Abstract
本发明公开了一种制备治疗骨质增生的中药制剂的方法,属于中药技术领域。该方法以艾叶、羌活、清风藤、鸡血藤、栀子、牡丹皮、马鞭草、伞莎草、黄龙藤、鹰嘴豆、葛根、山药、淫羊藿、薤白和甘草为原料制备得到中药制剂。本发明方法制得的中药制剂具有活血化瘀、舒经活络、祛风散寒除湿,补肾强筋壮骨的功效,且本发明方法制得的中药制剂渗透力强,见效快,针对性强,不复发,用药简单方便,无任何毒副作用。

技术领域
本发明属于中医药技术领域,具体涉及一种制备治疗骨质增生的中药制剂的方法。
背景技术
骨质增生病又称为增生性骨关节炎、骨性关节炎、退变性关节病、老年性关节炎、肥大性关节炎。通常发生在颈椎、腰椎、膝盖、足跟等部位,主要症状为关节疼痛,常发生于晨间,活动后疼痛反而减轻,但如活动过多,疼痛又可加重。检查受累关节可见关节肿胀、压痛,活动时有摩擦感或“咔嗒”声,病情严重者可有肌肉萎缩及关节畸形的症状。中医认为本病与外伤、劳损、瘀血阻络、感受风寒湿邪、痰湿内阻、肝肾亏虚等有关。祖国医学认为骨质增生常因感受风、寒、湿、热外伤、劳损致颈、腰椎、膝关节增生改变而发生,进而压迫神经根,闭阻经络,致气血运行不畅、气滞血瘀。西医则认为病因主要是由于骨钙质的丧失即钙离子代谢功能减退导致骨络细胞减少而造成的。
影响骨质增生发生的原因有很多,主要有年龄因素、慢性劳损、外伤、咽喉部炎症、发育性椎管狭窄、颈椎的先天性畸形、代谢因素、精神因素等。目前国内外治疗骨质增生症尚无有效的治疗药物,中医主要是以补肝肾,祛风湿,消肿止痛,舒筋活血为主,辅以现有的中草药进行治疗,西医的治疗原则是抗炎、镇痛。常规方法有直流电药物离子导入法、紫外线疗法、按摩与牵引、针灸、贴膏药、手术治疗等。直流电药物离子导入法充分发挥了药物的作用,但因导入药物为液体,易挥发,药效不持久,限制了该方法的治疗效果。按摩与牵引法完全靠外力的作用,只对局部进行治疗却忽视了对人体内整体地调节,因此,只能起到暂时的止痛作用,而不能达到治疗的目的。针灸法可通经活血并有止痛作用,可消除局部的水肿和炎症,但对骨质增生来说针灸只能起辅助作用。西医治疗常用地塞米松、布洛芬等消炎药或镇痛药治疗骨质增生,也有的采用雄激素、维生素、酸钙、长效降钙素进行治疗,关节肿胀有积液者采取局部抽取积液或局部封闭等,疗效只是缓解症状,治疗过程对肝、肾、胃肠都有一定的损伤,只是治标而不治本,病情易复发。因此选择及时、正规、有效、没有副作用的治疗方法是关键,中药外敷剂治疗骨质增生最大的优点在于从病理上进行根治。
如今在治疗骨质增生方面,由于口服药存在着药物通过脏腑分解到达患病部位的药量难以控制,药性强了伤及脏腑,药性低了达不到治疗效果的弊端,人们已将目光转向根据人的个体差异需要可调节药性强度的外敷药物上。据中医圣典《黄帝内经》记载,传统医骨奇法中最为有效的手段为水溶透皮术中的常温调敷疗法,俗称“糊药”,是指将优质中药配方粉碎后用水酒等调和成糊状,敷在局部进行治疗的一种透皮方法。特点是不加热,药量大,游离好。古有“一糊气百膏”之说,作用于病症部位,其效能超越普通膏剂几十部,乃至上百倍,具有渗透力强,吸收快等特点。
中国专利申请CN201210366681公开了一种骨质增生丸,由熟地、肉苁蓉、葛根、骨碎补、淫羊藿、鹿衔草、鸡血藤和莱菔子按一定重量比配制而成,该药物治疗骨质增生见效快,有效率高,但治标不治本,存在易复发的不足。
发明内容
为解决现有技术中存在的不足,本发明的目的是提供一种制备治疗骨质增生的中药制剂的方法。
为实现本发明的目的,本发明人依据中医理论的研究及临床反复的试验和验证,提供了一种治疗骨质增生的中药制剂,所述中药制剂由下列重量份数的原料制备而成:艾叶14-22份、羌活16-26份、清风藤12-18份、鸡血藤8-16份、栀子4-12份、牡丹皮5-10份、马鞭草10-16份、伞莎草6-14份、黄龙藤5-12份、鹰嘴豆12-18份、葛根10-16份、山药6-15份、淫羊藿4-8份、薤白8-14份和甘草6-14份。
优选地,所述的中药制剂由下列重量份数的原料制备而成:艾叶14份、羌活16份、清风藤12份、鸡血藤8份、栀子4份、牡丹皮5份、马鞭草10份、伞莎草6份、黄龙藤5份、鹰嘴豆12份、葛根10份、山药6份、淫羊藿4份、薤白8份和甘草6份。

本发明中药制剂所用中药材的来源、性味、归经及功效:
艾叶:本品为菊科植物艾的干燥叶;味辛、苦;性温。归肝、脾、肾经。散寒止痛,温经止血,用于吐血,衄血;外治皮肤瘙痒。
羌活:本品为伞形科植物羌活的干燥根茎及根;味辛、苦;性温。归膀胱、肾经。散寒,祛风,除湿,止痛,用于风寒感冒头痛,风湿痹痛,肩背酸痛。
清风藤:本品为清风藤科植物清风藤的茎叶或根;味苦、辛;性温。归肝经。祛风利湿;活血解毒。主风湿痹痛;鹤膝风;水肿;脚气;跌打肿痛;骨折;深部脓肿;骨髓炎;化脓性关节炎;脊椎炎;疮疡肿毒;皮肤瘙痒。
鸡血藤:本品为豆科崖豆藤属的植物;味苦、甘,性温;归肝、肾经;补血,活血,通络;活血舒筋,养血调经。
栀子:本品为茜草科植物栀子的果实;味苦,性寒;归心、肺、三焦经;清热利湿、凉血解毒、消肿止痛。
牡丹皮:本品为为毛茛科植物牡丹干燥根皮;苦、辛,微寒;归心、肝、肾经;清热凉血,活血化瘀。
马鞭草:本品为马鞭草科植物马鞭草的干燥地上部分;味苦;性凉。归肝、脾经。活血散瘀,截疟,解毒,利水消肿,用于症瘕积聚,经闭痛经,疟疾,喉痹,痈肿,水肿,热淋。
伞莎草:本品为莎草科植物风车草的茎叶;味甘、微苦;性凉。归肝经。行气活血;解毒。主瘀血作痛;蛇虫咬伤
黄龙藤:本品为五味子科植物合蕊五味子的藤茎及根;味苦、辛;性平。归肝经。清热解毒;活血消肿。主流感;痈肿疮毒;毒蛇咬伤;风湿麻木;跌打损伤。
鹰嘴豆:本品为豆科植物鹰嘴豆的种子;味甘;性平。归肺、胃经。清热解毒。主消渴,肝炎,脚气。
葛根:本品为豆科植物野葛或甘葛藤的干燥根;味甘、辛;性凉。归脾、胃经。解肌退热,生津,透疹,升阳止泻,用于外感发热头痛、项背强痛,口渴,消渴,麻疹不透,热痢,泄泻;高血压颈项强痛。
山药:本品为薯蓣科植物薯蓣的干燥根茎;味甘;性平。归脾、肺、肾经。补脾养胃,生津益肺,补肾涩精。用于脾虚食少,久泻不止,肺虚喘咳,肾虚遗精,带下,尿频,虚热消渴。
淫羊藿:本品为小檗科植物淫羊藿、箭叶淫羊藿、柔毛淫羊藿、巫山淫羊藿、或朝鲜淫羊藿的干燥地上部分;味辛、甘;性温。归肝、肾经。补肾阳,强筋骨,祛风湿。用于阳痿遗精,筋骨痿软,风湿痹痛,麻木拘挛;更年期高血压。
薤白:本品为百合科植物小根蒜或薤的干燥鳞茎;味辛、苦;性温。归肺、胃、大肠经。通阳散结,行气导滞,用于胸痹疼痛,痰饮咳喘,泄痢后重。
甘草:本品为豆科植物甘草、胀果甘草或光果甘草的干燥根;味甘;性平。归经,归心、肺、脾、胃经。补脾益气,清热解毒,缓急止痛,调和诸药,用于脾胃虚弱,倦怠乏力,心悸气短,咳嗽痰多,脘腹、四肢挛急疼痛,痈肿疮毒,缓解药物毒性、烈性。
本发明中药的组方分析:
依据中医对治疗骨质增生的辨证和中药材的配伍理论,结合现代人的身体素质和现代制药技术的特点,合理科学的搭配各味药材,形成本发明中药组方。本发明组方是以艾叶、羌活、清风藤、鸡血藤为君药,活血止痛,祛风利湿,散寒止痛;以栀子、牡丹皮、马鞭草、伞莎草、黄龙藤为臣药,活血消肿,行气活血;以鹰嘴豆、葛根、山药为佐药,通阳散结,清热解毒;以淫羊藿、薤白和甘草为使药,活血通经,缓急止痛,调和诸药;君臣佐使诸药配合,协同促进,相辅相成,最终达到活血化瘀、舒经活络、祛风散寒除湿,补肾强筋壮骨的功效,适用于骨质增生患者。
本发明一种制备治疗骨质增生的中药制剂的方法,包括以下步骤:
(1)按重量份称取鸡血藤8-16份,洗去杂质,干燥后粉碎,过80-100目筛,加入鸡血藤粗粉总重量6-8倍浓度为60-80%的乙醇,再加入鸡血藤粗粉总量0.8%-1.2%的纤维素酶,在40-45℃下浸提1-2小时,然后在80-90℃回流提取1-2小时,过滤,滤液减压浓缩至60℃下相对密度为1.10-1.25的浸膏,得鸡血藤提取物;
(2)按重量份称取艾叶14-22份、羌活16-26份、清风藤12-18份、栀子4-12份、牡丹皮5-10份、马鞭草10-16份、伞莎草6-14份、黄龙藤5-12份、鹰嘴豆12-18份、葛根10-16份、山药6-15份、淫羊藿4-8份、薤白8-14份和甘草6-14份粉碎混合,加入上述粗粉总量的4-8倍的蒸馏水,浸泡15-20小时,煎煮2-4小时,过滤并保留滤渣,滤液浓缩至60-70℃相对密度为1.20-1.25的浓缩液备用;
(3)向步骤(2)得到的滤渣中,加入滤渣总量3-4倍重量的体积浓度75-85%的乙醇溶液,回流提取1-2次,每次2-4小时,过滤,合并滤液,减压蒸馏除去乙醇,并浓缩至60-70℃相对密度为1.20-1.25的浓缩液备用;
(4)将步骤(1)得到的鸡血藤提取物与步骤(2)得到的浓缩液和步骤(3)得到的浓缩液混合,混匀后浓缩至60-70℃相对密度为1.30-1.35的中药浸膏,即得。
本发明方法制备的中药制剂,源于我国中医药理论和现代药理的研究成果,经过药效学实验研究、临床实践治疗应用,证明其在治疗骨质增生方面疗效确切,无不良反应,有活血化瘀、舒经活络、祛风散寒除湿,补肾强筋壮骨的功效。本发明中药制剂见效快,针对性强,皮肤吸收快,生物利用度高,适用方便,无毒副作用。

以下通过具体实施例进一步描述本发明,本发明不仅仅限于以下实施例。在本发明的范围内或者在不脱离本发明的内容、精神和范围内,对本发明进行的变更、组合或替换,对于本领域的技术人员来说是显而易见的,均包含在本发明的范围之内。
实施例l
实施例1中药制剂由以下重量份数的原料制备而成:
艾叶14份、羌活16份、清风藤12份、鸡血藤8份、栀子4份、牡丹皮5份、马鞭草10份、伞莎草6份、黄龙藤5份、鹰嘴豆12份、葛根10份、山药6份、淫羊藿4份、薤白8份和甘草6份。
制备方法:
(1)取鸡血藤,洗去杂质,干燥后粉碎,过100目筛,加入鸡血藤粗粉总重量8倍浓度为80%的乙醇,再加入鸡血藤粗粉总量1.2%的纤维素酶,在45℃下浸提2小时,然后在90℃回流提取2小时,过滤,滤液减压浓缩至60℃下相对密度为1.25的浸膏,得鸡血藤提取物;
(2)取艾叶、羌活、清风藤、栀子、牡丹皮、马鞭草、伞莎草、黄龙藤、鹰嘴豆、葛根、山药、淫羊藿、薤白和甘草粉碎混合,加入上述粗粉总量的8倍的蒸馏水,浸泡20小时,煎煮4小时,过滤并保留滤渣,滤液浓缩至70℃相对密度为1.25的浓缩液备用;
(3)向步骤(2)得到的滤渣中,加入滤渣总量3-4倍重量的体积浓度85%的乙醇溶液,回流提取2次,每次4小时,过滤,合并滤液,减压蒸馏除去乙醇,并浓缩至70℃相对密度为1.25的浓缩液备用;
(4)将步骤(1)得到的鸡血藤提取物与步骤(2)得到的浓缩液和步骤(3)得到的浓缩液混合,混匀后浓缩至70℃相对密度为1.35的中药浸膏,加入相应的辅料,制成软膏剂。
实施例2
实施例2中药制剂由以下重量份数的原料制备而成:
艾叶22份、羌活26份、清风藤18份、鸡血藤16份、栀子12份、牡丹皮10份、马鞭草16份、伞莎草14份、黄龙藤12份、鹰嘴豆18份、葛根16份、山药15份、淫羊藿8份、薤白14份和甘草14份。
制备方法:
(1)取鸡血藤,洗去杂质,干燥后粉碎,过80目筛,加入鸡血藤粗粉总重量6倍浓度为60%的乙醇,再加入鸡血藤粗粉总量0.8%的纤维素酶,在40℃下浸提1小时,然后在80℃回流提取1小时,过滤,滤液减压浓缩至60℃下相对密度为1.10的浸膏,得鸡血藤提取物;
(2)取艾叶、羌活、清风藤、栀子、牡丹皮、马鞭草、伞莎草、黄龙藤、鹰嘴豆、葛根、山药、淫羊藿、薤白和甘草粉碎混合,加入上述粗粉总量的4倍的蒸馏水,浸泡15小时,煎煮2小时,过滤并保留滤渣,滤液浓缩至60℃相对密度为1.20的浓缩液备用;
(3)向步骤(2)得到的滤渣中,加入滤渣总量3倍重量的体积浓度75%的乙醇溶液,回流提取1次,每次2小时,过滤,合并滤液,减压蒸馏除去乙醇,并浓缩至60℃相对密度为1.20的浓缩液备用;
(4)将步骤(1)得到的鸡血藤提取物与步骤(2)得到的浓缩液和步骤(3)得到的浓缩液混合,混匀后浓缩至60℃相对密度为1.30的中药浸膏,加入相应的辅料,制成贴膏剂。
实施例3
实施例3中药制剂由以下重量份数的原料制备而成:
艾叶18份、羌活20份、清风藤14份、鸡血藤10份、栀子8份、牡丹皮7份、马鞭草12份、伞莎草10份、黄龙藤8份、鹰嘴豆14份、葛根13份、山药10份、淫羊藿6份、薤白12份和甘草10份。
制备方法:
(1)取鸡血藤,洗去杂质,干燥后粉碎,过80目筛,加入鸡血藤粗粉总重量7倍浓度为70%的乙醇,再加入鸡血藤粗粉总量0.9%的纤维素酶,在43℃下浸提1小时,然后在85℃回流提取1小时,过滤,滤液减压浓缩至60℃下相对密度为1.15的浸膏,得鸡血藤提取物;
(2)取艾叶、羌活、清风藤、栀子、牡丹皮、马鞭草、伞莎草、黄龙藤、鹰嘴豆、葛根、山药、淫羊藿、薤白和甘草粉碎混合,加入上述粗粉总量的6倍的蒸馏水,浸泡18小时,煎煮3小时,过滤并保留滤渣,滤液浓缩至65℃相对密度为1.23的浓缩液备用;
(3)向步骤(2)得到的滤渣中,加入滤渣总量3倍重量的体积浓度80%的乙醇溶液,回流提取1次,每次2小时,过滤,合并滤液,减压蒸馏除去乙醇,并浓缩至65℃相对密度为1.23的浓缩液备用;
(4)将步骤(1)得到的鸡血藤提取物与步骤(2)得到的浓缩液和步骤(3)得到的浓缩液混合,混匀后浓缩至65℃相对密度为1.32的中药浸膏,软膏剂。
实验例1:本发明中药制剂对诱痛剂醋酸所致的大鼠疼痛的试验研究
1、试验动物:取普通级SD大鼠72只,体重(180±15g),雌雄各半,随机分为6组,每组12只。
2、试验药物及给药方案:
试验药物为本发明实施例3制得的本发明中药制剂软膏药。取3组大鼠,标记为高剂量组(1000mg/kg)、中剂量组(800mg/kg)和低剂量组(600mg/kg),造模前2天按剂量每天于实验部位涂抹3次,造模前1小时实施最后一次涂抹。
模型组:取1组大鼠,造模前2天,将等体积生理盐水每天于实验部位涂抹3次,造模前1小时实施最后一次涂抹。
阳性对照组:阳性对照药为云南白药膏(国药准字Z20073015,云南白药集团无锡药业有限公司),将药物按建议使用剂量每天于实验部位涂抹3次,造模前1小时实施最后一次涂抹。
3、实验过程:将实验大鼠的颈部、腰部用8%硫化钠饱和溶液脱毛,脱毛面积颈部约1.3cm×1.1cm,腰部约2.2cm×1.6cm。末次给药后1h,每鼠腹腔注射0.7%冰醋酸0.1ml/10g,观察记录注射冰醋酸后大鼠开始发生疼痛反应的潜伏期及发生疼痛反应后30min内大鼠的扭体次数。
4、试验结果:如表1所示。

注:与模型组比较,阳性对照组△△P <0.01;与模型组比较,本发明中药制剂药物试验组** P <0.01;与阳性对照组比较,本发明中药制剂药物试验组## P<0.01。
5、试验结论:通过表1的试验结果可以看出,与模型组比较,本发明中药制剂的高、低剂量对骨质增生的治疗有显著差异,本发明中药制剂低、中、高剂量组实验效果明显,可明显提高实验大鼠潜伏期及疼痛阈值,其中高剂量组效果最佳,潜伏期最长,一定时间内扭动次数最少,有明显的止痛作用,治疗效果强于阳性对照,差异具有统计学意义(P <0.01)。
实验例2:本发明中药制剂的临床试验
(一)受试对象选择
中医诊断标准:参照《中医病证诊断疗效标准》(2012版)和《中药新药临床研究指导原则》的“骨质增生”的内容拟定。
诊断标准为:
①病变部位出现受压迫和刺激症状,其中颈椎病变患者有长期颈肩背疼痛,头晕头
痛,双手麻木或转侧不利等症状。
② X 线摄片中可见颈椎正、侧位片,左右斜位片,颈椎生理弯曲变直或反弓,腰椎间盘突出,椎间孔狭窄,椎体骨质增生 ;膝关节正侧可见 :胫骨上端,股骨下端的内外侧髁骨赘形成,髌骨上下缘、髁间隆突,跟骨结节骨刺形成。
1、一般资料
选取2013年1月-2015年1月在骨科门诊就诊的患者共120例骨质增生患者随机分为两组,每组60例,年龄22-76岁,平均年龄45岁。两组患者在年龄、性别、发病时间、既往健康史、一般情况等方面无统计学差异(P>0.05)具有可比性。
2、试验方法
治疗组:试验药物为本发明实施例3制备的本发明中药制剂软膏剂,每日三次,7天为一疗程,2个疗程后评定疗效。
对照组:对照组用云南白药喷雾剂(国药准字Z20025478),每日三次,7天为一疗程,2个疗程后评定疗效。
3、疗效判定标准
疗效标准:参照《实用中西医结合诊断治疗学》筋腱软组织损伤疗效标准。
① 无效:治疗前后症状和体征无任何变化,或者病情加重;
② 有效:病状和体征有限程度的好转,功能有限程度的恢复;
③ 显著:病状和体征基本消失,功能恢复正常,但因天气变化或劳累等仍会出不适感;
④痊愈:病状和体征完全消失,功能完全恢复正常。
4、治疗结果:如表2所示

本次临床试验对照组为常用的治疗筋腱软组织损伤的药物云南白药膏,将其与本发明中药制剂的治疗临床效果进行比较,试验结果说明本发明中药制剂的疗效显著优于对照组药物。临床试验数据显示,对照组药物治疗骨质增生的效果痊愈率为53.3%;而本发明中药制剂治疗骨质增生效果痊愈效率高达75.0%,总有效率为96.7%,明显高于对照药73.3%的总有效率。临床实验结果证明本发明中药制剂特别是实施例3制备的中药制剂软膏剂治疗骨质增生效果显著,具有消肿止痛疗效,可作为临床治疗骨质增生药物使用,临床推广价值高。
Claims (5)
1.一种制备治疗骨质增生的中药制剂的方法,其特征在于包括以下步骤:
(1)按重量份称取鸡血藤8-16份,洗去杂质,干燥后粉碎,过80-100目筛,加入鸡血藤粗粉总重量6-8倍浓度为60-80%的乙醇,再加入鸡血藤粗粉总量0.8%-1.2%的纤维素酶,在40-45℃下浸提1-2小时,然后在80-90℃回流提取1-2小时,过滤,滤液减压浓缩至60℃下相对密度为1.10-1.25的浸膏,得鸡血藤提取物;
(2)按重量份称取艾叶14-22份、羌活16-26份、清风藤12-18份、栀子4-12份、牡丹皮5-10份、马鞭草10-16份、伞莎草6-14份、黄龙藤5-12份、鹰嘴豆12-18份、葛根10-16份、山药6-15份、淫羊藿4-8份、薤白8-14份和甘草6-14份粉碎混合,加入上述粗粉总量的4-8倍的蒸馏水,浸泡15-20小时,煎煮2-4小时,过滤并保留滤渣,滤液浓缩至60-70℃相对密度为1.20-1.25的浓缩液备用;
(3)向步骤(2)得到的滤渣中,加入滤渣总量3-4倍重量的体积浓度75-85%的乙醇溶液,回流提取1-2次,每次2-4小时,过滤,合并滤液,减压蒸馏除去乙醇,并浓缩至60-70℃相对密度为1.20-1.25的浓缩液备用;
(4)将步骤(1)得到的鸡血藤提取物与步骤(2)得到的浓缩液和步骤(3)得到的浓缩液混合,混匀后浓缩至60-70℃相对密度为1.30-1.35的中药浸膏,即得。
2.如权利要求1所述的制备治疗骨质增生的中药制剂的方法,其特征在于,所述的中药制剂的原料的重量份如下:艾叶14份、羌活16份、清风藤12份、鸡血藤8份、栀子4份、牡丹皮5份、马鞭草10份、伞莎草6份、黄龙藤5份、鹰嘴豆12份、葛根10份、山药6份、淫羊藿4份、薤白8份和甘草6份。
3.如权利要求1所述的制备治疗骨质增生的中药制剂的方法,其特征在于,所述的中药制剂的原料的重量份如下:艾叶22份、羌活26份、清风藤18份、鸡血藤16份、栀子12份、牡丹皮10份、马鞭草16份、伞莎草14份、黄龙藤12份、鹰嘴豆18份、葛根16份、山药15份、淫羊藿8份、薤白14份和甘草14份。
4.如权利要求1所述的制备治疗骨质增生的中药制剂的方法,其特征在于,所述的中药制剂的原料的重量份如下:艾叶18份、羌活20份、清风藤14份、鸡血藤10份、栀子8份、牡丹皮7份、马鞭草12份、伞莎草10份、黄龙藤8份、鹰嘴豆14份、葛根13份、山药10份、淫羊藿6份、薤白12份和甘草10份。
5.如权利要求1-4任一所述的制备治疗骨质增生的中药制剂的方法,其特征在于:所述中药制剂为软膏剂或贴膏剂。
