Description

Pain bone-knitting plaster and decocting method thereof
Technical Field
The invention belongs to the technical field of traditional Chinese medicines, and particularly relates to a pain bone-knitting plaster and a decocting method thereof.
Background
The bone disease is "osteopathy" from the professional perspective, which is one of three major pathologies of orthopedic diseases, and the other two are "orthopedics" and "tendon injury" respectively. Bone diseases are also the most common diseases in orthopedic diseases, and are mainly divided into: congenital bone disease, metabolic bone disease, osteonecrosis, occupational bone disease, endemic bone disease, joint degenerative bone disease, bone tumor, bone carbuncle and cellulitis, bone tuberculosis, osteoarticular arthralgia, fistula syndrome, tendon spasm, etc.
Bone diseases generally include: rheumatic arthritis, rheumatoid arthritis, degenerative arthritis, ankylosing spondylitis, cervical spondylosis, lumbar spondylosis, scapulohumeral periarthritis, hyperosteogeny, femoral head necrosis, puerperal rheumatism, etc.
Rheumatoid arthritis is a common acute or chronic inflammation of the connective tissue. Can repeatedly attack and involve the heart; clinically, it is characterized by migratory weakness, severe symptoms and pain of joints and muscles, and belongs to allergic diseases, and it is one of the main manifestations of rheumatic fever, and is mostly caused by acute fever and joint pain.
Rheumatoid arthritis is a chronic systemic autoimmune disease characterized by arthromeningitis; synovitis persists with repeated attacks, which can lead to cartilage and bone destruction in joints, joint dysfunction, and even disability. Vasculitis affects all organs of the body, so it is also called rheumatoid disease.
Degenerative arthritis, also known as hypertrophic arthritis, refers to the common symptoms of lumbago, leg pain, and arthralgia in some elderly people. It is also called senile arthritis because it is often found in the elderly. Senile degeneration is the main cause of degenerative arthritis. After the middle-aged and the elderly, all tissues and organs can generate degenerative changes; bone and joint tissues are no exception, degenerative changes occur particularly well in weight bearing joints and multi-mobile joints. Excessive weight bearing or excessive use of certain joints may promote degenerative changes. In addition, factors such as intra-articular fractures, diabetes, prolonged inappropriate use of adrenocortical hormones, etc., can promote the formation of degenerative changes and accelerate the development of existing degenerative changes.
Ankylosing spondylitis belongs to the category of rheumatism, and is one of seronegative spondyloarthropathies. The cause of the disease is not clear, and the chronic disease with the spine as the main pathological change affects the sacroiliac joints, causes the spinal rigidity and fibrosis, causes the pathological changes of eyes, lungs, muscles and bones with different degrees, and belongs to the autoimmune disease.
Cervical spondylosis is also known as cervical syndrome, is a general name of cervical osteoarthritis, proliferative cervical spondylitis, cervical radicular syndrome and cervical disc herniation, and is a disease based on degenerative pathological changes. Mainly, due to long-term strain of cervical vertebrae, hyperosteogeny, or prolapse of intervertebral disc and ligament thickening, the spinal cord, nerve root or vertebral artery of cervical vertebrae are pressed, resulting in a series of clinical syndromes of dysfunction. It is manifested as degeneration of cervical intervertebral disc and a series of secondary pathological changes, such as instability and loosening of vertebral segment; herniation or herniation of the nucleus pulposus; forming bone spurs; ligament hypertrophy, secondary spinal stenosis, and the like irritate or compress adjacent tissues such as nerve roots, spinal cord, vertebral artery, and cervical sympathetic nerves, and cause syndromes of various symptoms and signs.
The lumbar spondylosis in medicine comprises the diseases of lumbar disc herniation, lumbar hyperosteogeny, lumbar muscle strain, lumbar sprain, lumbar degenerative disease, rheumatism or rheumatoid lumbago, lumbar tuberculosis, lumbago due to wind-cold-dampness, lumbago due to blood stasis, lumbago due to damp-heat, lumbago due to kidney deficiency, cervical spondylosis and the like.
Scapulohumeral periarthritis is a common disease with pain in shoulder joints and difficulty in movement as main symptoms. The age of good onset of this disease is about 50 years old, while the incidence of female disease is slightly higher than that of male disease, which is often seen in physical workers. If no effective treatment is available, the functional activity of the shoulder joint may be seriously affected. According to clinical observation, hospitals often use the medicine combination tangzhong for treatment, the shoulder joint of the disease is paroxysmal pain in early stage, is usually induced by weather change and fatigue, gradually develops persistent pain later and gradually aggravates, is light in day and heavy in night, and has limited active and passive activities in all directions. When the shoulder is pulled, severe pain may be caused. The shoulder joints may have extensive tenderness and radiate towards the neck and elbows, and there may also be different degrees of atrophy of the deltoid muscles.
The hyperosteogeny is also called as proliferative osteoarthritis, degenerative joint disease, senile arthritis and hypertrophic arthritis, and is a disease with the symptoms of secondary hyperosteogeny, joint deformation, joint pain, limited movement and the like when abnormal load is applied. Hyperosteogeny is divided into primary and secondary.
Femoral head necrosis, also known as avascular necrosis of the femoral head, is one of the common osteoarthropathies. Most of them are caused by rheumatism, hematopathy, caisson's disease, burn, etc., which first destroy the blood supply of the adjacent articular tissues and then cause necrosis. The main symptoms of the traditional Chinese medicine gradually progress from intermittent pain to persistent pain, muscle spasm is caused by the pain, joint movement is limited, and severe disabling and lameness are caused finally. Hormonal drugs can also cause the disease. The Chinese medicine considers that the causes of the disease are external and internal, and the interaction of the external and internal causes can unbalance yin and yang of the human body and cause diseases due to the imbalance of qi and blood, which are also called as "bi-arthralgia of legs", "bi-arthralgia of bone" and "bone atrophy".
At present, there are many reports on patent documents for treating orthopedic diseases. For example: 1. the invention has application number of 201510026701.7 and is named as 'plaster for activating meridians and removing blood stasis and promoting bone growth and a preparation method thereof'. 2. General diagnostic criteria for lumbar disc herniation:
1) leg pain is more severe than lumbago, typical radicular sciatica;
2) paresthesia of the lower extremities, allodynia of single nerve roots in the legs or feet (lumbar 5, sacral 1 or lumbar 4 spinal nerve root distribution areas);
3) lower lumbar spinal nerve root involvement signs: the straight leg elevation test is less than 50 degrees; the straight leg elevation strengthening test is positive; positive in the Jian Gao test. One of the three signs must be positive.
4) Two of four types of muscular atrophy, muscle weakness, paresthesia and reflex change in the neurological physical examination were positive.
5) The examination of spinal cord radiography, lumbar intervertebral disc CT flat scan or lumbar nuclear magnetic resonance is positive result and is consistent with the clinical symptoms and physical signs of affected nerve roots.
The diagnosis of the lumbar intervertebral disc protrusion can be made only if the five standards are positive.
Diagnostic criteria for ankylosing spondylitis (new york criteria revised by the american college of rheumatology 1984):
(1) clinical criteria are as follows:
lower back pain persists for at least 3 months, with relief after activity (rather than rest).
Secondly, the movement of the lumbar vertebra in the vertical and horizontal planes is limited.
And the chest expansion degree is reduced compared with that of normal people of the same age and sex.
(2) The diagnosis standard is as follows:
has unilateral III-IV grade or II-III grade X-ray sacroiliac arthritis, and at least 1 of 3 clinical standard strips.
The activity should be diagnosed when diagnosing ankylosing spondylitis. Currently, the common indicators for clinically judging ankylosing spondylitis activities are: morning stiffness is more than or equal to 30 minutes; sleep is affected by pain and stiffness; peripheral arthritis; ESR is more than or equal to 30mm/1h (Weishi method); CRP is more than or equal to 20 mg/L; 1gA is more than or equal to 3.9 g/L; spinal pain; chest pain during normal breathing or pain or stiffness during neck movement; pain in the buttocks during the day or at night.
Lumbar spondylolisthesis: usually, a certain vertebral body is displaced forwards or backwards, which is commonly seen in lumbar vertebra, and lumbar spondylolisthesis in China is one of the common diseases in orthopedics. Currently, lumbar spondylolisthesis is divided into 6 categories of dysplasia (including high-grade and low-grade dysplasia), isthmus, degeneration, trauma and pathology. In addition, isthmus fissuring and degeneration are common. The incidence rate of lumbar spondylolisthesis varies according to race and region, is 4-6% in Europe and approximately 4.7-5% of the population in China; the spondylolisthesis caused by the isthmus fissure accounts for about 15%, and the degenerative lumbar spondylolisthesis accounts for about 35%. The age of onset of lumbar spondylolisthesis in China is more than 20-50 years old, accounting for 85%; more men than women develop the disease, and the ratio of men to women is 29: 1. the common parts of lumbar spondylolisthesis are L4-L5 and L5-S1, wherein the incidence rate of the lumbar 5 vertebral body is 82-90 percent
Judging the spondylolisthesis and hyperplasia of lumbar vertebrae:
type I: dysplasia; is a congenital defect of the superior articular process of the sacrum 1 or the lumbar vertebra 5, and can not prevent the anterior spondylolisthesis of the lumbar vertebra 5.
Type II: isthmus type; it is a stress fracture or extension of the isthmus (between the upper and lower articular processes) that causes the upper articular process to move forward along with the upper lumbar vertebrae.
Type III: deforming; the slippage occurs due to the instability of articular processes and intervertebral discs as a result of long-term degeneration, but to a lesser extent.
And IV, type: the type of trauma; it is a fracture of vertebral pedicle, vertebral plate and articular process caused by acute trauma, and this type is rare.
Type V: the pathological type; it is a systemic disease such as osteogenesis imperfecta, osteitis deformans, etc.
Hyperplasia of neck, shoulder, waist and knee: it also belongs to hyperosteogeny, and the hyperosteogeny is a disease with the main symptoms of local joint, muscle and ligament movement disorder caused by pathological hyperosteogeny and accompanying pain. It can occur in cervical vertebra, lumbar vertebra, joints and calcaneus. The disease is usually seen in middle-aged and old people over 40 years old and in the working of weight bearing, long standing and sitting, and the disease of joints and vertebras with large range of motion is common. The causes of hyperosteogeny are unknown so far, and are mostly in the category of senile degenerative diseases. The basic understanding is that the pathogenesis is caused by the induction of exogenous factors on the basis of endogenous factors. At present, the western medicine adopts non-hormone anti-inflammatory and analgesic drugs, physical therapy and surgical treatment, and has certain effect on relieving symptoms, but no special effective therapy exists so far.
The treatment method comprises the following steps:
the plaster is pasted on the affected part for treating patients with lumbar disc herniation, hyperplasia of neck, shoulder, waist and knee, ankylosing spondylitis and lumbar spondylolisthesis hyperplasia; the plaster adopts the following medicine formula: 15-25 g of saffron, 25-35 g of dragon's blood, 20-30 g of pangolin scales, 40-60 g of pseudo-ginseng, 30-45 g of monkshood, 30-45 g of kusnezoff monkshood root, 35-50 g of elderberry, 40-55 g of clematis root, 40-50 g of cortex acanthopanacis, 40-50 g of rhizoma drynariae, 30-45 g of homalomena rhizoma, 40-50 g of cassia twig, 30-45 g of native copper, 40-55 g of lesser spider, 40-55 g of arisaema tuber and 45-65 g of artemisia anomala.
