Abstract

本发明涉及一种在脊柱椎间融合手术中使用的高度可调节的椎间固定融合器,属于医疗器械领域。包括四边形的活动支架,所述活动支架上下面对称,并且上下方向上对称地分布有支撑平台和接触界面,所述的活动支架、支撑平台和接触界面中间都有中空的植骨槽,所述的活动支架水平方向有螺杆穿过固定块上的圆孔与调节螺母配合,所述的固定块两侧有上下方向的滑槽;支撑平台通过钩形结构连接固定块,所述钩形结构可在固定块的滑槽内滑动;所述支撑平台和接触界面通过限位结构固定连接。本发明可以分开支撑平台,从而实现了椎间融合器高度的变化。本发明在使植骨充分融合的同时,简化了手术步骤,缩短手术时间。

Description

高度可调节的椎间固定融合器
技术领域
本发明涉及一种在脊柱椎间融合手术中使用的高度可调节的椎间固定融合器,属于医疗器械领域。
背景技术
在脊柱外科手术中,相对于经前路椎体间融合(Anterior lumbar interbodyfusion)技术(ALIF)和经后路椎体间融合(Posterior lumbar interbody fusion)技术(PLIF),经椎间孔椎体间融合(Transforaminal lumbar interbody fusion)技术(TLIF)是一种较新的脊柱融合技术。TLIF技术具有创伤小、融合率高和符合脊柱生物力学的特点,在脊柱融合领域逐渐占主导地位[Christopher P,Ames,Frank L,et al.Biomechanicalcomparision of posterior lumbar interbody fusion and transforaminal lumbarinterbody fusion performed at 1and 2levels[J].Spine,2005,19:562-566.][AnnetteKettler,Werner Schmoelz,ErichKast.In vitro stabilizing effect of atransforaminal compared with two posterior lumbar interbody fusion cages[J].Spine,2005,22:665-670]。它是通过后路实行彻底的单侧关节突关节切除术,而不去显露神经根以及硬膜等椎管内结构。由此可降低包括神经并发症在内的多种并发症的风险。TLIF可以提供前柱的支撑和稳定,同时结合后路的融合,手术Ⅰ期可以达到360°融合的效果。该手术方法结合后路的内固定技术可以提高融合率。
通过此技术还可以重建相应节段的正常解剖曲度,从而维持或恢复腰椎整体的生理曲度。术中椎间盘切除后可以维持椎间隙的高度,恢复椎间孔的高度,从而改善椎间孔的狭窄。由于椎间隙高度的恢复,间接使得折叠的黄韧带和被压缩的纤维环得以恢复,从而使中央椎管的狭窄也可以得到明显的改善。TLIF技术自发明后得到了较快的发展,目前已经广泛应用于腰椎退变性疾病,腰椎不稳以及椎间盘源性疾病。
椎间融合器有助于提高椎间的融合效果,是椎间融合手术的必要补充。带倾斜角度的椎间融合器仿造椎间盘组织解剖结构,更加符合脊柱的生物力学,与上下椎体的骨性接触面积更大,可有效分散应力,中间有植骨槽,可有效提高融合率。但是,固定高度的融合器不能很好地适应椎间隙高度的变化。太高的高度不太容易装进椎间隙,击打时易将内植骨震出来;太低的高度虽然容易装入椎间隙,但是与上下椎体接触不好,稳定性差,不易发生骨融合,有滑入椎管的风险。
因此,如果有可以调节椎间高度的固定融合器装置,不仅可简化手术操作、缩短手术时间、提高手术疗效,而且可以减少患者的医疗费用,这将会是医生和患者共同的福音。
发明内容
本发明提供一种用于椎间融合手术的高度可调节的椎间固定融合器。使用该椎间固定融合器,可以简化手术操作和提高椎间融合效果。
一种高度可调节的椎间固定融合器,包括四边形的活动支架,所述活动支架上下面对称,并且上下方向上对称地分布有支撑平台和接触界面,所述的活动支架、支撑平台和接触界面中间都有中空的植骨槽,所述的活动支架水平方向有螺杆穿过固定块上的圆孔与调节螺母配合,所述的固定块两侧有上下方向的滑槽;支撑平台通过钩形结构连接固定块,所述钩形结构可在固定块的滑槽内滑动;
所述活动支架上下两面上,靠近固定块端有两个较矮的弧形凸起,远离固定块端有两个相对较高的楔形凸起;所述支撑平台上,靠近固定块端有两个弧形槽与弧形凸起配合,远离固定块端有两个楔形槽,用于插入楔形凸起;所述支撑平台和接触界面通过限位结构固定连接。
为了提高融合器的融合效果,在融合器的接触界面、支撑平台、活动支架中间预留有植骨槽,有利于上下椎体骨组织与植骨槽的骨组织融合,形成一个整体。
所述的接触界面为与脊柱椎体的表面相适应的弧形结构。
为了适应防止融合器的塌陷,融合器的接触界面选用弹性模量跟骨组织非常接近的PEEK材料。
为了实现融合器有足够而恒久的支撑力,支撑平台、活动支架、固定块均选用医用钛合金或钴铬钼合金材料。
本发明涉及一种提高脊柱融合率的椎间融合器,该融合器可根据椎间隙高度来调节自身的高度,上下支撑面有10度左右的倾斜角度,更符合椎间隙的生理结构。植骨槽植骨后融合效果更好,手术操作更简化,固定融合远期效果更好。

具体实施方式
如图1所示,一种高度可调节的椎间固定融合器,包括四边形的活动支架1,所述活动支架上下面对称,并且上下方向上对称地分布有支撑平台2和接触界面3,所述的活动支架、支撑平台和接触界面中间都有中空的植骨槽6,所述的活动支架水平方向有螺杆11穿过固定块4上的圆孔41与调节螺母5配合,所述的固定块4两侧有上下方向的滑槽42;支撑平台2通过钩形结构21连接固定块4,所述钩形结构21可在固定块的滑槽42内滑动;
所述活动支架上下两面上,靠近固定块端有两个较矮的弧形凸起12,远离固定块端有两个相对较高的楔形凸起13;所述支撑平台上,靠近固定块端有两个弧形槽22与弧形凸起12配合,远离固定块端有两个楔形槽23,用于插入楔形凸起13;所述支撑平台和接触界面通过限位结构固定连接。
与骨界面接触的接触界面3中间为四边形空心结构,用于植骨。接触界面3表面分布有倒钩的锯齿状结构31,防止融合器的向后滑脱,滑入椎管造成神经的损伤。接触界面为弧形结构,弧形结构与脊柱椎体的表面相适应。接触界面的内表面有限位孔32及限位凸起33,与支撑平台的限位柱25和限位槽26分别相适应。或者采用其它的类似限位结构,通过这些限位结构,使接触界面与支撑平台紧密的结合在一起。
活动支架1两侧面各有四个凸起,远端为高而倾斜的楔形凸起13,近端为低的弧形凸起12。支撑平台与活动支架接触的面有楔形槽23和弧形槽22,分别与活动支架的楔形凸起13和弧形凸起12相配合。支撑平台与固定块连接部分为钩状结构,此结构可以在固定块两侧的滑槽42内上下滑动。通过拧动调节螺母5,使得活动支架和支撑平台合拢或分开,从而实现融合器高度的调节。当活动支架和支撑平台合拢时,弧形凸起和弧形槽重合,楔形凸起与支撑平台的尾翼结构24互相作用,防止支撑平台的分开,有利于融合器的置入。当活动支架和支撑平台分开时,弧形凸起12和弧形槽22错位,楔形凸起13在楔形槽23滑动一段距离,融合器的高度就会升高。由于弧形凸起和楔形凸起高度的不一致,就可以实现融合器张开以后有10度的倾斜角,与椎间隙相匹配。当然,楔形凸起13和弧形凸起12的滑入及滑出由活动支架上的螺杆11控制,通过调节螺母5来实现。
所述调节螺母限位于固定块中间的圆孔中转动。限位的方法有多种。一种方式是,调节螺母5上连接有连接片51,连接片在圆孔中转动,连接片尾端有凸台52。凸台阻止了连接片和调节螺母的退出。如图3所示。另一种方式是,调节螺母5上连接有连接片51,连接片在圆孔中转动,固定块上安装固定螺栓53,顶住连接片51。如图4所示。
调节螺母5带有内螺纹,与活动支架上的螺杆上的外螺纹配套,通过拧转调节螺母,就可以实现活动支架的前进与后退,从而实现融合器的打开与闭合,也就实现了融合器高度的调节。
本发明通过拧动调节螺母,控制活动支架的移动,实现支撑平台的分离和合拢,从而实现椎间固定融合器的不同高度,以适应不同的椎间隙高度。

Claims (9)

1.一种高度可调节的椎间固定融合器,其特征在于,包括四边形的活动支架(1),所述活动支架上下面对称,并且上下方向上对称地分布有支撑平台(2)和接触界面(3),所述的活动支架、支撑平台和接触界面中间都有中空的植骨槽(6),所述的活动支架水平方向有螺杆(11)穿过固定块(4)上的圆孔(41)与调节螺母(5)配合,所述的固定块(4)两侧有上下方向的滑槽(42);支撑平台(2)通过钩形结构(21)连接固定块(4),所述钩形结构(21)可在固定块的滑槽(42)内滑动;
所述活动支架上下两面上,靠近固定块端有两个较矮的弧形凸起(12),远离固定块端有两个相对较高的楔形凸起(13);所述支撑平台上,靠近固定块端有两个弧形槽(22)与弧形凸起(12)配合,远离固定块端有两个楔形槽(23),用于插入楔形凸起(13);所述支撑平台和接触界面通过限位结构固定连接。
2.根据权利要求1所述的椎间固定融合器,其特征在于,所述的接触界面(3)表面分布有锯齿状结构(31)。
3.根据权利要求1所述的椎间固定融合器,其特征在于,所述的接触界面(3)为与脊柱椎体的表面相适应的弧形结构。
4.根据权利要求1所述的椎间固定融合器,其特征在于,所述的接触界面选用PEEK材料。
5.根据权利要求1所述的椎间固定融合器,其特征在于,接触界面的内表面有限位孔(32)及限位凸起(33),与支撑平台的限位柱(25)和限位槽(26)分别相适应。
6.根据权利要求1所述的椎间固定融合器,其特征在于,所述调节螺母限位于固定块中间的圆孔中转动。
7.根据权利要求6所述的椎间固定融合器,其特征在于,调节螺母(5)上连接有连接片(51),连接片(51)在圆孔中转动,连接片尾端有凸台(52)。
8.根据权利要求6所述的椎间固定融合器,其特征在于,调节螺母(5)上连接有连接片(51),连接片(51)在圆孔中转动,固定块上安装固定螺栓(53)顶住连接片(51)。
9.根据权利要求1所述的椎间固定融合器,其特征在于,支撑平台、活动支架、固定块均选用医用钛合金或钴铬钼合金材料。
