背景
本申请涉及具有抗炎特性的间充质干细胞、制备这种间充质干细胞的方法、获自这种间充质干细胞的材料,间充质干细胞和获自间充质干细胞的材料在治疗血管病变中的用途。血管病变包括但不限于动脉性肺动脉高压(PAH)、其他类型的肺动脉高压、外周血管疾病(PVD)、严重肢体缺血(CLI)、冠状动脉疾病和糖尿病性血管病变。
肺动脉高压是一种罕见的、进行性的且危及生命的疾病,其影响肺血管系统。具体而言,肺动脉高压导致肺血管系统中的压力增加,其可导致心力衰竭和其他结果。目前,根据世界卫生组织(WHO)临床分类系统(Dana Point 2008),肺动脉高压分为以下几类:
第1组:动脉性肺动脉高压(PAH);
第1’组:肺静脉闭塞性疾病(PVOD)和/或肺毛细血管瘤病(PCH);
第2组:左心疾病引起的肺动脉高压;
第3组:肺部疾病和/或低氧血症引起的肺动脉高压;
第4组:慢性血栓栓塞性肺动脉高压(CTEPH);和
第5组:具有不明确的多因素机制的PH。
动脉性肺动脉高压是一种特定类型的肺动脉高压,在诊断后若不进行治疗会在平均2.8~5年内导致死亡(Keily et al.(2013)BMJ 346:f2028)。肺循环收缩的增加导致右心的压力增加,这可能发展成右心衰竭。根据定义,慢性肺动脉高压患者的平均肺动脉压(mPAP)在静息时>25mmHg或在运动期间>30mmHg(正常值<20mmHg)。动脉性肺动脉高压的病理生理学的特征在于血管收缩和肺血管的重塑。在慢性PAH中,最初未肌型化的肺血管存在新肌型化,并且已经肌型化的血管的平滑肌周长增加。由此导致的肺动脉压升高导致右心进行性压力,其导致右心输出减少,最终导致右心衰竭(M.Humbert et al.,J.Am.Coll.Cardiol.2004,43,13S-24S)。PAH是一种罕见疾病,患病率为百万分之一至二。患者的平均年龄估计为36岁,只有10%的患者年龄超过60岁。女性受到的影响明显多于男性(G.E.D’Alonzo et al.,Ann.Intern.Med.1991,115,343-349)。
市场上现有的标准疗法(例如,前列环素类似物、内皮素受体拮抗剂、磷酸二酯酶抑制剂)能够改善患者的生活质量和运动耐受性。这些药物会导致严重的副作用和/或必须使用复杂类型的给药来递送。患者经常在单一疗法后病情开始恶化或恶化后进行联合治疗。尽管治疗取得了进展,但PAH仍无法治愈。
因此,需要开发用于治疗血管病变包括PAH的改进的治疗组合物和方法。
概要
在一个方面,本公开提供治疗或预防血管病变的方法,包括向有需要的受试者施用组合物,所述组合物包含(i)间充质干细胞(MSC)或(ii)与MSC接触并包含所述MSC的一种或多种组分的培养基的一部分,或(iii)衍生自MSC的外泌体。其中所述MSC已离体暴露于前列环素,并且与未暴露于前列环素的对照MSC相比,暴露于前列环素的所述MSC增加一种或多种抗炎因子的表达和/或减少一种或多种促炎因子的表达。
在一些实施方案中,在施用之前,将所述MSC暴露于0.3μg/ml-83.3μg/ml的前列环素。在其他实施方案中,在施用之前,将所述MSC暴露于0.3μg/ml-10μg/ml的前列环素。
在一些实施方案中,所述前列环素为曲前列环素、其衍生物或其药学上可接受的盐。
在一些实施方案中,所述MSC暴露于所述前列环素至少24小时。在其他实施方案中,所述MSC暴露于所述前列环素至少48小时。

发明详述
将间充质干细胞(MSC)暴露于相对低浓度的前列环素(例如曲前列环素)可赋予抗炎表型。通过将MSC暴露于低浓度的前列环素,基于其独特的基因表达模式增强了MSC的治疗潜力。
暴露于高浓度曲前列环素(例如250μg/mL)的MSC,促进MSC中血管生成的因子的表达增强。然而,将MSC暴露于高浓度的曲前列环素也促进了促炎因子的表达,这在某些情况下可能不适合PAH治疗,并且还对细胞活力产生负面影响。本发明人意外地发现,如本发明所述,暴露于低浓度前列环素(例如,10μg/mL曲前列环素)降低MSC中促炎因子的表达并增加抗炎因子的表达。例如,将MSC暴露于低浓度的曲前列环素,显著降低暴露于曲前列环素的MSC中TNFα的表达,并显著增加IL-10和IL-13的表达。将MSC暴露于低浓度的曲前列环素也增加了其他抗炎因子如IDO、iNOS、HLA和TGFβ的表达。

B.血管病变
血管病变包括但不限于肺动脉高压,包括动脉性肺动脉高血压(PAH)、外周血管疾病(PVD)、严重肢体缺血(CLI)、冠状动脉疾病和糖尿病性血管病变。
尽管发现许多原因和条件与PAH有关,但它们中的许多共同具有几种基本的病理生理学特征。这些过程中的一个重要特征是内皮功能障碍,内皮即所有血管壁的内部细胞层,其通常负责调节血管张力和修复并抑制凝块形成的大量物质的产生和代谢。在PAH的情况下,内皮功能障碍可导致有害物质的产生过量和保护性物质的产生受损。这是否是PAH发展的主要事件或下游级联的一部分仍然未知,但在任何一种情况下,它都是作为该疾病特征的进行性血管收缩和血管增殖的重要因素。

冠状动脉疾病(动脉粥样硬化)是人类的进行性疾病,其中一条或多条冠状动脉由于斑块的积聚逐渐闭塞。通常通过球囊血管成形术或插入支架来支撑以打开部分闭塞的动脉来治疗患有这种疾病的患者的冠状动脉。最终,这些患者需要在极高的费用和风险下进行冠状动脉搭桥手术。
C.间充质干细胞(MSC)
间充质干细胞(MSCs)是在骨髓、血液、牙髓细胞、脂肪组织、皮肤、脾脏、胰腺、大脑、肾脏、肝脏、心脏、视网膜、大脑、毛囊、肠、肺、淋巴结、胸腺、骨骼、韧带、肌腱、骨骼肌、真皮和骨膜中发现的细胞。MSC能够分化成不同的种系,例如中胚层、内胚层和外胚层。因此,MSC能够分化成大量细胞类型,包括但不限于脂肪、骨、软骨、弹性组织、肌肉组织和纤维结缔组织。由MSC分化的特定谱系定型和分化途径取决于各种影响,包括机械影响和/或内源生物活性因子,例如生长因子、细胞因子和/或由宿主组织建立的局部微环境条件。因此,MSC是分裂产生子细胞的非造血祖细胞,所述子细胞是干细胞或是前体细胞,其随着时间的推移将不可逆地分化以产生表型细胞。MSC的实例包括间充质前体细胞(MPC)。
如本申请所用,术语“干细胞”是指能够产生表型和基因型相同的子细胞以及至少一种其他最终细胞类型(例如,终分化细胞)的自我更新细胞。术语“干细胞”包括全能、多能和多潜能细胞,以及衍生自其分化的祖细胞和/或前体细胞。
如本申请所用,术语“全能细胞”是指能够形成完整胚胎(例如胚泡)的细胞。
如本申请所用,术语“多能细胞”是指具有完全的分化多功能性的细胞,即生长成哺乳动物身体的约260种细胞类型的任何一种的能力。多能干细胞可以自我更新,并且可以在组织内保持休眠或静止。
术语“多潜能细胞”是指能够产生几种成熟细胞类型中的任何一种的细胞。如本申请所用,该短语包括成体或胚胎干细胞和祖细胞,以及这些细胞的多能后代。与多能细胞不同,多潜能细胞不具有形成所有细胞类型的能力。
如本申请所用,术语“祖细胞”或“前体细胞”是指致力于分化成特定类型细胞或形成特定类型组织的细胞。
在一个实施方案中,从获自受试者的样品中富集细胞。术语“富集”及其变体在本申请中用于描述细胞群体,其中与未处理的群体相比,一种特定细胞类型的比例或多种特定细胞类型的比例增加。
在一个实施方案中,本公开中使用的细胞是TNAP+、STRO-1+、VCAM-1+,THY-1+、STRO-2+、CD45+、CD146+、3G5+或其任意组合。
对于给定标记,提及细胞“阳性”(也称为“+”)意味着它可以是该标记的低(lo或暗淡)或高(明亮、bri)表达者,这取决于存在于细胞表面上的标记的程度,其中所述术语涉及荧光或在细胞的颜色分选过程中使用的其他颜色的强度。lo(或者暗淡、或者晦暗)和bri的区别将在用于待分类的特定细胞群的标记的上下文中理解。对于给定标记,将细胞称为“阴性”(或“-”)并不意味着该标记根本不被该细胞表达。而意味着所述标记被该细胞以相对非常低的水平表达,并且当被可检测地标记时其产生非常低的信号。在一些实施方案中,“阴性”可以指在以某种方式处理(例如暴露于前列环素)的细胞中不存在或以减少的量存在的标记。在一些实施方案中,“阴性”是指与未暴露的对照样品相比,在暴露于前列环素的细胞中以减少至少50%的量存在的标记物。
当在本申请中使用时,术语“TNAP”旨在涵盖所有组织非特异性碱性磷酸酶的同种型。例如,该术语包括肝同种型(LAP)、骨同种型(BAP)和肾脏同种型(KAP)。在一个优选实施方案中,TNAP为BAP。在一个特别优选的实施方案中,如本申请所用的TNAP是指可以与根据布达佩斯条约的规定以保藏登记号PTA-7282于2005年12月19日保藏于ATCC的杂交瘤细胞系产生的STRO-3抗体结合的分子。
可用于所述方法的干细胞可衍生自成体组织、胚胎、胚外组织或胎儿。术语“成人”以其最广泛的含义使用,包括产后受试者。在一个优选的实施方案中,术语“成人”是指青春期后的受试者。如本申请所用的术语“成人”还可包括取自女性的脐带血。
在一些方面,所述干细胞可以是子代细胞(其也可以称为扩增细胞)。子代细胞可以产生自本申请所述干细胞的体外培养。根据培养条件(包括培养基中刺激因子的数量和/或类型)、传代次数等,本公开的扩增细胞可具有多种表型。在某些实施方案中,所述的子代细胞在亲本群体的约2次、约3次、约4次、约5次、约6次、约7次、约8次、约9次或约10次传代后获得。然而,所述子代细胞可以在亲本群体经过任何数量的传代后获得。并且可以通过在合适的培养基中培养来获得所述子代细胞。
在一个实施方案中,通过使用以STRO-3抗体标记的磁珠从骨髓中分离TNAP+细胞,并接种在添加有20%胎牛血清、2mM L-谷氨酰胺和100μm L-抗坏血酸-2-磷酸酯的α-MEM中以获得子代细胞。
在一个实施方案中,此类扩增的细胞(至少在5次传代后)可以是TNAP-、CC9+、HLAI+类、HLA II-类,CD14-、CD19-;CD3-、CD11a-c-、CD31-、CD86-和/或CD80-。然而,不同标记的表达可能根据培养条件而变化。而且,尽管这些表型的细胞可能在扩增的细胞群体中占优势,但可能存在少部分不具有该表型的细胞(例如,一小部分扩增的细胞可能是CC9-)。在一些实施方案中,这些细胞的存在量为所述扩增细胞群体中存在的细胞总数的30%、20%、15%、10%、5%或1%,或者更少。在一个优选的实施方案中,扩增的细胞具有分化成不同细胞类型的能力。
在一个实施方案中,一个扩增的细胞群体包含的细胞中,至少25%,更优选至少50%的细胞是CC9+的细胞。
在另一个实施方案中,本公开内容的方法中使用的扩增细胞群体所包含的细胞中,至少40%,更优选至少45%的细胞是STRO-1+的细胞。
在进一步的实施方案中,所述子代细胞可以表达选自由LFA-3、THY-1、VCAM-1、PECAM-1、P-选择素、L-选择素、3G5、CD49a/CD49b/CD29、CD49c/CD29、CD49d/CD29、CD29、CD18、CD61、整联素β、6-19、血栓调节素、CD10、CD13、SCF、PDGF-R、EGF-R、IGF1-R、NGF-R、FGF-R、Leptin-R(STRO-2=Leptin-R)、RANKL、STRO-1bright、CD146和这些标记的任何组合构成的群组中的标志物。
在一个实施方案中,所述子代细胞是如WO 2006/032092中所述的多潜能扩增MSC后代(MEMP)。WO 01/04268和WO 2004/085630中描述了制备富集的、可衍生出后代的MSC群体的方法。在体外环境中,MSC很少作为绝对纯的制剂存在,并且通常与组织特异性定型细胞(TSCC)的其他细胞一起存在。WO 01/04268涉及从骨髓中收获纯度为约0.1%~90%的此类细胞。包含衍生子代的MSC的群体可以从组织来源直接收获,或者可选地,它可以是已经离体扩增的群体。
例如,子代可以从收获的、未扩增的、基本上纯化的MSC群体中获得,所述群体包含其所在群体的至少约0.1%、1%、5%、10%、20%、30%、40%、50%、60%、70%、80%或95%的总细胞。例如,通过选择对于选自由TNAP、STRO-1bri、3G5+、VCAM-1、THY-1、CD146和STRO-2构成的群组中的至少一种标志物呈阳性的细胞可以实现该水平。
MSC起始群体可以源自WO 01/04268或WO 2004/085630中列出的任何一种或多种组织类型,即骨髓、牙髓细胞、脂肪组织和皮肤,或者更广泛地来自脂肪组织、牙齿、牙髓、皮肤、肝脏、肾脏、心脏、视网膜、大脑、毛囊、肠、肺、脾脏、淋巴结、胸腺、胰腺、骨骼、韧带、骨髓、肌腱和骨骼肌。
可以将MEMPS与新鲜收获的MSC区分开来,因为前者对于标记物STRO-1bri是阳性的并且对于标记物碱性磷酸酶(ALP)是阴性的。相反,新鲜分离的MSC对STRO-1bri和ALP均呈阳性。在本公开的一个优选实施方案中,至少15%、20%、30%、40%、50%、60%、70%、80%、90%或95%的施用的细胞具有STRO-1bri和ALP-表型。在进一步优选的实施方案中,所述MEMPS对于标志物Ki67、CD44和/或CD49c/CD29、VLA-3、α3β1中的一种或多种是阳性的。在另一个优选的实施方案中,所述MEMP不表现出TERT活性和/或对标记物CD18呈阴性。
在一个实施方案中,细胞取自患有血管病变的患者,在培养期的至少一部分期间使用标准技术在体外培养,所述细胞暴露于如本申请所述的前列环素,并作为自体或异体移植施用于患者。在另一个实施方案中,使用一种或多种已建立的人细胞系的细胞。在本公开的另一个有用的实施方案中,使用非人动物(或者如果患者不是人,而是来自另一物种)的细胞。
本技术可以使用来自任何非人动物物种的细胞来实施,包括但不限于非人灵长类动物细胞、有蹄类动物、犬科动物、猫科动物、兔形目动物、啮齿动物、鸟类和鱼类的细胞。可以用于进行本公开的灵长类动物细胞包括但不限于黑猩猩、狒狒、食蟹猴和任何其他新世界或旧世界猴的细胞。可以用于进行本公开的有蹄类细胞包括但不限于牛、猪、绵羊、山羊、马、水牛和野牛的细胞。可用于进行本公开的啮齿动物细胞包括但不限于小鼠、大鼠、豚鼠、仓鼠和沙鼠细胞。可用于进行本公开的兔形目物种的实例包括驯养的兔子、长腿大野兔(jack rabbits)、野兔、棉尾兔、雪地兔子和鼠兔。鸡(Gallus gallus)是可用于进行本公开的鸟类物种的实例。
细胞可以在使用前储存。用于保存和储存真核细胞,特别是哺乳动物细胞的方法和方案是本领域熟知的(参见,例如,Pollard,J.W.and Walker,J.M.(1997)Basic CellCulture Protocols,Second Edition,Humana Press,Totowa,N.J.;Freshney,R.I.(2000)Culture of Animal Cells,Fourth Edition,Wiley-Liss,Hoboken,N.J.)。保持分离的干细胞(例如间充质干/祖细胞或其子代)的生物活性的任何方法可以与本公开内容结合使用。在一个优选的实施方案中,通过使用低温保存来维持和储存细胞。
可以使用多种技术获得细胞。例如,可以使用许多细胞分选技术,该技术通过参考与细胞-抗体复合物相关的性质或与抗体连接的标记物理分离细胞。所述标记可以是磁性颗粒或荧光分子。所述抗体可以是交联的以此形成多个细胞的聚集体,通过这些细胞的密度可将其分离。或者,所需细胞粘附于其上的所述抗体可以附着于固定基质。
在一个优选的实施方案中,结合TNAP+、STRO-1+、VCAM-1+、THY-1+、STRO-2+、3G5+、CD45+、CD146+的抗体(或其他结合剂)用于分离细胞。更优选地,结合TNAP+或STRO-1+的抗体(或其他结合剂)用于分离细胞。
已知各种将抗体结合细胞与未结合细胞分离的方法。例如,可以标记与细胞结合的所述抗体(或抗同种型抗体),然后通过检测标记存在的机械细胞分选仪分离细胞。荧光激活细胞分选仪在本领域中是众所周知的。在一个实施方案中,抗TNAP抗体和/或STRO-1抗体附着于固体支持物。各种固体支持物是本领域技术人员已知的,包括但不限于琼脂糖珠、聚苯乙烯珠、中空纤维膜、聚合物和塑料培养皿。通过简单地将固体支持物与细胞悬浮液物理分离,可以从细胞悬浮液中除去抗体结合的细胞。
超顺磁性微粒可用于细胞分离。例如,可以用抗TNAP抗体和/或STRO-1抗体包被所述微粒。然后可以将抗体标记的超顺磁性微粒与含有目标细胞的溶液一起孵育。所述微粒结合到所需干细胞的表面,然后可以在磁场中收集这些细胞。
在另一个实例中,允许细胞样品物理接触,例如,固相连接的抗TNAP单克隆抗体和/或抗STRO-1单克隆抗体。所述固相连接可包括,例如将抗体吸附到塑料、硝酸纤维素或其他表面上。所述抗体也可以吸附在中空纤维膜的大孔(足够大以允许细胞流过)的壁上。或者,所述抗体可以与表面或珠子,例如Pharmacia Sepharose 6MB大珠共价连接。固相连接抗体与含有干细胞的悬浮液的确切条件和孵育持续时间取决于所用系统的几种特定因素。然而,适当条件的选择完全在本领域的技术范围内。
然后在给予足够时间使干细胞结合后,用生理缓冲液洗脱或洗去未结合的细胞。未结合的细胞可以被回收并用于其他目的,或在进行适当的测试确保已经实现所需的分离后丢弃。然后主要根据所述固相和所述抗体的性质,通过任何合适的方法将结合的细胞与固相分离。例如,可以通过剧烈搅拌从塑料培养皿中洗脱结合的细胞。或者,可以通过在所述固相和所述抗体之间酶促“切割”或消化酶敏感的“间隔区”序列来洗脱结合的细胞。与琼脂糖珠结合的间隔物可从,例如Pharmacia商购获得。
然后可以通过离心用缓冲液洗涤洗脱的、富集的细胞级分,并且可以根据常规技术将所述富集的级分冷冻保存在存活状态中供以后使用,培养扩增和/或引入患者体内。
D.培养基
本申请所用的“培养基”包括(a)含有用于培养MSC的典型组分例如氨基酸、葡萄糖和各种盐的培养基,带有或不带有MSC,和(b)从培养基中分离的组合物,包括含有培养过程中从MSC释放的组分的组合物。所述培养基可含有固体组分、液体组分、气体组分或相和材料的混合物组分。所述培养基组分包括但不限于琼脂、琼脂糖、明胶和胶原基质。“培养基”包括旨在用于细胞培养的材料,即使它尚未与细胞接触。例如,为细菌培养制备的富含营养物的液体可以是培养基。
“与MSC接触的培养基”是指与MSC接触的培养基(例如,为了培养MSC),因此包含从MSC释放的组分。此类释放组分的非限制性实例包括外泌体或其他微泡,其他微泡可包含信使RNA、非编码RNA、微小RNA、线粒体、生长因子或其他类型的生物活性剂。
E.微泡和外泌体
MSC可以在生长或分化过程中将化合物和其他物质释放到细胞外的环境中。在一些方面,此类材料包括细胞外囊泡。细胞外囊泡包含衍生自各种细胞类型的质膜片段。通常,细胞外囊泡的直径(或颗粒不是球形时的最大尺寸)在约10nm至约5000nm之间(例如,在约50nm至1500nm之间、在约75nm至1500nm之间、在约75nm之间至1250nm之间、约50nm至1250nm之间、约30nm至1000nm之间、约50nm至1000nm之间、约100nm至1000nm之间、约50nm至750nm之间等)。细胞外囊泡的替代名称包括但不限于微泡、外泌体、核外颗粒体、膜颗粒、外泌体样颗粒和凋亡小泡。除非另有说明,否则根据本公开内容可以使用任何特定类型的细胞外囊泡或细胞外囊泡类型的组合。例如,可以使用核外颗粒体,外泌体样颗粒或其组合代替外泌体。
外泌体是衍生自多泡体分选途径的囊泡。最近的研究表明,外泌体是有生物活性的囊泡,可用于细胞间通讯和促进免疫调节过程。MSC外泌体可含有20S蛋白酶体和许多RNA(信使RNA、非编码RNA、微小RNA)。在一些实施方案中,所述外泌体的直径为30nm~200nm,或者20nm~50nm。在一些实施方案中,所述外泌体在蔗糖中的密度为1.10~1.19g/mL,在100,000g沉降。在一些实施方案中,所述外泌体的膜可包含鞘磷脂、神经酰胺、脂筏和暴露的磷脂酰丝氨酸。
本发明的一个方面提供了一种组合物,其包含从已经暴露于前列环素(例如曲前列环素)的MSC中分离的外泌体。此类外泌体适用于治疗包括肺动脉高压的血管病变。通常,可以使用任何合适的纯化和/或富集外泌体的方法,例如包括磁性颗粒、过滤、透析、超速离心、ExoQuick TM(Systems Biosciences,CA,USA)和/或色谱法的方法。
在一些实施方案中,通过离心和/或超速离心分离外泌体。该方案描述于,例如Thery et al.Current Protocols in Cell Biol.(2006)3.22.。在一些实施方案中,通过单步骤尺寸排阻色谱法分离外泌体。该方案描述于,例如Boing et al.Journal ofExtracellular Vesicles(2014)3:23430。
除外泌体外,MSC还释放其他生物活性分子/囊泡。此类分子和囊泡包括但不限于线粒体和生长因子。制备含有此类分子和从MSC释放的囊泡并进一步分离特定分子和囊泡的培养基的方法是本领域已知的。参见Hu et al.,Frontiers in Genetics,2:56,1-9(2012)。
F.前列环素
本申请使用的术语“前列环素”明确包括任何前列腺素I2(PGI2),任何前列环素类似物和任何PGI2受体激动剂。适用于本技术的前列环素的非限制性实例包括依前列醇、曲前列环素、伊洛前列素和赛乐西帕,以及其任何盐,包括曲前列素钠。在一个方面,所述前列环素是曲前列环素、其衍生物、药学上可接受的盐或酯。
G.将MSC暴露于前列环素
在一些实施方案中,在施用之前,MSC或与MSC接触的培养基可以暴露于前列环素。因此,在一些实施方案中,还提供了制备用于体内递送的MSC或与MSC接触的培养基,或衍生自MSC的外泌体的方法,包括使MSC或MSC条件培养基与前列环素接触。另外一个实施方案提供了通过这种方法获得的MSC或MSC条件培养基。
将细胞或培养基暴露于化学化合物包括已知技术。在一个方面,可以将所述前列环素添加到含有MSC的培养基中并与之共孵育。然而,任选地,这种共孵育可以进一步包括添加生长因子(例如,VEGF和促血管生成素-1或-2、血小板衍生的生长因子)和/或缺氧。
可以以各种方式将MSC或与MSC接触的培养基暴露于前列环素。例如,在MSC扩增期间,MSC可以离体暴露于前列环素。也可以在MSC扩增后将其暴露于前列环素。根据本公开的一个实施方案,MSC可以从受试者自身的血液或骨髓制备。在这种情况下,MSC可以在从受试者中分离之前暴露于前列环素和/或MSC可以在分离后暴露于前列环素。
在一些实施方案中,MSC离体暴露于前列环素至少约12小时、至少约18小时、至少约24小时、至少约30小时、至少约36小时、约42小时、至少约48小时,或至少约54小时。
在一些实施方案中,将MSC暴露于浓度约0.001μg/mL至约100μg/mL、约0.001μg/mL至约50μg/mL、约0.01μg/mL至约20μg/mL、约0.1μg/mL至约10μg/mL,或约1μg/mL至约5μg/mL的离体前列环素。在一些实施方案中,用浓度为约0.3μg/mL至约83.3μg/mL、或约0.3μg/mL至约10μg/mL的前列环素以处理MSC。在一些实施方案中,将MSC暴露于浓度为约100μg/mL、75μg/mL、50μg/mL、25μg/mL、10μg/mL、5μg/mL、2.5μg/mL、1μg/mL、0.5μg/mL、0.25μg/mL、0.1μg/mL、0.05μg/mL或约0.01μg/mL的前列环素。前列环素的浓度是指MSC培养基中前列环素的浓度。
在一些实施方案中,暴露于前列环素的MSC的促炎因子或促炎细胞因子的表达水平比未暴露于前列环素的对照MSC的表达水平低至少30%、至少40%、至少50%、至少1倍、至少1.5倍、至少2倍、至少3倍、至少4倍或至少5倍。在一些实施方案中,所述促炎因子是TNF-α。在一些实施方案中,前列环素暴露降低了一种以上促炎因子的表达水平。
在一些实施方案中,暴露于前列环素的MSC的抗炎因子或抗炎细胞因子的表达水平比未暴露于前列环素的对照MSC的表达水平高至少30%、至少40%、至少50%、至少1倍、至少1.5倍、至少2倍、至少3倍、至少4倍、或至少5倍、至少6倍、至少7倍、至少8倍、至少9倍,或至少10倍。在一些实施方案中,前列环素暴露增加了一种以上抗炎因子的表达水平。在一些实施方案中,所述抗炎因子选自由IL10、IL13、IDO、iNOS、HLA、TGFβ及其组合构成的群组。
在一些实施方案中,MSC暴露于前列环素同时降低一种或多种促炎因子的表达水平,并增加一种或多种抗炎因子的表达水平。在一些实施方案中,MSC暴露于前列环素同时降低TNF-α和/或IL-4的表达水平,并且增加选自由IL10、IL13、IDO、iNOS、HLA和TGFβ构成的群组中的一种或多种抗炎因子的表达水平。
H.药物组合物
在一个方面,本公开提供了药物组合物,其包含治疗有效量的MSC,或与MSC接触并包含MSC的一种或多种组分的培养基的一部分,或衍生自MSC的外泌体。
在一些实施方案中,例如本申请所述的那些,所述药物组合物包含已经暴露于曲前列环素的MSC。在一些实施方案中,所述组合物中所有MSC的至少30%、至少40%、至少50%、至少60%、至少70%、至少80%、至少90%、至少95%、至少98%、或至少99%的TNFα表达水平比未暴露于前列环素的对照MSC的表达水平低至少50%。在一些实施方案中,所述组合物中所有MSC的至少30%、至少40%、至少50%、至少60%、至少70%、至少80%、至少90%、至少95%、至少98%、或至少99%的IL10、IL13、IDO、iNOS、HLA和TGFβ中至少一种的表达水平,比未暴露于前列环素的对照MSC的表达水平高至少50%。
在一些实施方案中,所述药物组合物还包含至少一种药学上可接受的载体。短语“药学上可接受的”是指在合理的医学判断范围内适合用于与人和动物组织接触而没有过多毒性、刺激性、过敏反应、或其他问题或并发症的那些化合物、材料、组合物和/或剂型,与合理的利益/风险比相称。本申请所用的短语“药学上可接受的载体”是指药学上可接受的材料、组合物或载体,例如液体或固体填充剂、稀释剂、赋形剂或溶剂包封材料。
药学上可接受的载体包括盐水、缓冲水溶液、溶剂和/或分散介质。这种载体的使用在本领域中是公知的。所述溶液优选是无菌的,并且在易于注射的程度上是能流动的。优选地,所述溶液在制造和储存条件下是稳定的,并且通过使用例如对羟基苯甲酸酯、氯丁醇、苯酚、抗坏血酸、硫柳汞等防止微生物如细菌和真菌的污染作用。

可用于本公开方法的药物组合物可包含聚合物载体或细胞外基质。

合适的聚合物载体包括由合成或天然聚合物以及聚合物溶液形成的多孔网或海绵。基质的一种形式是聚合网或海绵;另一种是聚合物水凝胶。可以使用的天然聚合物包括蛋白质,例如胶原、白蛋白和纤维蛋白;以及多糖,例如海藻酸盐和透明质酸的聚合物等。合成聚合物包括可生物降解聚合物和不可生物降解聚合物。可生物降解聚合物的实例包括羟基酸的聚合物,例如聚乳酸(PLA)、聚乙醇酸(PGA)和聚乳酸-乙醇酸(PLGA)、聚原酸酯、聚酐、聚磷腈及其组合。不可生物降解的聚合物包括聚丙烯酸酯、聚甲基丙烯酸酯、乙烯醋酸乙烯酯和聚乙烯醇。
可形成具有延展性的离子或共价交联水凝胶的聚合物用于包封细胞。水凝胶是当有机聚合物(天然或合成的)通过共价键、离子键或氢键交联以形成三维开放晶格结构时形成的物质,其捕获水分子以形成凝胶。可用于形成水凝胶的材料的实例包括离子交联的多糖,如海藻酸盐、聚磷腈和聚丙烯酸酯,或嵌段共聚物如分别通过温度或pH交联的普朗尼克(Pluronics)TM或者TetronicsTM,聚环氧乙烷-聚丙二醇嵌段共聚物。其他材料包括蛋白质如纤维蛋白,聚合物如聚乙烯吡咯烷酮、透明质酸和胶原蛋白。
通常,这些聚合物至少部分可溶于水溶液,例如水、缓冲盐溶液或含水醇溶液,其具有带电侧基,或其一价离子盐。具有可与阳离子反应的酸性侧基的聚合物的实例是聚(磷腈)、聚(丙烯酸)、聚(甲基丙烯酸)、丙烯酸和甲基丙烯酸的共聚物、聚(醋酸乙烯酯)和磺化聚合物,例如磺化聚苯乙烯。也可以使用具有通过丙烯酸或甲基丙烯酸与乙烯基醚单体或聚合物反应形成的酸性侧基的共聚物。酸性基团的实例是羧酸基、磺酸基、卤化(优选氟化的)醇基、酚羟基和酸性羟基。具有可与阴离子反应的碱性侧基的聚合物的实例是聚(乙烯基胺)、聚(乙烯基吡啶)、聚(乙烯基咪唑)和一些亚氨基取代的聚磷腈。聚合物的铵盐或季铵盐也可以由骨架氮原子或侧链亚氨基形成。碱性侧基的实例是氨基和亚氨基。
在一些实施方案中,所述药物组合物可包含至少一种另外的治疗剂。例如,所述组合物可含有镇痛剂以帮助治疗炎症或疼痛,或含有抗感染剂以预防用所述组合物治疗的部位的感染。更具体地,有用的治疗剂的非限制性实例包括以下治疗类别:镇痛药,例如非甾体抗炎药、阿片激动剂和水杨酸盐;抗感染药,如抗蠕虫药、抗厌氧菌药、抗生素、氨基糖苷类抗生素、抗真菌抗生素、头孢菌素类抗生素、大环内酯类抗生素、各种β-内酰胺类抗生素、青霉素类抗生素、喹诺酮类抗生素、磺胺类抗生素、四环素类抗生素、抗分支杆菌药、抗结核抗分支杆菌药、抗原虫药、抗疟疾抗原虫药、抗病毒药、抗逆转录病毒药、杀螨剂、抗炎药、皮质类固醇抗炎药、止痒药/局部麻醉药、外用抗感染药、抗真菌外用抗感染药、抗病毒外用抗感染药;电解质和肾脏制剂,如酸化剂、碱化剂、利尿剂、碳酸酐酶抑制剂利尿剂、袢利尿剂、渗透性利尿剂、保钾利尿剂、噻嗪类利尿剂、电解质替代物和尿酸排泄剂;酶,如胰酶和溶栓酶;胃肠道药物,如止泻药、胃肠道抗炎药、胃肠道抗炎药、抗酸抗溃疡药、胃酸泵抑制剂抗溃疡药、胃粘膜抗溃疡药、H2阻滞剂抗溃疡药、胆石溶解剂、消化剂、催吐剂、泻药和大便软化剂,以及促胃肠动力剂;全身麻醉药,如吸入麻醉药、卤代吸入麻醉药、静脉麻醉药、巴比妥类静脉麻醉药、苯二氮卓类静脉麻醉药、阿片类激动剂静脉麻醉药;激素和激素调节剂,如堕胎药、肾上腺药、皮质类固醇肾上腺药、雄激素、抗雄激素、免疫生物制剂,如免疫球蛋白、免疫抑制剂、类毒素和疫苗;局部麻醉剂,如酰胺局部麻醉剂和酯局部麻醉剂;肌肉骨骼制剂,如抗痛风抗炎药、皮质类固醇抗炎药、金化合物抗炎药、免疫抑制抗炎药、非甾体抗炎药(NSAID),水杨酸酯抗炎药、矿物质;和维生素,如维生素A、维生素B、维生素C、维生素D、维生素E和维生素K。
可用于本公开的方法的组合物可包括细胞培养组分,例如包括氨基酸、金属、辅酶因子的培养基,以及少量的其他细胞,例如其中一些可通过干细胞的后继分化产生的细胞。
可用于制备本发明方法的组合物,例如,通过从培养基中沉淀出受试细胞并将它们重悬于所需的溶液或材料中。所述细胞可以从培养基中沉淀和/或更换,例如,通过离心、过滤、超滤等。
I.施用
在一些实施方案中,所述药物组合物可以单独施用或与前列环素共同施用。在一些实施方案中,所述药物组合物和前列环素同时施用。在其他实施方案中,所述前列环素和所述组合物分开施用。当单独给药时,所述前列环素可以在所述MSC组合物施用之前或之后施用。
本领域技术人员可以容易地确定组合物中细胞和任选的载体的量,并且可以用本公开的方法施用。在一个实施方案中,任何添加剂(除活性细胞外)以磷酸盐缓冲盐水中0.001~50%(重量)的溶液的量存在,并且活性成分以微克至毫克的量级存在,例如约0.0001至约5wt%,优选约0.0001至约1wt%,更优选约0.0001至约0.05wt%或约0.001至约20wt%,优选约0.01至约10wt%,更优选约0.05至约5wt%。当然,对于施用于动物或人的任何组合物,以及任何特定的施用方法,优选视情况确定,因此:毒性,例如通过在合适的动物模型例如,啮齿动物,如小鼠中确定致死剂量(LD)和LD50;以及,所述组合物的剂量,及其中组分的浓度和施用组合物引起合适的反应的时间。根据本领域技术人员的知识、本公开的内容和本申请引用的文献,这样的确定不需要过多的实验。并且,无需过度实验即可确定顺序施用的时间。
可用于本公开的方法的组合物尤其可通过局部注射施用,包括导管施用、全身注射、局部注射、静脉内注射、子宫内注射或肠胃外施用。当施用本申请所述的治疗组合物(例如,药物组合物)时,通常将其配制成单位剂量可注射形式(溶液、悬液、乳液)。
根据本公开的一个实施方案,所述组合物可与至少一种其他血管病变药物共同施用。例如,所述药物组合物可与前列腺素I2(PGI2)、前列环素类似物、磷酸二酯酶-5(PDE-5)抑制剂、内皮素受体拮抗剂(ETRA)、酪氨酸激酶抑制剂或可溶性鸟苷酸环化酶刺激剂共同施用。
根据一个实施方案,治疗血管病变的所述方法还包括减少肺动脉血栓形成、减少肺动脉炎症、减少肺动脉内膜平滑肌的增殖、减少肺动脉丛状病变的形成、增加肺动脉中一氧化氮的量、增加肺动脉中PGI2的含量、降低肺动脉中内皮素-1的水平、减少肺动脉中生长因子的量,或促进肺动脉中适当的内皮形态。
尽管前述内容涉及特别优选的实施方案,但应理解,本发明不限于此。本领域普通技术人员可以想到,可以对所公开的实施例进行各种修改,并且这些修改旨在落入本公开的范围内。

实施例
以下实施例旨在为本领域普通技术人员提供如何制备和使用本申请所述方法和组合物的完整公开和描述,而非限制性的。
实施例1 MSC表型和形态学研究
本实施例验证了暴露于曲前列环素的MSC细胞的表型,并确定了暴露于曲前列环素而不会对MSC细胞产生细胞毒性作用的最佳剂量范围。
使用标准生长培养基扩增和接种一小瓶人骨髓来源的MSC。当95-99%汇合时,用磷酸盐缓冲盐水(PBS)彻底洗涤细胞,并暴露于含有剂量范围为250μg/mL至0.004μg/mL的曲前列环素的培养基中。培养48小时后,拍摄细胞并评估处理诱导的形态变化。
流式细胞术分析(图1)证明,在本研究中使用的骨髓MSC对于CD34、CD45和HLA-DR是阴性或低的,对于MSC标志物CD73、CD105和CD90是阳性的。MSC的定义由国际细胞疗法协会(Dominici et al.,Cytotherapy 8(4):315-7,2006)建立。
暴露于不同浓度的曲前列环素的MSC的图像(图2)显示,MSC在最高曲前列环素剂量(250μg/mL)下呈圆形并脱落,表明曲前列环素对MSC的细胞毒性作用。另一方面,剂量为83.3μg/mL或更低的曲前列环素不会引起与细胞死亡相关的形态学变化。
本实施例证明高剂量的曲前列环素负面影响MSC细胞活力,而低于83.3μg/mL剂量的曲前列环素不会引起这种细胞毒性作用(图2)。随后的研究包括一系列低曲前列环素剂量以避免细胞毒性作用。
实施例2曲前列环素对促炎细胞因子和抗炎细胞因子的影响
本实施例检测了曲前列环素对MSC中产生促炎细胞因子和抗炎细胞因子的影响。
如实施例1中所述,收获暴露于浓度范围为250μg/mL-0.004μg/mL曲前列环素的细胞。提取细胞的RNA,然后分析促炎细胞因子和抗炎细胞因子。已经确定细胞因子或参与内部和分泌细胞信号传导的小蛋白质在PAH的炎性发病机理中起作用(Groth et al.,Respiratory Research,15:47(2014))。
在体内慢性缺氧下,与对照相比,TNFα的循环水平增加了48%(图3A和3B)。在体外暴露于83.3和0.3μg/ml剂量之间的曲前列环素降低了MSC中促炎细胞因子TNFα的表达水平。
将MSC暴露于递增剂量的曲前列环素显示IL10和IL13信号传导相对于对照增加(图4)。
在缺氧诱导的PAH小鼠中,观察到TNFα循环水平增加48%(图3A)。有趣的是,与对照相比,暴露于0.3μg/mL~83.3μg/mL的曲前列环素使TNFα的表达降低了多达4.7倍(图3B)。进一步分析显示,相同的曲前列环素剂量范围增加了抗炎细胞因子IL10和IL13的表达水平。具体地,当暴露于9.3μg/mL曲前列环素时,MSC中IL10的表达增加5.4倍。当暴露于27.8μg/mL曲前列环素时,MSC中IL13的表达增加8.8倍(图4)。这些数据表明,低浓度曲前列环素暴露诱导了MSC的抗炎潜能。
实施例3曲前列环素影响对免疫抑制因子的产生
本实施例检测了曲前列环素对MSC产生几种免疫抑制因子的影响。
为几种其他抗炎因子,重新分析了对暴露于250μg/mL至0.004μg/mL的曲前列环素的MSC的RNA。评估了已经显示在MSC的免疫抑制性质中发挥特定作用的基因,包括IDO1、iNOS、HLA和TGFβ(Hoogduijn et al.,International Immunopharmacology,1496-1500(2010))。
相比于对照,暴露于增加剂量的曲前列环素的MSC显示出抗炎因子IDO1、iNOS、HLA和TGFβ的显著增加(图5)。
与未暴露的对照相比,MSC暴露于9.3μg/mL曲前列环素,TNFα(促炎细胞因子)减少,IL10、IL13、IDO、iNOS、HLA和TGFβ(抗炎因子)增加(图6)。
本实施例证明,相对于对照,暴露于增加剂量的曲前列环素的MSC显示出抗炎因子IDO1、iNOS、HLA和TGFβ显著增加(图5)。该基因表达谱与实施例2中的细胞因子谱相结合,表明在体外暴露于多种剂量曲前列环素48小时后MSC基因的表达模式改变。暴露于9.3μg/mL曲前列环素降低促炎细胞因子TNFα,并增加几种抗炎因子,包括IL10、IL13、IDO、iNOS、HLA和TGFβ(图6)。这些数据表明曲前列环素暴露的MSC可以作为疾病治疗中的免疫调节因子。
图7显示了曲前列环素暴露的免疫调节作用的建议模型。特别地,MSC的曲前列环素暴露通过细胞内(1)或核信号传导(2、3、4)降低促炎细胞因子和增加抗炎细胞因子。进一步的分析揭示,通过由RNA表达变化测量的核信号(2),暴露于曲前列环素的MSC向抗炎状态的遗传重编程(3)。
Claims (38)
1.一种治疗或预防血管病变的方法,包括
向有需要的受试者施用组合物,所述组合物包含(i)间充质干细胞(MSC)或(ii)与MSC接触并包含所述MSC的一种或多种组分的培养基的一部分,或(iii)衍生自MSC的外泌体;
其中,所述MSC已离体暴露于前列环素,并且与未暴露于前列环素的对照MSC相比,暴露于前列环素的所述MSC增加一种或多种抗炎因子的表达和/或减少一种或多种促炎因子的表达。
2.如权利要求1所述的方法,其特征在于,所述MSC已暴露于包括0.3μg/ml-83.3μg/ml的前列环素的组合物。
3.如权利要求1所述的方法,其特征在于,所述MSC已暴露于0.3μg/ml-10μg/ml的前列环素。
4.如权利要求1所述的方法,其特征在于,所述前列环素为曲前列环素、其衍生物或盐。
5.如权利要求1所述的方法,其特征在于,所述MSC暴露于所述前列环素至少24小时。
6.如权利要求1所述的方法,其特征在于,所述MSC暴露于所述前列环素至少48小时。
7.如权利要求1所述的方法,其特征在于,所述血管病变选自由动脉性肺动脉高压(PAH)、外周血管疾病(PVD)、严重肢体缺血(CLI)、冠状动脉疾病和糖尿病性血管病变构成的群组。
8.如权利要求1所述的方法,其特征在于,所述血管病变为动脉性肺动脉高压(PAH)。
9.如权利要求1所述的方法,其特征在于,所述MSC在扩增后暴露于所述前列环素。
10.如权利要求1所述的方法,其特征在于,与未暴露于所述前列环素的对照MSC相比,暴露于所述前列环素的所述MSC的肿瘤坏死因子α(TNFα)表达水平降低。
11.如权利要求1所述的方法,其特征在于,与未暴露于所述前列环素的对照MSC相比,暴露于所述前列环素的所述MSC的一种或多种抗炎因子的表达水平增加,所述抗炎因子选自由IL10、IL13、IDO、iNOS、HLA和TGFβ构成的群组。
12.如权利要求1所述的方法,其特征在于,暴露于所述前列环素的所述MSC的TNFα表达水平比未暴露于所述前列环素的对照MSC的表达水平低至少50%。
13.如权利要求1所述的方法,其特征在于,暴露于所述前列环素的所述MSC的IL10、IL13、IDO、iNOS、HLA和TGFβ中的至少一种的表达水平比未暴露于前列环素的对照MSC的表达水平高至少50%。
14.如权利要求1所述的方法,包括向有需要的受试者施用包含MSC的组合物,其中所述MSC已经离体暴露于浓度为0.3-10μg/mL的曲前列环素或其盐至少24小时。

18.一种制备包含间充质干细胞(MSC)或已与所述MSC接触并包含所述MSC的一种或多种组分的培养基的组合物的方法,包括将所述MSC离体暴露于前列环素,与未暴露于所述前列环素的对照MSC相比,暴露于所述前列环素的所述MSC增加一种或多种抗炎因子的表达和/或降低一种或多种促炎因子的表达;以及
分离所述MSC或至少一部分含有外泌体的所述培养基。
19.如权利要求18所述的方法,包括将所述MSC暴露于0.3μg/mL-50μg/mL的前列环素。
20.如权利要求18所述的方法,包括将所述MSC暴露于0.3μg/mL-10μg/mL的前列环素。
21.如权利要求18所述的方法,其特征在于,所述前列环素是曲前列环素、其衍生物或盐。
22.如权利要求18所述的方法,其特征在于,所述MSC暴露于所述前列环素至少24小时。
23.如权利要求18所述的方法,其特征在于,所述MSC暴露于所述前列环素至少48小时。
24.如权利要求18所述的方法,其特征在于,所述MSC在扩增后暴露于所述前列环素。

27.如权利要求18所述的方法,其特征在于,暴露于所述前列环素的所述MSC的TNFα表达水平比未暴露于所述前列环素的对照MSC的表达水平低至少50%。
28.如权利要求18所述的方法,其特征在于,暴露于所述前列环素的MSC的IL10、IL13、IDO、iNOS、HLA和TGFβ中的至少一种的表达水平比未暴露于所述前列环素的对照MSC的表达水平高至少50%。

37.如权利要求33所述的组合物,还包含至少一种用于治疗或预防血管病变的其他治疗剂。
38.一种包含具有改变的基因表达模式的MSC群体的组合物,其特征在于,所述改变的基因表达模式中,TNFα的表达水平比对照MSC的表达水平低至少50%,和/或IL10、IL13、IDO、iNOS、HLA和TGFβ中至少一个的表达水平比对照MSC的表达水平高至少50%,其中具有所述改变的基因表达模式的MSC群体占所述组合物中所有MSC的至少50%。
