Abstract

Description

病毒感染是当今科学和医学的巨大挑战之一。关注的人致病病毒之一是人免疫缺陷病毒(HIV)。HIV是一种属于逆转录病毒科并且属于慢病毒属的包膜病毒。未治疗的HIV感染通常在不同长度的无症状潜伏期(通常持续数年)后导致AIDS(获得性免疫缺乏综合征)。2010年在世界范围内在15至49岁成人中HIV的患病率为0.8%。对于中欧和西欧,其为0.2%。其在撒哈拉以南的非洲(5.0%)和加勒比地区(0.9%)高于平均值。在一些国家,诸如斯威士兰、博茨瓦纳或莱索托,15至49岁的人中约四分之一感染了HI病毒。在德国,2009年HIV患病率为大约0.1%。
为了繁殖,HI病毒需要在表面携带称为CD4受体的宿主细胞。主要携带CD4的T辅助细胞,称为CD4+细胞或CD4+淋巴细胞。HI病毒的主要储库是体内淋巴滤泡中的滤泡T辅助细胞,其构成了2 %的CD4+细胞。潜伏的受感染的、休眠的CD4+ T细胞或T记忆细胞,分别代表了HIV的长期储库。这些细胞导致了这样的事实:当今可得的抗逆转录药物仍不能使得HIV被根除,且其会在疗法中断后重复出现复发。
因为病毒的繁殖发生在正常细胞内部且与中心生化细胞机制紧密相连,考虑的抗病毒药物必须防止病毒颗粒渗透至宿主细胞中,干预细胞的代谢以损害病毒繁殖,或者在可能在细胞中发生病毒繁殖后,防止新病毒从细胞中逃逸。
但是,在另一方面,这些受青睐的活性药物还必须是总体上身体代谢、细胞结构和/或细胞内部代谢所能耐受的,否则不仅在细胞内的病毒繁殖而且在最糟的情况下全部受治疗的生物的(细胞的)生命也会停止。
因为这些条件非常难以调节,迄今开发的一些抗病毒药物通常与严重的副作用风险相关。
目前选择用于HIV感染和AIDS的疗法被称为高效抗逆转录病毒疗法(HAART)。HAART是一种联合药物疗法,其由至少三种抗逆转录病毒药物组成。目前使用以下几类活性成分:核苷和核苷酸类似物(NRTI)、非核苷逆转录抑制剂(NNRTI)、HIV蛋白酶抑制剂(PI)、进入和融合抑制剂和整合酶抑制剂。
一些上述活性药物被称为“直接作用抗病毒药物”(DAA)。DAA特异性地抑制病毒蛋白,但不抑制宿主的细胞蛋白。该作用原理应当导致较少的副作用。
但是,恰好在DAA中受到攻击的病毒还发展出抗性,这是它们所擅长的,由于它们极快的复制周期和该复制的生化特征。
此外,目前使用的抗病毒药物的一个缺点是它们是病毒抑制性的,而非病毒毒性的。它们抑制病毒的繁殖,但是不能杀死它们。
针对该背景,本发明的目的是提供新型抗病毒药物,其中目前可得的抗病毒药物的缺点被减弱了,甚至可能被避免了。尽可能地,应当提供这样的活性药物,其杀死病毒或病毒感染的细胞,即其为病毒毒性的。
该目标通过提供细胞毒性药物实现,其被配置为用于选择性结合至病毒感染的T淋巴细胞的膜受体,其中所述膜受体选自CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29。
使用新的基于FACS的筛选方法,本发明人惊讶地发现上述膜受体在病毒感染的T淋巴细胞中相比未感染病毒的T淋巴细胞过表达。因此,这些膜受体首先能够鉴定病毒感染的T淋巴细胞。
现有技术尚未描述上述膜受体和T淋巴细胞的病毒感染之间的关联。因此它们代表了具有治疗意义的新的生物标志物。
根据本发明的细胞毒性药物利用了发明人发现的现象。其选择性结合至病毒感染的T淋巴细胞并且在那里展示出细胞毒性活性。未感染的细胞没有结合或结合显著更弱。因此将副作用最小化。
与针对病毒疾病的大多数目前可得的治疗剂不同,根据本发明的细胞毒性药物不仅具有病毒抑制作用,因此不仅防止了病毒的扩散和繁殖。根据本发明的细胞毒性药物还具有病毒毒性作用且因此具有选择性清除受感染的T淋巴细胞和存在于细胞中的病毒和病毒前体的潜力。
根据本发明的细胞毒性药物因此满足了感染性病毒疾病治疗中的因此有效的治疗剂的长效需求。
根据本发明的“药物”意为这样的物质,由于其化学-物理特性,其作用于病毒感染的T淋巴细胞的存活性和/或繁殖。所述药物可以是化学限定的化合物、活性成分、药物等。
根据本发明,“细胞毒性”意为这样的活性,其对病毒感染的T淋巴细胞的存活性和/或繁殖发挥抑制影响。
根据本发明的“选择性结合”是使细胞毒性药物靶向和特异性偶联至膜受体,其是基于根据钥-锁原理的选择性相互作用。这与非选择性结合不同,所述非选择性结合是基于药物和受体之间的非特异性相互作用。
“T淋巴细胞”形成了一组作为免疫应答的白血细胞,并且根据本发明包含全部T细胞,特别是T辅助细胞或记忆细胞,其携带CD4受体(CD4+T淋巴细胞)。
“CD”代表“分化抗原簇”并且指的是可以根据生化或功能标准分类的细胞的免疫表型表面特征的组。CD分子大多数是膜结合糖蛋白,其通常是细胞特异性表达的且可以具有不同的功能:一些CD具有受体或信号传导功能,而其它的已经显示出具有酶促活性;另外,一些成簇分子在胞内通讯中起重要作用。
“CD134”也被称为肿瘤坏死因子受体超家族成员4(TNFRSF4)或OX40,其为在休眠的初始T-细胞上组成型表达的受体的TNFR超家族的成员。CD134是次级的、共次级的免疫检查分子。假定CD134在前三天期间对CD4+ T细胞的繁殖活性没有作用,但是在该时间之后繁殖减缓并且细胞以增加的程度死亡。CD134已经被描述为涉及与多种病毒感染(诸如H5N1禽流感)相关的称为细胞因子风暴的病理学(一种免疫系统的过度反应),其产生高浓度的炎性细胞因子,所述炎性细胞因子转而导致白细胞形成另外的细胞因子。
“CD132”也被称为常规γ链(γc)或白介素2受体亚基γ(IL-2RG),其为至少六中不同的白介素受体(IL 2、IL 4、IL 7、IL 9、IL 15和IL 21受体)的受体复合物的常规细胞因子受体亚基。淋巴细胞表达的CD132可以形成这些细胞因子的功能受体。现有技术已经描述了与CD132中的功能障碍或突变相关的疾病,诸如X-SCID(“重度联合免疫缺陷症”)或精神分裂症。

“CD54”也被称为ICAM 1(胞内黏附分子1),其为一种通常在内皮细胞和免疫系统的细胞中表达的跨膜糖蛋白。其为免疫球蛋白超家族的成员。在细胞因子刺激后,存在CD54浓度的显著增加。CD54被白介素1(IL1)和肿瘤坏死因子(TNF)指示并且通过血管内皮、巨噬细胞和淋巴细胞表达。CD54尤其在细胞的信号传导中起重要作用。CD54与蛛网膜下腔出血(SAH)有关,其中游离的血液进入脑脊液填充的蛛网膜下腔空间。
“CD39”也被称为“外核苷三磷酸二磷酸水解酶-1”(ENTPD1),其为催化三磷酸核苷和二磷酸核苷的 γ-和β-磷酸残基水解为单磷酸核苷衍生物的跨膜蛋白。
“BTLA”也被称为“CD272”,其在T细胞活化期间被诱导。其通过与肿瘤坏死因子受体的相互作用导致T细胞抑制。BTLA的活化涉及人CD8+癌症特异性T细胞的功能抑制。
“CD97”或“BL-Ac[F2]”是黏附GPCR家族的成员。CD97在造血干细胞或祖细胞、免疫细胞、上皮细胞、肌肉细胞和它们的恶性变体上表达。在免疫系统中,CD97在宿主防御中作为关键中介物起作用。在免疫系统外,CD97可以涉及细胞-细胞间相互作用。在多种肿瘤中发现了CD97,包括甲状腺癌、胃癌、胰腺癌、食道癌、结肠癌和口腔鳞状细胞癌。
“CD2”是在T细胞和自然杀伤细胞(NK细胞)的表面发现的细胞黏附分子。其它名称包括T细胞表面抗原T11/Leu-5、LFA 2、LFA 3受体、红细胞受体和莲座状受体(rosettereceptor)。CD2与其它黏附分子相互作用。一大部分T细胞淋巴瘤和白血病表达CD2,因此在现有技术中CD2被用来将这些病症与B细胞肿瘤进行区分。
“CD50”或“胞内黏附分子3”(ICAM 3)是跨膜糖蛋白。其通过全部白细胞组成型地且显著地表达,并且被认为是免疫应答起始中LFA 1的最重要的配体之一。
“CD161”也被称为“杀伤细胞凝集素样受体亚家族B成员1”、“NK1.1”、“KLRB1”、“NKR P1A”,其为通过NK细胞表达的跨膜糖蛋白并且表现出涉及NK细胞功能的调节。
“CD218”或“白介素18受体α”(IL 18 Ra)是IL 1受体超家族的成员。CD218在Th1细胞,一种亚型NK细胞,嗜中性细胞,IL 12刺激的扁桃体B细胞上表达。CD218似乎在天然免疫应答和自身免疫反应中起重要作用。
“CD226”也被称为“PTA1”(“血小板和T细胞活化抗原1”)或“DNAM 1”(“DNAX辅助分子1”),其为在NK细胞、血小板、单核细胞和部分T细胞上表达的跨膜糖蛋白。CD226介导细胞黏附至携带其配体CD112和CD155的其它细胞。
“CD7”是在胸腺细胞和成熟T细胞上表达的跨膜蛋白。其在淋巴系统的早起发育期间在T细胞的相互作用中起重要作用。
"“CD49d”是形成α4β1淋巴细胞归巢受体的部分的整合蛋白α亚基。
“CD29”或“整合蛋白β1”是整合蛋白亚基,其例如与α3亚基相互作用以形成α3β1-复合物,其与分子诸如轴突生长诱向因子1和颤蛋白反应。与其它整合蛋白一样,CD29涉及不同过程中的细胞黏附和细胞识别,所述过程包括胚胎发育、止血、组织修复、免疫应答和肿瘤的转移扩散。
上述膜受体在病毒感染的T淋巴细胞,例如在感染HIV的CD4+ T淋巴细胞中的过表达是令人惊讶的且没有被现有技术指出。因此这是未曾预料到的。
本发明的目标从而完全实现。
根据本发明的优选的实施方案,细胞毒性药物与用于选择膜受体的结合分子偶联。
使用该措施,本发明利用化合物的特性以高特异性和选择性的方式结合至本发明人发现的膜受体。“结合分子”包括任何能够将细胞毒性药物选择性和特异性偶联或连接至至少一种膜受体:CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29的化合物。
根据本发明进一步的实施方案,结合受体选自:免疫球蛋白、免疫球蛋白片段、适体、低分子量化合物、受体、受体片段、细胞因子。
在本发明的进一步发展中,使用的此类结合分子特别适合用于将细胞毒性药物选择性和特异性偶联至鉴定的过表达的膜受体。
免疫球蛋白可以是免疫球蛋白和抗体,优选激动性抗体。
根据本发明人的发现,所述细胞毒性药物可以特别有利地偶联至这样的结合分子并且可以特异性和选择性结合至病毒感染的T淋巴细胞的鉴定的膜受体之一。
在本发明的实施方案中,所述免疫球蛋白选自MEDI6469 (9B12)、MEDI6383、MEDI0562、Hu106-222/Hu119-122 (UTMDACC)。
这些是根据本发明优选的抗体并且涉及针对CD134膜受体,且它们因此特别适合作为结合分子。MEDI6469是来自MedImmun, AstraZeneca的激动性鼠抗体,其目前在I期临床试验中被测试用于治疗人中的实体瘤。

根据本发明的细胞毒性药物的进一步的实施方案,病毒感染的T淋巴细胞是病毒感染的CD4+T淋巴细胞。

根据提及的细胞抑制剂,使用的药物被广泛研究并且可以获得足够的药物安全性。它们因此特备适合用于实现本发明的目的。
根据优选的实施方案,根据本发明的细胞毒性药物包含病毒抑制药物。

其它主题涉及用于诊断病毒感染的方法,其包括确定病毒感染的T淋巴细胞相对未感染的T淋巴细胞的膜受体的任何过表达,其中膜受体选自CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29。
本发明人已经意识到过表达的提及的膜受体是诊断标志物,通过它们可以快速检测病毒感染。T淋巴细胞上的过表达检测与病毒表达的患病率相关。
根据本发明的细胞毒性药物的实施方案、进一步的发展、特征、特性和优点同样适用于根据本发明的方法。
本发明另外的主题是用于确定病毒感染的T淋巴细胞上的表面结构,优选膜受体的方法,所述表面结构适合作为细胞毒性药物的靶标,其中所述方法包括使用病毒感染T淋巴细胞并且确定病毒感染的T淋巴细胞相对未感染的T淋巴细胞的表面结构的任何过表达。
本发明人已经开发了通过病毒(例如HI病毒)感染的初级T淋巴细胞的模型。下一步是分析适合作为细胞毒性药物的靶标的哪些表面结构在感染的T淋巴细胞上过表达。与未感染的T淋巴细胞相比过表达越强,则高度特异性结合至该表面结构的物质更具有选择性且更适合作为根据本发明的细胞毒性药物。
要理解的是上文描述的和下文将要解释的那些特征不仅可以以指示的具体组合使用,还可以以其他组合或以分开的方式使用,而不背离本发明的范围。
本发明现在将通过示例性实施方案的方式更详细的解释,所述实施方案具有进一步的特征、特性和优点。所述实施方案不是限制性的。
要理解的是在实施方案中公开的个别特征不仅以非常具体的实施方案的背景公开,还以常规意义公开并且它们自身为本发明提供了分别的贡献。本领域技术人员因此可以将这些特征与本发明的其它特征自由组合。

图1 示意地显示了用于确定感染HIV-1的T淋巴细胞中表面结构的筛选方法的建立,其是用于寻找根据本发明的细胞毒性药物的基础。
图2显示了相对于未感染的初级人CD4+T淋巴细胞,在感染HIV-1的初级人CD4+T淋巴细胞的各种膜受体表达的增加(A),和从HIV患者中分离的在CD4+T淋巴细胞上的CD134的增加的表达(B)。
实施方案
1. 感染HIV-1的T淋巴细胞的筛选
从健康供体的外周血中分离CD4+T淋巴细胞。用植物血球凝集素(PHA)刺激分离的CD4+T淋巴细胞3天。随后,将刺激的细胞用经由内部核糖体进入位点(IRES)连同nef 开放阅读框表达CFP(“青色荧光蛋白”)的HIV-1感染。将感染的CD4+T淋巴细胞的表面在感染后用存在于4个96孔微量滴定板中的332 PE 缀合抗体染色48至72小时。为了关闭自体荧光,将感染的和未感染的细胞针对别藻蓝蛋白(APC)过滤。对于分析,使用感染的CFP阳性细胞和未感染的CFP阴性细胞的PE平均值。在图1中示意地显示了感染HIV-1 的 CD4+T 淋巴细胞的受体表面筛选的建立。
2. 感染HIV-1 的 CD4+T 淋巴细胞中的膜受体的过表达
在CD4+T 淋巴细胞感染48至72小时后,如预期的显示了已经在现有技术中描述的与HIV感染相关的膜受体(CD45RO、CD25、CD150和CD279)的过表达。这些结果确认了开发的筛选方法的稳健性和可靠性。
但是,令人惊讶的,本发明人能够发现大量在感染的细胞上过表达的其它的膜受体,其目前没有被描述过。在细胞表面表达中增加最高的是CD134 (OX40),其为T细胞共刺激分子和细胞因子受体,参见图2A。
数值指示感染的T 淋巴细胞上的受体表达相对于未感染的T 淋巴细胞的增加x倍(n=5,对于全部受体分子显示p值< 0.01)。其中已经描述了在HIV感染的CD4+T 淋巴细胞中表达增加的受体用黑色标记。对于全部其它白色标记的受体,在HIV感染的CD4+T 淋巴细胞中的过表达是未知的并且因此是令人惊讶的。
从感染HIV-1的个体的血液中分离CD4+T 淋巴细胞。分析细胞表面的CD134表达以及分析细胞内部的HIV-1的p24。通过流式细胞术实施测量。结果显示在图2B中。
图表显示在p24-(未感染)T细胞相对来自10位患者的p24+(感染的)T细胞中CD134的平均荧光强度(MFI)。使用配对student T检验进行统计计算。因此证实CD134表达在来自HIV-1患者血液中的感染的CD4+T 淋巴细胞上同样显著增加。
因此,基于CD134表达,感染HIV-1的CD4+T 淋巴细胞可以与未感染细胞特异性地区别。CD134因此是抗HIV-1免疫疗法的理想靶标。
3. 用于治疗HIV感染的细胞毒性药物
目前,现有技术已经可以获得一些针对人CD134的抗体。其中的一个实例是由MedImmune公司开发的分别命名为MEDI6469或9B12的抗体。该激动性抗体目前仅用于治疗实体瘤。已经在人中测试了其安全性和抗肿瘤活性(Curti 等人, 2013, loc. cit.)。目前在临床试验中测定9B12抗体用于肿瘤疗法的适用性。根据本发明人的发现,该抗体还潜在地适合用于清除病毒感染的CD4+T 淋巴细胞,并且因此将致病性病毒从受感染宿主中移除。
如下检测细胞毒性特性:
从健康供体的血液中分离PBMC。使用表达GFP或CFP的HIV-1感染分离的细胞。这允许由发色团的表达鉴定受感染的细胞群。添加不同量的抗CD134抗体,例如MEDI6469 (9B12)。使用抗小鼠PE作为二级抗体以通过流式细胞术检测结合所述抗体的细胞。该实验允许评价由于细胞CD134表达而特异性结合至HIV-1感染的细胞,而不结合未感染细胞的抗体的。其还允许确定不同剂量是否对于HIV-1感染的细胞和未感染的细胞的特异性或非特异性结合的水平具有影响。
接下来,在12天的时间段,观察了在感染HIV-1的PBMC群中的病毒复制和感染细胞的量。将不同量的抗CD134抗体添加至感染培养物中。在2天间隔,定量代表细胞毒性T淋巴细胞的感染的CD4+T细胞、未感染CD4+T细胞的绝对值、CD3+细胞和CD8+细胞的绝对水平。T细胞活化和繁殖通过对CD69和Ki67染色分析。通过CD14受体的表达测量单核细胞的水平。
另外,通过定量释放至上清液中的p24和传染性释放的病毒颗粒的量确定在报道细胞系上的病毒产生。对照包括未感染的PBMC培养物,使用或不使用抗CD134抗体处理,以及不进行任何处理的感染HIV-1的PBMC。将分析至少三个不同的供体。这些实验允许推论出CD134连接离体感染的初级细胞具有清除感染细胞或抑制病毒复制的潜力,并且未感染细胞不被所述处理影响或清除。
抗CD134治疗的功效可以在建立的用于HIV-1感染的人源化小鼠模型中证实。这样的小鼠模型通过“Transcure”公司提供。将这些小鼠使用HIV-1感染并且不进行处理(n=5)或施用三种不同剂量的抗CD134抗体(每个剂量n=5)以连接CD134并且特异性清除表达高水平CD134的HIV-1感染的细胞。观察病毒载量、CD4+T淋巴细胞计数和感染病毒的百分比。总体上,这些实验显示通过针对CD134的抗体(诸如MEDI6469 (9B12))对CD134的连接,具有将HIV-1感染的细胞从生物体中清除的潜力,并且因此代表了新的抗HIV-1免疫疗法。
Claims (16)
1.一种用于预防和/或治疗病毒感染的细胞毒性药物,其被配置为选择性结合至病毒感染的T淋巴细胞的膜受体,其中所述膜受体选自CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29。
2.权利要求1的细胞毒性药物,其偶联至对所述膜受体具有选择性的结合分子。
3.权利要求2的细胞毒性药物,其特征在于所述结合分子选自免疫球蛋白、免疫球蛋白片段、适体、低分子化合物、受体、受体片段、细胞因子。
4.权利要求3的细胞毒性药物,其特征在于所述免疫球蛋白是激动性抗体。
5.权利要求3或4的细胞毒性药物,其特征在于所述免疫球蛋白选自MEDI6469 (9B12)、MEDI6383、MEDI0562、Hu106-222/Hu119-122 (UTMDACC)。
6.前述权利要求任一项的细胞毒性药物,其特征在于所述病毒感染的T淋巴细胞是病毒感染的CD4+T淋巴细胞。
7.前述权利要求任一项的细胞毒性药物,其特征在于所述病毒感染是HIV感染。
8.前述权利要求任一项的细胞毒性药物,其特征在于其包含细胞抑制药物。
9.权利要求8的细胞毒性药物,其特征在于所述细胞抑制药物选自烷化剂、铂类似物、嵌入剂(intercalant)、抗生素、有丝分裂抑制剂、紫杉烷。
10.前述权利要求任一项的细胞毒性药物,其特征在于其包含病毒抑制药物。
11.一种药物组合物,其包含权利要求1-10中任一项的细胞毒性药物和药学可接受的载体。
12.细胞毒性药物用于预防和/或治疗病毒感染的用途,所述细胞毒性药物被配置为选自性结合至病毒感染的T淋巴细胞的膜受体,其中所述膜受体选自CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29。
13.一种用于寻找细胞毒性药物的方法,其包含选自可以选择性结合至病毒感染的T淋巴细胞的膜受体的化合物,其中所述膜受体选自CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29。
14.病毒感染的T淋巴细胞的膜受体用于诊断病毒感染的用途,所述病毒感染的T淋巴细胞的膜受体相比未感染的T淋巴细胞的过表达,其中所述膜受体选自CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29。
15.用于诊断病毒感染的方法,其包括确定病毒感染的T淋巴细胞的膜受体相对未感染的T淋巴细胞的过表达,其中所述膜受体选自CD134、CD132、CD71、CD70、CD54、CD39、BTLA、CD97、CD2、CD63、CD50、CD161、CD218、CD226、CD7、CD49d和CD29。
16.用于确定病毒感染的T淋巴细胞的表面结构的方法,所述病毒感染的T淋巴细胞的表面结构优选为膜受体,其适合作为细胞毒性药物的靶标,所述方法包括使用病毒感染T淋巴细胞,并确定所述病毒感染的T淋巴细胞的表面结构相对未感染的T淋巴细胞的任何过表达。
