背景技术

具体实施方式
本文中提供了具有抗癌活性和靶向癌症微环境以防止癌症复发和/或转移之能力二者的新的癌症治疗策略。本文中所述的抗癌策略至少部分依赖于对强效抗血管生成和抗肿瘤发生蛋白质血小板应答蛋白1(Tsp-1)的活性的刺激。“Tsp-1”是经二硫键连接的同三聚体蛋白的亚基。Tsp-1是介导细胞与细胞以及细胞与基质相互作用的黏着糖蛋白(adhesive glycoprotein)。Tsp-1与纤维蛋白原、纤连蛋白、层黏连蛋白、V型胶原蛋白和整联蛋白α-V/β-1结合,并已被示出在血小板聚集、血管生成和肿瘤发生中发挥作用。出于本公开内容的目的,Tsp-1是强效抗肿瘤发生和抗血管生成因子,其活化抑制了肿瘤生长和转移并阻遏了肿瘤微环境中的血管生成。
先前已示出鞘脂激活蛋白原(prosaposin)或鞘脂激活蛋白原来源的肽能够刺激Tsp-1的活性并且对于治疗多种类型的癌症是有效的(参见,例如PCT公开WO2009002931、WO/2011/084685和WO/2013/096868、WO2015148801和美国专利申请12/640,788和13/516,511,所有这些通过引用整体并入本文中)。
本公开内容至少部分地基于以下发现:鞘脂激活蛋白原或鞘脂激活蛋白原来源的肽对Tsp-1的刺激活性由低密度脂蛋白受体相关蛋白1(LRP1)介导。“低密度脂蛋白受体相关蛋白1(LRP1)”也称为α-2-巨球蛋白受体(alpha-2-macroglobulin receptor,A2MR)、载脂蛋白E受体(apolipoprotein E receptor,APOER)或分化群91(cluster ofdifferentiation,CD91),是形成参与受体介导的内吞作用的细胞的质膜中存在的受体的蛋白质。在人中,LRP1由LRP1基因编码。LRP1是关键的信号传导蛋白,并参与多种生物过程,例如脂蛋白代谢和细胞运动;以及疾病,例如神经退行性疾病、动脉粥样硬化和癌症。
LRP1基因编码600kDa的前体蛋白,该蛋白质在反式高尔基体复合体(trans-Golgicomplex)中由弗林蛋白酶加工,从而产生非共价缔合的515kDa的α链和85kDa的β链。作为LDLR家族的成员,LRP1包含富含半胱氨酸的补体型重复、EGF(基因)重复、β-螺旋桨结构域(β-propeller domain)、跨膜结构域和胞质结构域。LRP1的胞质结构域是α链,其包含分别包含两个、八个、十个和十一个富含半胱氨酸的补体型重复的四个配体结合结构域(称为“结合结构域I至IV”)。这些重复结合胞外基质蛋白、生长因子、蛋白酶、蛋白酶抑制剂复合体以及其他参与脂蛋白代谢的蛋白质。在这四个结构域中,II和IV结合大多数蛋白质配体。EGF重复和β-螺旋桨结构域用来在低pH条件下(例如在内体(endosome)中)释放配体,其中假定β-螺旋桨替换配体结合重复处的配体。跨膜结构域是β链,其包含100个残基的胞质尾。该尾包含负责蛋白质在内吞作用和信号转导中的功能的两个NPxY基序。

在一些实施方案中,本公开内容的抗体是多克隆抗体。“多克隆抗体”是与抗原的多于一个免疫原性决定子反应的不同抗体分子的混合物。多克隆抗体可从哺乳动物的血液、分泌物或其他流体或者从卵中分离或纯化。多克隆抗体也可以是重组的。重组多克隆抗体是通过使用重组技术产生的多克隆抗体。重组产生的多克隆抗体通常包含高浓度的不同抗体分子,其全部或大部分(例如,多于80%、多于85%、多于90%、多于95%、多于99%或更多)都对由多于一个表位构成的抗原显示出所期望的结合活性。

本文中使用的“小分子”是指可在调节生物过程中发挥作用的低分子量(例如,<900道尔顿)有机或无机化合物的分子。小分子的非限制性实例包括脂质、单糖、第二信使、其他天然产物和代谢物,以及药物和其他异生物质。

药物组合物可方便地以单位剂量形式存在,并且可通过药学领域中公知的任何方法来制备。术语“单位剂量”当关于本公开内容的药物组合物使用时,是指适合作为用于对象的单一剂量的物理上离散的单元,每个单元包含与所需稀释剂(即,载体或载剂)缔合的经计算产生所期望治疗作用的预定量活性物质。
药物组合物的制剂可取决于施用的途径。适合于肠胃外施用或瘤内、瘤周围、病灶内或病灶周围施用的可注射制剂包括例如无菌可注射水性或油性混悬剂,并且可根据已知技术使用合适的分散剂或润湿剂和助悬剂来配制。无菌可注射制剂还可以是在无毒的肠胃外可接受的稀释剂或溶剂中的无菌可注射溶液剂、混悬剂或乳剂,例如如1,3-丙二醇或1,3-丁二醇中的溶液剂。在可用的载剂和溶剂中,可使用水、林格液(U.S.P.)和等张氯化钠溶液。另外,无菌的固定油通常用作溶剂或助悬介质。出于这个目的,可使用任何温和的固定油,包括合成的甘油单酯或甘油二酯。另外,脂肪酸(例如油酸)可用于可注射剂的制剂中。可对可注射制剂进行灭菌,例如,通过细菌保留过滤器进行过滤,或者通过引入无菌固体组合物形式的灭菌剂,所述无菌固体组合物在使用之前可溶解或分散在无菌水或其他无菌可注射介质中。
对于表面施用(topical administration),药物组合物可被配制成如本领域中通常已知的软膏剂、油膏剂(salve)、凝胶剂、或乳膏剂。表面施用可利用本领域公知的经皮递送系统。一个实例是皮肤贴剂。
适合于经口施用的组合物可以以各自包含预定量抗炎剂的离散单元(例如胶囊剂、片剂、锭剂)存在。其他组合物包括水性液体或非水性液体中的混悬剂,例如糖浆剂、酏剂或乳剂。
其他递送系统可包括定时释放、延迟释放或持续释放递送系统。此类系统可避免抗炎剂的重复施用,提高对象和医师的便利性。许多类型的释放递送系统是可用的并且为本领域普通技术人员所知悉。它们包括基于聚合物的系统,例如聚(丙交酯-乙交酯)、草酸酯共聚物(copolyoxalate)、聚己内酯、聚酰胺酯、聚原酸酯、聚羟基丁酸和聚酐。包含药物的前述聚合物的微囊剂描述于例如美国专利5,075,109中。递送系统还包括非聚合物系统,它们是:脂质,其包括固醇(例如胆固醇、胆固醇酯)和脂肪酸或中性脂肪(例如甘油单酯、甘油二酯和甘油三酯);水凝胶释放系统;sylastic系统;基于肽的系统;蜡包衣;使用常规黏合剂和赋形剂的压制片剂;部分融合的植入物等。具体实例包括但不限于:(a)侵蚀系统,其中抗炎剂以在基质中的形式被包含,例如描述于美国专利No.4,452,775、No.4,667,014、No.4,748,034和No.5,239,660中的那些,以及(b)扩散系统,其中活性组分以受控速率从聚合物中渗透,例如美国专利No.3,832,253和No.3,854,480中所述。另外,可使用基于泵的硬件递送系统,其中一些适合于植入。
长期持续释放植入物的使用可特别地适合于治疗慢性病症。本文中使用的长期释放意指植入物被构建和布置成递送治疗水平的活性成分持续至少30天、并且优选60天。长期持续释放植入物是本领域普通技术人员公知的并且包括上述的一些释放系统。
在一些实施方案中,用于治疗性施用的药物组合物必须是无菌的。无菌通过无菌过滤膜(例如,0.2微米膜)通过过滤来容易地实现。或者,可使用防腐剂以防止微生物的生长或作用。多种防腐剂是公知的,并且包括例如苯酚和抗坏血酸。通常将环Psap肽和/或药物组合物以冻干形式或作为水性溶液(如果其对热和氧化变性高度稳定)储存。制剂的pH通常将约为6至8,尽管更高或更低的pH值在某些情况下也可适合。
本公开内容的另一些方面提供了使用本文中所述的药剂和药物组合物治疗癌症的方法。在一些实施方案中,所述方法包括向有此需要的对象施用有效量的刺激Tsp-1的药剂。在一些实施方案中,所述方法包括向有此需要的对象施用如本文中所述的与LRP-1的结合结构域I结合的药剂(例如,靶向LRP1的第151至172位氨基酸的抗体)。在一些实施方案中,所述方法包括向有此需要的对象施用抑制PRSS2阻遏Tsp-1之能力的药剂。在一些实施方案中,所述方法包括向有此需要的对象施用有效量的抑制PRSS2(例如,PRSS2阻遏Tsp-1之能力)的第一药剂和有效量的与LRP1的结合结构域I结合的第二药剂。当施用多于一种药剂时,它们可同时或依次施用。本领域技术人员(例如,医师)能够确定施用的方式。
癌症的“治疗(treat)”或“治疗(treatment)”包括但不限于防止、降低、或停止癌症的发展、降低或消除癌症的症状、阻抑或抑制癌症的生长、防止或降低现有癌症的转移和/或侵袭、促进或诱导癌症的消退、抑制或阻抑癌细胞的增殖、降低血管生成和/或提高凋亡癌细胞的量。
“有效量”是足以提供医学上所期望结果(例如癌症的治疗)的药剂的剂量。有效量将随着以下而变化:所治疗的特定疾病或障碍、所治疗的对象的年龄和身体状况、病症的严重程度、治疗的持续时间、任何同时治疗的性质、施用的具体途径以及健康实践者的知识和专门知识(expertise)中的类似因素。为了向对象(例如人)施用,通常可使用约0.001、0.01、0.1、或1mg/kg多至50、100、150、或500mg/kg或更多的剂量。
在一些实施方案中,有效量是不引起对象毒性的药剂剂量。在一些实施方案中,有效量是引起对象降低的毒性的药剂剂量。用于测量毒性的方法(例如,肝、脾和/或肾的活检/组织学;用于肝毒性的丙氨酸转移酶、碱性磷酸酶和胆红素测定;以及用于肾毒性的肌酐水平)是本领域公知的。

对象应意指人或脊椎动物或哺乳动物,包括但不限于啮齿动物(例如大鼠或小鼠)、狗、猫、马、牛、猪、绵羊、山羊、火鸡、鸡和灵长类(例如,猴)。本公开内容的方法可用于治疗有此需要的对象。有此需要的对象可以是具有发生癌症的风险(即,通过遗传测试)的对象或患有癌症的对象。

实施例
癌症进展至转移阶段是其致死性的主要贡献因素。为了使肿瘤形成致命性转移,其必须进入脉管系统或淋巴系统(内渗),在运输期间存活,离开血管或淋巴管(外渗),并在转移部位增殖[1]。在该过程中,肿瘤与其微环境之间的异型信号传导(heterotypicsignaling)可通过调节介导肿瘤生长、血管生成和免疫应答的因子的产生和分泌来影响肿瘤生长。通过功能蛋白质组学筛选鉴定了两种蛋白质鞘脂激活蛋白原和PRSS2,所述筛选设计成鉴定在微环境中调节Tsp-1的分泌蛋白[2]。鞘脂激活蛋白原优先由弱转移性肿瘤表达并刺激肿瘤微环境中的Tsp-1。相反地,PRSS2优先由高度转移性细胞表达并抑制肿瘤微环境中的Tsp-1表达。Tsp-1特别通过以下多方式的活性来抑制肿瘤的生长和进展:(1)其是广泛作用的抗血管生成因子,(2)其具有针对表达CD36之肿瘤的直接抗肿瘤活性,以及(3)其通过与CD47结合来促进巨噬细胞吞噬和T细胞活化[3-5]。已确定鞘脂激活蛋白原的Tsp-1刺激活性和PRSS2的Tsp-1阻遏活性二者是通过与LRP1结合而介导的。本文中提供了模拟鞘脂激活蛋白原的Tsp-1刺激活性并阻断PRSS2的Tsp-1阻遏活性的抗体。
当前对癌症患者的护理标准由广泛作用的细胞毒剂(化学治疗)、辐射以及靶向特定分泌蛋白、细胞表面受体或激酶的定向治疗剂组成。历史上,已存在靶向肿瘤微环境的两类治疗剂,抗血管生成药(其仅限于抗VEGF治疗)和免疫调节药。靶向微环境的治疗的主要缺点之一在于其不具有直接抗肿瘤活性,并且因此其作为单药治疗的效力受到限制。相反地,具有直接抗肿瘤活性的靶向治疗和化学治疗的主要缺点在于除了意外的有害副作用之外,患者对药物产生了抗性。正因为如此,已使用涉及靶向治疗/化学治疗和抗血管生成/免疫调节药之组合的治疗策略,其目的是首先通过抗肿瘤药物的活性使肿瘤缩小,并随后通过靶向肿瘤微环境的治疗的活性使肿瘤陷入困境(at bay)。对特异性地靶向肿瘤微环境以提高具有强效抗肿瘤、抗血管生成和免疫调节活性的蛋白Tsp-1表达的抗体的开发将具有巨大的治疗潜力。
鞘脂激活蛋白原首先被鉴定为肿瘤转移的新的抑制剂,并且这样的抑制被记载是通过刺激肿瘤微环境中的p53并随后刺激Tsp-1来实现的[2]。已鉴定了来自具有强效抗肿瘤和抗转移活性的鞘脂激活蛋白原的5-氨基酸环肽。在目的1中,将优化psap肽的结合亲和力和活性。在目的2中,经优化的psap肽将被开发成治疗剂,并在原发和转移癌症的原位异种移植、同基因和自发遗传模型中进行测试。在目的2中,还将筛选用于CD36、CD47、Tsp-1和psap表达的患者肿瘤组织样品,以扩大psap肽的潜在适应证。对于目的2,将与以下进行合作:Dana Farber Cancer Institute and Harvard Medical School的Drapkin实验室、以及Center for Cancer Biomarkers at the University of Bergen,Norway的Akslen实验室、以及Brigham and Women’s Hospital and Harvard Medical School的Kimmelman实验室。
psap肽抑制原发性胰腺肿瘤的生长和转移
已鉴定了刺激骨髓来源髓样细胞的来源于鞘脂激活蛋白原的4-氨基酸肽,该细胞通常被募集来刺激肿瘤生长以表达强效抗血管生成和抗肿瘤发生蛋白Tsp-1。
已示出鞘脂激活蛋白原肽通过刺激肿瘤微环境中骨髓来源细胞中的Tsp-1来抑制转移[6]。正因为如此,测试了以下假设:psap将在治疗转移性胰腺癌中具有效力,所述转移性胰腺癌是其中微环境包含大部分肿瘤团块的癌症[7]。因此,将表达萤火虫萤光素酶的1×106个AsPc1人胰腺癌细胞注射到SCID小鼠的胰腺中。允许肿瘤生长25天,这时所有肿瘤的萤光素酶强度大于1×108。然后用psap肽以20mg/kg和40mg/kg QD的剂量开始处理。在处理21天之后处死所有小鼠,此时经对照(载剂)处理的小鼠是濒死的。如图1和2A中所示,psap肽能够显著地抑制原发肿瘤生长。更显著的是,在检查肝和脾(一般对于细胞系和对于胰腺癌的两个最常见的转移部位)之后,均未观察到任何转移(图2B)。
多种人癌细胞表达CD36
由于Tsp-1通过与细胞表面受体CD36结合而引发其促凋亡活性,因此检查了人癌细胞系是否表达该受体。筛选了来源于患者腹水的一组原发性人卵巢癌细胞以及代表主要亚型(ER+、HER2+和三阴性)的三种乳腺癌细胞系。惊人地,发现所有细胞均表达易于检测水平的CD36(图3A和3B)。根据该观察,预测psap肽通过Tsp-1与CD36结合来诱导卵巢癌细胞凋亡以及通过抑制血管生成而具有针对卵巢癌的显著效力。该假设将在目的1中进行测试。
卵巢癌将骨髓来源髓样细胞募集至为肽之靶标的腹水。
已示出鞘脂激活蛋白原和治疗性肽的活性由Gr1+/Cd11b+单核细胞介导[6]。因此,通过FACS分析检查来自图4中处理小鼠的腹水液(ascites fluid)中Gr1+/CD11b+细胞的存在。观察到经对照处理的小鼠的腹膜液(peritoneal fluid)包含>70%的Gr1+/Cd11b+细胞,而通过肽处理“治愈”的小鼠腹腔中的流体包含~30%的Gr1+/Cd11b+细胞(图5)。这指示卵巢肿瘤细胞募集这些BM来源细胞。
LRP1介导鞘脂激活蛋白原对Tsp-1的刺激
已示出低密度脂蛋白受体相关蛋白(LRP)可介导鞘脂激活蛋白原的摄取[8]。因此,为了确定鞘脂激活蛋白原如何能够刺激肿瘤相关成纤维细胞中Tsp-1和p53的表达,在存在和不存在受体相关蛋白(RAP)的情况下,用PC3条件培养基处理前列腺成纤维细胞,其中所述受体相关蛋白是竞争性抑制剂或LRP1结合剂。western印迹分析揭示在存在RAP的情况下,PC3条件培养基不再刺激Tsp-1(图6A至6B)。还示出LRP1的连接释放了胞内Ca2+储存[9]。为了确定鞘脂激活蛋白原介导的对Tsp-1的刺激是否利用了该途径,在存在和不存在PKC抑制剂6983的情况下,用来自PC3细胞的CM处理成纤维细胞。通过western印迹分析观察到,PKC的抑制消除了对Tsp-1和p53的刺激。因此,认为鞘脂激活蛋白原通过与LRP1结合发挥作用。
此外,用两个独立的shRNA序列敲低肺成纤维细胞中的LRP1表达,并检查对Tsp-1表达的影响。发现在不存在刺激的情况下敲低LRP1对Tsp-1表达没有影响(图7)。与获得自用RAP处理细胞的结果一致,当用来自弱转移性PC3细胞系或转移性PC3-LIN4细胞系的条件培养基处理肺成纤维细胞时,未观察到对Tsp-1的刺激,正如用空载体转导的细胞中观察到的那样(图7)。惊人地,使LRP1沉默也消除了LN4条件培养基对LRP1的阻遏,表明该活性也由来自LRP1下游的信号传导途径介导(图7)。
为了描述通过鞘脂激活蛋白原的从LRP1导致Tsp-1刺激的途径,选择了已被报道的在不同条件下通过LRP1刺激的以下两种途径以进行检查:Rho和Rac[10]。观察到所述肽以LRP1依赖性方式刺激Tsp-1,这是由于以剂量依赖性方式与RAP共处理阻断了对Tsp-1的刺激(图8A)。此外,在Rho激酶抑制剂Y27632存在的情况下,当用所述肽处理肺成纤维细胞时对Tsp-1的刺激完全消除(图8B)。
然后将注意力转向PC3M-LN4细胞阻遏肿瘤微环境中Tsp-1的机制。为此,将LN4条件培养基(CM)用具有提高浓度的NaCl的Cu2+/肝素琼脂糖柱进行分级。将洗脱的级分用于处理肺成纤维细胞,并通过western印迹分析Tsp-1诱导。发现在0.9至1.0M NaCl之间洗脱的级分能够刺激Tsp-1(图9)。通过串联液相色谱/质谱法(liquid chromatography/massspectrometry,LC/MS)分析洗脱的级分中的蛋白质含量,鉴定出存在于LN4 CM的活性级分中,但不存在于邻近的级分中或来自PC3 CM的用相同NaCl浓度洗脱的级分中的蛋白质。鉴定的蛋白质是丝氨酸蛋白酶PRSS2。通过在PC3和LN4细胞中进行PRSS2表达的western印迹来验证LC/MS分析,其揭示了PRSS2在LN4细胞中以显著较高的水平表达(图10)。然后在存在和不存在丝氨酸蛋白酶抑制剂STI(其完全消除了对Tsp-1的抑制)的情况下,通过用LN4 CM处理肺成纤维细胞验证了PRSS2介导对Tsp-1的阻遏(图11)。最后,试图描来发源于LRP1的介导PRSS2诱导的对Tsp-1之阻遏的信号传导途径。推断如果psap通过LRP1介导的Rho途径的激活来刺激Tsp-1,则PRSS2可能通过LRP1介导的Rac途径的激活来诱导对Tsp-1的阻遏,因为这两种途径是拮抗的。在存在和不存在Racl抑制剂的情况下,通过用LN4 CM处理WI38细胞,观察到对Tsp-1的阻遏减轻并且Tsp-1水平恢复至基础水平(图12)。虽然STI能够阻断由PRSS2介导的对Tsp-1的阻遏,但由于其缺乏对PRSS2的特异性和抑制胰蛋白酶家族所有成员的能力,其并不代表可行的治疗策略。出于相同的原因,由于丝氨酸蛋白酶和胰蛋白酶家族的活性位点的保守性质,设计成抑制PRSS2的化学化合物也将缺乏特异性。因此,认为设计成与PRSS2结合的抗体将是强效的抗癌治疗剂,所述抗体对PRSS2阻遏Tsp-1的能力进行阻断。
最后,试图确定通过鞘脂激活蛋白原肽刺激Tsp-1所需的LRPl受体的最小区域。对于这些研究,利用了包含与跨膜结构域和胞内结构域融合的胞外结构域不同区域的LRP1的突变体(图13A)[11]。然后,用这些构建体转染293T细胞,所述293T细胞表达非常低水平的Tsp-1并且不受肽刺激而产生Tsp-1(图13B)。发现环psap肽能够刺激用miniLRP1a和miniLRP1b转染的293T细胞中的Tsp-1,但不能刺激用其他miniLRP1构建体转染的细胞中的Tsp-1。惊人地,miniLRP1a和miniLRP1b二者是仅有的包含N端和前两个β-螺旋桨结构域的突变体。基于这些结果,认为鞘脂激活蛋白原和psap肽通过与LRP1的该区域结合来刺激Tsp-1表达。认为与LRP1的该区域结合的抗体模拟了肽的活性并且可能以甚至更大的亲和力结合并具有改善的药动学(PK)和药效学(PD)性质。
与PSAP和PRSS2二者结合的LRP1肽
为了映射与鞘脂激活蛋白原(prosaposin,PSAP)和/或PRSS2结合的LRP1上的结合位点,测试了来源于LRP1结合结构域I的肽(SEQ ID NO:3和7至18)与PSAP或PRSS2免疫共沉淀的能力。结果表明肽12(对应于LRP1的第140至164位氨基酸)和肽13(对应于LRP1的第151至172位氨基酸)与PRSS2和PSAP二者结合(图16A至16B)。肽12和肽13在对应于LRP1的第151至164位氨基酸的区域中重叠,表明PRSS2和PASP的结合位点在LRP1的该区域内。

PRSS2活性位点中的突变不影响Tsp-1的阻遏
使用QuickChange诱变试剂盒对pCMVSPORT6.1中的野生型PRSS2进行突变:G191R、S200A、S200T和S200C(以下提供了核苷酸序列)。使用FuGene转染试剂将野生型和突变体构建体转染到293T细胞中。在48小时之后,收获包含突变体蛋白质的条件培养基并用于处理WI-38成纤维细胞过夜。在处理之后,收获细胞,裂解并通过Bio-Rad蛋白测定来确定蛋白质浓度。将等同水平的蛋白质添加至每个孔,并在聚丙烯酰胺SDS凝胶上运行。然后使用标准方案利用针对Tsp-1和b-肌动蛋白的抗体进行western印迹。
发现PRSS2阻遏Tsp-1的能力不受影响,表明LRP1的结合位点不在活性位点中,并且针对该区域的抗体不影响该酶的蛋白酶活性(图17)。
