技术领域

背景技术
本类型的覆膜支架被用在血管中,例如,以便于支撑异常狭窄、扩张或受损的血管。支架和膜的组合也被用于治疗需要治疗且需求更大长度且首要是具有柔韧性的植入物的延长的血管节段。特别地,覆膜支架被利用以桥接血管畸形,例如,以便隔离动脉瘤与血液循环。气囊导管通常被用于植入此类覆膜支架。
包含两个支架和一个柔性膜(诸如聚四氟乙烯)的最先进的覆膜支架已知被用于本任务。专利公开EP 2 151 217 A1中描述了此类覆膜支架。所公开的覆膜支架包含内支架和同轴地围绕内支架的外支架,在内支架与外支架之间布置有柔性可膨胀的膜。在膜位于支架之间的情况下,该支架的端部区域被焊接在一起。

在适于将膜连接到支架的上述覆膜支架中,仅在EP 1 266 635 A2中示出的支架将一个支架精确地与一个膜精确地连接。其他现有技术解决方案总是需求另一个支架或另一个膜,其被布置为同轴地围绕第一支架或第一膜以用于增强目的。
根据EP 1 266 635 A2的解决方案使用钩子或线和/或替代地使用两个部件的重叠部来进行支架与膜之间的连接。在这方面,钩子和线(其被暴露于血管内的高摩擦力)的耐久性总是有问题的。此外,由于钩子伸出或边缘突出在支架与膜之间,所以也存在组织刺激或损伤的风险。
WO 2012/084202 A1和WO 01/66035 A2中也描述了覆膜支架。在这种情况下,膜被固定在外围环形节段处,其意味着在放置期间支架的膨胀可能妨碍膜在支架上的固定。
发明内容
因此,本发明的目的是建议一种覆膜支架,该覆膜支架简单地且耐久地连接支架和膜,占据很少的血管腔且不引发组织刺激或损伤。特别地,膜与支架之间的连接不应该被设置在环形节段处以便于确保膜的安全固定。
所述目的是由提出了首先在上文提到的那种覆膜支架的本发明来实现,其中膜被固定在从支架的端部所见的第二环形节段的至少两个弯曲部与延伸到相邻末端环形节段的连接片之间。
第二环形节段的各个弯曲部或单个的弯曲部可以在相邻末端环形节段的方向上延伸。
由本发明所建议的覆膜支架包含由连接片互相连接的多个曲折(meandering)环形节段。这些连接片和环形节段相当于常规支架(如该常规支架经常被建议和被使用)的环形节段。仅位于邻近末端环形节段的环形节段(即,在从支架的端部观察时的第二环形节段)可以根据本发明来设计和修改。
提到外围环形节段,环形节段将被理解为被布置在支架的端部处,即,环形节段在端部处限制支架。如同其他环形节段,环形节段具有曲折片模式且通过连接片被连接至相邻环形节段(第二环形节段)。
环形节段的曲折片运行部(run)被理解为意味着波状片运行部和Z字形片运行部这两者。波状片运行部和Z字形片运行部以及环形节段通常被用于血管支架,以便于容许在膨胀期间发生的扩张。
常规连接片在环形节段之间延伸。这些连接片可以具有平直配置,且在这种情况下,这些连接片彼此连接相邻环形节段的谷部(内拱部(arch))或者替代地连接环形节段的弯曲部的拱部与相邻环形节段的弯曲部的谷部。连接片也可以具有曲线形,例如该连接片是S形或被螺旋卷绕(wound)或被盘绕(coiled)。上述布置和形状用于至少部分地补偿在支架的膨胀期间发生的长度收缩。
环形节段的拱部和谷部是环形节段的弯曲部的峰值或转折点,这一方面是由于环形节段的曲折配置和另一方面是由于抑制所导致的。每个拱部具有两个相邻谷部,每个谷部具有两个相邻拱部。谷部表示内拱部,峰是外拱部。

为了确保在由本发明所建议的覆膜支架中,在膨胀期间和在膨胀之后所述的固定被适当保持,优选地,每个第二环形节段的至少一个弯曲部(尤其是后面固定有膜的每个弯曲部)在各自支架的端部方向上被伸长。
为了能够实现膜的此种类型的固定,附连到支架的末端环形节段的连接片终止在第二环形节段的弯曲谷部中。这允许膜被推在连接片与环形节段的拱部之间且膜被保持在合适位置。在本区域中的连接片优选地是平直的且从向内指向支架的末端环形节段的拱部开始,延伸到第二环形节段的谷部中。替代地,连接片也可以连接末端环形节段的谷部与第二环形节段的谷部。
为了更好地在第二环形节段的区域中的保持所述膜,至少一个弯曲部被在支架的端部的方向中延伸。特别地,指向支架的外部的第二环形节段的弯曲部以交替样式来伸长。在这种情况下,第二环形节段的伸长的弯曲部的长度比第二环形节段的较短弯曲部的长度可以长了50%至150%。
在支架的膨胀期间,这种弯曲部的延长导致较长弯曲部经历了相对于其长度的相对较小缩短,而较短环的长度减少是相当足够的。这意味着膜在较长环下面的保留是非常有效的。
替代地或附加地,第二环形节段指向支架的外部的弯曲部可以被设置有指向支架外部的盲片。这些盲片不用于连接环形节段与相邻环形节段;它们有自由端且因此可以有助于膜的保持。此类盲片起始与弯曲部的拱部并可以将弯曲部延伸50%至100%。在此过程中,盲片可以伸入到末端环形节段的谷部中。盲片将被视为弯曲部延伸的形式,在其后面膜可以被固定。
作为一种规定,由本发明所建议的覆膜支架包括可以通过气囊导管而膨胀的支架,例如,医学上可接受的钢所制成的支架。替代地,也可能使用提供用于是自膨胀设计的支架的变体(例如,其是通过使用诸如镍钛合金的形状记忆合金)。
膜(通常包含薄膜或管)可以包含在医学领域中通常利用和批准的任何期望材料。然而,PTFE和聚酯是特别合适的。尤其优选是由ePTFE制成的膜。例如,膜也可以通过使用抗炎性物品药物(substances)、增殖抑制物品药物或治疗物品药物(诸如雷帕霉素、紫杉醇或肝素)被功能地覆盖。膜的形状优选是管状且具有与支架膨胀一起的所需求的伸长能力。
根据本发明所建议的覆膜支架被主要用于治疗血管畸形。这可以包括不仅闭合分叉血管或分支血管,而且包括堵塞动脉瘤或动静脉分流。
此外,也可能提供以下实施例:其中本发明的覆膜支架包括两个支架单元,该两个支架单元用于将中间管状膜夹紧到血管中。在这种情况下,支架具有膜仅在一端部处固定的夹紧连接;管状膜的另一端部被连接到第二支架。此类覆膜支架可以例如在血管的上皮细胞层的闭塞之后被植入到广泛受损血管中。

1.一种覆膜支架,所述覆膜支架包含支架(1)和至少一个膜(2),所述支架(1)包括具有曲折配置的多个环形节段(3、4),所述多个环形节段(3、4)被并排布置且由连接片(6)彼此连接,所述至少一个膜(2)覆盖所述支架(1)的整个外侧,被向内包裹在所述支架的端部并被固定在所述环形节段(4)的弯曲部与将相邻环形节段(3、4)彼此连结的连接片(6)之间,其特征在于,所述膜(2)被固定在从所述支架(1)的端部所见的第二环形节段(4)的至少两个弯曲部与到相邻的末端环形节段(3)的所述连接片(6)之间。
2.根据权利要求1所述的覆膜支架,其特征在于,所述末端环形节段(3)通过平直连接片(6)被连接到相邻环形节段(4),所述平直连接片(6)从指向所述支架的内部的所述末端环形节段(3)的外拱部(9)延伸进入所述第二环形节段(4)的谷部或内拱部(8)。
3.根据权利要求1或2所述的覆膜支架,其特征在于,所述第二环形节段(4)的指向所述支架的外部的各个所述弯曲部交替地具有不同长度。
4.根据权利要求3所述的覆膜支架,其特征在于,所述第二环形节段的短弯曲部与长弯曲部的长度比为1:2 至2:3。
5.根据权利要求1或2所述的覆膜支架,其特征在于,所述第二环形节段(4)的面向外的拱部(10)通过盲片(5)而被延伸朝向所述支架的外部。
6.根据权利要求5所述的覆膜支架,其特征在于,所述盲片(5)的长度为被设置有盲片(5)的所述弯曲部(12)的长度的50%与100%之间。
7.根据权利要求5所述的覆膜支架,其特征在于,所述盲片(5)伸入到所述末端环形节段(3)的谷部(7)中。
8.根据权利要求1或2所述的覆膜支架,其特征在于,所述支架(1)被设置有管形式的膜(2)。
9.根据权利要求8所述的覆膜支架,其特征在于,所述膜(2)包含ePTFE管。
10.根据权利要求1或2所述的覆膜支架,其特征在于,所述支架(1)是气囊可膨胀的支架。
11.根据权利要求10所述的覆膜支架,其被卷曲到气囊导管的气囊上。
12.根据权利要求1或2所述的覆膜支架,其被设置有由形状记忆合金制成的自膨胀支架。
13.根据权利要求12所述的覆膜支架,其中,所述形状记忆合金是镍钛合金。
14.根据权利要求1所述的覆膜支架,其特征在于,所述膜固定在所述环形节段(4)的位于邻近所述末端环形节段(3)的若干弯曲部(12)下面。
15.根据权利要求14所述的覆膜支架,其特征在于,所述膜固定在所述环形节段(4)的位于邻近所述末端环形节段(3)的每个弯曲部(12)或每间隔一个弯曲部(12)下面。
