Abstract
本发明公开了土贝母苷甲在制备治疗类风湿性关节炎药物中的应用,相对于现有技术,本发明提供了土贝母苷甲治疗类风湿性关节炎的新用途,土贝母苷甲能够抑制RA滑膜细胞的过度激活,并且在体内可以对滑膜炎症和骨破坏施加治疗保护,表明土贝母苷甲治疗类风湿性关节炎效果显著。
Description
技术领域
本发明涉及土贝母苷甲的新应用,特别涉及土贝母苷甲在制备治疗类风湿性关节炎药物中的应用。
背景技术
类风湿性关节炎(Rheumatoid Arthritis,RA)是一种以关节慢性炎症、滑膜组织增生和软骨、骨的侵蚀性破坏为主要表现的全身性疾病,具有较高的发病率和致残率。位于关节中的滑膜成纤维细胞(FLS)通过局部产生细胞因子来降解细胞外基质,其中软骨的蛋白水解酶起关键作用。与来自健康个体的FLS不同,来源于RA滑膜的FLS获得了在转化细胞中常观察到的表型特征,例如非依赖性锚定生长,对细胞凋亡不敏感以及增加的增殖和侵袭。已经证明FLS的过度激活与RA及其动物模型中的影像学和组织学损伤密切相关。此外,活化的RA-FLS即使在炎症减轻的情况下也能够破坏软骨和骨骼(Lange等人,2005)。众所周知,这种对骨和软骨的侵蚀性破坏与疾病的严重程度以及残疾、畸形和预期寿命缩短的风险增加有关。因此,抑制RA滑膜细胞的过度激活可有效控制RA的病情发展。
目前临床上常用的治疗药物包括非甾体类抗炎药(Nonsteroidal anti-inflammatory drugs,NSAIDs)、糖皮质激素、缓解病情的抗风湿药物(disease-modifyinganti-rheumatic drugs,DMARDs)和生物制剂。现已证明,NSAIDs和糖皮质激素虽然能减少炎症,但不能阻止骨破坏的进展;DMARDs和生物制剂虽然能延缓骨破坏的程度,但长期摄入这些药物会引起严重的感染甚至会患恶性肿瘤,而且生物制剂价格昂贵,给病人带来沉重的经济负担。
发明内容
发明目的:为解决上述技术问题,本发明目的在于提供了土贝母苷甲(TBMS I)在制备治疗类风湿性关节炎药物中的应用。
技术方案:本发明提供了土贝母苷甲在制备治疗类风湿性关节炎药物中的应用。
所述类风湿性关节炎为包含滑膜炎症的类风湿性关节炎。
所述类风湿性关节炎为包含骨破坏的类风湿性关节炎。
所述药物是以所述土贝母苷甲作为单一成分所制成的药物。
或者,所述药物是以所述土贝母苷甲作为活性成分,加上药学上可接受的辅料所制成的药物。
所述药物选自注射剂、片剂、粉剂、颗粒剂、胶囊或口服液,但不限于上述剂型。
本发明所述土贝母苷甲在治疗类风湿性关节炎时,可以单独使用,也可以与其他药物配合同时使用,或者与其他药物一起制成复方制剂使用,都可以达到治疗类风湿性关节炎的目的。
本发明在制备不同剂型时加入所需的各种常规辅料(即药学上可接受的辅料),例如稀释剂、黏合剂、崩解剂、助流剂、润滑剂、矫味剂、包合材料、吸附材料等以常规的制剂方法制备成任何一种常用的制剂,例如可以是注射剂、片剂、粉剂、颗粒剂、胶囊、口服液、缓释剂等。
本发明通过设计体内外实验检测土贝母苷甲对RA的治疗作用。体外实验:检测土贝母苷甲对RA滑膜细胞过度激活的抑制影响(包括抑制RA滑膜细胞的增殖、迁移和炎性因子的表达等);体内实验:检测土贝母苷甲对胶原诱导的RA大鼠的治疗作用。结果表明,土贝母苷甲对于治疗类风湿性关节炎,效果显著。
技术效果:相对于现有技术,本发明提供了土贝母苷甲治疗类风湿性关节炎的新用途,土贝母苷甲能够抑制RA滑膜细胞的过度激活,并且在体内可以对滑膜炎症和骨破坏施加治疗保护,表明土贝母苷甲治疗类风湿性关节炎效果显著。

下面结合具体实施例对本发明进行详细说明。以下实施例将有助于本领域的技术人员进一步理解本发明,但不以任何形式限制本发明。应当指出的是,对本领域的普通技术人员来说,在不脱离本发明构思的前提下,还可以做出若干变形和改进。这些都属于本发明的保护范围。
实施例1
一、实验耗材
土贝母苷甲I(TBMS I,纯度≥98%)从成都曼斯特生物科技有限公司(成都,四川)购入(图1A)。Dulbecco改良的Eagle培养基(DMEM)和胎牛血清(FBS)购自Gibco(Gibco BRL,Grand Island,NY)。重组肿瘤坏死因子α(TNFα)购自Peprotech(PeproTech,Inc.,RockyHill,NJ)。MTS试剂获自Sigma(Sigma-Aldrich)。靶向磷酸化的(p)ERK,p-p38,p-JNK,ERK,p38,JNK,IκBα和GAPDH的抗体购自Santa Cruz Biotechnology(Dallas,CA,USA)。抗-p-IκBα抗体购自CST(Cell Signaling Technology,Inc.,USA)。TNFα和IL-1βELISA试剂盒购自CUSABIO BIOTECH CO.,Ltd(中国武汉)。鸡II型胶原蛋白(CII)购自Sigma ChemicalCompany(St.Louis,MO,USA)。
二、实验方法
1、CIA大鼠模型的建立和TBMS I药物处理
从上海斯莱克实验动物有限公司(中国上海)购买38只雌性Wistar大鼠(160-180g)。将大鼠置于SPF条件下(22℃,12小时光照/12小时黑暗,50-55%湿度)饲养,大鼠自由饮水和取食。1周适应期后,将大鼠用10%水合氯醛(3.0mL·kg-1体重)麻醉,每只大鼠皮内注射1mg鸡II型胶原(溶解于完全弗氏佐剂)。七天后,给大鼠皮下加强注射0.5mg鸡II型胶原(溶解于不完全弗氏佐剂)。对照大鼠以相同的方式注射生理盐水(n=6)。将CIA大鼠(关节炎指数评分≥2)随机分为模型组和三个TBMS I治疗组(每组8只)。TBMS I治疗组腹腔注射不同剂量的TBMS I(低剂量1mg·kg-1·day-1;中剂量5mg·kg-1·day-1;高剂量10mg·kg-1·day-1),模型组每天注射0.9%生理盐水,治疗时间为14天。自第一次免疫接种后的第9天开始,每天用0~4分的评分系统进行临床关节炎指数学评分,评分标准为:0分:足爪无肿胀和红斑;1分:有红色斑点或轻度肿胀;2分:关节部位中度肿胀;3分:严重肿胀;4分:严重肿胀且不能负重。每只动物的得分介于0和最大值16之间。另外,用足爪测量仪(YLS-7A,山东省医学科学院设施站)测量爪肿胀。评分和爪体积测量由两位独立的观察员进行。治疗14天后,收集血清用于细胞因子分析,收集关节组织用于影像学、组织学和促炎因子检测的评估。所有动物实验均经南京师范大学实验委员会批准。
2、影像学评估
使用FX小动物活体成像系统在X射线下成像大鼠后爪。使用影像学评分进行分析。总分由每只大鼠的两只后爪的总和计算,最大值为6。影像学评分者对大鼠分组情况不清楚。
3、组织病理学评估
影像学评估后,将左踝关节固定在4%PFA(Sigma-Aldrich)中,在12%二钠EDTA中脱钙并包埋在石蜡中。切片(4μm)并用苏木精和伊红(H&E)染色,并记录炎症、滑膜增殖、血管翳形成、软骨损伤和骨侵蚀的变化,如前所述。组织病理学分析由独立的观察者以盲法进行评估。
4、CIA大鼠的细胞因子水平
收集血液,离心并将上清液保存在-80度直到开始分析。将大鼠右爪进行组织匀浆。根据说明书步骤,通过特异性ELISA试剂盒测量关节中的TNFα和IL-1β含量。使用市售的特异性放射免疫测定试剂盒(Sino-UK Institute of Biological Technology,Beijing,China).测定血清中TNFα和IL-1β含量。
5、CIA大鼠FLS的培养和鉴定
通过酶消化法从CIA大鼠膝关节滑膜组织中分离出FLS细胞。简而言之,将滑膜切成小块,并在37℃下用0.4%胶原酶(Sigma-Aldrich)在DMEM培养基中温育2小时。然后,更换培养基并用0.25%胰蛋白酶连续孵育细胞30分钟。随后将细胞重复洗涤并在含有10%FBS、100单位·mL-1青霉素和100μg·mL-1链霉素的DMEM中在5%CO2培养箱中培养过夜。洗去非贴壁细胞,并在相同培养基中培养贴壁细胞。利用免疫荧光技术,通过形态学和血管细胞粘附分子-1(VCAM-1)的表达来鉴定这些细胞是否为FLS。大部分培养的细胞在传代2次后包含同质的FLS群体。用于实验的细胞处于第三至第九代。
6、细胞活力测定
用MTS测定法检测TBMS I对大鼠FLS活力的影响。将大鼠FLS细胞用梯度稀释的TBMS I(0~80μM)处理48小时,然后根据说明书步骤加入MTS/PMS混合物再孵育4小时。在分光光度计(Model 680,BioRAD)和GraphPad Prism(version 5.0c;San Diego,CA,USA)中测量490nm处吸光度的变化以计算半数最大致死浓度(IC50)。
7、TNF-α诱导的FLS增殖
通过使用EdU掺入法测量DNA合成来分析细胞增殖。用TBMS I(1μM)预处理类大鼠FLS细胞6小时,然后在培养基中选择性加入TNFα(50ng·mL-1)再培养16小时。在实验结束前6小时,向每个孔中加入10μM EdU(Sigma-Aldrich),根据说明书步骤测量EdU在新合成的DNA中的掺入量。将细胞核用Hoechst 33342复染。细胞增殖率计算为每孔5个高倍视野中掺入EdU的有核细胞的比例。
8、细胞迁移能力测试
划痕实验用于分析TBMS I对FLS迁移率的影响。将FLS铺于24孔板中,待70-80%汇合度时,血清饥饿(0.1%FBS/DMEM)过夜。使用无菌200mL移液管尖端(设定为0h)划出线性划痕,然后用PBS洗涤三次以除去未附着的细胞。加药组和对照组一起温育1小时,然后将TNF-α(50ng·mL-1)加入孔中进行刺激。通过显微镜观察,在0h将划痕的每个区域底部创建一个参考点,刺激24h后立即在同一参考点处对无细胞区域进行拍摄和测量,分析计算细胞迁移率。
9、流式细胞术检测细胞凋亡
使用膜联蛋白V-FITC凋亡检测试剂盒(e-Bioscience,San Diego,CA)按照说明书的步骤进行细胞凋亡测定。将大鼠FLS细胞置于不同浓度(0、1、2.5、5和10μM)的TBMS I中处理24小时。然后将细胞悬浮于缓冲液中并用膜联蛋白V和碘化丙啶(PI)溶液染色。流式细胞分析用FACScan(Becton Dickinson)和CellQuest程序进行,并表示为每个群体中凋亡细胞的百分比。
10、实时荧光定量PCR检测
使用实时荧光定量PCR来检测TNF-α诱导的大鼠FLS中细胞因子的表达水平。将细胞置于12孔板中培养过夜,然后在指定浓度的TBMS I(0、0.5、1、2.5μm)作用下预处理1小时,再用TNF-α(50ng·mL-1)温育24小时,使用TRIzol试剂(Invitrogen LifeTechnologies,Carlsbad,CA)提取总RNA,并按照说明书进行cDNA合成。

RT-PCR实验严格按照2×Pfu PCR MasterMix说明书进行。按照体系将所需模板、引物、酶和无RNA酶水加入200μl的PCR管中,在PCR仪中进行扩增。每个实验用三个平行并重复三次。β-actin的表达为内参。
11、蛋白免疫印迹分析
将FLS以2×105个/孔接种到6孔板中,再分别用土贝母苷甲和TNFα(50ng·mL-1)处理不同的时间。将细胞置于放射免疫沉淀测定(RIPA)裂解缓冲液(50mM TriseHCl,150mMNaCl,5mM EDTA,1%Triton X-100,1mM氟化钠,1mM钒酸钠,1%脱氧胆酸盐和蛋白酶抑制剂)中,在12000×g下离心10分钟,收集上清液中的蛋白质。然后进行SDS-PAGE电泳,再将蛋白质转移到硝酸纤维素膜上。硝酸纤维素膜用5%牛血清蛋白封闭。用针对p-p38、p-ERK、p-JNK、p-IκB、p38、ERK1/2、-JNK,IκB和GAPDH的抗体探测印迹。然后用增强化学发光(ECL)试剂(Amersham)显影。最后用Image J软件分析每条带的强度。
三、统计分析
所有数据用三个或更多个实验获得的均数±标准差。统计分析使用非配对双尾t检验进行,P<0.05被认为有统计学意义。
四、实验结果
1、土贝母苷甲抑制CIA大鼠滑膜炎症和骨质破坏
为研究TBMS I对CIA动物模型的影响,选择明显发生关节炎(临床评分≥2)的大鼠,腹腔注射不同浓度的TBMS I(低剂量:1mg·kg-1·d-1;中剂量:5mg·kg-1·d-1;高剂量:10mg·kg-1·d-1)或0.9%生理盐水,每天一次,持续14天。如图2和图3所示,足爪外观形态(红斑或肿胀)证实TBMS I以剂量依赖性显著阻断CIA大鼠的发生和进展。与临床关节炎评分一致,CIA大鼠足爪肿胀的测量也显示TBMS I治疗是有效的(图4)。与无明显治疗作用的1mg·kg-1的TBMS I剂量相比,5mg·kg-1剂量显著降低CIA大鼠的关节炎症状,明显减轻关节炎的严重程度。高剂量10mg·kg-1的TBMS I治疗几乎完全抑制CIA的进展和发展,在CIA动物治疗中显示出巨大的治疗潜力(图2-4)。
随后,用影像学方法研究了TBMS I对CIA大鼠关节破坏的影响。在X射线成像条件下,无关节炎大鼠呈现正常的关节结构和关节间隙。没有药物治疗的CIA大鼠表现出RA的典型变化,观察到明显的骨侵蚀、关节破坏、关节移位和关节间隙变窄(图5)。然而,影像学检测评分显示TBMS I处理后的大鼠中关节的破坏程度被剂量依赖性减轻(图5),表明其除了在临床评分和足爪肿胀评分中所呈现的抗炎症效果外,TBMS I还可以发挥抗骨破坏作用。
为了进一步评估TBMS I对发展中的关节炎的影响,通过半定量分级量表进行组织病理学评估。结果显示,未加药物治疗的CIA大鼠的踝关节中,RA的病理学特征明显,包括炎性细胞浸润、滑膜增生、血管翳形成、软骨破坏和骨侵蚀。TBMS I处理后可剂量依赖性地抑制上述结构变化(图6和图7),进一步证明了TBMS I对RA的治疗作用。
2、TBMS I处理可显著抑制CIA大鼠中炎性细胞因子的产生
众所周知,促炎性因子在RA的发病过程中起关键作用。因此,收集了CIA大鼠关节组织和血清以测量TNFα和IL-1β的含量。与未加药物处理的CIA大鼠相比,TBMS I处理的大鼠关节组织中TNFα和IL-1β的含量显著降低(图8A)。与组织ELISA数据一致,用TBMS I处理对血清中TNFα和IL-1β的水平存在显著的抑制作用(图8B),表明TBMS I可通过下调炎性因子的合成来抑制关节炎的进程。
3、TBMS I可抑制TNFα诱导的关节炎大鼠FLS的增殖和迁移
为进一步研究TBMS I对类风湿性关节炎的治疗作用,首先从CIA大鼠膝关节中分离、培养和鉴定滑膜成纤维细胞。如图9A所示,培养的细胞呈梭形,与成纤维细胞形态学特征一致。此外,由免疫荧光实验(图9A)证实,VCAM-1呈阳性表达,培养的细胞与FLS的内膜亚群相一致。
接下来用MTS检测了TBMS I对大鼠FLS的潜在细胞毒性作用。如图9B所示,用不同浓度的TBMS I处理FLS48小时以上,结果显示IC50为14.83μM。因此,在接下来的实验中TBMSI的浓度不超过10μM。为了进一步研究TBMS I对FLS增殖的影响,申请人进行了EdU实验。如图10A和图10B所示,单独用TNFα处理24小时显著增加细胞增殖潜力,1μmTBMS I显著抑制TNFα诱导的FLS增殖。
为了研究TBMS I对FLS细胞迁移的影响,用不同剂量的TBMS I处理FLS细胞,经TNFα刺激后进行划痕实验。如图11A和11B所示,当剂量大于1μM时,TBMS I显著降低细胞迁移面积。在用5μMTBMS I处理后,划痕区域中仅发现约40%的FLS数量,表明TBMS I对FLS迁移潜力具有剂量依赖性抑制作用。
已有研究证实TBMS I可在几种癌细胞中诱导细胞凋亡,因此本发明研究了TBMS I在FLS中的细胞凋亡诱导作用。如图12A和12B所示,在较高剂量(10μM)下,膜联蛋白V阳性细胞的数量增加。然而,与对照细胞相比,用量少于5μM(其显著抑制增殖和迁移)的处理不增加膜联蛋白V阳性FLS的数目,这表明TBMS I在较低浓度下不诱导细胞凋亡。
4、TBMS I在培养的FLS中抑制TNFα诱导的促炎因子产生
本发明已经证明TBMS I可以减少CIA大鼠中TNFα和IL-1β的产生。为了进一步研究TBMS I在RA中的抗炎效果,将FLS细胞与不同浓度的TBMS I一起温育,然后用TNFα刺激24小时以模拟RA患者中的滑液流体环境。如图13所示,TNFα刺激显著增加包括IL-1β,IL-6,IL-8,TNFα和MMP-9等细胞因子的mRNA转录水平。然而,用TBMS I处理则显著抑制上述炎性细胞因子的表达,并下调MMP-9的产生,该基因被认为是与软骨退化和骨侵蚀有关的关键基因。
5、TBMS I抑制TNFα诱导的MAPK和NF-κB的激活
为了探索TBMS I抑制细胞因子产生并进一步减弱RA进展的潜在机制,通过免疫印迹分析了TNFα诱导的MAPK信号传导途径。和预期的一样,经过5分钟TNFα刺激,使包括细胞外信号调节激酶(ERK),C-Jun N-末端激酶(JNK)和p38激酶在内的三个MAPK家族成员磷酸化水平升高。相比之下,用TNFα和TBMS I共同处理FLS显著减弱了TNFα诱导的JNK和p38的磷酸化(图14A,B),而TBMS I对TNFα诱导的ERK激活具有增加的作用(图14A,B)。此外,检测到了NF-κB的激活,这证明NF-κB在RA的发展和进展中起关键作用。如图14C和14D所示,对照组TNF-α刺激的NF-κB抑制亚基IκBα磷酸化,而TBMS I处理抑制了磷酸化并降解了IκBα。
五、结论
近几十年来,TBMS I的主要药理学发现是证明其有效的抗癌活性。本发明发现TBMS I具有新的药理作用,即通过抗炎和抑制骨破坏有效抑制RA的发生发展。
众所周知,炎性因子通过促进自身免疫,慢性炎症和组织损伤而参与RA发病的各个阶段(Mcinnes等,2007)。因此,减少炎症反应和维持关节活动性是RA治疗的主要目的。在本发明中,TBMS I可以通过抑制炎症反应来有效减轻RA的严重程度。首先,TBMS I抑制CIA大鼠的局部和全身水平的促炎性因子;其次TBMS I在培养的FLS细胞中抑制了IL-1β,IL-6,IL-8和TNFα等细胞因子的产生;最后,TBMS I在临床和组织病理学水平降低了CIA大鼠的炎症反应。RA中TBMS I的这种抗炎作用可归因于其抑制一系列炎性细胞因子合成的能力。
除了抗炎症之外,TBMS I还被证明可以抑制CIA大鼠的关节破坏。影像学和组织病理学评估显示TBMS I剂量依赖性地减弱CIA诱导的关节损伤。
成纤维细胞样滑膜细胞(FLSs)通过产生炎性细胞因子和促进软骨破坏的蛋白酶,在RA发病机制中起关键作用。RA FLS的过度增殖和迁移是滑膜增生和侵袭性血管翳形成的主要原因,这进一步加剧了关节损伤。本发明应用中TBMSI能够通过抑制增殖和迁移来减弱关节炎FLS的破坏性表型。EdU掺入法显示,在1μM的剂量下,TBMS I显著降低了EdU阳性细胞的比例。TBMS I的这种抗增殖作用由体内数据进一步证实。发现TBMS I显著减弱了CIA大鼠的滑膜增生。此外,本发明证明了TBMS I能够降低FLS的迁移率,这通过在TBMS I处理的CIA大鼠体内抑制的血管翳形成来进一步证实。众所周知,MMPs特别是MMP-9已被证明在细胞迁移中发挥重要作用,尽管有些基质金属蛋白酶组分在无活性的FLS中组成型表达,但不受炎性细胞因子的影响,但MMP-9可被炎性细胞因子如TNFα和IL-1β诱导。本发明中证明了TBMSI下调TNFα诱导的MMP-9的表达,这与TNFα刺激的FLS迁移的结果一致,进一步证明了TBMS I在RA治疗中的抗迁移性质。
明确了TBMS I在CIA模型和FLS细胞培养中发挥治疗作用后,进一步研究了可能涉及的信号传导机制。因为MAPK在调节一系列细胞应答中发挥关键作用,特别是涉及细胞增殖,迁移和炎症应答,所以这些激酶的调节可以为包括RA在内的炎性相关疾病的治疗提供依据。而且在活化的类风湿关节炎中,MAPKs已被证明是普遍上调的。本发明发现TBMS I可抑制TNFα诱导的JNK和p38的磷酸化。这些抑制活性有助于抑制促炎症细胞因子的分泌,并且细胞因子水平的降低导致了FLS的抗增殖活性。
NF-κB是一种多效转录因子,在调节炎症细胞因子和MMPs的表达中起重要作用,所有这些都与RA的发病密切相关。这些基因的产物协同增强炎症反应,进一步激活NF-κB。鉴于RA中这种正调控环的重要性,抑制NF-κB途径可被认为是延缓RA发展的新策略。本发明TBMS I可显著抑制IκBα磷酸化和降解。总之,上述数据表明,TBMS I可能通过多个靶点发挥对RA的治疗作用。
综上所述,本发明证实了从土贝母(葫芦科)分离的天然产物TBMS I通过抗炎和抑制关节破坏对CIA大鼠起治疗作用。此外,TBMS I能够改变关节炎FLS的生物学功能,包括抑制其增殖和迁移。进一步分析表明,TBMS I抑制TNFα诱导的NF-κB和MAPK(p38和JNK)的活化,导致促炎因子的下调。因此,TBMS I有望发展成人类RA的新型替代性天然药物。

4.根据权利要求1所述的土贝母苷甲在制备治疗类风湿性关节炎药物中的应用,其特征在于,所述药物是以所述土贝母苷甲作为单一成分所制成的药物。
5.根据权利要求1所述的土贝母苷甲在制备治疗类风湿性关节炎药物中的应用,其特征在于,所述药物是以所述土贝母苷甲作为活性成分,加上药学上可接受的辅料所制成的药物。
6.根据权利要求4所述的土贝母苷甲在制备治疗类风湿性关节炎药物中的应用,其特征在于,所述药物选自注射剂、片剂、粉剂、颗粒剂、胶囊或口服液。
