Abstract
本发明公开一种治疗IgA肾病合并高尿酸血症的中药复方制剂及其制备方法和用途。所述中药复方制剂由水牛角25‑35份、干地黄10‑20份、赤芍8‑16份、牡丹皮6‑14份、紫草25‑35份、苍术8‑16份、黄柏8‑16份、川牛膝8‑16份、薏苡仁25‑35份、土茯苓25‑35份、萆薢8‑16份、车前子8‑16份、白芥子8‑16份、大黄3‑9份、白术10‑20份和山楂8‑16份组成。本发明组分均采用中药成分,药效温和不刺激,具有清热凉血、除湿泄浊的功效,可以强化IgA肾病的治疗,有效地降低蛋白尿、降尿酸、降血脂从而修复肾功能,治疗IgA肾病合并高尿酸血症,服用安全,疗效可靠且高尿酸血症不易复发。

技术领域
本发明涉及中药制剂领域。更具体地,涉及一种治疗IgA肾病合并高尿酸血症的中药复方制剂及其制备方法和用途。
背景技术
IgA肾病(IgA Nephropathy,IgAN)是最为常见的一种原发性肾小球疾病,是指肾小球系膜区以IgA或IgA沉积为主,伴或不伴有其他免疫球蛋白在肾小球系膜区沉积的原发性肾小球病。
目前IgA肾病是我国乃至全球慢性肾脏病(CKD)最为常见的类型,约占原发性肾小球疾病的40%-45%,并仍有不断上升的趋势。IgA肾病主要累及青壮年,同时IgA肾病又是一种进展性疾病,约20%-50%的患者在10-20年逐渐发展为终末期肾脏病(ESRD),给社会和家庭带来极大的负担。
回顾性研究表明,我国IgA肾病患者中高尿酸血症的患病率为36.6%,其中慢性肾脏病1至5期各期患者中,高尿酸血症的患病率分别为16.2%、37.4%、66.4%、87.7%和76.4%。高尿酸血症是影响IgA肾病预后的独立危险因素,特别是在不同肾功能分期的IgA肾病患者中,高尿酸血症均显著降低其肾脏累积存活率。一项10年的随访研究发现IgA肾病合并高尿酸血症,其预后不良的可能性是血尿酸水平正常IgA肾病患者的2.4倍。
目前尚无治疗IgA肾病合并高尿酸血症的确切药物,相关研究表明氯沙坦可用于治疗IgA肾病合并高尿酸血症,可以保护肾功能,降低蛋白尿、降低尿酸。但氯沙坦作为一种血管紧张素Ⅱ受体阻滞剂降蛋白尿、降尿酸的作用有限,停药后高尿酸血症复发率高。其他降尿酸的药物,如别嘌醇和苯溴马隆,不能够降低蛋白尿和保护肾功能。
因此,需要提供一种治疗IgA肾病合并高尿酸血症的中药复方制剂。
发明内容
本发明的第一个目的在于提供一种治疗IgA肾病合并高尿酸血症的中药复方制剂。中医理论认为IgA肾病与血热关系密切,治疗多以清热凉血为主;同时还认为高尿酸血症由过食肥甘、痰湿中阻、积浊于内、血行不畅而发病,治疗多以清热利湿、健脾化湿、利水渗湿、泄浊逐瘀为主。本发明为源头创新,国内首创,从祖国医药宝库中,筛选出补肾泄浊、健脾益气、凉血化瘀和扶正培本的天然食用植物药,制成中药复方制剂,其具有清热凉血、除湿泄浊之功效,可以有效地降低蛋白尿、降尿酸、降血脂从而修复肾功能,对治疗IgA肾病合并高尿酸血症效果良好。
本发明的第二目的在于提供一种治疗IgA肾病合并高尿酸血症的中药复方制剂的制备方法。本发明采用浸泡、煎煮等物理方法对组合物中有效成分提取利用,具有制备方法简便、药物见效快等优点。
本发明的第三目的在于提供一种治疗IgA肾病合并高尿酸血症的中药复方制剂的应用。该中药制剂立足于标本兼治的原则,对治疗IgA肾病合并高尿酸血症效果良好,同时不产生副作用,进而从根本上改善患者体质、提高生活质量。
为达到上述目的,本发明采用下述技术方案:
一种治疗IgA肾病合并高尿酸血症的中药复方制剂,所述中药复方制剂由按照重量份数计的以下原料制成:水牛角25-35份、干地黄10-20份、赤芍8-16份、牡丹皮6-14份、紫草25-35份、苍术8-16份、黄柏8-16份、川牛膝8-16份、薏苡仁25-35份、土茯苓25-35份、萆薢8-16份、车前子8-16份、白芥子8-16份、大黄3-9份、白术10-20份和山楂8-16份。
水牛角,为牛科动物水牛的角;味苦,性寒;归心经、肝经;具有清热、凉血、定惊、解毒之功效;主治温病高热、神昏谵语、发斑发疹、吐血衄血、惊风癫狂等症。
本发明中,所述水牛角典型但非限制性的含量为25份、26份、27份、28份、29份、30份、31份、32份、33份、34份或35份。
干地黄,为玄参科植物地黄的根茎;味甘,性寒;归心经、肝经、肾经;具有滋阴养血之功效;主治阴虚发热、月经不调、胎动不安等症。
本发明中,所述干地黄典型但非限制性的含量为10份、11份、12份、13份、14份、15份、16份、17份、18份、19份或20份。
赤芍,为毛茛科植物赤芍或川赤芍的干燥根;味苦,性微寒;归肝经;具有清热凉血、活血祛瘀之功效;主治热入营血、温毒发斑、吐血衄血、目赤肿痛,肝郁胁痛、经闭痛经、痈肿疮疡等症。
本发明中,所述赤芍典型但非限制性的含量为8份、9份、10份、11份、12份、13份、14份、15份或16份。
牡丹皮,为毛茛科植物牡丹干燥根皮;味苦、辛,性微寒,归心经、肝经、肾经;具有清热凉血、活血化瘀、退虚热之功效;主治热入营血、温毒发斑、吐血衄血、夜热早凉、无汗骨蒸、经闭痛经、跌扑伤痛、痈肿疮毒等症。
本发明中,所述赤芍典型但非限制性的含量为6份、7份、8份、9份、10份、11份、12份、13份或14份。
紫草,为紫草科植物新疆紫草、紫草或内蒙紫草的干燥根;味甘、咸,性寒;归心经、肝经;具有凉血、活血、解毒透疹之功效;主治用于血热毒盛、温热斑疹、湿热黄疸、尿血淋浊、热结便秘、湿疹痈疡等症。
本发明中,所述紫草典型但非限制性的含量为25份、26份、27份、28份、29份、30份、31份、32份、33份、34份或35份。
苍术,为菊科植物茅苍术的干燥根茎;味辛、苦,性温;归脾经、胃经、肝经;具有燥湿健脾、祛风散寒、明目之功效;主治湿阻中焦、脘腹胀满、泄泻水肿、风湿痹痛、风寒感冒、眼目昏涩等症。
本发明中,所述苍术典型但非限制性的含量为8份、9份、10份、11份、12份、13份、14份、15份或16份。
黄柏,为芸香科植物黄皮树的干燥树皮;味苦,性寒;归肾经、膀胱经;具有清热燥湿、泻火除蒸、解毒疗疮之功效;主治湿热泻痢、黄疸尿赤、热淋涩痛、骨蒸劳热、盗汗遗精、疮疡肿毒、湿疹湿疮等症。

川牛膝,为苋科植物川牛膝的干燥根;味甘、微苦,性平;归肝经、肾经,具有逐瘀通经、通利关节、利尿通淋之功效;主治经闭症瘕、胞衣不下、跌扑损伤、风湿痹痛、足痿筋挛、尿血血淋等症。

薏苡仁,为禾本科植物薏苡的干燥成熟种仁;味甘、淡,性凉;归脾经、胃经、肺经,具有利水渗湿、健脾止泻、除痹排脓、解毒散结之功效;主治风湿身痛、水肿喘急、肺痿咳嗽、痈疽不溃等症。
本发明中,所述薏苡仁典型但非限制性的含量为25份、26份、27份、28份、29份、30份、31份、32份、33份、34份或35份。
土茯苓,为百合科植物光叶菝葜的干燥根茎;味甘、淡,性平;归肝经、胃经;具有解毒除湿、通利关节之功效;主治湿热淋浊、脾虚泄泻、湿痹拘挛、痈肿等症。

萆薢,为薯蓣科植物绵萆薢和薯蓣科植物粉背薯蓣的干燥根茎;味苦,性平;归肾经、胃经;具有利湿去浊、祛风除痹之功效;主治膏淋、风湿痹痛、关节不利、腰膝疼痛等症。

大黄,蓼科植物掌叶大黄、唐古特大黄或药用大黄的干燥根及根茎;味苦,性寒;归脾经、胃经、大肠经、肝经、心包经;具有泻热通便、凉血解毒、逐瘀通经之功效;主治实热便秘、积滞腹痛、泻痢不爽、湿热黄疸、血热吐衄、目赤咽肿、肠痈腹痛、痈肿疔疮、瘀血经闭、跌打损伤等症。

白术,为菊科植物白术的干燥根茎;味苦,性温;归脾经、胃经;具有健脾益气、燥湿利水、止汗、安胎之功效;主治脾虚食少、腹胀泄泻、痰饮眩悸、水肿自汗、胎动不安等症。

山楂,为蔷薇科植物山里红或山楂的干燥成熟果实;味酸、甘,性微温;归脾经、胃经、肝经;具有消食健胃,行气散瘀,化浊降脂之功效;主治肉食积滞、胃脘胀满、泻痢腹痛、瘀血经闭、产后瘀阻、心腹刺痛、胸痹心痛、疝气疼痛、高脂血症等症。
本发明中,所述山楂典型但非限制性的含量为8份、9份、10份、11份、12份、13份、14份、15份或16份。
本发明中,通过选用清热凉血、健脾化湿、利水渗湿、泄浊逐瘀的中药组分配合使用,并经过反复试验对各组分所用药材以及其用量进行调整,从而得到本发明所述中药复方制剂。本发明中,水牛角、干地黄、赤芍、牡丹皮和紫草具有清热凉血、活血化瘀的功效,能够解决血热毒盛的问题,进一步治疗IgA肾病;而苍术、黄柏、川牛膝和薏苡仁具有清热利湿,健脾化湿、利水渗湿、泄浊逐瘀的功效,能够解决过食肥甘、痰湿中阻、积浊于内、血行不畅的问题,进一步治疗高尿酸血症。本发明的中药复方制剂,通过上述各原料药的配伍关系,能够治疗IgA肾病合并高尿酸血症,且具有愈后可停药、不复发且无毒副作用的优点,属于源头创新、国内首创。

(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,制得汤剂。

进一步地,本发明的中药复方制剂的剂型可采用中药制剂的常规方法制成任何常规口服制剂,如汤剂、片剂、颗粒剂、丸剂、胶囊剂、糖浆剂等。
本发明还提供了上述中药复方制剂在治疗IgA肾病合并高尿酸血症的应用。
通过使用本发明中所述中药复方制剂作为原料,进一步得到IgA肾病合并高尿酸血症的药物,所制得的药物对IgA肾病合并高尿酸血症有明显治疗效果同时具有不会复发等优点。
本发明的有益效果如下:
1、本发明的中药复方制剂选用药膳同源的天然食用植物为原料,各组分符合药政法规定。利用各种药物的综合作用治疗,药效温和不刺激,不添加化学试剂,对人体无毒无害。
2、本发明的中药复方制剂通过各中药组分的配伍关系,具有清热凉血、活血化瘀、健脾化湿、利水渗湿、泄浊逐瘀的功效,可以有效地降低蛋白尿、降尿酸、降血脂从而修复肾功能,对治疗IgA肾病合并高尿酸血症效果良好,同时标本兼治、治疗效果好而且愈后不易复发。
3、本发明的中药复方制剂制备工艺简单,操作简单易行,不需要特殊设备即可最大程度保留各个药材的药用功效,成本低,服用方便。

为了更清楚地说明本发明,下面结合优选实施例对本发明做进一步的说明。本领域技术人员应当理解,下面所具体描述的内容是说明性的而非限制性的,不应以此限制本发明的保护范围。
实施例1
一种中药复方制剂,按照重量份数计,由以下原料制成:水牛角30份、干地黄15份、赤芍12份、牡丹皮10份、紫草30份、苍术12份、黄柏12份、川牛膝12份、薏苡仁30份、土茯苓30份、萆薢12份、车前子12份、白芥子12份、大黄6份、白术15份和山楂12份。
按照如下步骤制得本实施例中的中药复方制剂:
(1)将各原料混合,向混合物中加入重量为各原料总重量10倍的水,浸泡45min,煎煮60min,过滤,得到药汁a和滤渣b。
(2)向滤渣b加入重量为原料总重量8倍的水,煎煮45min,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,制得汤剂。
实施例2
一种中药复方制剂,按照重量份数计,由以下组分组成:水牛角25份、干地黄10份、赤芍8份、牡丹皮6份、紫草25份、苍术12份、黄柏8份、川牛膝8份、薏苡仁25份、土茯苓25份、萆薢8份、车前子8份、白芥子8份、大黄3份、白术10份和山楂8份。
按照实施例1中的步骤,制得实施例2中的中药复方制剂。
实施例3
一种中药复方制剂,按照重量份数计,由以下组分组成:水牛角35份、干地黄20份、赤芍16份、牡丹皮14份、紫草35份、苍术16份、黄柏16份、川牛膝16份、薏苡仁35份、土茯苓35份、萆薢16份、车前子16份、白芥子16份、大黄9份、白术20份和山楂16份。
按照实施例1中的步骤,制得实施例3中的中药复方制剂。
实施例4
一种中药复方制剂,按照重量份数计,由以下组分组成:水牛角28份、干地黄13份、赤芍10份、牡丹皮8份、紫草28份、苍术10份、黄柏10份、川牛膝10份、薏苡仁28份、土茯苓28份、萆薢10份、车前子10份、白芥子10份、大黄5份、白术13份和山楂10份。
按照实施例1中的步骤,制得实施例4中的中药复方制剂。
实施例5
一种中药复方制剂,按照重量份数计,由以下组分组成:水牛角32份、干地黄17份、赤芍14份、牡丹皮12份、紫草32份、苍术14份、黄柏14份、川牛膝14份、薏苡仁32份、土茯苓32份、萆薢14份、车前子14份、白芥子14份、大黄7份、白术17份和山楂14份。
按照实施例1中的步骤,制得实施例5中的中药复方制剂。
实施例6
选用实施例1中的中药组成,按照如下步骤制得实施例6中的中药复方制剂:
(1)将各原料混合,向混合物中加入重量为原料总重量8倍量的水,浸泡60min,煎煮40min,过滤,得到药汁a和滤渣b。
(2)向滤渣b加入6倍的水,煎煮30min,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,制得汤剂。
实施例7
选用实施例1中的中药组成,按照如下步骤制得实施例7中的中药复方制剂:
(1)将各原料混合,向混合物中加入重量为原料总重量12倍量的水,浸泡30min,煎煮80min,过滤,得到药汁a和滤渣b。
(2)向滤渣b加入10倍的水,煎煮60min,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,制得汤剂。
实施例8
选用实施例1中的中药组成,按照如下步骤制得实施例8中的中药复方制剂:
(1)将各原料混合,向混合物中加入重量为各原料总重量10倍的水,浸泡45min,煎煮60min,过滤,得到药汁a和滤渣b。
(2)向滤渣b加入8倍的水,煎煮45min,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,得稠浸膏,加入适当辅料,干燥,制粒,整粒,制得颗粒剂。
实施例9
选用实施例1中的中药组成,按照如下步骤制得实施例9中的中药复方制剂:
(1)将各原料混合,向混合物中加入重量为各原料总重量10倍的水,浸泡45min,煎煮60min,过滤,得到药汁a和滤渣b。
(2)向滤渣b加入8倍的水,煎煮45min,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,得稠浸膏,加入适当辅料,干燥,制粒,压片,制得片剂。
实施例10
选用实施例1中的中药组成,按照如下步骤制得实施例10中的中药复方制剂:
(1)将各原料混合,向混合物中加入重量为各原料总重量10倍的水,浸泡45min,煎煮60min,过滤,得到药汁a和滤渣b。
(2)向滤渣b加入8倍的水,煎煮45min,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,得稠浸膏,干燥,制粒,填充入胶囊,制得胶囊剂。
实施例11
选用实施例1中的中药组成,按照如下步骤制得实施例11中的中药复方制剂:
(1)将各原料混合,向混合物中加入重量为各原料总重量10倍的水,浸泡45min,煎煮60min,过滤,得到药汁a和滤渣b。
(2)向滤渣b加入8倍的水,煎煮45min,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,得稠浸膏,加入适当辅料,制成丸剂。
实施例12本发明中药复方制剂临床试验
1.研究对象
1.1采集对象
选取郑州大学第一附属医院和武汉同济医院2013年5月至2015年6月收治的IgA肾病合并高尿酸血症患者。
1.2纳入标准:
经临床确诊为IgA肾病合并高尿酸血症伴轻度肾功能不全。
1.3排除标准:
①患有系统性红斑狼疮、糖尿病、过敏性紫癜、乙型肝炎等全身性疾病合并的继发性IgAN
②患有恶性肿瘤者
③患有继发性IgA肾病。
1.4筛选结果
根据以上纳入和排除标准,从原数据库中筛选248例IgA肾病合并高尿酸血症患者,其中男性133例,女性115例;年龄27-61岁。
2.研究方法
2.1试验分组
按照随机数字法分为西医组和中西医组。
西医组,124例,男68例,女56例;年龄最小27岁,最大61岁,平均年龄(44.69±6.96)岁。
中西医组,124例,男65例,女59例;年龄最小28岁,最大60岁,平均年龄(44.05±7.32)岁。
2.2试验过程
西医组给予氯沙坦治疗,每天两次,每次50mg,口服,连续用药12周。
中西医组在西医组基础上配合实施例1制备的中药复方制剂治疗,每天两次,每次150mL,口服,连续用药12周。
两组停药后,随访8周,复查血尿酸,观察复发率。
2.3试验疗效评价指标
检测两组患者治疗前后血压、血尿酸、血肌酐、尿素氮、24h尿蛋白定量、尿β2-MG、药物不良反应及高尿酸血症复发率的差异。
3.统计方法
采用SPSS17.0统计学软件对数据进行统计分析。计量资料以均数±标准差(x±s)表示,采用两样本t检验或配对t检验;计数资料采用卡方检验。P<0.05表示差异具有统计学意义。
4.试验结果
将两组患者的基本资料进行对比,见表1。

通过表1的比较,可知两组患者性别、年龄及病情等一般资料比较差异无统计学意义(P>0.05),具有可比性。
经过12周的治疗,两组患者治疗前后各指标的对比见表2,两组患者的药物不良反应的对比见表3,两组患者愈后复发率的对比见表4。

治疗12周后,中西药组患者的血压、血尿酸、血肌酐、尿素氮、24h尿蛋白定量和尿β2-MG较治疗前明显下降,且P<0.05,具有统计学差异。
治疗12周后,通过中西药组和西药组治疗情况对比,可以发现中西药组血尿酸、血肌酐、尿素氮、24h尿蛋白定量和尿β2-MG明显低于西药组,且P<0.05,具有统计学差异。同时,两组间的药物不良反应无显著差异,中西药组较西药组高尿酸血症的复发率低。
可以得到如下结论:
(1)通过中西医结合治疗,能够明显降低IgA肾病合并高尿酸血症患者的血压、血尿酸、肌酐、尿素氮、24h尿蛋白和尿β2-MG水平。
(2)与单纯的西医治疗方案相比,中西医结合治疗方案在治疗IgA肾病合并高尿酸血症方面不良反应相当。
(3)与单纯的西医治疗方案相比,中西医结合治疗方案在治疗IgA肾病合并高尿酸血症方面高尿酸血症的复发率更低。

Claims (10)
1.一种治疗IgA肾病合并高尿酸血症的中药复方制剂,其特征在于,所述中药复方制剂由按照重量份数计的以下原料制成:水牛角25-35份、干地黄10-20份、赤芍8-16份、牡丹皮6-14份、紫草25-35份、苍术8-16份、黄柏8-16份、川牛膝8-16份、薏苡仁25-35份、土茯苓25-35份、萆薢8-16份、车前子8-16份、白芥子8-16份、大黄3-9份、白术10-20份和山楂8-16份。
2.根据权利要求1所述的中药复方制剂,其特征在于,所述中药复方制剂由按照重量份数计的以下原料制成:水牛角28-32份、干地黄13-17份、赤芍10-14份、牡丹皮8-12份、紫草28-32份、苍术10-14份、黄柏10-14份、川牛膝10-14份、薏苡仁28-32份、土茯苓28-32份、萆薢10-14份、车前子10-14份、白芥子10-14份、大黄5-7份、白术13-17份和山楂10-14份。
3.根据权利要求2所述的中药复方制剂,其特征在于,所述中药复方制剂由按照重量份数计的以下原料制成:水牛角30份、干地黄15份、赤芍12份、牡丹皮10份、紫草30份、苍术12份、黄柏12份、川牛膝12份、薏苡仁30份、土茯苓30份、萆薢12份、车前子12份、白芥子12份、大黄6份、白术15份和山楂12份。
4.一种根据权利要求1-3任一项所述的中药复方制剂的制备方法,其特征在于,该方法包括如下步骤:
(1)将各原料混合,向混合物中加水,浸泡,煎煮,过滤,得到药汁a和滤渣b。
(2)再向滤渣b加水,煎煮,过滤,得到药汁c。
(3)将药汁a和药汁c合并,混合均匀,静置,取上层清液,浓缩,制得汤剂。
5.根据权利要求4所述的制备方法,其特征在于,步骤(1)中,所述加水的重量为各原料总重量的8-12倍。
6.根据权利要求4所述的制备方法,其特征在于,步骤(1)中,所述浸泡的时间为30-60min。
7.根据权利要求4所述的制备方法,其特征在于,步骤(1)中,所述煎煮的时间为40-80min。
8.根据权利要求4所述的制备方法,其特征在于,步骤(2)中,所述加水的重量为各原料总重量的6-10倍。
9.根据权利要求4所述的制备方法,其特征在于,步骤(2)中,所述煎煮的时间为30-60min。
10.根据权利要求1-3任一项所述的中药复方制剂在治疗IgA肾病合并高尿酸血症中的应用。
