本发明公开了一种生物制品的新用途,具体的说,是一种口服的促肝细胞生长素颗粒剂的新用途。
背景技术
全球有20亿人群感染乙肝病毒,其中2.4亿是慢性乙肝病毒表面抗原(HBsAg)携带者。慢性乙肝(CHB)有一部分人是不活跃的,但有一部分人持续HBsAg阳性可导致肝硬化、肝癌。
近年来,应用干扰素及核苷类似物治疗CHB,取得很大进展,大多数患者经过治疗后,血清谷丙转氨酶(ALT)、HBVDNA、HBeAg在外周血可以恢复正常或转为阴性,但外周血中HBsAg仍持续阳性,很难达到血清学转换。这些年来,国内外学者已经提出慢性乙肝要达到临床治愈,即功能性治愈的概念,也就是说,治疗慢性乙肝除了要求ALT、HBVDNA、HBeAg长期稳定正常以外,尚要求外周血HBsAg阴性,并出现乙型肝炎表面抗体(抗-HBs),即血清学由HBsAg阳性转换为阴性,其滴度<0.03IU/ml并出现抗-HBs阳转(>10mIU/ml)这也是对慢性乙肝治疗的终点要求或目标。达到这个目标,就是CHB功能性治愈。
遗憾的是,目前对于促使HBsAg血清学转换的药物和方法非常缺乏。
发明内容
针对上述的不足,本发明的目的在于提供一种口服的促肝细胞生长素颗粒剂的新用途,能够促进慢性乙肝患者HBsAg血清学转换,达到功能性治愈。
为了实现上述技术效果,本发明提供的技术方案是这样的:一种促肝细胞生长素颗粒剂的新用途,所述的促肝细胞生长素颗粒剂作为慢性乙型肝炎功能型治愈的药物新用途.
进一步的,所述的促肝细胞生长素颗粒剂作为促进乙型肝炎表面抗原血清学转换的药物新用途。
进一步的,所述的肝细胞生长素颗粒剂适用于慢性乙肝非活动性HBsAg携带者,其外周血HBsAg滴度低于1000IU/ml。
进一步的,所述的每天每次剂量在50mg~300mg。
1.历史回顾
目前,CHB主要是应用核苷类似物和干扰素-α治疗,但上述药物仍存在一些弱点,它仅是抑制乙肝病毒复制,不可能从肝细胞核内彻底清除cccDNA,因此必须长期服药,且停药后容易病情复发。近年来,学者们从理论和实践已认识到治疗慢性乙肝必须在抗病毒药物的基础上联合免疫调节药物,才有可能促使慢性乙肝达到功能性治愈的目标。但如何筛选免疫调节药物呢?我们认为pHGF就是最佳选择药物之一。
2.对促肝细胞生长素颗粒剂的再认识及在治疗慢性乙肝上的重新定位
用科学发展的观点,加深对pHGF的再认知,pHGF是从哺乳类动物肝组织匀浆后经超滤获得的小分子多肽类物质,至少有20多种组分。1975年,美国学者Labregue等通过大鼠再生肝组织分离纯化获得具有刺激肝细胞生长的物质,命名为肝细胞刺激物质(HSS)。Nakamura同时也从肝切除大鼠血浆中分离高纯化的促进DNA合成的物质命名为肝细胞生长因子(HGF)。我国科学家促肝细胞生长素发明单位,第一发明人张宜俊等,根据Labregue等提出的提取HSS和HGF的三原则,即HGF在哺乳类动物没有种属特异性;但有器官特异性和动物年龄依赖性。创造性地从乳猪肝脏提取了肝细胞生长因子(HGF),从而应用于人体获得成功。肝脏提取物为小分子多肽物质,根据当时国家药监局药审办评审专家的建议,命名为促肝细胞生长素(pHGF)。当时对pHGF的治疗作用的认识主要是促进肝细胞再生,恢复肝细胞功能。
pHGF的问世,在治疗肝病方面发挥了相当卓著的临床效果,挽救了成千上万的肝病患者。在当时历史条件下,由于科学水平的局限,pHGF的治疗作用多注重在促进肝细胞再生方面的研究,而对pHGF的免疫调节作用尚处于初级认识阶段。
近年来,对哺乳类动物肝脏组织的成分研究已有长足的进步。肝脏组织是由70~80%的肝实质细胞和20~30%的非实质细胞组成的。非实质细胞是由枯否氏细胞(KC)、窦状内皮细胞(LSEC)、肝脏星状细胞(HSC)、树突状细胞(DC)、自然杀伤细胞(NK)、T细胞等组成。枯否氏细胞是长期居住在肝窦内壁的巨噬细胞,占体内所有组织巨噬细胞80%,现已证明,非实质细胞具有免疫抗原提呈细胞功能。KC、DC、NK、T细胞等均参与先天固有免疫及后天获得性免疫的应答。肝脏非实质细胞在抗感染、调节免疫功能方面发挥非常重要的作用。
pHGf是多种小分子多肽物质,除了参与促进肝细胞再生、代谢、解毒、吞噬异物功能外,更重要的是参与Toll样受体介导的先天固有免疫应答从而调控乙型肝炎病毒(HBV)复制。
Toll样受体(Toll Like Receptors,TLRs)是参与非特异性免疫(先天固有免疫)和联系特异性免疫(获得性免疫)的桥梁,在哺乳类动物和人类至少已发现TLRs 11个,分布在20多种细胞中,在肝脏非实质细胞如DC、KC、NK、T细胞等均有TLRs表达。TLRs是先天固有免疫的眼睛,监视和识别各种不同疾病的相关分子模式(PAMP),产生各种细胞免疫应答。
Jun Wu等报导(hepatology 2007.12),从C57BL/6小鼠肝脏分离非实质细胞包括KC、LESCs等,应用TLR1-TLR9激活剂刺激这些非实质性细胞,结果表明TLR3诱生大量的干扰素-β,TLR4产生某些未知的因子,干扰素-β和未知因子能够强有力地抑制HBV复制,这种抑制作用是通过肝脏局部的先天固有免疫系统而体现的。作者认为,发展以TLR为基础的抗HBV策略将是抗HBV治疗的新亮点。其后有多位学者均证明TLRs参与抗HBV复制的免疫应答。
我们最近的实验也发现从乳猪肝脏提取的pHGF制剂中含有与TLRs激活剂相关的小分子多肽物质,应用C57BL/6小鼠乙肝模型实验也证明口服pHGF有促使HBsAg血清学转换作用。

下面结合具体实施方式,对本发明的权利要求做进一步的详细说明,但不构成对本发明的任何限制,任何在本发明权利要求保护范围内所做的有限次的修改,仍在本发明的权利要求保护范围之内。
实施例1
1.1促肝细胞生长素颗粒剂(原液)的生物利用度试验:
大白鼠实验表明:口服促肝细胞生长素原液后,大鼠血液中测到的相对生物利用度为30.7%。提示PHGF原液经口服后吸收良好。为了达到一定的有效性,必须适当加大剂量。
1.2PHGF原液在不同的PH2~9的稳定性。实验表明PHGF原液在PH2~9人工胃液中消化60~120分钟。仍保持良好的生物学活性。
1.3口服PHGF后,外周血中可检测到与PHGF的高效液相色谱相似的色谱行为物质。
实施例2
1.水动力法建立小鼠乙肝模型
1.1小鼠乙肝模型的建立
应用PAAV-1.2-HBV-A质粒(中国台湾陈培哲教授惠赠),选用体重20-25克C57BL/6小鼠,将PAAV质粒15μg溶于小鼠体重10%的生理盐水中。于5-8秒内经鼠尾静脉注射(高速、高压),然后通过心脏转染在肝细胞内,建立乙型肝炎感染模型。其后每周检测HBV-DNA、HBeAg、HBsAg动态变化,确定为HBV转染模型后,于注射第6周时,筛选小鼠血清HBsAg阳性者作为实验小鼠。第6周时,HBsAg阳性鼠(HBsAg滴度>0.04IU/ml)约占实验鼠60-80%,其中HBsAg呈现不同滴度分布。可分为高滴度(>250IU/ml)、中滴度(20.0-249.01U/ml)、低滴度(0.04-19.9IL/ml)。以上三种不同HBsAg滴度的模型鼠,从第6周至14周每周检测HBsAg以及抗-HBs产生的动态变化。以观察其自然恢复过程。

1.2分析和讨论
1.2.1水动力法建立的C57BL/6小鼠乙肝模型,在转染第6周,HBsAg可呈现不同的滴度分布。我们将其分为三种层次,>250IU/ml以上,占全部HBsAg阳性模型鼠中24.0%,其后至转染第14周时,HBsAg滴度逐渐下降,平均滴度已下降为73.7±38.0,在此期间,抗-HBs的产生(指抗-HBs阳性)的例数为0,说明尚未产生保护性抗体;在HBsAg中滴度的模型鼠,第6周时HBsAg平均滴度为91.9±80.0,至第14周时已下降至30.2±30.0,在49例中也未发现自然HBsAg阴转例数,抗-HBs仅出现1例阳转。相反,低HBsAg滴度的27只模型鼠中,至第14周时,已有11只(25.9%)自然转为HBsAg阴性,平均HBsAg滴度下降至3.7±5.8IU/ml,而且抗-HBs阳转率高达63.0%,抗-HBs滴度平均上升至394.1±220.5mIU/ml,提示在转染第6周时,HBsAg在20.0IU/ml以下的模型鼠,由于其在第14周内HBsAg自然阴转率很高,故不适宜作为研究HBsAg血清学转换的模型鼠。
1.2.2水动力法建立的C57BL/6乙肝模型鼠,于转染第6周时,HBsAg滴度在20.0IU/ml以上者,可作为研究筛选HBsAg血清学转换药物的模型鼠,但应该注意的是,慢性乙肝病程可分为免疫耐受期、免疫清除期和非活动性HBsAg携带期。从本次实验结果来看,我们建立的C57BL/6小鼠乙肝模型,它不属于慢性乙肝免疫耐受期和免疫清除期,而比较符合慢性乙肝非活动性HBsAg携带期。从理论上分析,人类慢性乙肝的判断标准,其病程要超过6个月以上,从模型鼠考虑,由于小鼠的平均寿命和人类差异很大,要筛选达到6个月以上的HBsAg持续阳性模型鼠,不仅数量很少,而且鼠龄逐渐老化,不一定能等同于人类的6个月以上的慢性乙肝模型,我们认为,在应用水动力法乙肝模型鼠时,只要淘汰或弃用在实验期(共14周内)可能自然阴转,指HBsAg滴度在20.0IU/ml以下的模型鼠,也即选择大于20.0IU/ml以上者,对于研究HBsAg血清学转换的药物是比较合适和可行的。美国著名肝病研究专家Chisary,于2003年报道水动力法建立的鼠模型确定为急性乙型肝炎模型,但其观察时间仅二周,我们建立的乙肝鼠模型长达14周,已经观察到单项HBsAg持续阳性的状况,故比较适合于筛选非活动性HBsAg携带者治疗药物的研究。
实施例3
口服PHGF(原液)对C57BL/6小鼠乙肝模型的HBsAg表达的影响。
按实施例2的方法建立水动力法C57BL/6小鼠乙肝模型,筛选其HBsAg滴度在20IU/ml~>250IU/ml者共70只,按其HBsAg滴度高低随机平均分为7组,每组均为10只,(各组间HBsAg滴度均有可比性)。其中口服PHGF原液者4组,其剂量分别为1.2mg/只、2.4mg/只、4.8mg/只、7.2mg/只,每天经灌胃一次;2个阳性对照组:注射用PHGF4.8mg/只,腹腔注射,每天一次;重组人白细胞介素-12,150ng/只,皮下注射,2/周;阴性对照组:灌胃(口服) 生理盐水0.2ml,以上各组疗程均为10周,疗程结束后停药观察6周。总共观察16周,治疗前治疗后(观察期)每周验血:HBsAg、抗-HBs、停药后及结束观察时增加检测IFN-γ、及外周血CD4+CXCR5+细胞以及脾脏CD4+CXCR5+细胞及IL-21水平。
实验结果见表4:

说明:HBsAg阴转,指HBsAg滴度<0.03IU/ml
抗-HBs阳转,指抗HBs滴度>10mIU/ml
分析:口服PHGF4.8mg/只组、7.2mg/只组,于治疗后10周时,实验组模型鼠HBsAg均出现HBsAg阴转率分别为30.0%和40.0%。IL-12 150mg组HBsAg阴转率也为30.0%。这三个治疗组与生理盐水对照组比较,P<0.05。同时,这三个治疗组的抗-HBs阳转率也同时显著高于生理盐水对照组,P<0.05。提示每天口服PHGF4.8mg/只~7.2mg/只,以及IL-12 150mg皮下注射(2周),疗程10周后,均可促使C57BL/6小鼠乙肝模型的HBsAg出现血清学转换,这是令人惊喜的现象。这三个治疗组,停药后观察6周,HBsAg阴转率仍继续保持稳定,而且抗-HBs继续稳定上升趋势。
实施例4
口服PHGF(原液)对促进外周血CD4+CXCR5+滤泡辅助性T细胞的表达,及与C57BL/6乙肝模型鼠HBsAg血清学转换的关系。
1.研究目的
滤泡辅助性T细胞(follicular helper T cells,Tfh)是一种新发现的辅助性T细胞亚群,存在于淋巴结滤泡,表达CXCR5、PD-1、ICOS、Bcl-6、IL-21等表面分子。Tfh在B细胞分化过程中起着重要作用,具有辅助B细胞成熟、增殖的功能,直接辅助抗体反应,调节体液免疫反应。外周血CD4+CXCR5+Tfh细胞被认为是外周记忆性Tfh细胞,具有与Tfh细胞相似的功能特性。Tfh细胞及其分泌的IL-21参与HBV感染后的免疫应答,在病程中发挥重要作用。本实施例建立小鼠乙肝模型,应用口服PHGF(原液)对促进HBsAg阴转及抗-HBs产生,探讨Tfh细胞表达及产生IL-21与HBsAg的关系。
在4个口服PHGF治疗组经过10周治疗后发生HBsAg阴转者8例,而生理盐水组无1例阴转。将上述已HBsAg阴转及生理盐水对照组未阴转各分别为8例,于第16周时,取其外周血PBMC及脾脏分离的脾细胞分别进行CD4+CXCR5T细胞比例检测。
2.方法
流式细胞术检测PBMC和脾细胞中CD4+CXCR5T细胞的频率,计数后将细胞转移至流式管中,保持细胞浓度为1×105/管。使用Alexa647标记的CXCR5、Alexa488标记的CXCR5、FITC标记的ICOS、PerCP标记的CD4、PE标记的PD-1和APC标记的CD40L等流式细胞术抗体及与之对应的同型对照抗体,严格按照说明进行PBMC染色。使用BD FACSCalibur流式细胞仪对处理好的标本进行检测,每份标本至少获取50000个细胞进行分析。用FlowJo 7.6软件(Tree Star Inc.)对获得的数据进行分析。
3.初步结果
流式细胞术检测小鼠脾细胞CD3+CD4+细胞中CXCR5+细胞表达率见表5及图1:

4.初步结论
由图1可知,HBsAg已阴转小鼠的Tfh表达率高于未转阴小鼠,P<0.05。
口服PHGF在增加外周血PBMC及脾细胞中CD4+CXCR5+Tfh的表达可能在促使HBsAg血清学转换及阐明其作用机理方面具有关键的作用。
5.讨论
HBV感染人体后,宿主发病与否以及病情的严重程度常常与机体免疫力有关,无论天然免疫应答还是特异性免疫应答都不同程度的参与机体对HBV的清除,细胞毒性T淋巴细胞是负责清除HBV的主要承担者,通过破坏受感染肝细胞而清除细胞内病毒。然而B细胞介导的特异性体液免疫反应也发挥着重要的作用,通过分泌大量的免疫球蛋白,以中和作用的方式清除循环血液中的HBV病毒。
本研究中,口服PHGF治疗组与生理盐水相比HBsAg显著下降,可能通过细胞免疫和体液免疫两方面实现。在细胞免疫方面,口服PHGF后可以通过胃肠道血管直接抵达肝脏,对肝窦内一些免疫细胞启动免疫应答促进大量的内源性IFN-γ的产生,并进入肝细胞等感染了乙肝病毒的细胞内阻止HBV的复制和再生产过程,形成细胞内非溶细胞性病毒清除模式,清除细胞内的HBV DNA及HBsAg;再者,在体液免疫方面,活化的CD4+T细胞分泌IL-4、IFN-γ,表达上调CXCR5;CXCR5是介导T细胞进入淋巴滤泡的重要分子,但对于大多数T细胞,CXCR5的表达只是一过性的;之后,部分CD4+CXCR5+T细胞细胞通过具有高亲和力的TCR与B细胞相互作用而获得一个额外信号,维持CXCR5的持续性表达,促进细胞进入淋巴滤泡GC,最终分化为Tfh;分化成熟的Tfh通过表达CD40L、Ices与GC内B细胞上的CD40、ICOSL相互结合,促进B细胞分化,成熟的B细胞部分继续留在GC形成记忆性B细胞,部分离开GC而分化为浆细胞,分泌产生大量的抗体;此外,PHGF可能通过记忆性CD4+T细胞调节,增强IL-21的分泌,而IL-21分泌辅助B淋巴细胞产生抗体,调节特异性体液免疫反应。特别是中和HBsAg促使HBsAg血清学转换。
综上所述,PHGF可有效地启动特异性和非特异性免疫反应,增强Tfh,可能增加IL-21分泌促进特异性体液免疫,有助于CHB患者出现病毒学应答,有利于宿主清除HBV。

1.一种促肝细胞生长素颗粒剂的新用途,其特征在于,所述的促肝细胞生长素颗粒剂作为慢性乙型肝炎功能型治愈的药物新用途。
2.根据权利要求1所述的一种促肝细胞生长素颗粒剂的新用途,其特征在于,所述的促肝细胞生长素颗粒剂作为促进乙型肝炎表面抗原血清学转换的药物新用途。
3.根据权利要求1所述的一种促肝细胞生长素颗粒剂的新用途,其特征在于,所述的肝细胞生长素颗粒剂适用于慢性乙肝非活动性HBsAg携带者,其外周血HBsAg滴度低于1000IU/ml。
4.根据权利要求1所述的一种促肝细胞生长素颗粒剂的新用途,其特征在于,所述的每天每次剂量在50mg~300mg。
