背景技术
开放性骨折的治疗需要彻底的冲洗和清创以预防感染并促进愈合。在冲洗液中添加表面活性剂、杀菌剂等可提高冲洗液去除细菌的能力,然而,有研究表明,与正常生理盐水相比,添加上述物质后,其去除细菌的能力虽然得到提升,但增加了冲洗液的生物毒性。因此,最近的研究推荐使用不添加任何物质的生理盐水进行冲洗,理由是这些物质增加了冲洗液的毒性反而影响了治疗效果。总体来说,在开放性骨折的治疗中,生理盐水仍是大多数外科医生首选的冲洗液,但生理盐水的除菌能力却不足。
发明内容
本申请提供了一种既可增加去除细菌能力又无毒的伤口冲洗液。
根据本申请的一个方面,本申请提供一种伤口冲洗液,其包含生理盐水和EDTA。
本申请的伤口冲洗液是将乙二胺四乙酸(EDTA)溶解在生理盐水(NS)中,获得所需EDTA浓度的EDTA‐NS溶液,然后调节溶液的pH至7.4,灭菌该溶液从而获得伤口冲洗液。
根据本申请的一些实施方式,本申请的伤口冲洗液中EDTA的浓度为0.5‐100mmol/L,例如为0.5‐20mmol/L,0.5‐10mmol/L,1‐10mmol/L,0.5‐5mmol/L,1‐5mmol/L,更具体为1mmol/L。
根据本申请的一些实施方式,本申请的伤口冲洗液是通过将EDTA溶解在0.9%生理盐水后,获得所需EDTA浓度的EDTA‐NS溶液,然后灭菌该溶液而获得。
根据本申请的另一个方面,本申请涉及EDTA在制备伤口冲洗液中的应用。

下述实施例仅用于举例说明本申请的伤口冲洗液的制备方法以及生物活性,这些实施例无意于对本申请保护范围的限制。
制备实施例
在0.9%生理盐水中添加EDTA,完全溶解后,得到EDTA浓度为1mmo/L的溶液,调节pH值至7.4,进行严格灭菌处理,得到EDTA‐NS的伤口冲洗液。
疗效和毒性试验
材料和方法
EDTA-NS溶液是指EDTA溶解于生理盐水中得到EDTA浓度为1mmol/L的EDTA-NS溶液(pH调至7.4)。生理盐水和肥皂液作为对照组。EDTA-NS溶液和对照组溶液都经高压灭菌处理。
本项研究包括体外毒性实验以及多种动物模型来评估EDTA-NS的疗效。体外毒性实验采用细胞活性试验(膜联蛋白V/PI染色)来比较EDTA-NS和生理盐水。然后我们用大鼠建立了不同伤口(伴或不伴骨外露)的动物模型,并用金黄色葡萄球菌或大肠杆菌污染或感染伤口,进而研究EDTA-NS冲洗的疗效。根据Helsinki宣言,本项研究中的人成纤维细胞和内皮细胞得到了上海交通大学附属第六人民医院伦理委员会的批准,所有捐赠人均填写了知情同意书。所有动物实验均根据我们医院的动物管理使用委员会批准的方案进行的。
细胞活性分析
成纤维细胞和内皮细胞广泛用于体外实验检测冲洗溶液的毒性,因为肉芽组织主要由这两种细胞构成[5,13,20,27]。在本研究中,我们用人脐静脉内皮细胞和成纤维细胞来测试和比较EDTA-NS与生理盐水的毒性。两种细胞系均在富有10%胎牛血清(Gibco,Dublin,Ireland)的α-MEM(Corning,New York,NY,USA)培养基中培养。培养基每2天用新鲜培养基予以替换;当培养基中细胞达到完全汇合,将细胞按1:3进行传代培养。用第5代的细胞进行细胞活性试验,用生理盐水或EDTA-NS刺激细胞15分钟后除去冲洗液并添加新鲜培养基。6小时后,采用膜联蛋白V/(PI)细胞凋亡分析试剂盒(Cell Signaling Technology,Danvers,MA,USA)进行细胞活性分析。采用Guava easyCyte Flow Cytometers(MerckKGaA,Darmstadt,Germany)对细胞进行计数。活细胞对于膜联蛋白V和PI均为阴性;晚期凋亡细胞对于这两者均为阳性,而早期凋亡细胞仅对膜联蛋白V为阳性。活细胞、早期凋亡细胞和晚期凋亡细胞分别位于流式细胞图的左下象限、右下象限和右上象限中,通过流式细胞仪提供的软件自动计算活细胞率。该实验重复独立进行三次后对平均活细胞率进行比较和统计学分析。
动物模型
根据不同的临床情况我们设计了四种动物模型(第I组:不伴骨外露的污染伤口;第II组:不伴骨外露的感染伤口;第III组:伴骨外露的污染伤口;第IV组:伴骨外露的感染伤口)来比较EDTA-NS溶液与生理盐水和肥皂液的疗效。伴骨外露的伤口明显不同于软组织的伤口。因此,我们在本项研究中建立了两种伤口类型(伴骨外露或不伴骨外露),然后感染或污染这些伤口。代表革兰氏阳性菌的金黄色葡萄球菌(ATCC 29213)及代表革兰氏阴性菌的大肠杆菌(ATCC 25922)用作致病菌。模型建立之后,按1:1:1的比例对大鼠进行随机化分配到三个治疗组中:生理盐水、肥皂液和EDTA-NS(图1)。
细菌培养液制备
将大肠杆菌和金黄色葡萄球菌在具有5%羊血的胰蛋白酶大豆琼脂(BDTMTrypticaseTM,BD,Heidelberg,Germany)培养基上培养,并在手术前24小时进行新鲜培养。通过棉签收集菌落并用生理盐水洗涤细胞三次,根据光密度的标准曲线将细胞浓度调为1×108菌落形成单位(CFU)/mL来制备接种物。每只大鼠将接受体积100μL的1×107CFU的细菌接种物。
冲洗液制备
通过EDTA溶解于生理盐水来制备EDTA浓度为1mmol/L的EDTA-NS溶液,并将pH值调至为7.4。用0.45%的肥皂在生理盐水中制备肥皂溶液。所有冲洗溶液在使用前均进行高压灭菌。
冲洗和清创步骤
大鼠腹腔注射400mg/kg水合氯醛进行麻醉,待麻醉后不同组别分别使用不同冲洗液对创面进行彻底冲洗和清创,大鼠冲洗量采用标准化体积(300mL)。清创的原则是建立活性组织和有灌注组织的边界。之后,三组中所有大鼠均采用100mL生理盐水进行额外冲洗以去除残留的添加剂或者被视作为对照。
污染模型组
为了建立不伴骨外露的污染伤口模型(第I组),在大鼠的背部皮肤打开2-cm的切口并接种大肠杆菌或金黄色葡萄球菌。在建立伴骨外露的污染伤口模型(第III组),我们采用了在对既往研究的改良方案[19]。简单地说,通过外科手术方式打开1-cm的切口使大鼠背部棘突外露,再用针刺穿骨头后接种大肠杆菌或金黄色葡萄球菌。6小时后,分别对两组进行冲洗和清创,然后用棉签获取细菌培养样品,并缝合伤口。48小时后,再次打开伤口寻找脓液(通过涂片检查和Gram染色证实),化脓是本研究中对感染的定义。白细胞的细菌吞噬作用被定义为阳性结果(存在感染)。在本研究中所有的肉眼判断均与涂片检查一致。
感染模型组
为了建立不伴骨外露的感染伤口模型(第II组),采用标准化的全层皮肤缺损(直径为18mm),并接种大肠杆菌或金黄色葡萄球菌。对于伴骨外露的感染伤口模型(第IV组),形成18-mm皮肤缺损后像第III组所述使得骨外露,然后接种大肠杆菌或金黄色葡萄球菌。48小时后,进行冲洗和清创,用棉签获得细菌培养样品后将伤口用无菌敷料覆盖。每48小时重复进行开放伤口的冲洗和清创,直到最后一次冲洗和清创后获得的培养在48小时内培养为阴性。
设盲
对于膜联蛋白/PI染色和细胞计数,在溶液冲洗和后续评估(膜联蛋白V/PI染色和细胞计数)期间中,研究者对组别信息不知情。但肥皂液通过观察可很容易地被识别出。因此,设盲方法仅对生理盐水组和EDTA组有效。
研究者进行外科清创时对组别信息不知情。即便在冲洗过程中对组别信息也是不知情的,但此设盲方法也仅对生理盐水组和EDTA组有效,因为肥皂液可通过观察被识别出。
统计学分析
组间二分变量的检验使用Pearson’s卡方检验或Fisher’s精确检验法。使用对数秩统计算和比较卡普兰-迈耶曲线(Kaplan-Meier curves)。所用统计学分析使用SPSS软件(SPSS,Chicago,IL,USA)。
结果
当与生理盐水比较时,EDTA-NS对成纤维细胞和内皮细胞无额外毒性(图2)。使用生理盐水时,成纤维细胞的平均活性百分比为97%,而在使用EDTA-NS,其成纤维细胞的平均活性百分比为98%(p=0.654);使用生理盐水时,内皮细胞的平均活性百分比为97%,而在使用EDTA-NS,其内皮细胞的平均活性百分比为98%(p=0.711)。相比之下,肥皂液导致67%的成纤维细胞和72%的内皮细胞死亡(与生理盐水或EDTA-NS相比,p<0.001)。当使用10mmol/L EDTA-NS(10倍有效浓度)刺激细胞时,也显示了无显著的细胞死亡(成纤维细胞的平均活性:95%;内皮细胞的平均活性:96%)。
在伴骨外露的污染模型中(表1),EDTA-NS冲洗显示了更低的金黄色葡萄球菌检出率(EDTA-NS:7/30[23%],生理盐水:20/30[67%],肥皂液:12/30[40%];p=0.003)以及更低的大肠杆菌检出率(EDTA-NS:8/30[27%],生理盐水:17/30[57%],肥皂液:9/30[30%],p=0.032);然而,感染风险却仅在采用EDTA-NS冲洗时降低(对于金黄色葡萄球菌,EDTA-NS:3/30[10%],生理盐水:10/30[33%],肥皂液:11/30[37%],p=0.039;对于大肠杆菌,EDTA-NS:1/30[3%],生理盐水:8/30[27%],肥皂液:7/30[23%],p=0.038)。在不伴骨外露中(表2),阳性培养(对于金黄色葡萄球菌,EDTA-NS:7/30[23%],生理盐水:6/30[20%],肥皂液:5/30[17%],p=0.812;对于大肠杆菌,EDTA-NS:6/30[20%],生理盐水:6/30[20%],肥皂液:5/30[17%],p=0.930)和感染(对于金黄色葡萄球菌,EDTA-NS:1/30[3%],生理盐水:2/30[7%],肥皂液:1/30[3%],p=0.770;对于大肠杆菌,EDTA-NS:0/30[0%],生理盐水:1/30[3%],肥皂液:0/30[0%],p=0.364)的发生率并没有统计学差异。
在不伴骨外露的感染模型中,与生理盐水(金黄色葡萄球菌:1.74±0.80,平均差值0.32[95%置信区间{CI},0.03-0.61];大肠杆菌:1.66±0.77,平均差值0.32[95%CI,0.03-0.61])和肥皂液(金黄色葡萄球菌:1.94±0.77,平均差值0.52[95%CI,0.23-0.81];大肠杆菌:1.90±0.81,平均差值0.56[95%CI,0.27-0.85])相比,EDTA-NS冲洗(金黄色葡萄球菌:1.42±0.64;大肠杆菌:1.34±0.56)后降低了冲洗和清创的次数。当骨外露时,与生理盐水(金黄色葡萄球菌:2.32±1.11,平均差值0.42[95%CI,0.02-0.82];大肠杆菌:2.14±1.14,平均差值0.40[95%CI,0.03-0.77])和肥皂液(金黄色葡萄球菌:2.48±1.03,平均差值0.58[95%CI,0.18-0.98];大肠杆菌:2.22±0.95,平均差值0.48[95%CI,0.11-0.85])相比,EDTA-NS冲洗同样可以减少冲洗和清创的次数(金黄色葡萄球菌:1.90±0.84;大肠杆菌:1.74±0.69)。生存分析揭示在与生理盐水和肥皂液相比,EDTA-NS可以在更少的冲洗和清创先得到培养阴性伤口(图3;金黄色葡萄球菌感染的不伴骨外露伤口:p=0.003,大肠杆菌感染的不伴骨外露伤口:p=0.001;金黄色葡萄球菌感染的伴骨外露伤口:p=0.012,大肠杆菌感染的伴骨外露伤口:p=0.022)。

讨论
伤口处理的基石是适当的清创以除去坏死组织并建立活体组织和灌注组织的界限。伤口冲洗的主要目的是为了清除粘附其表面的细菌并防止/减少感染。为了实现这一目标,通过抑制细菌黏附素的功能来干扰细菌粘附为一种方法。目前基于此原理的有效方法包括抑制黏附素及其宿主受体、黏附素接种疫苗以及干扰受体-黏附素相互作用,但局限于仅针对一种/几种类型细菌的特异性感染。因此凸显了对大多数菌群普遍有效方法的需求。在本申请中,本申请的伤口冲洗溶液可以清除多种金属离子(包括钙、锌和镁),而这些离子对细菌粘附功能的正常发挥是必需的。本申请的实验验证了EDTA-NS溶液可以增强细菌去除并降低感染率,EDTA-NS是有效且安全的。与大多数既往研究一样,本申请的实验使用革兰氏阳性致病菌和革兰氏阴性致病菌的典型代表金黄色葡萄球菌和大肠杆菌的单一菌株,但由于大多数细菌粘附的正常作用发挥需要金属离子,因此EDTA-NS对其他致病菌也可预测是有效的。
Claims (9)
1.伤口冲洗液,其包含生理盐水和EDTA。
2.根据权利要求1所述的伤口冲洗液,其中伤口冲洗液中EDTA的浓度为0.5‐100mmol/L。
3.根据权利要求2所述的伤口冲洗液,其中伤口冲洗液中EDTA的浓度为0.5‐10mmol/L。
4.根据权利要求3所述的伤口冲洗液,其中伤口冲洗液中EDTA的浓度为0.5‐5mmol/L。
5.根据权利要求4所述的伤口冲洗液,其中伤口冲洗液中EDTA的浓度为1mmol/L。
6.根据权利要求1‐5中任意一项所述的伤口冲洗液,其中所述伤口冲洗液是经灭菌处理的。
7.根据权利要求6所述的伤口冲洗液,其中所述伤口冲洗液是通过将EDTA溶解在0.9%生理盐水而获得。
8.EDTA在制备伤口冲洗液中的应用。
9.根据权利要求8所述的应用,其中所述伤口选自伴或不伴骨外露的污染伤口或感染伤口。
