Description
本发明涉及一种根据权利要求1前序部分所述的用于固定的装置。所述“固定”在医学领域尤其理解为对人体或人体部分(腿,臂)的稳固、固持、压紧和/或定位,其中尤其还应将医学目标装置(Zielvorrichtung)、标记件和/或用于图像辅助的、微创手术方法的外科仪器可靠地固定在病人身上。
在人类医学或者医学研究的众多领域中,需要对病人或者病人的身体部分进行可靠的固定/稳固/固持或定位或者进行装置或设备的(机械)安装。这尤其对于诊断的和治疗的放射学、放疗的领域或者对于手术/外科干预措施(例如神经外科)以及对于手术前或手术后的护理都有很重要的意义,
由于计算机科技在诊断学和治疗学中的应用,对于精确度和可重复性的要求不仅在将立体定位框架系统稳固在人身上时而且在固定人体本身时都有所提高。在应用时的舒适性和速度、移动性和成本在此扮演很重要的角色,其中应该尽量避免侵入式固定(通过螺栓固紧等实现)。
对于这种所谓的非侵入式固定类型在现有技术中是已知的:
a)利用绷带或者绑带固定身体:
在这种情况下病人的身体躺在泡沫塑料垫上并且横向地通过压在身体上的绷带将病人固定在垫子上。这种设计的缺点在于,通过绷带的较大的拉力(尤其在压紧的绷带或皮带的边缘处的不均匀的压力分布)会导致受压点、滑移和/或皮肤肿痛。
b)通过模板的固定:
在这种情况下,病人躺在一种“气垫”上,该气垫被泡沫小球所填充。通过吸走在这种气垫内的空气使得这种气垫通过泡沫小球的相互挤压实现固定。在此,首先在第一步骤中调整真空气垫,并且随后在第二步骤中进行进一步抽吸。这种设计的缺点在于,通常使用的“气垫”虽然可实现静止,但是不能保证精确的固定。对于不配合的病人实际上不能实现足够的静止。此外,通过形成褶皱或者太大的压力同样地会总是造成受压点,这尤其对于被麻醉的病人会造成组织损伤。
c)真空固定系统:
在真空固定系统中对身体施加真空。通过抽吸在病人表面上的空气能够提供一种良好的固定。这种设计的缺点是,真空泵必须一直运行。此外,真空系统相对复杂,因此不容易被操作并且不易被运输。在要求较高程度的无菌处理甚至要求无菌环境的干预措施中,真空泵的空气流会造成风险(病菌传播)。但是,很难形成较高的固定力,因为通过长时间持续的压力会造成损伤(例如轻微出血、血瘀等)。当真空不足时,所述固定立即失去,因而会对病人形成危险或者必须中断或重新进行(外科的/放射学的)干预措施。
其它的现有技术,例如夹板、塑料按压、石膏处理等都具有相似的缺点。此外,这些方法还会造成较高的经济上或时间上的耗费,因此只适合用于长期使用的情况。
因此,本发明所要解决的技术问题是,提供一种用于固定的装置,该装置能克服上述缺点,在结构上是简单的以及在应用时使得病人在很大程度上感到舒适。此外,这种装置应该能够实现在校准点(所谓的标记点)和/或目标装置和/或其它医学辅助器件(例如仪表固持器/内窥镜固持器、MR线圈等)上的精确安装,并且在需要时能够实现对手术区域的理想的可接近性。
所述技术问题通过具有权利要求1的特征的装置所解决。有利的设计方案由从属权利要求给出。
在此,一个基本的特征是强化(或增强)的无纺织物,因为无纺纤维与微搭扣交织,利用这种无纺织物能够在人身体上实现稳定的固定。例如通过拉伸或者在预先伸展的情况下可控地缠绕在辊子上的强化无纺织物具有限定的抗拉强度,并且自身用作对于微搭扣的配对元件。因此避免了在其它情况下经常使用的具有钩件和配对件的搭扣连接的双重设计,因为无纺织物可牢靠地被钩住。但是,无纺织条的边缘区域与皮带相比是非常柔软的,因而能够避免对病人的皮肤的损伤。根据身体区域或多或少(横向)弹性的无纺织物与病人的外形理想地匹配,因此确保了理想的压力分布并且避免了受压点。因此,病人身体能够关于检查台或者手术台而被固定,其中,通过较轻的压力实现对身体较轻的、无损的按压。作为副作用还能够在操作之前、中和之后利用所述按压,方法是通过在无纺物和伤口/手术区域之间例如设置伤口敷布和由此形成伤口按压。与具有缝上的搭扣连接件的皮带或者绷带相比,在此不再需要两个共同作用的搭扣件;相反,强化的无纺织物起到连接件的作用,微搭扣的钩件或者蘑菇状件咬合(可再次拆卸)在该无纺织物中。因此,用于固定人体或身体部分的这种装置的成本是非常低的,因为实际上只有商业上通用的价格低廉的容易得到的无纺织物才会形成耗材。这能够以医用纱带或纱布方式容易地应用,其中也满足卫生要求。必要时(例如与伤口/手术区域直接接触),无纺物还能够被无菌处理。
所使用的微搭扣优选设计为具有蘑菇状件和/或钩件的柔性薄膜,该蘑菇状件和/或钩件作为与无纺织物的连接件。在这种情况下,这种薄膜例如能够作为条带利用它背向钩件的一侧粘着在手术台的侧面上,以便由此用作稳定的锚固。这种微搭扣优选由聚丙烯(PP)或者聚酰胺(PA)制成,以便以简单的方式进行清洗。微搭扣的钩件/蘑菇状件相对较小(亚毫米范围),因而在使用清洗液时会形成毛细效应并且因此会对微搭扣进行完全的消毒。通常的搭扣带或者钩带不具有这样的性质;它们因此非常难以清洗或消毒,并且在医院里、尤其在具有较高卫生要求的手术中或者无菌区域中只能很有限地使用。固定件的另外的元件、即无纺织物主要被设计为(无菌的)一次性用品,从而它在与皮肤接触后类似于医用纱带一样可被容易地处理。
无纺织物主要设计为条式的并且能够为了提供作为对身体部分的手术位置的入口(例如穿刺口)而具有通孔或者规则的冲裁口(例如具有1cm直径,在纵向上分别间隔例如10cm)。因此,该无纺织物能够铺设在预设的手术位置上,并且随后在两侧被拉紧。该无纺织物备选地能够具有狭长的槽,该槽的长度比无纺织物的跨越人体的部分更长,并且它的宽度是足够在希望的所跨越的身体部分上进行手术。在这种情况下,无纺织物的用于固定的面不会被所述槽减少,由此不会妨碍在微搭扣上的固持性质。在此,无纺织物优选能够被辊子卷绕以及优选能够被单独切割(例如使用剪刀剪断等)。但是,在无纺织物中例如每米或者在比在微搭扣之间稍微更大的间距(稍微长于床宽)上还能够设置作为额定分隔位置的穿孔。无纺带还能够重叠或者在多个位置上相叠设置;在这种情况下,每条无纺条的机械拉力能够被“单独”调整,由此不同的身体区域能够被不同强度地压缩和固定。多条重叠的和/或相叠设置的无纺条的应用还是一种安全设计,因为由此能够保证通过松开一条或多条带不会使得对病人的整体固定失效。除了条式的设计之外,还能够考虑无纺织物的巾式(tuchartig)或者背心式(westenartig)设计;优点是更快和更简单的操作。
强化的无纺织物通常设计成在拉紧方向或(条带)纵向上抗拉和在横向方向(条宽度)弹性,以便完美地贴靠在病人身体上。无纺织物强化能够以不同的方法进行,例如在WIKIPEDIA中在“Vliesstoffe”下被详细阐述的那样,尤其作为所谓的针刺无纺织物。但是,明显可知,无纺织物是透气性好的并且因此使病人感到舒适。此外,在无纺织物中还能够集成所谓的标记件和/或增强件。
如前面所述,微搭扣能够固定在检查台或手术台的边缘区域上,尤其在型材中沿着边缘能移动或者粘接在检查台或手术台上。在此,能够使用标准型材,例如在手术台、病床、CT台或MRT台等上已经存在的型材。此外,微搭扣能够设计为具有胶带的补丁的样式,以及还能够简单地粘接或以其它方式安置在支承配件(头部壳、真空枕等)上。此外,在固定件上还能够固定辅助设备、尤其标记件或者目标装置,优选借助至少一个转接板进行,该转接板同样能够支承搭扣连接件或者粘接层。因此,无纺织物的外侧或上侧还能够用来锚固。
在进行固定和无菌遮盖之后能够例如在手术口内进行手术操作(必要时使用机器人辅助设备)。在这种情况下,能够可重复的设置上述用于成像方法或者手术导航的标记件。因为无纺织物是一种大面积的、人体工程学的固定件,所以在满足最大程度舒适度的情况下形成较高的固持力。当在该无纺织物上在朝向病人的接触区域中设置适于皮肤的喷胶剂或者薄的双面胶粘带时,还能够提升上述固持力。由此还能够固定柔软组织,以便例如使得该柔软组织远离辐照区域。
结合附图来阐述实施例。在此附图为:
图1示出用于固定的装置的安装;
图2示出该装置的放大细节图;和
图3示出附加装置的安放/安装。
用于固定人体或身体部分的装置由至少一个固定件1组成,该固定件能够定位(图1)在身体表面上(用阴影线表示;在此例如是背部的横截面,该背部贴靠在软垫4b上)。该固定件利用强化的无纺织物2实现,该无纺织物贴靠在皮肤上,以便随后在软垫4b的左侧和右侧被分别固定在微搭扣3上(通过钩住或抓住在微米范围内的钩件;如前面所述)。无纺织物2优选设计为条状并且尤其缠绕在辊子6上。无纺织物2在它的安装方向L上(主要横向于或垂直于身体纵向轴线)被拉紧,并且在它的横向方向Q上设计为相对弹性的。能够将转接板5a安装在这样固定的固定件1的外侧上,以便在此使标记件5(或者其它的附加装置)稳定地和可重复地锚固(参见图3)。为了固定在检查台或者手术台4的基板上,优选设置至少一个型材4a,例如倒圆的槽。还能够相互间隔地安置更多的固定件1,例如安置在背部上。随后由此空出相应的手术区域。
在图2中示出在无纺织物2和微搭扣3之间的连接的放大视图,其中,在微米范围内的较小的、蘑菇状或钩形的连接件3a(在此表示为T形)与无纺织物2的纤维多重咬合。微搭扣3在此优选以粘接带样式设在下侧(朝向病人),以便粘接在手术台4上(参见图1右侧)。同样地,根据待固定的身体部分的位置,即例如在肩膀区域内和/或在腰部区域内和/或在腿部区域内等,这种“补丁”还能够粘接在软垫4b的侧面上。无纺织物2的纤维尽量地沿着纵向方向L延伸并且因此形成较高的抗拉强度。在右侧上,在微搭扣3的外部(即朝向病人)还示出了粘合剂层7,它优选作为喷粘剂刚好在固定之前被涂覆。因此,粘着力被进一步提升或者柔软区域限定地远离手术区域(参见在图3中的入口2a)。
在图3中的横截面所示,能够在条状的无纺织物2中设置开口2a(例如用于在腰椎区域内的穿刺术)。因此,操作医生或者操作者能够实际上根据需要地固定每个身体区域(全部身体或者只是身体部分)。在此,给出一种对手术台的广泛的适用性和对外科手术仪器和标记件的简单的固定。附加地还可以考虑无纺织物2的垫子(提高舒适性),其中,无纺织物2与通常的带或者绑带相比能够实现通气,即在长时间使用的情况下不会引起对皮肤的刺激。总体上,由此利用限定的压力和非常低的成本固定了身体部分,其中,所建议的装置还可以与申请人发明的基于粘着力的装置(参见DE 20 2011 005 573)相结合。微搭扣3与无纺织物2共同作用的原理还可用来固定上述转接板5a,如图3所示。在此,转接板5a的下侧(同样设有微搭扣)可锚固在张紧的无纺织物2的外侧上。无纺织物2还能够被远离地导引越过上述的转接板5a,以便仍附加地固定该转接板。这种设计还适用于其它的医学附加装置,如MR线圈或者用于检测病人的(呼吸)运动的压力传感器,通过无纺织物2将它们牢靠地固定在身体附近(像第二皮肤一样)。
Claims (11)
1.一种装置,用于固定人体或身体部分,尤其用于安装医学目标装置、标记件或者安装用于成像辅助的、微创手术的外科手术仪器,所述装置具有至少一个固定件(1),所述固定件能够定位在身体表面上,
其特征在于,
所述固定件(1)由强化的无纺织物(2)构成,所述固定件借助微搭扣(3)能够被拉紧。
2.根据权利要求1所述的装置,其特征在于,所述微搭扣(3)设计为具有蘑菇状件和/或钩件(3a)的柔性薄膜,所述蘑菇状件和/或钩件(3a)作为与无纺织物(2)的连接件。
3.根据权利要求1或2所述的装置,其特征在于,所述微搭扣(3)由聚丙烯(PP)或者聚酰胺(PA)制成。
4.根据权利要求1至3之一所述的装置,其特征在于,所述无纺织物(2)设计为条式或巾式的,所述无纺织物尤其具有朝向身体部分的至少一个入口(2a)。
5.根据权利要求1至4之一所述的装置,其特征在于,所述无纺织物(2)能够被辊子(6)卷绕,以及优选在穿孔处能被切断。
6.根据权利要求1至5之一所述的装置,其特征在于,所述无纺织物(2)在纵向(L)上设计为抗拉的和在横向(Q)上设计为弹性的。
7.根据权利要求1至6之一所述的装置,其特征在于,所述无纺织物(2)设计为无菌的一次性用品。
8.根据权利要求1至7之一所述的装置,其特征在于,在所述无纺织物(2)中集成标记件和/或增强件。
9.根据权利要求1至8之一所述的装置,其特征在于,微搭扣(3)被固定在检查台或者手术台(4)的边缘区域上,尤其能够沿着边缘在型材(4a)中移动,或者被粘接在所述台(4)和/或病床(4b)上或者所述台(4)和/或病床(4b)处。
10.根据权利要求1至9之一所述的装置,其特征在于,在固定件(1)上能够固定至少一个辅助设备、尤其目标装置(5)或者用于成像的部件,优选借助至少一个转接板(5a)进行固定,所述转接板同样带有搭扣连接件(3a)。
11.根据权利要求1至10之一所述的装置,其特征在于,在无纺织物(2)上施涂粘接层(7),尤其以适于皮肤的喷粘剂或者薄的双面胶粘带的形式。
