Abstract
本发明公开了一种治疗偏头痛的中药组合物,它是由柴胡、延胡索、川芎、桃仁、红花、当归、白芍、熟地黄、香附、丹参、桂枝、天麻、栀子、黄芩、石菖蒲、吴茱萸、藁本、郁金、白芷、川牛膝、菊花、炙甘草按照一定的重量配比组成,具有祛风止痛、活血通经、散瘀定痛的功效,能够有效治疗偏头痛。

[0001] 本发明涉及一种中药组合物,具体涉及一种治疗偏头痛的中药组合物。本发明还 涉及该中药组合物的制备方法。
背景技术
[0002] 偏头痛是一种常见的慢性神经血管性疾患,多起病于儿童和青春期,中青年期达 发病高峰,女性多见,男女患者比例约为1 : 2〜3,人群中患病率为5%〜10%,常有遗传背 景。偏头痛是一类有家族发病倾向的周期性发作疾病。表现为发作性的偏侧搏动性头痛, 伴恶心、呕吐及羞明,经一段歇期后再次发病。在安静、黑暗环境内或睡眠后头痛缓解。在 头痛发生前或发作时可伴有神经、精神功能障碍。偏头痛发作的主要因素是,由于发作性血 管舒缩功能不稳定以及某些体液物质暂时性改变所致的一种有或不伴有脑及植物神经系 统功能暂时性障碍的头痛,分型较多,但在实际临床中,很少仅有单一类型的偏头痛存在, 常常表现为几个类型的偏头痛,甚至和其他类型头痛如紧张型头痛等同时存在。
[0003] 中医上讲,"头为诸阳之会"、"清阳之府",脏腑气血皆上会于头,故不论外感、内伤 的疾病都可通过经络、气血直接或间接地影响头部而发生头痛,所以头痛可以出现多种急 慢性疾病。
[0004] 目前偏头痛的病因尚不清楚,治疗也主要是以西药为主,这类药物在一定程度上 虽然缓解病症,但是治标不治本,通常需要长期服药,一旦停药,头痛症状又会发作,倘若用 药不当,还可能治疗无效甚至加重病情,病情西药毒副作用大,尤其是对肝肾功能的影响最 大,病情的反复,药物的副作用都给患者带来很大的痛苦。
发明内容
[0005] 本发明的目的是提供一种中药组合物,该中药组合物对于治疗偏头痛具有见效 快,疗效显著,价格低廉,无毒副作用,不易复发的特点。
[0006] 本发明的另一目的是提供该治疗偏头痛的中药制剂的制备方法。
[0007] 发明人在多年的临床医疗实践中,观察到偏头痛给患者带来的痛苦,根据该病症 的发展规律,以及中药的配伍原则,并通过大量的临床验证,反复探索总结出一种治疗偏头 痛的中药制剂的新配方,本发明选择柴胡、延胡索、川芎、桃仁、红花、当归、白芍、熟地黄、香 附、丹参、桂枝、天麻、栀子、黄苳、石菖蒲、吴茱萸、藁本、郁金、白ϊ£、川牛膝、菊花、炙甘草进 行组合,将这些药物组合使得各药物功效产生协同作用,使之具有祛风止痛、活血通经、散 瘀定痛的功效,从而能够有效治疗偏头痛。
[0008] 本发明中药物的作用: 其中: 柴胡:为伞形科多年生草本植物柴胡Bupleurum chinense DC.(北柴胡)或狭叶柴胡 Bupleurum scorzonerifolium Willd.(南柴胡)的根或全草。性味:苦、辛、微寒。归肝、胆 经。功效:和解退热、疏肝解郁、升阳举陷。柴胡具有明显的镇静、镇痛的作用,有解除胸闷 胀痛,开郁调经作用,并且有广泛的抗菌和抗病毒的作用。
[0009] 延胡索:为罂粟科植物延胡索Corydalis yanhusuo W. T. Wang的干燥块莖。性 味:辛、苦、温。归肝、脾经。功效:活血,利气,止痛。用于胸胁、脘腹疼痛,经闭痛经,产后瘀 阻,跌扑肿痛。本品辛散温通,能行血中气滞,气中血滞,活血行气止痛,善治血瘀气滞所致 的多种疼痛,其止痛作用优良。《本草求真》中记载:延胡索,不论是血是气,积而不散者,月艮 此力能通达,以其性温,则于气血能行能畅,味辛则于气血能润能散,所以理一身上下诸痛, 往往独行功多。
[0010] 川考:为伞形科植物川考Ligusticum chuanxiong Hort.的干燥根莖。性味:辛, 温。归肝、胆、心经。功效:活血行气,祛风止痛。本品辛散温通,能上行巅顶,下达血海,外彻 皮毛,旁通四肢,既能活血又能行气,昔人谓川芎为血中之气药,殆言其寓辛散、解郁、通达、 止痛等功能。川芎有明显的镇静作用。抗菌作用等体外试验:川芎对大肠、痢疾(宋内氏)、 变形、绿脓、伤寒、副伤寒杆菌及霍乱弧菌等有抑制作用。川芎水浸剂(1:3)在试管内对某 些致病性皮肤真菌也有抑制作用。
[0011] 桃仁:为蔷薇科植物桃Prunus persica (L. ) Batsch 或山桃 Prunus davidiana (Carr. )Franch.的干燥成熟种子。性味:苦、甘,平。归心、肝、大肠经。功效:活血祛瘀,润 肠通便,止咳平喘。桃仁祛瘀之力较强,又称破血药,用于治疗血瘀、癥瘕,常配红花、当归。 桃仁对血流阻滞、血行障碍有改善作用,能使各脏器各组织机能恢复正常。
[0012] 红花:为菊科植物红花Carthamus tinctorius L.的筒状花冠。性味:温,辛。归 心、肝经。功效:活血通经、散瘀止痛。红花辛散温通,专入血分,能活血祛瘀,通经调脉,常 与桃仁、当归、川芎等相须为用。红花有扩张血管作用。
[0013] 当归:为伞形科植物当归Angelica sinensis (Oliv.) Diels的干燥根。性味甘、 辛、苦,温。归肝、心、脾经。功效:补血;活血;调经止痛;润燥滑肠。当归对中枢神经系统 有抑制作用,挥发油有镇静、催眠、镇痛、麻醉等作用。用于血虚萎黄、眩晕心悸、虚寒腹痛、 风湿痹痛、跌扑损伤、痈疽疮疡。据《本草正》载:其味甘而重,故专能补血,其气轻而辛,故 又能行血,能养营养血,补气生精,安五脏,强形体,益神志,凡有形虚损之病,无所不宜。
[0014] 白芍:为双子叶植物毛茛科植物芍药Paeonia lactiflora Pali.的干燥根。性 味:凉,苦酸,微寒。功效:补血柔肝、平肝止痛,敛阴收汗。研宄表明:白芍中含有的芍药甙 有抗菌、解热、抗炎、增加冠状动脉流量、改善心肌营养血流、扩张血管、对抗急性心肌缺血、 抑制血小板聚集、镇静、镇痛、解痉、抗溃疡、调节血糖的作用。白芍煎剂能抑制痢疾杆菌、肺 炎链球菌、大肠杆菌、伤寒杆菌、溶血性链球菌、绿脓杆菌等。本品能养肝血、敛肝阴、抑肝 阳、疏肝气,又能缓挛急、止疼痛,为治疗各种疼痛之良药。
[0015] 熟地黄:为玄参科植物地黄(Rehmannia glutinosa(Gdertn) Iibosch. ex Fisch. et Mey.)的块根经加工炮制而成。性味:甘、微温。归肝、肾经。功效:滋阴补血,益精填 髓。用于肝肾阴虚,腰膝酸软,骨蒸潮热,盗汗遗精,内热消渴,血虚萎黄,心悸怔忡,月经不 调,崩漏下血,眩晕,耳鸣,须发早白。
[0016] 香附:为莎草科植物莎草Cyperus rotundus L.的干燥根莖。性味:辛、微苦、微 甘,平。归肝、脾、三焦经。功效:行气解郁,调经止痛,安胎,理气舒郁,调和肝胃。本品辛散 苦泄,为疏肝理气之要药,可用于郁滞所致诸痛证。
[0017] 丹参:为双子叶植物唇形科Labiatae鼠尾草属植物丹参Salvia / miltiorrhizaBge的干燥根及根莖。性味:苦,微寒。归经:入心、肝经。功效:活血调经, 祛瘀止痛,凉血消痈,清心除烦,养血安神。用于胸肋胁痛,风湿痹痛,症瘕结块。《日华子本 草》:养神定志,通利关脉。治冷热劳,骨节疼痛,四肢不遂;排脓止痛,生肌长肉;破宿血,补 新生血;安生胎,落死胎;止血崩带下,调妇人经脉不匀,血邪心烦;恶疮疥癣,瘿赘肿毒,丹 毒;头痛,赤眼,热温狂闷。
[0018] 桂枝:为樟科植物肉桂Cinnamomum cassia Presl的干燥嫩枝。性味:辛、甘,温。 归心、肺、膀胱经。功效:发汗解肌,温通经脉,助阳化气,平冲降气。桂枝性温,善通阳气,能 化阴寒,对阴寒遏阻阳气,津液不能输布,因而水湿停滞形成痰饮的病症。
[0019] 天麻:为兰科植物天麻Gastrodia elata Blume.的干燥块莖。性味:甘,平。归 肝经。功效:平肝息风止痉。用于头痛眩晕,肢体麻木,小儿惊风,癫痫抽搐,破伤风。药理 学研宄表明,天麻具有明显的镇静和抗惊厥作用。
[0020] 栀子:为茜草科植物栀子Gardenia jasminoides Ellis的干燥成熟果实。性味: 苦,寒。归心、肺、三焦经。功效:泻火除烦,清热利尿,凉血解毒。用于热病心烦,黄疸尿赤, 血淋涩痛,血热吐衄,目赤肿痛,火毒疮疡;外治扭挫伤痛。山栀又有凉血止血、清热解毒的 作用,用治血热妄行,用于热病发热,心烦不宁等症。
[0021] 黄苳:为唇形科植物黄苳Scutellaria baicalensis Georgi的干燥根。性味: 苦,寒。归肺、胆、脾、胃、大肠、小肠经。功效:清热燥湿,泻火解毒,止血,安胎,降血压。黄 芩的临床应用抗菌比较好,而且不产生抗药性。本品有清热凉血止血之功,用治火热炽盛, 迫血妄行。
[0022] 石菖蒲:为天南星科植物石菖蒲Acorus tatarinowii Schott的干燥根莖。性味 辛、苦,温。归心、胃经。功效:化湿开胃,开窍豁痰,醒神益智。石菖蒲有抗惊厥作用,石菖 蒲的挥发油有安神镇静作用,石菖蒲煎剂内服能促进消化液的分泌。
[0023] 吴茱萸:为芸香科植物吴茱萸 Evodia rutaecarpa (Juss. ) Benth.、石虎 Evodia rutaecarpa (Juss. ) Benth. var. officinalis (Dode) Huang 或疏毛吴茱萸 Evodia rutaecarpa (Juss. )Benth. var. bodinieri (Dode)Huang 的干燥近成熟果实。性味:辛、 苦,热;有小毒。归肝、脾、胃、肾经。功效:散寒止痛,降逆止呕,助阳止泻。用于厥阴头痛, 寒疝腹痛,寒湿脚气,经行腹痛,脘腹胀痛,呕吐吞酸,五更泄泻,外治口疮;高血压。
[0024] 藁本:为伞形科藁本属植物藁本Ligusticum sinense Oliv.或辽藁本Ligusticum jeholense Nakai et Kitag.的干燥根莖及根。性味:辛,温。归膀胱经。功效:祛风,散 寒,除湿,止痛。用于风寒感冒,巅顶疼痛,风湿肢节痹痛。本品辛温辛散,善达头之巅顶,有 止痛作用,故适用于感冒风寒引起的头痛、巅顶头痛、偏头痛,常与川芎、白芷等配伍应用。
[0025] 郁金:为姜科植物温郁金 Curcuma wenyujin Y. H. Chen et C. Ling、姜黄 Curcuma longa L.、广西莪术 Curcuma kwangsiensis S. G. Lee et C. F. Liang 或蓬莪 术Curcuma phaeocaulis Val.的干燥块根。性味:辛、苦,寒。归肝、心、肺经。功效:行气 化瘀,清心解郁,利胆退黄,凉血破瘀。主治:胸腹胁肋诸痛,失心癫狂,热病神昏,吐血,衄 血,尿血,血淋,妇女倒经,黄疸。本品辛散苦泄,性寒清热,能清心解郁开窍。
[0026] 白]E :为伞形科植物白]E Angelica dahurica (Fisch. ex Hoffm. ) Benth. et Hook. f.或杭白]E Angelica dahurica (Fisch. ex Hoffm. ) Benth. et Hook. f. var. formosana (Boiss.) Shan et Yuan的干燥根。性味:辛、温。归肺、胃经。功效:祛风散 寒,通窍止痛,消肿排脓,燥湿止带。用于头痛、牙痛。水煎剂对大肠杆菌、采氏痢疾杆菌、伤 寒杆菌、副伤寒杆菌、绿脓杆菌、变形杆菌、霍乱弧菌、人体结核杆菌等有抑制作用。《本草求 真》:白芷,气温力厚,通窍行表,为足阳明经祛风散湿主药。故能治阳明一切头面诸疾,如头 目昏痛,眉棱骨痛,暨牙龈骨痛,面黑瘢疵者是也。且其风热乘肺,上烁于脑,渗为渊涕;移于 大肠,变为血崩血闭,肠风痔瘘痈疽;风与湿热,发于皮肤,变为疮疡燥痒;皆能温散解托, 而使腠理之风悉去,留结之痈肿潜消,诚祛风上达,散湿火要剂也。
[0027] 川牛膝:为苋科植物川牛膝Cyathula officinalis Kuan的干燥根。性味:甘、微 苦、平。归肝、肾经。功效:逐瘀通经,通利关节,利尿通淋。用于经闭症瘕,胞衣不下,关节 痹痛,足瘘筋挛,尿血血淋,跌扑损伤。
[0028] 菊花:为菊科植物菊Chrysanthemum morifolium Ramat.的干燥头状花序。性味: 甘、苦,微寒。归肺、肝经。功效:散风清热,平肝明目。用于风热感冒,头痛眩晕,目赤肿痛, 眼目昏花。菊花疏风较弱,清热力佳,用于外感风热常配桑叶同用,也可配黄芩、山栀治热盛 烦燥等症。
[0029] 炙甘草:为豆科植物甘草Glycyrrhiza uralensis Fisch.,胀果甘草 Glycyrrhiza Inflata Bat.或光果甘草Glycyrrhiza Glabra L.的干燥根及根莖。炙甘草 为甘草的蜜烘制加工品。性味甘,平。归心、肺、脾、胃经。功效:补脾和胃,益气复脉。《日 华子本草》:安魂定魄。补五劳七伤,一切虚损、惊悸、烦闷、健忘。通九窍,利百脉,益精养 气,壮筋骨,解冷热。
[0030] 本发明的方解是:方中柴胡疏肝解郁、和解退热,延胡索活血止痛,川芎活血行气、 祛风止痛,桃仁、红花活血通经、散瘀止痛,以上五味共为君药,对于头部血液进行疏通,疏 风散热、活血止痛,从根本上促进患处血液循环,缓解头痛症状。当归补血活血、调经止痛, 白芍补血柔肝、平肝止痛,熟地黄滋阴补血、益精填髓,香附行气解郁、调经止痛,丹参、桂枝 养血安神、温通经脉,以上六味共为臣药,中医上讲通则不痛、痛则不通,臣药主要辅佐君药 活血通经,帮助患者减轻痛苦,并且臣药具有滋阴补血、益精填髓的作用,对患者气血进行 补养,促进患者的恢复。天麻息风止痉,栀子、黄芩泻火除烦、清热凉血,石菖蒲醒神益智,吴 茱萸、蒿本散寒止痛,郁金行气化瘀、清心解郁,白芷祛风散寒、通窍止痛,川牛膝逐瘀通经, 菊花清风散热,以上几味共为佐药,加强药物疗效,同时具有清心解郁、醒神益智的功效,使 患者心情舒畅,有利于头痛的消失。炙甘草引药归经、调和诸药,为使药。此方诸药合用,君 臣协力,佐使共辅,具有祛风止痛、活血通经、散瘀定痛的功效,能够有效治疗偏头痛。
[0031] 本发明药物原料用量是经发明人进行大量摸索总结得出的,各原料用量为在下述 重量份范围都具有较好的疗效:柴胡7-17份、延胡索13-23份、川芎7-17份、桃仁7-17份、 红花4-14份、当归10-20份、白芍10-20份、熟地黄10-20份、香附7-17份、丹参13-23份、 桂枝7-17份、天麻10-20份、栀子5-15份、黄芩7-17份、石菖蒲13-23份、吴茱萸3-9份、 藁本7-17份、郁金7-17份、白芷4-14份、川牛膝10-20份、菊花4-14份、炙甘草3-9份。

[0033] 本发明药物可以采用中药制剂的常规方法制备成任何常规内服制剂,如丸剂、散 剂、片剂、口服液等。例如可以将这些原料药研成粉末混合均匀制成散剂冲服;可以用水煎, 经过滤得滤液,加入防腐剂制成口服液;必要时也可以直接服用汤剂。
[0034] 本发明优选的制备方法是颗粒剂,其制备方法如下: 步骤一:按本发明重量配比,先将栀子、吴茱萸用纱布包成药包,放于水中浸泡约45分 钟,过滤,备用; 步骤二:按本发明重量配比,将桃仁、熟地黄研磨粉碎至180目筛,备用; 步骤三:按本发明重量配比,将川芎、当归、白芍、香附、丹参、黄苳、石菖蒲、蒿本、郁金、 白芷、川牛膝放入砂锅中加步骤一中浸泡后剩余冷水,超过药面3〜6厘米,用武火煎沸后, 再用文火煎20〜30分钟,过滤,得滤液,在剩余药渣中加入4厘米左右的水,然后加入柴 胡、延胡索、天麻以及步骤一中的药包煎煮约30分钟,之后加入红花、桂枝、菊花、炙甘草煎 煮15分钟过滤,得滤液,合并两次滤液,过滤,弃渣,合并两次滤液,并将滤液浓缩至80°C 左右时,加入75%乙醇,放置沉淀,过滤,弃沉淀,滤液回收乙醇,在60 °C环境下相对密度 1. 13〜1. 20的稠膏,备用; 步骤四:步骤二所得细粉加入步骤三所得稠膏中搅拌混合均匀,干燥,粉碎成细粉,加 入适量糖粉作为稀释剂,乙醇作为润湿剂以及微晶纤维素作为崩解剂,采用干法制粒,制成 IOg/袋,每袋含原料药5g的颗粒剂。
[0035] 本发明药物祛风止痛、活血通经、散瘀定痛等功效,且见效快,疗效显著,价格低 廉,无毒副作用,不易复发,该中药制剂采用的方式治疗可以减少西药对患者肝肾功能的损 伤,加快病情的好转,据对临床124例患者观察统计,使用本发明药物一个疗程总有效率 90. 3%,两个疗程总有效率97. 6%。

[0039] 下面是患者使用本发明药物的临床统计资料: 一、一般资料 收治门诊患者248例。将患者随机分为观察组和对照组,其中男性118例,女性130例, 男女比例I :1. 1,最大年龄79岁,最小年龄18岁,病程时间不等,两组患者年龄、性别、病程 等各项基本资料基本相同,差异无统计学意义(P > 〇. 05)。

通过对比两组患者的治疗总有效率可以发现观察组明显高于对照组,由此说明,本发 明中药在治疗偏头痛具有良好的临床效果,能有效提高治疗的治愈率,值得在临床应用上 推广。
[0046] 典型病例及疗效: 病例一、杨某,女,34岁,患有头痛症状约有一年左右,左侧太阳穴疼痛,有时伴有恶心 的症状,疼痛有时可持续几个小时,曾多次服用止痛药,虽然当时能止住疼痛,但是没过几 天又会复发,严重影响到睡眠和工作,后经人介绍,来我处就诊,采用本发明实施例二中所 制颗粒剂,患者服用两个疗程后,头痛发作次数明显减少,继续服用一个疗程,患者偏头痛 现象完全消失,晚上睡眠治疗也得到提高,同时工作效率也加强了,随访半年未见复发,痊 愈。
[0047] 病例二、王某,男,18岁,近期两侧太阳穴处疼痛,跳痛,经常多梦,睡眠质量严重下 降,记忆力跟不上,眼看就要高考了,家长和患者都很着急,后来我处就诊,采用本发明实施 例二中所制颗粒剂,患者服用两个疗程后,感觉症状明显减轻,睡眠质量得到好转,记忆力 也有所提升,为巩固疗效,继续服用两个疗程,头痛症状完全消失,可以正常学习和生活,并 未影响尚考,半年后电话回访,并未复发,疫愈。
[0048] 病例三、陈某,男,69岁,偏头痛多年,用过多种药物,效果一直不好,无法根治,停 药后疼痛有时加重,后来我处就诊,采用本发明实施例二中所制颗粒剂,服用三个疗程后, 症状感觉明显减轻,头痛发作次数减少,头痛感减轻,后继续服用一个疗程,头痛症状完全 消失,其他临床症状也治愈,并无其他不适症状,随访半年,未见复发,痊愈。
Claims (4)
1. 一种治疗偏头痛的中药组合物,其特征在于,该中药组合物是由以下重量份的药物 组成:柴胡7-17份、延胡索13-23份、川芎7-17份、桃仁7-17份、红花4-14份、当归10-20 份、白芍10-20份、熟地黄10-20份、香附7-17份、丹参13-23份、桂枝7-17份、天麻10-20 份、栀子5-15份、黄芩7-17份、石菖蒲13-23份、吴茱萸3-9份、藁本7-17份、郁金7-17份、 白芷4-14份、川牛膝10-20份、菊花4-14份、炙甘草3-9份。
2. 根据权利要求书1所述的治疗偏头痛的中药组合物,其特征在于,由以下重量份的 药物组成:柴胡12份、延胡索18份、川考12份、桃仁12份、红花9份、当归15份、白芍15 份、熟地黄15份、香附12份、丹参18份、桂枝12份、天麻15份、栀子10份、黄苳12份、石 菖蒲18份、吴茱萸6份、藁本12份、郁金12份、白芷9份、川牛膝15份、菊花9份、炙甘草 6份。
3. 根据权利要求1或2所述的治疗偏头痛的中药制剂,其特征在于:所述中药制剂为 颗粒剂。
