背景技术
视网膜变性是由视网膜细胞的逐步和最终死亡引起的视网膜退化。具有几种视网膜变性的原因,包括动物或静脉阻塞、糖尿病性视网膜病变、R.L.F./R.O.P.(晶状体后纤维性增生/早产儿视网膜病变)或疾病(通常遗传性的)。它们可能以许多不同的方式存在,如视力损害、夜盲症、视网膜脱落、光敏感性、管状视(tunnel vision)和周边视力丧失至视力的完全丧失。
在视网膜变性疾病中,色素性视网膜炎(RP)是非常重要的实例。RP代表最重要的致盲的遗传性眼疾病类型之一。RP是临床上相当均质的但遗传上多样化的疾病组,其特征在于原发的视杆细胞死亡和随后继发的视锥细胞损失。该疾病开始于视网膜赤道部(retinal mid-periphery),且破坏持续进展直到达到视网膜中央并引起由视野的严重环状限制导致的“管状视”直到法定盲。进一步的临床关键特征是特征性形状的色素沉积("骨针(bone spicule)")和视网膜血管的逐步弱化。时间过程从在儿童期达到终末期疾病的早期发作型到可能保留某些有用的视力直到较晚的成年期的晚期发作型之间变化。
RP的发病率大约是1:4,000。RP遗传上是非常多样化的,超过50个RP基因是目前已知的。10-25%的RP病例显示常染色体显性的遗传模式(adRP),6-18%是X-连锁的(xRP)和20-30%是常染色体隐性遗传的(arRP)。另外40-50%是散发的且最有可能占到没有家族史的arRP病例的大部分。arRP是遗传上最多样性的具有单一基因的RP亚组,其通常仅占到总数的一小部分。在arRP中最常见的突变基因是EYS(5-12%)、USH2A(5-15%)、CRB1(~5%)和PDE6B(4-10%),但这些比率主要取决于被研究的人群。
PDE6A编码视杆细胞cGMP磷酸二酯酶的α亚基(RP43基因座)。RP43基因座中分别引起所谓的PDE6A-关联RP或43型RP的突变在2-4%的arRP病例中发现。因此,具有PDE6A-关联的arRP的患者的绝对数在德国大约为900和在欧洲大约为5,000。
遗传性视网膜营养不良如RP是由基因突变引起的眼疾病。由于该疾病的遗传特性,还没有找到治愈性的或对症的常规治疗。疾病的负担是如此严重而使得临床专家目前将RP置于用于这种治疗的候选列表的顶部。
Wert等,Gene therapy provides long-term visual function in a pre-clinical model of retinitis pigmentosa,Human Molecular Genetics 22(3).p.558-567(2013),也在2012年10月29日提前公布,公开了包含细胞类型特异性的小鼠视紫红质(RHO)启动子控制下的小鼠PDE6A基因的重组AAV2/8载体:AAV2/8(Y733F)-Rho-Pde6α。具有在编码PDE6的α亚基的基因中的突变的小鼠(Pde6αnmf363)的视网膜用该载体转导。作者提到,注射增强感光细胞的存活并改善视网膜功能。但是,根据本发明人,这种方式的基因置换的结果并不是令人满意的,表明这一策略对于患视网膜变性(如43型RP)的人的治疗可能是具有较少前景的。
发明内容
针对这一背景,本发明的目的是提供解决这些局限并因此成为治疗视网膜变性疾病(如RP,特别是43型RP)的有价值的工具的多肽和核酸载体。
这一目的通过包含转基因表达盒的多核苷酸实现,该转基因表达盒包含(a)编码人视紫红质基因(hRHO)的启动子的核酸;(b)编码人视杆cGMP-特异性磷酸二酯酶6Aα亚基(hPDE6A)或其片段的核酸和(c)编码调控元件的核酸。
作为本发明基础的目的由此完全实现。
本发明人能够认识到,本发明的多核苷酸体现了允许成功治疗视网膜变性疾病(如RP)的遗传工具(其可应用于人类患者)的必要组分。
本发明人通过实验证明,接受作为载体质粒的组分的本发明多核苷酸的视网膜下注射的PDE6A-缺陷小鼠在视杆细胞中有效地和特异性地表达hPDE6A转基因。另外,还证明视杆介导的视力被赋予从出生起缺乏视杆功能的这些小鼠。
这一发现是令人惊异的。具有如本发明所提出的结构的多核苷酸导致视网膜中靶向的hPDE6A转基因表达,这在本领域中并非显而易见的。因此,所观察的高特异性和选择性以及本发明的多核苷酸在恢复视觉功能中的显著生物学功效对于本领域技术人员并不是不言而喻的。
如本发明人在初步实验中证明的,在注射到视网膜中后,本发明的多核苷酸特别地在视杆细胞中以功能活性的方式表达。另外的实验表明,多核苷酸在原位仅保留最小的脱靶器官转导。进一步的实验也表明,在本发明的多核苷酸注射到视网膜中后,没有发生针对所施用的多核苷酸的抗药物抗体的诱导。这允许得出本发明的多核苷酸非常适合作为用于治疗视网膜变性疾病如RP的药物组合物的活性药剂的结论。
根据本发明,"多核苷酸"是由在链中共价键合的13个或更多个核苷酸单体组成的生物聚合物分子。优选的多核苷酸的实例是DNA分子。尽管根据本发明的多核苷酸可以是单链的或双链的,但在优选的实施方式,多核苷酸是单链的。
"启动子"是促进特定基因的转录的DNA区域。作为转录过程的部分,合成RNA的酶(称为RNA聚合酶)靠近基因附着于DNA。启动子包含提供用于RNA聚合酶和用于募集RNA聚合酶的转录因子的初始结合位点的特定DNA序列和反应元件。
人视紫红质基因(hRHO)的启动子是指促进人视紫红质的转录的DNA区域。hRHO启动子的整个核苷酸序列公开于Allocca等,Novel adeno-associated virus serotypesefficiently transduce murine photoreceptors,Journal of Virology,81(2007)11372-11380中。在一个实施方式中,使用整个启动子核苷酸序列。在本发明的另一实施方式中,仅使用启动子的功能部分,其是hPDE6A的靶向表达所需要的。在本发明的又一实施方式中,使用之前所述的核苷酸序列与其它启动子核苷酸序列、内含子序列或调控元件序列的融合体。
"转基因表达盒"或"表达盒"包含核酸载体将要递送到靶细胞的基因序列。这些序列包括目标基因(例如,hPDE6A核酸)、一个或多个启动子和调控元件。
"调控元件"是在靶细胞中有效表达基因(例如,hPDE6A核酸)所需的调控元件,且因此应当包括在转基因表达盒中。这样的序列可以包括,例如,增强子序列、促进DNA片段插入到质粒载体内的多接头序列或负责内容子剪接和mRNA转录物的多腺苷酸化的序列。
"核酸"或"核酸分子"是由单体核苷酸的链组成的分子如,例如,DNA分子(例如,cDNA或基因组DNA)。核酸可以编码,例如,启动子、hPDE6A基因或其片段或者调控元件。核酸分子可以是单链或双链的。

在本发明的优选的实施方式,所述调控元件包含c1)编码土拨鼠口炎病毒(woodchuck stomatitis virus)转录后调控元件(WPRE)的核酸。

本发明的另一主题是包含根据本发明的上述多核苷酸的核酸载体。因此,多核苷酸的特征、优势和特性同样适用于本发明的核酸载体。
在优选的实施方式中,根据本发明的核酸载体是病毒载体如源自腺相关病毒、腺病毒、逆转录病毒、慢病毒、牛痘/痘病毒或疱疹病毒(例如,单纯疱疹病毒(HSV))的载体。在最优选的实施方式中,载体是腺相关病毒(AAV)载体(参见以下)。
在本发明的一个实施方式中,核酸载体是进一步包含长度≥5,000bp,优选≥5,500bp的骨架的环状质粒。
根据本发明,术语"骨架"是指表达盒或(如果存在)末端反向重复序列(ITRs)以外的载体分子的区段。换句话说,载体的骨架分别与表达盒或ITRs的5'和3'末端相邻,且形成根据本发明的多核苷酸之外的载体核酸的其余部分。
发明人已经认识到,这一优选尺寸的骨架会使骨架错误或反向包装到病毒颗粒中(而不是表达盒的包装)最小化。因此,这一手段确保基本上仅hPDE6A可在靶细胞中用于表达。

骨架优选包含作为唯一的编码和信息承载序列的选择标记和ORI,及对于其余随机序列,但不是ORFs、启动子或RERSs。

在优选的实施方式,本发明的核酸载体是腺相关病毒(AAV)载体。

本发明的另一主题涉及包含以上更详细地描述的核酸载体和药学上可接受的载体的药物制剂。因此,多核苷酸和核酸载体的特征、优势和特性同样适应于本发明的药物制剂。
药学上可接受的载体是本领域中公知的。举例来说,参照Rowe(Ed.)(2012),Handbook of Pharmaceutical Excipients,6th Edition,Pharmaceutical Press。药物制剂可以进一步包含添加剂。这些包括对于根据本发明的核酸载体的效力有利的任何化合物或组合物,如盐、粘合剂、溶剂、分散剂、佐剂和通常结合基因治疗方法使用的其它物质。

利用以上更详细地描述的多核苷酸和核酸载体,发明人首次提供了允许PDE6A-关联的RP的病因治疗(causative treatment)的治疗工具。
本发明的另一主题涉及包含(a)根据本发明的多核苷酸和/或根据本发明的核酸,和/或根据本发明的药物制剂,及(b)其使用说明的试剂盒。
本发明的进一步主题涉及制备重组腺相关病毒(rAAV)载体的方法,该方法包括将根据本发明的多核苷酸插入腺相关病毒载体中,优选地所述重组腺相关病毒载体是根据本发明的核酸载体。

实施例
1.本发明的核酸载体
在这一示例性的实施方式中,rAAV.hPDE6A载体是携带人视杆细胞cGMP磷酸二酯酶的hPDE6A亚基的cDNA的杂合AAV基载体。hPDE6A cDNA表达在视杆特异性的视紫红质启动子(hRHO)的控制下且使用突变的土拨鼠口炎病毒转录后调控元件(WPRE)序列增强。表达盒侧邻AAV血清型2末端反向重复序列(ITRs)且重组基因组包装在AAV血清型8衣壳中。表达盒包含以下元件:

2.phRHO.hPDE6A.WPREm顺式载体质粒
在一个示例性的实施方式中,使用phRHO.hPDE6A.WPREm顺式载体质粒骨架,其包含含有806bp视杆细胞特异性的人视紫红质(hRHO)启动子[参见Allocca等,Novel adeno-associated virus serotypes efficiently transduce murine photoreceptors,Journal of Virology,81(2007)11372-11380]和全长(2579bp)人PDE6A cDNA的表达盒。表达盒也包含543bp的具有突变的WXF-开放阅读框的土拨鼠肝炎病毒转录后调控元件(WPRE)[Zanta-Boussif等,Validation of a mutated PRE sequence allowing high andsustained transgene expression while abrogating WHV-X protein synthesis:application to the gene therapy of WAS,Gene therapy,16(2009),605-619]和207bp的牛生长激素多腺苷酸化信号(BGHpA)。包含卡那霉素抗性(KanR)的具有SEQ ID No.8中所示核苷酸序列的5591bp载体骨架位于距L-ITR 1943bp和距R-ITR 2853bp处且距pUC18ori2024bp。
rAAV.hPDE6A载体使用顺式载体质粒和反式pDB8-KanR辅助质粒在人胚肾293细胞(HEK293)中的瞬时双重转染产生。细胞裂解产物通过低速离心澄清且载体然后通过用于浓缩和缓冲液交换的2轮顺序氯化铯梯度超速离心接着切向流过滤步骤纯化。所得的rAAV.hPDE6A载体悬浮液然后无菌过滤并作为药物产品装入小瓶。
phRHO.hPDE6A.WPREm顺式载体质粒图谱的两种实施方式显示于图2A、B中。
3.通过rAAV.hPDE6A赋予的生物学活性和转基因表达
为证实生物学活性和转基因表达,本发明人递送rAAV.hPDE6A载体到2周龄PDE6AD670G突变小鼠的视网膜下腔中[参见Sakamoto等,New mouse models for recessiveretinitis pigmentosa caused by mutations in the Pde6a gene,Hum Mol Genet,18(2009)178-192]。递送过程类似于Michalakis等,Restoration of cone vision in theCNGA3-/-mouse model of congenital complete lack of cone photoreceptorfunction,Molecular therapy:The Journal of the American Society of GeneTherapy,18 2057-2063(2010)中描述的。小鼠在处理的眼(TE)中接受视网膜下注射,而另一只未处理的眼(UE)用作对照。载体效率在注射后的3周时通过视网膜电图(ERG)(体内分析中的客观功能(objective functional))评估。PDE6A D670G突变小鼠缺乏正常的视杆细胞功能。继发于视杆细胞,未受影响的视锥细胞也退化,导致在疾病的晚期阶段视锥功能的损失。因此,特别地测试视杆和视锥功能的ERG实验方案适合作为PDE6A功能的直接测量及用于评价rAAV.hPDE6A载体的生物学活性(BAA)。
代表性的结果参见图3。rAAV.hPDE6A载体处理导致通过处理和未处理的眼睛之间特定ERG成分幅度的明显侧边差异(side difference)反映的在处理的眼中的明显治疗效果。在完成ERG测量后,处死小鼠,眼睛去核和处理用于hPDE6A转基因表达的免疫组织学分析(转基因表达测定,TEA)。为此,组织被固定并冷冻包埋。纵向冷冻切片用针对人PDE6A蛋白的兔多克隆抗体(抗-PDE6A,#NBP1-87312,Novus,Littleton,CO)染色。免疫信号用Alexa488标记的驴抗-兔IgG二抗检测。使用Leica SP8SMD共焦激光扫描显微镜收集来自免疫染色的冷冻切片的共焦图像。抗-PDE6A抗体也检测小鼠Pde6a蛋白,并在野生型小鼠视网膜的视杆细胞外节中给出特异性的信号和在Pde6a-缺陷(PDE6A D670G突变体)视网膜中不给出信号。在用rAAV.hPDE6A载体处理后,PDE6A的明显和特异性的信号在处理的眼睛的视杆细胞中观察到,其在未处理的眼睛中不存在;参见图4。视网膜冷冻切片用核染料Hoechst3442的染色也揭示处理对视网膜变性的有益效果,这在处理的眼睛中作为与未处理的眼睛相比的视网膜外核层的实质保持来证明。
代表性的结果参见图4:来自用rAAV.hPDE6A载体处理的PDE6A D670G突变小鼠中hPDE6A的免疫组织学染色(浅灰色)的代表性共焦图像。抗体(抗-PDE6A,在所有实验中1:500的工作稀释度)也检测小鼠PDE6A蛋白并在野生型小鼠视网膜视杆细胞外节(图像上部强烈染色的结构)中给出特异性的信号(A,上图)和在未处理的PDE6A D670G突变小鼠的视网膜中没有特异性的信号(B,下图)。rAAV.hPDE6A载体编码的hPDE6A蛋白的特异性信号显示于(B,上图)中。(A)图中的下图显示仅二抗对照染色。视网膜截面也用Hoechst 3442染料染色以显现视网膜细胞核(灰色信号)。rAAV.hPDE6A-介导的hPDE6A表达明显延迟PDE6AD670G突变体视网膜中的视杆细胞退化:比较(B)图中的上部(处理的眼睛)和下部(未处理的眼睛)之间的外核层(ONL)厚度。
总之,rAAV.hPDE6A载体有效地和特异性地在PDE6A D670G突变小鼠的视杆细胞中表达hPDE6A转基因,这在处理的区域中显著地延迟光感受器退化。重要的是,视杆细胞的延长存活抵消了“旁观者效应(bystander effect)”(其导致视锥细胞的损失-与视杆损失的所有区域中的基础疾病无关)。生物学活性数据证明在这些小鼠中视锥介导的视力的延长保持,这对于在人色素性视网膜炎的相应形式中治疗的施用是特别有意义的。
4.非人灵长动物中视网膜下注射后的AAV8生物分布和脱落
在另一研究中,21只食蟹猴在三个群组(高剂量:1x1012载体基因组[vg],低剂量:1x1011vg,或仅溶媒对照)中接受视网膜下注射。病毒分布和脱落在非人灵长动物中在临床级重组腺病毒相关病毒(rAAV)的单次视网膜下施用后进行分析。这对于根据本发明的基因治疗方法的环境风险评价是重要的。
组织样品在尸检(第91天)时从处理的眼睛、引流的淋巴结、唾液腺和脾脏、视神经、脑和脊髓、心脏、肺、肝、肾上腺和性腺收获。血液、尿液、泪腺和鼻腔拭子在给药前及在载体施用后1、2和3天及1、4和13周后从各动物收获用于DNA提取和通过qPCR的载体基因组定量。
剂量依赖性的rAAV DNA在处理的视网膜中发现。可定量水平的rAAV DNA也在低剂量的两个动物和高剂量组的3个动物的视神经中检测到。rAAV DNA的瞬时脱落在鼻腔和泪腺流体及尿液中发现。rAAV DNA未在种系组织中检测到,且除了在少量动物中在淋巴结和脾中检测到的零星信号外,所有的其余组织是阴性的或显示低于定量极限的水平。血液样品在所有测试的时间点是阴性的或显示不可定量的水平。
这些数据与本发明的临床实施相关,其中实验受试者、研究者和监管者同样有兴趣鉴定与遗传修饰的生物体的施用相关的环境风险。尽管脱落到生物流体中似乎以剂量依赖性的方式发生,但脱靶器官的转导看起来是最少的。
5.非人灵长动物中对于视网膜下AAV8的体液免疫反应
了解在非人灵长动物中对于临床级重组腺相关病毒(rAAV)的单次视网膜下施用的体液免疫反应是开发用于根据本发明的视网膜基因疗法的安全和有效的临床实验方案的关键因素。出于这一原因,本发明人研究了在rAAV8-假型病毒的单次视网膜下施用后非人灵长动物(食蟹猴(Macacca fascicularis))中的抗药物抗体(ADA)滴度。
21只猴子在三个群组(高剂量:1x1012载体基因组[vg],低剂量:1x1011vg,或仅溶媒)中接受视网膜下注射和等同于临床实验场景的伴随的免疫抑制疗法。基线样品在施用载体后1、2和3天及1、4和13周获取的那些样品比较。
本研究提供了与本发明的临床应用相关的数据,其中rAAV8可以用于hPDE6A转基因的视网膜下递送。当模拟具有临床级载体、手术和伴随的免疫抑制的临床场景时,在非人灵长动物中不发生抗药物抗体的诱导。
6.毒性
毒性的评价是基于临床观察的评估、体重、眼底和裂隙灯(SLO)检查、宏观检查、眼底照相、光学相干断层扫描(OCT)-(包括吲哚青绿血管造影术和荧光血管造影术[FA])、眼内压(IOP)、视网膜电图(ERG)、器官重量及宏观和微观评价。
没有看到测试项相关的死亡或临床观察结果。
没有不良的或测试项相关的发现在体重、ECG、血压、临床病理、尿分析或免疫球蛋白评估中观察到。
在眼科评价期间没有看到测试项相关的变化。IOP(瞬时提高)、ERG(增加的潜时(implicit times))、OCT(光感觉器外节的缩短)、SLO(视网膜色素上皮[RPE]的聚结、RPE或脉络膜的萎缩、FLA的渗漏)和眼底镜检查(过度色素沉着和色素减退)发现是过程相关的,因为它们也在对照猴中发生。
非常细微的过程相关的(即,由视网膜下注射引起的)结构性变化发生于外层视网膜(光感受器外节[POS])中。这些变化包括POS的缩短,POS是经历恒定的循环/更新(每天10%)的结构。POS已知在视网膜下手术后缩短多达6个月(Schaffer等,1993)。
在器官重量或宏观观察上没有发现。
总之,rAAV.hPDE6A的视网膜下施用在雄性和雌性食蟹猴中直到1x 10E12vg的最高剂量是良好耐受的。唯一的测试品相关的变化是最少的短暂性视网膜血管周炎症,其可能与rAAV载体相关。RPE中的最小干扰在所有组(包括对照)中发现,且是过程相关的。

Claims (15)
1.一种多核苷酸,其包含

(c)编码调控元件的核酸。
