技术领域
本发明涉及一种用于设置、有针对性的监控、数值探测、后续调节以及尤其校准核磁共振治疗仪器的方法。此外,本发明还涉及一种核磁共振治疗仪器、一种治疗系统以及核磁共振尤其用于医治目的和美容目的的应用。
背景技术
已知核磁共振仪器用于获得医治功效和美容功效的应用。
在欧洲专利文献EP 1 089 792 B1(专利权人Axel Muntermann)中已经描述了在借助核磁共振治疗人类或动物组织的医治功效。
商标推出的治疗仪涉及一种仪器,其中,在治疗区域内具有均匀场强的磁场与垂直辐射的交变场叠加,从而在治疗区域内获得核磁共振。在交变场的频率与垂直的均匀磁场的场强相应协调时,在治疗区域内产生核磁共振。均匀磁场的场强借助调制频率fm进行调制并因此由基量和变化的调制量组成。由于调制,在每次经过中达到一次拉莫尔(Larmor)条件,从而在治疗总量内产生周期性的核磁共振。
静态基场B0的不均匀性通过绝热快速通道(Adiabatic Fast Passage,AFP)的路径补偿,其引起核自旋方向的回转。
通过安装在治疗单元内的具有电子监测装置的接收线圈可以光学和声学地监测核自旋治疗场符合规定的运行、治疗参数、金属等有影响的干扰因素在治疗区域内的补偿、施加到体内的能量吸收剂量的场强和强度变化、所需的共振条件以及其他预设的重要检验参数,并用于质量、效率和可靠性的后续调节过程。
已经证实,借助核磁共振治疗既可例如在治疗皮肤结缔组织缺陷和蜂窝组织炎方面取得美容效果,也可在骨关节、器官、带、肌肉、肌腱以及术后伤口愈合、皮肤烧伤领域内的细胞功能障碍或退化、骨区域内的新陈代谢或供血障碍、骨折愈合等所有领域内以及其他可再生领域内取得医治效果。
在科学界,本体信号和电学过程占据越来越重要的地位。最固有的信号例如为EKG或EEG。
研究显示,细胞再生领域内的压电过程是必须的(ATP),以补偿或获取障碍的细胞功能(Steinecker-Frohnwieser B,Weigl L,Weberhofer G,Kullich W,Kress HG所著的The Influence of Nuclear Magnetic Resonance Therapy(NMRT)and Interleukin IL1-b Stimulation on Cal 78Chondrosarcoma Cells and C28/I2Chondrocytes(核磁共振疗法(NMRT)和白介素IL1-b刺激对Cal 78软骨肉瘤细胞和C28/I2软骨细胞的影响),JOrthopedics Rheumatol,2014;1(3):9)。
其缺失或产生得不足(压和拉过程的动能缺失)造成退化效应,这可通过耦合的核磁共振场很高效并有效地补偿。
其基础在于在共振中保持至所选细胞群(软骨细胞、骨细胞、皮肤细胞、器官细胞等)再生位置的传递路径。
研究提供证据证明,通过核磁共振方法的监测和调控电子装置,核磁共振治疗之后的氧含量(机体缺氧)和细胞ATP十分显著地提升(参见Steinecker-Frohnwieser等),根据控制样本的情况,细胞分裂速度可能加快或减慢(N.Budny;Kom.LeiterProf.Dr.med.D.Palmes,Prof.Dr.med.Dipl.–Ing.H.U.Spiegel;Inaugural-Dissertationzur Erlangung des doctor rerum medicinalium der Medizinischen der Wilhelms- Münster(在明斯特的威斯特伐利亚·威廉斯大学的医学院获得医学博士的就职论文),Klinik für Allgemein-undViszeralchirurgie,Abteilung Chirurgische Forschung, Münster,Deutschland;Oktober 2015)。
核磁共振针对性的治疗成功的决定性因素是精确确定要再生的各个细胞群的细胞共振窗口。
不同的细胞群具有不同的低频共振相应样本,这在核磁共振治疗时利用。
当前核磁共振治疗的重点为忍受支撑和运动器官退化的变化困扰、例如关节炎、骨质疏松症以及运动和意外伤害等的疼痛患者。
因此,例如文章:KULLICH W.,J.OVERBECK,H.U.SPIEGEL(2013):One-year-surveywith multicenter data of more than4,500patients with degenerative rheumaticdiseases treated with therapeutic nuclear magnetic resonance(具有医治性核磁共振治疗的4500多例退化的风湿疾病患者的多中心数据的一年期调查).J BackMusculoskelet Rehabil 26,93-104描述了通过核磁共振疗法治疗的风湿患者的疼痛症状的显著改善。
另外还证实,治疗有效性还与均匀的场的调制频率、治疗次数、治疗时长以及间隔、即多次治疗之间的间隔相关。
发明内容
相比之下,本发明的目的是提供一种用于设置、尤其有针对性地监控、数值探测、后续调节以及校准核磁共振治疗仪器的方法,其中可以根据适应征以更简单,更有效的方式更精确地设置设备参数。
本发明的目的通过根据独立权利要求之一的一种用于设置核磁共振治疗仪器的方法、一种治疗系统以及核磁共振的应用实现。
本发明优选的实施形式和扩展方案由从属权利要求的主体、说明书以及附图中提出。
本发明涉及一种用于设置、尤其有针对性地监控、数值探测、后续调节以及尤其用于校准核磁共振治疗仪器的方法。
本发明尤其涉及用于产生核磁共振的仪器,其中,产生场强小于50高斯、尤其优选小于30高斯,并且调制频率fm小于100Hz的磁场。
垂直辐射的交变场的频率优选小于100kHz并尤其优选小于50kHz。
调制频率fm优选在1至100Hz之间。
根据本发明,确定核磁共振治疗对用户细胞时钟和/或用户生物钟类型(Chronotyp)的影响,并且其中,基于此设置核磁共振治疗仪器。
根据本发明,也可对人类细胞培养基或动物来源的细胞培养基施加核磁共振治疗,并确定核磁共振治疗对细胞培养基细胞的细胞时钟的影响。
尤其可使用人类来源的细胞培养基,例如肝脏细胞或软骨细胞。
肝脏再生领域的研究表明,改变治疗参数可以减慢或加快细胞分裂速度(参见N.Budny,Spiegel等人)。在细胞分裂过程中,观察到信使物质可以决定性地加速细胞分裂以及因此加速再生。
为了设置、尤其校准仪器,确定了核磁共振治疗对相应细胞类型的生物钟的影响。为此目的,使用于执行治疗的参数变化,并根据对生物钟的影响来设置用于治疗的参数。
还已知所有生活在光中的生物以及因此人类都具有内部生物钟,该生物钟使细胞层面的转录和代谢过程能够在多天中在没有例如光等外部刺激的情况下遵守时钟。该生物钟与缺氧信号路径相互关联。已知缺氧诱导因子1(Hif-1α)还对促红细胞生成素的表达负责,以及对适应低氧浓度时发生强烈变化的代谢负责,在此特别是葡萄糖代谢。
此外,还有许多受Hif-1调节的基因,它们在许多细胞回路中起作用,例如在细胞增殖,凋亡或新的血管形成时。

生物钟在哺乳动物中、因此也在人类中是分等级构造的,并且包括位于下丘脑的区域,所谓的视交叉上核(SCN),以及器官专用的和/或组织专用的外周时钟,以及位于生物的所有细胞中并和外周时钟一同与SCN相协调的细胞时钟。内部时钟与外部昼夜节律的同步在此主要通过可见光子的信息实现,可见光子通过眼睛的视网膜传输到SCN,并在那里引起神经元和体液的变化。在包括人类在内的所有生活在光中的生物中,许多生理过程都受到昼夜节律的调节。这尤其包括体温,血压,皮肤电阻和心脏活性,以及激素分泌或器官专有的活性(肝,肾,心脏等的活性分别受特定的昼夜节律影响)。昼夜节律与外部昼夜节律同步对生物有利,并且这种同步的长期干扰(由于轮班工作,时差或睡眠障碍)促使一系列疾病的产生,例如心血管疾病,激素诱发的肿瘤,糖尿病,肥胖症,以及关节炎等疼痛性和炎症性疾病。
细胞生物钟简化的分子机构由多个反馈回路组成,其中的中央反馈回路在下文中进行说明:
在中央负反馈回路中,大脑和肌肉的Arnt样蛋白(Bmall)和“昼夜运动输出周期故障l(Circadian locomotor output cycles kaput1)”(Clockl)蛋白二聚并结合到其对手DNS上的E-box启动子元素上,即基因周期(Perl-3)和隐花色素(Cryptochrom)(cryl/2)。因此,这些基因的mRNA的表达以及由此其在细胞质中的蛋白质产物增加,两种蛋白质都在其中积累并二聚化。然后从那里再次迁移到细胞核中,蛋白质复合物在细胞核中抑制Clock/Bmall基因的表达。负反馈回路以酪蛋白激酶Ιε(CKIε)对PER/CRY复合物的磷酸化作用以及随后在蛋白酶体中的降解而终结。

在分子层面上,缺氧信号路径与内部时钟紧密交织,并且在例如斑马鱼中以及哺乳动物中都是如此。由于两条信号路径显然都是非常保守的过程,因此也可以得出两条信号路径在人类中也存在相互作用的结论。像生物钟本身一样,缺氧信号路径在许多人类疾病中也起着核心作用。在此,首先应提及出现组织中氧饱和度降低的疾病,例如心肌梗塞或中风,或者在正常生理条件下需要的氧饱和度降低的疾病,例如在关节中。在此,首先应提及关节炎,实际上,缺氧信号路径对关节炎的产生和发展的意义已经进行了多次调查和说明。
在缺氧条件下调节和协调细胞代谢的生物学必需的蛋白质是来自缺氧诱导因子(Hif)组的转录因子,其中最重要的代表是Hif-1α。
两条信号路径,即生物钟和缺氧信号路径,还与可用的活性氧自由基(ROS)的量彼此相关,该量在生理条件下又得到非常严格的划分和调节,更确切地说通过例如过氧化氢酶、超氧化物歧化酶、谷胱甘肽过氧化物酶等十分不同的酶类氧自由基清除剂(抗氧化剂),或者也通过所谓的过氧化物还原酶(Peroxiredoxine,PRX)、低分子量蛋白质划分和调节,它们本身就进行昼夜节律的还原和氧化。活性氧自由基(ROS)在炎症和自然细胞衰老过程中的作用已经进行了广泛的讨论和说明,并仍然是当前研究的重点。
现在已经证实,例如斑马鱼成纤维细胞细胞系Z3的昼夜节律基因周期(period)1和隐花色素(cryptochrome)1的振荡通过核磁共振治疗,其相位发生了明显推移。这种相位推移是在完全排除光的情况下发生的(见图1)。此外,氧调节基因如hif-1α和hif-3α的表达在细胞和整个斑马鱼幼体的mRNA层面上均发生了显著变化(图2,A,B,E和F),而同种型(Isoform)hif-2α则完全不受影响(图2,C和D),这又表明了核磁共振对Hif同种型的特异性和选择性作用。在Hif-1α蛋白层面上,可以在细胞培养基中看出核磁共振对蛋白昼夜节律的明显同步作用(图3,A和B),这在4小时的一次治疗中以及重复四次的一小时治疗中都有出现。此外,氧化的过氧化物还原酶(Prx)、活性氧自由基和Hif-1蛋白的量(图4,A,B和C)也以与时段和计量相关的方式向相同方向得到调节,在此情况下减少。
基于该新的认知,能够以可以实现改善的治疗效果的方式来设置和使用用于产生核磁共振的仪器。
例如借助微机芯片卡,通过芯片卡读取器将存储的治疗序列读入控制装置,并确保核磁共振治疗精确、符合规定的流程。
例如可以存储在芯片卡上的多个可能的彼此相继的治疗时间间隔用于治疗仪器的最佳控制设定。
可以任意更改微机芯片卡上的顺序,治疗流程和治疗时间,以在治疗时间期间触发共振效应。这对于触发细胞生物钟的重新同步至关重要。
核自旋治疗场符合规定的运行、治疗参数、场强和强度的变化、引入体内的能量吸收剂量、所需的共振条件和其他预定的重要测试参数可以通过安装在治疗单元内的、具有监测电子装置的接收线圈进行光学和声学监测,并用于质量、有效性和可靠性的后续调节过程。
这种用于优化治疗参数的后续调节可以通过应答回路或反馈回路来实现。
另一监测电子装置测量并监测皮肤区域中的温度变化,以便探测新陈代谢的变化并将其作为优化参数计入治疗流程的控制电子装置的调节和控制过程中。
因此尤其能够以使细胞生物钟同步的方式对仪器进行参数化和使用。这改善了治疗的有效性,尤其是对于关节炎患者的治疗的有效性。
根据细胞时钟的相应相位尤其设置治疗时间点、治疗时长、治疗间隔、即治疗之间的间隔、绝热过程的场的调制频率和/或由用于借助磁场治疗的仪器产生的磁场场强。
根据本发明的一种实施形式,在治疗间隔方面尤其规定,治疗总是以12或24小时的节奏+/-2小时进行。其他治疗间隔也是可能的。
用于该目的的仪器优选具有存储器,在该存储器中保存了参数化数据,例如治疗时长,调制频率fm和/或治疗间隔以及患者数据。
基于此,可以在该仪器中存储治疗计划,从而仪器仅在根据所存储的治疗计划执行治疗时才实施治疗。
根据本发明,对于仪器的校准考虑了个体的生物钟,即用户的生物钟类型。
原则上,人类有两种生物钟类型(具有介于它们之间的运行形式),即所谓的早起型(“百灵鸟”)和晚起型(“猫头鹰”),它们已经在生理上进行了表征。
因此,为了显示用户的个人生物钟,在优选至少24小时,特别优选至少48小时的时间段内的体温、心率、皮肤电阻和/或血压的测量特别适合于此。
基于此可尤其确定治疗间隔和/或治疗的最优时间点。
此外,本发明还涉及一种核磁共振治疗仪器,其以前述方法设置。
此外,本发明还涉及一种具有核磁共振治疗仪器的治疗系统。该治疗系统具有用于确定用户生物钟类型的装置、尤其传感器。
根据本发明的一种实施形式,该传感器构造用于测量用户的体温、血压、皮肤电阻和/或心率。
在本发明的一种实施形式中,设置复合的传感器,其在至少24小时、优选至少48小时的时间段内测量体温、皮肤电阻和心率。
在另一实施形式中,在上述时间段内测量皮肤电阻,优选仅测量皮肤电阻。就此而言请参阅Vidacek等所著的Personality differences in the phase of the circadianrhythms:a comparison of morningness and extraversion(昼夜节律相位的个体差异:早起型和外向型的比较),ERGONOMICS 1988,Vol.31,No.6,873-888。据此已知,皮肤电阻可单独用于确定生物钟类型。
基于测得的数值对生物钟类型的计算可在其中设有传感器的外部单元上、在用于产生核磁共振的仪器的仪器控制装置内或者在其他外部计算单元上进行。
重要的是,用于产生核磁共振的仪器的仪器控制装置提供关于用户生物种类型的信息。
根据一种实施形式,传感器将数据无线地传递至仪器控制装置。
根据另一实施形式,存储在包含传感器的外部单元内测得的数据并通过连接外部单元尤其传递至仪器控制装置。
关于用户生物钟类型的信息可保存在外部存储器上,但也可保存在核磁共振治疗仪器的仪器控制装置的存储器内。
用于产生核磁共振的仪器优选通过移动存储器、尤其通过具有存储器区域的复合微处理器芯片卡控制。通过其,通过多个时间上可变的可编程的程序阶段触发生物钟再触发的生物学过程。
治疗流程优选自动化进行并经过多个时间上确定的程序阶段。
治疗数据和治疗时间优选暂存在控制装置内,从而在停电时能够继续进行治疗,而不出现治疗空缺或偏差。
在另一实施形式中,存储通过传感器在医治之前在患者处测量生物学或电学的患者数据,接着作为参考预设或治疗预设而直接用于治疗流程,或者在医治开始之前读入控制装置内。
在设置仪器时,可通过传感器数据考虑到用户的个人生物钟。尤其可根据获取的传感器数据确定治疗时间、治疗时长、治疗间隔和/或调制频率fm。
本发明涉及核磁共振用于用户细胞生物钟的同步和/或重计的应用。已经证实,尤其周期1和隐花色素l mRNA振荡的相位能够通过有针对性的核磁共振治疗实现推移,并且由此能够同步生物钟。此外,细胞层面的Hif-1α蛋白质的昼夜节律的振荡也通过核磁共振同步。此外,对Hif-1、Prx和活性氧自由基的量也可产生与时间和剂量相关的改变,其对于人类病理生理学的影响已经进行过说明(参见上文)。
根据本发明的核磁共振的应用既涉及纯美容的治疗,尤其蜂窝组织炎的治疗,也涉及医治的、尤其术后的治疗。
核磁共振方法能够在骨关节、器官、韧带、肌肉、腱区域内细胞功能紊乱或退化的所有领域内,以及在术后伤口愈合、皮肤烧伤、骨区域内的新陈代谢或血液循环障碍、骨折愈合等、以及其他可再生领域内都非常高效和有作用地使用。
根据期望的目的,根据本发明以这样的方式设置或读取核磁共振治疗的参数,特别是治疗时间点、治疗间隔、治疗时长和/或调制频率fm,从而实现最大相位推移或最小相位推移,或者说生物钟的最佳同步。
附图说明
下文中参照附图图1至图5进一步阐述本发明的主体。
图1中示出了随时间绘制的内部时钟的中央反馈回路的代表性基因的mRNA拷贝数,其反应了内部时钟的振荡。其中示出了隐花色素1、周期1、周期2和时钟1的振荡。可见未进行治疗的细胞与在相继的四天内的相同时间分别核磁共振治疗了一小时的细胞之间的波动的比较。明显可见相位的推移,其在基因周期1和隐花色素1中显著(cosinor fitanalysis,GraphPad Prism 6.0)。
图2示出了未进行治疗的细胞与相继的四天内在相同时间分别核磁共振治疗了一小时的细胞之间来自缺氧信号路径的hif-1α和hif-3α的mRNA浓度变化(A和E),或按照相同的治疗方案对整个斑马鱼幼体中的hif-1α和hif-3α的mRNA振荡进行的选择性同步(B和F),而hif-2α在细胞和整个斑马鱼幼体中均不受影响(图2,C和D)。
图3示出了在重复四次的每次一小时的治疗之后(A)或在一次四小时的治疗之后(B),由核磁共振产生的斑马鱼细胞中的昼夜节律Hif-lα蛋白振荡的同步。
图4示出了氧化的过氧化物还原酶(A)、活性氧自由基(B)和Hif-1α蛋白质(C)的减少的且与剂量相关的量。与仅一小时的治疗相比,4小时的治疗促使蛋白质的量、或活性氧自由基的量降低。
图5是用于设置核磁共振治疗仪器的方法的示意性流程图。
图6是核磁共振治疗仪器的示意图。
具体实施方式
参照图1至图6中的图表阐述核磁共振治疗对不同生物参数的影响。
在图1和2中分别在x轴上记录了时间,并且在y轴上记录了相对于16ng的总RNA的mRNA比拷贝数。分别对比未进行治疗的控制组以及在彼此相继的四天中分别借助治疗仪治疗一小时的细胞培养基。在此,治疗分别在相同的时间进行。
为此,使用斑马鱼成纤维细胞组成的细胞培养基。
在图1中示出了隐花色素(cryptochrome)1基因、周期(period)1基因、周期2基因和时钟(clock)1基因的mRNA振荡。
可见,相比于未进行治疗的控制组细胞,周期1基因和隐花色素1基因的振荡推迟了约两小时。由此实现了在没有外部光影响的条件下推移该基因振荡的相位。治疗组之间存在通过余弦曲线调整(Graphpad Prism 6)进行统计学验证的明显区别。
图2示出了随着时间的hif(缺氧诱导因子)-1α基因、hif-2α基因和hif-3α基因的mRNA量。
可见,同种型(Isoform)hif-1α和hif-3α通过核磁共振,在细胞层面上(A和E)以及在整个机体中(B和F),得到选择性地控制,而hif-2α在细胞内以及整个斑马鱼幼体中都保持未受影响。
图3示出了随着时间的细胞Hif-1α蛋白质量。可见,在两种治疗变体(一小时多次[A]-对比一次四小时治疗[B])中,核磁共振都促使Hif-1α蛋白质表达的昼夜节律同步。
图4示出了核磁共振功效与剂量的关系:相比于仅一次一小时的治疗,4小时的一次治疗显示出氧化Prx(A)、活性氧自由基(B)和Hif-1α(C)的量的显著降低。
图5示出了根据本发明的一个实施例的核磁共振治疗仪器的设置方法的流程图。
首先,通过皮肤电阻、血压、体温和/或心率确定患者生物钟的相位,即获取个人生物钟类型(Chronotyp)。
基于此而根据生物钟类型设置治疗时长、一系列治疗的时间间隔、治疗的时段和/或核磁共振治疗仪器的调制频率fm。
基于该设置进行多次治疗。在此,既可涉及医治性的治疗,也可涉及美容的治疗。
图6示出了根据本发明的用于借助核磁共振治疗用户的系统6的示意图。系统6包含用于在待治疗的组织中产生核磁共振的仪器1。
在此示意性示出的仪器1包含线圈2和3,其以亥姆霍兹(Helmholtz)布置彼此相对,并且借助该线圈产生均匀的磁场,该磁场沿着标记的x轴方向延伸。
为了产生核磁共振,借助线圈4垂直于由线圈2和3产生的均匀磁场辐射交变磁场。
协调其频率,从而达到拉莫尔条件。
此外,由线圈2和3产生的均匀的场由基量和调制量组成,该调制量借助例如矩形的调制频率fm调制。
通过调制,在治疗总量内,在每次经过中产生至少一次共振条件。
仪器1包含存储器,用户数据、治疗时长、治疗间隔和调制频率fm的数据保存在该存储器中。
根据待治疗的人员的生物钟类型执行治疗时段、治疗时长、治疗间隔和/或调制频率。

仪器1优选包含仪器控制器,其也包含具有所述数据的存储器,其中,仪器控制器优选自动地、例如基于计算程序或基于数据库而算出优化的治疗参数,将其对应至相应的用户数据,保存到存储器中。
通过治疗仪器参数基于此的设置能够以简单的方式改善核磁共振治疗的功效。
可随意选择地改变、控制和后续调节微机芯片卡上的序列、治疗流程和治疗时间,以在治疗时间期间触发共振效应。这对于ATP以及细胞缺氧的十分显著的上升以及对于细胞生物钟再同步化的触发是十分重要的。
试验显示,免疫系统区域能够造成增强的防御并能够由此消除退化的细胞功能。
Claims (14)
1.用于设置、尤其校准核磁共振治疗的仪器的方法,其中,确定核磁共振治疗对细胞培养基、用户的细胞时钟和/或用户的生物钟类型的影响,并且其中,基于此设置核磁共振治疗的仪器。
2.根据前述权利要求所述的方法,其特征在于,根据细胞时钟设置治疗时长、治疗间隔、治疗时段、场的调制频率和/或由借助磁场治疗的仪器产生的磁场的场强。
3.根据前述权利要求中任一项所述的方法,其特征在于,设置核磁共振治疗的仪器,使得达到特定的蛋白质表达的最大值或最小值,和/或生物钟的最大的或最小的相位推移,和/或生物钟的同步。
4.根据前述权利要求中任一项所述的方法,其特征在于,为了设置核磁共振治疗的仪器而确定用户的生物钟类型并根据该生物钟类型设置该仪器。
5.根据前述权利要求中任一项所述的方法,其特征在于,通过温度测量、血压测量、皮肤电阻测量和/或确定心率而确定生物钟。
6.核磁共振治疗的仪器,该仪器借助前述权利要求中任一项所述的方法设置。
7.具有核磁共振治疗的仪器、尤其根据前述权利要求的仪器的治疗系统,其特征在于,所述治疗系统具有用于确定用户的生物钟类型的装置、尤其传感器。
8.根据前述权利要求中任一项所述的治疗系统,其特征在于,所述传感器构造用于测量用户的体温、血压、皮肤电阻和/或心率。
9.根据前述权利要求中任一项所述的治疗系统,其特征在于,所述传感器构造用于优选无线地将数据传递至仪器控制装置。
10.根据前述权利要求中任一项所述的治疗系统,其特征在于,所述传感器为外部单元的一部分。
11.核磁共振用于改变、尤其同步用户细胞的生物钟的应用。
12.根据前述权利要求所述的应用,其特征在于,针对医治的和/或美容的目的改变所述生物钟。
13.根据前述权利要求中任一项所述的应用,其特征在于,改变用户细胞的缺氧信号路径。
14.根据前述权利要求中任一项所述的应用,其特征在于,改变用户细胞的活性氧自由基的量。
