技术领域
本公开的实施例总体涉及用于调节神经的装置和方法。更具体地,本公开的实施例涉及用于将可植入装置传送至适于神经调节的位置的装置和方法。
背景技术
神经调节具有通过与机体自身的自然神经过程相互作用来治疗许多生理病症和障碍的机会。神经调节包括在中央、外周、或自主神经系统中电的或化学的活性的抑制(例如阻塞)、刺激、修改、调整、或治疗性改变。通过调节神经系统的活性,例如通过刺激神经或阻塞神经信号,可以实现若干个不同的目标。运动神经元可在适当的时间被刺激以引起肌肉收缩。感觉神经元可能被阻塞,例如以减轻疼痛,或者被刺激,例如以提供信号给受试者。在其它示例中,调节自主神经系统可用来调节各种不自主的生理参数,比如心脏速率和血压。神经调节可以提供治疗若干种疾病或生理状况的机会,下面将对其中的一些例子进行详细地描述。
与睡眠相关的呼吸障碍比如打鼾和阻塞性睡眠呼吸暂停(OSA)是神经调节可应用于其中的状况。OSA是特征在于睡眠期间上气道的局部或完全阻塞的反复发作的呼吸系统病症。一个人在睡眠过程中没有OSA时,咽肌在睡眠时放松并逐渐塌陷,从而缩小气道。气道缩小限制了睡眠者呼吸的效果,造成血液中的CO2含量上升。CO2的增加导致咽肌收缩,以打开气道来恢复正常呼吸。对上气道扩张起最大作用的咽肌是颏舌肌,其是舌中的若干个不同肌肉之一。颏舌肌主要用于前舌运动和前咽壁的加强。在具有OSA的患者中,同正常的个体相比,颏舌肌的神经肌肉活性下降了,同正常的个体相比引起不足的响应和收缩以打开气道。响应的这种缺乏促进了部分或全部的气道阻塞,其显著限制了睡眠者呼吸的效果。在OSA患者中,夜间常常发生若干起气道阻塞事件。因为阻塞,血液中的氧气水平逐渐减小(低氧血症)。低氧血症导致夜间觉醒,其可以由EEG记录,表明大脑从睡眠的任何阶段至短觉醒而醒来。觉醒时存在有意识的呼吸或喘息,从而解决气道阻塞。交感紧张活动率通过激素比如肾上腺素和去甲肾上腺素的释放而增加也常发生作为对低氧血症的响应。由于交感紧张的增加,心脏扩大企图泵送更多的血液并且增加血压和心脏速率,进一步唤醒患者。在呼吸暂停事件解决之后,随着患者返回到睡眠,气道再次塌陷,导致进一步的觉醒。
与重复的低氧血症相结合,这些重复的觉醒剥夺了患者的睡眠,从而导致白天嗜睡且恶化认知功能。对于重症患者来说,这个周期可能每晚重复上演达数百次。因此,夜间血压升高的反复波动和交感紧张及发作引起整天都是高血压。随后,高血压和增加的心脏速率可能导致其他疾病。
患者打鼾往往是由部分气道阻塞引起的。某些患者会有咽部肌肉的松弛至引起部分阻塞的程度不足以引起睡眠过程中的随后觉醒。当咽肌松弛并缩小气道时,空气必须以较高的速度行进通过气道,以保持相似的体积流量。较高速度流动更可能引起湍流。这些湍流流动可以引起气道组织结构的振动,产生听得见的打鼾效果。打鼾可能对患者及其周围人产生一些不利影响。打鼾可能导致低通气,在这种情形下血液中的氧水平下降,导致睡眠很浅且不舒适。打鼾还可能与中风和动脉粥样硬化的风险增加相关。此外,打鼾可能对患者周围的那些人的睡眠不利。
用于治疗打鼾和OSA的努力包括连续气道正压通气(CPAP)治疗,其需要患者磨损空气通过其而被吹入鼻孔以保持呼吸道畅通的掩模。其他的治疗选择包括将刚性插入物植入软腭以提供结构支撑、气管切开术、或组织消融。
可以应用神经调节的另一情况是发生偏头痛。头部的疼痛感觉通过枕骨神经特别是枕大神经和三叉神经被传递到大脑。当受试者经历头部疼痛时,比如在偏头痛时,这些神经的抑制可用于减少或消除疼痛的感觉。
神经调节还可以应用于高血压。体内的血压经由多个反馈机构来控制。例如,颈动脉中的颈动脉体中的压力感受器对颈动脉内的血压变化敏感。压力感受器生成的信号在血压升高时经由舌咽神经传导到大脑,向大脑发出信号来激活人体的调节系统以降低血压,例如通过改变心脏速率和血管扩张/收缩。相反,肾动脉上的及周围的副交感神经纤维生成的信号被运送到肾脏来发起行动,比如盐潴留和血管紧张素的释放,由此提升血压。调节这些神经可以提供对血压施加一些外部控制的能力。
以上只是神经调节可能有益的情况的一些例子,但是下文所述的本发明的实施例不一定限于仅治疗上述情况。
发明内容
一些实施例包括一种植入物单元递送工具。所述植入物递送工具包括:主体;保持器,其设置在所述主体的远端并且适于保持植入物单元:以及植入物激活器(activator),其与所述主体相关,所述植入物激活器配置成从电源接收功率。所述植入物激活器可以配置成:在将所述植入物单元植入受试者身体期间,从所述电源选择性且无线地传输功率至所述植入物单元,以促使调节在受试者身体中的至少一个神经;并且确定源于从所述电源选择性且无线地传输功率至所述植入物单元的神经调节响应的程度。

具体实施方式
下面将详细地参照本公开的示例性实施例,其示例示出在附图中。只要有可能,相同的附图标记将贯穿整个附图来表示相同或相似的部件。
本公开的实施例总体涉及用于通过输送能量来调节神经的装置。神经调节包括在中央、外周、或自主神经系统中电的或化学的活性的抑制(例如阻塞)、刺激、修改、调整、或治疗性改变。神经调节可以采取神经刺激的形式,其可以包括提供能量给神经,以创建足以使神经激活或传播其自身的电信号的电压变化。神经调节还可以采取神经抑制的形式,其可以包括向神经提供能量,足以防止神经传播电信号。可以通过不断地施加能量来执行神经抑制,并且还可以通过施加足够的能量来执行,以在施加之后抑制神经的功能达一段时间。其他形式的神经调节可以修改神经的功能,引起灵敏度的升高的或减少的程度。如本文所指,神经的调节可以包括调节整个神经和/或调节神经的一部分。例如,可以执行运动神经元的调节,以仅影响在施加能量的位置的远端的神经元的那些部分。
例如,在患有睡眠呼吸障碍的患者中,神经刺激的主要目标响应可以包括舌肌(例如肌肉)的收缩,以便将舌移动到不阻塞患者气道的位置。在偏头痛的治疗中,神经抑制可以用于减轻或消除疼痛的感觉。在治疗高血压中,神经调节可以用来增加、减少、消除或以其他方式修改由身体产生的神经信号来调节血压。
虽然本公开的实施例可被公开用于具有特定情况的患者,但实施例可以与期望可进行神经调节的任何患者/身体的一部分一起使用。也就是说,除了用于具有睡眠呼吸障碍、偏头痛或高血压的患者之外,本公开的实施例还可以用于许多其他领域,包括但不限于:深部脑刺激(例如,治疗癫痫、帕金森和抑郁症);心脏起搏、胃肌肉刺激(例如,治疗肥胖症)、背部疼痛、尿失禁、经痛、和/或可能受到神经调节影响的任何其他情况。
图1示出了根据本公开的示例性实施例的植入物单元和外部单元。植入物单元110可以配置成用于植入受试者中允许其调节神经115的位置。植入物单元110可以位于受试者中,使得介入组织111存在于植入物单元110和神经115之间。介入组织可以包括肌肉组织、结缔组织、器官组织,或任何其他类型的生物组织。因此,植入物单元110的位置不需要与神经115接触来进行有效的神经调节。植入物单元110还可以定位成直接邻近神经115,使得不存在介入组织111。
在治疗睡眠呼吸障碍中,植入物单元110可以位于患者的颏舌肌上。这样的位置适于调节舌下神经,其分支运行于颏舌肌内。植入物单元110还可以配置成用于放置在其它位置。例如,偏头痛的治疗可能需要在颈部后面中的皮下植入,靠近受试者发际,或着在受试者的耳后,以调节枕大神经和/或三叉神经。治疗高血压可能需要在血管内将神经调节植入物植入于肾动脉或肾静脉内(以调节副交感神经肾神经),或单侧或双侧,在颈动脉或颈内静脉内(以通过颈动脉压力感受器调节舌咽神经)。可替代地或者另外,治疗高血压可能需要皮下植入神经调节植入物,例如在耳后或在颈部,以直接调节舌咽神经。
外部单元120可以配置成用于定位在患者外部,或直接接触或靠近患者的皮肤112。外部单元120可以配置成固定到患者,例如通过连接到患者的皮肤112,或者通过带或配置成将外部单元120保持就位的其它装置。外部单元120连接到皮肤可能发生,使得其在植入物单元110的位置附近。
图2示出了用于将能量输送在具有睡眠呼吸障碍的患者100中的神经调节系统的示例性实施例。该系统可以包括的外部单元120可以配置成用于定位在患者的外部。如图2所示,外部单元120可以配置成固定到患者100。图2示出了在具有睡眠呼吸障碍的患者100中,外部单元120可以配置成用于放置在患者的下巴下方和/或在患者的颈部前面。可以通过外部单元120和植入物单元110之间的通信来确定布置位置的适宜性,如下面更详细地讨论。在替代实施例中,为了治疗除睡眠呼吸障碍之外的情况,外部单元可以配置为固定在适于患者的任何地方,比如患者颈部的后面,即用于与偏头痛治疗植入物单元通信,在患者腹部的外部上,即用于与胃调节植入物单元通信,在患者的背部上,即用于与肾动脉调节植入物单元通信,和/或在患者皮肤上的任何其他适当的外部位置上,取决于特定应用的要求。
外部单元120可以进一步配置成固定到接近患者的可替代位置。例如,在一个实施例中,外部单元可以配置成固定地或可拆卸地连接到带,该带可以配置成环绕患者身体的一部分。可替代地或者另外,外部单元可以配置成保持在患者身体外部的期望位置,而不会连接到该位置。
外部单元120可以包括壳体。壳体可以包括配置成用于保持部件的任何合适的容器。此外,尽管外部单元示意性地示于图2,但壳体可以是任何合适的尺寸和/或形状,并且可以是刚性的或柔性的。用于外部单元100的壳体的非限制性示例包括贴片(patch)、按钮、或具有不同的形状和尺寸以及由任何合适的材料构成的其他容器中的一个或多个。例如,在一个实施例中,壳体可以包括柔性材料,使得外部单元可以配置成符合所希望的位置。例如,如图2所示,外部单元可以包括皮肤贴片,其又可以包括柔性基底。柔性基底的材料可以包括但不限于塑料、硅树脂、织造天然纤维、和其他合适的聚合物、共聚物、以及它们的组合。外部单元120的任何部分可以是柔性的或刚性的,这取决于特定应用的要求。
如前面所讨论,在一些实施例中,外部单元120可以配置成连接到期望的位置。因此,在一些实施例中,壳体的至少一侧可以包括粘合剂材料。粘合剂材料可以包括生物相容的材料,并且可以允许患者将外部单元连接到期望的位置并且在使用完成时移除外部单元。粘合剂可以配置成用于外部单元的单个或多个使用。合适的粘合剂材料可以包括但不限于生物相容的胶水、淀粉、柔性体、热塑性塑料和乳剂。
图3示意性示出的系统包括外部单元120和植入物单元110。在一些实施例中,内部单元110可以配置为要被植入到患者体内的单元,外部单元120可以配置成将信号发送到植入物单元110和/或从其接收信号。
如图3所示,各个部件可以包括在外部单元120的壳体内或者以其他方式与外部单元120相关。如图3所示,至少一个处理器144可以与外部单元120相关。例如,至少一个处理器144可以位于外部单元120的壳体内。在替代实施例中,至少一个处理器可以配置成用于与壳体外部位置的外部单元有线或无线通信。
至少一个处理器可以包括任何电路,其可以配置成对至少一个输入变量执行逻辑操作。因此,至少一个处理器可以包括一个或多个集成电路、微芯片、微控制器和微处理器,其可以是中央处理单元(CPU)、数字信号处理器(DSP)、现场可编程门阵列(FPGA)、或本领域技术人员已知的可适于执行指令或执行逻辑运算的任何其他电路的全部或部分。
图3示出了外部单元120可以进一步与电源140相关。电源可以在相对于外部单元的外部位置可拆卸地联接到外部单元。可替代地,如图3所示,电源140可以永久地或可拆卸地联接到外部单元120内的位置。电源可以进一步包括配置成与处理器电通信的任何合适的电源。例如,在一个实施例中,电源140可以包括电池。
电源可以配置成对外部单元内的各个部件供电。如图3所示,电源140可以配置成提供功率给处理器144。此外,电源140可以配置成提供功率给信号源142。信号源142可以与处理器144通信,并且可以包括配置成产生信号(例如正弦信号、方波、三角波、微波、射频(RF)信号、或任何其它类型的电磁信号)的任何设备。信号源142可以包括但不限于波形发生器,其可以配置成产生交流(AC)信号和/或直流(DC)信号。例如,在一个实施例中,信号源142可以配置成产生AC信号以便传输到一个或多个其他部件。信号源142可以配置成产生任何合适频率的信号。在一些实施例中,信号源142可以配置成产生具有约6.5兆赫至约13.6兆赫的频率的信号。在另外的实施例中,信号源142可以配置成产生具有约7.4至约8.8兆赫的频率的信号。在进一步的实施例中,信号源142可以产生具有频率低至90千赫或高达28兆赫的信号。
信号源142可以配置成与放大器146直接或间接的电通信。放大器可以包括配置成放大从信号源142产生的一个或多个信号的任何合适的设备。放大器146可以包括一个或多个各种类型的放大设备,例如包括基于晶体管的装置、运算放大器、RF放大器、功率放大器、或可以增加与信号的一个或多个方面相关的增益的任何其它类型的装置。放大器可以进一步配置成将放大的信号输出到外部单元120内的一个或多个部件。
外部单元还可以包括主天线150。主天线可以配置为外部单元120内的电路的一部分,并且可以或直接地或间接地联接到外部单元120中的各个部件。例如如图3所示,主天线150可以配置成用于与放大器146通信。
主天线可以包括可配置成产生电磁场的任何导电结构。主天线还可以是任何合适的尺寸、形状和/或配置。所述尺寸、形状和/或配置可以由患者的尺寸、植入物单元的放置位置、植入物单元的尺寸和/或形状、调节神经所需的能量的量、要调节的神经的位置、植入物单元上存在的接收电子器件的类型等来确定。主天线可以包括本领域技术人员公知的可以配置成发送和/或接收信号的任何合适的天线。合适的天线可以包括但不限于长导线天线、贴片天线、螺旋天线等。例如在一个实施例中,如图3所示,主天线150可以包括线圈天线。这样的线圈天线可以由任何合适的导电材料制成,并且可以配置成包括任何合适布置的导电线圈(例如,线圈的直径、数量、线圈的布局等)。适用作主天线150的线圈天线可以具有的直径在约1厘米至10厘米之间,并且可以是圆形或椭圆形的。在一些实施例中,线圈天线可以具有的直径在5厘米和7厘米之间,并且可以是椭圆形的。适用作主天线150的线圈天线可以具有任意数量的绕组,例如4、8、12或更多。适用作主天线150的线圈天线可以具有的线直径在约0.1毫米和2毫米之间。这些天线参数仅是示例性的,并且可以被调整得高于或低于对实现合适的结果而给定的范围。
如上所述,植入物单元110可以配置成植入患者体内(例如在患者的皮肤之下)。图2示出的是,植入物单元110可以配置成被植入用于调节与受试者的舌头130的肌肉相关的神经。调节与受试者的舌头130的肌肉相关的神经可以包括引起肌肉收缩的刺激。在进一步的实施例中,植入物单元可以配置成被放置成与人们可能期望调节的任何神经结合。例如,枕骨神经、枕大神经、和/或三叉神经的调节可用于治疗头部的疼痛感觉,比如来自偏头痛。调节肾动脉(即肾神经)、迷走神经、和/或舌咽神经上的及周围的副交感神经纤维可用于治疗高血压。此外,还可以调节外周神经系统(包括脊髓和颅)的任何神经(包括运动神经元、感觉神经元、交感神经元和副交感神经元),以达到期望的效果。
植入物单元110可以由适于植入到患者体内的任何材料形成。在一些实施例中,植入物单元110可以包括柔性载体161(图4),其包括柔性的生物相容的材料。例如,这样的材料可以包括硅氧烷、聚酰亚胺、苯基三甲氧基硅烷(PTMS)、聚甲基丙烯酸甲酯(PMMA)、聚对二甲苯C、聚酰亚胺、液态聚酰亚胺、层压聚酰亚胺、黑色环氧树脂、聚醚醚酮(PEEK)、液晶聚合物(LCP)、聚酰亚胺(Kapton)等。植入物单元110还可以包括电路,其包括导电材料,比如金、铂、钛、或任何其他生物相容的导电材料或材料的组合。植入物单元110和柔性载体161还可以被制造成具有的厚度适于植入患者皮肤之下。植入物单元110可以具有的厚度小于约4mm或小于约2毫米。
图3示出的其它部件可以包括在植入物单元中或者以其它方式与之相关。例如,植入物单元110可以包括副天线152,其安装到柔性载体161上或与之集成。类似于主天线,副天线可以包括本领域技术人员公知的可以配置成发送和/或接收信号的任何合适的天线。副天线可以包括任何合适的尺寸、形状和/或配置。所述尺寸、形状和/或配置可以由患者的尺寸、植入物单元的放置位置、调节神经所需的能量的量等来确定。合适的天线可以包括但不限于长导线天线、贴片天线、螺旋天线等。例如,在一些实施例中,副天线152可以包括具有圆形(还参见图4)或椭圆形的线圈天线。这样的线圈天线可以由任何合适的导电材料制成,并且可以配置成包括任何合适布置的导电线圈(例如,线圈的直径、数量、线圈的布局等)。适用作副天线152的线圈天线可以具有的直径在约5毫米至30毫米之间,并且可以是圆形或椭圆形的。适用作副天线152的线圈天线可以具有任意数量的绕组,例如4、15、20、30或50。适用作副天线152的线圈天线可以具有的线直径在约0.01毫米和1毫米之间。这些天线参数仅是示例性的,并且可以被调整得高于或低于对实现合适的结果而给定的范围。植入物单元110还可以包括与副天线152相关的部件154。
植入物单元110可以另外地包括多个产生场的植入物电极158a、158b。电极可以包括在植入物单元上的任何合适的形状和/或定向,只要电极可以配置成在患者的体内产生电场。植入物电极158a和158b还可以包括任何合适的导电材料(例如铜、银、金、铂、铱、铂-铱、铂-金、导电性聚合物等)或导电(和/或贵金属)材料的组合。例如,在一些实施例中,电极可以包括短线(short line)电极、圆形电极、和/或圆形成对电极。如图4所示,电极158a和158b可以位于细长臂162的第一延伸部162a的端部上。然而,电极还可以位于植入物单元110的任何部分上。另外,植入物单元110可以包括位于多个位置的电极,例如在细长臂162的第一延伸部162a和第二延伸部162b的端部上,例如如图5所示。将电极定位在细长臂162的两个延伸部上可以允许双边舌下神经刺激,如下面进一步讨论。植入物电极可以具有的厚度在约200纳米和1毫米之间。阳极和阴极电极对可以间隔开约0.2毫米至25毫米左右的距离。在另外的实施例中,阳极和阴极电极对可以间隔开约1毫米至10毫米的距离,或介于4毫米和7毫米之间。相邻的阳极或相邻的阴极可以间隔开的距离小至0.001毫米以下,或者大至25毫米以上。在一些实施例中,相邻的阳极或相邻的阴极可以间隔开的距离在约0.2毫米和1毫米之间。
图4提供了植入物单元110的示例性配置的示意图。如图4所示,在一个实施例中,产生场的电极158a和158b可以包括设置在柔性载体161上的两组四个圆形电极,其中一组电极提供阳极,另一组电极提供阴极。植入物单元110可以包括一个或多个结构元件,以促进将植入物单元110植入到患者的体内。例如,这样的元件可以包括细长臂、缝合孔、聚合手术网、生物胶、突出以锚定到组织的柔性载体的尖峰、用于相同目的的额外生物相容材料的尖峰等,其促进植入物单元110在患者体内沿期望的方向对准,并且提供连接点用于将植入物单元110固定在体内。例如,在一些实施例中,植入物单元110可以包括细长臂162,其具有第一延伸部162a且任选地第二延伸部162b。延伸部162a和162b可有助于相对于特定的肌肉(例如颏舌肌)、患者体内的神经、或者神经上方的身体内的表面定向植入物单元110。例如,第一和第二延伸部162a、162b可以配置成使得植入物单元能够顺从至少部分地围绕患者皮肤下面的软或硬组织(例如神经、骨骼、或肌肉等)。此外,植入物单元110还可以包括位于柔性载体161上的任意位置的一个或多个缝合孔160。例如,在一些实施例中,缝合孔160可被放置在细长臂162的第二延伸部162b上和/或在细长臂162的第一延伸部162a上。植入物单元110可以以各种形状来构造。另外或可替代地,植入物单元110可以包括手术网1050或其它可穿孔材料,下面将参照图10进行更详细地描述。在一些实施例中,植入物单元可能会出现基本上如图4所示。在其他实施例中,植入物单元110可能缺少图示的结构,比如第二延伸部162b,或者可以具有在不同方向的附加的或不同的结构。此外,植入物单元110可以形成有通常为三角形、圆形或矩形的形状,作为替代如在图4中所示的带翼的形状。在一些实施例中,植入物单元110的形状(例如如图4所示)可有利于将植入物单元110相对于要被调节的特定神经定向。因此,还可以采用其它规则或不规则的形状,以便于植入身体的不同部位。
如图4所示,副天线152和电极158a、158b可以安装在柔性载体161上或与之集成。各个电路部件和连接导线(下面进一步讨论)可以用来将副天线与植入电极158a和158b连接。为了保护天线、电极、电路部件和连接导线免受患者体内的环境影响,植入物单元110可以包括封装植入物单元110的保护性涂层。在一些实施例中,保护涂层可以由柔性材料制成,以使得能够随着柔性载体161弯曲。保护性涂层的封装材料还可以抵抗湿度渗透和防止腐蚀。在一些实施例中,保护性涂层可以包括多个层,其中包括在不同层中的不同材料或材料的组合。
在一些实施例中,包括在植入物110中的电路部件的全部或部分可以容纳在陶瓷壳体中。这样的壳体可以是陶瓷翻盖,并且可以与生物相容性金属比如金或钛焊接成封闭的。陶瓷壳体可以保护植入物10的部件免受身体内的环境影响。可以进一步采用用于封装植入物单元110的其余部分的材料对陶瓷壳体进行封装,如上所述。
图5a是根据本公开的示范性实施例的植入物单元110的替代实施例的透视图。如图5a所示,植入物单元110可以包括多个电极,例如位于第一延伸部162a和第二延伸部162b的端部。图5a示出的实施例中的植入物电极158a和158b包括短线电极。
图5b示出了根据本公开的示范性实施例的植入物单元810的另一替代实施例。植入物单元810配置成使得电路880位于与副天线852垂直布置。植入物单元810可以包括第一延伸部162a和第二延伸部162b,其中延伸部的一个或两个容纳电极158a和158b。
图10中还示出了封装的手术网1050。手术网1050可以提供更大的目标区域给外科医生以在植入过程中将植入物单元110缝合到位时使用。整个手术网1050可以由主胶囊1021进行封装,允许外科医生将针穿过网的任何部分,而不损害植入物单元110的完整性。手术网1050还可以用于覆盖缝合孔160,从而允许更大的缝合孔160可以向外科医生提供更大的目标区域。手术网1050还可以鼓励周围的组织与植入物单元110粘结。在一些实施例中,外科医生可以使手术缝合针穿过缝合孔160(位于植入物单元110的细长臂162的一个延伸部162a上),穿过受试者的组织,以及穿过手术网1050(设置在植入物单元110的细长臂162的第二延伸部162b上)。在此实施例中,由手术网1050提供的较大目标区域可便于缝合过程,因为在使缝合针穿过组织之后将其精确地定位可能会更加困难。可以通过使用递送工具使植入和缝合过程得以进一步促进,如下面更详细地描述。
回到图2和3,外部单元120可以配置成与植入物单元110通信。例如,在一些实施例中,主信号可以产生于主天线150,例如通过使用处理器144、信号源142和放大器146。更具体地说,在一个实施例中,电源140可以配置成提供功率给处理器144和信号源142中的一个或二者。处理器144可以配置成使信号源142产生信号(例如RF能量信号)。信号源142可以配置成输出所产生的信号到放大器146,其可以放大由信号源142产生的信号。放大的量且因此信号的幅度可被控制,例如由处理器144。处理器144促使放大器146施加到信号的增益或放大的量可取决于多种因素,包括但不限于主天线150的形状、大小和/或结构、患者的大小、植入物单元110在患者中的位置、副天线152的形状、大小和/或结构、主天线150与副天线152(以下进一步讨论)之间的耦合程度、要由植入物电极158a、158b产生的电场的期望幅度等。放大器146可以输出放大的信号到主天线150。
外部单元120可以将主天线上的主信号通信到植入物单元110的副天线152。此通信可能源于主天线150与副天线152之间的耦合。主天线与副天线152的这种耦合可以包括主天线与副天线之间的任何相互作用,其响应于被施加到主天线的信号而在副天线上导致信号。在一些实施例中,主天线与副天线之间的耦合可以包括电容耦合、电感耦合、射频耦合等以及它们的任意组合。
主天线150与副天线152之间的耦合可以取决于主天线相对于副天线的接近度。也就是说在一些实施例中,主天线150与副天线152之间的耦合的效率或程度可能取决于主天线与副天线的接近度。主天线和副天线的接近度可以表示为术语同轴偏移(例如当主天线和副天线的中心轴共对齐时主天线和副天线之间的距离)、横向偏移(例如,主天线的中心轴和副天线的中心轴之间的距离)、和/或角度偏移(例如,主天线的中心轴和副天线的中心轴之间的角度差)。在一些实施例中,当同轴偏移、横向偏移和角度偏移都为零时,耦合的理论最大效率可能存在于主天线150和副天线152之间。增加同轴偏移、横向偏移和角度偏移中的任何一个都可以具有减少主天线150与副天线152之间的耦合的效率或程度的效果。
由于主天线150和副天线152之间的耦合,当主信号存在于主天线150上时,副信号可能出现在副天线152上。这样的耦合可以包括电感/磁性耦合、RF耦合/传输、电容耦合、或任何其它机理,其中副信号可以响应于在主天线150上产生的主信号而产生在副天线152上。耦合可以指主天线和副天线之间的任何相互作用。除了主天线150和副天线152之间的耦合外,与植入物单元110相关的电路部件也有可能影响副天线152上的副信号。因此,副天线152上的副信号可以指任何和所有的存在于副天线152上的信号和信号分量,而不论其来源。
尽管在主天线150上存在的主信号可能会导致或诱发副天线152上的副信号,但是这两个天线之间的耦合还可能因在副天线152上存在的副信号而导致在主天线150上的耦合信号或信号分量。由副天线152上的副信号诱导的主天线150上的信号可被称为主耦合信号分量。主信号可以指主天线150上存在的任何和所有的信号或信号分量,而不论其来源,且主耦合信号分量可以是指因与副天线152上存在的信号耦合而出现在主天线上的任何信号或信号分量。因此,在一些实施例中,主耦合信号分量可能有助于主天线150上的主信号。
植入物单元110可以配置成响应于外部单元120。例如,在一些实施例中,主线圈150上产生的主信号可能导致副天线152上的副信号,这又可能通过植入物单元110导致一个或多个响应。在一些实施例中,植入物单元110的响应可以包括在植入物电极158a和158b之间产生电场。
图6示出了可以包括在外部单元120中的电路170和可以包括在植入物单元110中的电路180。附加的不同的或更少的电路部件可以包括在电路170和电路180的任一者或两者中。如图6所示,副天线152可以布置成与植入物电极158a、158b电连通。在一些实施例中,电路将副天线152与植入物电极158a、158b连接可能因在副天线152上存在副信号而在植入物电极158a、158b上造成电压电位。这种电压电位可以被称为场感应信号,因为这种电压电位可以在植入物电极158a和158b之间产生电场。更广泛地说,场感应信号可以包括被施加到与植入物单元相关的电极的任何信号(例如,电压电位),这可能会导致电场在电极之间产生。
场诱导信号可以因通过电路180调节副信号而产生。如图6所示,外部单元120的电路170可以配置成在主天线150上产生AC主信号,其可以在副天线152上导致AC副信号。然而,在某些实施例中,可能有利的是(例如为了产生单向电场用于神经调节),在植入物电极158a和158b提供DC场感应信号。为了将副天线152上的AC副信号转换成DC场感应信号,植入物单元110中的电路180可以包括AC-DC转换器。AC-DC转换器可以包括本领域技术人员公知的任何合适的转换器。例如,在一些实施例中,AC-DC转换器可以包括整流电路部件,例如包括二极管156和适当的电容和电阻。在替代实施例中,植入物单元110可以包括AC-DC转换器,或者没有转换器,以便在植入物电极158a和158b提供AC场感应信号。
如上所述,场感应信号可以配置成在植入物电极158a和158b之间产生电场。在一些情况下,源于场感应信号所产生的电场的幅度和/或持续时间可足以调节电极158a和158b附近的一个或多个神经。在这种情况下,场感应信号可被称为调节信号。在其它情况下,场感应信号的幅度和/或持续时间可能会产生不导致神经调节的电场。在这种情况下,场感应信号可被称为子调节信号。
各种类型的场感应信号可能构成调节信号。例如,在一些实施例中,调节信号可以包括适度的幅度和适度的持续时间,而在其他实施例中,调节信号可以包括更高的幅度和更短的持续时间。电极158a、158b上的场感应信号的各种幅度和/或持续时间可能会导致调节信号,且场感应信号是否会上升到调节信号的水平可以依赖于许多因素(例如,从要被刺激的特定神经的距离;神经是否分支;感应电场相对于神经的方向;在电极和神经之间存在的组织的类型等)。
在一些实施例中,电极158a和158b可以产生的电场配置成穿透电极与一个或多个神经之间的中间组织111。中间组织111可以包括肌肉组织、骨、结缔组织、脂肪组织、器官组织或它们的任意组合。例如对于患有阻塞性睡眠呼吸暂停症的受试者来说,中间组织可以包括颏舌肌。
下面参照图8a、8b、8c和9,对配置成穿透中间组织的电场的产生进行说明。响应于场感应信号,植入物电极158a和158b可以配置成产生电场,其中场线大致沿要被调节的一条或多条神经的纵向方向延伸。在一些实施例中,植入物电极158a和158b可以沿着神经的纵向方向彼此间隔开,以促进产生这样的电场。电场还可以配置成在基本平行于要被调节的神经的至少一些部分的纵向方向的方向上延伸。例如,相比于神经,基本平行的场可以包括的场线更多在纵向方向上而不是在横向方向上延伸。以这种方式定向电场可以便于电流流过神经或组织,从而增加引发动作电位来诱导调节的可能性。
图8a示出了一对电极158a、158b,它们沿着神经210的纵向方向彼此间隔开,以促进产生的电场具有的场线220基本上平行于神经210的纵向方向。在图8a中,调节电极158a、158b示出为线电极,虽然可以通过使用其它类型的电极例如一系列点电极来实现产生基本上平行的电场。利用具有沿神经210的纵向方向延伸的场线220的电场可用于降低实现神经调节所需的能量的量。
当然,通过沿其长度发送动作电位来实现神经元功能。在结构上,神经元包括沿其长度的多个离子通道,它们用于在神经元的内部和外部之间的整个质膜上保持电压电位梯度。离子通道通过保持质膜一侧上的正电钠离子与质膜另一侧上的负电钾离子之间的适当平衡操作。在离子通道附近产生的足够高的电压电位差可能超过离子通道的膜阈值电位。然后,离子通道可被诱导激活,泵送钠和钾离子穿过质膜以在激活的离子通道附近交换位置。这又进一步改变离子通道附近的电位差,这可以用来激活相邻的离子通道。相邻离子通道的级联激活可用于沿着神经元的长度传播动作电位。此外,激活个体神经元中的离子通道可以诱导激活相邻神经元中的离子通道,它们捆绑在一起形成神经组织。然而,激活单个神经元中的单个离子通道可能不足以诱导允许传播动作电位所必需的相邻离子通道的级联激活。因此,在可以由初始电位差恢复的位置的更多的离子通道,通过自然手段比如神经末梢的作用或通过人工手段比如施加电场而造成,更可能传播动作电位。沿着神经的长度人工诱导动作电位传播的过程可被称为刺激,或至多调节。
还可以通过恒定地或大致恒定地施加电压电位差防止神经元自然起作用。激活之后,每个离子通道经历不应期,在此期间,其“复位”整个质膜上的钠和钾的浓度回到初始状态。复位钠和钾的浓度导致膜阈值电位返回到初始状态。直到离子通道恢复整个质膜上的钠和钾的适当浓度,膜阈值电位将保持提升,因此要求较高的电压电位还引起离子通道的激活。如果膜阈值电位保持在足够高的水平,则由相邻离子通道传播的动作电位可以不创建足够大的电压电位差来超越膜阈值电位并激活离子通道。因此,通过在特定离子通道的附近保持足够的电压电位差,该离子通道可以用于阻挡进一步的信号传输。膜阈值电位还可以升高而不会引起离子通道的初始激活。如果一个离子通道(或多个离子通道)经受还不够高到超过膜阈值电位的升高的电压电位差,则其可能有助于随时间提高膜阈值电位,从而对尚未允许适当地还原离子浓度的离子通道具有同样的效果。因此,离子通道可被恢复为块,而无需实际促使初始动作电位传播。例如,这种方法可能对于疼痛管理是有价值的,其中疼痛信号的传播是不希望的。如上关于刺激所述,在可恢复成充当块的位置的离子通道的数量越大,沿神经长度传播的动作电位将通过恢复的离子通道而被阻止的机会越有可能,而不是行进通过相邻的不受阻挡的通道。
由电压电位差恢复的离子通道的数量可以以至少两种方式增加。第一,可以通过利用在局部区域中的较大的电压电位差来恢复更多的离子通道。第二,可以通过扩大受电压电位差影响的区域来恢复更多的离子通道。
返回到图8a,可以看出的是,由于电场线220运行于与神经210的纵向方向基本平行的方向,所以神经210的大部分可能会遇到场。因此,来自构成神经210的神经元的更多的离子通道可被恢复,而不使用较大的电位差。以这种方式,可以采用较低的电流和更少的功率使用来实现神经210的调节。图8b示出的实施例中电极158a和158仍沿神经210的至少一部分的纵向方向彼此间隔开。神经210的显著部分保持在电场的内部。图8c示出的情况中电极158a和158b沿神经210的横向方向彼此间隔开。在此图示中,可以看到的是,神经210的显著较小部分将受到电场线220的影响。
图9示出了电极配置对所产生的电场的形状的潜在影响。电极配置的顶排例如A、B和C示出了在调节恒定大小的电极之间的距离时对电场形状的影响。电极配置的底排例如D、E和F示出了在调节恒定距离的电极大小时对电场形状的影响。
在与本公开一致的实施例中,调节电极158a、158b可以布置肌肉或其它组织的表面上,以便调节嵌入肌肉或其它组织内的神经。因此,组织可以介于调节电极158a、158b和要调节的神经之间。调节电极158a、158b可以间隔开远离要被调节的神经。调节电极158a、158b的结构和配置可以在确定是否可以实现神经(其与电极间隔开一定的距离)调节上发挥重要的作用。
电极配置A、B和C示出,当恒定大小的调节电极158a、158b移动得进一步分开时,由电极促进的电场的深度增加。对于给定配置的电场的强度可能显著变化,这取决于场内的位置。然而,如果电流的恒定水平传递在调节电极158a和158b之间,则配置C的较大场区域可以表现出比配置A的较小场区域更低的总电流密度。较低的电流密度又意味着较低的电压电位差于相对于由配置A促进的场的在由配置C促进的场中的彼此等距离间隔开的两个点之间。因此,虽然移动调节电极158a和158b远离彼此增加场的深度,但这还降低了场的强度。为了调节与调节电极158a、158b间隔开的神经,电极之间的距离可以选择成以便促进电场强度足以超过在神经深度的神经的膜阈值电位(从而如果调节的话)。如果调节电极158a、158b太过靠近在一起,则电场可以不延伸得足够深到组织中以便调节位于其中的神经。如果调节电极158a、158b相隔太远,则电场可能太弱而不能在适当的深度调节神经。
调节电极158a、158b之间的适当距离可以取决于植入物位置和要被刺激的神经。例如,调节点901位于与每个配置A、B和C中的调节电极158a、158b的中心等距离的相同的深度。在该示例中,附图说明的是,配置B最有可能在调节点901实现最高可能的电流密度且因此电位。配置A的场可以不延伸得足够深,配置C的场可能在该深度太弱。
在一些实施例中,调节电极158a、158b可以相隔约0.2毫米至25毫米的距离。在另外的实施例中,调节电极158a、158b可以相隔约1毫米至10毫米的距离,或者介于4毫米和7毫米之间。在其他实施例中,调节电极158a、158b可以相隔约6毫米至7毫米。
电极配置D、E和F表明,当恒定距离的调节电极158a、158b的大小被改变时,由电极促进的电场的形状变化。如果恒定水平的电流在调节电极158a和158b之间传递时,配置D的较小电极可以促进的场比配置E和F更深,虽然相对于改变电极之间的距离效果更不显著。如上所述,促进的电场不是均匀强度的,且因此每个电场配置D、E和F内的表面上类似位置的电压电位可能有很大的不同。调节电极158a、158b的适当尺寸因此可以取决于植入物位置和要被刺激的神经。
在一些实施例中,调节电极158a、158b可具有的表面积介于约0.01毫米2和80毫米2之间。在另外的实施例中,调节电极158a、158b可具有的表面积介于约0.1毫米2和4毫米2之间。在其他实施例中,调节电极158a、158b可具有的表面积介于约0.25毫米2和0.35毫米2之间。
在一些实施例中,调节电极158a、158b可以布置成使得电极暴露在载体161的单侧上。在这样的实施例中,电场只产生于具有暴露的电极的载体161的一侧上。这样的配置可用于降低实现神经调节所需的能量的量,因为整个电场产生在与神经相同的载体的一侧上,且行进穿过组织远离要被调节的神经的电流很少或没有被浪费。这样的配置还可以用于使调节更具选择性。也就是说,通过在其中具有要被调节的神经的载体的一侧上产生电场,位于组织的其他区域中的神经(例如在从要被调节的神经的载体的另一侧上)可避免被意外地调节。
如以上所讨论,利用具有在基本平行于要被调节的神经的纵向方向的方向上延伸的电场线的电场可以用于降低调节的功率需求。功率需求的这种降低可以允许调节神经,使用穿过调节电极158a、158b之间的小于1.6毫安的电流、小于1.4毫安的电流、小于1.2毫安的电流、小于1毫安的电流、小于0.8毫安的电流、小于0.6毫安的电流、小于0.4毫安的电流、甚至小于0.2毫安的电流。
减少需要的电流流动可能对植入物单元110和外部单元120的配置具有附加的效果。例如,减少的电流需求可以使植入物单元110能够调节神经,而不需要功率储存单元比如电池或电容器被植入与植入物单元110结合。例如,植入物单元110能够调节神经,仅使用经由副天线152接收的能量。植入物单元110可以配置成充当穿过,其引导基本上所有接收的能量来调节电极158a和158b用于神经调节。基本上所有接收的能量可以是指能量的这样部分,其不消耗或者以其他方式流失到植入物单元110的内部部件。最后,所需电流的减少还可用于降低由外部单元120所需要的能量的量。通过利用具有容量小于240毫安、小于120毫安、甚至小于60毫安的电池,外部单元120可以配置成成功地操作用于整个治疗阶段,从1小时持续至10小时。
如上所述,利用平行的场可以使得植入物单元110能够以非接触的方式调节神经。与需要要被调节的神经或肌肉接触的调节技术相比,非接触神经调节可以增加植入的植入物单元110随着时间推移的功效。随着时间的推移,可植入装置可以在体内迁移。因此,需要神经接触以引发神经调节的可植入装置可能会失去功效,因为装置在体内移动并且失去与要被调节的神经接触。与此相反,通过采用非接触调节,植入物单元110仍可以有效地调节神经,即使其移向、远离、或者至相对于最初植入位置的另一位置。此外,组织生长和/或纤维化可能围绕可植入装置发展。这种增长可用于减少或甚至消除被设计成用于接触调节的装置与要被调节的神经之间的接触。与此相反,如果附加的组织形成在其与要被调节的神经之间,则植入物单元110通过利用非接触调节可以继续有效地调节神经。
场感应信号是否构成调节信号(产生的电场可能会导致神经调节)或子调节信号(产生的电场不旨在引起神经调节)可以最终由外部单元120的处理器144来控制。例如,在某些情况下,处理器144可确定神经调节是适当的。在这些条件下,处理器144可导致信号源144和放大器146产生在主天线150上的调节控制信号(即,信号具有的幅度和/或持续时间选择成使得副天线152上所得到的副信号将在植入物电极158a和158b提供调节信号)。

除了确定可被传递到植入物单元110的功率上限之外,处理器144还可以至少部分地基于所传递的功率的功效来确定较低的功率阈值。可以基于使得能够神经调节的功率的最小量来计算较低的功率阈值(例如,具有高于较低功率阈值的功率水平的信号可以构成调节信号,而具有低于较低功率阈值的功率水平信号可以构成子调节信号)。
还可以测量或以其他的方式提供较低的功率阈值。例如,植入物单元110中的适当电路或传感器可以测量较低的功率阈值。较低的功率阈值可以由附加的外部设备来计算或感测,且随后被编程到处理器144中,或被编程到植入物单元110中。可替代地,植入物单元110可以被构造有电路180,其特别选择成在至少较低功率阈值的电极处产生信号。在另一实施例中,外部单元120的天线可被调整成容纳或产生对应于特定较低功率阈值的信号。较低的功率阈值可以根据患者而异,并且可以考虑到多种因素,例如比如特定患者的神经纤维的调节特性、植入之后的植入物单元110与外部单元120之间的距离、以及植入物单元部件(例如,天线和植入物电极)的大小和结构等。

为了分析在主天线150上感应出的主耦合信号分量,外部单元120可以包括反馈电路148(例如信号分析仪或检测器等),其可以放置成与主天线150和处理器144直接或间接通信。子调节控制信号可以以任何需要的周期被施加到主天线150。在一些实施例中,子调节控制信号可以以每五秒钟(或更长)一个的速率被施加到主天线150。在其他实施例中,子调节控制信号可被更频繁地施加(例如,每两秒一次、每秒一次、每毫秒一次每纳秒一次、或每秒多次)。此外,应当注意的是,还可以在将调节控制信号施加至主天线150时接收反馈(即导致神经调节的那些),随着这样的调节控制信号还可能导致在主天线150上产生主耦合信号分量。
主耦合信号分量可以由反馈电路148馈送到处理器144,并且可以用作确定主天线150与副天线152之间耦合程度的基础。耦合的程度可以使得能够确定这两个天线之间的能量传递的功效。处理器144还可以将所确定的耦合程度用于调节输送到植入物单元110的功率。
处理器144可以配置有任何适当的逻辑,用于基于所确定的耦合程度来确定如何调节功率传递给植入物单元110。例如,当主耦合信号分量表示耦合程度已从基线耦合水平改变时,处理器144可以确定副天线152已经相对于主天线150移动(无论是同轴偏移、横向偏移、或角偏移、或者其任何组合)。例如,这样的运动可以与植入物单元110以及基于其植入位置而与其相关的组织的运动相关。因此,在这种情况下,处理器144可以确定患者体内的神经的调节是适当的。更具体地,响应于耦合变化的指示,在一些实施例中,处理器144可能会导致施加调节控制信号至主天线150,以便在植入物电极158a、158b产生调节信号,例如促使患者的神经的调节。
在用于治疗睡眠呼吸障碍的实施例中,植入物单元110的运动可以与舌的运动相关,这可以指示打鼾,睡眠呼吸暂停事件或睡眠呼吸暂停先兆的发病。每个这些条件可能需要刺激患者的颏舌肌来减轻或避免该事件。这种刺激可以导致肌肉的收缩和患者舌运动远离患者的气道。
调节控制信号可以包括刺激控制信号,且子调节控制信号可以包括子刺激控制信号。刺激控制信号可以具有在电极158a、158b导致刺激信号的任何幅度、脉冲持续时间、或频率组合。在一些实施例中(例如,在约6.5-13.6兆赫的频率),刺激控制信号可以包括大于约50微秒的脉冲持续时间和/或约0.5安培的幅度,或介于0.1安培和1安培之间,或介于0.05安培和3安培之间。子刺激控制信号可以具有的脉冲持续时间小于约500纳秒,或小于约200纳秒和/或幅度小于约1安培、0.5安培、0.1安培、0.05安培或0.01安培。当然,这些值是为了仅提供一般参考,因为高于或低于所提供的示例性指导的值的各种组合可能会或可能不会导致神经刺激。
在一些实施例中,刺激控制信号可以包括脉冲列,其中每个脉冲包括多个子脉冲。交流信号(例如,在约6.5-13.8兆赫之间的频率)可以用于产生如下的脉冲列。子脉冲可以具有50-250微秒之间的持续时间、或1微秒和2毫秒之间的持续时间,交流信号在此期间被接通。例如,10兆赫交流信号的200微秒子脉冲将包括约2000个周期。每个脉冲又可以具有100至500毫秒的持续时间,在此期间,子脉冲发生在25至100赫兹的频率。例如,50赫兹子脉冲的200毫秒脉冲将包括约10个子脉冲。最后,在脉冲列中,每个脉冲可以与下一个分开0.2至2秒的持续时间。例如,在200毫秒脉冲的脉冲列中,每个与下一个分开1.3秒,新脉冲将每1.5秒发生。例如,可以利用本实施例的脉冲列,以在治疗阶段期间提供持续的刺激。在睡眠呼吸障碍的情况下,治疗阶段可以是一段时间,在此期间受试者睡着了并且需要治疗来防止睡眠呼吸障碍。这样的治疗阶段可能在任何地方持续约三至十个小时。在本公开的神经调节器被施加的其它条件的情况下,治疗阶段可以根据治疗条件的持续时间而改变长度。
处理器144可以配置成通过监测通过反馈电路148所接收的主耦合信号分量的一个或多个方面来确定主天线150与副天线152之间的耦合程度。在一些实施例中,处理器144可以通过监测与主耦合信号分量、电流水平或可能依赖于主天线150和副天线152之间的耦合程度的任何其他属性相关的电压水平来确定主天线150和副天线152之间耦合的程度。例如,响应于施加到主天线150的周期性子调节信号,处理器144可以确定与主耦合信号分量相关的基线电压水平或电流水平。例如,此基线电压水平可以在睡眠呼吸暂停事件或其先兆没有发生时例如在正常呼吸过程中与患者舌头的运动范围相关。随着患者的舌头移向与睡眠呼吸暂停事件或其先兆相关的位置,主天线150和与副天线152之间的同轴、横向或角偏移可能改变。其结果是,主天线150和副天线152之间的耦合程度可能变化,并且主天线150上的主耦合信号分量的电压水平或电流水平也可能变化。处理器144可以配置成在与主耦合信号分量相关的电压水平、电流水平、或其它电特性改变预定的量或达到预定的绝对值时识别睡眠呼吸暂停事件或其先兆。
图7提供的曲线图更详细地说明了该原理。对于其中一个线圈接收射频(RF)驱动信号的双线圈系统来说,曲线图200描绘了作为线圈之间同轴距离的函数的接收线圈中感应电流的变化率。对于各种线圈直径和初始位移来说,曲线图200示出了感应电流对线圈之间的进一步位移的灵敏度,移动它们或靠近在一起或进一步分开。总体而言,还表明副线圈中的感应电流将随着副线圈移动远离主驱动线圈而降低,即感应电流的变化率(毫安/毫米)一致为负的。感应电流对线圈之间进一步位移的灵敏度随距离变化。例如,在10毫米的间隔距离,作为14毫米线圈中的附加位移的函数的电流的变化率为约-6毫安/毫米。如果线圈的位移为约22毫米,响应于附加位移的感应电流的变化率约为-11毫安/毫米,其对应于感应电流变化率的局部最大值,增加间隔距离超出22毫米继续导致副线圈中感应电流的下降,但该变化率减小。例如,在约30毫米的间隔距离,14毫米线圈经历约-6毫安/毫米的响应于附加位移的感应电流的变化率。采用这种类型的信息,在任何给定的时间,通过观察与主天线150上的主耦合信号分量相关的电流的幅度和/或幅度的变化率,处理器144能够确定主天线150与副天线152之间耦合的特定程度。
处理器144可以配置成通过监测主耦合信号分量的其他方面来确定主天线150与副天线152之间的耦合程度。例如,在一些实施例中,可以监测残余信号或回波信号。如图6所示,植入物单元110中的电路180可以包括电感器、电容器和电阻器,且因此可以构成LRC电路。如上面更详细地所述,当外部单元120发送调节(或子调节)控制信号,相应的信号发展在副天线152上。在副天线152上发展的信号使电流流入植入物单元110的电路180,激发LRC电路。当被激发时,LRC电路可以以其谐振频率振荡,涉及L(电感)、R(电阻)和C(电路中的电容值)的值。当处理器144停止产生控制信号时,主天线150上的振荡信号和副天线152上的振荡信号可能随着电流被耗散而在一段时间上衰减。随着副天线152上的振荡信号衰变,所以由主天线150接收的耦合反馈信号也是如此。因此,植入物单元110的电路180中的衰减信号可以通过外部单元120的处理器144来监测。可以通过配置外部单元120的电路170来进一步促进这种监测,以允许在主天线150中产生的控制信号耗散得比植入物单元110中的信号更快。监测残余信号并且将其与剩余信号的期待值进行比较可以向处理器144提供指示主天线150与副天线152之间的耦合程度。
监测植入物单元110中的衰减振荡信号还可以向处理器144提供有关植入物单元110的性能的信息。处理器144可以配置成将控制信号的参数与检测到的衰减植入物信号的参数进行比较。例如,衰减信号的幅度与保持在植入物单元110中的能量的量成比例;通过将在控制信号中传输的能量的量与保持在植入物中的能量的量进行比较,处理器144可以确定植入物中的功率消耗的水平。此外,通过将植入物中的功率消耗的水平与舌运动的检测量进行比较,处理器144可以确定发送的调节信号的功效水平。监测植入物单元110中的残留或回波信号可以允许实施多个不同的特征。因此,处理器144能够确定的信息包括植入物单元110中的功率消耗、由植入物单元110至组织的电流输送、至植入物单元110的能量输送、植入物单元110的功能、以及可通过残留信号分析确定的其他参数。
处理器144可以配置成以诊断模式监测残留植入物信号。例如,如果处理器144在发送控制信号之后在植入物单元110中没有检测到残留信号,则可以确定植入物单元110不能接收任何类型的传输,并且不能起作用。在另一潜在的故障中,如果处理器144在植入物中检测到的残留信号高于预期,则可以确定的是,虽然植入物单元正接收发送的控制信号,但发送的能量未以合适的速率由电极158a、158b传输到组织。
处理器144还可以配置成实施治疗方案,包括施加将由调节电极所施加的希望的目标电流水平(例如1毫安)。即使调节控制信号传送恒定幅度的信号,所传送的电流不能保持稳定。由主天线150检测到的耦合反馈信号可被用作植入物单元反馈控制的基础,以确保植入物在调节控制信号的每个施加期间输送稳定的1毫安电流。通过分析植入物中的残留信号,处理器144可以确定在调节控制信号的施加期间所输送的电流的量。然后,处理器144可以基于有关所传送的电流的确定的信息来增加或减少调节控制信号的幅度。因此,被施加到主天线150的调节控制信号可被调节,直到回波信号的观察的幅度表明已经达到目标电流水平。
在一些实施例中,处理器144可以配置成在治疗周期过程中基于所检测到的疗效来改变治疗方案。如上所述,通过残留信号分析,处理器144可以配置成确定通过电极158a、158b输送到组织的电流、功率或能量的量。处理器144可以配置成将由于调节控制信号的舌运动的检测量与最终传递到组织的功率的量相关。因此,不是将信号传输的影响与所传输的功率或能量的量进行比较(该处理器144也可以配置为成这么做),处理器144可以将信号传输的影响与输送的功率的量进行比较。通过将调节影响与所输送的功率进行比较,处理器144能够更精确地优化调节信号。
以上讨论的残留信号反馈方法可被适当地应用到本公开的其他几个实施例中的任一个。例如,通过残留信号反馈分析收集的信息可以包括存储在存储器单元143中的并且经由外部单元120的收发器被传送到中继站或最终目的地的信息。在另一示例中,上述残留信号反馈分析可被纳入到检测舌运动和舌震动的方法中。
在一些实施例中,最初检测到的耦合程度可以在患者将外部单元120连接到皮肤上时建立基线范围。据推测,当患者醒着时,舌并不阻止患者的气道并且随患者呼吸在自然的范围中移动,其中主天线150与副天线152之间的耦合可以在基线范围内。基线耦合范围可以涵盖主天线150与副天线152之间的最大耦合。基线耦合范围还可以包括的范围不包括主天线150与副天线152之间的最大耦合水平。因此,最初确定的耦合可以相当地代表非睡眠呼吸暂停状态,并且可以由处理器144用作确定主天线150与副天线152之间的耦合程度的基线。
随着患者佩戴外部单元120,处理器144可以周期性地扫描一定范围的主信号幅度,以确定耦合的电流值。如果周期性扫描导致确定的耦合程度不同于基线耦合,则处理器144可以确定基线初始状态已经出现了改变。
通过周期性地确定耦合程度值,处理器144可以配置成在原位置确定用于调节控制信号的适当参数值,其将最终导致神经调节。例如,通过确定主天线150与副天线152之间的耦合程度,处理器144可以配置成选择调节控制信号的特性(例如幅度、脉冲持续时间、频率等),其可以与所确定的耦合程度成比例地或以其它方式与之相关地在电极部158a、158b提供调节信号。在一些实施例中,处理器144可以访问将调节控制信号参数值与耦合程度相关的存储在存储器中的查找表或其他数据。以这种方式,处理器144可以响应于观察到的耦合程度来调节所施加的调节控制信号。
另外或可替代地,处理器144可以配置成在调节期间确定主天线150与副天线152之间的耦合程度。舌头,或植入物位于其上或附近的其他结构,且因此植入物单元110,可以因调节移动。因此,耦合程度可以在调节过程中改变。处理器144可以配置成确定耦合程度,因为其在调节期间发生变化,以便根据改变的耦合程度来动态地调整调节控制信号的特性。这种调节可以允许处理器144在整个调节事件过程中促使植入物单元110在电极158a、158b提供适当的调节信号。例如,处理器144可以根据改变的耦合程度来改变主信号,以便保持恒定的调节信号,或者促使调节信号以根据患者需要的受控方式减少。
更具体地,处理器144的响应可能与所确定的耦合程度有关。在处理器144确定主天线150与副天线之间的耦合程度已降至仅略微低于预定耦合阈值的情况下(例如,在打鼾过程中或者在舌的小震动或其他睡眠呼吸暂停事件先兆的过程中),处理器144可以确定只有小的响应是必要的。因此,处理器144可以选择将导致相对小的响应(例如神经的短刺激、小肌肉收缩等)的调节控制信号参数。然而,如果处理器144确定耦合程度已大幅下降低于预定的耦合阈值(例如,如果舌已移动得足以导致睡眠呼吸暂停事件),则处理器144可以确定较大的响应是必需的。其结果是,处理器144可以选择将导致更大响应的调节控制信号参数。在一些实施例中,只有足够的功率可被传递到植入物单元110以引起期望水平的响应。换言之,处理器144可以配置成基于主天线150与副天线152之间的所确定的耦合程度来导致计量的响应。随着所确定的耦合程度减小,处理器144可能导致以增加的量传输功率。这样的方法可以保存电池寿命于外部单元120中,可以保护电路170和电路180,可以增加处理检测到的状态(例如睡眠呼吸暂停、打鼾、舌运动等)的类型的有效性,并且可以对于患者来说更加舒适。
在一些实施例中,处理器144可以使用迭代过程,以便选择产生期望的响应水平的调节控制信号参数。例如,当确定应当产生调节控制信号时,处理器144可以基于一组预定的参数值导致产生初始的调节控制信号。如果来自反馈电路148的反馈指示神经已被调节(例如,如果观察到增加耦合程度),则处理器144可以通过发出子调节控制信号而返回到监测模式。另一方面,如果反馈表明预期的神经调节因预期的调节控制信号未发生或神经的调节发生但仅部分地提供所期望的结果(例如,舌头的移动仅部分地远离气道),则处理器144可以改变与调节控制信号(例如幅度、脉冲持续时间等)相关的一个或多个参数值。
如果没有发生神经调节,则处理器144可以周期性地增加调节控制信号的一个或多个参数,直到反馈表明神经调节已发生。如果神经调节发生了,但没有产生所需的结果,则处理器144可重新评估主天线150与副天线152之间的耦合程度且选择新参数用于目标朝向实现期望结果的调节控制信号。例如,如果神经刺激引起舌头移动仅部分地远离患者的气道,则附加刺激可能是期望的。然而,因为舌头已移动远离气道,所以植入物单元110可以更接近外部单元120,且因此可以增加耦合程度。其结果是,将舌头移动剩余距离到所需的位置可能需要转移到比在舌头的最后刺激诱发的运动之前被提供的更少量的功率的植入物单元110。因此,基于新确定的耦合程度,处理器144可以选择新参数用于刺激控制信号,旨在将舌头移动剩余距离到所需的位置。
在一种操作模式中,处理器144可经以配置成扫过一定范围的参数值,直到神经调节被实现。例如,在所施加的反馈子调节控制信号导致反馈表明神经调节是适当的情况下,处理器144可以将最后施加的子调节控制信号用作起点,用于产生调节控制信号。与施加到主天线150的信号相关的幅度和/或脉冲持续时间(或其他参数)可以迭代地增加预定的量且以预定的速率,直到反馈表明神经调节已经发生。
处理器144可以配置成基于所确定的主天线150与副天线152之间的耦合程度来确定或导出各种生理数据。例如,在一些实施例中,耦合程度可以指示外部单元120与植入物单元110之间的距离,该处理器144可以用来确定外部单元120的位置或患者舌头的相对位置。监测耦合程度还可以提供这样的生理数据作为患者舌头是否移动或振动(例如患者是否打鼾),通过舌头移动或振动的如何、舌头的运动方向、舌头的运动速率等。
响应于这些确定的生理数据中的任一个,处理器144可以基于所确定的生理数据来调节功率传送至植入物单元110。例如,处理器144可以选择用于特定调节控制信号或一系列调节控制信号的参数来解决有关所确定的生理数据的特定条件。例如,如果生理数据表明舌头振动,则处理器144可以确定睡眠呼吸暂停事件很可能会发生,并且可以通过以选定成解决特定情况的量传送功率至植入物单元110来发出响应。如果舌处于阻塞患者气道(或部分地阻塞患者气道)的位置,但生理数据表明舌正移动远离气道,则处理器144可以选择不传送电源,并且等待以确定舌头是否在其自身上扫过。可替代地,处理器144可以传送少量功率至植入物单元110(例如,特别是运动的确定的速率表明舌头正缓慢移动远离患者气道),以鼓励舌头继续移动远离患者气道或加速其进展远离气道。
上述的方案仅是示例性的。处理器144可以配置成具有软件和/或逻辑,使其能够解决具有特殊性的各种不同的生理情形。在每一种情况下,处理器144可以配置成使用生理数据来确定要被传递到植入物单元110的功率的量,以便采用适量的能量来调节与舌相关的神经。
所公开的实施例可以与调节输送功率到植入物单元的方法结合使用。该方法可以包括确定与外部单元120相关的主天线150和与植入患者体中的植入物单元110相关的副天线152之间的耦合程度。确定耦合程度可以通过位于植入物单元110外部的处理器144实现,并且可以与外部单元120相关。处理器144可以配置成基于所确定的耦合程度来调节从外部单元输送功率至植入物单元。
如前面讨论,耦合程度确定使得处理器能够进一步确定植入物单元的位置。植入物单元的运动可以对应于植入物单元可以连接的身体部分的运动。这可以被认为由处理器接收的生理数据。因此,处理器可以配置成基于生理数据调节输送功率从电源至植入物单元。在可替代的实施例中,耦合程度确定使得处理器能够确定关于植入物单元状况的信息。这样的状况可以包括位置以及关于植入物单元内部状态的信息。根据植入物单元的状况,处理器可以配置成基于状况数据调节输送功率从电源至植入物单元。
在一些实施例中,植入物单元110可以包括位于植入物上的处理器。位于植入物单元110上的处理器可以执行关于与外部单元相关的至少一个处理器所描述的过程的全部或一些。例如,与植入物单元110相关的处理器可以配置成接收促使植入物控制器开启的控制信号,并且促使调节信号被施加到植入物电极,用于调节神经。这样的处理器还可以配置成监测与植入物单元相关的各种传感器并且将该信息传送回到外部单元。用于处理器单元的功率可以由板载电源提供或者经由传输从外部单元接收。
在其它实施例中,植入物单元110可以是自给自足的,包括其自身的电源和配置成操作植入物单元110的处理器,而没有外部的相互作用。例如,采用合适的电源,植入物单元110的处理器可以配置成监测受试者身体中的状况(经由一个或多个传感器或其它装置),确定这些状况何时确保调节神经,并且产生信号到电极来调节神经。基于运动或生物学功能,电源可以是再生的;或电源可以周期性地从外部位置充电,例如通过诱导。
图11示出了用于植入物单元110的示例性植入位置。图11描绘了在颏舌肌1060附近的植入位置,其可以通过受试者下巴的下侧上的真皮得以访问。图11描绘了舌下神经(即颅神经XII)。舌下神经1051通过其外侧分支1053和内侧分支1052,支配舌头的肌肉和其它舌肌肉,包括颏舌肌1060、舌骨舌肌1062、下颚舌肌(myelohyoid)(未示出)和颏舌骨1061肌肉。下颚舌肌(myelohyoid)的肌肉(未在图11中示出)形成口腔的地板,并且缠绕在颏舌肌1060的侧面。颏舌肌1060的水平隔室主要由内侧分支1052的内侧终端纤维1054支配,其在终端分叉1055背离外侧分支1053。内侧分支1052的远端部分然后进入(variegate)到内侧终端纤维1054中。颏舌肌1060的水平隔室的收缩可用于打开或保持受试者的气道。其它舌肌肉的收缩可协助其他功能,比如吞咽、关节运动、和打开或给药气道。因为舌下神经1051支配几个舌肌肉,所以对于OSA治疗来说可能有利的是限制调节舌下神经1051至内侧分支1052或甚至舌下神经1051的内侧终端纤维1054。以这种方式,该舌肌(最负责舌头运动及气道保养)可以有选择地针对收缩引起神经调节。可替代地,颏舌肌的水平隔室可以选择性地被作为目标。然而,内侧终端纤维1054可能很难与神经调节影响,因为它们位于颏舌骨肌1061的纤维内。本发明的实施例促进调节内侧终端纤维1054,如下面进一步讨论。
在一些实施例中,植入物单元110(包括至少一对调节电极例如电极158a、158b)和至少一个电路可以配置成用于植入通过受试者下巴的下侧上的真皮(即皮肤)。当植入通过受试者下巴的下侧上的真皮时,植入物单元110可位于靠近受试者舌下神经1051的内侧分支1052的内侧终端纤维1054。图11示出了示范性的植入物位置1070。
在一些实施例中,通过施加电场至终端分叉1055的远端的舌下神经1051的截面至舌下神经1051的外侧和内侧分支1053、1052,植入物单元110可以配置成使得电极158a、158b导致调节受试者舌下神经的至少一部分。在另外或可替代的实施例中,植入物单元110可以定位成使得从调节电极158a、158b延伸的电场可以调节舌下神经1051的内侧分支1052的内侧终端纤维1054中的一个或多个。因此,内侧分支1053或内侧终端纤维1054可被调节成以便促使收缩颏舌肌1060,这可足以或打开或保持患者的气道。当植入物单元110位于靠近内侧终端纤维1054时,电场可以配置成以便促使基本上不调节受试者舌下神经1051的外侧分支。这可能具有的优点是提供颏舌肌1060的选择性调节目标。
如上所述,可能难以调节舌下神经1051的内侧终端纤维1054,因为它们在颏舌肌1060内的位置。植入物单元110可以配置成用于在颏舌肌1060表面上的位置。植入物单元110的电极158a、158b可以配置成产生平行的电场1090,足以促使调节内侧终端分支1054,即使当电极158a、158b不与神经的纤维接触时。也就是说,植入物的阳极和阴极可以配置成使得当经由与植入物110和电极158a、158b相关的电路通电时,在电极158a、158b之间延伸的电场1090可以以一系列大致平行弧的形式,其延伸通过并进入植入物所位于的肌肉组织。一对平行线状电极或两个系列圆形电极可以适当地配置成用于产生适当平行的电场线。因此,当适当地植入时,植入物单元110的电极可以通过产生平行的电场线以非接触形式调节神经。
此外,可以通过适于产生部分运行或基本上平行于要被调节的神经纤维的平行电场线的电极配置来增加调节的功效。在一些实施例中,如果电场线1090和要被调节的神经纤维部分地或基本上平行的话,则由平行电场线感应的电流可能对神经纤维具有更大的调节效果。图11的插图示出的电极158a和158b产生的电场线1090(示为虚线)大致平行于内侧终端纤维1054。
为了便于调节内侧终端纤维1054,植入物单元110可被设计或配置成确保电极在被植入时的适当位置。图12示出了示例性的植入。
例如,植入物的柔性载体161可以配置成使得植入物的柔性载体161的至少一部分位于颏舌肌1060与颏舌骨肌1061之间的位置。柔性载体161可以进一步配置成允许布置在柔性载体161上的至少一对电极位于颏舌肌1060与下颚舌肌(myelohyoid)的肌肉之间。细长臂161的延伸部162a和162b中的一个或两个可以配置成适应颏舌肌的轮廓。细长臂161的延伸部162a和162b中的一个或两个可以配置成沿着颏舌肌1060的轮廓延伸远离受试者下巴的下侧。延伸臂162a、162b中的一个或两个可以配置成在天线152位于颏舌肌1060与颏舌骨肌1061之间时环绕着颏舌肌。在这样的配置中,天线152可以位于与由受试者下巴的下侧限定的平面基本上平行的平面上,如图12所示。
柔性载体161可以配置成使得至少一对间隔开的电极可以位于受试者颏舌肌与相邻肌肉之间的空间中。柔性载体161可以配置成使得至少一对调节电极158a、158b配置成用于植入相邻于颏舌肌1060的水平隔室1065。颏舌肌1060的水平隔室1065示于图12,并且是肌肉的一部分,其中肌肉纤维大致水平运行,而不是垂直的、倾斜的、或横向的方向。在该位置,舌下神经纤维运行于颏舌肌纤维之间并且与它们平行。在这样的位置,植入物单元110可以配置成使得调节电极产生的电场基本上平行于肌肉纤维且因此水平隔室中的舌下神经的内侧终端纤维1054的方向。
本公开的一些实施例可以包括用于植入物单元110的植入的方法、装置以及工具。图13-15示出了用于植入物单元110及其各种特征的植入的递送工具的示例性实施例。图13-15示出的特征可以以任何合适的方式组合在与本公开一致的各种实施例中。递送工具1301可以在植入程序期间用于正确地定位植入物单元110,以测试植入物单元110,和/或协助外科医生将植入物单元110固定到适当的内部身体结构。
图13a-13b示出了递送工具1301的各个方面。在一些实施例中,递送工具1301可以包括主体1302、适于保持植入物单元的保持器1303、植入物激活器1304、以及与植入物激活器相关的电源1308,比如电池。主体1302可以包括手柄,其形成为以便接收用户的拇指和至少一个食指。保持器1303可以包括适于保持和释放植入物单元的任何结构。在一些实施例中,保持器1303可以适于保持植入物单元110。保持器1303可以包括各种部件来可释放地保持植入物单元110,比如真空元件(未示出)或闭锁元件(未示出)等。
保持器1303还可以包括一对夹爪1305,其设置成相对的关系,如图13c所示。相对夹爪1305的间距是可调节的,例如通过递送工具1301的镊子式运动。递送工具1301的镊子式运动可以由棘轮1406控制,如图14a所示,其配置成保持夹爪1305在被采用时它们之间的间距。因此,用户可以将植入物单元110定位在夹爪1305内,将夹爪1305按压在一起,直到植入物单元110被牢固地保持在弯曲位置,并且采用棘轮1406来保持夹爪1305间隔开。用户比如外科医生可以随后释放递送工具1301上的压力,同时保持植入物单元110的位置。保持植入物单元的位置在植入程序之前是有用的,因为外科医生准备进行植入。在植入程序期间也是有用的,因为外科医生可以随着植入步骤被执行将植入物单元110相对于植入位置保持就位。
例如,当植入物110是在患者内的期望位置被时,可以通过使夹爪1305打开来释放棘轮1408。植入物110然后可以从递送工具1301释放,且留在患者内于植入位置。如图14a所示,棘轮1408可以包括各种适合的配置。
在一些实施例中,递送工具1301的主体1302的腿可以具有弯曲(如图14c所示),其在保持器1303的近端。这样的弯曲(其可以允许工具的主体在手术过程中更平行于患者身体)可能会在主体1302的上部和下部之间产生角度α。角度α可能在90和150度之间。角度α可小于150度、小于135度、小于120度、小于105度、且约等于90度。植入物工具中的弯曲可能对于外科医生来说更容易在手术期间看到植入区域。
在一些实施例中,植入物工具可以设置有旋转部分,允许植入物工具主体1302的一部分相对于另一部分旋转。例如,旋转部分可被设置成使得植入物工具中的弯曲是可变的。可以设置一个或多个旋转部分。
植入物单元110的弯曲位置可以选择成使得植入物单元110将在被植入时完全符合组织轮廓。例如,保持器1303可以将植入物单元110保持处于弯曲压缩或拉伸的配置。如图13c所示,保持器1303可以保持植入物单元110,使得植入物保持器110的第一延伸部162a保持大致紧贴着第二夹爪1307,且植入物保持器110的第二延伸部162b保持大致紧贴着第一夹爪1306。
保持器1303还可以包括配置成接收外科缝合线的至少一个缝合线引导构件1501。如图15所示,缝合线引导构件1501可以包括适于插入手术针的第一缝合线引导部1502和第二缝合线引导部1503。第一缝合线引导部1502可以设置在保持器1303的第一侧上,第二缝合线引导部1503可以设置在保持器1303的第二侧上,其中第二侧与第一侧是相对的。因此,第一和第二缝合线引导部1502、1503可以与保持器1303相关。
第一缝合线引导部1502可以包括夹爪1305中的一个或多个孔,而第二缝合线引导部1503可以包括弯曲的通道。在一个实施例中,第一缝合线引导部1502可以包括第一夹爪1308中的孔。第二缝合线引导部1503的通道可以包括从第二夹爪1307延伸的弧形形状。通道可以具有的曲率半径对应于手术针。例如,缝合线引导构件1501可以成形为以便接收任何类型的手术缝合针,比如四分之一圆、3/8圆、5/8圆、1/2圆或1/2弯曲。缝合线引导构件1501可以配置成对应于缝合孔160和/或植入物单元110的手术网1050,且可能因此促进植入程序。
在植入程序期间,植入物单元110可被定位成符合受试者的组织结构,并且外科医生可以使用缝合引导构件1501来适当地定位并引导缝合针,以便将植入物单元110缝合在适当位置。缝合线引导构件1501可以配置成在缝合引导构件1501与患者的组织之间设置预定的角度,且因此可以通过允许以预定的角度插入缝合针来帮助外科医生。例如,缝合线引导构件1501可以在缝合线引导构件1501与患者的组织之间设置约90度的角度。在其他实施例中,缝合线引导构件1501可以设置约60度、约45度或约30度的角度。
在一些实施例中,递送工具1301可以包括自动缝合单元,以允许将植入物单元110机械化缝合到位。自动缝合单元可以配置成在植入物激活器1304的命令下推进缝合线通过植入物单元110中的孔,下面将进行更详细地描述。
下面返回到图13a-c,如上所述,递送工具1301可以包括植入物激活器1304。植入物激活器1304可以在植入程序期间被如下采用。植入物激活器1304可以包括主天线1310和与主天线1310和电源电通信的至少一个处理器。植入物激活器还可以从神经调节响应检测器1350接收信息。例如,神经调节响应检测器1350可以包括至少一对肌电图(EMG)电极,其配置成检测可由植入物激活器1304用于确定神经调节的肌肉活动。神经调节响应检测器1350可以设置在递送工具1301的一部分上,以便在植入程序期间与受试者接触。例如,神经调节响应检测器1350可以设置在保持器1303上,如图13c所示。在保持器1303包括夹爪1305的实施例中,神经调节响应检测器1350可以包括EMG电极,其定位在夹爪1305上,以便在植入程序期间接触受试者的组织。在替代实施例中,递送工具1301可以包括延伸臂1365,其配置成将神经调节响应检测器定位在以便接触受试者组织的位置,例如如图13b所示。
植入物激活器1304可以配置成与植入物单元110无线地交互,例如通过由主天线1310发送的信号。主天线1310可以包括本文中关于主天线152所描述的任何天线结构,并且还可以包括导线缠绕的线圈、蚀刻的线圈、非层叠的导线缠绕的线圈以及绞合线线圈。
植入物激活器1304可滑动地设置在递送工具1301上,使得植入物激活器1304可以在形成于第一和第二面1315、1316之间的轨道1314内滑动。因此,植入物激活器1304可滑动朝向和远离植入物110,设置在夹爪1305之间,于轨道1314内。例如,植入物激活器1304可以沿着主体1302从第一位置(图13a)至第二位置(图13b)滑动预定的距离。可以进一步设想的是,一个或多个固定装置1317可以在沿着主体1302向上向下滑动时固定和保持植入物激活器1340于轨道1314内。另外或可替代地,在植入物激活器1304上的突起1312可以在由轨道1314形成的凹槽内滑动。固定装置1317和/或突起1312可以选择性地将植入物激活器1304锁定在沿着主体1302的预定位置。
可滑动接合部1321可以激活植入物激活器1304,促使植入物激活器1304从第一位置滑向第二位置或者从第二位置滑向第一位置。例如,可滑动接合部1321可以包括配置成由用户接合的触发器或按钮。在一个实施例中,用户可以按压可滑动接合部1321来激活植入物激活器1304。可替代地,植入物激活器1304可以通过由枢轴连接(未示出)促进的枢转运动从第一位置移动到第二位置,且从第二位置到第一位置。还可以设想的是,用户可以将植入物激活器1304滑入所需的位置,以沿着轨道1314移动植入物激活器1304。如图14a-c所示,植入物激活器1304可以包括配置成符合用户手指或手掌的各种形状和凹陷。这样使得能够很容易地滑动植入物激活器1304。在不脱离所公开的实施例的情况下,还可以采用移动植入物激活器1304的其他装置。
处理器1340可以配置成确定和调节激活器天线310与副天线152之间的耦合程度。如图13a所示,处理器1340可以位于递送工具1301的主体1302的表面内或其上。在可替代的实施例中,处理器1340可以配置成用于从递送工具1301外部的位置有线或无线地通信。例如,处理器1340可以包括一个或多个集成电路、微芯片、微控制器和微处理器,其可以是中央处理单元(CPU)、数字信号处理器(DSP)、现场可编程门阵列(FPGA)、或可适用于执行指令或执行逻辑运算的本领域技术人员已知的任何其它电路的全部或部分。
电源1308可拆卸地或永久地联接到递送工具1301的主体1302。例如,如图13a所示,电源可以设置在植入物激活器1304的外或内表面上。电源1308可以与处理器1340和激活器植入物1310电且无线通信。例如,通过将功率传送到这些部件(包括激活器天线1310和处理器1340),电源1308可以配置成激活它们。电源308可以包括能够激活激活器天线1310或处理器1340的任何电源,例如电池。
激活器天线1310可以与处理器1340和电源1308通信,并且配置成与植入物110上的副天线152相互作用并且将其激活。因此,激活器天线1310可以配置成在植入程序期间将功率从电源1308选择性地传输到植入物单元110,以在最终固定植入物单元110之前促使调节受试者身体中的至少一个神经。激活器天线1310可以包括配置成产生电磁场的任何导电结构。激活器天线1310可以是任何合适的大小、形状和/或配置。该大小、形状、和/或配置可以由患者的大小、植入物单元110的放置位置、植入物单元110的大小和/或形状、激活植入物单元110所需的能量的量、植入物单元110上存在的接收电子器件的类型等来确定。
激活器天线1310可以包括本领域技术人员公知的可配置成发送和/或接收信号的任何合适的天线。合适的天线可以包括但不限于长导线天线、贴片天线、螺旋形天线等。例如,在一个实施例中,激活器天线1310可以包括线圈天线。这样的线圈天线可以由任何合适的导电材料制成,并且可以配置成包括导电线圈的任何适当的布置(例如直径、线圈的数量、线圈的布局等)。适合用作激活器天线1310的线圈天线可以具有的直径为约0.1厘米至10厘米,并且除其他合适的形状之外可以是圆形或椭圆形的。在一些实施例中,激活器天线1310可以具有的直径在0.5厘米与2厘米之间,并且可以是椭圆形的。适合用作激活器天线1310的线圈天线可以具有任何数量的绕组,例如4、8、12或更多。适合用作激活器天线1310的线圈天线可以具有的导线直径在约0.001毫米和2毫米之间。这些天线参数仅是示例性的,并且可被调整成高于或低于被给定的以实现合适结果的范围。进一步设想的是,激活器天线1310可以包括与副天线152相同的天线。
在示例性植入程序器件,外科医生可以使用递送工具1301将植入物单元110定位在希望的植入位置,并且可以接合夹爪1305来弯曲植入物单元110,使得其基本上符合在希望的植入位置的组织。希望的植入位置可以是适于神经调节的位置。外科医生然后可以将植入物激活器1304从图13a所示的第一位置移动到图13b所示的第二位置。递送工具主体1301的可滑动接合部1320可用于将植入物激活器1304从第一位置滑动到第二位置。可替代地,可以通过由枢轴连接(未示出)促进的枢转运动将植入物激活器1304从第一位置移动到第二位置。在不脱离所公开的实施例的情况下,还可以采用将植入物激活器从第一位置移动到第二位置的其他装置。将植入物激活器1304从第一位置移动到第二位置具有的作用是使主天线1310邻近于植入物单元110的副天线152。当在第二位置通电时,主天线1310可以将功率传送到植入物单元110,例如通过射频传输。
可以由致动器1320使得能够在主天线1310与副天线152之间进行功率传输,在图13b中示出为在植入物工具1301的手柄上的按钮。然而,致动器1320可以指激活被发送到植入物激活器的功率的任何装置。该装置可以包括具有开/关位置的开关,其可定位在递送工具的主体上,使得外科医生可以接合开关置于“开”的位置,以将功率传送至植入物激活器。致动器可以用作安全屏障,以防止任何意外激活植入物激活器。致动器还可以位于递送工具外侧,并且例如可以包括要与植入物激活器连接的电缆。在一些实施例中,植入物激活器1304的电源可以包括集成到植入物激活器1304中的电池。
在将植入物激活器从第一位置移动到第二位置之后,外科医生使得能够在主天线1310与植入物单元110的副天线152之间进行功率传输,以测试植入物单元110的功能。植入物激活器1304可以包括的处理器和电路具有前面关于外部单元120的处理器144和电路170所描述的功能的任何或全部。在一些实施例中,植入物激活器1304可以包括的处理器和电路类似于或等同于外部单元120的处理器和电路,其中部件移动仅出于对空间的考虑。在替代实施例中,植入物激活器1304包括配置成特别用于测试植入物单元110的处理器。
一旦由植入物激活器1304激活,则可以经由至植入物激活器1304的反馈并且通过观察患者的反应来对植入物单元110进行测试。也就是说,外科医生可以基于被施加到植入物单元110的激活调节电极158a、158b的信号是否有效地调节患者神经来确定希望的植入位置是合适的。例如,当放置植入物单元110用于治疗睡眠呼吸障碍时,外科医生可以通过在功率传送期间观察患者的舌头运动来确定希望的植入位置。在其它示例中,外科医生可以使用内窥镜来观察气道扩张。在一些实施例中,神经调节响应检测器1350可被接合,以检测神经调节响应,并且允许植入物激活器1304确定源于功率施加到植入物单元110的神经调节响应的程度。在神经调节响应检测器包括EMG电极的实施例中,EMG电极可被接合,以测量因由植入物单元110所造成的调节而发生的神经肌肉活动。
植入物激活器1304还可以配置成允许在确定神经调节的阈值程度时从植入物递送工具1301释放植入物单元110。因此,植入物激活器1304可以配置成以便不允许释放植入物单元110,直至实现有效的神经调节,如由阈值程度的确定所指示。在一些实施例中,植入物激活器1304可以配置有灯或音频设备,以提醒外科医生已经达到了神经调节的阈值程度。在确定希望的植入位置构成合适的调节位置之后,外科医生然后可以采用缝合线将植入物单元110固定就位。
植入物激活器1304还可以用来在开始植入程序之前验证植入单元110的功能。外科医生可以制备如上所述的具有植入物单元110的递送工具1301,并且将植入物激活器1304从第一位置移动到第二位置,而未将植入物单元110定位在身体中。在该方法中,外科医生然后可以激活植入物单元110,以验证植入物单元110没有受到制造缺陷影响。例如,植入物激活器1304可以使用本文所描述的耦合检测技术,以便验证植入物单元110的功能。例如,可以经由音频输出、视觉输出、和/或触觉输出来输出这样验证的结果。
对本领域技术人员来说,通过考虑到本发明的说明书及实践,本公开的其他实施方式将是显而易见的。
虽然本公开提供了用于治疗某些病症的神经调节递送装置的示例,但是所公开的神经调节递送装置的使用并不局限于所公开的示例。用于神经调节的本发明的实施例的使用的公开内容将被认为仅是示例性的。在其最广泛的意义上来说,本发明可以用于通过神经调节与任何生理状况的治疗结合。对本发明所属领域的技术人员来说,替代实施例将是显而易见的,而不背离其精神和范围。因此,本发明的范围由所附权利要求而不是由前面的描述限定。
Claims (5)
1.一种用于在植入程序期间测试睡眠呼吸暂停植入物的装置,包括:
植入物激活器,配置成在植入程序期间将功率从电源选择性地传输到睡眠呼吸暂停植入物,以在睡眠呼吸暂停植入物最终固定之前促使至少一个舌下神经至组织的调节;其中,所述植入物激活器包括电源,传输到睡眠呼吸暂停植入物的功率由电源提供,所述电源配置成从所述电源无线地传输功率至所述睡眠呼吸暂停植入物,并且所述植入物激活器可选择性地朝向和远离睡眠呼吸暂停植入物移动;以及
可滑动接合部,配置成激活植入物激活器,以促使植入物激活器沿着装置的主体从在轨道的第一端处的第一位置到在轨道的第二端处的第二位置移动预定的距离。
2.根据权利要求1所述的装置,其中所述电源包括电池。
3.根据权利要求1所述的装置,其中,所述植入物激活器包括处理器,该处理器配置成调节功率传递给睡眠呼吸暂停植入物。
4.根据权利要求1所述的装置其中,所述植入物激活器配置成通过射频传输将功率传输至所述睡眠呼吸暂停植入物。
5.根据权利要求1所述的装置,其中,所述可滑动接合部包括触发器,该触发器配置成被使用者接合以激活所述植入物激活器。
