件4-10被单独地消毒与包装,而在一些实施方式中,植入部件/增强件4-10以套件提供。例如,当以套件提供时,每个单独植入部件/增强件4-10都可以单独地包装并且包括在较大的容器或者包装中。然而,可以在不将多个植入部件/增强件4-10打包在单个包装中的情形中形成套件。
在矫正操作过程中,可以部分地或者完全地拆卸先前植入患者体中的多部件假体100。例如,如果多部件植入件100包括胫骨平台102、胫骨托4与距骨圆顶6,那么外科医生可以利用工具(诸如螺丝刀、榫钉或专用装置)使胫骨平台102、胫骨托104、距骨圆顶106脱开,如本领域普通技术人员将会理解的。例如,可以通过使莫氏锥度脱开或拧松植入部件使胫骨平台102与胫骨托104分开。例如,如果多部件假体100是距骨假体,那么可以通过使莫氏锥度联接脱开而将距骨圆顶6与距骨杆(未示出)分开。当完全地拆卸时,整个多部件假体被从患者移除。
通过多部件植入件被至少部分地拆卸,利用可适用的附接机构将一个或多个矫正植入部件/增强件4-10在原位装配到多部件假体。在一些实施方式中,原位附接包括:将一个或多个矫正植入部件/增强件4-10插入到预先存在的髓内腔中;以及将矫正植入部件/增强件4-10附接到多部件假体的植入部件。还可以非原位地将一个或多个矫正植入部件/增强件4-10与多部件假体的一个或多个部件联接在一起,并且然后将形成的组件联接到多部件假体的任何部件。例如,如果将多部件植入件完全地从患者移除,那么外科医生或其它医疗专家或者护理者可以利用一个或多个矫正植入部件/增强件4-10植入多部件假体的一个或多个部件。
如上所述,全踝部矫正系统2的各部件可以具有彼此不同的形状和/或与全踝部矫正系统2的其它部件的形状不同的形状。全踝部矫正系统2的各部件联接在一起以填充骨中的空间。此外,单个部件/增强件可以包括多种附接装置,诸如例如椎体、螺纹、卡口联接,但是仅列出几种可能性。
尽管已经根据示例性实施方式描述了装置、套件、系统和方法,但是它们不限于此。相反,应该广义地理解所附权利要求,以包括在不偏离装置、套件、系统和方法的范围与等效范围的情况下通过本领域技术人员可以做出的装置、套件、系统和方法的其它变型与实施方式。

Claims (12)

1.一种用于全踝部替换系统的矫正植入件,包括:
本体,其在第一平坦表面与第二平坦表面之间延伸;
头部,其从所述本体的第一平坦表面延伸并且构造为将所述矫正植入件联接到多部件假体的附加部件;
中间杆,其从所述第二平坦表面延伸,所述中间杆构造为将所述矫正植入件联接到构造为填充切除骨的一个或多个空间的增强件;以及
一个或多个突出部,其从所述第二平坦表面延伸,构造为将所述矫正植入件锚定到切除距骨,其中所述一个或多个突出部从所述第二平坦表面以预定角度延伸。
2.根据权利要求1所述的矫正植入件,其中,所述预定角度在5°和60°之间。
3.根据权利要求1所述的矫正植入件,其中,所述预定角度在15°和60°之间。
4.根据权利要求1所述的矫正植入件,其中,所述中间杆包括圆顶状本体。
5.根据权利要求1所述的矫正植入件,其中,所述头部限定从顶面至少部分地延伸到所述头部中的紧固件孔。
6.根据权利要求1所述的矫正植入件,其中,所述头部限定圆柱状突出部。
7.根据权利要求6所述的矫正植入件,其中,所述头部限定从所述圆柱状突出部的第一侧延伸到第二侧的对准凹槽。
8.根据权利要求1所述的矫正植入件,其中,所述一个或多个突出部各自包括构造为将所述矫正植入件联接到所述切除骨的尖状远端。
9.根据权利要求6所述的矫正植入件,进一步包括至少一个紧固件孔,所述紧固件孔构造为容纳通过其的紧固件,其中所述紧固件构造为将所述本体联接到所述切除骨。
10.根据权利要求1所述的矫正植入件,其中,所述本体包括距骨部分和跟骨部分,其中所述距骨部分构造为将所述本体联接到第一骨,并且其中所述跟骨部分从所述距骨部分延伸并且构造为将所述本体联接到第二骨。
11.根据权利要求10所述的矫正植入件,其中,所述跟骨部分限定穿过其形成的至少一个压紧孔。
12.一种用于全踝部替换系统的矫正植入件套件,包括:
根据任一前述权利要求所述的矫正植入件,作为第一矫正植入件;
第二矫正植入件,其包括构造为填充形成在所述骨中的空间的增强件,所述第二矫正植入件构造为联接到所述第一矫正植入件的本体的第二平坦表面,并且限定一个或多个切口,所述一个或多个切口中的每个的尺寸设计并且构造为容纳所述第一矫正植入件的所述一个或多个突出部中的一个;以及
第三矫正植入件,其包括限定铰接表面的本体,其中所述第三矫正植入件构造为联接到所述第一矫正植入件的第一平坦表面。
