本发明属于中药领域,具体涉及一种治疗痛风性关节炎的中药组合物及其制备方法。
背景技术
痛风是嘌呤代谢异常致使尿酸合成增加而导致的代谢性疾病,其病位主要在于关节、脾、肾,与肝、肺有一定关系,临床以痛风性关节炎反复发作最为常见。中医学认为,痛风多因过食肥膏厚腻,其发病与湿热痰浊瘀毒密切相关,属于中医“痹症”范畴,历代医家有“白虎力节”“脚气病”之称。近年来,随着生活水平的提高,饮食结构的调整,高糖、高蛋白、高脂肪、高嘌呤饮食的摄入增加,导致痛风的发病率逐年提升,严重影响了人们的正常工作和生活。
目前西药降尿酸的药物,如秋水仙碱、苯溴马隆等,虽然效果显著,但因伴有一定程度的肝肾脏损害、胃肠道不适及皮疹等不良反应,副作用较大,不利于长期使用,其应用受到限制。而传统中草药不良反应小,安全性好,以中医理论为指导,利用中医药治疗痛风疗效确切,副作用较少,因此越来越受到人们的青睐。
中国专利申请CN104825938A公开了一种治疗痛风的中药组合物,其由当归、川芎、熟地、知母、黄柏、生地、川牛膝、紫花地丁、薏苡仁、苍术、炙鳖甲、炙龟甲、泽泻、防风、赤芍、鸡血藤、蜈蚣、全蝎、萆薢、土茯苓、车前子、忍冬藤、威灵仙、制乳香、穿山甲、羌活、黄芪、甘草共28味药材组成,具有显著的降尿酸作用。
中国专利申请CN103920117A公开了一种治疗痛风的中药组合物,其包括当归、秦艽、桃仁、土鳖虫、地龙、茯苓、土茯苓、萆薢、威灵仙、白术、苍术、黄柏、生薏苡仁、泽兰、虎杖、川牛膝、泽泻、忍冬藤、黄芪、金钱草、赤芍、金银花、车前子、大黄、党参、熟地、海浮石、锁阳、南星、龟板、干姜、徐长卿、半枝莲、五加皮、独活、防风、青皮、丹参、白芍、桑枝、延胡索、路路通、木瓜、槟榔、升麻和甘草共46味药材,对痛风有较好的治疗或缓解症状的作用。
中国专利申请CN101264205A公开了一种用于治疗痛风的中药组合物,其有效成分由黄芪、附子、大黄、制半夏、羌活、车前子、白芍、仙灵脾、萆薢、当归、泽泻、川牛膝、茯苓、细辛、防己、独活、肉桂、炙甘草和川芎共19味药材制成,具有祛风除湿、活血通络、消肿止痛、降血尿酸的作用。
随着中医药防治痛风的临床及基础研究不断深入,涌现了许多具有治疗痛风特别是治疗痛风性关节炎作用的方药,但这些方药大部分药物组成复杂,不利于长期服用,且大多数方药仅通过临床的症状来评价其治疗效果,不清楚其作用原理,也不清楚其毒副作用。
发明内容
本发明的目的在于提供一种治疗痛风性关节炎的中药组合物,以解决上述技术问题中的至少一个。
本发明的另一个目的在于提供上述治疗痛风性关节炎的中药组合物的制备方法,以解决上述技术问题中的至少一个。
根据本发明的一个方面,提供了一种治疗痛风性关节炎的中药组合物,其有效成分主要由以下重量份的原料药制成:土茯苓30~60份,黄芪20~40份,桔梗5~10份,泽泻10~30份,车前子20~40份和淫羊藿15~40份。
优选的,本发明中药组合物的有效成分主要由以下重量份的原料药制成:土茯苓45份,黄芪30份,桔梗8份,泽泻20份,车前子30份和淫羊藿30份。
本发明中药组合物组方中所选用的原料药皆属于2016年卫生部颁布的《药食同源目录表》中“既是食品又是药品的物品名单”或“可用于保健食品的物品名单”中列举的中药材,且其药理作用均显示无明显毒性,可长期安全服用。其中,土茯苓为百合科植物光叶菝葜Smilax glabra Roxb.的干燥根茎,归胃、肝经,解毒,除湿,利关节;黄芪为豆科植物膜荚黄芪Astragalusmembranaceus(Fisch.)Bge.或蒙古黄芪Astragalusmembranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao的干燥根,归脾、肺经,补气固表,托疮生肌;桔梗为桔梗科植物桔梗Platycodongrandiflorum(Jacq.)A.DC.的干燥根,归肺经,开宣肺气,祛痰排脓;泽泻为泽泻科植物泽泻Alisma orientale(Samuels)Juzep.的干燥块茎,归肾、膀胱经,利水渗湿,泄热通淋;车前子为车前科植物车前Plantago asiatica L.或平车前Plantago depressa Willd.的干燥成熟种子,归肾、膀胱经,清热利尿,渗湿通淋,明目,祛痰;淫羊藿为为小檗科植物淫羊藿EpimediumbrevicornumMaxim.、箭叶淫羊藿Epimediumsagittatum(Sieb.etZucc.)Maxim.、柔毛淫羊藿EpimediumpubescensMaxim.、巫山淫羊藿EpimediumwushanenseT.S.Ying、或朝鲜淫羊藿EpimediumkoreanumNakai的干燥地上部分,归肝、肾经,补肾阳,强筋骨,祛风湿。
本发明中药组合物由黄芪、土茯苓、车前子、泽泻、桔梗、淫羊藿等组成,方中以土茯苓、黄芪为君药,健运肺脾之气利水湿;以淫羊藿、泽泻、车前子为臣药,补肾气助气化而泻肾浊;以桔梗为使药,行肺气而调水道,具提壶揭盖之意。本发明中药组合物组成药物少,配伍精当,摆脱了传统治疗痛风性关节炎的中药方剂组成药味多、药物组成复杂的不足,提高了用药的安全性,改善了汤剂的口感,为开发成新产品提供了有利的条件。
本发明中药组合药物组成简单,安全性好,无明显肝肾毒副作用,具有显著的降低血尿酸水平和肾保护作用。本发明中药组合物降低血尿酸水平、改善炎症指标的效果与降尿酸西药苯溴马隆相当,而改善肾小球滤过率的效果优于降尿酸西药苯溴马隆。
在一些实施方式中,本发明中药组合物可以为汤剂、片剂、胶囊剂、散剂、口服液、丸剂、膏剂或颗粒剂。
根据本发明的另一个方法,本发明还提供了上述中药组合物的制备方法,包括如下步骤:按配比取所有原料药,加水浸泡25~45min,水面没过药材1~3cm,浸泡完成后,武火煮沸,然后文火煎煮20~45min,过滤,得药液,即得中药组合物汤剂。
武火、文火为中药学名词,武火是指煎药时火力大而急,温度上升快,水分蒸发快,水泡大且凸起快;文火是指煎药时火力小而缓,温度变化不大,水分蒸发慢,药液呈微沸状态,药液中药物对流缓慢、不明显。
在一些实施方式中,上述制备方法还可以包括如下步骤:将药液浓缩成含水量为15~20%的稠浸膏或制成含水量为3~8%的干浸膏,然后加入药学上可接受的载体制成片剂、胶囊剂、散剂、口服液、丸剂、膏剂或颗粒剂等药物制剂。由此,可方便患者携带和服用。

下面结合具体实施例对本发明作进一步详细的说明。
下述列举了本发明实施例1~4的组分、比例和制备方法,按下述配比称取原料药(单位:重量份):
实施例1
处方:土茯苓45份,黄芪30份,桔梗8份,泽泻20份,车前子30份和淫羊藿30份。
制备方法:按配比取所有原料药,加水浸泡30min,浸泡的水量以水面没过药材1~2cm为宜,浸泡完成后,武火煮沸,然后文火煎煮25min,过滤,得药液,即得中药组合物。
实施例2
处方:土茯苓30份,黄芪20份,桔梗5份,泽泻10份,车前子20份和淫羊藿15份。
制备方法:按配比取所有原料药,加水浸泡45min,浸泡的水量以水面没过药材1~2cm为宜,浸泡完成后,武火煮沸,然后文火煎煮30min,过滤,得药液,药液浓缩成含水量为15~20%的稠浸膏,加入药学上可接受的载体制成片剂。
实施例3
处方:土茯苓55份,黄芪40份,桔梗10份,泽泻25份,车前子30份和淫羊藿35份。
制备方法:按配比取所有原料药,加水浸泡30min,浸泡的水量以水面没过药材1~2cm为宜,浸泡完成后,武火煮沸,然后文火煎煮25min,过滤,得药液,药液浓缩成含水量为15~20%的稠浸膏,加入药学上可接受的载体制成胶囊剂。
实施例4
处方:土茯苓40份,黄芪35份,桔梗8份,泽泻30份,车前子35份和淫羊藿20份。
制备方法:按配比取所有原料药,加水浸泡30min,浸泡的水量以水面没过药材1~2cm为宜,浸泡完成后,武火煮沸,然后文火煎煮25min,过滤,得药液,药液浓缩成含水量为15~20%的稠浸膏,加入药学上可接受的载体制成颗粒剂。
为了证明本发明中药组合物的效果,进行如下药效学试验:
一、本发明中药组合物治疗痛风性关节炎患者的临床试验
1、试验对象:就诊于广东省中医院风湿科门诊的痛风性关节炎患者。
2、诊断标准:西医诊断标准参考2015年ACR/EULAR发表的痛风诊断标准;中医证候诊断参照中华人民共和国中医药行业标准《中医病证诊断疗效标准》-浊瘀内阻证。
3、纳入标准:(1)符合上述诊断标准的痛风性关节炎患者;(2)年龄18~65岁;(3)近3个月内无使用抗生素治疗(4)自愿参加试验研究,并由受试者或其家属(监护人)签署知情同意书。
4、排除标准:符合纳入标准的潜在受试者如果满足以下任何一项均将被排除:(1)不符合上述西医诊断标准的患者;(2)至少1个月内未停用糖皮质激素的患者;(3)未获得知情同意书的患者;(4)患者本人是直接参与本试验工作的研究者;(5)有可能无法完成本研究全过程的患者;(6)不适宜用此临床试验方法进行治疗的患者;(7)合并有胃肠道疾病的患者,如胃肠道手术、克罗恩病、溃疡性结肠炎、急性腹泻等;(8)合并有心血管、肺部、肝脏、肾脏、造血系统等严重疾病的患者;(9)有药物滥用史;(10)年龄在18岁以下,65岁以上的患者;(11)孕妇或哺乳期妇女的患者;(12)精神病患者;(13)研究者认为不宜进行此项临床试验者。
5、治疗方案:将80例痛风性关节炎患者随机平均分为中药组与西药组,两组患者的年龄、病史、发作次数、体重指数、血尿酸水平、肾小球滤过率等指标经统计学分析,无显著差异,具有可比性,具体数据见表1。
用药周期为12周;其中:
中药组:本申请实施例1的中药组合物,服用方法:每日一剂,分早、晚2次温服。
西药组:苯溴马隆,服用方法:每日1次,每次一片,口服。

6、试验结果:见表2~表4。
6.1中药组患者治疗前后指标改善情况

由表2结果可知:
(1)治疗12周后,中药组患者的血尿酸水平与治疗前相比差异具有统计学意义(P<0.05),说明本申请中药组合物可显著降低血尿酸水平;
(2)治疗12周后,患者的肾小球滤过率与治疗前相比差异具有统计学意义(P<0.05),说明本申请中药组合物可显著提升肾小球滤过率,具有显著的肾保护作用;
(3)治疗12周后,患者的C反应蛋白、血沉水平与治疗前相比差异具有统计学意义(P<0.05),本申请中药组合物可显著降低炎症指标水平(C反应蛋白、血沉);
(4)治疗前后,中药组患者的谷丙转氨酶和谷草转氨酶无显著变化,说明本发明中药组合物无明显肝肾毒副作用。
6.2西药组患者治疗前后指标改善情况

由表3结果可知,治疗12周后,西药组患者的血尿酸水平、C反应蛋白水平和血沉水平与治疗前相比差异具有统计学意义(P<0.05),说明苯溴马隆可显著降低血尿酸水平和降低炎症指标水平,但是,治疗12周后,西药组患者的肾小球滤过率有降低的趋势、谷丙转氨酶水平显著升高且谷草转氨酶水平有升高的趋势,说明苯溴马隆可能具有肝肾毒副作用。

两组治疗前后各类指标变化趋势图见图1~8。
由表4和图1~8的结果可知,两组对各项指标的改善无明显差异(P>0.05),说明本申请中药组合降低血尿酸,改善炎症指标的效果与降尿酸药物(苯溴马隆片)效果相当。
7、试验结论:本试验进行了严谨的临床对照试验设计,将本发明中药组合物与疗效确切的临床一线降尿酸药苯溴马隆片进行对比,通过客观的血尿酸、肾小球滤过率等实验室指标的改善来评价本发明中药组合物的疗效,并经过统计学分析,得出的本发明中药组合物的临床疗效客观真实。试验结果表明,本发明中药组合物无明显的肝肾毒副作用,具有显著的降低血尿酸水平和改善炎症指标的作用,降尿酸效果确切、稳定,效果与降尿酸药物(苯溴马隆片)相当。本申请中药组合物在改善肾小球滤过率方面,效果优于降尿酸药物(苯溴马隆片),说明本发明中药组合物相对于降尿酸的西药(苯溴马隆片)具有改善肾功能作用。
以上所述的仅是本发明的一些实施方式。对于本领域的普通技术人员来说,在不脱离本发明创造构思的前提下,还可以做出若干变形和改进,这些都属于本发明的保护范围。
Claims (5)
1.治疗痛风性关节炎的中药组合物,其特征在于,其有效成分主要由以下重量份的原料药制成:土茯苓30~60份,黄芪20~40份,桔梗5~10份,泽泻10~30份,车前子20~40份和淫羊藿15~40份。
2.根据权利要求1所述的治疗痛风性关节炎的中药组合物,其特征在于,其有效成分主要由以下重量份的原料药制成:土茯苓45份,黄芪30份,桔梗8份,泽泻20份,车前子30份和淫羊藿30份。
3.根据权利要求1或2所述治疗痛风性关节炎的中药组合物,其特征在于,所述中药组合物为汤剂、片剂、胶囊剂、散剂、口服液、丸剂、膏剂或颗粒剂。
4.根据权利要求1或2所述的治疗痛风性关节炎的中药组合物的制备方法,其特征在于,包括如下步骤:按配比取所有原料药,加水浸泡25~45min,水面没过药材1~3cm,浸泡完成后,武火煮沸,然后文火煎煮20~45min,过滤,得药液,即得中药组合物。
5.根据权利要求4所述的制备方法,其特征在于,还包括如下步骤:将所述药液浓缩成含水量为15~20%的稠浸膏或制成含水量为3~8%的干浸膏,然后加入药学上可接受的载体制成药物制剂,即得中药组合物。
