本发明提供了一种止血用明胶海绵及其植入器,包括推送管以及外鞘,所述外鞘套接在推送管外圈,所述外鞘近端设有推送装置。所述推送装置包括前手柄、后手柄和安全锁,所述前手柄设置在外鞘后端,安全锁设置在外鞘末端,后手柄设置在推送管的末端。所述安全锁包括固定在前手柄末端的旋钮和卡爪,旋钮旋紧,卡爪固定推送管,用于将推送管固定在前手柄上。

一种止血用明胶海绵及其植入器
技术领域
本发明涉及医疗用穿刺止血产品领域,特别是一种止血用明胶海绵及其植入器。
背景技术
明胶海绵符合药典,具有优异的止血特性,是一种无菌、无热原能被人体组织所吸收的止血剂。可任其留在人体内不必取出,自然吸收。主要成分:蛋白含量占95%以上。适用于各种手术止血,具有填塞创口,促进愈合的功能。例如:神经外科、口腔科、耳鼻喉科、妇科等,凡需止血时均适用。使用时,应在严密无菌情况下打开内包装,切成需要的形状,轻轻揉搓后应用,或浸入生理盐水中,轻揉使之湿透后,挤尽液体,敷于出血处,按压待血液凝固为止。具体规格可以按照出血的面积选择。
临床上在实质性脏器穿刺后留下的腔道的止血,如行经皮穿刺胆道造影或支架植入术后,及经皮穿刺门静脉穿刺粒子条植入或支架植入术后,鞘管拔出前,要将可吸收明胶海绵通过鞘管植入穿刺通道进行止血。目前市场上只有通用明胶海绵,医生手术前要将明胶海绵剪成1mm×1mm的小块状,再通过高温高压处理压缩成颗粒状,手术时再将颗粒状海绵塞入鞘管中,通过推送植入器将明胶海绵送到穿刺通道止血,最后拔除鞘管,操作繁琐,需要设计一种预装的明胶海绵及植入器。
发明内容
发明目的:本发明所要解决的技术问题是针对现有技术的不足,提供一种止血用明胶海绵及其植入器。
为了解决上述技术问题,本发明提供了一种止血用明胶海绵植入器,包括推送管以及外鞘,所述外鞘套接在推送管外圈,所述外鞘近端设有推送装置。
本发明中,所述推送装置包括前手柄和后手柄,所述前手柄设置在外鞘后端,后手柄设置在推送管的末端。
本发明中,所述外鞘末端设有安全锁,安全锁包括固定在前手柄末端的旋钮和卡爪,旋钮旋紧,卡爪固定推送管,用于将推送管固定在前手柄上。
本发明中,所述推送装置包括枪式手柄,枪式手柄上设有一按压揿手,按压揿手连接齿轮机构,所述外鞘末端外侧设有套筒,套筒外固定有第一交错齿轮,套筒外设有一个以上的固定在枪式手柄上的滚动轴承,齿轮机构带动第一交错齿轮,所述套筒内侧设有内螺纹,外鞘末端的外侧固定套接一个与套筒内侧内螺纹适配的外鞘齿轮,推送管近端固定在枪式手柄上。
本发明中,所述齿轮机构包括齿条、棘轮、传动齿轮和第二交错齿轮,按压揿手连接齿条,齿条带动棘轮,棘轮带动传动齿轮,传动齿轮带动第二交错齿轮,第二交错齿轮带动第一交错齿轮。
本发明中,所述传动齿轮的中心轴上连接一拨动按钮,按压拨动按钮将传动齿轮与棘轮分开,使得传动齿轮与变向齿轮啮合,变向齿轮与棘轮啮合。在传动过程中增加了变向齿轮,传动方向随之变化,可将外鞘向其远端推动,若明胶海绵释放位置错误,复位拨动按钮,可将传动齿轮与棘轮再次啮合,可将明胶海绵收回。
本发明中,所述外鞘远端外侧设有显影环,方便在影像下看到明胶海绵达到人体的位置。
本发明中,所述外鞘上设有刻度,方便操作者直观了解植入器植入的深度。
本发明还公开了一种明胶海绵的制备方法,步骤如下:
步骤1:取样片状明胶海绵,采用粉碎工艺将片状明胶海绵研磨成颗粒;
步骤2:将颗粒状明胶海绵放入管状模具中,用冲头从两端向中间压制;
步骤3:从管状模具中取出圆柱状明胶海绵。
本发明中,所述步骤2中管状模具直径小于外鞘的直径。使得圆柱状明胶海绵能够植入外鞘内。管状模具表面光滑,质地坚硬,压制后产品容易脱出。
本发明还公开了一种设置在止血用明胶海绵植入器内的止血用明胶海绵,其密度大于1g/cm3。止血效果更好。
有益效果:本植入器采用明胶海绵止血,止血效果优于以往的颗粒状海绵,通过本装置的设计,使得明胶海绵易于植入,方便手术,节省手术时间,减少病人痛苦,并且明胶海绵易于被人体吸收。

下面将结合附图对本发明作详细说明。
实施例:
如图1,本实施例提供的一种止血用明胶海绵植入器,包括推送管1、外鞘2、前手柄3、后手柄4、安全锁5、显影环18和刻度19,外鞘2由聚合物塑料制成,壁薄、强度高、不易弯折,外鞘2套接在推送管1外圈,外鞘上设有刻度19,可以方便地观察到明胶海绵的长度。外鞘2远端设有显影环18,方便在影像下观察植入器植入到人体的位置。前手柄3设置在外鞘后端,后手柄4设置在推送管的后端,安全锁5设置在外鞘末端。
如图2a和2b,为安全锁的结构示意图,安全锁的作用在于将推送管与前手柄锁定,当不需要推送时,锁紧推送管,防止误操作而将明胶海绵推出外鞘,优选结构如图2,前手柄上设有卡爪结构,并带有螺纹,当旋紧旋钮时,旋扭6使卡爪7压紧在推送杆上,从而将前手柄3与推送管固定在一起。
使用时将植入器外鞘插入手术穿刺通道的外鞘内,在透视下,观察植入器外鞘相对手术穿刺通道外鞘的距离,到适当距离时,固定住推送管,将手术穿刺通道外鞘及植入器外鞘一起往人体外移动,由于推送管远端顶住明胶,这时外鞘向近端外移,推送杆将明胶推出植入器外鞘,从而将明胶海绵释放到手术穿刺通道,达到止血的作用。
一种装填在外鞘内的明胶海绵的制备方法,步骤如下:
步骤1:取片状明胶海绵,采用粉碎工艺将片状明胶海绵研磨成颗粒;
步骤2:将颗粒状明胶海绵放入管状模具中,用冲头从两端向中间压制;
步骤3:从管状模具中取出圆柱状明胶海绵。
如将0.1克的海绵压制成直径3mm,长度10mm的圆柱状明胶,结构紧密,单位体积的明胶海绵含有量是普通明胶海绵的120倍以上,所以对于大通道类的创口止血效果将更好。
本发明中,明胶海绵的密度大于1g/cm3。该制备方法制备出来的明胶海绵密度更大,止血效果更好。
实施例2:
如图3,本实施例提供的一种止血用明胶海绵植入器,包括推送管1、外鞘2、枪式手柄8、按压揿手9、套筒10、第一交错齿轮11、滚动轴承12、外鞘齿轮13、齿条14、棘轮15、传动齿轮16、第二交错齿轮17和显影环18,该实施例可以实现单手操作,通过按压揿手9来驱动齿条14,齿条14依次驱动棘轮15、传动齿轮16和第二交错齿轮17,由于有棘轮在,驱动力只能提供一个方向;第二交错齿轮17驱动第一交错齿轮11;外鞘末端外侧设有套筒10,套筒外固定有第一交错齿轮11,套筒外设有一个以上的固定在枪式手柄上的滚动轴承12;第二交错齿轮17驱动第一交错齿轮11,使套筒转动起来,依靠套筒10的转动带动外鞘2移动,带动套筒和外鞘往近端移动,推送管1不动,将外鞘内的明胶海绵顶出。
如需要将外鞘向远端移动,可以拨动按钮,使传动齿轮16的大齿轮与棘轮15啮合脱离,使传动齿轮16的小齿轮与变向齿轮20啮合,变向齿轮20由棘轮15带动,这样,在传动过程中增加了变向齿轮20,传动方向随之改变,外鞘就能向相反方向运动。
优选地,本实施例中,外鞘和套筒可拆卸的连接,方便更换外鞘和明胶海绵。
本发明提供了一种止血用明胶海绵植入器,具体实现该技术方案的方法和途径很多,以上所述仅是本发明的优选实施方式,应当指出,对于本技术领域的普通技术人员来说,在不脱离本发明原理的前提下,还可以做出若干改进和润饰,这些改进和润饰也应视为本发明的保护范围。本实施例中未明确的各组成部分均可用现有技术加以实现。
