背景技术
创伤后应激障碍(PTSD)是一种严重的神经精神疾病,影响全世界1–15%的个人,其中包括10–60%的退伍军人。PTSD的诊断可以使用本领域标准教科书中描述的标准来确定,例如美国精神病学协会出版的《精神障碍诊断与统计手册第五版(Diagnostic andStatistical Manual of Mental Disorders-fifth edition)》(DSM-5)。
PTSD被定义为特征性和持续性的症状的发展,伴随着暴露于已知会引起严重压力的情况(包括死亡、威胁性死亡、实际或威胁性严重伤害或实际或威胁性性暴力)下的功能困难(difficulty functioning)。压力情况可能直接或间接地被经历。

PTSD的症状也可以归类为(categorized as)重新经历、主动回避(activeavoidance)、情绪麻木(emotional numbing)、烦躁觉醒(dysphoric arousal)和焦虑觉醒(anxious arousal)。重新经历既可以指积极事件,也可以指消极事件。正面和负面的疏离(dissociation)都可能发生。
PTSD的症状也可以用诸如《事件影响量表修订版(Impact of Event Scale-Revised)》之类的量表分为侵入性(intrusion)、回避性(avoidance)和过度觉醒性(hyperarousal)类别。PTSD症状的其他细分也可以提出。
PTSD症状严重程度的差异可能会在个体之间被观察到。这些症状类型可能对特定类型的药理或行为干预有不同的应答。
除了上述症状外,患有PTSD的个体自杀率也有所增加。类似地,PTSD患者的重度抑郁症(major depression)患病率也明显升高。
在被诊断为PTSD的个体中,终生自杀未遂风险可能高达25%。自杀的可能性可以使用诸如《哥伦比亚自杀严重性等级量表》之类的标准工具进行评估。对于自杀意念(suicidal ideation)和自杀行为,可能会获得单独的评分。
重度抑郁症是一种临床综合征,包括持续的悲伤情绪或对活动的兴趣丧失,在缺乏治疗的状况下会持续至少两周。重度抑郁的症状通常使用汉密尔顿抑郁量表(HAM-D)、蒙哥马利阿斯堡抑郁评定量表(MADRS)或贝克抑郁自评量表(BDI)等量表进行测量。除包括与抑郁情绪有关的症状外,HAM-D还包含精神病敏感的症状,包括的项目有罪恶感、去个性化/去现实化和偏执狂。可以在诸如强迫游泳实验的动物模型中研究抑郁症。

发明内容
本文描述了包含NMDAR拮抗剂的制剂(formulations),在某些实施方案中,其与抗抑郁或抗精神病剂组合,以及用此类药剂治疗PTSD和PTSD症状的方法。
在一个特别的实施方案中,NMDAR拮抗剂可以选自NMDAR的竞争性拮抗剂。在其他实施方案中,NMDAR拮抗剂可以选自在甘氨酸、谷氨酸或氧化还原/多胺识别位点处用作拮抗剂的试剂。在其他实施方案中,NMDAR拮抗剂可以是非选择性拮抗剂或在NMDAR处含有特定亚基(subunit)(例如NR2A或NR2B亚基)的选择性拮抗剂。

一、术语

D-环丝氨酸,或称DCS,是指化学物D-环丝氨酸(CA索引名:3-Isoxazolidinone,4-amino-,(4R)-(9CI);CAS注册号68-41-7),或其药学上可接受的盐。DCS为FDA(美国食品药品管理局)批准的用于治疗肺结核的药物,并由Eli Lilly and Company以商标名销售。DCS为D-丙氨酸的结构类似物,且为兰花链霉菌(Streptomyces orchidaceus)和S.garphalus的某些菌株产生的广谱抗生素。
二、治疗PTSD的方法
本文描述了用于治疗PTSD,及其症状(例如重新经历、回避和/或增强的觉醒)的方法中的口服或肠胃外给药方案。
所描述的方法的一个实施方案包括向有需要的受试者施用治疗有效量的N-甲基-D-天冬氨酸受体(NMDAR)拮抗剂。
在另一个实施方案中,本文提供了用于治疗PTSD或其症状的方法中的口服剂量方案,其中该剂量方案(dosage regimen)包括两种治疗剂(活性成分)。第一种活性成分是NMDAR拮抗剂,第二种是抗抑郁或非典型抗精神病剂。

Hung et al.,2014(Hung WY,Yu MC,Chiang YC,Chang JH,Chiang CY,Chang CC,Chuang HC,Bai KJ.Serum concentrations of cycloserine and outcome ofmultidrug-resistant tuberculosis in Northern Taiwan.Int J Tuberc LungDis.2014;18(5):601-6)评估了DCS临床治疗期间的PK水平。受试者平均剂量为8.8mg/kg,大多数受试者(n=27)每天接受500mg的DCS,少数受试者750mg/d(n=4)或250mg/d(n=2)。给药后2和6小时的DCS浓度为19.7和18.1微克/mL。

在所述方法的特别的实施方案中,第二种治疗剂(与NMDAR拮抗剂联合使用)是本文所述的任何药剂,例如四环抗抑郁药(TeCA)、选择性5-羟色胺再摄取抑制剂(SSRI)、5-羟色胺/去甲肾上腺素再摄取抑制剂(SNRI)、去甲肾上腺素和特异性血清素(NaSSA)、非典型抗抑郁药、5-HT2A受体拮抗剂、经批准用于治疗抑郁症的非典型抗精神病药或其组合。

在所描述的方法的一些实施方案中,受试者还患有自杀倾向(suicidality),包括自杀意念或行为。在一些实施方案中,本发明提供了用于降低自杀倾向严重程度以及PTSD症状的方法。但是,应该强调的是,患有PTSD且可以从所述方法中受益的患者并不一定也患有自杀倾向。
如上所述,在特别的实施方案中,NMDAR受体拮抗剂用于单药(single-agent)制剂中,用于治疗PTSD或其症状。在这样的实施方案中,以治疗所述受试者时,被认为对于治疗所述受试者的抑郁症次最优(suboptimal)的剂量来施用NMDAR拮抗剂。
用于所述方法的药物组合物可以通过几种途径中的任一种或组合施用给受试者,所述途径例如口服、静脉内、经粘膜(例如鼻、阴道等)、肺、经皮、眼、颊、舌下、腹膜内、鞘内、肌内或长效持久制剂(long term depot preparation)。用于口服的固体组合物可包含合适的载体或赋形剂,例如玉米淀粉、明胶、乳糖、阿拉伯树胶、蔗糖、微晶纤维素、高岭土、甘露醇、磷酸二钙、碳酸钙、氯化钠、脂质、藻酸或可控缓慢释放的成分。可以使用的崩解剂包括但不限于微晶纤维素、玉米淀粉、羟乙酸淀粉钠和藻酸。可以使用的片剂粘合剂包括但不限于阿拉伯胶、甲基纤维素、羧甲基纤维素钠、聚乙烯吡咯烷酮(聚维酮)、羟丙基甲基纤维素、蔗糖、淀粉和乙基纤维素。
可以配制组合物以减少D-环丝氨酸或其他成分的胃降解,例如通过应用可降低pH依赖性胃水解的肠溶包衣剂。还可以使用标准方法将组合物配制成用于延长释放。
在水或其他水性溶剂(aqueous vehicles)中制备的用于口服的液体组合物可以包括溶液、乳剂、糖浆和酏剂(elixirs),其中含有活性化合物、润湿剂、甜味剂、着色剂和矫味剂。可以通过常规方法制备各种液体和粉末组合物,以使其可被吸入待治疗患者的肺中。
注射用组合物可包含各种载体,例如植物油,二甲基乙酰胺、二甲基甲酰胺、乳酸乙酯、碳酸乙酯、肉豆蔻酸异丙酯、乙醇、多元醇(甘油、丙二醇、液态聚乙二醇等)。对于静脉内注射,可以通过滴注法施用化合物,由此将包含活性化合物和生理上可接受的赋形剂的药物组合物注入(infused)。生理上可接受的赋形剂可以包括,例如5%葡萄糖、0.9%盐水、林格氏溶液或其他合适的赋形剂。对于肌肉内制剂,可以将化合物的合适可溶性盐形式的无菌组合物溶解,并在药物赋形剂(例如注射用水,0.9%盐水或5%葡萄糖溶液或可溶于麻油的药性持久形式(depot forms)的化合物(例如癸酸盐(decanoate)、棕榈酸盐(palmitate)、十一碳烯酸(undecylenic)、庚酸盐(enanthate))中给药。或者,可以将药物组合物配制成口香糖,棒棒糖等。
用所述方法进行治疗的受试者可经历PTSD症状的显著改善。在一些实施方案中,如通过任何临床公认的PTSD评价方法(例如CAPS量表)测定的那样,相对于用PTSD的替代疗法治疗的受试者,按照所述方法治疗的受试者将经历更大的改善或更持久的改善。

实施例
实施例1:NMDAR拮抗剂和抗抑郁药治疗PTSD
本实施例表明,NMDAR拮抗剂出乎意料地减少了与PTSD相关的重新经历、回避和增强的觉醒症状。特别地,以旨在产生NMDAR拮抗剂作用的剂量给予的D-环丝氨酸出乎意料地减少了与PTSD有关的重新经历、回避和增强的觉醒症状。用靶向谷氨酸结合位点的竞争性NMDAR拮抗剂并未观察到类似效果。
近亲交配的(Inbred)Wistar-Kyoto(WKY)大鼠已被用作几种行为缺陷的模型,例如压力脆弱性和睡眠障碍(sleep disturbance)。WKY大鼠在施加轻度电击后也表现出持久的回避。在我们关于恐惧条件(fear conditioning)的验证研究中,与Wistar大鼠相比,WKY大鼠在恐惧绝种(fear extinction)方面表现出明显的缺陷。这些证据表明,WKY大鼠可能是筛选具有治疗PTSD精神疾病潜能的化合物的理想模型。
恐惧记忆在精神疾病(如PTSD)中至关重要。巴甫洛夫式恐惧条件(FC)测试是啮齿动物中一种广泛使用的行为分析方法,用于通过厌恶刺激(轻度电击)与特定暗示(例如音调(tone),暗示调节(cued conditioning))或发生触电的环境(环境条件)。建立关联后,条件刺激(conditioned stimulus,CS,即音调或情境(context))可以以类似于非条件刺激(unconditioned stimulus,US,即电击)的方式诱发恐惧反应。但是,如果CS在没有US组合的情况下反复出现,则正常动物会出现消退(extinction)。在没有US的情况下的反复或长时间CS表现,恐惧消退(fear extinction)通常被视为一种新的抑制性学习,这会导致条件反应(即FC测试中的冻结反应)的大小(magnitude)和/或频率逐渐降低。这种抑制性学习的缺陷可以被认为是临床PTSD的标志(hallmark)。
研究是由07645美国新泽西州蒙特维尔(Montvale)飞利浦公路(PhilipsParkway)100号的PsychoGenics Inc.进行的。
本研究使用从Charles River Laboratory购买的年轻成年(约7周龄),雄性WKY和Wistar大鼠。收到后,将动物分组饲养(每笼2只)并适应(acclimated)至少7天。在开始研究之前,对所有大鼠进行检查,处理和称重,以确保足够的健康和适合性(suitability)。在研究过程中,维持12/12光照/黑暗(light/dark)周期。室温保持在20至23℃之间,相对湿度保持在50%左右。将大鼠随机分配至每个治疗组。所有测试均在动物的光照(light)周期中进行。
以下化合物用于本研究:
·将30mg/kg(低剂量)和300mg/kg(高剂量)的D-环丝氨酸溶解于盐水中,并在消退测试(extinction test)前30分钟腹腔注射(IP)给药,剂量为2ml/kg。
·将舍曲林20mg/kg溶解于盐水中,并在试验前30分钟腹腔注射给药,剂量为2ml/kg。当与DSC组合使用时,在消退测试前30分钟以鸡尾酒(cocktail)的形式给予该组合
·将D-CPP-ene(5mg/kg)溶于酸化盐水中,并在消退测试前30分钟皮下注射给药,剂量为2ml/kg
·将卢拉西酮(Lur),0.2mg/kg溶于生理盐水,在测试前30分钟腹腔注射给药,剂量为1ml/kg
·将喹硫平(QTP,10mg/kg)溶于酸化盐水中,并在测试前30分钟皮下注射给药,剂量为1ml/kg
实验是在Coulbourn Instruments(美国宾州)制造的恐惧条件系统(fearconditioning system)中进行的。动物的身体运动利用视频系统和FreezeFrame软件捕获,并通过FreezeView软件(美国宾州Coulbourn Instruments)自动分析凝滞(freezing)情况。
恐惧条件训练:将动物暴露于5个CS-US对:10秒音调(tone)(70dB条件刺激/CS)与短暂的电击(0.5mA,1.0秒非条件刺激/US)同时终止。5个CS-US对以60秒的间隔分开,第一个CS为120秒。在最后一次电击后,老鼠们在(stage)箱子中一分钟,然后被移出。
消退训练:FC训练后24和72小时进行两次消退训练。使用恐惧条件反射消退模型(cued FC extinction paradigm)。消退训练是在“圆柱(cylinder)”中进行的,这是与FC训练箱不同的变化了的情境。消退训练持续了32–40分钟,包括8–10个相同的箱子(bin)。每个箱子包括2分钟的音调周期,然后是2分钟的非音调周期(试验间隔,ITI)。
在消退训练的箱7–8中(24小时和72小时)的凝滞百分比被用于评估治疗的有效性。通过单因素方差分析(ANOVA)分析数据,并将治疗/组和消退阶段(暗示(cues)或箱位(bins))设置为因素。如果适用,在事后比较中使用Fisher LSD方法。
WKY与对照组(Wistar)大鼠的区别在于,在第1天和第3天给予暗示后,其凝滞行为的程度不同,表现为对>85%的刺激表现出凝滞行为的动物数量存在显着差异(F=24.6,df=1,76,p<0.0001)(表1)。
仅对WKY大鼠进行了药理学研究,如表2所示。不同天数(across days)的凝滞行为有非常显著的治疗效果(F=9.24,df=9362,p<0.0001)。天数的影响微不足道(F=3.24,df=1,362,p=0.07)。每天的治疗效果不显著(F=0.99,df=9362,p=0.45)。在事后测试中,高剂量(300mg/kg)DCS(LSD p<0.001),单独的舍曲林(LSD p=0.023)和高剂量DCS+舍曲林的组合(LSD p=0.023),相对于溶剂组,均显著降低了不同天数的凝滞行为。相比之下,低剂量DCS(30mg/kg)对凝滞行为没有显著影响(p=0.3)。低剂量和高剂量DCS的效果差异在趋势水平上显著(p=0.08)。
与高剂量DCS相反,竞争性NMDAR拮抗剂D-CPPene(p=0.009)和非典型抗精神病药卢拉西酮(p=0.1)和喹硫平(p=0.009)倾向于增加凝滞行为。当高剂量DCS加入卢拉西酮(p=0.09)时,往往会减少凝滞行为,但与喹硫平联用(p=0.3)则无统计学意义。
这些发现代表了第一个实证:NMDAR拮抗剂可以减少啮齿动物的重新经历、回避和增强的觉醒(PTSD相关)行为。值得注意的是,D-环丝氨酸的抗PTSD效果,在啮齿类动物中是通过给予300mg/kg的剂量(即已知可产生NMDAR拮抗作用的D-环丝氨酸剂量),选择性诱导出来的,但剂量为30mg/kg(即已知产生NMDAR激动剂作用的D-环丝氨酸剂量)时则未观察到相应效果。而且,在存在SSRI舍曲林的情况下该效果持续存在,但在5-HT2A受体拮抗剂卢拉西酮或喹硫平的存在下则不存在。竞争性NMDAR拮抗剂D-CPPene对凝滞行为增加而不是减少,表明NMDAR功能的甘氨酸位点与谷氨酸位点拮抗作用出乎意料的有益作用。

实施例2:抗抑郁药可逆转NMDAR拮抗剂拟精神药物的作用
先前的实施例显示了使用NMDAR拮抗剂和抗抑郁药联合治疗PTSD症状。但是,NMDAR拮抗剂治疗的一个已知问题是增加了精神病的倾向(liability),这可以使用行为亢进试验在啮齿动物中建模。在该实施例中,显示了NMDAR拮抗剂和抗抑郁药联合治疗的出乎意料的协同作用。
对于本研究,在存在或不存在抗抑郁药的情况下,在D-环丝氨酸给药后,使用啮齿动物旷场试验(open field test)评估D-环丝氨酸的精神运动(psychomotor)作用。
所有测试均在10591美国纽约州塔里敦(Tarrytown)老锯木厂河路(Old Saw MillRiver Road)765号的PsychoGenics Inc进行。
使用了来自缅因州巴港(Bar Harbor)Jackson Laboratories的雄性C57BL/6J小鼠(8周大)。收到后,为小鼠分配唯一的识别号(尾标),并分组饲养在OPT小鼠笼(OPTImice)中。在测试之前,使所有动物适应新居住室(colony room)1周。在适应期间,定期检查动物,处理并称重以确保足够的健康和适合性。将动物维持在12/12光照/黑暗周期。室温保持在20至23℃之间,相对湿度保持在30%至70%之间。在研究期间随意提供食物和水。所有测试均在动物的光照周期阶段进行。
测试的化合物包括:
·将D-环丝氨酸(300mg/kg)溶解在PTS溶剂(5%PEG200:5%吐温80:90%NaCl)中,并在旷场试验中以10mL/kg的剂量腹腔注射给药。
·将安非他酮(10mg/kg)溶解于盐水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将去甲丙咪嗪(10mg/kg)溶解于盐水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将舍曲林(20mg/kg)溶解于无菌水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将文拉法辛(40mg/kg)溶解于盐水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将度洛西汀(40mg/kg)溶解于盐水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将氟西汀(10mg/kg)溶解于盐水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将丙咪嗪(10mg/kg)溶解于盐水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将西酞普兰(10mg/kg)溶解于盐水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将左旋米那普仑(40mg/kg)溶解于无菌水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将米那普仑(40mg/kg)溶解于无菌水中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将维拉唑酮(1mg/kg)溶解在PTS溶剂(5%PEG200:5%吐温80:90%NaCl)中,并在旷场试验中,
在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
·将沃替西汀(10mg/kg)溶解在PTS溶剂(5%PEG200:5%吐温80:90%NaCl)中,并在旷场试验中,在D-环丝氨酸前30分钟,以10mL/kg的剂量腹腔注射给药。
使用被有机玻璃(16×16×16)包围的Plexiglas方室(square chamber)(27.3×27.3×20.3cm;佛蒙特州圣奥尔本斯(St Albans)Med Associates Inc.)进行旷场视野(OF)测试,以测量水平和垂直活动。在测试之前,将小鼠带入活动实验室至少适应实验室条件1小时。给动物施用溶剂或测试化合物并置于OF中。为了评估D-环丝氨酸的作用,在进入测试室之前先给小鼠注射DCS,并监测其活动60分钟。或者,在用苯丙胺(4mg/kg)或苯环己哌啶(5mg/kg)挑战前30分钟施用溶剂或D-环丝氨酸,并在施用溶剂或D-环丝氨酸后,总计60分钟的活动情况。对于其他情况,用溶剂或抗抑郁药处理动物,然后记录30分钟的基线活性。之后小鼠接受DCS注射,并放回OF室中60分钟。在每次OF测试结束时,将OF腔室彻底清洁。
数据通过方差分析(ANOVA)进行分析,然后根据需要使用Fishers LSD测试进行事后比较。如果p<0.05,则认为效果显着。
结果:在以30至1000mg/kg的剂量施用溶剂或D-环丝氨酸30分钟后,评估剂量反应(dose-response)。在所有条件下,效果都非常显著(F=19.0,df=3,35,p<0.001)。以30mg/kg的剂量施用D-环丝氨酸对自主活动(Locomotor activity)没有明显影响(p=0.5),但300mg/kg(p<0.001)和1000mg/kg(p<0.001)剂量的D-环丝氨酸显著提高了自主活动(表3)。

当根据药物类型区分不同条件时,包括NDRIs(安非他酮)、三环抗抑郁药(去甲丙米嗪、丙米嗪)、与高血清素转运(serotonergic transport)(SERT)抑制活性相关的SSRIs/SNRIs(舍曲林、文拉法辛、度洛西汀、氟西汀、西酞普兰),相对于涉及到其他靶标如去甲肾上腺素转运体(norepinephrine transporters)(左旋米那普仑、米那普仑、维拉唑酮、沃替西汀)的与低血清素转运抑制活性相关的新型药剂,观察到药物类别之间出乎意料的差异(表4)。

SERT=血清素转运(serotonin transport)
这是据我们所知的首次研究,表明D-环丝氨酸可在啮齿动物中自行产生自主活动亢进,与其临床拟精神病作用一致。结合先前的研究表明以160mg/kg的剂量给予D-环丝氨酸没有明显的作用(Carlsson et al.,J Neural Transm 95:223-233,1994),这些发现表明,在血浆水平超过25微克/毫升时,优先地观察到了拟精神病药物的作用(参见实施例3)。
这也是据我们所知的首次研究,表明在D-环丝氨酸存在下,抗抑郁药对自主活动亢进具有不同的作用,相对于传统的SSRI/SNRIs或安非他酮(多巴胺-去氧肾上腺素再摄取抑制剂)而言,对诸如TCAs或新型的抗抑郁药维拉唑酮、沃替西汀、米那普仑和左旋米那普仑等药物具有优先的作用。
新型SNRI和非典型抗抑郁剂与传统药物的不同之处在于,它们对5-羟色胺转运蛋白以外的靶标具有更高的特异性,因此与TCA相对更相似。这些发现出乎意料地表明,新型SNRI/非典型抗精神病药与血浆水平>25微克/mL的D-环丝氨酸剂量联合使用,具有优选的有益活性,因此包含高剂量D-环丝氨酸和新型抗精神病药联合使用,显示了出乎意料的效用。
实施例3:DCS和米氮平对啮齿类动物埋珠行为的协同作用
埋珠被用作焦虑症(包括PTSD)和强迫症(OCD)的模型,并且与主动回避和焦虑觉醒的构造最相关。
单独放置在装有玻璃珠的笼子中,已证明老鼠会掩埋玻璃珠。诸如苯二氮卓类(benzodiazepines)抗焦虑药会降低啮齿类动物的埋珠行为。在这里,我们假设NMDAR拮抗剂和抗抑郁药将对埋珠产生协同作用。
在测试之前,将小鼠带入活动实验室至少一小时以适应实验室条件。将小鼠分别放置在干净的小鼠笼子中,该笼子包含约6cm的硬木垫料(hard wood bedding),和二十个黑色玻璃珠,玻璃珠间距摆放成5行,测试进行30分钟。测试过程中通过相机捕获行进距离,并使用Video Tracker软件(法国ViewPoint Life Sciences Software)进行量化。在测试结束时,将小鼠从笼子中移出并计数未埋玻璃珠的数量。如果玻璃珠至少被三分之二覆盖,则被认为已埋。
使用以下化合物。所有化合物以10ml/kg的剂量给药:
·将D-环丝氨酸(Sigma,DSC;30和300mg/kg)溶于5%PEG 200:5%吐温80:90%盐水(PTS)中,并在测试前30分钟,以10ml/kg的剂量腹腔注射给药。
·将米氮平(Sigma,5.5mg/kg)溶于5%PEG 200:5%吐温80:90%盐水(PTS)中,并在测试前30分钟,以10ml/kg的剂量腹腔注射给药。
·帕罗西汀(Sigma,5mg/kg)在埋珠测试中用作阳性参考。将该化合物溶于20%环糊精中,并在测试前30分钟,以10ml/kg的剂量腹腔注射给药。
·在测试前30分钟,以单次注射的形式,将DSC(300mg/kg)+米氮平(5.5mg)的组合以鸡尾酒形式腹腔注射,剂量为10ml/kg。
在以下每个测试组中测试了10只小鼠:
·溶剂(5%PEG 200;5%吐温80;90%盐水)
·帕罗西汀(5mg/kg)
·米氮平(5.5mg/kg)
·D-环丝氨酸(30mg/kg)
·D-环丝氨酸(300mg/kg)
·D-环丝氨酸(300mg/kg)+米氮平(5.5mg/kg)
研究结果如图1所示。单因素方差分析显示了显著的治疗效果。事后比较表明,与溶剂相比,帕罗西汀(5mg/kg)和米氮平(5.5mg/kg)显著减少了被埋藏的玻璃珠数量。用任一剂量的D-环丝氨酸(30mg/kg和300mg/kg)治疗动物均不影响该指标。与单独使用溶剂和米氮平(5.5mg/kg)相比,D-环丝氨酸(300mg/kg)和米氮平(5.5mg/kg)的组合显着减少了埋藏的玻璃珠数量。
这些发现表明,DCS(300mg/kg剂量,作为NMDAR拮抗剂给药)与抗抑郁药米氮平在与焦虑症,强迫症和PTSD有关的行为上具有显著的出乎意料的协同作用,并支持了NMDAR拮抗剂与PTSD的抗抑郁治疗相结合。
实施例4:米氮平的R-和S-异构体对埋珠的差异影响
米氮平是分别的R(-)和S(+)异构体的外消旋混合物。后续研究独立评估了两种异构体单独各自的作用。方法与实施例3相同。测试的化合物如下:
·将5%PEG 200:5%吐温80:90%盐水(PTS)在测试前30分钟,以10ml/kg的剂量腹腔注射给药
·将帕罗西汀(5mg/kg)溶解在盐水中,并在测试前30分钟,以10ml/kg的剂量腹腔注射给药

·将S-米氮平(TRC,1,2.5,5.0和10mg/kg)溶于5%PEG 200:5%吐温80:90%盐水(PTS)中,并在测试前30分钟,以10ml/kg的剂量腹腔注射给药
·将R-米氮平(TRC,1,2.5,5.0和10mg/kg)溶于5%PEG 200:5%吐温80:90%盐水(PTS)中,并在测试前30分钟,以10ml/kg的剂量腹腔注射给药
图2显示了帕罗西汀,米氮平,S-米氮平,R-米氮平和D-环丝氨酸对埋珠行为的影响。单因素方差分析显示了显著的治疗效果。事后比较显示帕罗西汀(5mg/kg),米氮平(1、2.5、5和10mg/kg),S-米氮平(1、2.5、5、10mg/kg)和R-与溶剂相比,米氮平(10mg/kg)显着减少了被埋藏的玻璃珠数量。R-米氮平(1、2.5和5mg/kg)不影响该指标。S-米氮平的作用明显强于外消旋米氮平,而R-米氮平的作用则不那么强(图2),显示出S-异构体优于外消旋体在焦虑相关疾病(包括OCD和PTSD)方面的优越性。
在3种米氮平制剂(外消旋体,R-,S-)中,DCS治疗具有非常显著的主要作用(F=27.2,df=1,54,p<0.001),支持了先前的发现。与单独使用米氮平(2.5mg/kg)相比,米氮平(2.5mg/kg)+DSC(300mg/kg)的组合显著减少了埋藏的玻璃珠数量(p<0.01)。此外,与R-米氮平(2.5mg/kg)+PTS溶剂的组合相比,R-米氮平(2.5mg/kg)+DSC(300mg/kg)的组合显着减少了埋藏的玻璃珠数量。在存在S-米氮平时,在不存在和存在DCS的情况下均观察到最低水平的影响,因此无法进行比较。
实施例5:DCS在啮齿动物中的药代动力学
为了评估啮齿动物的行为影响与血浆DCS水平之间的关系,进行了DCS的药代动力学研究。使用了来自Jackson Laboratories(Bar Harbor,Maine)的雄性C57BL/6J小鼠(8周大)。收到后,为小鼠分配唯一的识别号(尾标),并分组饲养在OPTI小鼠笼中。在测试之前,使所有动物适应新居住室1周。在适应期间,定期检查动物,处理并称重以确保足够的健康和适合性。将动物维持在12/12光照/黑暗周期。室温保持在20至23℃之间,相对湿度保持在30%至70%之间。在研究期间随意提供食物和水。所有测试均在动物的光照周期阶段进行。
对于这些研究,将DCS(30、100、300、500和1000mg/kg)溶解在PTS溶剂(5%PEG200:5%吐温80:90%NaCl)中,并以10mL/kg的剂量腹腔注射给药。
每个时间点在以下每个处理组中使用8只小鼠:
·D-环丝氨酸30mg/kg
·D-环丝氨酸100mg/kg
·D-环丝氨酸300mg/kg
·D-环丝氨酸500mg/kg
·D-环丝氨酸1000mg/kg
在30、60和120分钟时从单独分别的动物收集样品。使用30–120分钟时间点的平均血浆水平进行分析。在每个时间点,将躯干血(trunk blood)收集在装有K2EDTA的试管中,并保存在冰上以进行短期保存。在采血的15分钟内,将试管在冷冻离心机中以10,000RPM离心10分钟。提取上清液(血浆)并转移到干冰上的预先标记的试管中。样品在大约-80℃下保存直至分析。
血浆和脑样品中DCS的分析是使用UPLC/MS/MS系统完成的,该系统由Waters的Acquity UPLC色谱系统和Quattro Premier XE三重四极杆质谱仪组成。使用5分钟(总运行时间)HILIC方法可实现DCS的分离,该方法的LLOQ为5ng/mL。
用于血浆样品分析的DCS标准品是在小鼠血浆中制备的,该标准品随后进行过滤(3kDa截留(cutoff)Amicon过滤器),然后在分析之前将5μL滤液与45μL 1.0ng/mL D9-Ach内标溶液(由10%水/乙腈制备)合并进行稀释。所使用的标准品范围是1.0–1000ng/mL。血浆样品的制备方法相同,但样品量为10μL,内标混合物为90μL,稀释后可提供100μL样品进行三次分析。
通过监测由DCS母离子形成的独特碎片(母体102.7Da到两个碎片74.7、57.7Da)来执行分析物的检测。将内标D9-Ach(母体154.95DA到碎片86.7DA)掺入样品中,以校正样品基质和仪器浮动性(variability),从而提供更可靠的数据集。
实验结果显示在图3中。如图所示,尽管差异在统计学上不显着(p=0.2),但是30mg/kg剂量的DCS产生的血浆水平在数值上低于25μg/mL。在100mg/kg及以上时,浓度显著>25微克/mL(p<0.01)。这些发现表明,在先前的实施例中,由300mg/kg DCS产生的作用反映了DCS血浆水平>25微克/mL的结果。
鉴于可以应用所公开的发明的原理的许多可能的实施例,应当认识到,所示的实施例仅是本发明的优选示例,而不应视为限制本发明的范围。相反,本发明的范围由所附权利要求书限定。因此,我们要求保护所有落入这些权利要求的范围和精神内的发明。

17.如权利要求15所述的组合物,其中所述氯胺酮是特定的对映体S-氯胺酮或R-氯胺酮。
18.一种药物组合物,包含:
净拮抗剂有效量的D-环丝氨酸;和
S-(+)-米氮平,R-(-)-米氮平或其外消旋混合物,
其中D-环丝氨酸的净拮抗剂有效量为≥500mg/d至≤1000mg/d的剂量,并经配制可产生超过25微克(μg)/mL的血液水平。
19.如权利要求18所述的药物组合物,其中所述D-环丝氨酸的净拮抗剂有效量等于或大于10mg/kg。
